Description
- Die Erfindung betrifft Implantate mit einer porösen Oberfläche. Insbesondere betrifft sie Implantate mit einer porösen Oberfläche, wobei die Implantate durch Anbringen des Implantatkörpers an der porösen Oberfläche durch Formen hergestellt werden.
- Zunehmende Beachtung findet der Einsatz nichtmetallischer Materialien zum Aufbau prothetischer orthopädischer Implantate. Materialien wie thermoplastische Kunststoffe, thermoplastische Verbundstoffe, Keramikstoffe u. a. wurden zur besseren Anpassung an die Biegesteifigkeit von Knochen und zur Beseitigung der Patientenexposition gegenüber Metallionen verwendet. Häufig werden diese Materialien auch durch wirtschaftliche Formgebungsverfahren, z. B. spritzgießen oder Formpressen, vorteilhaft geformt. Wie bei Implantaten, die aus anderen Materialien aufgebaut sind, ist es wünschenswert, auf der Implantatoberfläche eine poröse Oberfläche herzustellen, damit Knochen einwachsen oder ein Ineinandergreifen mit Zement erfolgt. Normalerweise weist die poröse Oberfläche eine Drahtgeflecht- oder Perl- oder Grübchenoberfläche auf.
- Die US-Patentschriften 4978355, 4955911, 4813960 und die UK-Patentanmeldung GB 2216425 A lehren das Ausbilden einer porösen Oberfläche auf einem Kunststoffimplantat durch Eindrücken eines erwärmten Drahtgeflechts in den Körper des Kunststoffimplantats. Die US 4955911 und die GB 2216425 A lehren eine bleibende Massivschicht innerhalb des Drahtgeflechts, um zu verhindern, daß die äußeren Poren des Geflechts mit Kunststoff gefüllt werden. Die US 4778469 lehrt die Ausbildung einer texturierten Oberfläche auf einen Kunststoffimplantat durch Eindrücken einer erwärmten, texturierten Platte in die Implantatoberfläche und anschließendes Auflösen der Platte, um den Platteneindruck zurückzulassen. Schließlich lehrt die US 4479271 das Anformen eines Kunststoffimplantats an ein Drahtgeflecht.
- Eine Aufgabe der Erfindung besteht darin, ein Implantat bereitzustellen, das zur schnellen und wiederholbaren Herstellung geeignet ist.
- Eine weitere Aufgabe der Erfindung besteht darin, ein Implantat bereitzustellen, das gleichzeitig in eine Endform geformt und an einer porösen Oberfläche angebracht werden kann.
- Außerdem besteht eine Aufgabe der Erfindung darin, ein Implantat bereitzustellen, das eine starke Bindung zwischen einer porösen Oberfläche und dem Implantatkörper hat.
- Eine weitere Aufgabe der Erfindung besteht darin, ein Implantat bereitzustellen, das billig herzustellen ist.
- Ferner besteht eine Aufgabe der Erfindung darin, ein Implantat bereitzustellen, das sich leicht durch Spritzgießen herstellen läßt.
- Schließlich besteht eine Aufgabe der Erfindung darin, ein Verfahren zur Herstellung eines Implantats bereitzustellen, das billig und wiederholbar ist und eine starke Bindung zwischen einer porösen Oberfläche und dem Implantatkörper erzeugt.
- Die Erfindung sieht folglich ein Implantat mit einen Körper und einer an dem Körper angebrachten porösen Oberfläche vor, dadurch gekennzeichnet, daß der Korper in einer Verarbeitungsumgebung geformt wurde; die poröse Oberfläche ein poröser Einsatz ist, der an einer Oberfläche des Körpers angebracht ist; und dadurch, daß das Implantat ferner ein Füllstoffmaterial aufweist, das in dem porösen Material eingebettet ist, wobei das Füllstoffmaterial für eine vorbestimmte Zeitdauer stabil ist, wenn es der Verarbeitungsumgebung ausgesetzt wird, in der der Körper geformt wurde.
- Kurz gesagt sieht die Erfindung ein Implantat mit einem Kunststoffkörper und einer metallischen, porösen Oberfläche vor, die sicher am Körper befestigt ist. Das Implantat wird gefertigt, indem zunächst ein poröser Formen-Einsatz mit einer porösen Metallstruktur und ein lösliches Füllstoffmaterial, das einen Abschnitt seiner Poren füllt, hergestellt werden. Anschließend wird der Formen-Einsatz in eine Form gegeben, und Kunststoff wird in die Form eingespritzt, um den Implantatkörper zu formen. Danach wird das Implantat mit einem Lösungsmittel in Berührung gebracht, das das Füllstoffmaterial auflöst, um jenen Abschnitt des Einsatzes freizulegen, der gefüllt wurde. In einer bevorzugten Ausführungsform kann das Implantat einen Metallkern aufweisen, der vor dem Formen in die Form gegeben wird.
- Die genannten Aufgaben und Vorteile sowie weitere Aufgaben und Vorteile der Erfindung gehen aus der nachfolgenden näheren Beschreibung und den Zeichnungen hervor. Es zeigen:
- Fig. 1 eine Perspektivansicht eines Beispiels für ein erfindungsgemäßes Knochenimplantat;
- Fig. 2 eine Perspektivansicht eines Beispiels für eine poröse Unterlage, deren Poren teilweise gefüllt sind, um einen Formen-Einsatz zu bilden;
- Fig. 3 eine Detailansicht der porösen Unterlage von Fig. 2;
- Fig. 4 eine Perspektivansicht eines Beispiels für ein Implantat und eine Form in aufgelösten Einzelheiten; und
- Fig. 5 eine Perspektivansicht eines Beispiels für ein erfindungsgemäßes Knochenimplantat mit einer porösen Oberfläche und einem Füllstoffmaterial, das in der porösen Oberfläche eingebettet ist.
- Fig. 1 zeigt als Beispiel ein prothetisches Implantat als femoralen Hüftschaft. Der Hüftschaft 1 weist einen Körper 2 und eine poröse Oberfläche 3 auf. Zur Implantatherstellung wird zunächst eine poröse Unterlage 9 aus einem geeigneten Material durch eines von vielfältigen bekannten Verfahren gebildet. Vorzugsweise weist sie Metallfasern auf, z. B. Titan und seine Legierungen oder Cobalt-Chrom-Legierungen. Sie könnte auch aus Sintermetallperlen, Sinterkeramikstoffen oder einem anderen geeigneten porösen Material hergestellt sein. Als nächstes wird ein poröser Formen-Einsatz 4 gemäß Fig. 2 hergestellt, indem ein Abschnitt der porösen Unterlage mit einem Füllstoffmaterial 5 gefüllt wird, was deutlicher aus Fig. 3 hervorgeht. Vorzugsweise ist das Füllstoffmaterial ein amorphes Polymer, z. B. Polysulfon, aufgrund der Verarbeitungsfähigkeit amorpher Polymere und ihrer Löslichkeit in bestimmten Lösungsmitteln. Zu anderen Beispielen für geeignete amorphe Polymere gehören Polyethersulfon, Polycarbonat, Polyetherimid und Polymethylmethacrylat. Bestimmte wasserlösliche Polymere können auch verwendet werden, z. B. Polyethylenoxid. Eine bevorzugte Möglichkeit zum Einbringen des Füllstoffmaterials in das poröse Material besteht darin, die beiden Materialien zusammenzudrücken und dabei das poröse Material nahe der Glasumwandlungstemperatur des Füllstoffs zu erwärmen. Der Füllstoff tritt dann in das poröse Material kontrollierbar ein. Da diese amorphen Polymere sehr wärmestabil sind und eine Glasumwandlungsphase aufweisen, fließen sie nicht unkontrollierbar in das poröse Material, sondern extrudieren auf die gewünschte Tiefe. Wird z. B. ein 0,4 mm (0,01% Zoll) dikker Film aus Polysulfon vollständig in eine Unterlage gedrückt, die zu fünfzig Prozent porös ist, dringt er mit hohem Wiederholbarkeitsgrad bis in eine Tiefe von etwa 0,8 mm (0,032 Inch) ein. Der Füllstoff kann in das poröse Material eingebracht werden, bevor oder nachdem das poröse Material in seine Endform geformt wird. Im gezeigten Beispiel wird das poröse Material in eine gewünschte Form geschnitten, der Füllstoff wird darin eingebettet, um einen Einsatz zu bilden, und anschließend wird es (bedarfsweise) in seine Endform gebogen. Der Einsatz weist Löcher 10 auf, um seine Positionierung und Befestigung in einer Form zu unterstützen, z. B. durch Stifte, die von der Form vorragen und in die Löcher eingreifen.
- Anschließend wird der Formen-Einsatz mit der porösen Oberfläche in eine geeignete Formenhälfte 6 gegeben, wobei einige der ungefüllten Poren mit dem Formenhohlraum gemäß Fig. 4 in Verbindung stehen. Die gegenüberliegende Formenhälfte ist nicht gezeigt. Das Körpermaterial wird unter geeigneter Wärme und geeignetem Druck in die Form eingeleitet, um den Implantatkörper zu bilden und zu bewirken, daß es mit dem Einsatz ineinandergreift. Verschiedene Formgebungstechniken können zum Einsatz kommen, zu denen Spritzgießen, Formpressen und Gießen gehören. Vorzugsweise wird jedoch Spritzgießen verwendet. Das stabile Füllstoffmaterial verhindert, daß das Körpermaterial in die gefüllten Poren des Einsatzes eintritt, läßt es jedoch in die ungeschützten Poren eindringen, um eine sichere Bindung des Einsatzes am Körper herzustellen. Vorzugsweise ist das Körpermaterial ein halbkristallines Polymer, z. B. Polyetheretherketon. Halbkristalline Polymere sind wegen ihrer Festigkeit, Verarbeitungsfähigkeit und Unlöslichkeit in gewöhnlichen Lösungsmitteln wünschenswert. Zu anderen geeigneten halbkristallinen Polymeren gehören Polyaryletherketon und Polyphenylinsulfid. Vorteilhaft können Kohlenstoffasern oder andere verstärkende Mittel im Körpermaterial aufgenommen sein. Fig. 5 zeigt ein geformtes Implantat nach der Entnahme aus der Form. Abschließend wird das Füllstoffmaterial vom porösen Material entfernt. Im vorstehend näher beschriebenen bevorzugten Beispiel kann ein Lösungsmittel, z. B. Methylenchlorid, Methylethylketon oder Aceton den amorphen Polymerfüllstoff lösen, ohne das halbkristalline Polymer zu beeinträchtigen.
- Während vorstehend eine beispielhafte und bevorzugte Ausführungsform der Erfindung beschrieben wurde, sind Abwandlungen möglich. Beispielsweise könnten die Materialien und Verfahren zur Herstellung eines Implantats mit poröser Oberfläche verwendet werden, das für jeden Körperabschnitt geeignet ist. Außerdem könnten vor dem Formen des Körpers andere Strukturen in der Form aufgenommen werden. Zu solchen Strukturen gehören Massiv- und Verbundkerne, Fixierstifte und Hohleinsätze. Im gezeigten Beispiel ist ein Metallkern 8 zwischen zwei Einsätzen in die Form eingelegt. Der Kern hat einen Hals 11, einen Schaft 12 und einen Knopf 13. Der Hals und der Knopf greifen in die Form ein, um Abdichtungen zu bilden und zu verhindern, daß das eingespritzte Material ihre Oberflächen überzieht. Ferner kann der poröse Unterlageneinsatz an einem im wesentlichen vollständig geformten Implantat unter Verwendung des vorstehend beschriebenen Verfahrens angebracht werden, wobei der Einsatz teilweise gefüllt und ein fluides Material zwischen Einsatz und Implantat so eingeleitet wird, daß bei Erstarren des fluiden Materials das fluide Material eine Oberfläche des Körpers bildet, in dem der Einsatz eingebettet ist und der Füllstoff entfernt werden kann. Deutlich ist außerdem, daß die poröse Oberfläche vorteilhaft im wesentlichen den gesamten Körper wie im Beispiel abdecken oder für die spezifische Anwendung gegebenenfalls nur einen kleinen Abschnitt des Körpers überziehen kann. Schließlich können andere Materialien für den Körper und das Füllstoffmaterial verwendet werden, solange das Füllstoffmaterial irgendwie in das poröse Material eingebracht werden kann und das Füllstoffmaterial stabil ist, während es der Verarbeitungsumgebung ausgesetzt ist, in der der Körper geformt wird. Die Verarbeitungsumgebung wird durch solche Bedingungen wie Temperatur, Druck, chemisches Reaktionsvermögen und mechanische Kräfte bestimmt. Ferner sollte der Füllstoff ohne Beeinträchtigung des Körpers entfernbar sein. Beim Spritzgießen und Formpressen muß das Füllstoffmaterial bei einer Temperatur und einem Druck, bei der (dem) das Körpermaterial geformt wird, für jene Zeit stabil sein, in der diese Temperatur und dieser Druck beibehalten werden. Beim Gießen, z. B. mit Epoxidharz oder einem gelösten Material, kann der Füllstoff durch Verwendung eines geeigneten Lösungsmittels, durch Schmelzen oder anderweitig entfernt werden. In jedem dieser Verfahren kann das Füllstoffmaterial mechanisch sowie chemisch oder thermisch entfernt werden. Dem Fachmann wird deutlich sein, daß die vorgenannten Abwandlungen und vielfältige weitere an der bevorzugten Ausführungsform innerhalb des Schutzumfangs der Erfindung gemäß der Festlegung in den beigefügten Ansprüchen vorgenommen werden können.
Claims (19)
1. Implantat mit einem Körper (2) und einer an dem Körper
(2) angebrachten porösen Oberfläche (3), dadurch
gekennzeichnet, daß der Körper (2) in einer
Verarbeitungsumgebung geformt wurde; die poröse Oberfläche (3) ein
poröser Einsatz (4) ist, der an einer Oberfläche des Körpers
angebracht ist; und dadurch, daß das Implantat ferner
ein Füllstoffmaterial (5) aufweist, das in dem porösen
Material eingebettet ist, wobei das Füllstoffmaterial
(5) für eine vorbestimmte Zeitdauer stabil ist, wenn es
der Verarbeitungsumgebung ausgesetzt wird, in der der
Körper (2) geformt wurde.
2. Implantat nach Anspruch 1, dadurch gekennzeichnet, daß
das Füllstoffmaterial (5) von dem porösen Einsatz (4)
durch Mittel entfernbar ist, die die mechanischen
Eigenschaften des Körpers (2) nicht beeinträchtigen.
3. Implantat nach Anspruch 1 oder 2, dadurch
gekennzeichnet, daß das Füllstoffmaterial (5) in einem
Lösungsmittel stärker als der Körper (2) löslich ist.
4. Implantat nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet, daß der Implantatkörper (2)
Kunststoff aufweist und das in dem porösen Material
eingebettete Füllstoffmaterial (5) Kunststoff ist.
5. Implantat nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet, daß der Körper (2) aus einem
Material geformt ist, das spritzgießfähig ist, und der
Füllstoff (5) bei einer Temperatur und einem Druck stabil
ist, bei der (dem) das Körpermaterial spritzgießfähig
ist.
6. Implantat nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet, daß der Körper (2) ein
halbkristallines Polymer aufweist und das Füllstoffmaterial (5)
ein amorphes Polymer aufweist.
7. Implantat nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet, daß der Körper (2) ein Polymer
aufweist, das ausgewählt ist aus der Gruppe, die aus
Polyaryletherketon, Polyetheretherketon und
Polyphenylinsulfid besteht.
8. Implantat nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet, daß das Füllstoffmaterial (5) ein
Polymer aufweist, das ausgewählt ist aus der Gruppe, die
aus Polysulfon, Polyethersulfon, Polycarbonat und
Polymethylmethacrylat besteht.
9. Implantat nach einem der vorhergehenden Ansprüche,
dadurch gekennzeichnet, daß das poröse Material eine
Metallstruktur aufweist.
10. Implantat nach Anspruch 9, dadurch gekennzeichnet, daß
die Metallstruktur ein Metall aufweist, das ausgewählt
ist aus der Gruppe, die aus Titan, Titanlegierungen und
Cobalt-Chrom-Stahl besteht.
11. Implantat nach Anspruch 9, dadurch gekennzeichnet, daß
die Metallstruktur ein Element aufweist, das ausgewählt
ist aus der Gruppe, die aus Draht und Sinterperlen
besteht.
12. Verfahren zur Herstellung eines Implantats mit einem
Körper (2) und einer porösen Oberfläche (3), dadurch
gekennzeichnet, daß das Verfahren die folgenden Schritte
aufweist:
Positionieren eines porösen Einsatzes (4) in einer Form,
wobei der Einsatz (4) einen Abschnitt seiner Poren mit
einem Füllstoffmaterial (5) gefüllt hat, und
Formen des Körpers (2) des Implantats in der Form.
13. Verfahren nach Anspruch 12, dadurch gekennzeichnet, daß
das Verfahren ferner den Schritt des Entfernens des
Füllstoffmaterials (5) von dem Einsatz (4) aufweist,
nachdem der Körper (2) des Implantats geformt ist.
14. Verfahren nach Anspruch 13, dadurch gekennzeichnet, daß
das Füllstoffmaterial (5) auf eine Weise entfernt wird,
die ausgewählt ist aus der Gruppe, die aus Schmelzen,
Auflösen und mechanisches Herauslösen besteht.
15. Verfahren nach Anspruch 12 bis 14, dadurch
gekennzeichnet, daß der poröse Einsatz (4) gebildet wird durch
Einführen eines amorphen Polymers in einige der Poren eines
porösen Materials, gefolgt von Einspritzen eines
halbkristallinen Polymers in die Form, um den
Implantatkörper (2) zu bilden, und anschließendes Auflösen des
amorphen Polymers in einem Lösungsmittel.
16. Verfahren nach Anspruch 15, dadurch gekennzeichnet, daß
das amorphe Polymer ausgewählt ist aus der Gruppe, die
aus Polysulfon, Polyethersulfon, Polycarbonat und
Polymethylmethacrylat besteht.
17. Verfahren nach Anspruch 15 bis 16, dadurch
gekennzeichnet, daß das halbkristalline Polymer ausgewählt ist aus
der Gruppe, die aus Polyetheretherketon,
Polyaryletherketon und Polyphenylinsulfid besteht.
18. Verfahren nach Anspruch 15 bis 17, dadurch
gekennzeichnet, daß das Lösungsmittel ausgewählt ist aus der
Gruppe, die aus Methylenchlorid, Methylethylketon und Aceton
besteht.
19. Verfahren nach Anspruch 12 bis 18, dadurch
gekennzeichnet, daß das poröse Material erwärmt wird, während das
Füllstoffmaterial (5) in das poröse Material gedrückt
wird.
