Предшествующий уровень техники
Принятые средства введения терапевтических агентов (активных агентов) человеку или животному обычно ограничены до некоторой степени биологическими, химическими и физическими барьерами. Примерами физических барьеров являются кожа и различные оболочки органов, которые должны быть преодолены прежде, чем агент достигнет цели. Химические барьеры включают вариации рН, двойные липидные слои и разлагающие ферменты. Оба, как биологически, так и химически активные агенты, особенно чувствительны к данным барьерам.

По своей структуре кожа состоит их двух составных частей - относительно тонкого наружного слоя (эпидермиса) и более толстого внутреннего участка (дермы). Наружный слой эпидермиса (ороговевающий слой) состоит из уплощенных мертвых клеток, которые заполнены кератином. Область между уплощенными мертвыми клетками ороговевающего слоя заполнена липидами, которые образуют ламеллярные фазы, обусловливающие свойства кожи как естественного барьера.
Для эффективной чрескожной доставки физиологически активного агента, который наносят на поверхность кожи (наружное нанесение), агент должен сначала перераспределиться из носителя в ороговевающий слой, затем он, как правило, должен распространиться внутри ороговевающего слоя перед перераспределением из ороговевающего слоя в жизнеспособный эпидермис, дерму и кровоток.
Для преодоления ряда проблем, возникающих при чрескожной доставке, которые связаны с транспортом через дермальные слои (чрескожная абсорбция (всасывание)), физиологически активные агенты могут быть приготовлены с включением одного или более агентов, способствующих проникновению лекарственного препарата. Например, в препаратах для наружного применения в качестве носителя может быть использован водный раствор этанола. Этанол может действовать как агент, способствующий проникновению, который увеличивает поток активного агента через кожу вследствие эффекта растворителя увлекать за собой агент (см. статью Вегпег и соавт., 1. РЕагт. 8ег, 78 (5), 402-406, (1989)). Октилпараметоксициннамат (падимат-О), октилсалицилат и Азон™ представляют собой дополнительные примеры агентов, способствующих проникновению, которые, как было показано, улучшают чрескожное всасывание (см. патент США № 6299900).
В заявке РСТ/АиОО/01419 описан спрей без пропеллента для композиции кожного пластыря, который образует гибкий пористый кожный пластырь для улучшения заживления ран и введения лекарственного препарата, однако, композиция ограничена водорастворимыми соединениями.
Применение чрескожных аэрозольных систем доставки лекарственных препаратов имеет потенциальную возможность преодолеть ограничения существующих устройств для чрескожной доставки лекарственных препаратов, таких как чрескожные пластыри. В частности, потенциальная возможность избежать раздражения кожи (см. статью Могдап и соавт., 1. РЕагт. 8с1. 87,1226-28, (1998)) дает существенное преимущество относительно существующих способов доставки с помощью пластыря и назальной доставки, оба из которых, как показано, вызывают реакции в области нанесения в до 50% случаев у пациентов, использующих дозированные формы данных типов (см. статью Ьорек и соавт., МаШгЕах. 38, 831-39, (2001)).

В чрескожных системах, в которые введены оба компонента, лекарственный препарат и способствующий агент, важно, чтобы способствующий агент высвобождался со скоростью, которая будет приводить к оптимальному эффекту на проникновение лекарственного препарата через кожу. Вследствие этого в пленкообразующей системе адгезив должен демонстрировать эффективную проницаемость для лекарственного препарата и способствующего агента, определяемую профилем доставки рассматриваемого лекарственного препарата. Если растворимость либо лекарственного препарата, либо способствующего агента не оптимизирована, будет затронут профиль всасывания (см. статью Уепка!гашаи и соавт., (1998)). Системы типа лекарственный препарат в адгезивной основе представляют собой более современные системы второго поколения, в которых лекарственный препарат диспергирован в самом адгезивном компоненте. Насыщенная растворимость многих соединений в адгезивах является низкой, поэтому возрастает тенденция к осаждению лекарственного препарата, приводя к уменьшению стабильности (см. статью Койуаи и соавт., (2001)).
Жидкие наполнители (включая лекарственный препарат) будут до некоторой степени пластифицировать адгезив, что могло бы привести к образованию нежелательного остатка на коже. Этот пластифицированный остаток часто бывает липким, собирающим пыль и хлопковый пух и, вследствие этого, является косметически неприемлемым.
Существует необходимость в эффективной чрескожной композиции, которую можно легко нанести на кожу и которая обеспечивает эффективное чрескожное введение.
Неудивительно, что было обнаружено, что до настоящего времени не было дозированной чрескожной аэрозольной композиции, которая улучшает чрескожную доставку путем соответствующего выбора пропеллента и растворителя, существующей в виде однофазного раствора с выбранным агентом, способствующим проникновению, и не оставляющей остатка или пленки.

Сущность изобретения
Данное изобретение является результатом исследований, касающихся готовых дозированных препаратов, которые содержат агенты, способствующие проникновению и повышающие чрескожное всасывание терапевтического агента.
Данное изобретение основывается, по меньшей мере, отчасти, на том выявленном факте, что улучшение чрескожной доставки может достигаться соответствующим выбором гидрофторуглеродного пропеллента, растворенного в низшем спирте, таком как этанол или изопропиловый спирт или их комбинация, и который может также существовать в виде однофазного раствора с выбранным агентом, способствующим проникновению. Кроме того, аэрозольная композиция может изначально содержать воду в количестве до 50 мас./об.%, предпочтительно до 10 мас./об.% воды и более предпочтительно может изначально содержать до 5 мас./об.% воды, не оказывая воздействия на способность летучего носителя растворять требующееся количество терапевтического агента и агента, способствующего проникновению, используемых в данных дозированных чрескожных аэрозольных композициях в их наиболее предпочтительной форме однофазных растворов.
Соответственно, в первом аспекте данное изобретение представляет композицию, включающую по меньшей мере один физиологически активный агент, по меньшей мере один агент, способствующий проникновению, и летучий фармацевтически приемлемый растворитель, содержащий низший спирт и гидрофторуглеродный пропеллент, а также необязательно до 50 мас./об.% воды, где физиологически активный агент, агент, способствующий проникновению в кожу, летучий фармацевтически приемлемый растворитель и пропеллент в комбинации предпочтительно образуют однофазный раствор.
Композиции с относительно более высоким содержанием воды (до 50 мас./об.% воды) могут быть использованы в наружном носителе, который можно наносить на раздраженную кожу, поврежденную кожу или слизистые оболочки, причем композиция может существовать в виде однофазного раствора, эмульсии или микроэмульсии, в которых активный агент и/или агент для проникновения либо полностью растворены в одной из вышеупомянутых фаз носителя, либо, альтернативно, диспергированы в одной из фаз данного носителя или их комбинации, например, когда физиологически активный агент находится в композиции в растворенном виде, а агент, способствующий проникновению в кожу, диспергирован в той же композиции.
Предпочтительными являются композиции, содержащие воду в количестве до 10 мас./об.%.
Композиция, соответствующая данному изобретению, может преодолевать, по меньшей мере, некоторые из недостатков композиций, описанных в вышеупомянутом патенте США № 6325990, которые

Поглощение воды в полисилоксановых системах, таких как описаны в патенте США № 6325990, является моментом, на который следует обратить внимание, в силу необратимых изменений в свойствах полимера, вызываемых водой. Например, показано, что введение воды вызывает как набухание системы, так и нарушение способности адгезивного связывания (см. статью СаЬапе1аз, е! а1., 2003). Кроме того, данное изобретение представляет способ лечения субъекта физиологически активным агентом, предусматривающий нанесение чрескожной композиции, как описано выше в данном контексте, на участок кожи субъекта.

Сведения, подтверждающие возможность осуществления изобретения
Композиция, соответствующая изобретению, содержит гидрофторуглеродный пропеллент. Гидрофторуглеродный пропеллент предпочтительно представляет собой гидрофторалкан, такой как НРС-134а или НРС 127. Наиболее предпочтительным гидрофторуглеродным пропеллентом является НРС-134а.
Обнаружено, что НРС-134а является особенно эффективным в композициях, предназначенных для введения чрескожно, как композиций, соответствующих изобретению, наносимых на кожу, причем НРС134а дает большее насыщение лекарственного препарата по сравнению с другими пропеллентами, такими как диметиловый эфир. Авторами обнаружено, что быстрое получение высокого насыщения активного агента и агента, способствующего проникновению, на коже повышает уровень перераспределения лекарственного препарата и агента, способствующего проникновению, в кожу, быстро создавая резервуар активного агента и агента, способствующего проникновению, в коже. Кроме того, авторами обнаружено, что включение НРС-134а приводит к сокращению времени высыхания, что позволяет физиологически активному агенту и агенту, способствующему проникновению, образовать аморфное отложение после испарения летучего носителя. Предпочтительными агентами, способствующими проникновению, для применения в композиции, соответствующей изобретению, являются солнцезащитные сложные эфиры формулы (I)

Prior art
Adopted means of administering therapeutic agents (active agents) to a human or animal is usually limited to some degree by biological, chemical and physical barriers. Examples of physical barriers are the skin and various organ membranes that must be overcome before the agent reaches its goal. Chemical barriers include pH variations, lipid bilayers, and decomposing enzymes. Both biologically and chemically active agents are particularly sensitive to these barriers.
Percutaneous delivery of therapeutic agents offers a number of inherent clinical benefits and patient benefits relative to conventional oral preparations in the form of tablets and capsules, especially for drugs that cannot be safely administered orally, for example due to irritant effects on the gastrointestinal tract, comprehensive so-called presystemic metabolism and, thus, are largely inactivated in the liver immediately after oral administration, poorly sasyvayutsya or exhibit low bioavailability after oral administration, it is best to operate upon delivery in small amounts standardized for a long period of time, rather than a peak amounts that may be associated with side effects.
The introduction of physiologically active agents through the skin (percutaneous drug delivery) attracts increased attention, since it not only represents a simple dosing regimen, but also provides a relatively slow and controlled release pathway of the physiologically active agent into the systemic circulation. However, transdermal drug delivery is complicated by the fact that the skin behaves as a natural barrier and, as a result, the transport of agents through the skin is a complex mechanism.
According to its structure, the skin consists of two components - a relatively thin outer layer (epidermis) and a thicker inner region (dermis). The outer layer of the epidermis (the stratum corneum) consists of flattened dead cells that are filled with keratin. The region between the flattened dead cells of the stratum corneum is filled with lipids, which form lamellar phases, which determine the properties of the skin as a natural barrier.
For effective percutaneous delivery of a physiologically active agent that is applied to the skin surface (topical application), the agent must first be redistributed from the carrier to the stratum corneum, then it should usually spread inside the stratum corneum before redistribution from the stratum corneum to the viable epidermis, dermis and blood flow.
To overcome a number of problems arising during percutaneous delivery, which are associated with transport through the dermal layers (percutaneous absorption (absorption)), physiologically active agents can be prepared with the inclusion of one or more agents that promote the penetration of the drug. For example, in preparations for external use as a carrier can be used an aqueous solution of ethanol. Ethanol can act as a penetration agent that increases the flow of the active agent through the skin due to the effect of the solvent to entrain the agent (see Vegpeg et al., 1. Reag. 8r, 78 (5), 402-406, (1989) ). Octylparamethoxycinnamate (padimat-O), octyl salicylate and Azon ™ are additional examples of penetration enhancers that have been shown to improve percutaneous absorption (see US Patent No. 6,299,900).
PCT / AiOO / 01419 describes a propellant-free spray for a skin patch composition that forms a flexible porous skin patch to improve wound healing and drug administration, however, the composition is limited to water-soluble compounds.
The use of percutaneous aerosol drug delivery systems has the potential to overcome the limitations of existing percutaneous drug delivery devices, such as transdermal patches. In particular, the potential to avoid skin irritation (see Mogdap et al., 1. Reag. 8c1. 87,1226-28, (1998)) provides a significant advantage over existing methods of delivery using a patch and nasal delivery, both of which , as shown, cause reactions in the area of application in up to 50% of cases in patients using the dosage forms of these types (see the article by Liorek et al., Macyx. 38, 831-39, (2001)).
US patent No. 6325990 relates to a film-forming composition for splashing on the skin, which contains a physiologically active agent, polysiloxane glue, a suction stimulator, a solvent, a volatile silicone, and a propellant. The authors found that this invention suffers from a number of disadvantages.

In percutaneous systems in which both components, the drug and the promoting agent are administered, it is important that the promoting agent be released at a rate that will lead to an optimal effect on the penetration of the drug through the skin. As a consequence, in the film-forming system, the adhesive must demonstrate effective permeability to the drug and the promoting agent, determined by the delivery profile of the drug in question. If the solubility of either the drug or the facilitating agent is not optimized, the absorption profile will be affected (see the article Gapashi et al., (1998)). Drug-type systems in adhesive form are more advanced systems of the second generation, in which the drug is dispersed in the adhesive component itself. The saturated solubility of many compounds in adhesives is low, and therefore the tendency for the drug to precipitate increases, leading to a decrease in stability (see Koyuai et al. (2001)).
Liquid fillers (including the drug) will plasticize the adhesive to some extent, which could lead to the formation of an undesirable residue on the skin. This plasticized residue is often sticky, collecting dust and cotton fluff and, therefore, is cosmetically unacceptable.
There is a need for an effective percutaneous composition that can be easily applied to the skin and which provides an effective transdermal administration.
Not surprisingly, it has been found that, to date, there has been no dosed percutaneous aerosol composition that improves percutaneous delivery by appropriate selection of propellant and solvent existing as a single phase solution with a selected penetration agent and leaving no residue or film.
No assumptions are made that any reference, including any patent or patent document cited in this specification, constitutes prior art. In particular, it is understood that, unless otherwise indicated, a reference to any document in this context does not imply an assumption that any of these documents form part of well-known basic knowledge of engineering in Australia or any other country. Discussion of references suggests that their authors declare, and the applicant retains the right to verify the accuracy and relevance of any of the documents cited in this context.
Summary of Invention
This invention is the result of research on finished dosage formulations that contain penetration agents and increase percutaneous absorption of a therapeutic agent.
This invention is based, at least in part, on the fact that an improvement in percutaneous delivery can be achieved by appropriate selection of a hydrofluorocarbon propellant dissolved in a lower alcohol, such as ethanol or isopropyl alcohol, or a combination thereof, and which can also exist as a single phase solution with a selected penetration agent. In addition, the aerosol composition may initially contain water in an amount up to 50% w / v, preferably up to 10% w / v% water and more preferably may initially contain up to 5% w / v water without affecting the ability of the volatile carrier to dissolve the required amount of therapeutic agent and penetration enhancer used in these metered-dose transdermal aerosol compositions in their most preferred form of single phase solutions.
Accordingly, in the first aspect, the present invention provides a composition comprising at least one physiologically active agent, at least one penetration agent, and a volatile, pharmaceutically acceptable solvent containing a lower alcohol and hydrofluorocarbon propellant, and optionally up to 50% w / v .% water, where the physiologically active agent, the agent that promotes penetration into the skin, the volatile pharmaceutically acceptable solvent and the propellant in combination preferably form single-phase solution.
Compositions with a relatively higher water content (up to 50 w / v% water) can be used in an external carrier that can be applied to irritated skin, damaged skin or mucous membranes, and the composition can exist as a single phase solution, emulsion or microemulsion in which the active agent and / or penetration agent are either completely dissolved in one of the aforementioned carrier phases, or, alternatively, dispersed in one of the phases of the given carrier or their combination, for example, when physiologically the active agent is present in the composition in dissolved form, and the agent promoting penetration into the skin is dispersed in the same composition.
Preferred are compositions containing water in an amount up to 10 w / v%.
The composition according to the invention may overcome at least some of the disadvantages of the compositions described in the aforementioned US Pat. No. 6,325,990, which
- 007351 may result in a biphasic solution or an emulsion, as opposed to a single phase solution according to the invention.
Water absorption in polysiloxane systems, such as those described in US Pat. No. 6,325,990, is a point to which attention should be paid due to irreversible changes in polymer properties caused by water. For example, it has been shown that the introduction of water causes swelling of the system, as well as a violation of the ability of adhesive binding (see the article Saberpe, et al., 2003). Any water uptake during storage of compositions, such as those described in US Pat. No. 6,325,990, may cause changes in the physical properties of the carrier phase separation, reducing the tendency for the physiologically active agent to exit and subsequently reducing percutaneous penetration and / or the need to shake the carrier containing container. before applying it on the skin.
This invention also provides a device for dosed application of a sprayable composition (spray), containing the above composition for percutaneous use.
In addition, the present invention provides a method for treating a subject with a physiologically active agent, comprising applying a percutaneous composition, as described above in this context, to a region of the subject’s skin.

Information confirming the possibility of carrying out the invention
The composition according to the invention contains a hydrofluorocarbon propellant. The hydrofluorocarbon propellant is preferably hydrofluoroalkane, such as HPC-134a or HPC 127. The most preferred hydrofluorocarbon propellant is HPC-134a.
HPC-134a has been found to be particularly effective in formulations intended to be administered transdermally, as compositions according to the invention, applied to the skin, and HPC134a gives a greater saturation of the drug than other propellants, such as dimethyl ether. The authors found that the rapid production of high saturation of the active agent and penetration enhancer on the skin increases the level of redistribution of the drug and the penetration agent into the skin, quickly creating a reservoir of the active agent and penetration agent in the skin. In addition, the authors found that the inclusion of HPC-134a leads to a reduction in drying time, which allows the physiologically active agent and the penetration agent to form an amorphous deposit after the evaporation of a volatile carrier. When the composition is applied to the skin, it is preferable that the volatile solvent evaporates and the composition becomes dry on the skin for 2 minutes, more preferably for 1 minute, leaving no residue or film on the skin.
The amount of propellant in the composition according to the invention is preferably from 15 to 50% v / v and more preferably from 20 to 40% v / v.
The composition according to the invention contains a penetration enhancer.
Preferred penetration agents for use in the composition according to the invention are sunscreen esters of formula (I)
in which K one is hydrogen, lower alcohol, lower alkoxy, halide, hydroxy or ΝΚ 3 Κ four ;

Particularly preferred sunscreen esters are those selected from the group consisting of C eight -C1 eight alkylamine, C eight -C1 eight alkylmethoxycinnamate, C eight -C1 eight alkylsalicylate and mixtures thereof. More preferably, the penetration enhancers are selected from padimate-0 and octyl salicylate.
The amount of penetration enhancer that is present in the composition according to the invention is preferably in the range from 0.1 to 10 May / volume% and more preferably from 2 to 8% / v.

The composition according to the invention contains a lower alcohol, preferably ethanol, propanol (including its isomers) or a mixture thereof. Preferably, when the volatile solvent contains at least 60 May / vol.% One or more lower alcohols. More preferably, the volatile solvent component consists mainly of ethanol, isopropanol, or a mixture thereof. It is present in an amount sufficient to produce a single phase with a penetration agent and a propellant. Typically, the alcohol will be present in an amount of from 40 to 80% v / v% and more preferably from 50 to 70% v / v.
The selection of the solvent used in the composition can be made on the basis of the required profile for percutaneous delivery, as measured by percutaneous penetration in order to achieve the desired pharmacological effect. The composition according to the invention may contain water. The decision about the need for the presence of water and the amount of water will depend on the active physiological agent and its stability and interaction with water and on whether the composition will be applied to irritated skin, damaged skin or mucous membranes. In some cases, water may be an effective solvent, while in other circumstances, the instability of the active agent in the presence of water may cause that water is not included. Of course, in some cases, special precautions against the presence of water, such as the use of moisture scavengers, may be desirable.
The composition according to the invention includes a physiologically active agent. Examples of suitable physiologically active agents include steroid, hormone derivative, nonsteroidal anti-inflammatory drug, opioid analgesic, anti-nausea drug, anti-estrogen, aromatase inhibitor, 5-o-reductase inhibitor, tranquilizer, prostaglandin, antiviral drug, anti-migraine compound, antihypertensive, antihypertensive, antihypertensive, antihypertensive drug antimalarial compound, bronchodilator, antidepressant, anti-Alzheimer's agent, neuroleptic or antipsychotic agent, anticholinergic agent, antiparkinsonic agent, antiandrogen or anorexic agent.
Preferred aesthetics are: tolterodine, ropinirol, granisetron, rivastigmine, buspirone, rizatriptin, zolmitriptan, lacidipine, tropisetron, olanzapine and methylphenidate, 4-hydroxyandrostenedione and its derivatives, finastat read, dutasterid, turosteride, BU 191704, MK-386, alprazolam prodrug forms of minoxidil, pilocarpine, salbutamol, terbutaline, salmeterol, ibogaine, bupropian, rolipram, takrin, flufenazide, haloperidol, N-0923, cyproterone acetate, ΜΕΝΤ (7-methyl-19-testosterone) or mazindol or a pharmaceutical system. Examples of suitable anticholinergic agents include oxybutynin, darifenacin and tolterodine.
More preferably, physiologically acceptable agents include apomorphine, oxybutynin, ropinirole, fentanyl, granisetron, rivastigmine, buspirone, rizatriptan, zolmitriptan, lacidipine, tropisetron, olanzapine and methylphenidate or a pharmaceutically acceptable salt or derivative of any of the above groups.
One aspect of the invention is a device for dosed application of a sprayable composition (spray), containing the composition for percutaneous use. The composition according to the invention will generally remain under pressure inside the container, so that a significant portion of the propellant is in liquid form. A device for the dosed application of a sprayable composition may contain a tip and means for ensuring the dosing of the aerosol composition from the tip. A device for the dosed application of a sprayable composition may also contain a means of spatial separation of the tip of this device at a specified distance from the skin of the subject, to which it is necessary to deliver the sprayed composition.

Example 7 is described with reference to the attached figure. The figure shows a graph showing the penetration of buspirone into the skin over time.
The use of HES propellant in the composition will lead to the formation of a single-phase solution with an improved saturation of the drug as compared to other propellants. By providing a high level of saturation of the active agent and penetrating agent on the skin, it is possible to achieve the formation of amorphous sediment of the drug in the stratum corneum. As a result, an increase in the penetration of the drug into the skin can be expected, as shown in the figure.
Finally, it should be understood that various other modifications and / or changes may be made without departing from the scope of the present invention, as shown in this context.
