现有研究显示,肿瘤高表达PD-L1和浸润至肿瘤的CD8+T细胞的数量与免疫检查点抑制剂的应答率密切相关,因此,增加肿瘤组织PD-L1表达、吸引CD8+T细胞至肿瘤组织是提升免疫检查点抑制剂疗效的一种重要策略。现有研究中表明溶瘤病毒感染肿瘤即起到既可以诱导肿瘤组织PD-L1的表达,也可以招募CD8+T细胞到达肿瘤组织的作用,其原理主要是通过感染并破坏肿瘤细胞,通过释放损伤相关分子模式(DAMPs)、病原体相关分子模式分子(PAMPs)和细胞因子来刺激全身抗肿瘤免疫,招募CD4+和CD8+细胞以破坏肿瘤。因此,利用溶瘤病毒的这一特性,联合免疫检查点抑制剂,达到提高肿瘤治疗疗效的目的是***的另一个策略。
卡瑞利珠单抗(商品名:艾瑞卡Camrelizumab for Injection)属于免疫检查的抑制剂,或者叫PD-1抑制剂,是一种人类免疫球蛋白G4(IgG4)单克隆抗体(HuMAb),可与PD-1受体结合,阻断其与PD-L1和PD-L2之间的相互作用,进而阻断PD-1通路介导的免疫抑制反应。安柯瑞,也称H101,为E1B55KD和E3区基因片段缺失的溶瘤病毒,也是中国国家食品药品监督管理局于2005年批准的首个也是唯一一个治疗头颈癌的腺病毒。现有研究中,暂时并未发现两者联合应用用于***的研究,也没有其两者具有协同治疗癌症的报道。
现有技术通过瘤内注射的方式注射一种E1B55KD缺失的腺病毒ONYX-015治疗胰腺癌,其I/II期临床试验结果显示,其具有安全性高、抗肿瘤效果微弱的特点。因此,虽然在过去的几十年中许多临床研究表明溶瘤病毒具有强大的抗肿瘤作用,但其临床试验的结果并不满意,而免疫检查点抑制剂治疗应答率低。因此,针对溶瘤病毒及免疫检查点抑制剂治疗癌症的不足,提供一种联合两者的优缺点,用以达到提高***疗效的目的,是肿瘤治疗研发的重要方向。
发明内容
针对进展期或转移性癌无法治愈的问题,针对利用安柯瑞联合传统的治疗方法,安柯瑞感染效率及溶瘤效果不好的问题,本发明提供了一种新的方案,通过安柯瑞和卡瑞利珠单抗的联合应用,显著增强了卡瑞利珠单抗对癌症的治疗效果。
为了达到上述目的,本发明采用以下技术方案:
本发明的第一个目的是提供一种用于***的药物组合物,所述药物组合物包括安柯瑞和卡瑞利珠单抗。
优选的,所述药物组合物的剂型为注射剂。
优选的,所述肿瘤为对安柯瑞不敏感的肿瘤。
优选的,所述肿瘤为实体瘤。
优选的,所述实体瘤选自肺癌、乳腺癌、结肠癌、卵巢癌、胰腺癌、结肠直肠癌、膀胱癌、***癌、***、肾癌和黑素癌。
优选的,所述肿瘤选自手术、化学治疗、放射治疗或其组合的治疗之后是复发性的或进行性。
优选的,所述药物组合物的施用方式包括向受试者同时或依次施用所述安柯瑞和卡瑞利珠单抗。
优选的,所述安柯瑞的施用剂量为8×104pfu/mm3,每2天一次;所述卡瑞利珠单抗施用剂量为0.16μg/mm3,每4天一次。
本发明的第二个目的是提供一种如上述所述的药物组合物在制备用于治疗受试者中癌症的药物中的用途。
本发明采用安柯瑞和卡瑞利珠单抗进行联合应用,其设计原理及构思如下:
首先,本发明通过采用人源化免疫***小鼠肿瘤模型研究了安柯瑞与卡瑞利珠单抗联合使用对癌症的抗肿瘤作用,结果表明安柯瑞与卡瑞利珠单抗联合治疗比单独使用任何一种药物都能产生更显著的抗肿瘤效果。
其次,为了进一步研究安柯瑞介导抗肿瘤免疫反应的机制,本发明检测了安柯瑞感染的肿瘤细胞中免疫相关标志物,发现病毒感染癌症细胞后,导致癌症细胞CD47下调,并且促进了诱导的THP-1细胞对YTS-1细胞的吞噬作用。由此可以得到,安柯瑞介导抗肿瘤免疫反应的机制,是通过抑制肿瘤细胞表面的CD47,进而促进巨噬细胞对病毒感染的肿瘤细胞的吞噬作用。
然后,为了进一步研究CD47的下调以及巨噬细胞吞噬作用的激活对T细胞活力影响,本发明通过实验进一步证明了安柯瑞激活巨噬细胞的吞噬作用,并进一步促进了CD8+T细胞的活化。
最后,由于巨噬细胞来源的IL-12在诱导T细胞分泌IFN-γ的过程中起重要作用,本发明通过采用抗IL-12中和抗体去除巨噬细胞来源的IL-12进行实验,结果发现T细胞不再分泌IFN-γ,证实了巨噬细胞来源的IL12诱导了T细胞分泌IFN-γ。
综上所述,由上述实验结果表明安柯瑞通过抑制受感染细胞CD47的信号通路增强了卡瑞利珠单抗对癌症的治疗效果,也就是说,安柯瑞与卡瑞利珠单抗的联合应用,显著增强了对膀胱癌的治疗效果。本发明为安柯瑞联合卡瑞利珠单抗***提供了依据,具有重要的临床意义,为进一步开展临床试验奠定了基础。
与现有技术相比,本发明具有以下有益效果:
本发明利用动物模型(小鼠皮下肿瘤模型)研究了安柯瑞与卡瑞利珠单抗联合使用对癌症的抗肿瘤作用,实验结果表明,与单药相比,安柯瑞与卡瑞利珠单抗联合使用可协同抑制肿瘤生长,并延长肿瘤小鼠的生存时间,安柯瑞与卡瑞利珠单抗联合治疗比单独使用任何一种药物都能产生更显著的抗肿瘤效果。
此外,本发明还进一步通过实验验证了安柯瑞介导抗肿瘤免疫反应的机制,为安柯瑞联合免疫检查点抑制剂***提供了依据,具有重要的临床意义,为进一步开展临床试验奠定了基础。

具体实施方式
以下通过实施例形式的具体实施方式,对本发明的上述内容作进一步的详细说明。但不应将此理解为本发明上述主题的范围仅限于以下实施例。
1、实验用细胞株
人肌层浸润性膀胱癌细胞株T24,中表达CAR,购自上海细胞库;人恶性胶质母细胞瘤U-87MG,高表达CAR,购自上海细胞库;人肌层浸润性膀胱癌细胞株YTS-1,不表达CAR,由日本东北大学提供。其中,YTS-1存在p53突变、p16甲基化;T24存在p53突变。YTS-1低表达腺病毒受体(CAR),T24细胞中等表达CAR,U-87MG高表达CAR,用于对照。
其中,T24和YTS-1细胞在添加10%(vol/vol)胎牛血清(FBS)和1%抗生素(100U/mL青霉素,100μg/mL硫酸链霉素)的RPMI-1640培养基中培养;U87-MG细胞在添加10%胎牛血清(FBS)和1%抗生素的DMEM中培养;THP-1细胞在添加10%胎牛血清、1%抗生素和β-巯基乙醇(0.05mM)的RPMI-1640培养基中培养。
所有细胞置于37℃加5%(vol/vol)CO2的湿化培养箱中。
2、实验用溶瘤腺病毒
安珂瑞:安柯瑞,也称H101,为E1B55KD和E3缺失的溶瘤腺病毒。实验前对病毒扩增并进行滴度测定,方法如下:293细胞培养于10cm培养皿中,培养至合适密度时加入腺病毒,感染3-5天至细胞漂浮后,收集细胞用冻融法使细胞裂解,高速离心,收集上清液即病毒液,-80℃保存。用293T细胞测定病毒滴度。
3、实验动物
NSG小鼠(雌性,6-8周龄)购自集萃药康(中国江苏),小鼠饲养在SPF环境中,实验方案经浙江大学医学院动物护理与使用委员会批准。
4、数据分析
本发明实验结果均利用SPSS软件进行数据分析,结果用平均值±标准误表示,采用非配对t检验进行统计学分析,P<0.05表示统计学上有显著性差异。
实施例1安柯瑞单药介导的抗肿瘤效果
1.1不同细胞对安柯瑞介导的细胞凋亡和溶瘤效果影响实验
为了确定不同细胞对病毒的易感性是否会影响安柯瑞对细胞的毒性效果及溶瘤效果,并进一步影响抗肿瘤免疫效果,本发明采用表达不同水平的细胞株验证安柯瑞介导的细胞凋亡和溶瘤效果。
采用western blotting检测不同水平的细胞株CAR的表达,实验步骤如下:收集细胞,PBS洗2次。然后用加入1mM PMSF的RIPA裂解缓冲液(碧云天,P0013B)在冰上裂解细胞30分钟。细胞裂解物经SDS-PAGE(12%)分离,转移到PVDF膜上(Millipore,ISEQ00010)。用5%BSA封闭,并与抗CAR(Proteintech,11777-1-AP)或抗GAPDH(Abways Technology,AB0036)抗体在4℃下孵育过夜。第二天,用PBST冲洗膜3次,室温下用山羊抗兔IgG(H+L)HRP抗体(联科,GAR007)孵育1h。在膜上涂抹UltraSignal ECL Western Blotting Detection Reagent(四正柏,4AW011-1000),然后使用Tanon 4500凝胶成像***(tanon 4500,天能公司)对膜进行扫描。
通过凋亡分析检测安柯瑞诱导细胞凋亡:采用Annexin V/PI双染色法进行凋亡分析,将培养好的细胞收集,洗涤,离心,重悬,加入荧光(SA-FLOUS)溶液4℃下孵育20min,避光并不时振动。采用流式细胞计数法检测凋亡细胞,具体地,收集U87-MG、YTS-1或T24细胞,用PBS洗涤,用Annexin V-APC/PI凋亡试剂盒(联科,AP107)按说明书进行检测。
实验结果如图1a和图1b所示,由图1a可知,CAR高表达的细胞株U87-MG被安柯瑞感染后凋亡显著,且凋亡比例与病毒滴度密切相关;由图1b可知,低表达CAR的YTS-1细胞株无明显凋亡。
1.2安柯瑞对皮下肿瘤的抗肿瘤效果实验
为了进一步确定安柯瑞对小鼠皮下肿瘤模型的治疗效果,本发明通过小鼠皮下肿瘤模型,采小鼠皮下肿瘤形成后,瘤内注射人外周血单个核细胞(PBMC),随后瘤内注射安柯瑞。测定小鼠肿瘤体积并观察鼠的生存率来评价治疗效果。采取组织行病理学检查及免疫相关指标。
1.2.1构建人源化小鼠肿瘤模型
单个NSG小鼠皮下注射3×106U87-MG,5×106T24或YTS-1细胞建立的皮下肿瘤模型。当肿瘤体积达到100mm3时,每2天通过瘤内注射安柯瑞(每个肿瘤2.5×107pfu)。联合治疗时,当肿瘤体积达到300mm3时,每2天通过瘤内注射安柯瑞(每个肿瘤2.5×107pfu)和/或每4天接受50μg Camrelizumab治疗。在NSG小鼠瘤内注射1×106从健康捐献者外周血中分离出PBMCs。其中,PBMCs为根据说明书用淋巴细胞分离培养基(TBDsciences,LTS1077)分离得到。每2或3天用游标卡尺测量肿瘤大小,肿瘤体积按以下公式计算:体积=最大直径×最小直径2×0.5。
1.2.2PBMC的提取
抽取健康人血液,再加入生理盐水稀释备用。取离心管加入淋巴细胞分离液(LTS1077,天津),再加入上述稀释的血液至淋巴细胞分离液上面,离心。吸出PBMC,离心,加T细胞培养液重悬,再次离心,用T细胞培养液重悬后加入CD3抗体孵化的6孔板培养72小时
1.2.3组织病理学及免疫相关指标检测
取肿瘤组织,免疫荧光检测CD8+T浸润程度。ELISA(BioLegend)检测各组肿瘤裂解液(Tumor Lysate)中相关细胞因子TNF-α(430204)、IL-12(431704)和IFN-γ(430104)。
所述免疫荧光检测过程如下:治疗后第7天分离肿瘤组织。将组织放入Tissue-Tek中O.C.T.化合物。然后,使用CryoStar NX50(Thermo Fisher)切割10μm厚的冷冻切片。然后用-20℃预冷甲醇固定切片10分钟,0.1%Triton-X-100渗透。用5%BSA和3%山羊血清PBS中封闭后。抗CD8(proteintech,60181-1-Ig)和抗IFN-γ(proteintech,15365-1-AP)在4℃孵育过夜。第二天,用PBS洗细胞3次,用IFLUORTM 488-(华安,HA1211)and IFLUORTM549-(华安,HA1126)标记的二抗,室温孵育30分钟。PBS洗涤3次后,用DAPI(Invitrogen,D3571)染色细胞核。用Olympus IX83FV3000共聚焦显微镜检测荧光信号。
1.2.4实验结果
如图1c和图1d所示,由图1c和图1d可知,中、高表达CAR的U87-MG、T24细胞建立的皮下肿瘤模型在瘤内注射安柯瑞治疗后显示了显著的肿瘤生长抑制效果,并显著延长了小鼠生存时间;而低表达CAR的YTS-1细胞株建立的皮下肿瘤模型在瘤内注射安柯瑞治疗后只有轻微的肿瘤生长抑制效果。
1.3结果分析
由上述实验结果可知,安柯瑞导致的细胞凋亡和溶瘤效果与CAR的表达密切相关。
实施例2安柯瑞联合卡瑞利珠单抗介导的抗肿瘤效果
建立免疫***人源化小鼠皮下肿瘤模型,采用瘤内注射安柯瑞或腹腔内注射卡瑞利珠单抗治疗。通过实施例1所述方法对安柯瑞联合卡瑞利珠单抗介导的抗肿瘤效果进行检测和评估,并通过免疫组化检测肿瘤组织CD8+T细胞。
实验结果如图2所示,由图2a可知,安柯瑞单独使用显著抑制了CAR阳性细胞株建立的皮下肿瘤模型,而对CAR阴性细胞株YTS-1建立的皮下肿瘤模型无明显抑制效果;卡瑞利珠单抗单独使用也不同程度地抑制了肿瘤生长;两药联合治疗组无论是对CAR阳性还是阴性细胞株建立的皮下肿瘤都显示了比单药组更加显著的肿瘤生长抑制效果;由此可知,安柯瑞联合卡瑞利珠单抗治疗显著抑制了小鼠皮下肿瘤生长。由图2b可知,安柯瑞联合卡瑞利珠单抗显著延长了荷瘤小鼠生存时间。由图2c可知,免疫组化检测肿瘤组织CD8+T细胞,免疫荧光显示了两药联合治疗组中浸润至肿瘤组织的表达IFN-γ的CD8+T细胞数量显著增加,显著高于单药组,证实了联合治疗增强了表达IFN-γ的CD8+T细胞的抗肿瘤反应。因此,由图2的实验结果可知,与单药相比,安柯瑞与卡瑞利珠单抗联合使用可协同抑制肿瘤生长并延长荷瘤小鼠的生存时间,免疫组化实验结果证实了联合治疗增强了表达IFN-γ的CD8+T细胞的抗肿瘤反应。
现有研究表明,肿瘤浸润性T细胞与抗PD-1治疗的应答率有关,对PD-1阻断治疗无应答的患者更可能在肿瘤病灶内缺乏CD8+T细胞。而本发明通过安柯瑞联合卡瑞利珠单抗的联合应用,利用安柯瑞募集CD8+T细胞进入肿瘤的策略进一步提高PD-1阻断治疗的抗肿瘤效果,是癌症免疫治疗的一种有前途的策略。
实施例3安柯瑞联合卡瑞利珠单抗介导抗肿瘤免疫反应的机制
由实施例2的实验结果安柯瑞诱导肿瘤微环境(TME)中PD-L1的上调,联合抗PD-L1治疗可产生协同作用,从而获得更好的治疗效果,可以初步推断出安柯瑞介导抗肿瘤免疫反应的机制可能为:安柯瑞感染肿瘤后募集CD8+T细胞到肿瘤微环境(TME)中,PD-1阻断进一步激活CD8+T细胞攻击肿瘤。
为了进一步地研究并验证安柯瑞介导抗肿瘤免疫反应的机制,本发明检测安柯瑞感染的肿瘤细胞中免疫相关标志物。
3.1安柯瑞感染肿瘤细胞后免疫因子mRNA的变化
使用RNAiso plus(TAKARA,9109)从肿瘤细胞中提取总RNA,按照说明书,用HiScriptII Q RT SuperMix(诺唯赞,R223-01)将600ng RNA反转录为cDNA。定量实时PCR采用ChamQ Universal SYBR qPCR Master Mix(诺唯赞,Q711-02)进行,使用CFX96PCR检测***(Bio-Rad),其中,PCR采用以下热循环条件:95℃,90s,1个循环,随后95℃,5s,40个循环,60℃,34s,40个循环,反应体系如下表1所示。

实验结果如图3a所示,由图3a可知,安柯瑞感染肿瘤细胞导致肿瘤细胞CD47mRNA表达明显下降。
3.2流式检测CD47在安柯瑞感染细胞中的表达
收集细胞,用PBS洗涤3次。用表面标记物抗体染色后,用IC固定缓冲液(ThermoFisher Scientific)渗透细胞,染色检测细胞内细胞因子。其中,流式细胞仪采用CytoFlex流式细胞仪(Beckman Coulter,Brea,CA,USA)进行流式细胞检测,数据分析采用FlowJo软件(TreeStar Ashland,OR,USA)。在染色细胞中使用了以下抗体:Fixable Viable DyeeFluorTM 520(Invitrogen,65-0867-14)、抗人PE-CD8a(BioLegend,301007)和抗人APC-IFN-γ(BioLegend,502511)、抗人APC-CD45((BioLegend,982304)。
实验结果如图3b所示,由图3b可知,安柯瑞感染肿瘤细胞导致肿瘤细胞CD47蛋白水平也下降。
3.3流式检测巨噬细胞吞噬肿瘤细胞的数量
由于CD47在抑制巨噬细胞吞噬肿瘤细胞中发挥重要作用,肿瘤表达CD47不被巨噬细胞吞噬,导致免疫逃逸。证明了病毒介导的CD47下调是否会促进吞噬,本发明通过诱导THP-1细胞分化成为M0巨噬细胞,将安柯瑞感染的肿瘤细胞与诱导的THP-1细胞共同培养,检测细胞因子TNF、IL-12、IFN-γ的变化。
用50ng/mL PMA刺激THP-1 12小时诱导M0巨噬细胞。根据说明书,用ELISA试剂盒分析诱导的THP-1和肿瘤组织的细胞因子水平浓度。其中,人IFN-γ(430104)、IL-12(431704)、TNF-α(430204)的ELISA试剂盒均购自BioLegend。
在收集肿瘤细胞前,用安柯瑞(MOI=1.5)刺激肿瘤细胞24小时。根据说明书,用CFSE(Invitrogen,C34570)对细胞进行染色。将CFSE标记的细胞种到6孔板中,与诱导的THP-1细胞按3:1的比例共培养。8小时后,收集诱导的THP-1细胞,用CytoFlex流式细胞仪(Beckman Coulter,Brea,CA,USA)检测。
实验结果如图3c所示,由图3c可知,安柯瑞感染导致膀胱癌细胞CD47下调,并促进了诱导的THP-1细胞对YTS-1细胞的吞噬作用,由此可见,安柯瑞的感染显著增强了吞噬细胞对肿瘤细胞的吞噬作用。
3.4结果分析
由上述实验结果可知,由此可推断出,安柯瑞可能通过感染肿瘤细胞致使其CD47表达下调从而增强了巨噬细胞对病毒感染的肿瘤细胞的吞噬作用。
实施例4巨噬细胞的激活对T细胞活力的影响
4.1采用ELISA方法检测THP-1与安柯瑞感染的肿瘤细胞共培养后细胞因子TNF、IL-12和IFN-γ的表达。实验结果如图4a所示,THP-1细胞与安柯瑞感染的膀胱癌细胞共培养导致了细胞因子TNF、IL-12和IFN-γ表达增加。
4.2将巨噬细胞的上清液添加到CD8+T细胞中,THP-1细胞与安柯瑞感染的癌细胞共培养上清液刺激后流式检测CD8+T细胞IFN-γ的表达(YTS-1细胞株)。实验结果如图4b所示,IFN-γ的表达升高。
4.3在安柯瑞与卡瑞利珠单抗联合治疗的肿瘤组织中,采用ELISA方法检测肿瘤组织TNF、IL-12和IFN-γ的表达;免疫组化检测YTS-1肿瘤组织IFN-γ的表达。实验结果如图4c和图4d所示,在安柯瑞与卡瑞利珠单抗联合治疗的肿瘤组织中,TNF、IL-12和IFN-γ的水平增加。
由上述实验结果可知,安柯瑞激活巨噬细胞的吞噬作用并进一步促进了CD8+T细胞的活化。也即安柯瑞激活了巨噬细胞,进一步激活了T细胞。
实施例5安柯瑞联合卡瑞利珠单抗介导抗肿瘤免疫反应的机制的进一步验证
由于IL-12在诱导T细胞分泌IFN-γ的过程中起重要作用,为了验证IL-12在T细胞分泌IFN-γ的过程中所发挥的作用,本发明采用抗IL-12中和抗体处理上述共培养的上清液消除IL-12,通过流式检测CD8+IFN-γ+T细胞。实验结果如图5a所示,实验结果显示T细胞不再产生IFN-γ,表明病毒感染促进巨噬细胞吞噬肿瘤细胞并产生IL-12进而诱导T细胞分泌IFN-γ。
另有研究显示IFN-γ可诱导肿瘤PD-L1的上调,进而影响T细胞PD-1信号通路。本发明通过流式检测受病毒感染肿瘤细胞的PD-L1表达,实验结果如图5b所示,安柯瑞感染YTS-1细胞后诱导其PD-L1表达增加,从而激活了T细胞PD-1信号通路,增强抗肿瘤免疫反应。
综上所述,由本发明实验结果可知,在小鼠皮下肿瘤模型中,安柯瑞H101与卡瑞利珠单抗Camrelizumab for Injection联合使用比单药治疗显示了更强的抗肿瘤效果。通过分析治疗的肿瘤,发现联合治疗组中肿瘤浸润性T细胞增加,尤其是表达IFN-γ的CD8+T细胞;此外,在H101治疗或联合治疗的肿瘤组织中,包括TNF、IL-12和IFN-γ在内的细胞因子表达增加。这些结果表明安柯瑞H101间接激活巨噬细胞,然后诱导T细胞的活化,支持与卡瑞利珠单抗阻断的联合治疗。
结合安柯瑞介导抗肿瘤免疫反应的机制,本发明采用安柯瑞和卡瑞利珠单抗进行联合应用,安柯瑞可以通过抑制受感染细胞CD47的信号通路,进而增强卡瑞利珠单抗对膀胱癌的治疗效果,因此,本发明为安柯瑞联合免疫检查点抑制剂***提供了依据,具有重要的临床意义,为进一步开展临床试验奠定了基础。

Claims (9)
1.一种用于***的药物组合物,其特征在于,所述药物组合物包括安柯瑞和卡瑞利珠单抗。
2.根据权利要求1所述的药物组合物,其特征在于,所述药物组合物的剂型为注射剂。
3.根据权利要求1所述的药物组合物,其特征在于,所述肿瘤为对安柯瑞不敏感的肿瘤。
4.根据权利要求1所述的药物组合物,其特征在于,所述肿瘤为实体瘤。
5.根据权利要求5所述的药物组合物,其特征在于,所述实体瘤选自肺癌、乳腺癌、结肠癌、卵巢癌、胰腺癌、结肠直肠癌、膀胱癌、***癌、***、肾癌和黑素癌。
6.根据权利要求1所述的药物组合物,其特征在于,所述肿瘤选自手术、化学治疗、放射治疗或其组合的治疗之后是复发性的或进行性。
7.根据权利要求1所述的药物组合物,其特征在于,所述药物组合物的施用方式包括向受试者同时或依次施用所述安柯瑞和卡瑞利珠单抗。
8.根据权利要求1所述的药物组合物,其特征在于,所述安柯瑞的施用剂量为8×104pfu/mm3,每2天一次;所述卡瑞利珠单抗施用剂量为0.16μg/mm3,每4天一次。
9.一种如权利要求1所述的药物组合物在制备用于治疗受试者中癌症的药物中的用途。
