Description
Die Erfindung betrifft eine Vorrichtung der im Oberbegriff des Patentanspruchs 1 angegebenen Art.
Bei einer bekannten Vorrichtung dieser Art (DE-OS 43 781), die als flexibles Endoskop ausgebildet ist und
zum gleichzeitigen Betrachten der inneren Oberfläche eines Körperhohlraums und zur Durchführung einer
medizinischen Behandlung an dieser Oberfläche von außen dient, erstrecken sich die optischen Faserbündel als
gesonderte Beobachtungsteilbündel, Laserteilbündel und Beleuchtungsteilbündel über die gesamte Länge der
Hülse, und das Laserteilbündel steht über den Kopf vor. Es sind Flüssigkeitszufuhrkanäle vorgesehen, die dem
Zweck dienen, ein Fenster an dem Kopf mittels einer Wasch- oder Reinigungsflüssigkeit sauber zu halten
bzw. eine Flüssigkeit an die Arbeitsoberfläche zu richten. Der Kanal zum Absaugen von Bruchstücken ist am
Ende des Kopfes seitlich versetzt angeordnet, was den Nachteil hat, daß sich rund um den Brennpunkt des
Laserstrahls entstehende Verstopfungsbruchstücke nicht sofort absaugen lassen. Außerdem ist es bei der
bekannten Vorrichtung schwierig, den Kopf so an der Verstopfung zu halten, daß eine wirksame Befestigung
der Verstopfung durch die Laserstrahlung und die Abfuhr der entstehenden Bruchstücke möglich ist. Schließlich
ist es bei der bekannten Vorrichtung auch nicht auf einfache Weise möglich, genau den Zeitpunkt zu erkennen,
in welchem mit der Laserstrahlung die Verstopfung durchbohrt worden ist, so daß es zur Beschädigung von
hinter der Verstopfung befindlichen Teilen kommen kann.
Es ist zwar bereits ein flexibles endoskopisches Laserskalpell mit Beobachtungsoptik bekannt (DE-OS
21 06 470), bei dem zur Beobachtung, Bestrahlung und Beleuchtung von Operationsstellen zu einem Bündel zusammengefaßte
optische Fasern benutzt werden, bei diesem Laserskalpell ist jedoch die Entfernung von größeren
Bruchstücken nicht vorgesehen.
Aufgabe der Erfindung ist es, eine Vorrichtung der eingangs genannten Art so zu verbessern, daß sich bei
kompakterem Aufbau und einfacherer Handhabbarkeit Verstopfungen in Röhren auf einfachere Weise finden,
beobachten und entfernen lassen.
Diese Aufgabe ist durch die im Kennzeichen des Patentanspruchs 1 angegebenen Merkmale gelöst.
Bei der Vorrichtung nach der Erfindung wird durch die Zusammenfassung der optischen Faserbündel zu einem
einzigen Faserbündel zwischen der Hülse und dem zusammengefaßten Faserbündel ein Ringraum ermöglicht,
der zu der Saugöffnung führt, so daß am Kopfende der Hülse durch Laserstrahlung entstehende Bruchstükke
der Verstopfung abgesaugt werden können. Da der Ringraum rund um den Brennpunkt der Laserstrahlung
angeordnet ist, lassen sich die entstehenden Bruchstükke jofort absaugen. Außerdem ermöglicht der Ringraum,
daß sich der Kopf der Vorrichtung an der Verstopfung festsaugt, was eine wirksame Beseitigung der
Verstopfung durch die Laserstrahlung und eine wirksame \bfuhr der entstehenden Bruchstücke ermöglicht.
Mit dem durchsichtigen Behälter läßt sich genau der Zeitpunkt erkennen, in welchem die Laserstrahlung die
Verstopfung durchbohrt hat. Die Beendigung des Durchbohrungsvorganges der Verstopfung wird dadurch
angezeigt, daß in der Röhre hinter der Verstopfung vorhandene Flüssigkeit beim Absaugen auch in
den durchsichtigen Behälter gelangt. Die Vorrichtung nach der Erfindung hat einen sehr kompakten Aufbau,
so daß sie auch in enge Blutgefäße, in den Harnleiter oder in den Gallenleiter eingeführt werden kann, um
Gefäßpfropfen, Harnsteine bzw. Gallensteine zu finden, zu beobachten und zu entfernen. Außerdem läßt sich die
Vorrichtung ohne weiteres industriell anwenden, beispielsweise um eine Verstopfung in einer Luftröhre zu
entfernen, in welchem Fall das Ende des Entfernungsvorganges durch das Erscheinen von Luft in dem durch-
sichtigen Behälter angezeigt wird.
Es ist zwar bereits an sich bekannt, einen Behälter bei einer rektoskopischen Vorrichtung zu verwenden (US-PS
30 51 176), in diesem Fall dient der Behälter jedoch
ausschließlich zurr Auffangen von abgesaugten Flüssigkeiten.
In der Ausgestaltung der Erfindung nach Anspruch 2 läßt sich mittels des Absperrorgans auf einfache Weise
der Durchfluß zwischen dem Ringkanal einerseits und der Saugöffnung bzw. dem Flüssigkeitszufuhranschluß
andererseits umschalten, um die Zufuhr einer Flüssigkeit, z. B. einer Kontrastsubstanz, abzusperren, wenn
der Entfernungsvorgang beginnt, bzw. um den Saugvorgang während des Findens und vor dem Entfernen einer
Verstopfung zu unterbrechen. Wenn mittels des Ab· sperrorgans die Flüssigkeitszufuhr abgesperrt ist. kann
während des Entfernungsvorganges die ungefährliche Abfuhr der Bruchstücke der Verstopfung über die Saugöffnung
erfolgen.
Ausführungsbeispiele der Erfindung werden im folgenden unter Bezugnahme auf die Zeichnung näher beschrieben. Es zeigt

Die F i g. 1 und 2 zeigen eine flexible Hülse 10 mit einem Vorderende 10ö und einem Hinterende 10a, an
dem eine Saugöffnung 12 vorgesehen ist Die Saugöffnung
12 ist mit einer nicht dargestellten Unterdruckquelle verbindbar, mit deren Hilfe eine Verstopfung 14
entfernbar ist An dem Vorderende 106 der Hülse 10 ist ein optisches Faserbündel 16 angeordnet, das mit einem
Kopf 18 der Hülse 10 verbunden ist, der eine öffnung 20 aufweist Zwischen dem Faserbündel 16 und der Hülse
10 ist ein Ringkanal gebildet Drei Faserteilbündel sind zu dem Faserbündel 16 zusammengefaßt, nämlich ein
Beobachtungsteilbündel 22, ein Beleuchtungsteilbündel 24 und ein Laserteilbündel 26, welche mit einem Okular,
einer Lichtquelle (beide nicht dargestellt) bzw. einem Laser 28 verbunden sind. Das Laserteilbündel 26 könnte
auch zur Beleuchtung dienen, wenn der Laser mit kleiner Intensität betrieben wird, wodurch sich die Verwendung
des gesonderten Beleuchtungsteilbündels 24 erübrigen würde.
Nahe dem Hinterende 10a der Hülse 10 ist ein durchsichtiger Behälter 30 für die Anzeige des Endes des
Entfernungsvorganges der Verstopfung 14 angeordnet. Zwischen dem Hinterende 10a und dem Vorderende
10b der Hülse 10 befindet sich ein Flüssigkeitszufuhranschluß 32, über den eine Salzlösung oder ein strahlungsundurchlässiges
Material, das als Kontrastsubstanz dient, zuführbar ist. Die Kontrastsubstanz erleichtert
das Finden der Verstopfung.
Ein hier als Klappe ausgebildetes Absperrorgan 34 dient zum Umschalten des Durchflusses zwischen dem
Ringkanal einerseits und der Saugöffnung 12 bzw. dem Flüssigkeitszufuhranschluß 32 andererseits. Dadurch
läßt sich die Saugwirkung während des Vorgangs des Findens der Verstopfung bzw. die Zufuhr der Kontrastsubstanz
während des Entfernungsvorganges abstellen.
F i g. 3 zeigt einen Querschnitt durch das optische Faserbündel 16, von welchem ein Teil 36 das Laserteilbündel
26 und ein Teil 38 das Beleuchtungsteilbündel 24 enthält. Der zentrale Teil enthält das Beobachtungsteilbündel
22.
Das Vorderende der Hülse 10' kann auch die in F1 g. 4
dargestellte Form haben, in welcher der Kopf 18' konisch ausgebildet ist und das hier ebenfalls konisch ausgebildete
Faserbündel 16 enthält. In dieser Ausführungsform kann die Laserenergie für verschiedene Verwendungszwecke
in ihrer Intensität verstärkt und besser gebündelt werden, insbesondere für medizinische
Zwecke, um eine Beschädigung eines Blutgefäßes 42 (F i g. 1) od. dgl. zu vermeiden.
Die Arbeitsweise der Vorrichtung wird nun unter Bezugnahme auf die F i g. 1 und 2 beschrieben. Als Beispiel
für die Verstopfung 14 wird eine Thrombose in einer peripheren Vene, d. h. in dem Blutgefäß 42 angenommen.
Die Hülse 10 wird in das Blutgefäß 42 vor der Verstopfung 14 eingeführt. Nachdem die Hülse 10 etwas
in das Blutgefäß 42 eingeführt worden ist, wird das Absperrorgan 34 in die in F i g. 1 dargestellte Stellung gebracht
und eine Kontrastsubstanz über den Flüssigkeitszufuhranschluß 32 zugeführt Die Hülse 10 kann
dann unter Beobachtung auf einem Röntgenschirm bis zu euer Stelle vorbewegt werden, an der ein Widerstand
spürbar und das Anliegen des Kopfes 18 an der Verstopfung 14 sichtbar wird. Dann wirrt das Absperrorgan
34 in die in Fi g. 2 dargestellte Stellung gebracht um den Flüssigkeitszufuhranschluß 32 zu verschließen
und den Ringkanal zu der Saugöffnung 12 hin zu öffnen, wodurch die Kontrastsubstanz abgesaugt wird. Das Absperrorgan
34 wird wieder in die in F i g. 1 dargestellte Stellung gebracht Es wird eine Salzlösung zugeführt,
und der Bereich der Verstopfung 14 -wird mittels des Beobachtungsteilbündels 22 beobachtet, während der
Bereich mittels eines Lasers niedriger Intensität über das Laserteilbündel 26 oder mittels einer Lichtquelle
über das Beleuchtungsteilbündel 24 beleuchtet wird. Nachdem das Absperrorgan 34 in die in F i g. 2 dargestellte
Stellung gebracht und die Salzlösung abgesaugt worden ist, wird der Laser 28 eingeschaltet, der ein Loch
durch die Verstopfung 14 bohrt, wobei Bruchstücke 44 in Richtung der Pfeile 46 in Bewegung gesetzt werden
(F i g. 2) und zu dem Hinterende 10a der Hülse 10 gelangen. Wenn die Verstopfung 14 vollständig durchbohrt
worden ist, kann Blut 48 auf die andere Seite der Verstopfung gelangen und mit den Bruchstücken 44 zu der
Saugöffnung 12 transportiert werden, so daß in dem durchsichtigen Behälter 30 Blut sichtbar wird. Auf diese
Weise kann eine Bedienungsperson das Ende des Entfernungsvorganges leicht beobachten und eine Beschädigung
von hinter der Verstopfung befindlichen Teilen verhindern.

Claims (2)
1. Vorrichtung zum Finden, Beobachten und Entferner,
einer Verstopfung in einer Röhre, mit einer Hülse mit einem Vorder- und einem Hinterende, mit
einem Kopf mit einer öffnung am Vorderende und mit optischen Faserbündeln, die in ein Beobachtungsteilbündel
für die Beobachtung der Verstopfung, ein Laserteilbündel zur Verwendung bei der Entfernung der Verstopfung und ein Beleuchtungsteilbündel
zur Verwendung mit dem beobachtungsteilbündel bei der Beobachtung der Verstopfung
umfassen, sowie mit Kanälen zum Zuführen von Flüssigkeiten und Absaugen von Bruchstücken der
Verstopfung, dadurch gekennzeichnet,
daß die Faserteilbündel (22, 24, 26) zu einem einzigen Faserbündel (16) zusammengefaßt sind, das sich
in einem Teil der Hülse (10) befindet und in der Kopföffnung endet, daß das zusammengefaßte Faserbündel
(16) zwischen sich und der Hülse (10) einen Ringkanal bildet, der zu einer am Hinterende
(10a,) der Hülse (10) vorgesehenen Saugöffnung (12) führt, daß ein Flüssigkeitszufuhranschluß (32) an
dem Ringkanal zwischen dessen Enden vorgesehen ist und daß nahe dem Hinterende (10a,l der Hülse
(10) ein durchsichtiger Behälter (30) für die Anzeige des Endes des Entfernungsvorganges der Verstopfung
vorgesehen ist.
2. Vorrichtung nach Anspruch 1, gekennzeichnet durch ein Absperrorgan (34) zum Umschalten des
Durchflusses zwischen dem Ringkanal einerseits und der Saugöffnung (12) bzw. dem Flüssigkeitszufuhranschluß
(32) andererseits.
