早在19世纪中叶,法国人A.Vulpian就在镜下观察并描述了肥厚型心肌病,但是直到20世纪50年代后期,肥厚型心肌病独特的临床特点才被***地论述。它通常以不对称的左和/或右心室肥厚为特征,可累及心室的不同部位,尤以心室间隔为著,左室容积可正常或减少。典型的形态学改变为肌纤维排列紊乱。临床常表现为呼吸困难、心绞痛及晕厥,心律失常、猝死。肥厚型心肌病以往被认为是一种少见的心脏疾病,但Maron等进行的流行病学调查显示,年轻人的发病率达0.2%(Maron BJ,Gardin JM,Flack JM,et al.Prevalence ofhypertrophic cardiomyopathy in a general population of youngadults:echocardiographic analysis of 4111 s ubjects in theCARDIA study.Circulation.1995;92:785-789)。我们对中国九省市8080个正常人的超声心脏多普勒检查表明,中国人中的肥厚型心肌病亦非常常见,患者可多达0.08%(Zou Y.等,Am.J.Med.2004,116:14-18)。肥厚型心肌病至少有60%-70%以上为常染色体显性遗传,称家族性肥厚型心肌病(FHC)。分子遗传学研究发现,编码肌小节结构蛋白的基因突变是家族性肥厚型心肌病的根本病因[Seidman JG,Seidman C.The genetic basis for cardiomyopathy:from mutationidentification to mechanistic paradigms.Cell.2001;104(4):557-67;Bonne,Carrier L,Richard P.Familialhypertrophic cardiomyopathy:from mutations to functionaldefects.Cir Res.1998;83:580-593]。
肌小节是心肌收缩的基本单位,主要由粗、细肌丝两部分组成。粗肌丝主要为肌球蛋白(myosin)和肌球蛋白结合蛋白C(MyBPC),前者包括两条肌球蛋白重链(MHC)和两条轻链——必需/碱性轻链(MELC,MLC-1)及调节/磷酸化轻链(MRLC,MLC-2)。细肌丝包括肌动蛋白(actin)、α-原肌球蛋白(α-Tm)和肌钙蛋白复合体(TnT,TnC,TnI)。此外,肌小节成分还有肌连蛋白(titin)等。
1.家族性肥厚型心肌病致病基因
1.目前,已证实13个基因160多个位点的突变与FHC的发病有关,其中有10种是编码肌小节结构蛋白的基因,绝大部分突变位于这些基因(Marian AJ,Roberts R.The molecular menetic basis for hypertrophiccardiomyopathy.J Mol Cell Cardio.2001;33:655670)(表1)。

然而,这些突变并未囊括所有的遗传型心肌肥厚型致病基因突变。
本发明的目的在于发现新的突变,便于肥厚型心肌病的早期诊断和预防。
发明内容
本发明基于的是肥厚型心肌病致病基因中存在一些以前尚未报道的突变这一发现基础之上的。本发明提供了含有这样的突变的分离核酸。本发明还提供了可用于检测这些突变的寡核苷酸,以及利用这样的寡核苷酸检测突变的方法。本发明提供了利用这样的突变进行肥厚型心肌病诊断或者药物筛选或评估的方法。本发明另外涉及微阵列。
具体实施方式
在本发明中,术语“样品”是指来源于人类个体的任何生物学样品,包括但不限于例如细胞,组织样品如机体软组织例如骨组织、毛发等,生物学流体如淋巴液、血液、唾液、尿液和其它机体***液等。术语“分离的”意指非天然存在的。“分离的核酸”指并不与其所来源的个体基因组中相邻的5’和/或3’侧翼序列相毗邻的核酸。因此,“分离的核酸”也包括***载体内的核酸,掺入异源细胞的基因组内的核酸,或者以独立体存在的核酸。该术语还包括与其它核酸序列相连接的重组核酸。这样的重组核酸可能用于生产融合蛋白。
在本发明中,野生型基因指的是与肥厚型心肌病致病基因对应的正常基因。它们的序列在现有技术中是已知的,并列于例如Genbank数据库中。例如,野生型β-MHC基因的序列以Genbank登录号gi:M57965.1公开,在本发明中以SEQ ID NO:1表示;野生型cMYBPC基因的序列以Genbank登录号gi:Y10629公开,在本发明中以SEQ IDNO:2表示;野生型cTnT基因的序列以Genbank登录号gi:X90780公开,在本发明中用SEQ ID NO:3表示。但是,本领域普通技术人员十分清楚,由于基因的测序过程中可能存在一些错误,将会导致野生型基因的序列与本文列出的登录号指示的序列有所不同,或者核苷酸残基的编号有所变化。此时,将以最终确定的序列为准。
本发明中基因突变的表示方法如下:
“x nt1>nt2”表示在参考序列中发生了核苷酸的置换。其中“x”指突变发生的位点,它遵循的是Genbank中相应登录号给出的野生型基因的核苷酸残基编号。“>”表示发生了置换。“nt1”指突变前的核苷酸,即正常基因(野生型基因)中相应位点的核苷酸残基。“nt2”指突变后的核苷酸残基。例如,cTnT基因中的“4693C>T”表示在cTnT基因中发生了置换突变,导致在相应于Genbank登录号gi:X90780(SEQ ID NO:3)列出的序列中第4693位核苷酸残基位点处的“C”被取代为“T”。
“x1-x2 del nt1...nt2”表示在参考序列的第“x1”位到第“x2”位的核苷酸位点处发生了缺失,从而缺失了“nt1”到“nt2”的核苷酸残基。例如,“14262-14264delAAG”表示第14262位到第14264位的核苷酸“AAG”被缺失了。
“x1^x2 ins nt1...nt2”表示在参考序列的第“x1”位与第“x2”位的核苷酸位点之间***了“nt1...nt2”核苷酸残基。例如,“18115^18116insGCAAG”表示在第18115位与第18116位核苷酸位点之间***了“GCAAG”。
在一个实施方案中,本发明提供了一种突变的β-MHC基因或其片段,其在对应于Genbank登录号gi:M57965.1所列序列(SEQ ID NO:1)中的下述位置处,含有选自下组的一个或多个突变:10353G>A,10429A>C,10895A>T,11811A>C,13653A>C,14449T>C,14452G>A,14557G>A。
在一个实施方案中,本发明提供了一种突变的cMYBPC基因或其片段,其在对应于Genbank登录号gi:Y10629(SEQ ID NO:2)所列序列中的下述位置处,含有选自下组的一个或多个突变:4826G>A,8697G>A,8887G>A,12101G>A,13261G>A,14262-14264delAAG,14364delG,18115^18116insGCAAG。
在一个实施方案中,本发明提供了一种突变的cTnT基因或其片段,其在对应于Genbank登录号gi:X90780(SE Q ID NO:3)所列序列中的下述位置处,含有选自下组的一个或多个突变:4693C>T,4769G>A。
在进一步的实施方案中,本发明的突变基因或片段中还含有额外的突变,包括任何已知的或将来获知的突变或核苷酸多态性。
本发明还涉及由上述突变基因或片段衍生的mRNA或cDNA或者编码的蛋白质。
本发明还涉及可与上述任一突变基因或片段或由其衍生的mRNA或cDNA或者它们的互补序列特异性杂交,但不能与相应的野生型基因序列或其互补序列杂交的核酸,例如寡核苷酸,它们可作为探针或引物用于特异性检测人类个体中的上述突变。本领域中的技术人员均清楚,探针一般具有与目的核酸的序列互补、并与包含待检测突变的核酸特异性杂交而不与具有野生型基因序列的核酸相杂交的序列。该寡核苷酸一般定位于突变位点周围,特异性地检测样品中上述突变的存在与否。优选地,所述核酸或其互补序列中含有上文所定义的任何一个或多个突变。作为例示,寡核苷酸引物或探针可设计成待检测的突变位点位于其5’或3’末端或者在中间,或者待检测的突变位点位于该引物或探针的互补序列的5’或3’末端或者在中间。在本发明的上下文中,术语“特异性检测”是指该寡核苷酸只与包含上文定义的突变的核酸或其互补序列发生杂交,而不与分别具有野生型基因序列或其互补序列的核酸发生杂交。
在突变位点涉及多个核苷酸时,寡核苷酸探针或引物可设计成使得突变位点的核苷酸序列或与其互补的序列的整个或部分包含在探针或引物的核苷酸序列中。这样,有可能利用由此制备的寡核苷酸作探针通过杂交或差异杂交的存在与否来测定突变的存在情况,或者作为引物通过扩增并随后检测(通过例如测序、限制性酶切等)突变的存在情况。
寡核苷酸探针或引物一般长度大于约10bp,例如10-100个核苷酸,优选地,其长度为15-75个核苷酸,更优选18-50个核苷酸,尤其优选为20-35个核苷酸,最优选为23-30个核苷酸。本领域中的技术人员可以很容易设计出合适的寡核苷酸探针或引物,用于突变位点的检测。至于其制备,则可通过本领域技术人员熟知的方法进行,包括但不限于例如化学合成法等。优选地,探针/引物是被可检测标记的,以便于检测。标记可以是例如生物素、荧光染料、放射性同位素等,但并不限于此。它们是现有技术中已知的,并可通过常规方法连接到探针/引物上。该核酸可以是单链的,也可以是双链的。
在本领域中众所周知,可利用引物通过添加游离核苷酸而延伸核酸序列。杂交指的是在严谨条件下与核酸序列发生选择性结合。在本文中,严谨条件是仅允许引物与含突变的核酸序列或其互补序列杂交,而不与野生型核酸序列或其互补序列杂交的条件。依照所使用的引物,普通技术人员可以通过常规实验确定相应的严谨杂交条件。在这方面有众多的现有技术可供参考,包括例如Sambrook等,分子克隆实验室手册,第2版,冷泉港实验室出版社,冷泉港,纽约。
本发明还涉及利用设计成在扩增后将得到包含待检测突变的片段的引物进行突变检测。所述引物一般定位于突变位点周围或者其5’或3’方向1000bp、优选500bp、更优选250bp、尤其优选200bp、最优选100bp的范围内,其长度一般大于约10bp,优选为15-100bp,更优选为18-75bp,尤其优选为20-50bp,最优选为25-40bp。
还可以在本发明的寡核苷酸序列5’和/或3’末端连接一些额外的核苷酸序列。这样的寡核苷酸也在本发明的范围内。
利用这样的寡核苷酸,通过对来自人类个体的遗传材料进行扩增和随后分析,可以检测所述人类个体中所述突变的存在与否。例如,可以将扩增产物进行琼脂糖凝胶电泳,随后用溴化乙锭、SYBR绿色溶液、诸如此类染色,从而以条带(DNA片段)的形式检测扩增产物。由此,可以以DNA片段的形式检测包含突变位点的基因片段。除了琼脂糖凝胶电泳以外,也可以进行聚丙烯酰胺凝胶电泳或毛细管电泳。进一步地,可以分离扩增产物,随后通过测序或者限制性酶切分析突变的存在与否。还有可能使用预先经诸如荧光染料等物质标记的引物进行PCR,并检测扩增产物。还可以采用不需要电泳的检测方法;在这种方法中,将扩增产物结合到固体支持物诸如微型板上,并通过荧光、酶反应、诸如此类等手段来检测目的DNA片段。
本发明提供了检测上述突变的方法,包括应用上文所述寡核苷酸探针或引物检测生物学样品,或者应用设计成在扩增后将得到包含该突变的片段的引物对扩增该生物学样品并进行检测的步骤。在检测中,通常应用待测个体的生物学样品,例如其血液,但并不限于此。在一个实施方案中,直接应用该生物学样品进行检测。例如,利用上述核酸作为引物,通过PCR法扩增包含待检测突变位点的片段。在另一个实施方案中,在检测前首先从该生物学样品分离遗传材料,例如基因组DNA,但并不限于此。检测可通过例如核酸杂交、PCR法、测序法、限制性片段长度多态性测定法等方法常规进行,但并不局限于这些方法。其条件是本领域普通技术人员所知的,或可非常容易地确定。优选地,使用如上所述制备的寡核苷酸作为引物,用DNA聚合酶扩增编码包含待检测突变位点的DNA片段。或者,将如上所述制备的探针与模板DNA杂交,从而检测那些具有感兴趣突变的DNA。可以依照常规方法来制备模板DNA,如氯化铯梯度离心、SDS裂解法、或酚/氯仿抽提。
本发明还涉及一种微阵列,其中含有上述寡核苷酸探针或引物。该寡核苷酸可以固定在固相支持物上。在一个实施方案中,所述微阵列是DNA芯片、基因芯片、微型芯片、珠阵等的形式。DNA芯片是将很多种DNA探针排列并固定在基片例如玻璃片上,然后在上面进行标记DNA的杂交、通过检测探针上的标记信号而可区分地检测完全匹配和核苷酸错配,由此对核酸进行检测。芯片的常规制备方法是本领域技术人员已知的。
本发明还涉及可用于检测上述突变的试剂盒,其中包括一种或多种相关的诊断性引物或探针和/或该突变基因特异的寡核苷酸引物,例如前文所述的寡核苷酸探针或引物。试剂盒中还可包括容器、使用说明书、缓冲液、酶和/或其它反应成分。优选地,其中还包含阳性对照和阴性对照,以便进一步确认检测结果。在一个实施方案中,本发明的试剂盒包含用于预定测定法的任何和所有成分酶或成分。这些成分的实例包括但不限于寡核苷酸、聚合酶(如Taq聚合酶)、缓冲液(如Tris缓冲液)、dNTP、对照试剂(如组织样品、用作阳性和阴性对照的靶寡核苷酸、等)、标记和/或检测试剂(荧光染料诸如VIC、FAM)、固体支持物、说明书、例示性图表和/或产品信息、抑制剂等。本发明的试剂盒可以是只包含部分必需成分的部分试剂盒。在这种情况中,使用者可以提供其余成分。本发明的试剂盒可以包含两个或多个分开的容器,每个都包含将要使用的部分成分。例如,在试剂盒中,第一个容器包含酶,而第二个容器包含寡核苷酸。酶的具体实例包括适当保存缓冲液或容器中的结构特异性切割酶。
本发明还涉及诊断肥厚型心肌病或确定肥厚型心肌病预后风险的方法,包括对个体的体外样品检测下述中的一种或多种突变的步骤:β-MHC基因中的下述突变:10353G>A,10429A>C,10895A>T,11811A>C,13653A>C,14449T>C,14452G>A,14557G>A;cMYBPC基因中的下述突变:4826G>A,8697G>A,8887G>A,12101G>A,13261G>A,14262-14264delAAG,14364delG,18115-18116insGCAAG;或者cTnT基因中的下述突变:4693C>T,4769G>A。优选地,检测步骤是按照前文所述突变检测方法进行的。例如,使用位于突变位点上下游的寡核苷酸首先对包含待检测的突变位点的核酸片段进行PCR扩增。检测可通过例如PCR法、测序法、限制性片段长度多态性等进行。
本发明还涉及利用上述突变基因作为靶点筛选或评估药物的方法,包括例如对携带所述突变的细胞、组织、器官或动物个体给予一种或多种药物,并评估其一个或多个指示肥厚型心肌病状况的指标,以指示疾病改善状况。优选地,所述细胞、组织、器官或动物个体是通过基因敲除(gene knockout)或敲入(gene knockin)或者转基因的方法而得到的。基因敲除或敲入或者转基因的方法是本领域技术人员熟知的,包括例如设计两个与内源基因同源的侧翼片段。在基因敲除的情况下,两同源片段之间包含预定使体内内源基因失活的突变,从而在导入体内后,能将相应的内源基因失活。在基因敲入的情况下,两同源片段之间包含预定导入体内内源基因处的核酸,从而在导入体内后,能将目的核酸***预定的位点。操作对象可以是个体,例如小鼠。或者,操作对象也可以是细胞,例如人的心肌细胞。这样,可以在个体或细胞水平上对药物进行筛选或评估。
下面以实施例的方式对本发明作出说明。这些实施例都是示范性的,它们不以任何方式限定本发明的范围。
实施例
实施例1基因组DNA的提取
1研究对象的选择
研究对象为在中国医学科学院阜外心血管病医院诊断明确的门诊就诊或住院的肥厚型心肌病病人,共100例,经家族调查无血缘关系。其中包括肥厚型心肌病家系先证者51人,相关家系成员356人,散发病例49人。正常研究对照120例为性别、年龄匹配的健康人。入选者填写《分子遗传学研究病例调查表》,内容包括:病人联系方式、个人一般情况、临床症状、体检结果、家族史调查、实验室检查、心电图(必要时Holter),胸片报告,超声心动图报告(必要时需心脏MRI或左室及冠脉造影结果)。建立资料数据库。
肥厚型心肌病诊断标准(WHO/ISFC 1996年公布)
(1)基本标准:超声心动图诊断的原因不明的心脏左室壁厚度大于或等于13mm;或心尖部肥厚或右室肥厚。
(2)参考标准:心电图左室高电压表现,伴/不伴II、III、avF、avL及V4V5导联病理性Q波及ST-T改变。
(3)排除标准:长期高血压、缺血性心肌病及其他可引起心室肥厚的原发性疾病。
(4)家族性肥厚型心肌病确定标准:有心肌病的家族遗传倾向,即病人(先证者)家族中有多人发病,并符合常染色体显性遗传特点。
2基因组DNA的分离
取患者肘静脉血5ml,EDTA(0.5M)抗凝,2500rpm离心10min。小心吸取上层血浆,用1.5ML的Eppendorf管分装。在血细胞中加入3倍体积的溶血液(NH4Cl 8.29g、KHCO3 1.0g、EDTA-Na2 0.03734g,加双蒸馏水至1000ml,调节PH为7.4),摇匀,冰浴15min。2500rpm离心10min,弃上清。向细胞沉淀中加入10ml溶血液,摇匀,冰浴15min。3000rpm离心10min,弃上清,得到白细胞,直接用于提取基因组DNA,或者于-80℃冻存备用。
如下提取基因组DNA:取白细胞,加4.5ml SE(NaCl 4.383g、EDTA-Na2 9.306g加双蒸馏水至1000ml,调节PH值为8.0),使劲吹打,混匀。沿管壁加入0.5ml的10%SDS,然后加蛋白酶E(100mg/ml,MilliQ水)50μl,首尾轻摇10min,混匀,然后于37℃水浴过夜。第二天取出,沿管壁加3ml饱和NaCl,迅速手摇15s,使蛋白变性析出,然后于4000rpm离心10min。取上清,转入另一管中,加等体积的三氯甲烷,摇10min,混匀。2500rpm离心10min。取上清,转入另一管中,加等体积的三氯甲烷,摇10min,混匀。2500rpm离心10min,取上清,转入50ml管中,加3倍体积无水乙醇后,首尾轻摇,见DNA絮状沉淀析出。将DNA挑至管壁,用75%乙醇洗涤2遍。将DNA挑至0.5ml管中,冻干。最后加TE(10mM Tris-HCl、1mM EDTA,调节PH为8.0)300μl,使DNA溶解,紫外分光光度计测OD值定量,4℃保存。
实施例2 PCR法筛选β-肌球蛋白重链(B-MHC)基因中的突变
所用引物及扩增条件如下表1所示(基因组序列来自Genebankdatabase,登陆号为M57965):

制备如下PCR反应基本体系(50μl):

按照表1中的PCR条件对各患者或对照的基因组DNA进行PCR扩增,并按下述步骤对PCR反应产物进行测序前纯化步骤:
(1)将50μl PCR反应产物同其6倍体积的无水乙醇与5M醋酸胺(5∶1)混合溶液混合,置于-80℃30分钟以上。
(2)14000rpm,4℃离心15分钟,吸上清弃去。
(3)加入200μl的75%酒精,14000rpm,4℃离心5分钟,吸上清弃去。
(4)室温或冷冻干燥后,加入20μl TE溶解。
(5)吸2μl点样,定量。
如下进行测序反应:
(1)反应体系(20μl)
BigDyeTM 4μl
BigDyeTM Buffer 2μl
引物(同PCR扩增引物) 10pmol/μl
(2)反应条件(根据引物的退火温度而定):
(3)测序产物的纯化:
20μl产物中加25μl醋酸铵和125μl乙醇,置-70℃ 1小时,14,000rpm离心10分钟,弃上清,加75%的乙醇100μl,14,000rpm离心5分钟,弃上清,95℃1分钟烤干,加入2μl loading buffer,混匀,94℃变性5分钟,然后迅速置于冰上。
(4)将纯化后的测序反应产物在ABI PRISM377自动测序仪上测序。
(5)用GeneJockey、AssemblyLIGNTM、Sequencing Analysis 3.4软件阅读及核对测序图,并与以公布的基因序列相比较。由此筛选出来的基因突变列于表5中。
实施例3 PCR法筛选心脏肌钙蛋白T(cTnT)基因中的突变

实施例6限制性片断长度多肽性(RFLP)检测
因基因某位置的碱基改变,破坏了或建立了可被限制性内切酶识别的特异性酶切位点,当该样品被特异性限制性内切酶处理后,因其与正常样品的酶切片段长度不同,在凝胶电泳时,可显示不同的迁移速度。以此原理,可进行已知突变位点的患者家系成员的基因突变筛查。此检测中使用Promega公司酶切试剂盒,并按照所附的产品说明书进行。
