Abstract
Description
Gehörgangswand-Prothese und Ear canal wall prosthesis and
Process for their manufacture The invention relates to a prosthesis for the total or partial replacement of the posterior wall of the auditory canal according to patent (patent application P 31 17 025o0) and a method for their production, a prosthesis of the initially named type is used to reconstruct the posterior auditory canal tract, An essential prerequisite for a good fusing of bony tissue in vivo is the use of bioactive prosthesis material, in particular Bioglaskeramikadec Bio glass.
It is known that such bioactive bone replacement materials have a certain surface solubility, which is presumably one for the occurrence of Bone / implant composite is a necessary prerequisite. On the other hand, this special property can occur under particularly unfavorable local anatomical conditions Conditions such as those particularly encountered when a prosthesis is implanted in the soft tissue bed or when such a prosthesis comes into contact with soft tissue lead to the fact that the long-term stability of the implant is impaired. As the property the bioactivity due to the special chemical composition of the implant material is achieved, such a material for implantation in other tissues is not particularly advantageous. For example, it can be observed that the implantation of bioactive material triggers more sensitive reactions in soft tissue than one bio-inert material.
Prothasenteile anzugeben. The invention is therefore based on the object in the present Prosthesis those partial surface areas that are not for the actual bond are provided with the bone (implant / osseous hard tissue adhesion zone), to be trained in such a way that they are completely biochemically resistant and bioinactive. In addition, the task is to find a method for subsequent bioinactivation of selected sub-areas of inherently bioactive prostheses or prosthesis parts to specify.
In the case of a prosthesis of the type mentioned at the beginning, this is the task solved that they are also insoluble in at least one surface area bioinertes Contains material, it is also achieved in that the relevant surface subregions this prosthesis either with a targeted supply of substance a surface treatment to generate at least one additively applied or with targeted substance removal or mass transfer of a chemical aftertreatment to generate at least one subtractively preserved, permanently fixed, in vivo as a biochemical barrier layer active, bio-inert protective layer or conversion (leaching) layer further refinements are described in the other subclaims, In the drawings, exemplary embodiments of the invention are shown schematically, whereby the reference numbers - as far as possible - with those in the main patent ,,, (main patent application P 31 17 025o0) match, It shows: Fig, 1: a perspective representation a teilbeschi ended total auditory canal wall prosthesis according to the invention with additive Protective layer; FIG. 2 shows a section through that shown in FIG. 1 along the line A-A; 3 shows a representation corresponding to FIG. 1, but with additional additive coating of the two long sides of the cuff; Fig, 4: a section through what is shown in FIG. 3 along the line A-A; Fig, 50 that shown in Fig. 2, but with a subtractive layer; Fig0 6 * the one in Fig. 5 Shown, but with additional subtractive coating of both Long sides of the cuff Both - the additive and the subtractive - protective layers S (+) and S (-) have the same effect in terms of their function. You put in a sense biochemical corrosion protection layers, the complete transport and passage barriers for any substance (ion) exchange between the chemical components of the implant and ensure the components of the physiological-biochemical body fluids.
Auslaugungs-) und Austausch-Reaktionen nebeneinander ablaufen mit der Folge, daß es zunächst zu einer Verarmung und schließlich zu einer vollständigen Zerstörung (Umwandlung) einer Phase - insbesondere der kristallinen Komponente(n) - des glaskeramischen Verbundsystems kommt Zum chemischen Angriff können aber auch Basen und Puffersysteme verwendet werden, die in Abhängigkeit von ihrem konkreten Chemismus, ihrer Konzentration und ihrem pH-Wert gezielt auf bestimmte Phasenkomponenten des Bioglaskeramik-"Verbundsystems" einwirken und so die genvünschte Eigenschaftsänderung der Implantatoberfläche bewirken, Die nach dieser kombinierten chemischen Behandlung verbleibende Glaskeramik, die ihrer Apatitanteile beraubt ist, sei mit "Residual-Bioglaskeramik" bezeichnet, Sie ist bezüglich ihrer chemischen Eigenschaften unlöslich, porenfrei und stoppt jeglichen lonentransport; sie ist bezüglich ihrer biochemisch-physiologischen Wirkung bioinaktiv, also bio-"inert", und bezüglich ihrer mechanischen Eigenschaften abriebresistent und auf dem Prothesenkernmaterial festhaftend Im Bedarfsfall kann die so erhaltene Schicht S(-) zusätzlich auf thermischem Wege verdichtet bzw versiegelt werden Darüber hinaus ist auch eine Silanisierungsschicht zusätzlich aufbringbar, Aufstellung der verwendeten Bezugsziffern und -zeichen 1 - Manschette 4 - Längsseiten von (1) 9 - oberer Manschettenrand 10 - unterer Manschettenrand 11 - Innenwand von (1) 12 - Außenwand von (1) - additive Schutzschicht S(-) - subtraktive Schutzschicht L e e r s e i t eFor example, a subtractive protective layer S (.-) can be used in vitro can be obtained in that in one made of bioglass ceramic solid material or prosthesis made of bioglass material those surface areas that compulsory - i.e. due to anatomical mixer conditions - or Wanted by the otosurgeon - for example, when lining certain implant areas with epithelial tissue - come into permanent contact with soft tissue, chemically in the way are pretreated that aqueous acidic solutions and / or aqueous salt solutions in normalities between 0.001 and 0.1 the original bio-glass ceramic surface attack, with attack (dissolution or leaching) and exchange reactions run side by side with the result that it initially leads to impoverishment and finally to a complete destruction (transformation) of a phase - especially the crystalline one Component (s) - of the glass-ceramic composite system Comes to chemical attack but bases and buffer systems can also be used, which depend on Their specific chemistry, their concentration and their pH value are targeted towards certain Phase components of the bioglass ceramic "composite system" act and thus the desired Change the properties of the implant surface, which combined after this chemical treatment remaining glass-ceramic, which robs of its apatite components is, be referred to as "residual bioglass ceramic", it is with regard to its chemical Properties insoluble, pore-free and stops any ion transport; she is regarding their biochemical-physiological effect bioinactive, in other words bio- "inert" and wear-resistant with regard to their mechanical properties and adheres firmly to the prosthesis core material Layer S (-) can also be thermally compressed or sealed over it In addition, a silanization layer can also be applied, Lineup of the reference numbers and symbols used 1 - sleeve 4 - long sides of (1) 9 - upper cuff edge 10 - lower cuff edge 11 - inner wall of (1) 12 - outer wall of (1) - additive protective layer S (-) - subtractive protective layer L. e e r e i t e
