Abstract

活动式人工椎体由切口弹性元件[1]和固定平台[2]组成,切口条数在前后位多于两侧位。使弹性元件具有前屈、后伸、左右侧弯及纵向弹性功能,弹性元件切口内设有限位凸耳[3],限制弹性元件受过载应力时的变形幅度,并用埋头螺栓[4]锁定平台高度。

Description

活动式人工椎体
本实用新型涉及人工器官、结构力学及生物力学领域。
结核、肿瘤切除椎体后,往往造成一到二段椎体缺损,无法用一般植骨方法修复,而必需代之以人工椎体,目前国内外使用的人工椎体(如[Sp ine,1986,4:4620;中华外科杂志,1981,2:107;中华外科杂志,1990,5:328])虽然可以支撑固定,但是弹性极小而且不能活动,术后脊柱强直,患者活动受到限制,更严重的是做为一种长期植入物,以往的人工椎体不具备活动性和预应力弹性功能,不能与人体脊柱的力学环境相适应,在脊柱活动时易在假体两端与椎体的界面上产生应力集中,导致假体的松动,引起严重的后果。
本项发明的目的是研制一种具有弹性和活动功能的人工椎体。
该人工椎体由切口弹性元件[1]和两端的可升降固定平台[2]组成。切口弹性元件[1]的结构是在管状壁上切割出切口,从而使其产生弹性,切口在前后位上的数目多于两侧位,使前后位的活动度大于两侧位,从而满足人体脊柱前屈、后伸、左右侧弯的生物力学要求;并具有一定可压缩性,受到过度载荷时弹性元件[1]可以缓冲、并靠弹性元件上的限位凸耳[3]实现闭合立定,控制椎体在受到过载应力时的变形幅度。固定平台[2]与弹性元件[1]采取正反向螺纹联接,转动弹性元件即可升降两端固定平台,调节安装尺寸,手术时可先通过旋转弹性元件[1]降低固定平台[2],安装到缺损部位后再反向旋转弹性元件升高两端固定平台,使假体与相邻椎体牢固结合,同时将固定平台[2]上的固定钉压入上下方相邻椎体,并在弹性元件内产生预压应力。安装调整完毕后用两端的锁紧螺栓[4]定位固定。
本实用新型的优点在于活动式人工椎体在植入人体后两端可产生预紧压应力,能起到缓冲和减震的作用。同时该人工椎体具有前屈、后伸、左右侧弯的可变形能力,能够避免假体两端的界面产生应力集中。另外当受到过载时,弹性元件切口内的限位凸耳能使弹簧立定,能起到传递更大的载荷而不引起过量变形的作用。该人工椎体无摩擦部件,不会产生对人体的害的金属粉末,从而具有较好的人体相容性。

本实用新型的实施实例,参看附图说明。取直径30mm的钛合金棒材(Ti-6AI-4V)做为加工人工椎体的材料,分两步加工:1.切口弹性元件。取30mm长的棒材,外面车光至直径25mm,内部车空,形成内径15mm,壁厚5mm的管,在管壁上用电切割加工出切口和限位凸耳。在弹性元件两端垂直其纵轴的管壁上各开6个固定旋动孔,孔径3mm,内车螺纹(供旋动弹性元件和上紧固定螺栓之用),孔间隔30°,两端的孔呈交错分布。弹性元件两端8mm长的内壁上车出正、反向螺纹。2.固定平台。平台呈椭圆形,纵径28mm,横径24mm端面平滑、周围有6个突起的尖刺、刺根径31mm,长4mm,平台内面突出有14mm直径的高度调节轴、长15mm,表面车有正、反向螺纹,与弹性元件内壁的螺纹相适应。另外制作两枚直径3mm的锁紧螺栓。加成型后进行表面喷砂处理。

3根据权利要求1所述的人工椎体,其特征是弹性元件[1]切口内具有限位凸耳[3]。
4根据权利要求1所述的人工椎体,其特征是固定平台[2]与弹性元件[1]由正反向螺纹联接。
5根据权利要求1所述的人工椎体,其特征是弹性元件[1]和固定平台[2]的高度用埋头螺栓[4]锁定。
