Description
- Gebiet der Erfindung
- Die Erfindung betrifft allgemein einen Apparat und ein Verfahren zur Durchführung von diagnostischen Verfahren in einem Körper. Im Besonderen betrifft die Erfindung ein diagnostisches Modul, das an das distale Ende einer Eingriffsvorrichtung anfügbar ist, um Endofluoreszenz-Imaging von Gewebe in einem Körper durchzuführen.
- Hintergrund der Erfindung
- Die Fluoreszenz-verstärkte endoskopische Untersuchung von Gewebe umfasst typischerweise ein Endoskop, das eine Lichtquelle (z. B. einen Laser) und Bildsensoren aufweist, die Filter enthalten, die spezifische Wellenlängen, die mit Displasie oder Krebsgewebe in Zusammenhang stehen, passieren lassen. Zunächst erhält der Bediener ein Weißlichtbild, um einen allgemeinen zu untersuchenden Gewebebereich lokalisieren zu können. Als Nächstes betätigt der Bediener eine zweite Lichtquelle, um das Gewebe mit Licht geeigneter Wellenlänge(n) anzustrahlen, so dass eine gewünschte Fluoreszenz-Emission des Gewebes ausgelöst wird. Diese Endoskope sind notwendigerweise aufwändiger und kostspieliger als herkömmliche Endoskope, da sie zusätzliche Lichtquellen, externe Steuerungselektronik und Anzeigegeräte, und Lichtleitungen erfordern, die Wellenlängen im Ultraviolett-(UV-)Bereich, die oft wichtig für Zwecke der Fluoreszenz-Spektroskopie von Gewebe sind, übertragen. Ferner sind diese Endoskope von Natur aus kostspielig und müssen nach jedem Gebrauch vorsichtig wiederaufbereitet und gereinigt werden. Die Wiederaufbereitung stellt eine Belastung für Krankenhäuser dar, mehrere Endoskope zu unterhalten, so dass zu einer gegebenen Zeit wenigstens eines der Endoskope der verschiedenen Bauarten, Längen und Typen zur Verfügung stehen kann.
- Die WO-A-9822805, die ein Dokument ist, das nur nach Art. 54(3) EPÜ gültig ist, offenbart:
Ein miniaturisiertes Spektrometer wird zur Platzierung in einem Körper nahe des zu charakterisierenden Gewebes hergerichtet. Das Spektrometer umfasst eine Lichtquelle (3) und eine Vielzahl an Lichtdetektoren (61). Die Lichtquelle generiert Licht, um das Gewebe anzustrahlen. Die Detektoren detektieren optische Signale von dem angestrahlten Gewebe und wandeln diese optischen Signale in elektrische Signale um. Das miniaturisierte Spektrometer kann am distalen Ende einer Eingriffsvorrichtung (4) angeordnet sein. Optische Leitungen, wie Lichtleitfaserkabeln oder Stränge, die die Länge der Eingriffsvorrichtung erweitern, werden nicht benötigt, wenn das miniaturisierte Spektrometer eingesetzt wird. - Ein weiteres Spektrophotometer ist in EP-A-0777119 gezeigt, allerdings lässt sich dieses nicht in einen Körper einführen und zeigt auch keinen Lichtmodulator.
- Zusammenfassung der Erfindung
- Es ist daher ein grundlegendes Ziel der Erfindung, ein Mittel zur Abwandlung eines herkömmlichen Endoskops vorzusehen, so dass es zur Durchführung von Gewebe-Spektroskopie eingesetzt werden kann. Es ist ein weiteres Ziel der Erfindung ein reversibles Mittel zur Abwandlung des Endoskops vorzusehen, so dass dessen normaler Aufbau auf einfache Weise wiederhergestellt werden kann, ohne dass es dabei zu Beschädigungen kommt oder Justierungen notwendig sind. Es ist noch ein weiteres Ziel dieser Erfindung, einen Aufsatz vorzusehen, der unmittelbar vor der Benutzung eines solchen Endoskops angebracht werden kann, so dass der Benutzer die endoskopische Ausrüstung nicht unnötigerweise vorbereiten, aufbrauchen oder kontaminieren muss. Es ist noch ein weiteres Ziel dieser Erfindung, einen preiswerten Aufsatz vorzusehen, der für eine große Auswahl an Endoskopen verwendet werden kann, und der Input und Output von Licht von/zu dem Endoskop umwandeln kann, so dass Fluoreszenz-Bilder einfach, schnell und kostengünstig hergestellt werden können.
- Ein Endofluoreszenz-Abbildungsmodul wurde entwickelt, das mit einer Vielzahl an Eingriffsvorrichtungen verwendet werden kann, einschließlich, aber nicht beschränkt auf, Endoskope und Zytoskope. Das Modul umfasst ein Gehäuse, das an das distale Ende der Eingriffsvorrichtung angebracht werden kann. Eine oder mehrere Lichtquellen sind in dem Gehäuse angebracht, um das Gewebe in dem Körper anstrahlen zu können.
- Eine Steuerungseinheit ist am proximalen Ende der Eingriffsvorrichtung befestigt. Ein Lichtmodulator ist in dem Gehäuse untergebracht, um das Licht, das von wenigstens einer der Lichtquellen emittiert wird, umzuwandeln.
- In einer Ausführungsform umfasst das Gehäuse einen optisch durchsichtigen Bereich, um das Licht, das von der Lichtquelle abstammt und vom Gewebe empfangen wird, durchzulassen. Lichtquellen können lichtemittierende Dioden, Laser, gepulste Lichtquellen, Quellen ultravioletter Energie oder ein Blitzlicht umfassen, sind aber nicht darauf beschränkt.
- Der Lichtmodulator kann einen Filter umfassen, der dem optischen Kanal der Eingriffsvorrichtung eine Bandbreite an Wellenlängen zur Verfügung stellt. Der Filter kann ein akusto-optisch steuerbarer Filter, ein Interferenzfilter, ein Gitter, ein Prisma, ein holographischer Filter oder eine andere Komponente sein, die einen spektralen Durchlassbereich bereitstellt. Der Lichtmodulator umfasst außerdem einen Verschluss, der eine Flüssigkristallvorrichtung sein kann. In einer weiteren Ausführungsform kann das Lichtsignal durch ein Signal, das mit dem Betrieb des Blitzlichtes korrespondiert, geregelt werden. In einer weiteren Ausführungsform sieht die Steuerungseinheit elektrische Energie und Steuerungssignale zum Betreiben der Lichtquelle und des Detektors in dem Modul vor.
- Der Apparat der Erfindung kann in einem Verfahren zur Durchführung von Endofluoreszenz-Imaging von Gewebe in einem Körper eingesetzt werden. Das Verfahren umfasst das Befestigen eines Gehäuses, das wenigstens eine Lichtquelle und einen Lichtmodulator aufweist, am distalen Ende eines Endoskops. Das Verfahren umfasst weiterhin das Beleuchten des Gewebes mit wenigstens einer der Lichtquellen. In einer Ausführungsform umfasst das Verfahren die Übertragung von Impulsen optischer Energie. In einer weiteren Ausführungsform umfasst das Verfahren die Steuerung der Exposition des optischen Kanals des Endoskops. In einer weiteren Ausführungsform umfasst das Verfahren die spektrale Filterung der optischen Energie von dem Gewebe. In einer weiteren Ausführungsform umfasst das Verfahren ferner die folgenden Schritte: Bereitstellung einer Steuerungseinheit, die eine Stromversorgungseinheit und Steuerungselektronik aufweist; Anbringen der Steuerungseinheit an das proximale Ende eines Endoskops; und Bereitstellung von elektrischer Energie und Steuerungssignalen für die Lichtquelle und den Detektor.

1 zeigt eine Seitenansicht eines Endofluoreszenz-Abbildungsmoduls3 , das durch Gleitpassung an der distalen Spitze eines Endoskops5 angebracht ist. Die Steuerleitungen7 , die elektrische Energie und Steuersignale führen, erstrecken sich vom Endofluoreszenz-Abbildungsmodul3 zurück durch den Arbeitskanal6 des Endoskops5 . Die Steuerungslinien7 erstrecken sich über den Auslass des Arbeitskanals9 hinaus. -
2 zeigt eine Rückansicht des Abbildungsmoduls3 , das an dem Endoskop5 angebracht ist. Eine Instrumentenhalterung11 ragt in das Zentrum des Moduls3 hinein. Die Halterung11 ist so positioniert, dass sie nicht störend auf den optischen Kanal13 oder den Arbeitskanal6 des Endoskops5 einwirkt. Das Abbildungsmodul3 umfasst auch eine Lichtmodulator-Anordnung17 , die über dem optischen Kanal13 angeordnet ist. Der optische Kanal13 kann ein Bündel aus optischen Fasern, ein Fernseh-Abtastelement (z. B. eine CCD-Anordnung), oder ein anderes Abbildungssystem sein. Solche Systeme können auch diejenigen umfassen, die Bildinformationen auf optischem Weg transportieren oder die auf elektrischen oder optischen Abtastsystemen basieren. Das detektierte Bild wird am proximalen Ende des Endoskops5 oder an einer entfernten Stelle betrachtet. -
3 zeigt eine Seitenansicht einer Steuerungseinheit41 für ein Endofluoreszenz-Abbildungsmodul, das am proximalen Griffstück20 des Endoskops5 anbringbar ist. Ein Okular25 ermöglicht die unmittelbare optische Betrachtung des Bildes oder wird als Aufsatz für eine Fernseh- oder Filmkamera verwendet. Ein Arbeitskanal-Auslass9 bietet einen geeigneten Zugang zum Arbeitskanal6 , der sich bis zum distalen Ende des Endoskops5 erstreckt. Die Steuerungslinien7 gehen über den Arbeitskanal-Auslass9 hinaus und enden in einer Buchse mit mehreren Anschlüssen33 . Die Buchse mit mehreren Anschlüssen33 ist klein genug, um sich durch den Arbeitskanal6 hindurch ohne Beeinträchtigung zu erstrecken, wodurch die elektrische Verbindung der Steuerungseinheit41 mit dem distal angebrachten Endofluoreszenz-Abbildungsmodul3 ermöglicht wird. Ein Kabel44 , das einen Stecker43 aufweist, ragt aus der Steuerungseinheit41 heraus und lässt sich mit der Buchse33 am proximalen Ende der Steuerungsleitungen7 verbinden. Ein Flansch45 lässt sich mit einer Bajonettfassung31 zum sicheren Anbringen der Steuerungseinheit41 auf praktische Weise verbinden. Die Elektronik49 der Steuerungseinheit, die elektische Energie und Signale reguliert, wird von einer Batterie47 betrieben. Ein Schalter51 an der Steuerungseinheit41 setzt die Steuerungselektronik49 in Betrieb. - Bezugnehmend auf
4 ist die detaillierte Schnittzeichnung eines Endofluoreszenz-Abbildungsmoduls3 gezeigt. Die Steuerungsleitungen7 enden in der Instrumentenhalterung11 , die das Transformator-Gehäuse61 trägt. Zwei Leitungspaare63a und63b gehen von dem Transformator-Gehäuse61 aus. Das erste Leitungspaar63a ist in einer Nut70 untergebracht und endet in einer zirkulären Xenon-Blitzröhre65 , die durch einen Aufnahmeflansch67 in Position gehalten wird. Der Transformator (nicht gezeigt) im Transformatorgehäuse61 stellt eine Funktion zur Hochregelung der Spannung zum Betreiben der Blitzröhre65 bereit. Das zweite Leitungspaar63b ist in einer zweiten Nut71 untergebracht und endet an der Lichtmodulator-Anordnung17 . Die Lichtmodulator-Anordnung17 umfasst einen Verschluss68 und eine Filteranordnung69 . - Das Endofluoreszenz-Abbildungsmodul
3 weist einen steifen Abschnitt73 am distalen Ende auf, das aus relativ steifem Material, wie Polystyrol, Polycarbonat oder Polysulfon hergestellt worden ist, und bunt oder durchsichtig ist. Ein Urethan-Klebstoff75 befestigt den sich am distalen Ende befindlichen Abschnitt73 an einem flexiblen Ring77 . Der flexible Ring77 rutscht über das Ende des Endoskops5 und sorgt für sichere Befestigung. Materialien für diesen Ring umfassen elastomere Materialien wie Silicon, Gummi, Latex oder Polyurethan. Andere Plastikarten oder Stoffe können verwendet werden, solange das Abbildungsmodul während des gesamten Endoskopie-Vorgangs sicher am Endoskop5 angebracht ist. -
5 zeigt eine detaillierte Lichtmodulator-Anordnung17 . Ein Flüssigkristall-Verschluss81 und ein benachbarter akusto-optisch einstellbarer Filter (AOTF)83 sind mit einem Klebstoff an einer Oberfläche des Endofluoreszenz-Abbildungsmoduls3 befestigt. Ein Halterungsblock99 aus Metall stellt eine Befestigungsoberfläche für die einzelnen Bestandteile des Verschlusses81 und des AOTF83 bereit. Die AOTF-Steuerungsleitungen101 und Verschluss-Steuerungsleitungen103 führen durch die Nut71 (gezeigt in3 ). Der Flüssigkristall-Verschluss81 umfasst einen Flüssigkristall85 , der zwischen zwei Elektroden87 angebracht ist. Die Elektroden können metallisierte Schichten auf Glas-Deckgläsern89 sein. Bei Anlegen eines elektrischen Feldes zwischen den Elektroden87 wird Licht, das durch den Flüssigkristall85 tritt, polarisiert. Ein Polarisationsfilter91 ist auf Kreuzpolarisation mit dem Flüssigkristall85 in dessen aktivem Zustand abgestimmt. Dadurch wird verhindert, dass optische Energie zum optischen Kanal13 des Endoskops5 durchtritt, wenn ein elektrisches Signal an die Elektroden87 angelegt wird. Ein elektrisches Signal kann für die Dauer des von der Blitzröhre65 ausgehenden optischen Pulses angelegt werden, um vorübergehend den optischen Kanal13 zu schließen. - Ein elektrisches Signal, das an beide Seitenelektroden
93 des AOTF83 angelegt wird, ändert den Brechungsindex des AOTF-Kristalls95 und polarisiert die übertragene optische Energie. Schwächung oder Auswahl bestimmter Wellenlängen wird erreicht, wenn der AOTF-Kristall95 in Verbindung mit Polarisationsfiltern97 verwendet wird. Das Anlegen von Spannung an die Elektroden93 steuert die ausgewählte Wellenlänge an, wodurch die Transmission spezifischer Farben ermöglicht wird, während andere Farben reflektiert werden. - Die Inbetriebnahme des Endofluoreszenz-Abbildungsmoduls
3 erfolgt nach dem Anbringen an die Eingriffsvorrichtung. Die Blitzröhre65 wird in Verbindung mit der Betätigung des Verschlusses81 gezündet, und wird außerdem mit dem Filter83 abgestimmt, um für einen temporären oder repetitiven Fluoreszenz-Algorithmus zu sorgen. Der Verschluss81 wird verwendet, um zeitweilig das Eindringen jeglichen Weiß- oder Farblichtes in den optischen Kanal13 während des Zündens der Blitzröhre65 zu verhindern. - Die Blitzröhre
65 kann einen eingebauten UV-Filter aufweisen, um jegliches Licht mit Wellenlängen außerhalb des UV-Bereichs abzuschwächen. Ein typischer UV-Wellenlängen-Bereich von Interesse kann den Bereich von 290 Nanometern bis 370 Nanometern umfassen. Rote Fluoreszenz, die auftreten kann, kann wirksam mit dem Endoskop5 detektiert werden, indem der AOTF83 auf eine mittlere Wellenlänge von etwa 600 Nanometern eingestellt wird, wodurch alle Signale bis auf das Fluoreszenz-Signal ausgeblendet werden. Blaue Fluoreszenz oder UV-Fluoreszenz, die auftreten können, können ebenfalls detektiert werden, wenn der AOTF83 in Sequenz mit dem Beginn und der Dauer der von der Blitzröhre65 emittierten optischen Energie eingestellt und abgestimmt wird. - Äquivalente
- Während die Erfindung insbesondere mit Bezug auf spezifische bevorzugte Ausführungsformen gezeigt und beschrieben wurde, sollte sie vom Fachmann so verstanden werden, dass verschiedene Änderungen in der Ausgestaltung und in Einzelheiten erfolgen können.
Claims (13)
- Ein diagnostischer Apparat zur Verwendung mit einer Eingriffsvorrichtung (
5 ), welche in einen Körper einführbar ist, umfassend: eine an das proximale Ende der Eingriffsvorrichtung anfügbare Steuerungseinheit (41 ); ein an das distale Ende der Eingriffsvorrichtung (5 ) entfernbar anfügbares Gehäuse; mindestens eine in dem Gehäuse angeordnete Lichtquelle für die Beleuchtung von Gewebe innerhalb des Körpers; ein in dem Gehäuse angebrachter Lichtmodulator (17 ) für die Anpassung des von der Lichtquelle ausgesandten Lichts nach Sendens eines Steuerungssignals an den Lichtmodulator (17 ) durch die Steuerungseinheit (41 ). - Der diagnostische Apparat (
3 ) nach Anspruch 1, worin das Gehäuse einen flexiblen Ring (77 ) zur Absicherung am distalen Ende der Eingriffsvorrichtung durch Aufpressen umfasst. - Der diagnostische Apparat (
3 ) nach Anspruch 1, worin mindestens ein Teil des Gehäuses (73 ) ein optisch durchscheinendes Material oder ein farbiges Material umfasst. - Der diagnostische Apparat (
3 ) nach Anspruch 1, worin mindestens eine der Lichtquellen eine lichtemittierende Diode, einen Laser, ein Blitzlicht, eine pulsierende Lichtquelle oder eine Quelle, die ultraviolette Energie emittiert, umfasst. - Der diagnostische Apparat (
3 ) nach Anspruch 1, worin der Lichtmodulator (17 ) die optische Energie, welche von dem Gewebe emittiert wird, anpasst. - Der diagnostische Apparat (

3 ) nach Anspruch 6, weiterhin umfassend eine an das proximale Ende der Eingriffsvorrichtung (5 ) anfügbare Steuerungseinheit (41 ), wobei die Steuerungseinheit (41 ) elektrische Energie und Steuerungssignale für mindestens eine der Lichtquellen und den Filter (69 ) bereitstellt. - Der diagnostische Apparat (
3 ) nach Anspruch 1, worin der Lichtmodulator (17 ) einen Verschluss (68 ) umfasst, und wobei der Verschluss (68 ) gegebenenfalls eine Flüssigkristallvorrichtung (81 ) ist. - Der diagnostische Apparat (
3 ) nach Anspruch 1, worin die Steuerungseinheit (41 ) elektrische Energie und Steuerungssignale für mindestens eine der Lichtquellen bereitstellt. - Ein diagnostischer Apparat nach Anspruch 1, worin das (die) Eingriffsvorrichtungen) ein Endoskop (
5 ) ist und der Lichtmodulator (17 ) einen Verschluss (68 ) zur Steuerung der Belichtung eines optischen Kanals (13 ) des Endoskops (5 ) durch optische Energie, welche durch mindestens eine Lichtquelle emittiert wird, umfasst, und wobei der Lichtmodulator (17 ) ebenso einen Filter (69 ) zur Begrenzung des Wellenlängenbereichs einer durch das Endoskop detektierten optischen Energie (5 ) umfasst, und ggf.: (a) mindestens eine der Lichtquellen ein Blitzlicht ist; (b) der Verschluss eine Flüssigkristallvorrichtung (81 ) ist; (c) der Filter (69 ) ein akusto-optisch einstellbarer Filter (83 ) ist; oder (d) der Lichtmodulator (17 ) auf ein Signal reagiert, welches durch die Anwendung mindestens einer der Lichtquellen ausgelöst wird, welche ein Blitzlicht ist. - Der diagnostische Apparat (
3 ) nach Anspruch 1, worin der Lichtmodulator (17 ) weiterhin einen Transformator umfasst. - Der diagnostische Apparat (
3 ) nach Anspruch 12, worin der Lichtmodulator (17 ) weiterhin umfasst: (a) Steuerleitungen (7 ), umfassend ein erstes Ende und ein zweites Ende, wobei das erste Ende in elektrischer Verbindung mit dem Transformator steht und das zweite Ende in elektrischer Verbindung mit der Steuerungseinheit (41 ) steht; (b) ein erstes Paar an Drähten (63a ) umfassend ein erstes Ende und ein zweites Ende, worin das erste Ende in elektrischer Verbindung mit dem Transformator steht und das zweite Ende in elektrischer Verbindung mit der Lichtquelle um (65 ) steht; oder (c) ein zweites Paar an Drähten (63b ) umfassend ein erstes Ende und ein zweites Ende, worin das erste Ende in elektrischer Verbindung mit dem Transformator und das zweite Ende in elektrischer Verbindung mit dem Lichtmodulator (17 ) steht.
