Abstract

The prosthesis has a cup (100) with peripheral groove (101) adjacent to the opening (102) of the cup. An expansible strap (103) has a section corresponding to that of the groove. The strap is seated and held in the groove before positioning of the prosthesis to be pinched in the contour of the cup. There are locking screw openings (107-110) to hold the strap compressed in the groove. The strap has an exterior surface with extending hooking formations (104). The locking screws can be released after positioning of the cup to allow the strap to expand into the cup seating.

Description

The present invention relates to a cotyloid prosthesis comprising - a cup - an insert receiving the prosthetic femoral head of the pro
femoral thesis.

There are different types of acetabular prostheses allowing different types of fixation in the acetabular bone cavity.

The standard technique is to fix the prosthesis by sealing the acetabulum in the bone using cement. Sealing with cement of a high density polyethylene (HDPE) cup remains classic and usual, but the rigidity of the fixation frequently leads to loosening after 10 years due to a lack of adaptation over time to changes in the bone periphery. For screwing, an acetabular cavity is produced with a bone thread into which the prosthesis is screwed. There are also prostheses with self-tapping threads.

Another solution is to fix the prosthesis using screws passing through the wall and screwing into the bone.

s niere.
This screw fixing can complete the impaction fixing; the prosthesis can also externally have pins functioning as nails and which sink into the bone when the prosthesis is put in place, thus avoiding the possibility of rotation of the latter
s niere.

In general, the installation of a prosthesis by impaction requires a precise preparation of the acetabular cavity preserving the bone capital that the preparation of the bone is limited to the exact volume of the cup, without risk of microfractures at the time of l impaction. This impaction technique is currently used preferentially because the bone preparation is very limited; in other words, this only weakens the bone of the cavity receiving the prosthesis to a very limited extent.

In general and as already indicated above, to stabilize the prosthesis which is generally a hemisphere, in its housing during and after placement, it is necessary to ensure minimum primary stability. This is done in part using the pins or pins provided on the external surface of the prosthesis but above all by an impact effect of a workpiece of diameter greater than the bone milling. This part is then either in thick metal (- 5 mm) and then does not take into account the bone flexibility, or in thinner metal (3 mm) to ensure a certain deformability but this is likely to cause either wear of the insert, a metallic fragility.

This stability is supplemented by a so-called secondary stability ensured subsequently by the adhesion between the metal and the bone. This adhesion is facilitated either by a surface treatment of the metal, generally a deposition of apatite hydroxide ensuring an active bonding role and promoting the bonding of the bone, or by the particular nature of the surface obtained by a mechanical treatment. such as spraying metal, or attaching a wire mesh to the frame. These different possibilities can be combined.

To allow fixing with screws, the prosthesis has holes. These holes are provided in a larger number than the number of screws actually used, to allow adaptation to each particular case. The holes not occupied by screws are free. The insert therefore appears through the hole. However, the bone develops in the more or less long term through the hole, in contact with the insert which is generally made of polyethylene; but such contact is to be avoided.

The present invention proposes to create an acetabular prosthesis, facilitating its installation, ensuring its primary fixation in the bone cavity and guaranteeing good stability of the prosthesis by providing a certain elasticity necessary to avoid the loosening of the prosthesis over time. guaranteeing its longevity thanks to its non-deformability in the area of contact with the polyethylene insert.

To this end, the invention relates to a prosthesis of the type defined above, characterized in that the cup comprises - a peripheral groove in the vicinity of the opening of the cu
pule, - an expandable belt with a section corresponding to that of
the throat,
- housed and blocked in the groove before fitting the
prosthesis, retracted in the contour of the cu
pule,
- provided with locking means to be maintained esca
moted in the throat,
- the belt having means on its outer surface
projecting attachment, - the locking means which can be unlocked after
fitting of the prosthesis to allow the belt to
expand and cling to the bone wall receiving the
prosthesis.

The prosthesis according to the invention offers the advantage of being simple to manufacture. It does not have an opening constituting passages between the external surface of the prosthesis which would allow contact of the bone and the surface of the insert. This prosthesis having no protruding part, is easily placed in the bone cavity.

This positioning can be corrected according to particular cases before the prosthesis is locked in place by the release, by unlocking, of the belt. As this belt is retracted at the time of positioning and it remains inside the hemispherical envelope of the outer surface of the cup, it does not in any way interfere with the positioning and impaction. This modification of the orientation of the cup is easy before final fixing by unlocking the belt.

The external shape of the belt with its attachment means promotes the bone / metal connection and constitutes a connection of a certain elasticity between the cavity and the cup.

Very importantly, the peripheral adaptation of the cup is permanent, without deformation of its central part over time. The expansive belt fitted to the cup allows immediate adaptation over time: on the one hand to the imperfections of the "machining" of the acetabulum, on the other hand, to the evolution over time of the geometry of this same cavity and moreover, to the non-deformability of the metal cup receiving the insert and therefore to the non-interaction of the metal-insert surfaces.

In a particularly advantageous manner, the belt is in the form of an open ring, the two ends of which are held at the bottom of the groove in the cup by locking means constituted by screws accessible from the inside of the cup.

The screws holding the ends of the belt are easily unscrewed from the inside of the cup after it has been placed in the bone cavity. It is only after this positioning and blocking of the cup that the insert is housed there. The primary stability which is accompanied by a permanent elasticity of the cup by the expansive belt allows an adaptation with the bony acetabulum at the level of the lower horns, that is to say of the bony zones which change the most over time , with the pressure exerted in the hip joint, the age of the individual and the bone metabolism.

Advantageously, the belt is secured to a circular area whose extent allows the control of the expansion force at the level of the acetabular horns.

As the belt is fixed to the cup, there is no sliding movement possible between the belt and the cup. Only the elastic expansion movement of the belt is possible.

When the belt is constituted by a split ring it is particularly advantageous that the point of attachment of the belt at the bottom of the groove of the cup is located in a position diametrically opposite to that of the two free ends of the belt.

Advantageously, the screws retaining the belt have break zones between the threaded part and the head, which makes it possible to force the head and detach it from the screw to release the belt; the threaded part of the screw remains in the thread of the belt and closes it; the screw head is imperatively recovered.

According to a variant, the threaded part of the screw is locked in the belt, for example by pressure, a welding spot or the like.

According to another advantageous characteristic of the invention, the means for hooking the belt are preferably constituted by peripheral strips forming hooking edges. These hooking edges disappear almost completely in the outer contour of the cup when the belt is in the locked position.

According to the invention, the cup and the belt are preferably made of titanium and the insert of high density polyethylene (HDPE). The belt is preferably made of the same material as the cup.

To facilitate the installation and the holding of the insert in the cup, especially in the case of an insert which does not have in all its parts a rotational symmetry and comprises for example an anti-dislocation reinforcement intended to retain the prosthetic femoral head in the insert, it is advantageous for the insert to be locked in rotation with respect to the cup, itself locked in rotation with respect to the bone cavity while leaving the possibility, during placement, to choose the best orientation of the insert relative to the cup.

For this, and according to another advantageous characteristic of the invention, the bottom of the cup is provided with an axial orifice provided with indexing means and the insert ends in a stud corresponding to the orifice of the cup and comprising indexing means complementary to those of the cup.

The insert / cup indexing is only useful when the insert is not a solid of revolution, that is to say an insert having an anti-dislocation device.

The means of indexing the orifice of the cup and the corresponding or complementary means of the stud of the insert are preferably constituted by longitudinal grooves. The peripheral dimensions of the longitudinal grooves define the indexing pitch of the insert relative to the cup.

Finally, to prevent the material of the cup from being able to be in contact with the bone material, the nipple of the insert has on its external face, that is to say that appearing through the orifice of the cup, a pad, for example in the shape of a nail, in the same material as that of the cup. Thus, after the insertion of the insert in the cup, the cup has a closed surface, the orifice receiving the nipple of the insert being occupied on the side of the external surface of the cup by the lining of the nipple of the insert.

The present invention will be described below in more detail with the aid of the accompanying drawings in which
FIG. 1 is a perspective view of an embodiment of an acetabular prosthesis cup according to the invention,
FIG. 2 is a side view of an insert intended to be housed in the cup of the acetabular prosthesis of FIG. 1,
FIG. 3 is a schematic side view partially cut away from the cup, showing, in the upper part, the unlocked belt and, in the lower part, the locked belt,
FIG. 4 is a schematic side view of the insert,
- Figure 5 is a side view, on an enlarged scale, of the nail lining the insert.

According to Figures 1 and 2, the invention relates to an acetabular prosthesis comprising a cup 100 and an insert 200. The cup 100 is placed in the cotyloid bone cavity; after its establishment the cup receives the insert 200 which itself receives the ball of the femoral prosthesis or prosthetic femoral head not shown.

Generally the cup 100 is made of a rigid material nevertheless offering a certain elasticity and which is moreover biocompatible. This material is currently titanium.

The insert 200, which provides the interface between the cup and the prosthetic femoral head, is made of a material having a certain elasticity as well as sliding properties to favor the movement of the prosthetic femoral head. Currently the inserts are made of high density polyethylene (HDPE).

According to Figure 1, the cup is of substantially hemispherical shape. This outer surface is provided with a peripheral groove 101, located in the vicinity of the opening 102 of the cup. This peripheral groove receives an expandable belt 103 provided at its periphery with fastening members 104 in the form of strips constituting edges. hanging. The expandable belt 103 is formed of a split ring, the two ends 105, 106 of which are normally free. However, to allow the cup to be placed in the bone cavity, the belt is locked in the groove, the ends 105, 106 of this split ring-shaped belt being fixed to the bottom of the groove 101 by screws passing through the holes 107, 108 at the bottom of the groove. These orifices then correspond to the threaded orifices 109, 110 of the ends 105, 106 of the extensible belt 103.

When the belt is locked at the bottom of the groove, the external contour of the belt, that is to say the edges of the lamellae 104, does not exceed the envelope formed by the hemispherical external contour of the cup 100.

The cup 100 may also include impaction pins 111, 112, 113.

Finally, for positioning and blocking the insert 200 in rotation in the cup, the apex of the cup is crossed by an orifice 114 whose edge is provided with indexing and rotation blocking means 115 constituted for example by ribs / grooves distributed regularly.

The insert blocked in rotation by the stud in the central orifice of the cup prevents premature wear and allows the position of the anti-dislocation reinforcement to be chosen, that is to say the most stabilizing position.

The insert 200 (FIG. 2) is usually presented in the form of a hemispherical part, made of a relatively elastic material and having sliding characteristics while being biocompatible, such as for example high density polyethylene. At its front edge, the cup 200 has a flange 201 which comes to bear on the outer edge 116 of the opening 102 of the cup 100. The flange is also provided with a tongue 202 or antiluxation reinforcement which closes the 'Equatorial opening of the insert 200 to retain the prosthetic femoral head as already indicated. The reinforcement or overhang extends the hemisphere beyond the equator over a limited part, the orientation of which remains at the discretion of the operator. The prosthetic femoral head can nevertheless be introduced into the cup thanks to a notch located on the opposite edge.

At its outer top, the body 203 of the insert is provided with a stud 204 of suitable dimensions to be housed in the orifice 114 of the cup. This pin 204 has a peripheral surface provided with indexing means 205 complementary to those of the orifice 114. These means are for example ribs and grooves.

Finally, on its face which appears through the orifice 114, the stud 204 has a lining 206, preferably in the same material as that of the cup 100, for example made of titanium, in the shape of a pawn whose tip is pressed into the nipple.

Thus, when the insert 200 is placed in the cup 100, the outer surface of the cup is uniformly closed and the material of the insert cannot come into contact with the bone material.

Figure 3 shows a partially cut side view of the cup 100 with its peripheral groove 101 and the expandable belt 103 housed therein. The belt is of the same type as that of FIG. 1.

In the lower half of Figure 3, the belt 103 is locked in the retracted position in the groove 101 by a screw not shown, accessible from the interior of the cup and passing through the orifice 107 of the cup to be housed in the tapping 109 at the end 105.

On the other hand, the locking screw of the other end 106 of the belt 103 has been removed, releasing this end and the corresponding branch of the belt which relaxes and takes the position shown in the upper part of the figure; the belt partially emerges from the groove 101 and protrudes relative to the contour of the spherical cap of the cup 100. This figure also shows the orifice 114 at the top of the cup and the indexing means 115 constituted by grooves / ribs.

The insert 200 shown in Figure 4 in a position aligned with that of the cup 100 of Figure 3 reveals the particular shape of the tongue 202 and, in dotted lines, that of the prosthetic femoral head 300 of the femoral prosthesis.

The mushroom-shaped stud 204 has, on its cylindrical peripheral surface, the indexing means 205 constituted by grooves.

The pin or nail 206 covering the visible face of the stud 205 is fixed by its tail 207 (FIG. 5) in an orifice 208 of the stud. This positioning is done before the insert 200 is placed in the cup.

The relative orientation between the insert 200 and the cup 100 is chosen at the time of the installation of the insert and according to the particular physiological data of the patient, so as to properly position the tongue 202.

Figure 5 shows the pin 206 extended by its tail 207, grooved to promote its blocking in the orifice 208 of the insert.

In general, the description given above allows many variants for the embodiment of the belt locking means, by the shape given to the belt and by the hooking means which it carries.

Thus this belt could be produced in several lockable parts in the groove.

Likewise, the integral fixing of the belt in the groove could be done at another point than in the middle of the split ring.

The belt hooking means 104 may also have a shape different from that of the example.
