Abstract
Ein modulierter physiologischer Sensor ist eine nichtinvasive Vorrichtung, die auf eine physiologische Reaktion eines Lebewesens auf eine innere oder äußere Störung anspricht, die sich zu einem Hautoberflächenbereich ausbreitet. Der modulierte physiologische Sensor weist einen Detektor auf, der so konfiguriert ist, dass er als Antwort auf die physiologische Reaktion ein Signal erzeugt. Ein Modulator variiert die Kopplung des Detektors an die Haut, um zumindest in Abständen das Detektorsignal zu maximieren. Ein Überwachungsgerät steuert den Modulator und empfängt ein effektiv verstärktes Detektorsignal, das verarbeitet wird, um einen physiologischen Parameter zu berechnen, der auf die physiologische Reaktion schließen lässt.
Description
- Aus physiologischer Sicht weist der menschliche Körper eine Gruppe von interagierenden Systemen auf, die jeweils spezifische Funktionen und Zwecke haben. Diese Systeme bewahren die innere Stabilität des Körpers, indem sie die Reaktion seiner Teile auf jede Situation oder jeden Stimulus koordinieren, die (der) darauf hinauslaufen würde, seinen normalen Zustand oder seine normale Funktion zu stören. Das Nervensystem schließt das Zentralnervensystem und das periphere Nervensystem ein. Das Zentralnervensystem ist das Gehirn und das Rückenmark. Das Muskel-Skelett-System schließt das Skelett und die anhaftenden Muskeln ein und umfasst Knochen, Bänder, Sehnen und Knorpel. Das Kreislaufsystem umfasst Herz und Blutgefäße, zu denen Arterien, Venen und Kapillaren gehören. Das respiratorische System umfasst Nase, Luftröhre und Lunge. Das Gastrointestinalsystem umfasst Mund, Speiseröhre, Magen, Dünn- und Dickdarm, Leber, Pankreas und Gallenblase. Das Haut- und Hautanhangsystem umfasst Haut, Haare, Nägel, Schweißdrüsen und Talgdrüsen. Das Harnsystem umfasst Nieren und Blase. Das Immunsystem umfasst weiße Blutkörperchen, Thymus und Lymphknoten. Das endokrine System umfasst Hirnanhangdrüse, Schilddrüse, Nebenniere und Nebenschilddrüse.
- Verschiedene Sensoren können angewandt werden, um die in den oben angeführten physiologischen Systemen auftretenden Prozesse zu analysieren und zu messen und physiologische Parameter zu erzeugen, die als Ergebnis auf Gesundheit oder Wohlbefinden schließen lassen. Als ein Beispiel generiert ein Pulsoxymetrie-Sensor eine Plethysmogramm-Wellenform, aus der, neben anderen Parametern, die Sauerstoffsättigung von arteriellem Blut und die Pulsfrequenz bestimmt werden können. Als weiteres Beispiel kann ein akustischer Sensor verwendet werden, um Luftstromgeräusche in Lunge, Bronchien oder Luftröhre zu erfassen, die auf die Atemfrequenz schließen lassen.
- Die oben angeführten physiologischen Systeme bewahren die Stabilität, Balance und das Gleichgewicht eines Lebewesens. Modulation kann vorteilhaft angewandt werden, um die Erfassung von Prozessen zu verstärken, die innerhalb dieser physiologischen Systeme auftreten. Ein Beispiel für natürliche Modulation ist die Gewebevibration in der Luftröhre infolge des Zuflusses und Abflusses von Luft zwischen Lunge und Nase und Mund. Diese Vibration erzeugt Schallwellen mit höherer Frequenz als die zugrunde liegende Atmung. Ein akustischer Sensor, der eine am Hals angebrachte piezoelektrische Vorrichtung nutzt, kann diese Schallwellen erfassen und eine modulierte Schallwellenhüllkurve ausgeben, die demoduliert werden kann, um die Atemfrequenz abzuleiten. Ein akustischer Atemfrequenzsensor und ein entsprechender Sensorprozessor werden beschrieben in der US-Patentanmeldung Nr. 12/904789, eingereicht am 14. 10. 2010, mit dem Titel ”Acoustic Respiratory Monitoring Systems and Methods” (Systeme und Verfahren zur akustischen Atmungsüberwachung), zediert an die Masimo Corporation, Irvine, CA (”Masimo”) und hier durch Verweis einbezogen.
- Ein weiteres Beispiel natürlicher Modulation ist der pulsierende arterielle Blutfluss an einer peripheren Gewebestelle, wie z. B. einer Fingerspitze, der von durch das Herz erzeugten Druckwellen herrührt. Ein optischer Sensor generiert eine Plethysmogramm-Wellenform, die auf Veränderungen einer Lichtabsorption infolge des pulsierenden Blutflusses anspricht, um die Blutzusammensetzung zu messen, wie z. B. Hämoglobinbestandteile. Dieses Plethysmogramm moduliert außerdem eine Atmungshüllkurve, die demoduliert werden kann, um die Atemfrequenz abzuleiten.
- Ein Beispiel künstlicher Modulation ist ein physiologischer Sensor mit einem Beschleunigungsmesser und einem Vibrationselement, die so auf einem Träger montiert sind, dass das Vibrationselement in mechanischer Verbindung mit dem Beschleunigungsmesser steht. Eine Schnittstelle überträgt mindestens eine Achse des Beschleunigungsmessersignals zu einem Überwachungsgerät. Der Träger wird an der Hautoberfläche eines Lebewesens befestigt, und das Vibrationselement wird aktiviert, um die Verbindung zur Hautoberfläche mit einer Modulationsfrequenz zu modulieren. In einer Ausführungsform reagiert ein künstlich modulierter Sensor auf die durch die Atmung ausgelösten Bewegungen an der Hautoberfläche.
- Ein Aspekt eines modulierten physiologischen Sensors ist ein nichtinvasiver Sensor, der auf eine physiologische Reaktion eines Lebewesens auf eine innere oder äußere Störung anspricht, die sich zu einem Oberflächenbereich des Lebewesens ausbreitet. Der modulierte physiologische Sensor weist einen Detektor auf, der für Kommunikation mit einem Oberflächenbereich eines Lebewesens konfiguriert ist, um als Antwort auf eine physiologische Reaktion des Lebewesens auf die Störung ein Signal zu erzeugen. Ein Modulator variiert die Kopplung des Detektors zu dem Oberflächenbereich, um das Detektorsignal zumindest in Abständen zu maximieren. Ein Überwachungsgerät steuert den Modulator und empfängt ein Detektorsignal, um einen physiologischen Parameter zu berechnen, der auf einen physiologischen Zustand des Lebewesens schließen lässt.
- In verschiedenen Ausführungsformen ist der Modulator ein Vibrationselement, das die Kopplung des Detektors zu dem Oberflächenbereich mechanisch verstärkt. Auf einem Träger sind der Detektor und das Vibrationselement zusammen montiert. Durch eine Halterung werden Träger, Detektor und Vibrationselement lösbar an dem Oberflächenbereich befestigt. In einer Ausführungsform ist der Detektor ein Beschleunigungsmesser, und das Vibrationselement ist ein Kleinstmotor. Der Träger ist eine Leiterplatte, auf welcher der Beschleunigungsmesser und der Kleinstmotor mechanisch montiert und elektrisch verbunden sind. Die Halterung ist ein Band mit einer klebrigen Seite, die an dem Oberflächenbereich haftet, und einer Gehäuseseite, welche die Leiterplatte einschließt.
- Ein anderer Aspekt eines modulierten physiologischen Sensors ist ein Abtastverfahren, das einen Detektor bereitstellt, der auf eine in einem Lebewesen erzeugte physiologische Welle anspricht, die sich zu einer Hautoberfläche ausbreitet, und das den Detektor an die Hautoberfläche koppelt. Die Detektorkopplung wird so moduliert, dass ein moduliertes Detektorausgangssignal erzeugt wird, das auf die physiologische Welle schließen lässt. Das Detektorsignal wird demoduliert, um ein physiologisches Signal abzuleiten, und aus dem physiologischen Signal wird ein physiologischer Parameter bestimmt. In verschiedenen Ausführungsformen ist die Modulation eine Vibration des Detektors durch gemeinsame Montage des Detektors und eines Vibrationselements. Detektor und Vibrationselement können zusammen auf einem gemeinsamen Träger montiert werden, der an der Hautoberfläche befestigt wird. Ein zweiter Detektor und ein zweites Vibrationselement können auf dem gemeinsamen Träger montiert und von der Kombination aus Detektor und Vibrationselement entkoppelt werden.
- Ein weiterer Aspekt eines modulierten physiologischen Sensors ist eine Detektoreinrichtung, die auf sich ausbreitende physiologische Signale anspricht, die eine Hautoberfläche eines Lebewesens erreichen, und eine Modulatoreinrichtung, um die Kopplung der Detektoreinrichtung an die Hautoberfläche zu variieren. Ein Überwachungsgerät demoduliert ein Sensorsignal von der Detektoreinrichtung, um die sich ausbreitenden physiologischen Signale zu analysieren und einen physiologischen Ausgabeparameter zu erzeugen. In verschiedenen Ausführungsformen nimmt eine Trägereinrichtung die Detektoreinrichtung und die Modulatoreinrichtung auf, und eine Halterung sichert den Träger an der Hautoberfläche. Ein Steuersignal vom Überwachungsgerät stellt eine Frequenz der Modulatoreinrichtung oberhalb einer unteren Grenzfrequenz der Detektoreinrichtung ein. In einer Ausführungsform ist die Modulatoreinrichtung ein Vibrationselement, die Detektoreinrichtung sind mehrere Detektore, die Modulatoreinrichtung sind mehrere Vibrationselemente, und die Trägereinrichtung enthält mindestens ein Trennelement, um die Detektor/Vibrationselement-Paare voneinander zu trennen bzw. zu isolieren. In einer Ausführungsform moduliert das Vibrationselement den Detektor ferngesteuert mittels einer Schallwelle.

Physiologische Reaktionen50 auf äußere30 oder innere Störungen40 breiten sich zur Körperoberfläche12 aus und werden zu einem oder mehreren Detektoren120 gekoppelt110 . Diese physiologischen Reaktionen50 lassen auf Zustände und Prozesse der physiologischen Systeme20 schließen. Die Detektoren120 sprechen auf gekoppelte physiologische Reaktionen112 an und erzeugen Detektorausgangssignale122 . Ein oder mehrere Überwachungsgeräte20 reagieren auf die Detektorausgangssignale122 und berechnen physiologische Parameter6 , welche die Zustände und Prozesse der physiologischen Systeme20 quantifizieren. 1 dargestellt, umfassen die Detektoren120 jede Vorrichtung, die auf die gekoppelten physiologischen Reaktionen112 anspricht, wie z. B. optische, akustische, elektrische, mechanische, chemische und thermische Mechanismen, um einige zu nennen. Die Detektorausgangssignale122 können Blut-Photoplethysmogramme, EKG, EEG und Körperschallwellenformen einschließen; Anzeigen von Hautfarbe, Temperatur, Bewegung oder Druck und chemische Reaktionen und Messungen von Feuchtigkeit, Atem, Schweiß oder Gerüchen, um einige zu nennen. Das eine oder die mehreren Überwachungsgeräte20 können jedes beliebige Gerät oder jede Gerätekombination einschließen, die auf die Detektorausgangssignale122 allein oder in Kombination ansprechen, um physiologische Parameter6 zu berechnen oder auf andere Weise abzuleiten, die einen oder mehrere Aspekte der physiologischen Systeme20 und entsprechende Zustände und Prozesse, die den physiologischen Reaktionen50 entsprechen, messen, grafisch darstellen, quantitativ bestimmen oder auf andere Weise anzeigen. Beispiele von Parametern sind unter anderem Messungen des Kreislaufsystems, wie z. B. der Sauerstoffsättigung, Herzfrequenz, des Blutzuckergehalts und des Blutdrucks; und Messungen des respiratorischen Systems, wie z. B. der Atemfrequenz und des Atemvolumens, um nur einige zu nennen. Parameter6 können auch Anzeigen bestimmter anomaler physiologischer Zustände einschließen, wie z. B. Schlafapnoe, Anämie und Hypoglykämie, um einige zu nennen. - Wie gleichfalls in
1 dargestellt, können äußere Störungen30 natürlich sein, wie z. B. Änderungen der physikalischen Umgebung einer Person, zu der beispielsweise Temperatur, Druck, Licht und Geräusch gehören. Äußere Störungen30 können auch künstlich sein, wie z. B. der mechanische Druck, der beispielsweise durch ein Atemgerät zu Atmungsunterstützung oder durch einen Impulsgeber an einer Fingerspitze zur Messung der venösen Sauerstoffsättigung ausgelöst wird. Innere Störungen40 schließen normales und anomales Funktionieren und Interaktionen verschiedener physiologischer Systeme20 ein; dazu gehören Kreislauf- und Atmungsfunktionen, um einige zu nennen. Innere Störungen40 können auch künstlich sein, wie z. B. als Folge eines Schrittmachers oder eines anderen implantierten Geräts. Physiologische Reaktionen50 , die aus äußeren Störungen30 oder inneren Störungen40 resultieren, umfassen beispielsweise eine Expansion oder Kontraktion der Körperoberfläche, etwa aufgrund einer Inflation/Deflation der Lunge; eine Schallwelle, die aufgrund eines Herzschlags oder Darmgeräuschs vom Körperinneren zur Körperoberfläche gelangt; oder eine Transversalwelle, die infolge eines Muskelkrampfs entlang der Körperoberfläche wandert. Im Allgemeinen kann eine physiologische Reaktion50 ein Impuls, eine Welle oder andere Schwankung oder Veränderung optischer, akustischer, elektrischer, mechanischer, chemischer oder thermischer Natur sein. Ferner brauchen äußere Störungen30 oder innere Störungen40 nicht vom gleichen Typ oder von gleicher Art (z. B. optisch, akustisch, elektrisch, mechanisch, chemisch oder thermisch) zu sein, wie die entsprechende physiologische Reaktion50 oder das auf die physiologische Reaktion50 ansprechende Detektorelement120 . Zum Beispiel kann eine Injektion (äußere chemische Störung) eine Herzrhythmusstörung auslösen, die zu einer akustischen oder mechanischen Welle (physiologischen Reaktion) führt, die sich zur Hautoberfläche ausbreitet und durch einen akustischen oder mechanischen Sensor oder beide erfasst wird. Ferner kann die Herzrhythmusstörung zu einer arteriellen Pulsanomalie führen, welche die optischen Merkmale einer Gewebestelle verändert, die durch einen an der Gewebestelle angebrachten optischen Sensor gemessen werden. -
2 veranschaulicht eine Ausführungsform eines modulierten physiologischen Sensors200 , der an einer Körperoberfläche12 anhaftet und so konfiguriert ist, dass er auf physiologische Reaktionen50 anspricht, wie oben beschrieben. Der Sensor200 weist eine Kopplung210 , einen Detektor220 , eine Schnittstelle230 und einen Modulator240 auf. Ein Überwachungsgerät (nicht dargestellt) gibt Steuersignale232 ,234 an den Sensor200 aus und empfängt Signale232 vom Sensor200 . - Die Schnittstelle
230 überträgt als Reaktion auf Treibersignale222 zum Detektor220 Detektorsignale222 zum Überwachungsgerät. Die Schnittstelle230 überträgt außerdem ein Modulatorsteuersignal242 zum Modulator240 . Der Modulator240 reagiert auf das Modulatorsteuersignal242 , um eine Modulation244 für die Kopplung210 zu erzeugen. - Wie in
2 dargestellt, variiert der Modulator240 die Kopplung210 des Detektors220 zur Körperoberfläche12 und daher zu der physiologischen Reaktion50 . Insbesondere verändert sich die Körperoberfläche12 einer Person, einschließlich Haut und darunter liegender Gewebe, mit dem Individuum und tatsächlich mit dem Ort auf einem bestimmten Individuum. Diese Veränderungen betreffen die Form, Textur, Farbe und Elastizität, um einige zu nennen. An sich ist es unwahrscheinlich, dass eine feste Kopplung eine optimale Schnittstelle zwischen Körperoberfläche und Detektor liefert. In der Tat ist die effiziente und effektive Kopplung zwischen Körperoberfläche und Detektor für die meisten, wenn nicht alle physiologischen Sensoren ein Problem. Zum Beispiel erfordern übliche EKG-Elektroden ein leitfähiges Gel für eine wirksame Kopplung zu einer Hautoberfläche. Der Modulator240 variiert vorteilhaft die Detektorkopplung210 zur Hautoberfläche kontinuierlich über einen Bereich von Kontaktkräften an der Haut/Sensor-Schnittstelle. Für einen elektrischen Detektor etwa verändert diese variierte Kopplung den elektrischen Widerstand des Detektors an der Hautoberfläche über einen Wertebereich. Für einen mechanischen Detektor verändert die variierte Kopplung die mechanische Impedanz des Detektors an der Hautoberfläche über einen Wertebereich. Für einen akustischen Detektor beispielsweise verändert die variierte Kopplung die akustische Impedanz des Detektors an der Hautoberfläche über einen Wertebereich. Als Ergebnis dieser variablen Detektorkopplung zur Hautoberfläche weist der Detektor in jedem Modulationszyklus maximale und minimale Kopplung auf. Ferner kann die Modulationsfrequenz oberhalb etwaiger unterer Ansprechgrenzfrequenzen des Detektors eingestellt werden. Dementsprechend verstärkt die Modulation günstigerweise das Detektorsignal222 , wie nachstehend in Bezug auf die3A –D ausführlicher beschrieben. - Die
3A –D veranschaulichen eine Reaktion physiologischer Systeme auf Störungen und eine entsprechende modulierte und erfasste Abtastung der Reaktion.3A zeigt ein typisches Zeitbereichsdiagramm310 einer physiologischen Systemreaktion301 mit relativ niedriger Amplitude und niedriger Frequenz auf irgendeine Form einer inneren oder äußeren Störung.3B zeigt ein entsprechendes typisches Frequenzbereichsdiagramm320 der physiologischen Systemreaktion301 . Die physiologische Reaktion301 kann schwer nachweisbar sein, entweder wegen eines kleinen Amplitudensignals301 oder wegen einer Signalfrequenz fr302 , die niedriger ist als die Detektorgrenzfrequenz fc304 , d. h. außerhalb des Detektordurchlassbereichs303 liegt. -
3C zeigt ein typisches Zeitbereichsdiagramm330 einer modulierten Detektorantwort305 auf die oben beschriebene Reaktion301 (3A ). Die Antwort305 weist eine Modulation306 und eine Hüllkurve307 auf. Insbesondere weist der physiologische Sensor200 (2 ) eine modulierte Kopplung210 (2 ) auf, die mindestens einmal pro Modulationszyklus eine maximale Kopplung eines Detektors220 (2 ) zu einer Körperoberfläche12 (2 ) erreicht oder annähert, wie oben in Bezug auf2 beschrieben. Dementsprechend verstärkt der modulierte Detektor220 (2 ) das physiologische Signal301 (3A ) während der maximalen Kopplung und schwächt das physiologische Signal301 (3A ) während der minimalen Kopplung. Diese zyklische Verstärkung/Abschwächung erzeugt eine Hüllkurve307 , die effektiv eine Verstärkung der physiologischen Reaktion301 ist (3A ). -
3D zeigt ein typisches Frequenzbereichsdiagramm340 einer modulierten physiologischen Sensorantwort305 (3C ). in verschiedenen Ausführungsformen wird die Modulationsfrequenz fmod308 wesentlich höher eingestellt als eine etwaige untere Grenzfrequenz fc304 des Detektors, so dass die Sensorantwort305 völlig innerhalb des Detektordurchlassbereichs303 liegt (3B ). - Wie in Bezug auf die
3A –D beschrieben, wird in verschiedenen Ausführungsformen eine verstärkte Version der physiologischen Reaktion301 (3A ) aus der Sensorantwort305 (3C ) durch eines von verschiedenen bekannten AM-Demodulationsverfahren abgeleitet. Dazu gehören die Hüllkurvendetektion mit einem Gleichrichter oder die Produktdetektion mittels Multiplikation durch einen lokalen Oszillator, um einige zu nennen. -
4 veranschaulicht eine Ausführungsform eines vibrationsmodulierten physiologischen Sensors400 . Der Sensor400 weist einen Detektor410 , ein Vibrationselement (”vib”)420 , einen Träger430 und eine Schnittstelle440 zu einem Überwachungsgerät auf. Der Detektor410 und das Vibrationselement (vib)420 sind beide auf dem Träger430 montiert. In einer Ausführungsform ist der Detektor410 so montiert, dass er direkt an die Körperoberfläche12 ankoppelt401 . Zum Beispiel kann der Detektor410 , wie dargestellt, durch den Träger430 durchgehend montiert werden. In anderen Ausführungsformen wird der Detektor410 benachbart zum Träger430 befestigt. In weiteren Ausführungsformen kommt der Detektor410 unter Umständen überhaupt nicht in Kontakt mit der Körperoberfläche12 , wie z. B. bei einem Detektor auf Beschleunigungsmesser-Basis, der weiter unten in Bezug auf die7 –10 beschrieben wird. In einer Ausführungsform ist das Vibrationselement420 ein Kleinstmotor, wie weiter unten in Bezug auf die7 –10 beschrieben. In anderen Ausführungsformen ist das Vibrationselement420 eine(r) von verschiedenen Unwuchtmotoren, Schwingspulen oder ähnlichen elektromechanischen Vorrichtungen. In weiteren Ausführungsformen ist das Vibrationselement420 irgendeine mechanische, elektromagnetische, piezoelektrische, pneumatische, elektrische, akustische oder magnetische Vorrichtung, die als Reaktion auf ein elektrisches Signal vibriert. - Wie in
4 dargestellt, wird der Detektor410 und daher die Kopplung401 über den Träger430 vibrationsmoduliert420 . Der Träger430 kann irgendein Material sein, das Vibrationen effizient vom Vibrationselement420 zum Detektor401 überträgt oder leitet. In einer vorteilhaften Ausführungsform ist der Träger430 ein Leiterplattenmaterial, das mechanische Halterungen für Sensorkomponenten bereitstellt und elektrische Verbindungen zwischen ihnen trägt. - Wie gleichfalls in
4 dargestellt, gibt ein Überwachungsgerät (nicht dargestellt) Steuersignale442 ,444 an den Sensor400 aus und empfängt Signale442 vom Sensor400 . Als Reaktion auf Antriebssteuersignale412 zum Detektor420 überträgt die Schnittstelle440 Detektorsignale412 zum Überwachungsgerät. Die Schnittstelle440 überträgt außerdem ein Vibrationssteuersignal422 zum Vibrationselement420 . Das Vibrationselement420 reagiert auf das Vibrationssteuersignal422 , indem es über den Träger430 eine Modulation der Kopplung401 generiert. In verschiedenen Ausführungsformen kann der Detektor410 mechanisch sein, wie z. B. ein weiter unten in Bezug auf die7 –10 beschriebener Beschleunigungsmesser. In anderen Ausführungsformen kann der Detektor410 elektrisch sein, wie z. B. eine Elektrode zum Abtasten von EKG- oder EEG-Signalen; oder optisch, wie z. B. eine Photodiode, oder akustisch, wie z. B. eine piezoelektrische Vorrichtung; oder thermisch, wie z. B. eine Thermosäule, ein Pyrometer, ein Thermistor, ein Thermoelement, eine IR-Photodiode oder Temperaturdiode, um einige zu nennen. -
5 veranschaulicht eine Ausführungsform eines vibrationsmodulierten Mehrelementsensors500 mit zwei oder mehreren Sensorelementen510 ,520 , einem oder mehreren Vibrationselementen (vibs)530 ,540 , einem Träger550 und einer Schnittstelle560 zu einem Überwachungsgerät. Die Sensorelemente510 ,520 können jeweils Detektoren oder eine Kombination aus einem oder mehreren Detektoren und einem oder mehreren Emittern sein. In einer Ausführungsform sind die Sensorelemente510 ,520 unterschiedliche Detektortypen. Zum Beispiel kann das Element1510 mechanisch sein, und das Element2 kann elektrisch sein. In einer Ausführungsform können die Sensorelemente510 ,520 ein Emitter und ein entsprechender Detektor sein. Zum Beispiel kann das Element1510 eine Leuchtdiode (LED) zum Beleuchten einer Gewebestelle sein, und das Element2520 kann ein optischer Detektor sein, wie z. B. eine Diode oder eine Diodenmatrix zum Empfang des LED-Lichts nach Abschwächung durch die Gewebestelle. Günstigerweise bilden mehrere Elemente510 ,520 auf einem einzigen Träger550 eine Matrix von gleichartigen Sensoren für eine erhöhte Nachweisfähigkeit oder für richtungsabhängige Abtastfähigkeit, wie etwa zur Bestimmung der Quelle eines Körpergeräuschs, um nur ein Beispiel anzuführen. Günstigerweise bilden mehrere Elemente510 ,520 auf einem einzigen Träger550 eine Matrix von unterschiedlichen Sensoren in einer einzigen Sensorbaugruppe bzw. einem einzigen Sensorpaket für gleichzeitige Erfassung und Analysen von mehreren Typen oder Arten physiologischer Reaktionen auf die gleichen oder verschiedene äußere oder innere Störungen. - Wie in
5 dargestellt, können mehrere Vibrationselemente530 ,540 durch einen Trägerisolator570 getrennt werden. Auf diese Weise bewirkt das Vibrationselement1530 ausschließlich die Kopplung501 des Elements1510 an eine Körperoberfläche12 , und ebenso bewirkt das Vibrationselement2540 ausschließlich die Kopplung502 des Elements2520 an eine Körperoberfläche12 . Mehrere isolierte Vibrationselemente530 ,540 ermöglichen günstigerweise, dass jedes Vibrationselement530 ,540 bezüglich Amplitude und Frequenz an ein bestimmtes Element510 ,520 angepasst oder auf andere Weise darauf abgestimmt wird. In einer Ausführungsform ist der Isolator570 ein Material, das mechanische/akustische Wellen bei der einen oder den mehreren Frequenzen des Vibrationselements signifikant abschwächt. - Wie gleichfalls in
5 dargestellt, gibt ein Überwachungsgerät (nicht dargestellt) Steuersignale562 zum Sensor500 aus und empfängt Signale562 vom Sensor500 . Als Reaktion auf Antriebssteuersignale512 ,522 überträgt die Schnittstelle560 Elementsignale512 ,522 zum Überwachungsgerät. Die Schnittstelle560 überträgt außerdem Vibrationssteuersignale (vib-Steuersignale)564 zu den Vibrationselementen (vibs)530 ,540 . Die Vibrationselemente530 ,540 reagieren auf die Vibrationssteuersignale564 , indem sie eine Modulation ihrer jeweiligen Kopplungen501 ,502 generieren. - Die
6A –F veranschaulichen verschiedene modulierte physiologische Sensorkonfigurationen. Wie in6A dargestellt, weist eine Ausführungsform eines integrierten Sensors610 einen Träger612 , einen Detektor614 , ein Vibrationselement (vib)615 , einen E/A (Eingang/Ausgang)617 , eine Halterung618 und eine elektrische Verbindung619 zu einem Überwachungsgerät oder ähnlichen Gerät (nicht dargestellt) auf. Der Träger612 nimmt den Detektor614 , das Vibrationselement615 und den Eingang/Ausgang617 auf. In einer Ausführungsform stellt der Träger612 außerdem Zwischenverbindungen für elektrische Leiterbahnen zwischen dem Eingang/Ausgang und dem Detektor614 sowie dem Vibrationselement615 bereit. Der Eingang/Ausgang617 sendet/empfängt Sensorsignale/Steuersignale und insbesondere Antriebssignale zum Vibrationselement615 und Signale vom Detektor614 . Die Halterung618 befestigt bzw. klebt den Träger612 und die darauf montierten Komponenten614 –617 an eine Körperoberfläche an. In einer Ausführungsform wird der Detektor614 durch den Träger612 durchgehend montiert, um ihn direkt oder über die Halterung618 an eine Körperoberfläche anzukoppeln. Das Vibrationselement615 moduliert günstigerweise die Kopplung des Detektors614 an die Körperoberfläche über den Träger612 , auf dem der Detektor614 und das Vibrationselement615 zusammen montiert sind. - Wie in
6B dargestellt, weist eine semiintegrierte Ausführungsform620 des Sensors einen Träger622 , einen Detektor624 , ein Vibrationselement625 , einen Eingang/Ausgang627 , eine Halterung628 und eine elektrische Verbindung629 zu/von einem Überwachungsgerät oder einem anderen Steuerungs- oder Anzeigegerät auf. Die semiintegrierte Sensor-Ausführungsform620 ist der integrierten Sensor-Ausführungsform610 ähnlich, außer dass der Eingang/Ausgang627 außerhalb des Sensors620 liegt und im Überwachungsgerät (nicht dargestellt) oder in einer Kapsel (nicht dargestellt) zwischen dem Sensor620 und dem Überwachungsgerät montiert werden kann. Der Eingang/Ausgang627 steht in elektrischer Verbindung626 mit dem Detektor624 und dem Vibrationselement625 , beispielsweise über Verkabelung oder eine andere Verbindungstechnologie. Der Eingang/Ausgang627 steht außerdem in elektrischer Verbindung629 mit einem Überwachungsgerät. - Wie in
6C dargestellt, weist eine trägerlose Ausführungsform eines Sensors630 einen Detektor634 , ein Vibrationselement635 , einen Eingang/Ausgang637 , eine Halterung638 und eine elektrische Verbindung639 auf, die Signale und Steuersignale zwischen dem Eingang/Ausgang637 und einem Überwachungsgerät oder ähnlichen Gerät (nicht dargestellt) überträgt. In dieser Ausführungsform wird ein Träger durch den Detektor634 oder genauer die Detektorbaugruppe ersetzt, wie z. B. durch einen Chipträger. Dementsprechend sind das Vibrationselement635 und der Eingang/Ausgang637 innerhalb der Detektorbaugruppe634 oder darauf montiert oder auf andere Weise direkt damit gekoppelt, so dass die Detektorbaugruppe634 direkt an die Körperoberfläche angekoppelt ist und mit der Halterung638 in Position gehalten wird. In einer Ausführungsform ist die Halterung638 einfach eine Haftschicht an der Detektorbaugruppe634 . - Wie in
6D dargestellt, weist eine Sensormatrix-Ausführungsform640 einen Träger642 , mehrere Detektoren644 , ein Vibrationselement645 , einen Eingang/Ausgang647 , eine Halterung648 und eine elektrische Verbindung649 auf. Die Sensormatrix-Ausführungsform640 ist der semiintegrierten Ausführungsform620 (6B ) ähnlich, mit Ausnahme der mehreren Detektoren644 . Die Detektoren644 können alle der gleiche Gerätetyp sein (mechanisch, elektrisch, akustisch usw.), können alle verschieden oder eine Mischung aus einer oder mehreren Untergruppen vom gleichen Gerätetyp mit einem oder mehreren unterschiedlichen Gerätetypen sein. Günstigerweise bilden mehrere Detektoren644 auf einem einzigen Träger642 eine Matrix von gleichartigen Sensoren für eine erhöhte Nachweisfähigkeit oder für Richtungs-Abtastfähigkeit, wie etwa zur Bestimmung der Quelle eines Körpergeräuschs. Günstigerweise bilden mehrere Detektoren644 auf einem einzigen Träger642 eine Matrix von unterschiedlichen Detektoren in einer einzigen Sensorbaugruppe für gleichzeitige Erfassung und Analysen von mehreren Typen oder Arten physiologischer Reaktionen auf die gleichen oder verschiedene äußere oder innere Störungen. Günstigerweise stellt eine Mischung von Detektoren und Transmittern (nicht dargestellt), wie z. B. eine oder mehrere LEDs und ein oder mehrere Photodioden-Detektoren, aktive Abtastfähigkeiten bereit, wie z. B. Ausleuchtung und Analyse von arteriellem (pulsierendem) Blutfluss. Günstigerweise können ein oder mehrere Vibrationselemente645 sowohl Modulation als auch einen aktiven Puls bereitstellen, etwa zur Analyse von nicht pulsierendem (venösem) Blutfluss, um nur ein Beispiel anzuführen. - Wie in
6E dargestellt, weist eine nicht integrierte Sensor-Ausführungsform650 einen Detektor654 , ein Vibrationselement655 und Halterungen658 auf. Der Detektor654 und das Vibrationselement655 sind getrennt658 an einer Körperoberfläche angebracht. Der Eingang/Ausgang657 steht in elektrischer Verbindung656 mit dem Detektor654 und dem Vibrationselement655 , beispielsweise über Verkabelung oder eine andere Verbindungstechnologie, einschließlich drahtloser Verbindung. Ferner liegt der Eingang/Ausgang657 außerhalb des Sensors650 und kann in dem Überwachungsgerät (nicht dargestellt) oder in einer Kapsel (nicht dargestellt) zwischen dem Sensor650 und dem Überwachungsgerät mit elektrischen Verbindungen659 zwischen dem E/A657 und dem Überwachungsgerät montiert werden. Günstigerweise ist das Vibrationselement655 an der Körperoberfläche in unmittelbarer Nähe zum Detektor654 angebracht, so dass durch das Vibrationselement im Körper erzeugte Oberflächenwellen601 die Kopplung zwischen dem Detektor654 und der Körperoberfläche modulieren. - Wie in
6F dargestellt, weist eine berührungslos arbeitende Sensor-Ausführungsform660 einen Detektor664 und einen Modulatorbaustein665 auf. Der Modulatorbaustein668 weist ein Vibrationselement665 und einen Eingang/Ausgang667 auf. Günstigerweise moduliert das Vibrationselement665 berührungslos den Detektor664 , wenn es in die Nähe des Detektors664 gebracht wird. Insbesondere erzeugt das Vibrationselement665 eine Schallwelle602 , die den Detektor in Vibration versetzt, um die Detektorkopplung zur Körperoberfläche zu modulieren. Insbesondere breitet sich die Schallwelle602 durch ein zwischen dem Vibrationselement665 und dem Detektor664 liegendes Medium aus. Dieses Medium kann ein Luftspalt sein, wenn der Baustein668 unmittelbar über dem Detektor664 positioniert ist, oder das Medium kann Gewebe sein, wenn der Baustein668 unmittelbar über oder auf der Körperoberfläche nahe dem Detektor664 positioniert ist. -
7 veranschaulicht allgemein eine Ausführungsform eines modulierten physiologischen Sensors700 mit einem Beschleunigungsmesser710 und einem Vibrationselement (vib)720 , die auf einem gemeinsamen Träger730 montiert sind. Eine Halterung (nicht dargestellt) klebt oder koppelt den Träger730 auf andere Weise an eine Körperoberfläche12 an. Der Beschleunigungsmesser710 hat drei Ausgangssignale712 , die auf Beschleunigungen in drei Dimensionen (x, y, z) ansprechen und günstigerweise den Sensor700 befähigen, die Amplitude, Richtung und/oder Ausbreitungsart (translatorisch85 ,87 und longitudinal86 ,88 ) zu erfassen und festzustellen, ob die sich ausbreitenden Wellen Körperwellen85 ,86 oder Oberflächenwellen87 sind. Das Vibrationselement720 moduliert mechanisch die Kopplung des Trägers730 und entsprechend die Kopplung des Beschleunigungsmessers710 an die Körperoberfläche12 . Die Frequenz des Vibrationselements720 wird so gewählt, dass sie wesentlich höher ist als die Frequenz der sich ausbreitenden Wellen85 –88 . An sich sind die x-, y- und z-Ausgangssignale712 des Beschleunigungsmessers jeweils amplitudenmodulierte (AM) Darstellungen der Ausbreitungen85 –87 . Günstigerweise verstärkt die modulierte Kopplung die Ausbreitungen erheblich wegen einer maximalen Wechselstromkopplung, die einmal in jedem Zyklus des Vibrationselements auftritt. Diese maximale Wechselstromkopplung ist erheblich größer, als sie mit irgendeiner statischen Kopplung des Beschleunigungsmessers an die Körperoberfläche12 praktisch erzielt werden kann. Dementsprechend können Ausbreitungen mit sehr niedriger Amplitude erfasst und gemessen werden, um physiologische Parameter zu liefern. Siehe zum Beispiel einen Atemfrequenzsensor, der nachstehend in Bezug auf8 –10 beschrieben wird. -
8 zeigt ein detailliertes Blockdiagramm einer Ausführungsform eines vibrationsmodulierten physiologischen Sensors800 . Der Sensor800 weist eine Halterung810 , einen Träger820 , einen Beschleunigungsmesser830 , einen Kleinstmotor840 , der eine Vibrationsmodulation generiert, eine Beschleunigungsmesserschnittstelle850 , eine Geschwindigkeitssteuerung860 und Überwachungsgerät-Eingänge/Ausgänge (E/A)801 ,802 auf. In einer Ausführungsform ist der Beschleunigungsmesser830 ein 3-achsiger (X, Y und Z) linearer LIS352AX-Beschleunigungsmesser mit ±2 g Skalenendwert und analogem Ausgang, beziehbar von STMicroelectronics, Genf, Schweiz. In einer Ausführungsform ist der Kleinstmotor840 ein 10 mm-Kleinstmotor310 –101 , beziehbar von Precision Microdrives Ltd., London, UK. In einer Ausführungsform ist der Träger820 ein Leiterplattenmaterial, auf dem der Beschleunigungsmesser830 , der Kleinstmotor840 , die Beschleunigungsmesserschnittstelle850 und die Geschwindigkeitssteuerung860 mechanisch montiert und elektrisch miteinander verbunden sind. In einer Ausführungsform ist die Halterung810 ein Klebestreifen, der den Sensor800 an einer Körperoberfläche eines Lebewesens befestigt. In einer Ausführungsform ist der Überwachungsgerät-Eingang/Ausgang802 von/zur Geschwindigkeitssteuerung über einen I2C-Bus. In einer Ausführungsform schließt der Überwachungsgerät-Eingang/Ausgang801 vom/zum Beschleunigungsmesser830 einen Multiplexer-Steuereingang zum Beschleunigungsmesser830 ein, um eine der Achsen X, Y und Z für den Beschleunigungsmesser-Ausgang832 zum Überwachungsgerät auszuwählen. In einer anderen Ausführungsform werden alle Achsen X, Y und Z gleichzeitig am Beschleunigungsmesser-Ausgang832 bereitgestellt. - Die
9A –B zeigen Darstellungen einer Ausführungsform eines vibrationsmodulierten (vib) physiologischen Sensors900 im zusammengesetzten bzw. explodierten Zustand, der an einer Hautoberfläche in der Nähe verschiedener Teile des Körpers einer Person befestigt werden kann, wie z. B. des Brustkorbs, der Rippen, des Magens, der Taille, der Arme oder des Rückens, um beispielsweise atmungsbezogene Parameter zu bestimmen. In einer anderen Ausführungsform kann ein modulierter physiologischer Sensor900 einen mit dem Beschleunigungsmesser und dem Vibrationselement kombinierten optischen Sensor (Strahler und Detektor) aufweisen. Auf diese Weise kann der Sensor physiologische Messungen des pulsierenden Blutflusses für eine Analyse der Blutbestandteile, physiologische Messungen des nicht pulsierenden (venösen) Blutflusses der durch das Vibrationselement künstlich zum Pulsieren gebracht wird, und Atmungsmessungen generieren, die auf plethysmographisch modulierten Wellenformen optischer Sensoren oder vibrationsmodulierten mechanischen (Beschleunigungsmesser-)Wellenformen oder auf beiden basieren. -
10 zeigt ein Ausgangssignal1000 eines physiologischen Vibrations-Beschleunigungsmesser-Sensors, das Atmungshüllkurven-Amplituden1010 in drei Achsenrichtungen als Funktion der Zeit1020 darstellt. Die Vibration moduliert kontinuierlich die Kopplung des Beschleunigungsmessers an die Haut, wodurch die gemessene Beschleunigung infolge Atmung effektiv mit der Beschleunigung infolge der Vibration multipliziert wird. Dadurch ergeben sich amplitudenmodulierte Wellenformen1001 –1003 , die eine (stark vergrößerte) Atmungshüllkurve anzeigen. Dieser Effekt wird beim Vergleich der Differenz in der Beschleuniger-Reaktion bei eingeschalteter1012 und ausgeschalteter1014 Vibration (Kopplungsmodulator) zur Genüge veranschaulicht. - Es gibt verschiedene Anwendungen für einen modulierten physiologischen Sensor, wie oben beschrieben. Ein am Brustkorb montierter Sensor könnte zur Überwachung auf Schlafapnoe zu Hause eingesetzt werden, ebenso wie im Krankenhaus für Patienten in den allgemeinmedizinischen Stationen, die Narkosemittel erhalten. Ein am Unterleib angebrachter Sensor könnte Darmgeräusche überwachen, um eine quantifizierbare Messung der Peristaltik zu ergeben. Eine Doppelsensor-Konfiguration mit einem am oberen Teil und einem am unteren Teil des Abdomens angebrachten Sensor wird zur Diagnose von Darmverschluss, Dünndarmverschlingung oder Intussuszeption (Darminvagination) eingesetzt. Ein über der Speichenarterie montierter Sensor würde eine semikontinuierliche Blutdruckmessung liefern. Eine weitere Konfiguration ist ein Screening-Werkzeug für subklinische Stenose von großen Blutgefäßen. Statt beispielsweise ein Stethoskop über den Halsschlagadern oder dem Abdomen anzusetzen, um den Durchfluss durch die Aorta abzuhören, könnte ein modulierter Sensor eine besser quantifizierbare Messung von Stenose liefern, eine Stufe besser als Auskultation, aber eine Stufe unter einem bildgebenden Verfahren. Eine weitere Anwendung ist die Differentialdiagnose von Herzgeräuschen, unterstützt durch Geräuschunterdrückung der Atmung und anderer mechanischer Bewegungen, um charakteristische Geräuschmuster zu unterscheiden (z. B. Crescendo/Decrescendo).
- Ein modulierter physiologischer Sensor ist in Verbindung mit verschiedenen Ausführungsformen ausführlich offenbart worden. Diese Ausführungsformen werden lediglich als Beispiele offenbart und sollen den Umfang der nachstehenden Ansprüche nicht einschränken. Für den Durchschnittsfachmann werden viele Veränderungen und Modifikationen ersichtlich sein.

- Modulierter physiologischer Sensor, der auf eine physiologische Reaktion eines Lebewesens auf eine innere oder äußere Störung anspricht, die sich zu einem Oberflächenbereich des Lebewesens ausbreitet und durch einen Detektor erfasst wird, wobei der modulierte physiologische Sensor aufweist: einen Detektor, der so konfiguriert ist, dass er mit einem Oberflächenbereich eines Lebewesens kommuniziert, um ein Signal zu erzeugen, das auf eine physiologische Reaktion des Lebewesens auf eine Störung anspricht; einen Modulator, der die Kopplung des Detektors zu dem Oberflächenbereich variiert, um zumindest in Abständen das Detektorsignal zu maximieren; und ein Überwachungsgerät, das den Modulator steuert und das Detektorsignal empfängt, um einen physiologischen Parameter zu berechnen, der auf einen physiologischen Zustand des Lebewesens schließen lässt.
- Modulierter physiologischer Sensor nach Anspruch 1, wobei der Modulator ein Vibrationselement ist, das die Kopplung des Detektors zu dem Oberflächenbereich mechanisch verstärkt.
- Modulierter physiologischer Sensor nach Anspruch 2, der ferner einen Träger aufweist, auf dem der Detektor und das Vibrationselement zusammen montiert sind.
- Modulierter physiologischer Sensor nach Anspruch 3, der ferner eine Halterung aufweist, die den Träger, den Detektor und das Vibrationselement lösbar an dem Oberflächenbereich befestigt.
- Modulierter physiologischer Sensor nach einem der Ansprüche 1 bis 4, wobei der Detektor ein Beschleunigungsmesser ist.
- Modulierter physiologischer Sensor nach einem der Ansprüche 2 bis 5, wobei das Vibrationselement ein Kleinstmotor ist.
- Modulierter physiologischer Sensor nach Anspruch 6, wobei der Träger eine Leiterplatte ist, auf welcher der Beschleunigungsmesser und der Kleinstmotor mechanisch montiert und elektrisch verbunden sind.
- Modulierter physiologischer Sensor nach Anspruch 7, wobei die Halterung ein Band mit einer klebrigen Seite, die an dem Oberflächenbereich anhaftet, und einer Gehäuseseite ist, welche die Leiterplatte einschließt.
- Moduliertes physiologisches Abtastverfahren, das aufweist: Bereitstellen eines Detektors, der auf eine innerhalb eines Lebewesens erzeugte physiologische Welle anspricht, die sich zu einer Hautoberfläche ausbreitet; Ankoppeln des Detektors an die Hautoberfläche; Modulieren der Detektorkopplung, um ein moduliertes Detektorausgangssignal zu generieren, das auf die physiologische Welle schließen lässt; Demodulieren des Detektorsignals, um ein physiologisches Signal abzuleiten; und Bestimmen eines physiologischen Parameters aus dem physiologischen Signal.
- Moduliertes physiologisches Abtastverfahren nach Anspruch 9, wobei das Modulieren Vibrieren des Detektors beinhaltet.
- Moduliertes physiologisches Abtastverfahren nach Anspruch 10, wobei das Vibrieren die gemeinsame Montage des Detektors mit einem Vibrationselement beinhaltet.
- Moduliertes physiologisches Abtastverfahren nach Anspruch 11, wobei die gemeinsame Montage das Montieren des Detektors und des Vibrationselements auf einem gemeinsamen Träger beinhaltet.
- Moduliertes physiologisches Abtastverfahren nach Anspruch 12, das ferner die Befestigung des gemeinsamen Trägers an der Hautoberfläche aufweist.
- Moduliertes physiologisches Abtastverfahren nach einem der Ansprüche 12 oder 13, das ferner aufweist: Montieren eines zweiten Detektors und eines zweiten Vibrationselements auf dem gemeinsamen Träger; und Trennen der Kombination aus Detektor und Vibrationselement von der Kombination aus dem zweiten Detektor und dem zweiten Vibrationselement.
- Modulierter physiologischer Sensor, der aufweist: eine Detektoreinrichtung zur Reaktion auf physiologische Ausbreitungen, die eine Hautoberfläche eines Lebewesens erreichen; eine Modulatoreinrichtung zum Variieren der Kopplung der Detektoreinrichtung an die Hautoberfläche; und ein Überwachungsgerät, das ein Sensorsignal von der Detektoreinrichtung demoduliert, um die physiologischen Ausbreitungen zu analysieren und einen physiologischen Ausgabeparameter zu erzeugen.
- Modulierter physiologischer Sensor nach Anspruch 15, der ferner aufweist: eine Trägereinrichtung zur Montage der Detektoreinrichtung und der Modulatoreinrichtung, und eine Halterungseinrichtung zum Befestigen des Trägers an der Hautoberfläche.
- Modulierter physiologischer Sensor nach Anspruch 15 oder 16, der ferner ein Steuersignal von dem Überwachungsgerät aufweist, das eine Frequenz der Modulatoreinrichtung oberhalb einer unteren Grenzfrequenz der Detektoreinrichtung einstellt.
- Moduliertes physiologisches Abtastverfahren nach einem der Ansprüche 15 bis 17, wobei die Modulatoreinrichtung ein Vibrationselement ist.
- Moduliertes physiologisches Abtastverfahren nach einem der Ansprüche 16 bis 18, wobei: die Detektoreinrichtung eine Mehrzahl von Detektoren ist; die Modulatoreinrichtung eine Mehrzahl von Vibrationselementen ist; und die Trägereinrichtung mindestens ein Trennelement enthält, um Detektor- und Vibrationselement-Paare zu trennen.
- Moduliertes physiologisches Abtastverfahren nach Anspruch 18 oder 19, wobei das Vibrationselement den Detektor ferngesteuert mittels einer Schallwelle moduliert.
