Esta divulgación se refiere a anticuerpos específicos para glipicano 3 (GPC3) o heparán sulfato en GPC3, y su uso para el tratamiento del cáncer.
10 ANTECEDENTES
El cáncer de hígado es la quinta neoplasia más prevalente en el mundo y la tercera causa más común de mortalidad relacionada con el cáncer (Bosch et al., Gastroenterology 127: S5-S16, 2004; El-Serag et al., Gastroenterology 132: 2557-76, 2007). Según la American Cancer Society, el carcinoma hepatocelular (HCC) representa aproximadamente 15 el 75 por ciento de los casos de cáncer de hígado. A menudo no hay síntomas de cáncer de hígado hasta los estadios posteriores. La cirugía es el tratamiento estándar para el cáncer de hígado ya que este tipo de cáncer no responde bien a la mayoría de los medicamentos de quimioterapia. Por lo tanto, existe una necesidad urgente de desarrollar nuevos medicamentos con diferentes mecanismos de acción. La inmunoterapia representa un nuevo enfoque, pero sigue siendo un desafío principalmente debido a la falta de bunas dianas específicas de tumor.
20 La familia de glipicano de proteoglicanos de heparán sulfato se ancla a la superficie celular a través de un enlace covalente a glicosilfosfatidilinositol (GPI). En vertebrados, se han identificado seis miembros de la familia (GPC1-6). Las proteínas glipicano son capaces de modificar las vías de señalización celular y contribuir a la proliferación celular y al crecimiento tisular. El glipicano 3 (GPC3) está altamente expresado en HCC y algunos otros cánceres humanos incluyendo melanoma, carcinomas de células escamosas del pulmón y carcinomas de células claras del ovario, pero 25 no se expresa en tejidos normales (Ho y Kim, Eur J Cancer 47(3): 333-338, 2011). El gen GPC3 codifica una proteína central precursora de 70 kDa, que puede escindirse mediante furina para generar un fragmento amino (N) terminal de 40 kDa y un fragmento carboxilo unido a la membrana (C) terminal de 30 kDa. El término C tiene dos cadenas de glicano de sulfato de heparina (HS). La proteína GPC3 está unida a la membrana celular mediante un anclaje de glicosilfosfatidilinositol. GPC3 se une a las proteínas de señalización Wnt y Hedgehog (Capurro et al., Dev 30 Cell 14: 700-711, 2008; Capurro et al., Cancer Res 65: 6245-6254, 2005), y también puede unirse a factores de crecimiento básicos tales como factor de crecimiento de fibroblastos 2 a través de sus cadenas de glicano de HS (Song et al., JBiol Chem 272: 7574-7577, 1997).
Las mutaciones de pérdida de función de GPC3 causan el síndrome de Simpson-Golabi-Behmel, una rara 35 enfermedad de sobrecrecimiento ligado a X (Pilia et al., Nat Genet 12: 241-247, 1996). Los ratones deficientes en GPC3 tienen síntomas similares (Cano-Gauci et al., J Cell Biol 146: 255-264, 1999). En ratones transgénicos, la sobreexpresión de GPC3 suprime la proliferación de hepatocitos y la regeneración hepática (Liu et al., Hepatology 52(3): 1060-1067, 2010). Además, Zittermann et al. mostraron recientemente que las células de HCC infectadas con lentivirus que expresaban GPC3 soluble (sGPC3) tienen una tasa de proliferación celular más baja (Zittermann et al., 40 Int J Cancer 126: 1291-1301, 2010). La mayor parte de la actividad de unión a proteínas de HSPG se debe a las cadenas de HS (Kim et al., J Endocrinol 209(2): 139-151, 2011). La 50 cadena de HS promedio es de 50-200 unidades repetidas de disacárido de longitud. La metástasis tumoral es la principal causa de muerte relacionada con el cáncer, pero los mecanismos moleculares subyacentes a la metástasis tumoral siguen siendo poco conocidos. Se ha aceptado ampliamente que la metástasis del cáncer se ve facilitada por la actividad proteolítica de las proteasas tales como las metaloproteinasas de matriz. 15 Los anticuerpos y composiciones que se proporcionan en el presente documento pueden usarse para una diversidad de propósitos, tales como para confirmar el diagnóstico de un cáncer que expresa GPC3, por ejemplo HCC, en un sujeto. Por lo tanto, en el presente documento se proporciona un método para confirmar el diagnóstico de cáncer en un sujeto poniendo en contacto una muestra del sujeto diagnosticado con cáncer con un anticuerpo monoclonal humano que se une a GPC3 o cadenas de HS en GPC3 y detectando la unión del anticuerpo a la muestra. Un 20 aumento en la unión del anticuerpo a la muestra en relación con la unión del anticuerpo a una muestra de control confirma el diagnóstico de cáncer. En algunas realizaciones, el método comprende además poner en contacto un segundo anticuerpo que reconoce específicamente el anticuerpo específico de GPC3 con la muestra, y detectar la unión del segundo anticuerpo.
25 De forma similar, en el presente documento se proporciona un método para detectar un cáncer que expresa GPC3, tal como HCC, en un sujeto que incluye poner en contacto una muestra del sujeto con un anticuerpo monoclonal humano descrito en el presente documento, y detectar la unión del anticuerpo a la muestra. Un aumento en la unión del anticuerpo a la muestra en relación con una muestra de control detecta cáncer en el sujeto. En algunas realizaciones, los métodos comprenden además poner en contacto un segundo anticuerpo que reconoce 30 específicamente el anticuerpo específico de GPC3 con la muestra, y detectar la unión del segundo anticuerpo.
Se proporciona además un método para tratar un sujeto con cáncer, por ejemplo HCC, seleccionando un sujeto con un cáncer que expresa GPC3 y administrando al sujeto una cantidad terapéuticamente eficaz de un anticuerpo monoclonal específico para GPC3 o cadenas de HS en GPC3, o un inmunoconjugado que comprende el anticuerpo. 35
En otras realizaciones, el cáncer se trata o se diagnostica mediante la administración de un anticuerpo monoclonal que incluye los residuos de aminoácidos 26-33, 51-57 y 96-105 de SEQ ID NO: 2; o mediante la administración de un anticuerpo monoclonal que incluye los residuos de aminoácidos 26-33, 51-58 y 97-105 de SEQ ID NO: 14 y los residuos 27-32, 50-52 y 89-97 de sEq ID NO: 16 o SEQ ID NO: 31. Las FIGS. 17A-17C son una serie de figuras que muestran que HS20 inhibe el crecimiento tumoral en un modelo de ratón de xenoinjerto de HCC. (A) Se implantaron 10 millones de células HepG2 en ratones sin pelo mediante inyección subcutánea. La FIG. 19A es un esquema de la inmunotoxina HN3(VH)-PE38. Para fabricar una inmunotoxina anti- GPC3, el VH de HN3 se fusionó a un PE38 truncado. La FIG. 19B es un gráfico que muestra que la proteína de inmunotoxina HN3(VH)-PE38 eluida de una columna mono-Q se procesó por una columna de exclusión por tamaño de filtración en gel TSK. La FIG. 20 es una serie de gráficos que muestran la inhibición de la proliferación celular en líneas celulares de HCC por la inmunotoxina HN3(VH)-PE38. Células cancerosas incubadas con diversas concentraciones de las inmunotoxinas anti-GPC3 que contienen HN3(VH)-PE38 o BL22 durante 72 h. La proliferación celular se determinó mediante un ensayo de WST. La línea discontinua indica una inhibición del 50 % de la proliferación celular, que es la concentración de toxinas que reduce la viabilidad celular en un 50 % en comparación con las células que no se trataron con la toxina. BL22: una inmunotoxina específica para CD22 usada como control no específico.
La FIG. 21 es un gráfico que muestra la actividad antitumoral de la inmunotoxina HN3(VH)-PE38 en el modelo de xenoinjerto. Los ratones BALB/c nu/nu se inocularon s.c. con 3 millones de células G1. Claims (12)
- REIVINDICACIONES1. Un receptor de antígeno quimérico (CAR), anticuerpo biespecífico o inmunoconjugado, que comprende un anticuerpo monoclonal de dominio único pesado variable (VH) humano que se une a glipicano 35 (GPC3), en el que el anticuerpo monoclonal comprende las secuencias de la región determinante de complementariedad (CDR) 1, CDr2 y CDR3 de la SEQ iD NO: 2.
- 2. El CAR, anticuerpo biespecífico o inmunoconjugado de la reivindicación 1, en el que la CDR1, CDR2 y CDR3 se determinan usando el esquema de numeración IMGT o Kabat.10
- 3. El CAR, anticuerpo biespecífico o inmunoconjugado de la reivindicación 1 o la reivindicación 2, en el que el anticuerpo monoclonal comprende los residuos 26-33, 51-57 y 96-105 de la SEQ ID NO: 2, o los residuos 3135, 50-65 y 96-105 de la SEQ ID NO: 2.15 4. El CAR, anticuerpo biespecífico o inmunoconjugado de una cualquiera de las reivindicaciones 1-3, enel que el anticuerpo monoclonal comprende la secuencia de aminoácidos de SEQ ID NO: 2.
- 5. El CAR de una cualquiera de las reivindicaciones 1-4, que comprende además un endodominio.20 6. El CAR de la reivindicación 5, en el que el endodominio comprende "CD3Z" o "FceRIy".
- 7. El anticuerpo biespecífico de una cualquiera de las reivindicaciones 1-4, que comprende además un anticuerpo monoclonal o fragmento de unión a antígeno del mismo, que se une a un componente del receptor de linfocitos T.25
- 8. El anticuerpo biespecífico de la reivindicación 7, en el que el segundo anticuerpo monoclonal ofragmento de unión a antígeno se une específicamente a un componente del receptor de linfocitos T.
- 9. El inmunoconjugado de una cualquiera de las reivindicaciones 1-4, que comprende además un agente30 terapéutico.
- 10. El inmunoconjugado de la reivindicación 9, en el que el agente terapéutico comprende un fármaco.
- 11. Una composición que comprende el CAR, anticuerpo biespecífico o inmunoconjugado de una 35 cualquiera de las reivindicaciones 1-10 y un vehículo farmacéuticamente aceptable.
- 12. El CAR, anticuerpo biespecífico o inmunoconjugado de una cualquiera de las reivindicaciones 1-10 para su uso en un método para tratar un sujeto con un cáncer que expresa GPC3, que comprende administrar una cantidad terapéuticamente eficaz del CAR, anticuerpo biespecífico o inmunoconjugado al sujeto.40
- 13. El CAR, anticuerpo biespecífico o inmunoconjugado de una cualquiera de las reivindicaciones 1-10 para su uso en un método de inhibición del crecimiento tumoral o metástasis de un cáncer que expresa GPC3, que comprende seleccionar un sujeto con un cáncer que expresa GPC3 y administrar una cantidad terapéuticamente eficaz del CAR, anticuerpo biespecífico o inmunoconjugado al sujeto.45
- 14. El CAR, anticuerpo biespecífico o inmunoconjugado de la reivindicación 12 o la reivindicación 13, en el que el cáncer que expresa GpC3 es carcinoma hepatocelular (HCC), melanoma, cáncer de pulmón o cáncer de ovario.
