Abstract

本发明公开了一种基于NGS方法检测人类BRCA1和BRCA2基因突变的引物组合物,包括:263对的第一组引物,20对的第二组引物;每对第一组引物或第二组引物均包括正向引物序列与反向引物序列,且所述第一组引物的正向引物序列与反向引物序列均包括可特异性扩增BRCA1/BRCA2基因外显子、内含子或UTR区域的序列,所述第二组引物的正向引物序列与反向引物序列均包括可特异性扩增人类基因组特定参考序列。根据本发明,可有效地检测BRCA1/BRCA2基因的致病点突变、短缺失/***以及拷贝数变异,既可以检测基因组DNA样本,又可以检测肿瘤FFPE样本,可满足对于卵巢癌、乳腺癌的遗传风险评估,以及PARPi抑制剂用药指导,预后评估等目的,整体流程简单高效,具有良好的前景。

Description

一种基于NGS方法检测人类BRCA1和BRCA2基因突变的引物组 合物及其试剂
技术领域
本发明涉及基因组合物的技术领域,特别涉及一种基于NGS方法检测人类BRCA1和BRCA2基因突变的引物组合物及其试剂。
背景技术
根据2018年全球癌症统计报告显示,全球女性乳腺癌新发病例占总体的11.6%,与肺癌并列第一。乳腺癌是女性最常见的癌症(发病率为24.2%),乳腺癌死亡率为15.0%,发病率和死亡率均居于女性发病和死亡的首位。卵巢癌发病率占比3.4%,死亡率占比为4.4%,其发病率和死亡率位于女性发病率和死亡率的第八位。
研究表明,约15%的乳腺癌和卵巢癌患者携带胚系(germline,即来自父母生殖细胞先天遗传)的BRCA1和/或BRCA2基因突变。携带此类胚系突变的女性在25至30岁时就有可能患上乳腺癌或者卵巢癌,而且随着年龄的增长其发病风险逐渐增加,最高竟可达60%至80%。
针对BRCA1和BRCA2基因的功能,目前已经开发了靶向治疗药物—— PARP抑制剂(PARP inhibitor,PARPi)。PARP是一种单链DNA修复酶,与BRCA1/2 基因参与的双链DNA修复机制共同组成细胞主要的DNA修复方式。当BRCA 基因突变后,双链DNA修复机制受阻。这时使用PARPi,能进一步阻滞单链 DNA修复机制,促进癌细胞的DNA损伤得不到修复,不断积累,最终死亡。目前,美国FDA批准的PARPi包括奥拉帕尼(Olaparib),鲁卡帕尼(Rucaparib)和尼拉帕尼(Niraparib),其中奥拉帕尼已在中国被批准上市。随着此类药物的上市,检测BRCA1/2基因突变显得尤为重要:既可以筛查乳腺癌和卵巢癌的高危风险人群,又可以筛选那些潜在的PARPi治疗收益人群。此外,BRCA1/2 基因的突变也参与了其他一些癌症的发生与发展过程,可以作为遗传风险分析因子,如原发性腹膜癌、胰腺癌、***癌等。总之,BRCA1/2基因突变检测的市场前景十分广阔。
BRCA1和BRCA2基因所包含的序列较长,均大于80kb,且不存在热点突变,常规的覆盖少数位点的检测方法不能完整揭示该基因的突变风险。因此,临床上普遍建议采用下一代测序技术(Next-Generation Sequencing,NGS,又称二代测序技术)对BRCA1/2基因进行检测。二代测序技术,是近年来兴起的可以一次对数十万乃至数亿条DNA分子进行测序的高通量检测技术,目前已广泛地应用于产前检测,肿瘤突变分析和用药指导,病原体微生物筛查等领域。与其他技术平台相比,二代测序技术具有检测通量高、速度快、测序长、可检测未知突变等优势,已成为BRCA1/2基因突变检测首选技术平台。
随着临床研究的不断深入,发现不但BRCA1/2胚系突变的肿瘤患者可受益于PARPi类药物,那些肿瘤组织中发生BRCA1/2突变(又称BRCA基因的“体细胞突变”,somaticmutation)的患者同样可以受益于PARPi治疗,其肿瘤无进展生存周期(progression-freesurvival,PFS)与BRCA胚系突变的患者类似。BRCA体细胞突变患者占乳腺癌和卵巢癌患者总数的10%左右,完全可作为肿瘤的用药指导和预后判断因素。有鉴于此,《基于下一代测序技术的BRCA1/2基因检测指南(2019版)》正式将BRCA体细胞突变检测纳入治疗方案评估手段,并列为I级推荐。
然而,目前市场上的产品并不能很好地满足不断提高的BRCA基因突变检测需要。首先,二代测序技术根据其文库构建方式不同,分为探针杂交捕获法和多重扩增法两大类:探针杂交捕获法成本高昂,操作也较为复杂;多重扩增法则需要设计一些相互重叠的扩增片段,而这些重叠片段会相互干扰,因此必须分为2管甚至6-8管进行操作;无论哪种路线的产品操作均比较费时费力。其次,已有的产品特异性、灵敏度普遍不够,需要的样本输入量高,通常需要数百纳克甚至数微克的样本,这对组织样本而言是无法接受的;再次,已有的产品由于其原始设计问题,常不能检测肿瘤组织,尤其是石蜡包埋组织(Formalin-Fixedand Parrffin-Embedded,FFPE)中的体细胞突变;最后,现有的资料表明,一部分BRCA1/2基因突变,无论是胚系还是体细胞突变,属于整个基因或部分外显子的“拷贝数变异”(copynumber variation, CNV),常规的产品无法有效检测。
发明内容
针对现有技术中存在的不足之处,本发明的目的是提供一种基于NGS方法检测人类BRCA1和BRCA2基因突变的引物组合物,除了常规的点突变 (SNVs)和短***/缺失(shortin_dels)之外,还可以检测这两个基因的拷贝数变异(CNV)。在样本类型上,它既可以检测基因组DNA样本,又可以检测肿瘤FFPE样本,可满足对于卵巢癌、乳腺癌的遗传风险评估,以及PARPi 抑制剂用药指导,预后评估等目的,整体流程简单高效,具有良好的前景。为了实现根据本发明的上述目的和其他优点,提供了一种基于NGS方法检测人类BRCA1和BRCA2基因突变的引物组合物,包括:
263对的第一组引物;
每对第一组引物包括正向引物序列与反向引物序列,且所述正向引物序列与反向引物序列均包括可特异性扩增BRCA1/BRCA2基因外显子、内含子或 UTR区域的序列;
所述可特异性扩增BRCA1/BRCA2基因外显子、内含子或UTR区域的序列包括SEQ IDNO.1~526所示的序列,优选的,为其中20-25个碱基。
优选的,还包括20对的第二组引物,且所述第二组引物均包括正向引物与反向引物,所述正向引物与反向引物均包括可特异性扩增人类基因组特定参考区域的序列,作为BRCA1/BRCA2基因拷贝数变异的参考,所述可特异性扩增人类基因组特定参考区域序列均包括SEQ ID NO.527~566所示的序列,优选的,为其中20-25个碱基。
优选的,所述第一组引物、第二组引物的正向引物均包括SEQ ID NO.567 所示的序列,优选的,为其中16-25个碱基。
优选的,所述第一组引物、第二组引物的反向引物均包括SEQ ID NO.568 所示的序列,优选的,为其中16-25个碱基。
一种用于BRCA1和BRCA2基因检测的试剂,包括上文所述的第一组引物、第二组引物。
优选的,还包括三对接头,所述三对接头分别对应Illumina、Ion Torrent、 MGI三种测序平台,且所述三对接头包括与SEQ ID NO.567,SEQ ID NO.568 所述序列配对的DNA片段。
优选的,所述试剂用于来自新鲜或冰冻组织、全血、唾液或FFPE的DNA 样本的分子检测。
优选的,所述试剂用于基于NGS方法的BRCA1和BRCA2基因的检测。
本发明与现有技术相比,其有益效果是:
(1)本发明所述的引物组合物,可用于人类卵巢癌、乳腺癌敏感基因 BRCA1和BRCA2的分子检测,包括点突变、短***/缺失、以及拷贝数变异的检测。检测结果可满足卵巢癌、乳腺癌遗传风险分析,PARPi用药指导,预后评估等需要。
(2)本发明所述的试剂构建文库时为单管反应,操作简单便捷,可在数小时内完成NGS文库构建。扩增子长度为150bp左右,对测序平台的要求较低。测序结果的比对率、中靶率和均一度都较好。
(3)本发明所述的试剂适用于来自受试者的新鲜或冰冻组织、外周血白细胞DNA、唾液DNA、FFPE等多种类型样本的检测,且对样本的要求较低。单次反应的常规样本输入量为20ng,通过优化实验可进一步低至5-10ng,对一定程度片段化的FFPE样本也能有效地检测。检测灵敏度为25%(胚系突变)或2%(体细胞突变)。
(4)本发明所涉及方法简单,总体成本低,填补了国内BRCA1和BRCA2 基因检测领域多重PCR技术构建NGS文库的空白。本发明有助于实现试剂从实验室向临床应用和体外诊断试剂的转化,具有良好的应用前景。

263对的第一组引物;
每对第一组引物包括正向引物序列与反向引物序列,且所述正向引物序列与反向引物序列均包括可特异性扩增BRCA1/BRCA2基因外显子、内含子或 UTR区域的序列;
所述可特异性扩增BRCA1/BRCA2基因外显子、内含子或UTR区域序列包括SEQ IDNO.1~526所示的序列,优选的,为其中20-25个碱基。

本发明所述的引物组合物共计566条,又分为两组:第一组引物为263 对,其中的正向引物3’端DNA序列分别如SEQ ID 1~263所示,5’端DNA 序列如SEQ ID NO.567所示,反向引物的3’端DNA序列分别如SEQ ID 264~526 所示,5’端DNA序列如SEQ ID NO.568所示;第二组引物为20对,其中的正向引物3’端DNA序列分别如SEQ ID 527~546所示,5’端DNA序列如SEQ ID NO.567所示,反向引物的3’端DNA序列分别如SEQ ID 547~566所示,5’端DNA序列如SEQ ID NO.568所示。
该引物组合物可通过多重PCR的方法,特异性地扩增BRCA1和BRCA2 基因所有外显子区域和至少10bp的内含子毗邻区,以及人类基因组中多个特定的参考位点,可测的突变类型包括点突变,短***/缺失和拷贝数变异。该引物组合物的整体设计使得所有对目的片段的扩增可在同一管中进行,避免了分管的麻烦,节省了相关物料和时间的成本。扩增产物的平均长度在150bp左右,比通常的设计要短,因此该引物组合物既可用于检测来自全血或其他组织的基因组DNA样本,又可用于检测来自肿瘤组织,尤其是FFPE 的DNA样本。
实施例1
本发明多重PCR建库测序流程采取的验证方法如下:
1.样本准备
1.1样本提取
使用Qiagen公司的QIAamp DNA Mini Kit(货号:51304)提取全血临床样本的基因组DNA;使用Qiagen公司的GeneRead DNA FFPE Kit(货号:180134) 提取FFPE临床样本的DNA。
1.2样本质控
使用Thermo Fisher公司的Qubit dsDNA HS Assay Kit(货号:Q32851/ Q32854)对提取的DNA进行定量。各临床样本的SNV/Indel突变交由第三方 NGS检测验证,CNV突变由MLPA验证。
2.文库构建
2.1第一轮基因特异性PCR
2.1.1按下表所示配制基因特异性PCR反应液,每管加入相应的DNA样本。

2.1.2盖上管盖,震荡混匀后,快速离心。
2.1.3按以下程序运行PCR反应。

2.2引物酶解
2.2.1 PCR板/管短暂离心,移除封口或揭开管盖。
2.2.2将核酸外切酶颠倒混匀,离心机上短暂离心。
2.2.3核酸外切酶用无核酸酶水3:2稀释。
2.2.4每管加入5μL外切酶稀释液,并用移液器吹打混匀。
2.2.5将PCR板/管封口,振荡5-10秒,然后短暂离心。
2.2.6按以下程序运行PCR反应。

2.3基因特异性PCR产物纯化

2.3.3将各管转移至磁力架上,室温放置5分钟或至液体澄清。小心吸去上清,注意避免碰到磁珠。
2.3.4每管加入150μL 70%乙醇清洗,孵育30秒后吸去乙醇,注意避免碰到磁珠。重复清洗一次。
2.3.5盖上管盖,将各管快速离心后放置于磁力架上,小心地将管中剩余的乙醇吸净。
2.3.6室温放置2-3分钟至磁珠干燥。各管加入32μL无核酶水,盖上管盖,震荡混匀并快速离心。
2.3.7将各管转移至磁力架上,室温放置5分钟。转移上清至新管中。

2.5文库纯化
2.5.1各管从PCR仪中取出后快速离心将液体甩至管底。
2.5.2每管加入1倍体积的纯化磁珠,震荡混匀并快速离心。室温静置5 分钟。
2.5.3将各管转移至磁力架上,室温放置5分钟或至液体澄清。小心吸去上清,注意避免碰到磁珠。
2.5.4每管加入150μL 70%乙醇清洗,孵育30秒后吸去乙醇,注意避免碰到磁珠。重复清洗一次。
2.5.5盖上管盖,将各管快速离心后放置于磁力架上,小心地将管中剩余的乙醇吸净。
2.5.6室温放置2-3分钟至磁珠干燥。各管加入32μL无核酶水,盖上管盖,震荡混匀并快速离心。
2.5.7将各管转移至磁力架上,室温放置5分钟。转移上清至新管中。
2.6文库质控
使用Qubit对文库进行定量;使用Qseq100对文库片段大小和纯度进行分析。
3.上机测序
3.1将各文库根据浓度统一稀释至5nM,各取少量等体积混合。
3.2加入等体积0.2N NaOH变性,再用HT1溶液稀释至15pM。
3.3取594μL 15pM混合文库,加入6μL 12.5pM PhiX library control。
3.4将上述600μL加样至测序试剂盒,装载入Illumina Miseq测序仪测序。测序读长PE150,测序深度不少于2000x。

Claims (8)

1.一种基于NGS方法检测人类BRCA1和BRCA2基因突变的引物组合物,其特征在于,包括:
263对的第一组引物;
每对第一组引物包括正向引物序列与反向引物序列,且所述正向引物序列与反向引物序列均包括可特异性扩增BRCA1/BRCA2基因外显子、内含子或UTR区域的序列;
所述可特异性扩增BRCA1/BRCA2基因外显子、内含子或UTR区域序列包括SEQ ID NO.1~526所示的序列,优选的,为其中20-25个碱基。
2.如权利要求1所述的一种基于NGS方法检测人类BRCA1和BRCA2基因突变的引物组合物,其特征在于,还包括20对的第二组引物,且所述第二组引物中每对引物均包括正向引物与反向引物,所述正向引物与反向引物均包括可特异性扩增人类基因组中特定的参考区域的序列,所述可特异性扩增人类基因组特定参考区域的序列包括SEQ ID NO.527~566所示的序列,优选的,为其中20-25个碱基。
3.如权利要求1-2所述的一种基于NGS方法检测人类BRCA1和BRCA2基因突变的引物组合物,其特征在于,所述第一组引物、第二组引物正向引物均包括SEQ ID NO.567所示的序列,优选的,为其中16-25个碱基。
4.如权利要求1-2所述的一种基于NGS方法检测人类BRCA1和BRCA2基因突变的引物组合物,其特征在于,所述第一组引物、第二组引物的反向引物均包括SEQ ID NO.568所示的序列,优选的,为其中16-25个碱基。
5.一种用于BRCA1和BRCA2基因检测的试剂,其特征在于,包括权利要求1-4所述第一组引物、第二组引物。
6.如权利要求5所述的一种用于BRCA1和BRCA2基因检测的试剂,其特征在于,还包括三对接头,所述三对接头分别对应Illumina、Ion Torrent、MGI三种测序平台,且所述三对接头包括与权利要求3、4中所述的序列配对的DNA片段。
7.如权利要求5-6所述的一种用于BRCA1和BRCA2基因检测的试剂,其特征在于,所述试剂用于来自新鲜或冰冻组织、全血、唾液或FFPE的DNA样本的分子检测。
8.如权利要求5-6所述的一种用于BRCA1和BRCA2基因检测的试剂,其特征在于,所述试剂用于基于NGS方法的BRCA1和BRCA2基因的检测。
