Description

Femorale Komponente einer Hüftgelenkendoprothese
Die Erfindung bezieht sich auf eine femorale Komponente einer Hüftgelenkendoprothese mit einem in den Femurkanal einsetzbaren, mittels Knochenzement befestigbaren Schaft, einem Hals und einem mit der Hüftpfanne zusammenwirkenden Kopf.
Die Hüftpfanne ist· normalerweise eine aus ein oder zwei Teilen bestehende prothetische Schale, die im Acetabulum fixiert wird. Der Gelenkkopf ist entweder einteilig mit Schaft und Hals geformt oder eine getrennte Kugel, die einen Τητι erikonus auf τ weist, der mit einem Außenkonus am Hals des femoralen Prothesenteils zusammenwirkt.

Der Schaft wird entweder mittels Knochenzement im Femurkanäl befestigt oder ohne weitere Bindemittel im Feinurkanal verkeilt. Die Neuci^ung bezieht sich auf den femoralen Teil einer Hüftgelenkendoprothese, dessen Schaft mittels Knochenzement befestigt wird. Da die anatomischen Verhältnisse sehr variabel sind, weisen die bekannten Huftgelenkendoprothesen verschiedene Schaftlängen auf sowie unterschiedliche Breitenabmessungen, so daß die jeweils optimalen Abmessungen ausgesucht werden können. Dies hat jedoch den Nachteil, daß eine Anzahl unterschiedlicher Schäfte bereitgestellt werden muß, was die Herstellungskosten sowie die Lagerkosten erhöht.
Der Neuerung liegt die Aufgabe zugrunde, eine mit Knochenzement fixierbare Femurkomponente einer Biüftgelenkendoprothese zu schaffen, die für annähernd alle Yersorgungsfälle universell mit gleichen Schaftabmessungen einsetzbar ist.
Diese Aufgabe wird neuerungsgemäß durch folgende Merkmale gelöst:
a) Der Schaft hat eine Länge von etwa 1^0 bis 170 mm
b) Der Schaft ist im proximalen Bereich im Querschnitt oval bzw. rechteckig mit stark abgerundeten Kanten
c) Der Schaft hat zum distalen Ende hin im unteren Drittel annähernd trapezförmigen Querschnitt mit abgerundeten Kanten, wobei die laterale Seite breiter als die mediale Seite ist

d) Der ovale Querschnitt geht ohne Unstetigkeiten in den trapezförmigen' über.
Der Schaft der neuerungsgemäßen Hüftgelenkendoprothese ist verhältnismäßig lang und sehr schlank ausgeführt. Die Querschnitte sind so gewählt, daß eine optimale Verankerung im vorgebohrte;n Femurschaft gewährleistet ist. Die Abmessungen sind so gewählt, daß filx nahezu alle Versorgungsfälle bei erwachsenen Personen der gleiche Schaft verwendet werden kann. Lediglich im Hinblick auf unterschiedliche Halsabmessungei^ müssen unterschiedliche Typen vorgesehen werden. Aus dem erwähnten Grunde ist daher der neuerungsgemäße Schaft verhältnismäßig unaufwendig herstellbar und verursacht geringe Lagerkosten.
In einer Ausgestaltung der Neuerung ist vorgesehen, daß der Querschnitt des Schaftes im unteren Drittel annähernd konstant ist. Auf diese Weise läßt sich, der Schaft gut zentrisch in die Bohrung im Enochenkanal einbringen, wodurch eine wirksvaie Halterung des Schaftes erzielt wird, insbesondere relativ zu Querbelastungen.
Nach einer weiteren Ausgestaltung der Neuerung ist vorgesehen, daß das Verhältnis von der lateral-medialen Abmessung zur dorsal-frontalen Abmessung im ovalen Querschnitt des Schaftes annähernd Ztk bis 2,7 beträgt. In einer anderen Ausgestaltung der Neuerung ist vorgesehen, daß die laterale Seite des Schaftes

Nach einer weiteren Ausgestaltung der Neuerung ist Torgeseben» daß an der dorsalen -and der frontalen Seite des Sciiaftes mindestens eine gerade verlaufende Hut eingeformt ist« Der Knochenzement füllt auch, die Hüten auf und sichert dadurch, den Schaft gegen Hotation und auch, gegen Kippbewegungen... Nach einer weiteren Ausgestaltung der Neuerung endet eine Nut nahe dem distalen Ende des Schaftes mit Übergang und fällt annähernd mit der Schaftlängsachse zusammen, wobei das obere Ende der Hut bis kurz unterhalb des Kragens endet« Xn vorteilhafter ¥eise ist im proximalen Schaftbereich eine veitere parallele Nut medial von der ersten Nut geformt. Die Breite der Nuten entspricht etwa einem Drittel bis etwa einem Viertel der Schaftbreite am distalen Bnde.
Der Kragen, der annähernd ovale Umfangsgestalt hat, ist verhältnismäßig groß. Nach einer Ausgestaltung der Neuerung ist das Verhältnis der Abstützfläche des Kragens zur Schaftquerschnittsfläche unterhalb des Kragens annähernd 1,5 bis 1,7» vorzugsweise etwa 1,6.

Nach einer anderen Ausgestaltung der Neuerung ist auf der lateralen Seite des Übergangsbereichs vom Kragen zum Hals eine jEUgeUcalottenfönnige Vertiefung geformt. Diese dient zum ΈΆη-schlagen des Schaftes«
Ein Ausführmigsbeispiel der Neuerung vird nachfolgend anhand von Zeichnungen näher erläutert.

Bevor auf die in den Zeichnungen dargestellten Einzelheiten näher eingegangen virdr sei vorangestellt, daß jedes der beschriebenen und gezeigten Merkmale für sich öder in Verbindung mit Merkmalen der Ansprüche von neuerungsvesentlicher Bedeutung ist.

In den Figuren 1 bis 5 ist die sowohl für den linken als auch den. rechten Fenrar verwendbare femorale Komponente einer Hüftgelenkendoprothese allgemein mit 10 bezeichnet. Sie weist einen langen Schaft 11, einen Kragen 12, einen Hals 13 sowie einen sich, an den HaXs anschließenden Äußenkonus lh auf. Letzterer ■wirkt zusammen mit dem Innenkonus einer separaten Gelenkkugel, die aus einer gewebefreundlichen Metallegierung, aus Kunststoff oder Keramik "Desteht*
Der zvisehen 1^0 ut?d 170 mm lange Schaft 11 ist außerordentlich, schlank geformt» Xm unteren Drittel, etwa beginnend im Punkt 15* 1st der Schaft 11 annähernd im Querschnitt konstant« Die Form des Querschnittes geht aus Fig« 5 hervor» Man erkennt« daß der Querschnitt trapezförmig ist mit stark abgerundeten Ecken, wobei die laterale Seite 16 breiter ist als die mediale Seite 17. Am distalen Ende 18 ist der Schaft Π abgerundet»
Oberhalb des Punktes I5 ist der Schaft 11, wie in Fig. h gezeigt, ebenfalls trapezförmig und von gleicher Form wie im unteren Drittel, jedoch vergrößert. Wie aus den Figuren 2 und 3 hervorgeht, ist der Schaft 11 proximal und im prpximalen Bereich annähernd oval oder rechteckig mit stark abgerundeten Ecken. Im mittleren Bereich des Schaftes 11 erfolgt dann der Übergang vom ovalen bzw. rechteckförmigen auf den trapezförmigen Querschnitt. Dieser Übergang ist stetig, damit keine abrupten

Kanten das Einschlagen beTiJTüdern bzw. Verletzungen hervorrufen.
Die l&terale Seite 16 des Schaftes erstreckt sich, bis in α .e Nähe des Kragens 12 entlang einer geraden Linie« und zwar bis zum Punkt 1£. Das restliche Viertel bzw. Fünftel geht mit bogenfönaiger Krümmung, wie bei 2O gezeigt, in den Kragen 12. über. Die mediale Seite 17 des Schaftes 11 verläuft nur annähernd bis ztan Punkt 15 gerade y»f»3 geht dass in eines kreisförmiger!. Bogen 21 mit großem Radius in den Kragen 12 über. Die Verbreiterung des Schaftes 11 oberhalt- des Punktes 15 erfolgt nur sehr langsam» und auch die Breite des Schaftes in proximal an Bereich, ist relativ gering» im dargestellten Äusfüiirungsbeispiel etwa 30 mm, gemessen parallel und in der Nähe des Kragens 12» Die Dicke des Schaftes .11 im Bereich des Kragens beträgt annähernd 12,5 mnu
Erste Nuten 22, 23 auf gegenüberliegenden Seiten des Schaftes 11 (dorsal und frontal) erstrecken sich annähernd in der Längsmitte des Schaftes 11 Tom keilförmig zulaufenden, abgerundeten distalen Ende 18 bis annähernd zur lateralen Seite 16 des Schaftes 11. Distal laufen die Nuten 22, 23 in die Fläche des Schaftes aus· Psirallel zu den ersten Nuten 22 bzw. 23 verlaufen zweite Nuten, von rlere-n eine bei 24 in Fig. 1 dargestellt ist. Sie liegen auf der medialen Seite der Nuteu 22 bzw. 23 und verlaufen nur im proximalen Bereich des Schaftes Die Nuten können den Knochenzement aufnehmen und verhindern mit-

!hin eine Rotation des Schaftes 11 sowie ein Kippen desselben.
Der ovale Kragen 12 besitzt eine relativ große Abstützfläche 25, <die im Bereich der lateralen Seite 16* des Schaftes 11 endet, wie aus Fig« 2 zu erkennen. Das Verhältnis der Abstützfläche 25 zur Querschnittsflächo des Schaftes T1 beträgt annähernd 1,5 bis 1,7r vorzugsweise annähernd 1,6.
Zwischen Kragen 12 und Hals 13 befindet sich in üblicher Weise ein abgerundeter Übergangsberefah 2.6. Tn diesen Bereich 26 ist lateral eine kugelkalottenförmige Vertiefung 27 geformt zwecks Aufnahme eines Siiischlägers bsiis Jinplantieren. des Prothesenteils 11.
Die Nuten 22, 2k sind relativ schmal. Ihre Breite ist gleich "oder kleiner als ein Drittel der Breite der zugeordneten Seite des Schaftes 11 im distalen Abschnitt i6.

    . Femorale Komponente einer Hüftgelenkendoprothese mit einem in den Femurkanal einsetzbaren und mittels Knochenzement befestigbaren Schaft, einem HaXs und einem mit der Hüft— pfanne zusammenwirkenden Kopf, gekennzeichnet durch folgende Merkmale:
    a) Der- Schaft (ii) hat sine Langs von etea IkO bis 1?0 aaa
    b) Der Schaft (11) ist im proximalen Bereich im Querschnitt oval hzvt rechteckig mit stark abgerundeten Kanten
    c) Der Schaft (11) hat zum distalen Ende (18) hin im unteren Drittel annähernd trapezförmigen Querschnitt mit abgerundeten Kanten, wobei die laterale Seite (16) breiter ist als die mediale Seite (17)
    d) Der ovale Querschnitt geht ohne Unstetigkeittin in den trapezförmigen über.
  2. 2. Hüftgelenkendoprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Querschnitt des Schaftes (11) im anteren Drittel annähernd konstant ist.
  3. 3· Hüftgelenkendoprothese nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß das Verhältnis der lateral-medialen Abmessung zur dorsal-frontalen Abmessung im ovalen Querschnitt des Schaftes (ii) annähernd 2,k bis 2,7 beträgt.

  5. 5. Hüftgelenkendoprothese nach, einem der Ansprüche Λ bis ht dadurch gekennzeichnet, daß die mediale Seite (17) bis zum unteren Drittel des Schaftes (11) in einem BogfeJS mit großem Radius verläuft»
  6. 6* Hüftgelenkendoprothese nach einem der Ansprüche 1 bis 5* dadurch gekennzeichnet, daß an der dorsalen und dsr frontalen Seite des Schaftes (11) mindestens eine gerade ver laufende Nut (22, 23, 24) eingeformt ist.
