Description
Die Erfindung bezieht sich auf eine Schulterendoprothese mit einem
Humerus-Teil, das einen im Humerus verankerbaren Stiel, einen Kragen
am proximalen Ende des Stiels, und einen kugelabschnittsförmigen
Gelenkkopf auf dem Kragen enthält, und auf eine Schulterendprothese mit
einem Glenoid-Teil mit einem in der Scapula verankerbaren Befestigungs
element, einem Plateau an einem Ende des Befestigungselementes, und
einem Kunststoffinlay auf dem Plateau zur Aufnahme des Gelenkkopfes.
Eine solche Schulterendoprothese ist beispielsweise aus der
EP 0 329 854 A1 bekannt. Bei dieser bekannten Prothese ist am proxi
malen Ende des Stiels ein Kragen angeordnet, der den Stiel teilweise
radial überragt und zur Auflage auf der Resektionskante des Knochens
dient.
Aus der FR 2 685 633 A1 ist ein Humerus-Teil einer Schulterendoprothese
bekannt, das ein Koppelelement zwischen Gelenkkopf und Stiel enthält, von
dem ein Kragen radial abzweigt. Das Koppelelement besitzt zwei einander
gegenüberliegende Außenkonen, die jeweils mit einer Konusausnehmung an der
Unterseite des Gelenkkopfes beziehungsweise am proximalen Ende des Stiels
koppelbar sind. Die Rotationsachsen der Außenkonen des Koppelelementes
schließen einen vorgegebenen Winkel ein, so daß intraoperativ ein gewünsch
ter Antetorsions- bzw. Retrotorsionswinkel des Gelenkkopfes eingestellt
werden kann.
Aus der DE-42 20 271 A1 ist ein Schultergelenkkopf mit einer eine kalotten
förmige Außenfläche aufweisende Kappe bekannt, deren Innenfläche eine of
fenzellige oder offenporige Oberflächenstruktur aufweist.
Weiterhin ist aus der DE-31 06 917 C2 ist ein Verfahren zur Herstellung
eines Implantates mit offenen Poren oder Zellen bekannt, in die Knochenge
webe einwachsen kann, um eine sichere zementfreie Fixation des Implantats
zu gewährleisten.
Nachteilig bei diesen bekannten Prothesen hat sich die im Verhältnis
zu dem sehr geringen zur Verfügung stehenden Raum relativ große Bau
höhe des Gelenkkopf-Abschnittes und der damit verbundenen Verlage
rung des physiologischen Gelenkmittelpunktes erwiesen. Außerdem ist
die dauerhafte, insbesondere zementfreie Verankerung der Schulteren
doprothese im Humerus bzw. in der Scapula nicht immer zufrieden
stellend.
Aufgabe der vorliegenden Erfindung ist es daher, die Funktion einer
Schulterendoprothese der eingangs genannten Art derart weiterzubil
den, daß eine zuverlässige Fixation der Prothesenteile und eine zu
verlässige Funktion der Prothese gewährleistet ist.
Diese Aufgabe wird bei einer Schulterendoprothese der eingangs ge
nannten Art erfindungsgemäß dadurch gelöst, daß der Kragen im we
sentlichen kreisscheibenförmig ausgebildet ist und besitzt und mindestens ab
schnittsweise eine offenzellige oder offenporige Oberflächenstruktur
an der Unterseite aufweist, und daß sich der Gelenkkopf radial von
seiner Rotationsachse mindestens bis zum Umfangsrand des Kragens
erstreckt.
Die Vorteile der Erfindung bestehen insbesondere darin, daß der Kra
gen durch seine im wesentlichen kreisförmige Gestalt auf der gesam
ten Resektionskante aufliegen kann und in seine offenzellige oder
offenporige Oberflächenstruktur an der Unterseite spongiöser Knochen
einwachsen kann. Hierdurch wird die Langzeitfixation des Humerus-
Teils im Humerus erbeblich verbessert und die Gefahr einer Luxation
entsprechend verringert. Dadurch, daß sich der Gelenkkopf radial von
seiner Rotationsachse mindestens bis zum Umfangsrand des Kragens er
streckt, wird ein Gelenkkopf realisiert, der sich zugleich durch
eine geringe Bauhöhe und eine große Gelenkfläche auszeichnet. Hier
durch wird eine zuverlässige Funktion der Prothese gewährleistet.
Darüber hinaus bleibt durch die niedrige Bauhöhe auch der physiolo
gische Gelenkdrehpunkt erhalten.
Diese Aufgabe wird bei einer Schulterendoprothese der eingangs ge
nannten Art ferner durch ein Glenoid-Teil gelöst, dessen Plateau an
der dem Knochen zugewandten Seite mindestens abschnittsweise eine
offenzellige oder offenporige Oberflächenstruktur aufweist.
Hierdurch wird eine verbesserte Verankerung des Glenoid-Teils er
reicht. Nach einer operativen Primärfixation kann spongiöses Kno
chengewebe in die Zellen oder Poren der Oberflächenstruktur einwach
sen, wodurch sich ein besonders inniger Verbund zwischen Knochen und
Prothese ergibt.
Besonders bevorzugt erstreckt sich der Gelenkkopf radial von seiner
Rotationsachse bis zum Umfangsrand des Kragens und grenzt axial un
mittelbar an den Umfangsrand des Kragens an. Auf diese Weise wird
erreicht, daß der Umfangsrand des Kragens bei entsprechender Forman
passung zusammen mit dem Gelenkkopf eine Kugelkalotte bilden kann,
die insgesamt als Gelenkfläche zur Verfügung steht. Der für den Ge
lenkkopf nur in beschränktem Maß zur Verfügung stehende Raum wird
auf diese Weise optimal genutzt. Die wirksame Gelenkoberfläche er
streckt sich bis zur Resektionskante des Humerus.
Alternativ hierzu überragt bei einer anderen besonders bevorzugten
Ausführungsform der Erfindung der Gelenkkopf den Umfangsrand des
Kragens auch axial. Auch bei dieser Ausführungsform entstehen keine
nennenswerten Zwischenräume zwischen Resektionskante und Gelenkkopf.
Der Gelenkkopf erstreckt sich axial und radial über den Kragen und
liegt mit seiner Abschnittskante der Resektionskante des Humerus
unmittelbar gegenüber. Der zur Verfügung stehende Raum im Schulter
bereich wird hierdurch besonders gut für den Gelenkkopf genutzt. Die
große Gelenkfläche gewährleistet eine zuverlässige Funktion der
Prothese.
Besonders bevorzugt ist der Gelenkkopf auf dem Kragen lösbar befe
stigt, um bei einer entsprechenden Bevorratung von verschiedenen
Gelenkkopfgrößen intraoperativ jeweils einen geeigneten Gelenkkopf
für den Patienten auswählen zu können. Als vorteilhafte Verbindung
zwischen Gelenkkopf und Kragen haben sich Steckkonus-Verbindungen
erwiesen. Steckkonus-Verbindungen sind selbsthemmend und stellen
deshalb besonders einfache Verbindungsmittel dar.
Es hat sich darüber hinaus als vorteilhaft erwiesen, die Konusaus
nehmung an der Unterseite des Gelenkkopfes vorzunehmen und einen
Steckkonus auf dem Kragen anzuordnen, um die Bauhöhe des Humerus-
Teils insgesamt gering zu halten.
Besonders bevorzugt wird zwischen dem Gelenkkopf und dem Kragen ein
Koppelelement angeordnet, das mit einem Innenkonus auf den Steckko
nus auf dem Kragen aufsteckbar ist, und das einen Außenkonus ent
hält, der in die Konusausnehmung an der Unterseite des Gelenkkopfes
einsetzbar ist. Die Vorteile dieser besonders bevorzugten Ausfüh
rungsform liegen insbesondere darin, daß bei einer entsprechenden
Bevorratung während der Operation nicht nur die Gelenkkopfgröße,
sondern auch der Abstand zwischen Plateau und Gelenkkopf wählbar
ist. Bevorzugt schließen die Rotationsachsen des Innenkonus und des
Außenkonus des Koppelelementes einen vorgegebenen Winkel zueinander
ein. Dadurch kann der Antetorsions- bzw. Retrotorsionswinkel des
Gelenkkopfes entsprechend den Patientenerfordernissen gewählt und
intraoperativ eingestellt werden.
Besonders bevorzugt enthält das Humerus-Teil der Schulterendoprothe
se mindestens zwei Flügel, die jeweils um 90° versetzt radial vom
Stiel abzweigen und an der Unterseite des Kragens enden. Vorteilhaf
terweise sind die Flügel nach medial gerichtet. Denkbar ist auch
eine Ausführungsform mit vier Flügeln, von denen zwei nach dorsal
und zwei nach medial gerichtet sind. Besonders bevorzugt weisen die
Flügel mindestens abschnittsweise eine offenzellige oder offenporige
Oberflächenstruktur auf. Durch die Flügel können quer zum Humerus
auf den Gelenkkopf wirkenden Kräfte aufgenommen werden und die
Befestigung des Humerusteils im Humerus insgesamt verbessert werden.
Durch das Einwachsen von spongiösem Material in die offenzellige
oder offenporige Oberflächenstruktur der Flügel entsteht ein beson
ders inniger Knochen-/Prothesenverbund.
Besonders bevorzugt verläuft durch den Kragen und den proximalen Ab
schnitt des Stiels ein Durchgangsloch, durch das eine Knochenschrau
be zur Verankerung des Humerusteils im Humerus gesetzt werden kann.
Die Rotationsachsen des Gelenkkopfes und des Durchgangsloches fallen
dabei vorteilhafterweise zusammen. Hierdurch wird es dem Operateur
ermöglicht, in das Durchgangsloch im Kragen eine Knochenschraube
einzuführen, die lateral am Stiel austritt und im Spongiosa-Gewebe
des Humerus verankerbar ist, um eine ausreichende Primärfixation des
Humerusteils im Humerus zu verwirklichen.
Vorteilhafterweise besitzt der Stiel im wesentlichen kreisrunde
Querschnittsflächen und verjüngt sich zu seinem distalen Ende hin,
um sich den anatomischen Gegebenheiten des Humerus anzupassen.
Das Plateau des Glenoid-Teils ist vorteilhafterweise rotationssym
metrisch ausgestaltet und weist in seiner Mitte ein Durchgangsloch
zum Setzen einer Knochenschraube auf, mit dem das Plateau im Scapula
verankerbar ist.
Darüber hinaus enthält das Befestigungselement des Glenoid-Teils
einen im Spongiosa-Gewebe des Scapula verankerbaren Stiel, in dessen
Mitte das Durchgangsloch angeordnet ist. Hierdurch wird eine beson
ders gute Primärfixation des Glenoid-Teils in der Scapula erreicht.
Vorteilhafterweise enthält das Befestigungselement des Glenoid-Teils
mindestens einen Flügel mit offenzelliger oder offenporiger Ober
flächenstruktur. Der Flügel dient insbesondere der besseren Aufnahme
von Querkräften und begünstigt das Hineinwachsen des spongiösen
Gewebes in die Zellen oder Poren der Oberflächenstruktur.
Besonders bevorzugt ist zwischen Kunststoffende und Plateau eine
Schnappverbindung vorgesehen. Gemäß einer bevorzugten Ausgestaltung
der Erfindung enthält das Plateau einen umlaufenden Randflansch, der
das Inlay randseitig umschließt, wobei an der Innenseite des Rand
flansches und im Umfangsrand des Inlays miteinander korrespondieren
de Anformungen bzw. Ausnehmungen zur lösbaren Befestigung des Inlays
im Plateau vorgesehen sind. Hierdurch wird einerseits die sichere
Befestigung des Kunststoffinlays auf dem Plateau gewährleistet und
andererseits dem Operateur die Möglichkeit eröffnet, nach der Be
festigung des Plateaus in der Scapula ein für den Gelenkkopf passen
des Kunststoffinlay auszuwählen und anschließend auf dem Plateau zu
befestigen.
Vorteilhafterweise weist das Kunststoffinlay im cranialen Bereich
eine vergrößerte Dicke auf, um eine Luxationsgefahr des Gelenkes zu
verringern.
Im folgenden werden Ausführungsbeispiele der Erfindung anhand der
Zeichnung beschrieben. Es zeigen im einzelnen:

Die Fig. 1 und Fig. 2 zeigen ein Humerus-Teil 1 einer Schulteren
doprothese mit einem kugelabschnittsförmigen Gelenkkopf 2, das mit
einem Stiel 4 im Humerus 16 verankert ist.
Vom Stiel 4 zweigen zwei Flügel 8 jeweils 90° versetzt radial in me
dialer Richtung ab, die sich nach außen verjüngen. Die Flügel 8 und
der Stiel 4 sind in Fig. 3 auch im Querschnitt dargestellt. Die
Flügel 8 dienen der besseren Aufnahme von Querkräften und besitzen
zur besseren Fixation mit dem Knochen eine offenzellige oder offen
porige Oberflächenstruktur 7, in die Knochenmaterial einwachsen
kann. Die radiale Erstreckung der Flügel 8 nimmt entsprechend den
anantomischen Gegebenheiten des menschlichen Humerus 16 von distal
nach proximal zu.
Am proximalen Ende des Stieles 4 ist ein Kragen 5 angeordnet, der im
wesentlichen kreisscheibenförmig ausgebildet ist. Der Kragen 5 liegt
mit seiner distalen Unterseite 6 auf der Resektionskante 18 des
Humerus 16 auf. An der Unterseite 6 des Kragens 5 ist eine offenzel
lige oder offenporige Oberflächenstruktur 7 vorgesehen, in die
spongiöses Gewebe zur Verbesserung der Fixation des Humerusteils 1
mit dem Humerus 16 einwachsen kann. Auf dem Kragen 5 ist ein Steck
konus 10 angeordnet, auf dem der Gelenkkopf 2 mit einer hierzu
korrespondierenden Konusausnehmung 11 an seiner distalen Unterseite
aufsteckbar ist. Durch den Steckkonus 10, den Kragen 5 und den Stiel
4 verläuft ein Durchgangsloch 12 zur Aufnahme einer Knochenschraube
17, dessen Rotationsachse im wesentlichen mit dem des Steckkonus 10
zusammenfällt. Das Durchgangsloch 12 ist im Steckkonus 10 aufgewei
tet, um den Kopf der Knochenschraube 17 aufnehmen zu können. Die
Knochenschraube 17 wird intraoperativ vor dem Aufstecken des Gelenk
kopfes 2 in das Durchgangsloch 12 von oben eingeführt, tritt an der
lateralen Seite des Stieles 4 aus und wird in das Spongiosa-Gewebe
des Humerus 16 eingeschraubt.
Der kugelabschnittsförmige Gelenkkopf 2 besitzt an seiner Unterseite
eine dem Kragen 5 entsprechende Ausnehmung und überragt in der in
Fig. 1 dargestellten Ausführungsform den Kragen 5 sowohl radial als
auch axial. Auf diese Weise erstreckt sich die Oberfläche des Ge
lenkkopfes 2 bis nahe an die Resektionskante 18 des Humerus 16.
Trotz der großen Gelenkfläche ist die Bauhöhe des Humerusteils 1
oberhalb der Resektionskante 18 gering, der physiologische Gelenk
mittelpunkt bleibt somit erhalten.
Dieselben Vorteile lassen sich auch mit der in Fig. 2 dargestellten
Ausführungsform erreichen, in der sich der Gelenkkopf 2 radial von
seiner Rotationsachse bis zum Umfangsrand des Kragens 5 erstreckt
und unmittelbar an den Umfangsrand des Kragens 5 angrenzt. Der Um
fangsrand des Kragens 5 ist in seiner Form an die Oberfläche des
Gelenkkopfes 2 angepaßt und bildet zusammen mit dem Gelenkkopf eine
Kugelkalotte, die insgesamt als Gelenkfläche zur Verfügung steht.
Durch die Konussteckverbindung zwischen Plateau 5 und Gelenkkopf 2
ist es intraoperativ möglich, nach der Befestigung des Stiels 4 in
dem resizierten Humerus 16 einen Gelenkkopf 2 auszuwählen, der be
züglich seiner Größe geeignet erscheint. Darüber hinaus kann zwi
schen Gelenkkopf 2 und Plateau 5 auch ein separates Koppelelement 13
angeordnet werden, welches in Fig. 4 im Querschnitt dargestellt
ist. Das Koppelelement 13 besitzt einen Innenkonus 14 und einen
Außenkonus 15, die jeweils mit dem Steckkonus 10 bzw. der Konusaus
dehnung 11 korrespondieren. Die Rotationsachsen 14a und 15a des
Innenkonus 14 bzw. des Außenkonus 15 des Koppelelementes 13 schlie
ßen einen vorgegebenen Winkel α zueinander ein, der im Bedarfsfall
auch 0° sein kann. Durch Auswahl eines geeigneten Koppelelementes
kann neben den Ante- bzw. Retrotorsionswinkel auch der Abstand zwi
schen Plateau 5 und Gelenkkopf 2 eingestellt werden.
In Fig. 5 ist ein Glenoid-Teil 20 der Schulterendoprothese im Quer
schnitt dargestellt. Das Glenoid-Teil 20 besitzt ein metallisches
Plateau 22 mit einem umlaufenden Randflansch 21, der ein Inlay 30
randseitig umschließt. In dem dargestellten Ausführungsbeispiel sind
zur lösbaren Befestigung des Kunststoffinlays 30 am Plateau 22 zwi
schen Randflansch 21 und Kunststoffinlay 30 miteinander korrespon
dierende Anformungen 21a bzw. Ausnehmungen 31a vorgesehen. Hierdurch
wird einerseits eine sichere Befestigung des Kunststoffinlays 30 auf
dem Plateau 22 gewährleistet und andererseits wird es dem Operateur
ermöglicht, zu einem ausgewählten Gelenkkopf 2 jeweils ein geeigne
tes Kunststoffinlay 30 zu verwenden.
Wie insbesondere in Fig. 6 zu erkennen ist, nimmt die Materialstär
ke des Kunststoffinlays 30 zum cranialen Bereich zu, um eine Ver
schiebung des Gelenkkopfes 2 nach cranial und damit eine Luxation
des Gelenkes zu verhindern.
An der Unterseite des Plateaus 22 ist ein Stiel 29 angeordnet, der
zur Verankerung im Spongiolagewebe der Scapula vorgesehen ist. Durch
das Plateau 22 und den Stiel 29 hindurch verläuft ein Durchgangsloch
25, durch das eine Knochenschraube 27 in Richtung des Scapulakno
chens hindurchreicht. An der Unterseite des Plateaus 22 ist eine
offenzellige oder offenporige Oberflächenstruktur 7 vorgesehen, in
die Knochenmaterial einwachsen kann, um die Befestigung des Glenoid-
Teils 20 am Schulterknochen zusätzlich zu verbessern. Hierzu sind
auch an der dem Knochen zugewandten Seite senkrecht vom Plateau 22
abzweigende Flügel 33 angeordnet, die eine offenzellige oder offen
porige Oberflächenstruktur 7 aufweisen. Die Flügel 33 dienen der
besseren Aufnahme von Querkräften und vergrößern gleichzeitig die
Oberfläche, in die spongiöses Knochenmaterial einwachsen kann.
Claims (30)
1. Schulterendoprothese mit einem Humerusteil, das einen im
Humerus verankerbaren Stiel, einen Kragen am proximalen Ende des
Stiels und einen kugelabschnittsförmigen Gelenkkopf auf dem Kragen
enthält,
dadurch gekennzeichnet, daß der Kragen (5) im wesentlichen kreis
scheibenförmig ausgebildet ist und mindestens abschnittsweise eine
offenzellige oder offenporige Oberflächenstruktur (7) an der Unter
seite (6) aufweist, und daß sich der Gelenkkopf (2) radial von
seiner Rotationsachse (3) mindestens bis zum Umfangsrand des Kragens
(5) erstreckt.
2. Schulterendoprothese nach Anspruch 1,
dadurch gekennzeichnet, daß sich der Gelenkkopf (2) radial von sei
ner Rotationsachse (3) bis zum Umfangsrand des Kragens (5) er
streckt.
3. Schulterendoprothese nach Anspruch 2,
dadurch gekennzeichnet, daß der Umfangsrand des Kragens (5) axial
unmittelbar an den Gelenkkopf (2) angrenzt.
4. Schulterendoprothese nach Anspruch 3,
dadurch gekennzeichnet, daß der Umfangsrand des Kragens (5) an die
Oberfläche des Gelenkkopfes (2) so angepaßt ist, daß Gelenkkopf (2)
und Kragen (5) eine Kugelkalotte bilden.
5. Schulterendoprothese nach Anspruch 1,
dadurch gekennzeichnet, daß der Gelenkkopf (2) den Umfangsrand des
Kragens (5) axial überragt.
6. Schulterendoprothese nach einem der vorstehenden Ansprüche,
dadurch gekennzeichnet, daß der Gelenkkopf (2) auf dem Kragen (5)
lösbar befestigt ist.
7. Schulterendoprothese nach einem der vorstehenden Ansprüche,
gekennzeichnet durch eine Steckkonus-Verbindung (10, 11) zwischen
dem Gelenkkopf (2) und dem Kragen (5).
8. Schulterendoprothese nach Anspruch 7,
gekennzeichnet durch eine Konusausnehmung (11) an der Unterseite des
Gelenkkopfes (2) und einen Steckkonus (10) auf dem Kragen (5).
9. Schulterendoprothese nach einem der vorstehenden Ansprüche,
gekennzeichnet durch ein Koppelelement (13) zwischen Gelenkkopf (2)
und Kragen (5) mit einem Innenkonus (14) und einem Außenkonus (15),
daß der Innenkonus (14) mit dem Steckkonus (10) und der Außenkonus
(15) mit der Konusausnehmung (11) koppelbar ist.
10. Schulterendoprothese nach einem der vorstehenden Ansprüche,
dadurch gekennzeichnet, daß die Rotationsachse (3) des Gelenkkopfes
(2) in vorgegebenem Winkel zur Unterseite (6) des Kragens (5)
verläuft.
11. Schulterendoprothese nach Anspruch 9 oder 10,
dadurch gekennzeichnet, daß die Rotationsachsen vom Innenkonus (14)
und Außenkonus (15) des Koppelelementes (13) einen vorgegebenen
Winkel α zueinander einschließen.
12. Schulterendoprothese nach einem der vorstehenden Ansprüche,
dadurch gekennzeichnet, daß der Gelenkkopf (2) an der Unterseite
eine dem Kragen (5) entsprechende Ausnehmung enthält.
13. Schulterendoprothese nach einem der vorstehenden Ansprüche,
gekennzeichnet durch mindestens zwei radial vom Stiel (4) abzweigen
de Flügel (8), die sich bis zur Unterseite (6) des Kragens (5)
erstrecken.
14. Schulterendoprothese nach Anspruch 13,
dadurch gekennzeichnet, daß die Flügel (8) mindestens abschnitts
weise eine offenzellige oder offenporige Oberflächenstruktur (7)
aufweisen.
15. Schulterendoprothese nach einem der Ansprüche 13 oder 14,
dadurch gekennzeichnet, daß mindestens zwei Flügel (8) nach medial
gerichtet sind.
16. Schulterendoprothese nach einem der vorstehenden Ansprüche,
gekennzeichnet durch vier Flügel (8), die jeweils um 90° versetzt
radial vom Stiel (2) abzweigen.
17. Schulterendoprothese nach einem der vorstehenden Ansprüche,
gekennzeichnet durch ein durch den Kragen (5) und den Stiel (4)
hindurch verlaufendes Durchgangsloch (12), durch das eine Knochen
schraube (17) zur Verankerung des Humerusteils (1) im Humerus
gesetzt werden kann.
18. Schulterendoprothese nach Anspruch 17,
dadurch gekennzeichnet, daß die Rotationsachsen des Gelenkkopfes (2)
und des Durchgangsloches (12) übereinstimmen.
19. Schulterendoprothese nach einem der vorstehenden Ansprüche,
dadurch gekennzeichnet, daß der Stiel (4) im wesentlichen kreisrunde
Querschnittsflächen aufweist.
20. Schulterendoprothese nach einem der vorstehenden Ansprüche,
dadurch gekennzeichnet, daß der Stiel (4) sich zu seinem distalen
Ende hin verjüngt.
21. Schulterendoprothese mit einem Glenoidteil mit einem in der
Scapula verankerbaren Befestigungselement, einem Plateau an einem
Ende des Befestigungselementes, und einem Kunststoffinlay auf dem
Plateau zur Aufnahme des Gelenkkopfes, insbesondere nach einem der
vorstehenden Ansprüche,
dadurch gekennzeichnet, daß das Plateau (22) an der dem Knochen zu
gewandten Seite mindestens abschnittsweise eine offenzellige oder
offenporige Oberflächenstruktur (7) aufweist.
22. Schulterendoprothese nach Anspruch 21,
dadurch gekennzeichnet, daß das Plateau (22) des Glenoidteils (20)
rotationssymmetrisch ausgebildet ist.
23. Schulterendoprothese nach einem der Ansprüche 21 oder 22,
dadurch gekennzeichnet, daß das Plateau (22) ein Durchgangsloch (25)
zum Setzen einer Knochenschraube (27) aufweist.
24. Schulterendoprothese nach einem der Ansprüche 21 bis 23,
dadurch gekennzeichnet, daß das Befestigungselement einen im Spon
giosagewebe der Scapula verankerbaren Stiel (29) enthält.
25. Schulterendoprothese nach Anspruch 24,
dadurch gekennzeichnet, daß das Durchgangsloch (25) durch den Stiel
(29) hindurch verläuft.
26. Schulterendoprothese nach einem der Ansprüche 21 bis 25,
dadurch gekennzeichnet, daß das Befestigungselement des Glenoidteils
(20) mindestens einen Flügel (33) enthält.
27. Schulterendoprothese nach Anspruch 26,
dadurch gekennzeichnet, daß der Flügel (33) mindestens abschnitts
weise eine offenzellige oder offenporige Oberflächenstruktur (7)
aufweist.
28. Schulterendoprothese nach einem der Ansprüche 21 bis 27,
gekennzeichnet durch eine Schnappverbindung zwischen Kunstoffinlay
(30) und Plateau (22).
29. Schulterendoprothese nach einem der Ansprüche 21 bis 28,
dadurch gekennzeichnet, daß das Plateau (22) einen umlaufenden
Randflansch (21) enthält, der das Inlay (30) randseitig umschließt,
und daß an der Innenseite des Randflansches (21) und am Umfangsrand
(31) des Inlays (30) miteinander korrespondierende Anformungen (21a)
bzw. Ausnehmungen (31a) zur lösbaren Befestigung des Kunststoffin
lays (30) am Plateau (22) vorgesehen sind.
30. Schulterendoprothese nach einem der Ansprüche 21 bis 29,
dadurch gekennzeichnet, daß das Kunststoffinlay (30) im cranialen
Bereich eine vergrößerte Dicke aufweist.
