Einsatz für operativ geschaffene Ausnehmungen von Röhrenknochen .
Die Erfindung betrifft einen Einsatz für operativ geschaffene •Ausnehmungen von Röhrenknochen, in denen Verankerungsschäfte von Endoprothesen mit Hilfe von Knochenzement verankert werden.
Wird eine Endoprothese in einem Knochen mit Hilfe von Knochenzement verankert, so wird zunächst der Knochenzement in die operativ vorbereitete Ausnehmung des Knochens eingefüllt und anschliessend in diese teigartige Masse der Schaft der Prothese eingedrückt oder eingeschlagen. Bei diesem^-Eindrücken oder Einschlagen des Schaftes treten unter Umständen Schwierigkeiten auf, weil der in der Ausnehmung befindliche Knochenzement, der zur;'festen Verankerung des Schaftes möglichst gegen die im wesentlichen in Richtung der Schaftachse verlaufenden Seitenwände der Ausnehmung verdrängt werden soll, in Achsrichtung des Schaftes bzw. der Ausnehmung weiter in den Knochen hineingedrückt wird.
Aufgabe der Erfindung ist es, dieses Ausweichen des Knochenzements in eine im wesentlichen axiale Richtung zu verhindern. Erfindungsgemäss erfolgt die Lösung dieser Aufgabe durch einen propfenartigen Sperrkörper, der mit einer Anzahl in seiner Achsrichtung hintereinander liegender Kränze von Verankerungsrippen ausgerüstet ist, wobei jeder Kranz aus einer Vielzahl Lappen besteht, die durch sich in radialer Richtung erweiternde Schlitze voneinander getrennt und mindestens in Achsrichtung des Körpers elastisch verformbar sind.
Der Sperrkörper, der vorzugsweise aus einem in der Implantattechnik gebräuchlichen Kunststoff, z.B. hochmolekularem Polyäthylen (HDPE) besteht, wird nach dem Ausräumen der Aus-

nehmung in diesen eingesetzt, wonach Knochenzement in die Operationshöhlung eingebracht wird. Die auf den Durchmesser der operativ erzeugten Ausnehmung - beispielsweise durch die Bereitstellung unterschiedlicher Grossen - abgestimmten elastisch verformbaren Lappen des Einsatzes werden dabei zu einer Art aus mehreren Lagen bestehendem Kelch nach oben gebogen und in den Seitenwänden der Ausnehmung eingeklemmt; auf diese Weise schliessen die Lappen die Ausnehmung weitgehend in axialer Richtung ab.
Die Ausbildung in mehreren axialen hintereinander liegenden Kränzen erhöht das Anhaften des Körpers in den Seitenwänden, so dass er nach dem Setzen auch durch das Eindrücken des Schaftes in den Zement nur noch unbedeutend in axialer Richtung verschoben werden kann. Das Einsetzen des Einsatzes in die Ausnehmung und ein Ausschieben in Gegenrichtung lassen sich weiter vereinfachen bzw. verhindern, wenn die freien Enden der Rippen auf einer Seite abgerundet und auf der anderen scharfkantig ausgebildet sind. Eine Zentrierung des Keepers in der Ausnehmung lässt sich erreichen, wenn die radiale Länge der Rippenlappen der einzelnen Kränze in einer axialen Richtung mindestens über einen Teil der axialen Höhe des Sperrkörpers zunimmt, wobei die Ausbildung des Kelchs erleichtert werden kann, wenn der axiale Abstand zwischen den einzelnen Kränzen in der Richtung zu grösseren radialen Längen der Rippenlappen ebenfalls zunimmt.
Ein Druckausgleich auf beiden Seiten des Einsatzes kann durch eine Durchgangsbohrung erreicht werden, in der darüberhinaus gleichzeitig Mittel für eine lösbare Befestigung eines Setzinstruments vorgesehen sein können. Diese Mittel bestehen z.B. in einem Schraubengewinde, Die Durchgangsbohrung kann darüberhinaus auch zum Absaugen von Blut und Knochensplittern aus dem von der Operationsöffnung aus jenseits des Einsatzes liegenden Teil der Ausnehmung dienen.
Im folgenden wird die Erfindung anhand eines Ausführungsbeispiels im Zusammenhang mit der Zeichnung näher erläutert.

In dem nur schematisch angedeuteten Knochen 1 ist für die Aufnahme eines nicht dargestellten Verankerungsschaftes eine Ausnehmung 2 operativ geschaffen worden, wobei das spongiose Gewebe mindestens weitgehend entfernt worden ist. In die nach unten konisch zulaufende Ausnehmung 2 ist ein als Zementsperre dienender Sperrkörper 3 eingeschoben, an dessen zylindrischen Stamm, über die axiale Höhe verteilt, eine Anzahl Kränze 4 von durch Schlitze 5 (Fig. 2) voneinander getrennten Lappen 6 angesetzt sind. Die Lappen 6 verschiedener Kränze 4 können dabei in ümfangsrichtung versetzt zueinander oder fluchtend hintereinander angeordnet sein. Die Lappen 6 des vorzugsweise aus Kunststoff angefertigten Körpers 3 sind elastisch verformbar, so dass sie beim Einpressen des Einsatzes in die Ausnehmung 2 zu einer Art Blütenkelch nach oben gebogen werden. Dabei verklemmen sich die Lappen 6 an den Wänden 7 und gewährleisten so einen festen Sitz des Einsatzes in der Ausnehmung 2.
Die Darstellung der Fig. 1 gibt einen Zwischenzustand des Einsetzens des Körpers 3 zwischen locker und fest wieder, bei dem die elastischen Lappen 6 die Wände 7 der Ausnehmung 2 gerade berühren, aber noch nicht zu einem Kelch umgebogen und eingeklemmt sind.
Wie Fig. 1 erkennen lässt, verlängern sich die radialen elastischen Längen der Lappen β nach oben, weswegen gleichzeitig die Abstände a zwischen einzelnen Kränzen 4 in der gleichen Richtung ebenfalls grosser werden, damit für die längeren Lappen 6 bei dem Aufbiegen genügend Platz zur Verfügung steht. konische Form des Einsatzes erleichtert seine Zentrierung beim Einsetzen in die Ausnehmung 2.
Die dünnen äusseren Ränder der Lappen 6 sind an der Unterseite mit einer Rundung 8 versehen, die ein Hineingleiten des Einsatzes in die Ausnehmung 2 erleichtert. Auf der Oberseite ist dieser Rand dagegen scharfkantig, um ein Ausschieben des gezackten Körpers zu erschweren und das Verklemmen der Lappen 6 an den Wänden 7 zu verbessern.
Der Körper 3 hat eine zentrale Durchgangsbohrung 9, in die ein Gewinde IO für die Befestigung eines nicht gezeigten Setzinstruments, das z.B. aus einem mit Tiefenmarken für die Setztiefe des Einsatzes - gemessen vom Rand der Operationsöffnung der Ausnehmung 2 aus - versehenen, entsprechend dem Achsverlauf der Ausnehmung geformten Stab bestehen kann, der an einem Ende ein Gegengewinde zum Gewinde 10 und am anderen Ende einen Griff trägt.

Claims (6)

    P. 5301/Wq/IS Patentansprüche
    1/ Einsatz für operativ geschaffene Ausnehmungen von Röhrenknochen, in denen VerankerungsSchäfte von Endoprothesen mit Hilfe von Knochenzement verankert werden, gekennzeichnet durch einen propfenartigen Sperrkörper (3), der mit einer Anzahl in seiner Achsrichtung hintereinander liegender Kränze (4) von Verankerungsrippen ausgerüstet ist, wobei jeder Kranz (4) aus einer Vielzahl Lappen (6) besteht, die durch sich in radialer Richtung erweiternde Schlitze (5) voneinander getrennt und mindestens in Achsrichtung des Körpers (3) elastisch verformbar sind.
  2. 2. Einsatz nach Anspruch 1, dadurch gekennzeichnet, dass die radiale Länge der Rippenlappen (6) der einzelnen Kränze (4) in einer axialen Richtung mindestens über einen Teil der axialen Höhe des Sperrkörpers (3) zunimmt.
  3. 3. Einsatz nach Anspruch 1, dadurch gekennzeichnet, dass der Körper (3) eine zentrale, axiale Durchgangsbohrung (9) hat.
  4. 4. Einsatz nach Anspruch 3, dadurch gekennzeichnet, dass in der Durchgangsbohrung (9) Mittel (10) zur lösbaren Befestigung eines Setzinstruments vorgesehen sind.
  5. 5. Einsatz nach Anspruch 1, dadurch gekennzeichnet, dass die freien Enden der Rippen (6) auf einer Seite abgerundet und auf der anderen scharfkantig ausgebildet sind.
  6. 6. Einsatz nach Anspruch 2, dadurch gekennzeichnet, dass der axiale Abstand (a) zwischen den einzelnen Kränzen (4) in der Richtung zu grösseren radialen Längen der Rippenlappen (6) ebenfalls zunimmt.
