Abstract
Die Erfindung betrifft ein Implantat, das als Hemi- oder Endoprothese Verwendung findet. Aufgabe der Erfindung ist es, ein Implantat zu schaffen, das trotz Bioaktivitaet und dadurch bedingter Auslaugung eine hohe mechanische Stabilitaet behaelt und das verbleibende Implantatgeruest das Einwachsen von biologischem Gewebe beguenstigt. Die Aufgabe wird geloest durch einen Formkoerper aus Siliciumnitrid oder Siliciumcarbid und gegebenenfalls Anteilen bioaktiver Materialien. Der poroese Formkoerper weist eine Koernung von 20 bis 100 Mikrometer und eine Porositaet von 10 bis 30% auf. In und auf diesem Formkoerper sind Wachstumsstimulatoren eingelagert und/oder bioaktive Glaskeramiken und/oder Bioglas mit einem substratspezifischen Stuetzgeruest aufgebracht.The invention relates to an implant which finds use as hemi or endoprosthesis. In mehreren komplizierten Verfahrensschritten wird versucht, die Oberflächen der Implantate gezielt gewebefreundlich durch Aufrauhen, Aufsintern, Aufdampfen u.a. zu bearbeiten. Trotz hohen Aufwandes entspricht die erreichte Qualität der gewebefreundlichen aufgerauhten Oberflächen nicht den medizinischen Erfordernissen, da diese Implantate nach geraumer Zeit vom Körper wieder abgestoßen werden. Um diesem Vorgang im bestimmten Maße entgegenzuwirken, werden zum Steuern der Gewebereaktion und zur Stimulation des Aufwachsens des Gewebes in die aufgebrachte Schicht des Implantates bestimmte Ionen wie Li, B, C, F, Na, Mg, Si, P, K, Ca eingebracht, wie es z. B. für Sinterkeramik in DE-OS 2324867 beschrieben wird. Da die Schicht sehr dünn ist, werden diese Ionen schnell verbraucht. In DD 202613 und DE-OS 2838759 wird versucht, diese physiologisch aktiven Stoffe, z. B. CaF2, auf eine passivierende Schicht wie Siliciumnitrid mittels aufwendiger vakuumtechnischer Beschichtungsverfahren aufzubringen. Bei dieser Variante ist eine geringe Verbundstabilität vorhanden. Die Geschlossenheit der dünnen Schicht ist nicht gewährleistet, so daß Diffusionsprozesse auf dem Substratmaterial die Biokompatibilität negativ beeinflussen. Die Porosität liegt bei 20% bei mittleren Porendurchmessern von 180 Mikrometer.
