HIF-1 es una proteína heterodimérica compuesta por dos subunidades: (i) una subunidad beta (β) expresada de forma constitutiva también conocida como translocador nuclear de hidrocarburo arilo (ARNT) (que está compartida por otros factores de transcripción relacionados (por ejemplo, el receptor de dioxina/hidrocarburo arilo (DR/AhR)); y

El CNTF (factor neurotrófico ciliar) es una neurocitoquina expresada por células de la glía en nervios periféricos y el sistema nervioso central. Sonic hedgehog (Shh) controla importantes procesos del desarrollo, incluyendo supervivencia de células neuronales y de la glía.

La lisilo oxidasa (LOX) oxida la cadena lateral del peptidilo lisina convirtiendo de ese modo ciertos restos de lisina en alfa-aminoadipico-delta-semialdehído. Este es un cambio post-traduccional que, por ejemplo, posibilita la unión cruzada covalente de las cadenas componentes de colágeno y elastina. Estabiliza los depósitos fibrosos de estas proteínas en la matriz extracelular. Para algunas aplicaciones de terapia génica del SNC, puede ser necesario controlar la actividad transcripcional. Para este fin, puede obtenerse la regulación farmacológica de la expresión génica con vectores víricos incluyendo diversos elementos reguladores y promotores sensibles a fármacos como se describe, por ejemplo, en Haberma et al. (1998) Gene Ther. 5:1604-16011; y Ye et al. (1995) Science 283:88-91.
En ciertas realizaciones, la concentración o el título del vector en la composición es de al menos: (a) 5, 6, 7, 8, 9, 10, 15, 20, 25, o 50 (x 1012 gp/ml); (b) 5, 6, 7, 8, 9, 10, 15, 20, 25, o 50 (x 109 tu/ml); o (c) 5, 6, 7, 8, 9, 10, 15, 20, 25, o 50 (x 1010 iu/ml).
En un aspecto de la descripción, el transgén codifica una molécula biológicamente activa, cuya expresión en el SNC provoca la corrección al menos parcial de la neuropatología y/o la estabilización de la progresión de la enfermedad. En algunos casos, el producto transgénico terapéutico es una proteína IGF-1 que alivia y/o previene los síntomas de ALS. Véase, Roaul et al. (2005) Nat. Med. 11(4):423-428 y Ralph et al. (2005) Nat. Med. 11(4):429-433. En otros aspectos de la descripción, se codifican dos transgenes, por ejemplo, IGF-1 y VEGF, cuya expresión en el SNC provoca la corrección al menos parcial de la neuropatología, tal como el alivio y/o la prevención y/o estabilización y/o ralentización de la progresión de los síntomas de ALS.
En un aspecto cuando se realizan estos métodos, el transgén expresa una cantidad terapéutica de factor de crecimiento de insulina-1 (IGF-1), calbindina D28, parvalbúmina, HIF1-alfa, SIRT-2, VEGF, SMN-1, SMN-2, CNTF (factor neurotrófico ciliar), sonic hedgehog (shh), eritropoyetina (EPO), lisilo oxidasa (LOX), progranulina, prolactina, grelina, neuroserpina, angiogenina y lactógeno placentario.
Para suministrar la solución u otra composición que contenga el vector vírico específicamente a una región particular del sistema nervioso central, tal como a un ventrículo particular, por ejemplo, a los ventrículos laterales o al cuarto ventrículo del cerebro, se puede administrar por microinyección estereotáxica. Por ejemplo, en el día de la cirugía, los pacientes tendrán el armazón base estereotáxico fijado en su sitio (atornillado en el cráneo). Se harán imágenes del cerebro con el armazón base estereotáxico (MRI-compatible con marcas fiduciarias) usando MRI de alta resolución. Las imágenes MRI después se transferirán a un ordenador que ejecuta el software estereotáxico. Se usará una serie de imágenes coronales, sagitales y axiales para determinar el sitio diana de la inyección del vector y la trayectoria. El software traduce directamente la trayectoria en coordenadas tridimensionales apropiadas para el armazón estereotáxico. Se taladrarán agujeros de trepanación por encima del sitio de entrada y el aparato estereotáxico localizado con la aguja implantada a la profundidad dada. Después se inyectará la solución de vector en un vehículo farmacéuticamente aceptable. Pueden usarse vías adicionales de administración, por ejemplo, aplicación cortical superficial bajo visualización directa u otra aplicación no estereotáxica.
Un modo para suministrar el vector vírico es usar una bomba. Dichas bombas están disponibles en el mercado, por ejemplo, en Alzet (Cupertino, CA) o Medtronic (Minneapolis, MN). La bomba puede ser implantable. Otro modo conveniente de administrar el vector es usar una cánula o un catéter.
La presente descripción proporciona métodos para modular, corregir o aumentar la función motora en un sujeto afectado con daño neuronal motor. Con fines de ilustración solamente, el sujeto puede padecer uno o más de esclerosis lateral amiotrófica (ALS), atrofia muscular bulboespinal, atrofia muscular espinal, ataxia cerebelosa espinal, esclerosis lateral primaria (PLS) o lesión traumática de médula espinal.
Sin limitarse a teoría alguna, la patología asociada con el daño a neuronas motoras puede incluir degeneración de neuronas motoras, gliosis, anormalidades de neurofilamentos, pérdida de fibras mielinizadas en los tractos corticoespinales y las raíces ventrales. Se reconocen dos tipos de inicio: inicio bulbar, que afecta a las neuronas motoras del tronco cerebral (afecta a los músculos faciales, el habla y la deglución); e inicio en las extremidades, que afecta a neuronas motoras de la médula espinal, está reflejado por espasticidad, debilidad generalizada, atrofia muscular, parálisis e insuficiencia respiratoria. En ALS, los sujetos han tenido inicio tanto bulbar como en las extremidades. En PLS, los sujetos han tenido inicio bulbar.
La capacidad de organizar y ejecutar actos motores complejos depende de señales desde las áreas motoras en la corteza cerebral, es decir, la corteza motora. Las órdenes motoras corticales desciendes en dos tractos. Las fibras

Los siguientes ejemplos ilustran la presente descripción. Un experto en la materia reconocerá las numerosas modificaciones y variaciones que pueden realizarse sin alterar el espíritu o alcance de la presente descripción. Los ejemplos no limitan de ningún modo la invención.
Titulación de vectores recombinantes
Los títulos del vector AAV se miden de acuerdo con el número de copias del genoma (partículas genómicas por mililitro). Las concentraciones de partículas genómicas se basan en PCR Taqman® del ADN del vector como se ha informado previamente (Clark et al. (1999) Hum. Gene Ther., 10:1031-1039; Veldwijk et al. (2002) Mol. Ther., 6:272278).
Los vectores que portan un gen marcador ensayable, tal como el gen de la β-galactosidasa (Lac Z) o de la proteína fluorescente verde (GFP) pueden titularse usando un ensayo de infectividad. Las células susceptibles (por ejemplo, células HeLa o COS) se transducen con el AAV y se realiza un ensayo para determinar la expresión génica, tal como tinción de células transducidas con el vector de la β-galactosidasa con X-gal (5-bromo-4-cloro-3-indolil-β-Dgalactopiranósido) o microscopía de fluorescencia para células transducidas con GFP. Por ejemplo, el ensayo se realiza del siguiente modo: se siembran 4 x 104 células HeLa en cada pocillo de una placa de cultivo de 24 pocillos usando medio normal de cultivo. Después de la fijación, es decir, aproximadamente 24 horas después, las células se infectan con Ad tipo 5 a una multiplicidad de infección (MOI) de 10 y se transducen con diluciones en serie del vector empaquetado y se incuban a 37ºC. De uno a tres días después, antes de observarse los extensivos efectos citopáticos, se realiza el ensayo apropiado sobre las células (por ejemplo, tinción X-gal o microscopía de fluorescencia). Si se usa un gen indicador tal como β-galactosidasa, las células se fijan en paraformaldehído al 2%, glutaraldehído al 0,5% y se tiñen para la actividad β-galactosidasa usando X-gal. Se recuentan las diluciones de vector que dan células bien separadas. Cada célula positiva representa 1 unidad de transducción (tu) de vector.
Modelo terapéuticamente relevante de esclerosis lateral amiotrófica (ALS).
La esclerosis lateral amiotrófica (ALS) es una enfermedad neurodegenerativa mortal que se caracteriza por una pérdida selectiva de neuronas motoras en la corteza, el tronco cerebral y la médula espinal. La progresión de la enfermedad puede conduce a atrofia de los músculos de las extremidades, axiales y respiratorios. La muerte celular de las neuronas motoras está acompañada por gliosis reactiva, anormalidades de neurofilamentos y una pérdida significativa de fibras grandes mielinizadas en los tractos corticoespinales y las raíces ventrales1-6 . Aunque la etiología de ALS está poco comprendida, las evidencias que se acumulan indican que la ALS esporádica (SALS) y familiar (FALS) comparten muchas características patológicas similar; por tanto, hay esperanza de que el estudio de cualquiera de las formas conduzca a un tratamiento común7. FALS representa aproximadamente el 10% de los casos diagnosticados, de los cuales el 20% están asociados con mutaciones de herencia dominante en la superóxido dismutasa de Cu/Zn (SOD1)8. Los ratones transgénicos que expresan la proteína SOD1 humana mutante (por ejemplo, ratones SOD1G93A) recapitulan muchas características patológicas de ALS y son un modelo animal disponible para estudiar ALS9. Para SALS, se ha implicado una miríada de mecanismos patológicos como causa subyacente, incluyendo la excitotoxicidad inducida por glutamato, la exposición a toxinas, la disfunción del proteasoma, el daño mitocondrial, la desorganización de neurofilamentos y la pérdida de soporte neurotrófico10,11 .
Hasta la fecha, no existe terapia eficaz para el tratamiento de ALS. Se han investigado extensivamente factores neurotróficos tales como el factor de crecimiento de insulina-1 (IGF-1) por su utilidad potencial en el tratamiento de ALS. El suministro intracraneal de vectores víricos a regiones del SNC que están interconectadas con el tronco cerebral y las neuronas motoras espinales mediante el LCR proporciona un medio de administración de agentes terapéuticos potenciales, tales como IGF-1, a áreas que por lo demás serían difíciles de abordar a través de medios de la técnica anterior.
Sin limitarse a teoría alguna, IGF-1 es una proteína terapéutica para el tratamiento de ALS debido a sus muchas acciones a diferentes niveles de neuraxis (véase, Dore et al., Trends Neurosci, 1997, 20:326-331). En el cerebro: se cree que reduce la apoptosis tanto neuronal como de la glía, protege a las neuronas contra la toxicidad inducida por hierro, colchicina, desestabilizantes del calcio, peróxidos y citoquinas. También se cree que modula la liberación de los neurotransmisores acetilcolina y glutamato. También se cree que induce la expresión de neurofilamentos, tubulina y la proteína básica de mielina. En la médula espinal: se cree que IGF-1 modula la actividad ChAT y atenúa la pérdida del fenotipo colinérgico, potencia el brote de neuronas motoras, aumenta la mielinización, inhibe la desmielinización, estimula la proliferación y diferenciación de neuronas motoras a partir de células precursoras y promueve la división, maduración y crecimiento de células de Schwann. En el músculo: se cree que IGF-1 induce la formación de grupos de receptores de acetilcolina en la unión neuromuscular y aumenta la función neuromuscular y la fuerza muscular. En los siguientes experimentos, se utilizó la forma IGF-1Ea de la proteína.
