Abstract
Herniareparaturpatch
mit einem Drahtrahmen (12) und einem prothetischen Gewebematerial
(14), das längs
des Umfangs des Drahtrahmens (12) durch diesen gehaltert ist, dadurch
gekennzeichnet, dass der Drahtrahmen (12) die Form einer geschlossenen Schleife
hat, an der das prothetische Gewebematerial (14) befestigt ist,
und dass der Drahtrahmen (12) derart aus einer Form-Memory-Legierung
gebildet ist, dass er sich durch Erwärmung auf die Körpertemperatur
des Patienten von einer zusammengerollten Form zu seiner vorbestimmten
Form entfaltet.
Description
- Die Erfindung betrifft einen Herniareparaturpatch gemäß Anspruch 1.
- Seit 1991 hat der Erfinder eine große Anzahl von laparoskopischen Herniareparaturen bei inguinalen, ventralen, inzidierenden und umbilikalen Hernien mit großem Erfolg hinsichtlich der Patientenzufriedenheit durchgeführt. Im Vergleich zur offenen Chirurgie haben Patienten bei laparoskopischem Eingriff wesentlich weniger Schmerzen und kehren schneller zu ihrer normalen Aktivität zurück.
- Beim Durchführen dieser Verfahren verwendete der Erfinder anfänglich einen prothetischen Patch, der während des chirurgischen Eingriffs aus einem sterilen, gewobenen Polypropylen-Netzmaterial hergestellt wurde, das auf die Hälfte gefaltet wurde, um eine doppelte Lage zu erzeugen, und das anschließend auf die richtige Größe, typischerweise auf ein Rechteck mit 6 cm × 9 cm zurechtgeschnitten wurde. Zur Verbindung der vier Ecken des Rechtecks wurden Nähte verwendet. Zwei zusätzliche Nähte wurden ungefähr auf halbem Weg längs der nicht gefalteten Kante positioniert. Anschließend wurde zwischen diesen zwei zusätzlichen Nähten ein Schlitz erzeugt, um die unteren epigastrischen Gefäße aufzunehmen. Nach dem Auseinanderschneiden des Herniasacks vom ipsilateralen Testikel und den Bändern, der Erzeugung eines peritonealen Einschnitts und der nachfolgenden Dissektion des peritonealen Lappens und des Herniasacks vom Herniadefekt und der umgebenden Fascia und den Bändern, wurde der Patch zu einem Rohr gerollt und in eine Trokarhülle eingeführt, die anschließend durch einen Trokar größeren Durchmessers hindurch eingeführt wurde, und in den peritonealen Hohlraum eingeführt. Anschließend wurde ein laparoskopischer Forceps verwendet, um den Patch zu entfalten und ihn vor den Herniadefekt und um die unteren epigastrischen Gefäße herum zu positionieren, wobei das Netz sowohl die direkten als auch indirekten Herniaräume bedeckte. Anschließend wurde der Netzpatch festgelegt, indem er an die darunterliegende Fascia geklammert oder mit dieser vernäht wurde. Im folgenden wurde das Peritoneum über dem Patch geschlossen, so dass das gesamte Netzstück durch dieses bedeckt war.
- Obwohl sich das obige Verfahren im Ergebnis als ziemlich erfolgreich erwiesen hat, erhöht sich der zum Durchführen des Verfahrens erforderliche Zeit- und Kostenaufwand beträchtlich, da der Netzpatch während des chirurgischen Eingriffs hergestellt werden muss, später beim Entrollen und Positionieren des Netzpatchmaterials relativ zu den direkten und indirekten Herniaräumen Schwierigkeiten auftreten und anschließend der Netzpatch festgeklammert oder festgenäht werden muss.
- Aus der
EP 0 544 485 B1 ist ein Herniareparaturpatch gemäß dem Oberbegriff des Anspruchs 1 bekannt, der aus zwei scheibenförmigen, am Umfang miteinander verbundenen und zwischen sich einen Hohlraum ausbildenden Patchlagen aus prothetischem Material besteht. Nachdem dieses prothetische Material laparoskopisch an die gewünschte Stelle des Patienten eingeführt worden ist, kann ein Versteifungsdraht in den Hohlraum zwischen den beiden Patchlagen eingeführt und diese dadurch aufgespannt werden. Weiterhin ist aus dieser Druckschrift ein regenschirmartig zusammenfaltbarer und entfaltbarer Reparaturpatch mit radialen Streben bekannt. Dieser benötigt ebenfalls ein mechanisches Hilfsmittel und einen zusätzlichen, zeitaufwendigen Arbeitsgang zum Entfalten. - Aus der WO 94/27535 A1 ist ein scheibenförmiger Reparaturpatch bekannt, dessen Rahmen aus Polypropylen, Silikon oder einem anderen Kunststoffmaterial besteht.
- Ausgehend von diesem Stand der Technik liegt der Erfindung die Aufgabe zugrunde, einen Herniareparaturpatch der eingangs genannten Art zu schaffen, mit dem der Herniareparaturpatch auf einfachere, schnellere und sicherere Weise implantierbar ist.
- Diese Aufgabe wird erfindungsgemäß durch die Merkmale des Anspruchs 1 gelöst. Vorteilhafte Ausführungsformen der Erfindung sind in den weiteren Ansprüchen beschrieben.
- Beim erfindungsgemäßen Herniareparaturpatch hat der Drahtrahmen die Form einer geschlossenen Schleife, an der das prothetische Material befestigt ist.
- Der Drahtrahmen ist ferner derart aus einer Form-Memory-Legierung gebildet, dass er sich durch Erwärmung auf die Körpertemperatur des Patienten von einer zusammengerollten Form zu seiner vorbestimmten Form entfaltet.
- Der erfindungsgemäße Herniareparaturpatch ist derart vorgefertigt, dass er sich anatomischen Strukturen anpasst und sich ohne weiteres entfaltet, wenn er aus einer rohrförmigen laparoskopischen Einführeinrichtung freigegeben wird. Weiterhin bleibt der Patch an Ort und Stelle, ohne dass das Einklammern oder Vernähen mit der darunter liegenden Fascia erforderlich ist.
- Ein synthetisches prothetisches Material, beispielsweise gewobenes Polypropylen oder expandiertes PTFE (Gortex), ist am Drahtrahmen befestigt und wird von diesem gehalten. Der Drahtrahmen, der das Netzmaterial trägt, kann aus NiTiNOL oder einer anderen geeigneten Form- Memory-Legierung gebildet und am prothetischen Material derart befestigt sein, dass er eine stundenglasartige Form einnimmt, wenn die Legierung in einer Austenitform ist, und eine gerollte zylindrische Form, wenn sie in einer Martensitform ist. Der Atomprozentsatz des Nickels in der Legierung ist derart, dass die Legierung eine Transformationstemperatur von etwa 37°C (Körpertemperatur) hat. Der Patch kann somit, wenn er abgekühlt ist, ohne weiteres zylindrisch geformt werden, um in einem Zuführungstrokar plaziert zu werden. Wird er aus dem Trokar in den abdominalen Hohlraum des Patienten ausgestoßen, erwärmt er sich bis zu dem Punkt, an dem die Legierung in ihrer Austenitform vorliegt, so dass sie in ihre funktionale vorbestimmte Gestalt springt. Der verengte Mittelabschnitt des stundenglasförmigen Patchs nimmt die unteren epigastrischen Gefäße und Bandstrukturen auf, während die gegenüberliegenden Endkeulen den direkten und indirekten Herniaraum überdecken. Da der Drahtrahmen integral mit dem Patch ausgebildet ist, wandert er nicht und muss dementsprechend nicht festgenäht oder festgeklammert werden. Er bleibt verankert, nachdem er durch das Peritoneum auf eine sandwichartige oder laminatartige Weise bedeckt worden ist.
- Die vorstehenden Merkmale, Ziel und Vorteile der Erfindung ergeben sich für den Fachmann aus der folgenden detaillierten Beschreibung eines bevorzugten Ausführungsbeispiels in Verbindung mit den Zeichnungen. In diesen zeigen:

1 ist eine vergrößerte Darstellung eines erfindungsgemäß aufgebauten Patchs zur Herniareparatur ersichtlich. Die Patchprothese ist allgemein mit dem Bezugszeichen10 versehen und weist einen äußeren Drahtrahmen12 auf, der einen Draht umfasst, der aus einer Formmetalllegierung gebildet ist, vorzugsweise einer Nickeltitanlegierung mit 49–51 Atomprozent Nickel und Rest Titan. Eine derartige Legierung wird allgemein als NiTiNOL bezeichnet. Durch eine geeignete Einstellung der Relativkonzentration von Nickel in der Legierung kann der Drahtrahmen12 derart hergestellt werden, dass er eine Übergangstemperatur zwischen der Austenitform und der Martensitform bei etwa 37°C zeigt, die der Körpertemperatur entspricht. Wie ersichtlich, weist der Draht eine geschlossene Schleife auf, die eine im wesentlichen ovale Form hat, jedoch eine eingeengte kleinere Achse aufweist, wodurch ein stundenglasartiges Profil erzeugt wird. Obwohl eine derartige Form unter Umständen bevorzugt ist, ist die Erfindung nicht auf eine derartige Formgestaltung begrenzt. - Innerhalb des Drahtrahmens
12 ist ein prothetisches Gewebematerial14 gehaltert, das vorzugsweise aus gewobenen Strängen aus Polypropylenkunststoff oder expandiertem PTFE (Gortex) besteht. Die prothetische Vorrichtung10 kann auf diese Weise dampfsterilisiert werden. Bei Gebrauch können die Keulen16 und18 über den direkten bzw. indirekten Herniaräumen positioniert werden. Ist dies geschehen, ermöglicht der verengte Mittelabschnitt, der allgemein mit dem Bezugszeichen20 versehen ist, eine Plazierung, ohne dass die unteren epigastrischen Gefäße gestört werden. - Wird der Drahtrahmen
12 unter den Transformationszustand der Form-Memory-Legierung gekühlt, so dass er in seiner Martensitform vorliegt, kann die Prothese10 schraubenförmig gewickelt werden, um einen zylindrischen Aufbau zu bilden, wie in den2 und3 gezeigt ist. Hierdurch kann die Prothese durch einen rohrförmigen Trokar hindurch in den abdominalen Hohlraum eingeführt werden. Erwärmt sich der Form-Memory-Drahtrahmen12 bis zur Körpertemperatur, geht er in seine Austenitform über, wie in1 gezeigt ist. Unter Verwendung eines laparoskopischen Forzeps kann die Prothese10 von1 gegriffen und durch den Chirurgen erneut positioniert werden, bis die Keulen16 und18 und der verengte Mittelabschnitt20 ordnungsgemäß positioniert sind, um den Herniadefekt abzudecken. - Für viele Fachleute ist erkennbar, dass die Prothese in einer großen Vielzahl von Formen und Größen hergestellt werden kann, um für Kinder, Erwachsene, Männer, Frauen und insbesondere für den einschlägigen Herniatyp zu passen. Sie kann bis zum Gebrauch in einer sterilen Hülle eingeschlossen sein. NiTiNOL ist zwar eine bevorzugte Form-Memory-Legierung, andere Legierungen wie beispielsweise Gold-Cadmium, Nickel-Aluminium und Mangan-Kupfer können jedoch ebenfalls geeignet sein. Darüber hinaus muss das prothetische Gewebematerial
14 kein Polypropylengeflecht sein, sondern kann auch andere geeignete Materialien umfassen, beispielsweise körperkompatible biaxial orientier te Polymerfilme. - Die größere Achse der Prothese
10 kann, ohne dass dies eine Beschränkung darstellen würde, im Bereich von etwa 6 bis 12 cm liegen, während die verengte kleinere Achse im Bereich von etwa 4 bis 8 cm liegen kann. Eine derartige Vorrichtung kann dicht zu einem Zylinder gerollt werden, wie in2 ersichtlich, um in das interne Lumen eines 10 mm oder kleineren Trokars oder einer Einführhülle zu passen.
Claims (7)
- Herniareparaturpatch mit einem Drahtrahmen (
12 ) und einem prothetischen Gewebematerial (14 ), das längs des Umfangs des Drahtrahmens (12 ) durch diesen gehaltert ist, dadurch gekennzeichnet, dass der Drahtrahmen (12 ) die Form einer geschlossenen Schleife hat, an der das prothetische Gewebematerial (14 ) befestigt ist, und dass der Drahtrahmen (12 ) derart aus einer Form-Memory-Legierung gebildet ist, dass er sich durch Erwärmung auf die Körpertemperatur des Patienten von einer zusammengerollten Form zu seiner vorbestimmten Form entfaltet. - Herniareparaturpatch nach Anspruch 1, dadurch gekennzeichnet, daß der Drahtrahmen (
12 ) aus einer im wesentlichen ovalen geschlossenen Schleife besteht, jedoch eine verengte kleinere Achse aufweist, wenn er in einer Form ist, die er bei höherer Temperatur einnimmt. - Herniareparaturpatch nach Anspruch 1, dadurch gekennzeichnet, daß die Form-Memory-Legierung NiTi aufweist, wobei der Prozentsatz von Nickel in der Legierung im Bereich zwischen 49 und 51 Atomprozent liegt.
- Herniareparaturpatch nach Anspruch 3, dadurch gekennzeichnet, daß die Form-Memory-Legierung eine Transformationstemperatur von etwa 37°C hat.
- Herniareparaturpatch nach Anspruch 1, dadurch gekennzeichnet, daß der Drahtrahmen (
12 ) eine stundenglasartige Form hat, wenn die Form-Memory-Legierung in einer Austenitform ist, und eine zusammengerollte kompakte Form, wenn sie in einer Martensitform vorliegt. - Herniareparaturpatch nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, daß das prothetische Gewebematerial aus einer Gruppe ausgewählt ist, die aus einem gewobenen Netz aus Polypropylenfasern und expandiertem PTFE besteht.
- Herniareparaturpatch nach Anspruch 5, dadurch gekennzeichnet, daß das prothetische Gewebematerial im wesentlichen planar ist und eine größere Längendimension im Bereich von 6 bis 12 cm und eine kleinere Querdimension im Bereich von 4 bis 8 cm hat.
