Description
- Die vorliegende Erfindung betrifft eine bei Kühlschranktemperatur stabile wässrige Folinatlöslung.
- Folinsäure ist ein Metabolit der Folsäure und stellt die aktive Form dar, in welche die Folsäure im Körper umgesetzt wird. Die Umsetzung wird durch gewisse cytostatische Mittel, wie beispielsweise Methotrexat, inhibiert. Um diesem entgegen zu wirken, ist die Verabreichung von Folinsäure erforderlich, wobei die pharmazeutisch verabreichte Form der Folinsäure des Calciumfolinat ist.
- In der Regel wird Calciumfolinat durch Infusion oder Injection verabreicht, dessen Dosierungen immer grösser werden. Die Folge ist ein Bedarf nach konzentrierteren Lösungen. Calciumfolinat ist bei Raumtemperatur leicht löslich, aber nicht stabil und kann demzufolge nicht über eine ausreichend lange Zeit gelagert werden. Bei Kühlschranktemperatur, bei der es sehr viel stabiler ist, können höhere Konzentrationen nicht zubereitet werden. Bei der normalen Kühlschranktemperatur von 4 ºC kann eine Kristallisation bereits bei 15 mg/ml auftreten, wobei dieses bereits bei 12 mg/ml auftreten kann, wenn der Kühlschrank gelegentlich etwas kälter eingestellt wird und eine Temperature von etwa 0 ºC hat. In Übereinstiummung damit wird in dem Beitrag "Rote Liste", 1989, Editio Cantor, Aulendorf/Württ. ein Calciumfolinat-Produkt als Nr. 12022 diskutiert und ein Gehalt von 127 mg in 10 ml, d.h. 12,7 mg/ml offenbart. Diese Zusammensetzung enthält die aktive Verbindung in Kombination mit Natriumchlorid und Wasser. Dementsprechend besteht ein Bedarf nach einer wässrigen Folinatlösung, die bei Kühlschranktemperatur stabil ist, d.h. bei dieser Temperatur nicht kristallisiert.
- Es wurde festgestellt, dass die Lösung ohne eine Gefahr der Kristallisation im Kühlschrank sehr viel höhere Folinatkonzentrationen aufnehmen kann, wenn das Calcium komplex gebunden ist.
- Die Erfindung gewährt daher eine bei Kühlschranktemperatur stabile wässrige Folinatlösung, die Folinationen und komplex gebundene Calciumionen sowie möglicherweise freie Calciumionen in einer Gesamtmenge von mehr als 12 mg/ml und darüber hinaus Ionen eines Komplexbildners für Calcium enthält. Auf diese Weise lassen sich Lösungen zubereiten, die im Kühlschrank gelagert werden können und sogar 50 mg/ml Folinat enthalten. Wenn ihre Lagerung bei Raumtemperatur angestrebt wird, bei der selbstverständlich die Lagerungsdauer sehr viel kürzer ist, kann sogar ein Höchstwert von 100-120 mg/ml erreicht werden.
- Die Erfindung gewährt ebenfalls ein Verfahren zur Herstellung einer wässrigen Calciumfolinatlösung, die bei Kühlschranktemperatur stabil ist und eine Konzentration von über 12 mg/ml hat, wobei das Verfahren dadurch gekennzeichnet ist, dass in die Lösung ein Komplexbildner für Calcium einbezogen wird.
- Selbstverständlich wird die maximale Wirkung des Komplexbildners erzielt, wenn dieser in einer äquivalenten molaren Menge in Bezug auf das Calciumfolinat zugesetzt wird. An sich gibt es keinen Grund dagegen, mehr Komplexbildner zu verwenden unter der Voraussetzung, dass diese keine nachteiligen Nebenwirkungen ausübt, wobei jedoch von einer solchen grösseren Menge selbstverständlich keine Vorteile erzielt werden.
- Im Prinzip kann jeder Komplexbildner für Calcium verwendet werden, der für die therapeutische Verabreichung zulässig ist. Das Mittel, das bevorzugt ist und erster Linie in Betracht kommt, ist Ethylendiamintetraessigsäure (EDTE) in Form des Dinatrium- oder Tetranatriumsalzes. Das Ergebnis seiner Verwendung ist, dass die erhaltene Lösung Natriumionen enthält, komplex gebundene Calciumionen und möglicherweise freie Calciumionen als die Kationen, und als die Anionen Folinationen und Ethylendiamintetraessigsäureionen. Kurz gesagt, wird das Calcium damit ganz oder teilweise gegen Natrium ausgetauscht. Dieses hat keinen Einfluss auf die Aktivität, da die aktive Substanz die Folinsäure ist.
- Der geeignete pH-Bereich der Lösung ist der gleiche wie bei weniger konzentrierten Lösungen ohne Komplexbildner, d.h. im allgemeinen 4,0 ... 8,8, vorzugsweise 5,5 ... 8,8 und insbesondere 6-8. Ein Komplexbildner wie EDTE hat einen geringen oder keinen Einfluss auf die Stabilität der erhaltenen Lösung.
- Aus Gründen der Vollständigkeit ist zu bemerken, dass in der US-P-2,695,860 vom 30. November 1954 eine Stabilisierung von Folsäure oder ihre Derivate enthaltenen wässrigen Lösungen mit EDTE erwähnt wird. In diesem Patent wird Folinsäure in Anspruch 7 erwähnt, jedoch kein Calciumfolinat, und darüber hinaus sind nur sehr geringe Konzentrationen betroffen, wobei weiterhin nicht nur die aktive Verbindung eine andere ist, sondern auch das damit auftretende Problem. Wie in Spalte 1 dieses Patents diskutiert wurde, ist hier das Problem eine Stabilisierung gegenüber der Ausfällung von Verunreinigungen während der Lagerung bei Raumtemperatur, ein Problem, das bei Calciumfolinat nicht auftritt.
- Die folgenden, nicht beschränkenden Beispiele erläutern die Erfindung lediglich zur Veranschaulichung.
- Es wird eine Lösung zubereitet, welche pro ml die folgenden Bestandteile enthält:
- Wasserfreies Calciumfolinat 27,01 mg
- (entsprechend 25 mg Folsäure. 2H&sub2;O) 19,67 mg
- Wasser zur Injektion 1,0 ml
- Die erhaltene Lösung hatte einen pH-Wert von 7,8.
- Es wurde eine Lösung zubereitet, die pro ml folgendes enthielt:
- Wasserfreies Calciumfolinat 27,01 mg
- (entsprechend 25 mg Folinsäure) Dinatrium-EDTE-2H&sub2;O 5,08 mg
- Wasser zur Injektion 1,0 ml
- Die erhaltene Lösung hatte einen pH-Wert von 7,8.
- Die Lösungen beider Proben können durch Zusatz von NaCl isotonisch gemacht werden.
- Wie aus Beispiel II ersichtlich, liefert der Zusatz von Natrium-EDTE in einer Menge von etwa 25% der äquivalenten molaren Menge bereits eine Lösung, die wesentlich mehr als 20 mg/ml Folinat enthält. Wie experimentell nachgewiesen wurde, kristallisiert diese Lösung im Kühlschrank nicht. Auf Wunsch können wesentlich grössere Mengen Folinat in die Lösung nach Beispiel I einbezogen werden, als es in diesem Beispiel der Fall war. Das erwähnte Experiment wurde in erster Linie für Orientierungszwecke durchgeführt.
Claims (6)
1. Bei Kühlschranktemperatur stabile wässrige Folinatlösung, die
Folinationen und komplex gebundene Calciumionen und
möglicherweise freie Calciumionen in einer Gesamtmenge von mehr
als 12 mg/ml und darüber hinaus Anionen eines Komplexbildners für
Calcium enthält.
2. Lösung nach Anspruch 1, dadurch gekennzeichnet, dass die Anionen
des Komplexbildners in Bezug auf die Folinationen in einer
äquivalenten molaren Menge vorliegen.
3. Lösung nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die
Anionen des Komplexbildners Ethylendiamintetraacetationen sind.
4. Verfahren zur Herstellung einer bei Kühlschranktemperatur stabilen
wässrigen Calciumfolinatlösung mit einer Konzentration von mehr als
12 mg/ml, gekennzeichnet durch die Einbeziehung eines
Komplexbildners für Calcium in die Lösung.
5. Verfahren nach Anspruch 4, gekennzeichnet durch die Verwendung
des Komplexbildners in einer in Bezug auf das Calciumfolinat
aquivalenten molaren Menge.
6. Verfahren nach Anspruch 4 oder 5, dadurch gekennzeichnet, dass ein
Natriumsalz einer Ethylendiamintetraessigsäure als der
Komplexbildner gewählt wird.
