Description
Die Erfindung betrifft einen Genchip zum Bestimmen der genetischen Prädisposition für das Ausbilden arteriosklerotischer Erkrankungen.
Erkrankungen des Herzkreislaufsystems stellen weltweit eine der häufigsten Todesursachen dar. In der Mehrzahl der Fälle sind solche Erkrankungen auf eine Verkalkung der großen und mittelgroßen Schlagader (Arteriosklerose) zurückzuführen. Von besonderer Bedeutung ist eine Arteriosklerose der Herzkranzgefäße und der Halsschlagader, da diese zum Herzinfarkt oder Schlaganfall führen kann. In Deutschland erleiden jährlich derzeit etwa 250.000 Menschen einen Herzinfarkt, der in der Hälfte der Fälle tödlich endet. Oftmals führt der Herzinfarkt bei den überlebenden Patienten zu erheblichen Folgeschäden und Behinderungen. Die Häufigkeit des Schlaganfalls liegt in Deutschland etwa bei der Hälfte der Herzinfarktfälle; es verlaufen etwa ein Viertel aller Fälle tödlich. Die Folgeschäden sind oft verheerender als beim Herzinfarkt.
Bei der Arteriosklerose handelt es sich um ein komplexes Krankheitsbild, das auf multifaktorielle Ursachen zurückgeht. Dabei werden vornehmlich chemische und mechanische Schädigungen der Gefäße beispielsweise durch anhaltenden Nikotinkonsum, Stoffwechselstörungen wie z.B. Diabetes mellitus oder zu Adrenalin- oder Noradrenalin-Ausschüttungen führende psy-

chosomatische Einflüsse als wesentliche Ursachen diskutiert. Daneben spielen ein anhaltender Bluthochdruck und ein hoher Cholesteringehalt im Blut eine maßgebliche Rolle.
Darüber hinaus wurde bereits vor über 100 Jahren erkannt, daß sich innerhalb einiger Familien Herzinfarkte und Schlaganfälle häuften, so daß der Vererbung eine wichtige Rolle bei der Ausbildung der Arteriosklerose beigemessen wurde. Erst in den letzten 10 bis 15 Jahren hat man jedoch begonnen, die Gründe für die familiäre Häufung dieser Erkrankungen auf molekularer Basis zu verstehen.
Innerhalb der medizinischen Diagnostik kommt der Erfassung der Risikofaktoren zur Bestimmung des Artenoskleroserisikos ein hoher Stellenwert zu. Dazu werden biochemische Analysen beispielsweise der Cholesterin- bzw. der Fettwerte im Blut durchgeführt und der Patient wird nach Lebensgewohnheiten sowie nach in der Familie häufig vorkommenden Krankheiten befragt. Sofern überhaupt die genetischen Risikofaktoren erfaßt werden, erfolgt dies über eine vollständige Sequenzierung der entsprechenden Nukleotidsequenz des relevanten Gens oder durch eine Darstellung der betroffenen Genabschnitte beispielsweise mittels der allelspezifischen PoIymerasekettenreaktion, Restriktionslängenpolymorphismen oder der single stand conformation polymorphism-MeXhoae (SSCP).
Nachteilig an diesen Methoden zur Erfassung der genetischen Disposition ist, daß sie in der Regel nur einzelne Sequenzveränderungen, allenfalls einzelne Gene erfassen. Darüber hinaus sind sie oft sehr arbeitsintensiv und erfordern Spezialkenntnisse sowohl bei der Durchführung als auch bei der Interpretation. Daher eignen sie sich nicht für die Untersuchung einer Vielzahl möglicherweise betroffener Personen.

Der Erfindung liegt daher das Problem zugrunde, eine Diagnosemöglichkeit bereitzustellen, die eine schnelle und aussagekräftige Diagnose über die genetische Prädisposition zur Erkrankung an der Arteriosklerose erlaubt.
Dieses Problem wird gelöst mit einem Nukleotidträger gemäß dem Hauptanspruch. Vorteilhafte Weiterentwicklungen sind Gegenstand entsprechender Unteransprüche.
Der Erfindung liegt dabei der Gedanke zugrunde, eine geeignete Auswahl von für das genetische Arterioskleroserisiko relevanten Nukleotidsequenzen (bzw. die dazu komplementäre Sequenzen) sowie deren funktionell charakterisierte Mutationen auf einen Träger (nachfolgend auch Genchip) zu bringen, diese Nukleotidsequenzen mit einer Nukleotidsequenzprobe eines Patienten zu hybridisieren und die Hybridisierung multifaktoriell auszuwerten. Die Nukleotidsequenzen stellen dabei Sonden für sog. Referenzsequenzen dar, deren indirekter oder direkter funktioneller Zusammenhang mit der Arteriosklerose bekannt ist oder vermutet wird. Sofern eine Sonde auf dem Träger mit Oligonukleotiden aus der Patientenprobe hybridisiert, ist der Nachweis für das Vorhandensein der entsprechenden Referenzsequenz bei dem Patienten erbracht.
Mit dieser simultanen Erfassung und multifaktoriellen Analyse einer Vielzahl von für das Arterioskleroserisiko relevanten mutierten Sequenzen geht der besondere Vorteil einher, daß etwaige Synergieeffekte zwischen verschiedenen Mutationen erkannt und dargestellt werden können. So ist es möglich, daß einzelne Mutationen für sich allein genommen kein erhebliches Risiko darstellen, treten sie jedoch in Kombination mit anderen - wiederum für sich allein genommen wenig bedeutsamen - Mutation auf, kann sich daraus ein beachtliches genetisches Risiko ergeben, das deutlich das Risiko aus der Summe der Einzelmutationen übersteigt. Dies gilt insbesondere auch für Mutationen, die im Zusammenhang mit der Arteriosklerose

diskutiert werden, ohne daß ihnen jedoch derzeit eine konkrete Funktion zugewiesen werden kann (sog. Kandidatenmutationen).
Der erfindungsgemäße Nukleotidträger erlaubt damit eine genauere Diagnose des Arterioskleroserisikos, insbesondere dann, wenn die Diagnose mit der Erfassung der konventionellen Risikofaktoren kombiniert wird. Eine genauere Diagnose ist eine wichtige Voraussetzung für neue Behandlungsmethoden, die eine spezifische Behandlung genetischer Defekte in naher Zukunft erlauben werden, wie zum Beispiel die Gentherapie.
Ein weiterer Vorteil der Erfindung liegt darin, daß eine Vielzahl relevanter Mutationen der ausgewählten Referenzsequenzen in nur einem Arbeitsgang erfaßt werden können, dessen Durchführung keiner Spezialkenntnisse bedarf. Die Nukleotidträger eignen sich darüber hinaus für die Automatisierung und somit für die rasche Bearbeitung umfangreicher Probenserien. Somit wird erfindungsgemäß die gleichzeitige Erfassung relevanter Genveränderungen kostengünstig, schnell und zuverlässig möglich.
Die erfindungsgemäßen Nukleotidträger eignen sich aber nicht nur für die klinische Diagnostik sondern insbesondere auch als Hilfsmittel für die wissenschaftliche Forschung. So können damit z.B. Bevölkerungsstudien, die zum Ziel haben, den Zusammenhang zwischen genetischen Veränderung und dem Risiko für Herzinfarkt oder Schlaganfall zu präzisieren, erleichtert werden, da eine im Vergleich zu herkömmlichen Studien sehr viel höhere Anzahl von Studienteilnehmern erfaßt werden kann.
Da der erfindungsgemäße Nukleotidträger die Identifikation von Interaktionen zwischen Mutationen bzw. Mutationskombinationen sowie zwischen Mutationen und konventionellen Risikofaktoren beispielsweise aus der

Familienanamnese, dem Stoffwechsel oder dem Blutcholesterinspiegel erlaubt, kann er zum besseren Verständnis der Entstehung arteriosklerotischer

Erscheinungen beitragen. Er weist somit eine besondere Eignung als Forschungsreagenz auf.
Der Einsatz des erfindungsgemäßen Nukleotidträgers beruht auf der Hybridisierung der auf dem Träger aufgebrachten Nukleotidsequenzen mit komplementären Nukleotidsequenzen aus Proben material des Patienten. Da die ausgewählten Nukleotidsequenzen Sonden für funktionell für das Ausbilden der Arteriosklerose relevanten Referenzsequenzen bestimmter Gene darstellen, können mittels der Hybridisierung einzelne Gene oder Teile davon in einer Präparation genomischer DNA oder das Transkriptionsprodukt eines Genes (mRNA) nachgewiesen werden. Die Hybridisierung kann mit einer Nachweisreaktion und anschließender Identifizierung einer Nukleinsäuresequenz kombiniert werden. Dazu kann ein Nukleinsäurestrang beispielsweise durch Farbstoffe, Radioaktivität oder chemolumineszierende oder fluoreszierende Moleküle markiert werden, während der zweite an einem festen Träger beispielsweise aus Kunststoff oder Glas gebunden ist. Die Markierung der Nukleotidsequenz erfolgt dabei vorzugsweise bei der Synthese der Sequenz aus der Patientenprobe. Auf den Träger können mehrere tausend Oligonukleotide als Sonden aufgebracht werden. Die sich anschließende Hybridisierung auf dem Träger kann vorteilhafterweise über einen Scanner ausgewertet werden, so daß sich die Auswertung weitgehend automatisieren läßt.
Im Kontext dieser Anmeldung werden die nachfolgenden Begriffe wie folgt verwendet:
Der Begriff "Hybridisierung" schließt jede Form der Paarung oder Aneinanderlagerung von Nukleotidsequenzen ein. Die Hybridisierung kann sowohl unter stringenten als auch unter relaxierten Bedingungen erfolgen.
Der Begriff "Nukleotidsequenz" bezieht sich auf jede oligomere oder polymere Sequenz aus Ribonukleotiden oder Desoxyribonukleotiden oder aus einer Kombination daraus. Ebenso sind davon Ribonukleotide oder Desoxyribonukleotide mit Basenanaloga erfaßt. Nukleotidsequenzen können synthetischen oder natürlichen Ursprungs sein sowie auf rekombinante Verfahren zurückgehen.
Der Begriff "Arterioskleroserisiko" bezieht sich auf jede Erhöhung (oder Erniedrigung) der Wahrscheinlichkeit zur Ausbildung einer Artehosklerose. Die Wahrscheinlichkeit bemißt sich an der Durchschnittswahrscheinlichkeit in der gesamten Bevölkerung. Sofern das Arterioskleroserisiko auf genetische Ursachen zurückzuführen ist, wird der Begriff genetisches Arterioskleroserisiko bzw. genetische Prädisposition oder nur Disposition verwendet.
Unter "Mutationen" werden alle Formen der Veränderung der genetischen Informationen einer Nukleotidsequenz im Vergleich mit der Wildtypsequenz auch native Sequenz - verstanden. Dabei kann es sich beispielsweise um Substitutionen, Insertionen oder Deletionen handeln. Ebenso sind komplexere Mutationen wie Inversionen und Indels möglich.
Der Begriff "Referenzsequenz" bezieht sich auf bestimmte Nukleotidsequenzen ausgewählter Gene, wobei ein Gen eine Vielzahl von Referenzsequenzen einschließen kann. Die Referenzsequenzen können nativ, d.h. in der Wildtypformung, oder mutiert sein. Ein Zusammenhang zwischen den Muta-
tionen und dem Arterioskleroserisiko ist bekannt oder wird vermutet, d.h. die Mutation ist funktionell charakterisiert bzw. relevant.
Der Begriff "Sonde" bezieht sich auf spezifische Oligonukleotidabschnitte einer Referenzsequenz bzw. dazu komplementärer Sequenzen. Damit erlaubt die Hybridisierung einer Oligonukleotidprobe mit einer Sonde den Nachweis der Referenzsequenz in dem Ursprungsgewebe der Probe. Die Sonden werden auf dem Nukleotidträger aufgebracht.
Auf dem Träger sind Sonden für Referenzsequenzen der in der nachfolgenden Tabelle 1 aufgeführten Gene aufgebracht, deren Mutationen zu einer Erhöhung des genetischen Arterioskleroserisikos direkt oder indirekt beitragen. Alternativ können auch Sonden von zu den Referenzsequenzen komplementären Sequenzen verwendet werden (nachfolgend auch Komplementärsequenzen). Die Gene werden sowohl über ihre Codenummer aus der GenBank des National Center for Biotechnology Information (NCBI) als auch über das für sie verwendete Symbol spezifiziert. Des weiteren enthält die Tabelle Angaben über die Häufigkeit genetischer Veränderungen dieser Gene und die Anzahl bekannter Mutationen.

Im folgenden wird die Erfindung anhand zweier Ausführungsbeispiele des näheren erläutert.
Sonden der Komplementärsequenzen der in der Tabelle 2 aufgelisteten Referenzsequenzen werden als Oligonukleotide mit Hilfe von Standardtechniken (DE 19612356; US5837832) auf einen geeigneten Träger, beispielsweise mit Gold beschichtetes Glas, aufgebracht. Dabei werden 10- bis 25-mere Nukleotidsequenzen, vorzugsweise 15-mere, verwendet. Die Sequenzen werden dabei so gewählt, daß sie die funktioneilen Mutationen einschließen und die mutierte Komplementärsequenz (i.e. Komplementärsequenz zu der mutierten Referenzsequenz) relativ zu der nativen Komplementärsequenz (i.e. Komplementärsequenz zu der nativen Referenzsequenz) etwa in der Mitte liegt.
Die Oligonukleotidsonden werden so auf den Träger aufgebracht, daß sich auf den Feldern des Trägers jeweils nur eine Variante der Oligonukleotide befindet.
Die Tabelle 3 enthält Angaben über die Nukleinsäuresequenzen, deren komplementäre Sequenzen als Sonden auf den Genchip aufgebracht werden (zwecks besserer Übersichtlichkeit wird nur der zentrale Bereich der 10-bis 25-mere gezeigt, die Position mit Bezug auf die native Referenzsequenz
wird durch Angabe der Kodon-Nummer spezifiziert). Zu jeder Referenzsequenz wird an genau definierter Stelle auf dem Träger die komplementäre Sequenz der aus der dritten Spalte ersichtlichen nativen Referenzsequenz aufgebracht. Die angegebenen Kodon-Nummem beziehen sich auf die native Gensequenz.
Die Anzahl der benötigten Hybridisierungsstellen und diagnostizierbaren Erkrankungen ist aus Tabelle 1 herleitbar. Die bekannten funktionell relevanten Mutationen sind in der Tabelle 3 aufgeführt.
Insgesamt werden in diesem Ausführungsbeispiel 410 Hybridisierungsstellen für die Ermittlung der genetischen Prädisposition für die Arteriosklerose auf einem DNA-Chip verwendet. Die gemeinsame Präsenz dieser Oligonukleotidsonden auf einem Träger erlaubt die simultane Erfassung dieser Sequenzvariationen aus einer Patientenprobe. Deshalb eignet sich dieser DNA-Chip insbesondere für den Einsatz in epidemiologischen Studien, die das Ziel verfolgen, die relative Wertigkeit genetischer Faktoren für das Herzinfarktrisiko zu erforschen.
Die funktionell relevanten Mutationen der in Tabelle 1 aufgeführten Gene sind in der Tabelle 3 enthalten. Diese oder auch ihre Komplementärsequenzen werden auf einen Nukleotidträger aufgebracht. Der Übersichtlichkeit halber werden die Mutationen in verschiedenen "Typen" aufgeführt.

Claims (10)
1. Nukleotidträger zur Ermittlung des genetischen Arterioskleroserisikos, dadurch gekennzeichnet, daß der Nukleotidträger Oligonukleotid-sonden zur Untersuchung der Gene aus der Tabelle 1 aus einer Patientenprobe aufweist, die jeweils zumindest teilweise identisch oder komplementär sind zu Abschnitten der genannten Gene der Tabelle 1.
2. Nukleotidträger zur Ermittlung des genetischen Arterioskleroserisikos, dadurch gekennzeichnet, daß der Nukleotidträger Sonden aus 10- bis 25-meren Oligonukleotiden aufweist.
3. Nukleotidträger nach Anspruch 1 oder 2, gekennzeichnet durch 15- bis 18-mere Oligonukleotidsonden.
4. Nukleotidträger nach einem der Ansprüche 1 bis 3, gekennzeichnet durch 15-mere Oligonukleotidsonden.
5. Nukleotidträger nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß die Oligonukleotidsonden jeweils zumindest teilweise identisch oder komplementär sind zu Referenzsequenzen aus der Tabelle 2.
6. Nukleotidträger nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß die Oligonukleotidsonden jeweils zumindest teilweise identisch oder komplementär sind zu Mutationen der Tabelle 3.
7. Nukleotidträger nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß die Oligonukleotidsonden jeweils zumindest teilweise identisch oder komplementär sind zu Polymorphismen der Tabelle 4.
8. Nukleotidträger nach einem der Ansprüche 1 bis 7, gekennzeichnet durch Oligonukleotidsonden mit Sequenzabschnitten, die zu mindestens 90% identisch oder komplementär zu einer Auswahl der in Tabelle 3 oder 4 angegebenen Mutationen sind.
9. Nukleotidträger nach einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, daß der Nukleotidträger Oligonukleotidsonden zur Untersuchung eines oder mehrerer der folgenden Gene aufweist: Cholesterin 7- alpha-Hydroxylase (CYP7A1), Plasminogen-Aktivator Inhibitor-1 (PAI1) und Paraoxonase 1 (PON1).
10. Nukleotidträger nach einem der Ansprüche 1 bis 9, dadurch gekennzeichnet, daß dieser zusätzlich weitere Sonden aufweist, vorzugsweise solche, die zu den Wildtypsequenzen der jeweiligen Referenzsequenz zumindest teilweise identisch oder komplementär sind.
