Description
- Diese Erfindung betrifft die Verwendung von Estern von L-Carnitin und Acyl-L-carnitin mit Hydroxysäuren zur Erzeugung von pharmazeutischen Zusammensetzungen, die solche Ester als aktive Bestandteile enthalten, die für die topische Auftragung für die Behandlung von Dermatosen geeignet sind.
- Die erfindungsgemäß nützlichen Verbindungen sind die Ester der folgenden Hydroxysäuren:
- α-Hydroxybuttersäure,
- α-Hydroxyisobuttersäure,
- β-Hydroxybuttersäure,
- γ-Hydroxybuttersäure,
- α-Hydroxyisocapronsäure,
- α-Hydroxyisovaleriansäure,
- Äpfelsäure und
- Tartronsäure.
- Die Agyl-Gruppe ist C&sub1;&submin;&sub5;-Alkanoyl, ausgewählt aus Acetyl, Propionyl, Butyryl, Isobutyryl, Valeryl und Isovaleryl.
- In den erfindungsgemäß zu verwendenden Verbindungen sind sowohl die inneren Salze als auch die Salze der oben genannten Ester mit pharmakologisch akzeptablen Säuren eingeschlossen.
- Pharmakologisch akzeptable Salze umfassen zusätzlich zu den inneren Salzen alle pharmazeutisch akzeptablen Salze, die durch die Addition einer Säure an L-Carnitin hergestellt werden und die keine unerwünschten toxischen oder kollateralen Wirkungen ergeben. Die Bildung von pharmazeutisch akzeptablen Säureadditionssalzen ist in der pharmazeutischen Technologie gut bekannt.
- Nicht-beschränkende Beispiele von geeigneten Salzen umfassen Chlorid-, Bromid-, Orotat-, saure Aspartat-, saure Citrat-, saure Phosphat-, Fumarat-, saure Fumarat-, Lactat-, Maleat-, saure Maleat-, saure Oxalat-, saure Sulfat-, Glucosephosphat-, Tartrat- und saure Tartratsalze.
- Die Ester von L-Carnitin und dem oben genannten Alkanoyl-L-carnitin mit β-Hydroxybuttersäure und die pharmakologisch akzeptablen Salze davon sind bekannte Verbindungen.
- Z.B. offenbart EP-0443996 A1 die Aktivität dieser Ester bei der Inhibition von neuronaler Degenerierung (wie es z.B. bei der Alzheimer- Krankheit und Parkinsonschen-Krankheit auftritt) und Leberproteolyse und bei der Behandlung von Koma.
- Ebenso sind die Ester von L-Carnitin und dem oben erwähnten Alkanoyl-L-carnitin mit γ-Hydroxybuttersäure und die pharmakologisch akzeptablen Salze davon bekannte Verbindungen (siehe z.B. EP-A-429403 und EP-A-442850). Diese Ester haben die gleichen pharmakologischen Eigenschaften wie die β-Hydroxybuttersäureester.
- Auf der anderen Seite sind die Ester von L-Carnitin und den oben genannten Alkanoyl-L-carnitinen mit anderen Hydroxysäuren als β- und γ- Hydroxybuttersäure neue Verbindungen. Deren Herstellung kann ähnlich wie die der bekannten Ester durchgeführt werden, wie es in den oben genannten europäischen Patentanmeldungen offenbart ist, wobei nur geringfügige Änderungen in Abhängigkeit von der ausgewählten Hydroxysäure, die dem Durchschnittsfachmann der organischen Synthese klar sind, durchgeführt werden müssen.
- Die Herstellung einiger der Ester, die für die dermatologische Verwendung entsprechend dieser Erfindung geeignet sind, wird nachfolgend beschrieben.
- γ-Brombuttersäure (3,3 g; 0,02 mol) wurde in Benzylalkohol (15 ml) suspendiert. Die Suspension wurde auf 0ºC gekühlt, und Thionylchlorid (8 ml; 0,01 mol) wurde tropfenweise langsam dazugegeben Die resultierende Mischung wurde 16 h bei Raumtemperatur gehalten, dann unter Vakuum konzentriert, zur Entfernung des Thionylchlorids, und zur Entfernung des Benzylalkohols destilliert. Es zeigte sich, daß der Destillationsrest die Zielverbindung war.

- Das innere Salz von Carnitin (0,8 g; 0,005 mol) wurde in 10 ml wasserfreiem Dimethylformamid suspendiert. Benzylester von γ-Brombuttersäure (1,3 g; 0,005 mol) wurde zu der Suspension gegeben. Die resultierende Reaktionsmischung wurde bei 60ºC für 48 h unter einem Stickstoffstrom unter Rühren gehalten und dann im Vakuum bis zur vollständigen Lösungsmittelentfernung destilliert; 1,3 g des Restes wurden erhalten, der sich als Zielverbindung erwies.

- Die Verbindung von Schritt b (1,3 g) wurde in 20 ml einer 1:1 H&sub2;O:EtOH-Mischung aufgelöst. Die resultierende Lösung wurde in der Gegenwart von 150 mg 10 % Pd/C bei 3 Atmosphären Wasserstoff für 2 h hydriert. Die Mischung wurde filtriert und im Vakuum konzentriert. 1 g der Zielverbindung wurden erhalten.

- Die Verbindung von Schritt c (1 g) wurde auf 30 ml AMBERLITE IRA 402, stark basischem Harz, aktiviert in der Cl&supmin;-Form, eluiert. Das Eluat wurde lyophilisiert. Ein stark hygroskopischer Feststoff wurde erhalten.

- γ-Brombuttersäure (3,3 g; 0,02 mol) wurde in Benzylalkohol (15 ml) suspendiert. Die Suspension wurde auf 0ºC gekühlt, und Thionylchlorid (8 ml, 0,01 mol) wurde langsam tropfenweise zugegeben. Die resultierende Mischung wurde 16 h lang bei Raumtemperatur gehalten, dann unter Vakuum konzentriert, zur Entfernung des nicht-reagierten Thionylchlorides, und zur Entfernung des Benzylalkohols destilliert. Der Destillationsrest wurde durch Silicagel-Chromatographie unter Verwendung von Hexan-AcOET 98:2 als Eluent gereinigt.

- Das innere Salz von Acetyl-L-carnitin (1,62 g; 0,008 mol) wurde in 12 ml wasserfreiem Dimethylformamid suspendiert. γ-Brombuttersäurebenzylester (2,05 g; 0,008 mol) wurde zu der Suspension gegeben. Die resultierende Reaktionsmischung wurde 24 h unter einem Stickstoffstrom unter Rühren gehalten. Ethylether wurde dann bis zur vollständigen Ausfällung einer Verbindung zugegeben, die abfutriert wurde. 3,43 g der Zielverbindung wurden somit erhalten.

- Die Verbindung von Schritt b (1 g) wurde in 20 ml absolutem Ethanol aufgelöst. Die resultierende Lösung wurde in der Gegenwart von 100 mg 10 % Pd/C bei bei 3 Atmosphären Wasserstoff hydriert und unter Vakuum konzentriert. 0,75 g der Zielverbindung wurden erhalten Ausbeute 98 %.

- Die Verbindung von Schritt c (1 g) wurde auf 30 ml eines stark basischen Harzes AMBERLITE IRA 402, aktiviert in der HCO&sub3;&supmin;-Form, eluiert. Das Eluat wurde lyophilisiert. Ein hochhygroskopischer Feststoff wurde erhalten.

- Das Natriumsalz von β-Hydroxybuttersäure (1,2 g; 0,01 mol) wurde in Benzylbromid (6 ml, 0,05 mol) suspendiert. 18-Krone-6 (0,264 g), aufgelöst in 7 ml Acetonitril, wurde zu der Mischung gegeben. Die resultierende Lösung wurde unter Stickstoffstrom teilweise konzentriert und dann unter Rühren bei 80ºC 90 min lang gehalten. Zu der gekühlten Lösung wurde eine Mischung aus Hexan-H&sub2;O gegeben. Die getrennte und getrocknete organische Phase wurde konzentriert und dann unter Vakuum destilliert, zur Entfernung des überschüssigen Benzylbromids. 1,1 g fester Rest wurden erhalten, der als Zielverbindung identifiziert wurde. Ausbeute 56 %.

- Thionylchlorid (7,7 ml; 0,1 mol) wurde zu Isovaleryl-L-carnitinchlorid (10 g; 0,035 mol) gegeben. Die resultierende Mischung wurde 4 h lang bei Raumtemperatur gehalten, dann unter Vakuum konzentriert, zur Entfernung des überschüssigen Thionylchlorides. Der Rest wurde dreimal mit wasserfreiem Ethylether gewaschen. Das somit erhaltene rohe Reaktionsprodukt wurde ohne weitere Reinigung in dem anschließenden Schritt verwendet.
- Das Säurechlond von Isovaleryl-L-carnitinchlorid (0,035 mol) von Schritt b wurde in wasserfreiem Tetranydrofuran (25 ml) aufgelöst. Zu der resultierenden Lösung wurde der β-Hydroxybuttersäurebenzylester (7 g; 0,035 mol) von Schritt a gegeben. Die Reaktionsmischung wurde unter Rühren über Nacht bei 25ºC gehalten. Ethylether wurde dann bis zur vollständigen Ausfällung dazugegeben Der somit erhaltene Feststoff wurde abfiltriert und mit Ethylester gewaschen. 14 g der Zielverbindung wurden erhalten. Ausbeute 89 %.

- Die Verbindung von Schritt c (14 g; 0,031 mol) wurde in H&sub2;O-Ethanol 1:1 (100 ml) aufgelöst und in der Gegenwart von 1,5 g 10 % Pd/C bei 4 Atmosphären 2 h lang hydriert. Die Reaktionsmischung wurde filtriert, das Filtrat unter Vakuum zur Trockne konzentriert und der Rest von Aceton- Ethylether kristallisiert. 10 g einer hygroskopischen Verbindung wurden erhalten.

- Die Dermatosen, die geeignet mit den erfindungsgemäßen Zusammensetzungen behandelt werden, sind insbesondere Ichthyose, Psoriasis und solche Dermatosen, die durch mangelhafte Keratinisierung induziert werden, wie Schuppen, Akne und Palmar und Fußsohlenhyperkeratose. Ichthyose ist eine Dermatose, die durch allgemeine Trockenheit, Raubheit und Abschuppen der Haut gekennzeichnet ist. Sie kann als eine angeborene Krankheit, die von Geburt an vorhanden ist, oder als eine metabolische Erkrankung auftreten, die mit einer Unterfunktion der Schilddrüse oder mit der Aufnahme von Arzneimitteln (wie Butyrophenolen), die die Lipid- Synthese inhibieren, oder als ein paraneoplastisches Syndrom, Manifestierung eines Tumorverlaufes, an dem innere Organe beteiligt sind, auftreten.
- Xeroderma, die mildeste Form von Ichtyose, ist weder angeboren, noch mit systemischen Abnormalitäten verbunden. Üblicherweise tritt sie an den Unterbeinen von Patienten mittleren Alters oder älteren Patienten, am häufigsten während des kalten Wetters, und bei Patienten auf, die häufig baden. Es kann ebenfalls ein geringes bis mäßiges Jucken und eine damit verbunden Dermatitis aufgrund von Detergentien oder anderen irritierenden Stoffen auftreten.
- Die ererbten Ichthyosen, die alle durch eine übermäßige Akkumulation von Schuppen auf der Hautoberfläche gekennzeichnet sind, sind entsprechend klinischen, genetischen und histologischen Kriterien klassifiziert.
- Bekannte Behandlungen für jegliche Form der Ichthyose umfassen das topische Aufbringen von hydratisierenden Emollentien auf die Haut. Weiterhin werden Salicylsäure oder Vitamin A-haltige Salben in großem Umfang verwendet.
- Ein keratolytisches Mittel, das insbesondere zur Entfernung von Schuppen bei der Ichthyose vulgaris, lamellaren Ichthyose und geschlechtsgebundene Ichthyose wirksam ist, umfaßt 6 % Salicylsäure in einem Gel, das sich aus Propylenglykol, Ethylalkohol, Hydroxypropylcellulose und Wasser zusammensetzt.
- Weiterhin umfassen bekannte Arzneimittel zur Behandlung dieser Erkrankung: 50 % Propylenglykol in Wasser, hydrophiles Petrolatum und Wasser (in gleichen Teilen) und kalte Creme und eine α-Hydroxysäure (z.B. Milchsäure und Pyruvinsäure) in verschiedenen Basen. Bei der lamellaren Ichthyose wird eine 0,1 % Tretinoin (Vitamin A-Säure; Retinsäure)-Creme verwendet. Keine dieser Behandlungen hat sich als ausreichend wirksam erwiesen.
- Hyperkeratose ist eine Verdickung der Hornhautschicht der Haut.
- Die Behandlung der Wahl ist die topische Auftragung von Arzneimitteln, die Harnstoff, Propylenglykol und Salicylsäure enthalten. Ebenfalls in diesem Fall zeigte sich keine der bekannten Behandlungen als ausreichend effektiv.
- Es wurde nun festgestellt, daß die erfindungsgemäßen Verbindungen, wenn sie topisch als Lösungen, Lotionen, Cremes oder Salben aufgebracht werden, die von 0,01 bis 20 %, bevorzugt von 1 bis 15 % und am meisten bevorzugt von 2 bis 10 Gew.-% von zumindest einer der vorgenannten Verbindungen enthalten, sehr wirksam sind, um die vollständige Remission von ichthyotischen Zuständen bei Menschen und bei der Heilung von Psoriasis und solchen Erkrankungen zu erzielen, die durch eine geänderte Keratinisierung wie Schuppen, Akne und Palmar und Fußsohlenhyperkeratose verursacht werden. Es wurde ebenfalls festgestellt, daß dann, wenn die Lösungen, Cremes oder Salben dieser Erfindung regulär auf täglicher Basis aufgebracht werden, die behandelten Hautflächen innerhalb von etwa 2 bis 3 Wochen den Normalzustand erreichen. Die erfindungsgemäß verwendeten Verbindungen werden durch Verfahren hergestellt, deren Schritte in dem folgenden Reaktionsschema erläutert werden, worin R, R&sub1; und X die zuvor definierten Bedeutungen haben.
- Zur Herstellung der erfindungsgemäßen Verbindungen wird zumindest einer der Ester dieser Erfindung anfangs bevorzugt in Wasser oder Ethanol aufgelöst. Die somit hergestellte Lösung kann auf koventionelle Weise mit allgemein erhältlichen Salbengrundlagen wie hydrophiler Salbe (USP) oder Petrolatum (USP) vermischt werden.
- Das zum Auflösen der erfindungsgemäßen Verbindungen verwendete Wasser oder Ethanol kann in einer Konzentration von 1 bis 30 Vol.-% der Gesamtzusammensetzung vorliegen. Die erfindungsgemäßen Verbindungen können ebenfalls in einer Lösungs- oder Lotionform formuliert werden. Z.B. wird ein Ester entsprechend dieser Erfindung direkt in einer Mischung aus Wasser, Ethanol und Propylenglykol (40:40:20, bezogen auf das Gewicht) aufgelöst.
- Einige Formulierungsbeispiele werden nachfolgend beschrieben:
- Formulierung 1:5 % Lösung
- 5 g eines Esters dieser Erfindung wurden in 5 ml Wasser aufgelöst, und die resultierende Lösung wurde mit 40 ml Ethanol und 20 ml Propylenglykol versetzt. Ausreichend Wasser wurde zugegeben, zur Herstellung von 100 ml der Formulierung.
- Formulierung 2:5 % Salbe
- 5 g eines Esters dieser Erfindung wurden mit 95 g hydrophiler Salbe vom USP-Grad vermischt, bis eine gleichmäßig Konsistenz resultierte.
Claims (10)
1. Verwendung von Estern von L-Carnitin und Acyl-L-carnitinen mit
Hydroxysäuren, ausgewählt aus der Gruppe&sub1; bestehend aus
α-Hydroxybuttersäure, α-Hydroxyisobuttersäure, β-Hydroxybuttersäure,
γ-Hydroxybuttersäure, α-Hydroxyisocapronsäure, α-Hydroxyisovaleriansäure, Äpfelsäure und
Tartronsäure, worin die Acylgruppe C&sub1;&submin;&sub5;-Alkanoyl ist, ausgewählt aus
Acetyl, Propionyl, Butyryl, Isubutyryl, Valeryl und Isovaleryl, und der
pharmakologisch akzeptablen Salze davon zur Herstellung von
pharmazeutischen Zusammensetzungen, die zur topischen Auftragung für die
Behandlung von Dermatosen geeignet sind.
2. Verwendung nach Anspruch 1, worin die Ester von L-Carnitin und
den Acyl-L-carnitinen in der Form von inneren Salzen vorliegen.
3. Pharmazeutische Zusammensetzung, die zur topischen Auftragung
zur Behandlung von Dermatosen geeignet ist, umfassend einen Ester gemäß
Anspruch 1 mit der Ausnahme der Ester mit β-Hydroxybuttersäure und γ-
Hydroxybuttersäure als aktiven Bestandteil und einen pharmakologisch
akzeptablen Exzipienten dafür.
4. Zusammensetzung nach Anspruch 3, zur Behandlung von Ichthyose
und Psoriasis.
5. Zusammensetzung nach Anspruch 3, zur Behandlung von Dermatosen,
die durch mangelhafte Keratinisierung verursacht werden.
6. Zusammensetzung nach Anspruch 5, zur Behandlung von Schuppen,
Akne und Palmar und Fußsohlenhyperkeratose.
7. Zusammensetzung nach einem der Ansprüche 3 bis 6 in der Form
einer Lösung, Lotion, Salbe oder Creme.
8. Zusammensetzung nach Anspruch 7, umfassend von 0,01 bis 20 %,
bevorzugt von 1 bis 15 %, am meisten bevorzugt von 2 bis 10 Gew.-% von
zumindest einer der Verbindungen nach Anspruch 1.
9. Ester von L-Carnitin und Acyl-L-carnitin mit
α-Hydroxybuttersäure, α-Hydroxyisobuttersäure, α-Hydroxyisocapronsäure, α-
Hydroxyisovaleriansäure, Äpfelsäure und Tartronsäure, worin die Acyl-
Gruppe C&sub1;&submin;&sub5;-Alkanoyl ist, ausgewählt aus Acetyl, Propionyl, Butyryl,
Isobutyryl, Valeryl und Isovaleryl.
10. Pharmakologisch akzeptables Salz des Esters nach Anspruch 9.
