技术领域

背景技术
高海拔诱发的中枢性睡眠呼吸暂停是常见的紊乱,多达25%的人群在7500英尺的高度处呈现出症状,并且约85%的人群在高于12000英尺的高度处呈现出症状。在高海拔处,呼吸性碱中毒是常见的,并且可能导致通气不稳定,具体是由于PaCO2与呼吸暂停阈值之间的差的降低,但也可能是由于回路增益的低碳酸血症增加。在睡眠期间,这种通气不稳定表现为呼吸的周期性暂停的持续时间超过十秒,这典型地与血氧饱和度降低、睡眠片断化和如头痛、疲劳、维持睡眠困难(睡眠维持性失眠)等的其它后遗症相关联。呼吸的暂停最典型地是由于呼吸努力的停止(中枢性呼吸暂停),但是由于回路增益的变化,阻塞性睡眠呼吸暂停也可能在海拔处显著恶化。
通常,高海拔诱发的中枢性睡眠呼吸暂停经常预防性地通过乙酰唑胺(Diamox)来处理,和/或通过夜间补充的低流量氧气来处理。两种处理都具有有问题的副作用,包括对于乙酰唑胺的感觉异常、头痛、恶心、尿频和味觉改变。低流量氧气使用具有皮肤刺激和鼻干燥的副作用。由于使用和运输低流量氧疗(即鼻插管和氧气浓缩器或压缩氧气瓶)的不便,低流量氧疗在许多情况下可能不优选用于处理。

发明内容

睡眠呼吸紊乱检测模块
睡眠呼吸紊乱检测模块的意图是确定睡眠呼吸紊乱的存在和严重性,优选地在每晚的基础上。如本领域技术人员所知,存在一定数目的不同方法来评估睡眠呼吸紊乱严重性。如本文所采用的,表述“一定数目的”及其变体应广义地指代任何非零数量,包括一的数量。
生理传感器数据
在所公开和要求保护的构思中,生理传感器数据可以从光电体积描记器(PPG)和加速度计/陀螺仪获得,光电体积描记器(PPG)具有用于监测脉动血容积变化以及SpO2两者的适当电磁频谱分量,加速度计/陀螺仪用于测量诸如智能手表或其它可穿戴设备的可穿戴形状因子中的体动记录。在一些实施例中,仅测量提及的信号的子集。备选实施例可以使用不同的传感器配置,诸如通过使用麦克风、光学传感器、压电传感器等。
问卷数据

SDB数据输出
在所公开和要求保护的构思中,SDB数据是数字严重性标度,该数字严重性标度包括SDB严重性的客观量度(Obj.SDB Measure)和睡眠质量的主观量度(Subj.SDB Measure)的函数。在示例性实施例中,根据以下等式1组合客观和主观量度:

客观SDB量度
在所公开和要求保护的构思中,处理PPG/SpO2和体动记录信号以确定中枢性呼吸暂停指数(CAI),即每小时睡眠的中枢性呼吸暂停的数目。在另一个所公开和要求保护的构思中,使用全呼吸暂停/呼吸不足指数(AHI)。为了确定CAI或AHI,***处理PPG和加速计传感器以确定觉醒、呼吸停止和氧饱和度降低的指示。在其它实施例中,仅使用SpO2信号以便确定针对客观SDB量度的氧饱和度降低指数(ODI)。可以用于客观睡眠呼吸紊乱严重性的其它量度包括觉醒(例如,作为心率加速的数目的觉醒指数等)、睡眠片断化(例如,醒来的数目)、睡眠开始之后的醒来(WASO,其通常被测量为在最初入睡之后醒来的总分钟数)等的指数。
主观SDB量度
在所公开和要求保护的构思中,主观SDB量度是对单个问题的回答,该单个问题诸如为:“在零到十的标度上,您认为您昨晚的睡眠使精力恢复得如何?”。在其他实施例中,询问一定数目的问题,并且主观SDB量度是对该一定数目的问题的评分合计响应。被询问以确定主观SDB量度的问题可以与诸如警觉性、疲劳等的白天功能和/或睡眠质量/满意度有关,并且问题可以在一天中被询问一次或多次。
海拔相关模块
海拔相关模块的目的是确定人有可能遭受预定水平的SDB的海拔,该预定水平的SDB典型地将是显著水平的SDB。海拔相关模块还有利地确定可以外插和内插的函数,以便确定在不同海拔处的SDB严重性的预测程度。海拔相关模块将SDB严重性和确定SDB严重性的海拔的量度作为输入。海拔相关模块确定SDB的严重性超过(或预计超过)预定水平的严重性的海拔。在所公开和要求保护的构思中,该预定水平是十分制标度中的二,这将是AHI增加大约10,这取决于白天的影响。
图3中所示的示例描绘了行进到不同海拔的人的每日SDB严重性评级和对应海拔。以下示例中的人在高于4000英尺的任何海拔处具有显著的海拔诱发的睡眠呼吸紊乱。

在所公开和要求保护的构思中,SDB相关模块使用最小平方误差方法根据海拔来导出SDB严重性的示例性二次曲线拟合。诸如指数、幂等的其它曲线形状因子以及其它曲线拟合方法也在所公开和要求保护的构思的范围内,包括分段线性和非线性等。
海拔数据
在所公开和要求保护的构思中,海拔数据由可穿戴设备上的大气压力传感器提供,以估计SDB严重性度量被确定的夜晚的当前有效海拔。在该实施例中,以kPa为单位测量大气压力,并将大气压力与有效海拔相关(例如,海平面=101kPa,5000英尺海拔=84.3kPa,10000英尺海拔=69.7kPA等)。在其它实施例中,大气压力或海拔通过其他手段提供,该其他手段包括GPS、直接用户数据输入、与诸如智能电话的另一设备的通信等。
高海拔诱发的SDB诊断和用户通知

剂量功效建模模块

剂量数据
在所公开和要求保护的构思中,每日剂量由人手动提供,即,Diamox的剂量为125mg或2LPM的低流量氧气。在其它实施例中,诸如通过氧气输送设备或药物分配器自动提供剂量信息。
个性化剂量模型
在所公开和要求保护的构思中,用于导出在给定海拔和剂量处的预计SDB的剂量模型如下(等式2),等式2采用等于处于海平面的人的SDB严重性的最小SDB严重性“C”。等式2还考虑了有效海拔(Altitude)和SDB治疗的剂量(Dose)。

在所公开和要求保护的构思中,在人处于比根据海拔相关模块预测他们具有显著的海拔诱发的SDB的海拔更大的海拔处的任何时间通知该人。这样的通知可以指示该人处于SDB的升高的风险中,意味着SDB严重性高于预定阈值或预定值,并且可以进一步指示为了改善症状将期望的适当的治疗剂量。通知的示例包括智能电话或智能手表通知、电子邮件等。在一些实施例中,仅在人接近习惯性就寝时间时做出通知。在其他实施例中,该通知是在该人被指示(例如通过自我报告或以其他方式)服药时的时间做出的。在其他实施例中,向该人呈现由剂量预测模块准备的数据的报告以呈现给医师,以便于治疗的处方或治疗的剂量范围(例如乙酰唑胺)。
装置和方法

输入装置24可以被描述为包括多种多样的监测设备,诸如(通过示例的方式,但不限于)PPG监测设备32、大气压力监测设备36、GPS监测设备40、加速计/陀螺仪44、以及诸如麦克风、光学传感器、压电传感器等的各种其他监测设备中的任一种,由输入装置24将这些输入信号中的每个输入信号作为输入信号输入到处理器12。
输出装置28包括一定数目的输出设备,该一定数目的输出设备包括视觉显示器52和控制致动器56。视觉显示器52可以在视觉上描绘例如图3-图5的主题,和/或可以以处方的形式描绘对临床医生的指令。
控制致动器56可以多种多样的方式中的任一种来使用,诸如提供个性化睡眠治疗。在这点上,可以理解,控制致动器56和输入装置24结合处理器装置8和例程20可以形成闭环控制***,该闭环控制***使定制的处方剂量适于该人或者使氧气流速适于该人。例程20的其他执行方法可以包括机器学习模块等,以便以实现的简单性为代价来提高用户的个性化准确度。

Claims (19)

1.一种对人的海拔诱发的中枢性睡眠呼吸暂停(CSA)进行建模的方法,包括:
针对所述人的一定数目的睡眠时段中的每个睡眠时段,确定一定数目的睡眠呼吸紊乱(SDB)严重性值和一定数目的有效海拔值,所述一定数目的SDB严重性值中的每个SDB严重性值对应于所述一定数目的有效海拔值中的有效海拔值;
至少部分地基于所述一定数目的SDB严重性值的至少子集和所述一定数目的有效海拔值的至少子集,确定所述人有可能遭受预定水平的SDB严重性的有效海拔;以及
生成至少部分地基于所述有效海拔的输出。
2.根据权利要求1所述的方法,其中所述SDB严重性值中的至少一些SDB严重性值各自至少部分地基于SDB严重性的客观量度和SDB严重性的主观量度中的至少一者。
3.根据权利要求1所述的方法,其中所述SDB严重性值中的至少一些SDB严重性值各自至少部分地基于SDB严重性的客观量度和SDB严重性的主观量度两者。
4.根据权利要求1所述的方法,进一步包括:生成表示所述人处于所述有效海拔处或处于高于所述有效海拔处的通知,作为所述输出。
5.根据权利要求4所述的方法,进一步包括:在接近所述人的习惯性就寝时间的时间生成所述通知。
6.一种确定用于人的海拔诱发的中枢性睡眠呼吸暂停(CSA)处理的剂量的方法,包括:
针对所述人的一定数目的睡眠时段中的每个睡眠时段,确定一定数目的睡眠呼吸紊乱(SDB)严重性值,所述一定数目的SDB严重性值中的每个SDB严重性值至少部分基于对应有效海拔值和CSA处理的对应剂量;
针对给定的有效海拔,至少部分地基于所述一定数目的睡眠呼吸紊乱(SDB)严重性值的至少子集来确定所述CSA处理的优选剂量,在所述优选剂量处可能的SDB严重性值不大于预定的SDB严重性值;以及
生成至少部分地基于所述剂量的输出。
7.根据权利要求6所述的方法,其中所述优选剂量是所述CSA处理的最小剂量,在所述最小剂量处所述可能的SDB严重性值不大于所述预定的SDB严重性值。
8.根据权利要求6所述的方法,进一步包括:生成表示所述优选剂量的通知,作为所述输出。
9.根据权利要求8所述的方法,进一步包括:在接近所述人的习惯性就寝时间的时间生成所述通知。
10.根据权利要求8所述的方法,其中所述通知进一步表示所述人处于所述给定的有效海拔处或处于高于所述给定的有效海拔处。
11.根据权利要求6所述的方法,其中所述一定数目的SDB严重性值中的至少一些SDB严重性值中的每个SDB严重性值进一步至少部分地基于处于海平面的所述人的SDB严重性值。
12.根据权利要求6所述的方法,进一步包括:在至少部分地基于所述一定数目的睡眠呼吸紊乱(SDB)严重性值的所述至少子集来确定所述优选剂量之前,至少部分地基于数据集来确定初始优选剂量,所述数据集包括针对具有海拔诱发的CSA的一定数目的其他个体的一个或多个SDB严重性值和一个或多个对应有效海拔值以及所述CSA处理的一个或多个对应剂量。
13.一种被构造为确定用于人的海拔诱发的中枢性睡眠呼吸暂停(CSA)处理的剂量的装置,包括:
处理器装置,其包括处理器和存储设备;
输入装置,其被构造为向所述处理器装置提供一定数目的输入信号;
输出装置,其被构造为从所述处理器装置接收一定数目的输出信号;以及
所述存储设备具有存储在其中的一定数目的指令,所述一定数目的指令当在所述处理器上执行时使所述装置执行一定数目的操作,所述一定数目的操作包括:
针对所述人的一定数目的睡眠时段中的每个睡眠时段,确定一定数目的睡眠呼吸紊乱(SDB)严重性值,所述一定数目的SDB严重性值中的每个SDB严重性值至少部分地基于对应有效海拔值和CSA处理的对应剂量;
针对给定的有效海拔,至少部分地基于所述一定数目的睡眠呼吸紊乱(SDB)严重性值的至少子集来确定所述CSA处理的优选剂量,在所述优选剂量处可能的SDB严重性值不大于预定的SDB严重性值;以及
生成至少部分地基于所述剂量的输出。
14.根据权利要求13所述的装置,其中所述优选剂量是所述CSA处理的最小剂量,在所述最小剂量处所述可能的SDB严重性值不大于所述预定的SDB严重性值。
15.根据权利要求13所述的装置,其中所述操作进一步包括:生成表示所述优选剂量的通知,作为所述输出。
16.根据权利要求15所述的装置,其中所述操作进一步包括:在接近所述人的习惯性就寝时间的时间生成所述通知。
17.根据权利要求15所述的装置,其中所述通知进一步表示所述人处于所述给定的有效海拔处或处于高于所述给定的有效海拔处。
18.根据权利要求13所述的装置,其中所述一定数目的SDB严重性值中的至少一些SDB严重性值中的每个SDB严重性值进一步至少部分地基于处于海平面的所述人的SDB严重性值。
19.根据权利要求13所述的装置,其中所述操作进一步包括:在至少部分地基于所述一定数目的睡眠呼吸紊乱(SDB)严重性值的所述至少子集来确定所述优选剂量之前,至少部分地基于数据集来确定初始优选剂量,所述数据集包括针对具有海拔诱发的CSA的一定数目的其他个体的一个或多个SDB严重性值和一个或多个对应有效海拔值以及所述CSA处理的一个或多个对应剂量。
