Bekannter technischer HintergrundKnown technical background
Severe disturbances of the automatic movement - the so-called Parkinson's syndrome - occur more frequently with increasing age.
The cerebral deficiency of the neurotransmitter dopamine necessary for extrapyramidal motor activity in the basal ganglia of the brain of the patients, especially in the corpus striatum, as a result of nigrostrial degeneration of unknown aetiology leads to an imbalance between the dopaminergic and cholinergic neurotransmitter systems mediating the movement.
·A substitution of the missing biogenic amine can be realized by supplying its blood-brain barrier penetrating precursor levodopa, which is taken up in the dopaminergic neurons and decarboxylated to dopamine (Birkmayer, Hornkiewicz, Vienna, Klin. Wochenschrift 73 (1961), 787). It is known that the peripheral dopa decarboxylation of levodopa is largely suppressed by the simultaneous oral administration of a suitable decarboxylase inhibitor, such as carbidopa (DE-PS 3012 602, US-PS 3,769,424) or benserazide (DE-PS 3235093), resulting in a significant increase in serum levodopa levels, which allows for a relevant reduction in the therapeutically necessary dose and associated reduction of gastrointestinal and cardiovascular side effects. The production of levodopa and carbidopa-containing capsules, film-coated tablets and gastric fluid-buoyant tablets is known. PS 1243474, US-PS 4,424,235, BE-PS 894376) It is also known that international standards so far require only a very rapid in vitro liberation (USP XXI). Also known are methods for the preparation of medicaments from which the drugs levodopa and carbidopa are released slowly and simultaneously. These medicaments can be, for example, polymeric matrices (EP-PS 0253490, EP-PS 0320051), pellets (DE-PS 3841955, EP-PS 0260236, EP-PS 0324947) or multilayer molded articles (EP-PS 0302693, EP-PS 0314206) his. Furthermore, it is known that the conversion of a drug or a drug mixture to the dosage form is an essential means of influencing the bioavailability. At the same time, it is a method for making it possible to handle very low drug doses by means of suitable pharmaceutical-technological auxiliaries or to counteract drug incompatibilities in drug mixtures or to ensure the chemical stability of one or more drugs. Furthermore, a large number of processes are known for the production of tablets, the aim of which is to convert powdered medicaments or mixtures of powdered pharmaceutical substances and pharmaceutical, ceutical-technological auxiliaries under technical conditions into tablets. It is known that the properties of the compositions, such as stability, electrostatic chargeability, flowability and tableting behavior as well as their bioavailability, can be influenced and changed by adding suitable pharmaceutical-technological auxiliaries to drugs or drug-excipient mixtures. ·
ä. in Hartgelatinekapseln beschrieben. Described in hard gelatin capsules. Furthermore, it is known that a significant delay of the drug liberating can be achieved by the addition of polyvinyl alcohols (U. Meyer, Diss., Berlin 1977) and that polyvinyl alcohol generally fast disintegrating solid tablets can be produced (W. Rietschel, "The Tablet", 104, Aulendorf, 1966).

The considerable variability of the clinical picture of Parkinson's disease requires a sensitive, individually adjustable drug therapy.
On the one hand, drug therapies must be available to the therapist, which can compensate for transmitter deficits that have occurred during the course of a day by immediate complete liberation of the drugs levodopa and carbidopa, and can thus achieve a rapid improvement in the overall condition of the patients.
On the other hand, however, occur in a large number of patients and as a result of the rapid release of active ingredient long-term therapy with subsequent high levodopa plasma levels unwanted and unpleasant, the health of severely debilitating side effects.
The invention has for its object to develop a perorally administrable drug form and a process for their preparation, according to which from Levodopa and Carbidopa tablets and capsules can be prepared from which the drugs controlled in the flooding period of about 30-45min and another desired Period completely and evenly, but not significantly delayed, released.
According to the invention, this object is achieved in that 100 to 250 parts by weight of levodopa and 10 to 25 parts by weight Carbidopa with a polymer mixture consisting of 0 to 100 parts by weight of a fully saponified polyvinyl alcohols with 0-3% vinyl acetate content, a mialeren molecular weight of 60,000 to 80,000, a total surface of 0.1 mVg to 0.18m 2 / g and 100 to 0 parts by weight of a partially saponified polyvinyl alcohols having 10 to 18% vinyl acetate content, an average molecular weight of 80,000, a total surface area of 0.5m 2 / g to 0.69mVg and a pecular pore volume from 0.2 cm 3 / g to 0.36 cm 3 / g, in proportions of from 10 to 200%, based on the amount of drugs. This drug-polymer mixture are added to conventional galenic adjuvants in an amount which makes it possible to prepare the dosage form in a manner known per se, for example by direct tableting, tabletting after granulation with a suitable binder or by filling into hard gelatine capsules.
The polyvinyl alcohols used are preferably commercial products whose k values (Fikentscher, Cellulose Chemistry 13 (1932), 58) for fully hydrolyzed polyvinyl alcohols are in the range from 40 to 59 and for partially hydrolyzed polyvinyl alcohols in the range from 50 to 59.
Surprisingly, it has been found that the mixture according to the invention of the excipients and drugs and the resulting simple pharmaceutical-technological processing leads to the solution of the problem.
Furthermore, it has surprisingly been found that obtained by the process according to the invention, the use of combinations of Polyvinylaikoholen different residual acetate contents, levodopa / Carbidopa drug formulations that release the drugs quickly and completely, but their drug can be varied in the Befloutungsphase as needed or controlled ,
The procedure according to the invention leads to a dosage form which is beidoAr; % liberates rapidly and over a certain period of time in defined proportions, thus ensuring optimal bioavailability of the peripheral dopa decarboxylic acid carbidopa and the dopamine precursor levodopa.
For different patient groups, this makes it possible to provide pharmaceutical forms with optimal release rates at minimum expense.
With the dosage form described, the requirement of using levodopa / carbidopa dosage forms with immediate and complete or initial delayed liberation can thus be met according to the clinical picture.
