本发明属于食品组合物技术领域,具体涉及一种用于2型糖尿病(Type 2diabetesmellitus,T2DM)患者胰岛素强化治疗后的低GL膳食组合物及其制备方法。
背景技术
糖尿病是一种以高血糖为特征的代谢性疾病。根据国际糖尿病联盟(IDF)2021年底公布的全球糖尿病地图显示,我国糖尿病患者、血糖异常人群近年来持续增长,并逐渐年轻化、大众化。此外,血糖控制情况普遍不理想,导致相关的并发症和寿命降低,也给社会、家庭和个人带来沉重的负担。糖尿病的预防和控制成为当前社会亟需解决的一大难题。近年来,T2DM不再被认为是终身疾病,尤其是高体重的早期T2DM患者存在“血糖缓解”的可能,中国也在2021年发布了《2型糖尿病逆转中国专家共识》。众多国内外临床研究证实,病程较短、相对较年轻的新诊断T2DM患者采用短期胰岛素强化治疗(Short termintensiveinsulin therapy,SIIT)后的无药血糖缓解效果最佳,而“无药缓解期”的生活方式干预在一定程度上决定了缓解与否及缓解时长,其中又以饮食控制最为重要。饮食控制是糖尿病综合治疗的基础措施,无论使用何种降糖方案,医学营养治疗(Medical nutrion therapy,MNT)都应该贯穿糖尿病患者治疗的始终。
对于糖尿病患者来说,能够长期坚持的平衡膳食结构,仍然要以碳水化合物作为每日的主要能量来源,《中国糖尿病医学营养治疗指南(2022版)》建议糖尿病患者饮食中碳水化合物占能量摄入的45%~60%。因此糖尿病患者不能一味地减少碳水化合物的摄入量,而是要更注重碳水化合物的质。为了更好地指导糖尿病患者合理饮食,学术界提出血糖生成指数(Glycemic index,GI)的概念,也俗称为升糖指数,是表示人体食用某种食物后升高血糖效应与含等量碳水化合物的标准食品(通常为50克葡萄糖)升高血糖效应之比,反映的是食物中的碳水化合物能够引起人体血糖升高程度的能力。一般认为:GI在55以下的食物是低GI食物,如黑米、黑麦、番薯、豆类、叶菜、坚果类、牛奶、豆腐、樱桃、柚子、桃子、梨子、苹果等;GI在55-70范围之间的为中GI食物,如玉米、燕麦、荞麦面、全麦面包、土豆、果汁、蔗糖等;GI在70以上为高GI食物,如白米饭、白面条、白馒头、糯米饭、烙饼、白面包、胡萝卜、南瓜、西瓜、蜂蜜、葡萄糖、麦芽糖等。GI值仅代表食物的升糖能力,与进食量无关。而具体每餐实际摄入的食物量中碳水化合物对血糖影响的大小,则用膳食血糖负荷(Glycemic load,GL)表示,计算公式为:食物GL=食物GI×食物中碳水化合物含量(克)/100,GL综合反映了摄入食物中碳水化合物的质和量对餐后血糖的影响。按照国外的标准,一般常规进食量下GL≥20为高GL食物,表示对血糖影响很大;10≤GL≤19为中GL食物,表示对血糖影响中等;GL<10为低GL食物,表示对血糖的影响很小。就全天的膳食来说,国外文献报道对“低GL膳食”的定义有两种:一种是每天膳食GL低于80,另一种是不超过45/1000kcal。亚洲人群的精细主食进食量相对较大,国内尚缺乏对本土膳食GL划分的标准。
目前市场上也有大量低糖或无糖代餐食品(如低糖面包、无糖饼干等),很多糖尿病人以为可以随意进食。但所谓的“低糖或无糖”仅仅代表食品中添加有较少的蔗糖或不添加蔗糖,这些食品往往本身含有大量的精细淀粉,因此食用后仍然会消化吸收成葡萄糖进入体内,且GI指数常大于70,正常进食量的GL也远大于20,仍然会对糖尿病患者的血糖产生较大的影响。加上商家对于这类食品的过度造势和包装,使得价格水涨船高,性价比低,不是糖尿病患者的良选。
中国专利CN112385840A中公开了一种低GL谷物源营养特膳食品及其加工方法,该发明以大宗粮谷作物为原料,通过其与3-脱氧花青素含量大于3mg/100g的天然植物浸提液以及富含硫的植物或动物蛋白质进行混合,使得含硫蛋白质与3-脱氧花青素互作实现淀粉颗粒原位包裹,即可制备得到低GI且慢消化淀粉含量高的谷物源营养特膳食品。该发明方法不仅工艺绿色环保、操作工序简单、生产成本低,而且产品实现餐后血糖平缓释放,可作为糖代谢异常人群代餐食品、长跑运动员食品、中小学营养早餐食品等,但是该食品没有关注到糖尿病患者。
中国专利CN110692676A公开了一种低GI/GL值的藜麦米代餐复合食品及其制备方法和用途。由包括小麦,藜麦米,燕麦,荞麦,白芸豆,鹰嘴豆和抗性淀粉的原材料制备得到。食用该发明的低GI/GL值的藜麦米代餐复合食品后,能有效控制糖尿病患者餐后血糖,使GI值为19.8-33.7;GL值为10.6-18.2。即GI值均小于55,GL值小于19,为中或低负荷食品,属于对血糖影响不大或无影响范筹,但是该复合食品中不含有油、奶制品等组分,不能更好地均衡营养。
因此,亟需开发一种营养均衡且用于2型糖尿病患者早期胰岛素强化治疗后维持长期无药“血糖缓解”的低GL膳食组合物及其制备方法。
发明内容
基于现有技术中存在的不足,本发明旨在提供一种用于2型糖尿病患者早期胰岛素强化治疗后可以长期维持的低GL膳食组合物及其制备方法,本发明所述的低GL膳食组合物组分及配比更加合理,营养更均衡,可用于2型糖尿病患者胰岛素强化治疗后的营养配餐,能够有效控制糖尿病患者餐后血糖,有利于患者达到长期无药“血糖缓解”。
为了实现上述目的,本发明采用如下技术方案:
一种适合T2DM强化治疗后维持的低GL膳食配方,包括以下成分:
配方一:蛋白质10-22g、脂肪9.85-16.43g、碳水化合物25-44g、复合维生素1.6-5.0mg、复合矿物质0.28-0.86g。
配方二:蛋白质20-36g、脂肪34-52.5g、碳水化合物48-96g、复合维生素141-147mg、复合矿物质1.26-1.75g。
配方三:蛋白质23-35g、脂肪34-52.1g、碳水化合物48-96g、复合维生素141-147mg、复合矿物质1.26-1.75g。
其中,
所述的蛋白质选自分离乳清蛋白粉、络蛋白、大豆分离蛋白粉、大米分离蛋白粉中的一种或几种;
优选地,配方一中所述的蛋白质为分离乳清蛋白粉和络蛋白的混合物;
再优选地,配方一中所述的蛋白质为质量比5-10:1的乳清蛋白和络蛋白。
进一步优选地,配方一中所述的蛋白质为质量比8:1的乳清蛋白和络蛋白。
优选地,配方二和配方三中所述的蛋白质为大豆分离蛋白粉和大米分离蛋白粉的混合物;
再优选地,配方二和配方三中所述的蛋白质为质量比2-3:1的大豆分离蛋白粉和大米分离蛋白粉。
进一步优选地,配方二和配方三中所述的蛋白质为质量比2.5:1的大豆分离蛋白粉和大米分离蛋白粉。
所述的脂肪包括油脂、分离乳清中的脂肪和大米蛋白中的脂肪中的至少一种。
其中,所述的油脂为植物油;所述的植物油由豆油、菜籽油、玉米胚芽油、葵花籽油、花生油、芝麻油、橄榄油、山茶籽油、红花籽油、亚麻籽油、米糠油、核桃油、葡萄籽油、小麦胚芽油、奇亚籽油、沙棘籽油和茶叶籽油中的多种混合而成。
优选地,所述的植物油为玉米胚芽油、芝麻油、菜籽油、葵花籽油、橄榄油、亚麻籽油、花生油、核桃油和奇亚籽油混合而成。
再优选地,所述的植物油为质量比120-150:1-3:8-10:1-3:1-3:5-7:12-15:3-5:1-3的玉米胚芽油、芝麻油、菜籽油、葵花籽油、橄榄油、亚麻籽油、花生油、核桃油和奇亚籽油;
进一步优选地,所述的植物油肪为质量比150:3:10:3:3:5:12:5:3的玉米胚芽油、芝麻油、菜籽油、葵花籽油、橄榄油、亚麻籽油、花生油、核桃油和奇亚籽油。
本发明在实施过程中通过合理控制油脂的种类;使得到的产品中富含饱和脂肪酸、不饱和脂肪酸、单不饱和脂肪酸、多不饱和脂肪酸、n-3脂肪酸和胆固醇;
通过控制各种油脂的添加量,使得产品中饱和脂肪酸的含量满足3.1-6.6g;使得产品中不饱和脂肪酸的含量满足1.3-3.9g;使得产品中单不饱和脂肪酸的含量满足1-12.6g;使得产品中多不饱和脂肪酸的含量满足0.2-3.7g;使得产品中n-3脂肪酸的含量满足0.05-0.075g;使得产品中胆固醇的含量满足36.2-394mg。

本发明还提供了上述低GL膳食配方的制备方法,包括如下步骤:
(1)称量各种原料进行混合,得到混合料;
(2)将混合料投入混料机中进行混合,得到所述的低GL膳食配方的组合物;
(3)对组合物进行检验,并装袋。
本发明还提供了上述低GL膳食配方组合物在制备低GL食品中的应用。
所述的低GL食品为用于2型糖尿病患者胰岛素强化治疗后代餐食品。
与现有技术相比,本发明的有益效果在于:
(1)该低GL膳食配方包括碳水化合物、膳食纤维、脂肪、蛋白质、多种维生素(维生素A、维生素D、维生素E、维生素K、维生素B1、维生素B2、维生素B6、维生素B12、烟酸、泛酸、胆碱、生物素和维生素C)和多种矿物质(钠、钾、钙、磷、镁、铁、锌、铜、锰、碘和硒),丰富的营养元素组合,满足人体所必需的营养需求;
(2)该低GL膳食配方的脂肪主要由植物油、分离乳清中的脂肪和大米蛋白中的脂肪组成,在实施过程中通过合理控制脂肪的种类,使得到的产品中富含饱和脂肪酸、不饱和脂肪酸、单不饱和脂肪酸、多不饱和脂肪酸、n-3脂肪酸和胆固醇,脂肪碳链短,从而更易吸收,对脂肪吸收不良人群友好;
(3)该低GL膳食配方营养丰富有助于2型糖尿病患者全天的精准饮食控制,且原料来源广,制备方法简单,适合大批量生产;
(4)本发明提供的低GL膳食配方,其中配方一、配方二和配方三对应每天的早餐、午餐和晚餐,三个配方的成分组合可以满足糖尿病患者全天的营养需求,还可以根据个体化(如身高、体重、年龄、血糖水平等)进行定制,即患者可以根据自身需求选择配方一、配方二或配方三的组分作为代餐。

(1)称量各种原料进行混合,得到混合料;
(2)将混合料投入混料机中进行混合,得到所述的低GL膳食配方的组合物;
(3)对组合物进行检验,并装袋。
实施例1一种适合T2DM强化治疗后维持的低GL膳食配方,包括以下成分:
配方一:乳清蛋白18g、络蛋白2g、分离乳清中的脂肪6.52g、大米蛋白中的脂肪4.33g、小米粉12g、小麦粉8g和糟米粉15g、复合维生素(维生素A的含量为135μg;维生素E的含量为0.98mg;维生素K的含量为0.22μg;维生素B1的含量为0.105mg;维生素B2得含量为0.135mg;维生素B6的含量为0.11mg;维生素B12的含量为0.495μg;烟酸的含量为1.235mg;泛酸的含量为0.825mg)、复合矿物质(钠的含量为170.05mg;钾的含量为161.2mg;钙的含量为31mg;磷的含量为156.25mg;镁的含量为32.5mg;铁的含量为9.85mg;锌的含量为1.045mg;铜的含量为0.21mg;锰的含量为0.775mg;碘的含量为12.375μg;硒的含量为9.225μg)、叶黄素277.2μg。
配方二:大豆分离蛋白粉22.5g、大米分离蛋白粉7.5g、植物油39.4g、分离乳清中的脂肪0.10g和大米蛋白中的脂肪0.5g、小米粉25g、小麦粉20g和糟米粉35g、复合维生素(维生素A的含量为250μg;维生素D的含量为0.125μg;维生素E的含量为5.395mg;维生素K的含量为9.03μg;维生素B1的含量为0.355mg;维生素B2得含量为0.065mg;维生素B6的含量为0.15mg;维生素B12的含量为0.25μg;烟酸的含量为5.05mg;泛酸的含量为0.85mg;胆碱的含量为14.895mg;生物素含量为0.5mg;维生素C含量为117mg)、复合矿物质(钠的含量为40.72mg;钾的含量为771.6mg;钙的含量为125.12mg;磷的含量为336.4mg;镁的含量为121.2mg;铁的含量为4.15mg;锌的含量为2.36mg;铜的含量为0.42mg;锰的含量为2.14mg;碘的含量为1.46μg;硒的含量为7.36μg)、叶黄素3490μg和胡萝卜素90μg
配方三:大豆分离蛋白粉22.5g、大米分离蛋白粉7.5g、植物油39.4g、分离乳清中的脂肪0.10g和大米蛋白中的脂肪0.5g、小米粉25g、小麦粉20g和糟米粉35g、复合维生素(维生素A的含量为250μg;维生素D的含量为0.125μg;维生素E的含量为5.395mg;维生素K的含量为9.03μg;维生素B1的含量为0.355mg;维生素B2得含量为0.065mg;维生素B6的含量为0.15mg;维生素B12的含量为0.25μg;烟酸的含量为5.05mg;泛酸的含量为0.85mg;胆碱的含量为14.895mg;生物素含量为0.5mg;维生素C含量为117mg)、复合矿物质(钠的含量为40.72mg;钾的含量为771.6mg;钙的含量为125.12mg;磷的含量为336.4mg;镁的含量为121.2mg;铁的含量为4.15mg;锌的含量为2.36mg;铜的含量为0.42mg;锰的含量为2.14mg;碘的含量为1.46μg;硒的含量为7.36μg)、叶黄素3490μg和胡萝卜素90μg
其中,所述复合矿物质中的钠元素组分来源于柠檬酸钠;钾元素组分来源于柠檬酸钾和氯化钾;钙元素组分来源于碳酸钙和磷酸氢钙,磷酸氢钙同时作为磷元素组分的来源;镁元素组分来源于碳酸镁;铁元素组分来源于焦磷酸铁,焦磷酸铁同时作为磷元素组分的来源;锌元素组分来源于柠檬酸锌;铜元素组分来源于硫酸铜;锰元素组分来源于硫酸锰;碘元素组分来源于碘化钾,碘化钾同时作为钾元素组分的来源;硒元素组分来源于***钠。
其中,上述配方一、配方二、配方三对应每天的早餐、午餐和晚餐。
本发明还提供了上述低GL膳食配方的制备方法,包括如下步骤:
(1)分别称量配方各种原料进行混合,得到混合料;
(2)将混合料投入混料机中进行混合后压成产条形,得到所述的低GL膳食配方的组合物;
(3)对组合物进行检验,并装袋。
效果实验:

3、制备的产品对2型糖尿病患者胰岛素强化治疗后的血糖缓解情况
3.1目的:探索低GL膳食配方干预是否可以提高新诊断T2DM患者短期胰岛素强化治疗后的血糖缓解率,并改善患者的胰岛功能及相关代谢指标。
3.2方法:采用随机平衡对照设计,纳入未经治疗的新诊断T2DM患者,统一收入院进行为期2周左右的胰岛素强化治疗后,按随机数字表法分配到对照组和低GL膳食干预组,观察停药3个月后血糖缓解及代谢改变情况。
3.3结果:本研究共纳入86例患者,两组患者在年龄、性别构成比、BMI、腰围、血压、HbA1c、糖化白蛋白、血脂谱、肝肾功能、胰岛素敏感性、胰岛功能及每日膳食碳水化合物(CHO)、能量摄入及GL的差异均无统计学意义(P>0.05)。两组血糖缓解率的差异有统计学意义(分别为24.3%和77.8%,P<0.05);在校正了年龄、性别、受教育程度、BMI、HbA1c等混杂因素后,每日膳食能量摄入、GL/1000kcal与血糖缓解呈显著负相关,OR值分别为0.30(0.10,0.90)和0.17(0.05,0.57),P<0.05;ΔGL、ΔGL/1000kcal与血糖缓解呈显著正相关,OR值分别为11.58(3.34,40.20)和11.67(3.44,39.66),P<0.05;每日膳食CHO、GL、GL/1000kcal与体重变化呈显著负相关(P<0.05),ΔGL/1000kcal与体重变化呈显著正相关(P<0.05)。GL/1000kcal与胰岛功能指标ISIced变化值显著相关(相关系数r=-0.443,P<0.05),而膳食指标与胰岛素敏感性指标的相关性均没有统计学意义(P>0.05)。
检测结果见下表3-表5,图1-图2。

低血糖负荷(GL)膳食干预3个月后,膳食能量摄入、经能量矫正的GL、GL下降值和经能量矫正的GL下降值都与血糖缓解显著相关(P<0.05)。

两组患者在经过膳食干预后体重指数(BMI)和腰围均显著下降(P<0.05),组间差异没有统计学意义(P>0.05)。

膳食碳水化合物摄入量(CHO)、血糖负荷(GL)、经能量校正的GL及其差值都与体重下降显著相关(P<0.05)。
根据图1可知,对照组和低GL膳食干预组患者出院后随访3个月直至1年的膳食能量校正GL值(GL/1000kcal)有显著差异(P<0.05)。
根据图2可知,两组患者在经过低GL膳食干预后体重指数(BMI)和腰围均显著下降(P<0.05)。
3.4结论:低GL膳食干预可以显著改善新诊断T2DM患者胰岛素强化治疗后的血糖缓解和体重下降情况,且GL下降越多,效果越显著;低GL膳食干预对代谢的获益可能主要归因于胰岛功能的改善。

Claims (10)
1.一种适合T2DM强化治疗后维持的低GL膳食配方,其特征在于:包括以下成分:
配方一:蛋白质10-22g、脂肪9.85-16.43g、碳水化合物25-44g、复合维生素1.6-5.0mg、复合矿物质0.28-0.86g;
配方二:蛋白质20-36g、脂肪34-52.5g、碳水化合物48-96g、复合维生素141-147mg、复合矿物质1.26-1.75g;
配方三:蛋白质23-35g、脂肪34-52.1g、碳水化合物48-96g、复合维生素141-147mg、复合矿物质1.26-1.75g。
2.根据权利要求1所述的低GL膳食配方,其特征在于:所述的蛋白质选自分离乳清蛋白粉、络蛋白、大豆分离蛋白粉、大米分离蛋白粉中的一种或几种;配方一中所述的蛋白质为分离乳清蛋白粉和络蛋白的混合物;配方二和配方三中所述的蛋白质为大豆分离蛋白粉和大米分离蛋白粉的混合物。
3.根据权利要求1所述的低GL膳食配方,其特征在于:所述的脂肪包括油脂、分离乳清中的脂肪和大米蛋白中的脂肪中的至少一种;所述的油脂包括植物油;所述的植物油为质量比120-150:1-3:8-10:1-3:1-3:5-7:12-15:3-5:1-3的玉米胚芽油、芝麻油、菜籽油、葵花籽油、橄榄油、亚麻籽油、花生油、核桃油和奇亚籽油。
4.根据权利要求1所述的低GL膳食配方,其特征在于:所述的碳水化合物为全谷物粉;所述的谷物粉选自小麦粉、玉米粉、荞麦粉、小米粉、高粱面粉、大米粉、糟米粉和青稞粉中的至少一种。
5.根据权利要求1所述的低GL膳食配方,其特征在于:配方一中所述的复合维生素为维生素A、维生素E、维生素K、维生素B1、维生素B2、维生素B6、维生素B12、烟酸和泛酸;
所述维生素A的含量为135-276μg;所述维生素E的含量为0.375-1.355mg;所述维生素K的含量为0.22μg;所述维生素B1的含量为0.05-0.155mg;所述维生素B2得含量为0.025-0.16mg;所述维生素B6的含量为0.11mg;所述维生素B12的含量为0.495μg;所述烟酸的含量为1.125-2.36mg;所述泛酸的含量为0.825mg。
6.根据权利要求1所述的低GL膳食配方,其特征在于:配方二中所述的复合维生素为维生素A、维生素D、维生素E、维生素K、维生素B1、维生素B2、维生素B6、维生素B12、烟酸、泛酸、胆碱、生物素和维生素C;
所述维生素A的含量为247.8-252.2μg;所述维生素D的含量为0.1-0.15μg;所述维生素E的含量为4.515-6.35mg;所述维生素K的含量为6.02-9.03μg;所述维生素B1的含量为0.26-0.46mg;所述维生素B2得含量为0.045-0.09mg;所述维生素B6的含量为0.15mg;所述维生素B12的含量为0.2-0.3μg;所述烟酸的含量为3.455-6.2mg;所述泛酸的含量为0.7-1mg;所述胆碱的含量为14.88-14.895mg;所述生物素含量为0.4-0.6mg;所述维生素C含量为117mg。
7.根据权利要求1所述的低GL膳食配方,其特征在于:配方三中所述的复合维生素为维生素A、维生素D、维生素E、维生素K、维生素B1、维生素B2、维生素B6、维生素B12、烟酸、泛酸、胆碱、生物素和维生素C;
所述维生素A的含量为247.8-252.2μg;所述维生素D的含量为0.1-0.15μg;所述维生素E的含量为4.515-6.35mg;所述维生素K的含量为6.02-9.03μg;所述维生素B1的含量为0.26-0.46mg;所述维生素B2得含量为0.045-0.09mg;所述维生素B6的含量为0.15mg;所述维生素B12的含量为0.2-0.3μg;所述烟酸的含量为3.455-6.2mg;所述泛酸的含量为0.7-1mg;所述胆碱的含量为14.88-14.895mg;所述生物素含量为0.4-0.6mg;所述维生素C含量为117mg。
8.根据权利要求1所述的低GL膳食配方,其特征在于:所述的复合矿物质组成为钠、钾、钙、磷、镁、铁、锌、铜、锰、碘和硒;
所述钠的含量为97.5-175.3mg;所述钾的含量为76.5-903.8mg;所述钙的含量为0.2-142.8mg;所述磷的含量为84.5-456.5mg;所述镁的含量为27-158.2mg;所述铁的含量为1-10.9mg;所述锌的含量为0.5-3.2mg;所述铜的含量为0.1-0.6mg;所述锰的含量为0.7-3.0mg;所述碘的含量为1.20-12.5μg;所述硒的含量为1.5-10.8μg。
9.根据权利要求1所述的低GL膳食配方,其特征在于:所述的低GL膳食配方中还含有膳食纤维、叶黄素和胡萝卜素,其中配方一中膳食纤维的含量为2.1-4.3g,叶黄素的含量为277.2μg;配方二和配方三中膳食纤维的含量为5.9-9.2g,叶黄素的含量为3490μg、胡萝卜素的含量为90μg。
10.权利要求1-9任一项所述的低GL膳食配方组合物在制备低GL食品中的应用,所述的低GL食品为用于2型糖尿病患者胰岛素强化治疗后代餐食品。
