背景技术
随着枕头表面细菌数量增加,会增加患者发生医院感染的可能性,枕头表面细菌数量每增加1CFU/cm2时,患者发生感染的可能性将增加0.81倍。其中在多重感染患者病房环境中,空气、枕头和棉被上均检测到鲍氏不动杆菌、大肠埃希菌和表皮葡萄球菌,微生物的分布具有明显的同型和时间共存性,与院内感染菌群具有相对一致性,是患者发生感染的重要感染源。
目前临床一般采用无菌巾或尿垫隔离保护伤口,污染后更换枕套,但枕芯消毒往往不彻底,成为颅后窝手术伤口感染的危险因素之一,从而导致无法保护伤口,不能有效地预防头部压力性损仿的发生,院内感染率高,舒适度差。

S2、对合成棉纤维布料进行化学预处理,使合成棉纤维布料与NaOH溶液进行碱水解反应,使得纤维大分子链上大量存在的氰基转化成羧酸钠反应基团,得到羧酸钠型纤维;
S3、将合成棉纤维布料浸入到季鏻盐溶液中,使得羧酸钠型纤维与季鏻盐进行离子交换,将含不同烷基链长度的三苯基溴化磷季鏻阳离子与Na-PANF上的Na+进行离子交换,使得季鏻阳离子与纤维结构进行有机结合,得到季鏻阳离子纤维布料;
S4、用AgNO3溶液对季鏻阳离子纤维布料进行洗涤,使得部分Na+与硝酸银进行离子交换,再用蒸馏水清洗、干燥后,制得改性抗菌棉纤维布料,即外面料层。
S5、将聚四氟乙烯微孔薄膜经过双向拉伸覆盖在改性抗菌棉纤维布料的下表面,形成防水透气层,再在防水透气层的下表面覆盖一层无纺布,得到里面料层,从而形成外枕套本体。

具体实施方式
请参阅图1~4,本发明实施例中,一种防压抗感染伤口多功能保护枕,包括外枕套1以及位于外枕套1内侧的枕芯2,外枕套1包括外枕套本体11,外枕套本体11的顶端的中部设置有头部放置槽15,外枕套本体11的顶端靠近头部放置槽15的两侧均设置有侧面放置槽14,外枕套本体11的顶端位于头部放置槽15的前侧设置有颈部定位槽10,外枕套本体11的顶端的后侧开设有贯穿头部放置槽15内侧的引流管开口槽17,外枕套本体11的顶端靠近引流管开口槽17的一侧设置有固定带19,外枕套本体11的顶端靠近引流管开口槽17的另一侧设置有魔术母贴16,外枕套本体11的一侧设置有条形更换口13,固定带19的一侧设置有魔术子贴18,引流管开口槽17的内侧设置有凸起块110,条形更换口13的内侧设置有拉链12,通过拉链12和条形更换口13可以快速更换外枕套1,可以把患者颅后窝手术后留置在颅内的引流管放置在引流管开口槽17内,再把魔术子贴18贴在魔术母贴16上,使得引流管位于引流管开口槽17与固定带19之间,可以有效地防止引流管受到头部的压力的风险,通过头部放置槽15和颈部定位槽10可以更换患者的舒适度,且利于固定头部位置,机保护头部伤口的作用。
优先的,外枕套本体11包括外面料层111,外面料层111的下侧设置有防水透气层112,防水透气层112的内侧设置有里面料层113。
优先的,外面料层111的材质为改性抗菌棉纤维,防水透气层112的材质为聚四氟乙烯微孔薄膜,里面料层113的材质为无纺布,聚四氟乙烯微孔薄膜的微孔直径为2±0.1μm,水蒸气的分子直径为0.0004μm,毛细水的分子直径为400μm,薄膜的孔径对水蒸气直径大5000倍,而毛细水的直径比薄膜的微孔直径大200倍,毛细水不能透过薄膜微孔,水蒸气可自由通过。
优先的,枕芯2包括枕芯内套21,枕芯内套21的内侧设置有乳胶层23,乳胶层23的内侧设置有艾草芯24,乳胶层23的外表面的上侧均匀开设有透气孔22。
优先的,枕芯内套21的材质为棉纱布,枕芯内套21嵌入在外枕套本体11的内侧。
优先的,外枕套本体11制作工艺包括以下步骤:
S1、将80%的纯棉纤维与20%的聚丙烯腈纤维进行混纺,制成合成棉纤维布料,再依次对合成棉纤维布料进行皂洗和水洗,使其保持洁净;
S2、对合成棉纤维布料进行化学预处理,使合成棉纤维布料与NaOH溶液进行碱水解反应,使得纤维大分子链上大量存在的氰基-CN转化成羧酸钠-COONa反应基团,得到羧酸钠型纤维Na-PANF;
S3、将合成棉纤维布料浸入到季鏻盐溶液中,使得羧酸钠型纤维Na-PANF与季鏻盐进行离子交换,将含不同烷基链长度的三苯基溴化磷季鏻阳离子与Na-PANF上的Na+进行离子交换,使得季鏻阳离子与纤维结构进行有机结合,得到季鏻阳离子纤维布料;
S4、用AgNO3溶液对季鏻阳离子纤维布料进行洗涤,使得部分Na+与硝酸银进行离子交换,再用蒸馏水清洗、干燥后,制得改性抗菌棉纤维布料,即外面料层111。
S5、将聚四氟乙烯微孔薄膜经过双向拉伸覆盖在改性抗菌棉纤维布料的下表面,形成防水透气层112,再在防水透气层112的下表面覆盖一层无纺布,得到里面料层113,从而形成外枕套本体11。
为了更好地说明本发明的技术效果,通过下述对比例和实施例试验进行阐述:
在一段时间内,随机抽取某一医院神经外科行颅后窝手术的患者200例,分成对比例组和实施例组,每组100例,其中,对比例组中男性67例,女性33例,平均年龄为(51.73+13.62)岁,平均病程为(2.32+1.07)月,病变为:听神经鞘瘤57例,桥小脑角脑膜瘤23例,三叉神经鞘瘤9例,舌咽神经鞘瘤6例,第四脑室脑膜瘤5例,并按照医嘱进行用药;实施例组中男性61例,女性39例,平均年龄为(54.32+11.35)岁,平均病程为(2.18+1.32)月,病变为:听神经鞘瘤52例,桥小脑角脑膜瘤18例,三叉神经鞘瘤14例,舌咽神经鞘瘤8例,第四脑室脑膜瘤8例,并按照医嘱进行用药。
对比例组患者使用医院病床普通的棉枕头,实施例组患者使用本发明的一种防压抗感染伤口多功能保护枕。
并采用常规护理方法对患者进行护理,根据统计学分析,两组患者在年龄、病程、用药量和用药时间等方面差异,无显著差异,具有可比性。
一、分别分析评定对比例和实施例中枕头的舒适度、透气性、以及使用前、使用后2d和使用后7d枕头上的细菌(金黄色葡萄球菌数和克雷伯氏肺炎杆菌,单位cfu/g)含量。
舒适度评定:对枕头舒适度进行手感评定,用1~5级表示,1级为最差,5级为最好。
透气量测定:在恒温恒湿(温度20±2℃,相对湿度(65±3)%)条件下,选择实验面积为20cm2,压强100Pa,单位mm/s,检测气体通过外枕套本体的含量。
金黄色葡萄球菌数和克雷伯氏肺炎杆菌的测定:分别检测患者在使用前、使用后2d和使用后7d枕头上的金黄色葡萄球菌数和克雷伯氏肺炎杆菌的含量。
对比例和实施例各项指标进行检测,并汇集到表1中。
表1:对比例和实施例各项指标的比较:
从表1分析可以得出:实施例的舒适度评分为5分,舒适度较高,软硬适宜,而对比例的舒适度评分为3分,舒适度一般,过软,有一定的霉味;
实施例中外枕套本体的透气量为105mm/s,对比例中外枕套本体的透气量为98mm/s,实施例和对比例的透气性均较好,从而便于患者头颈部热量和汗水的排出;
实施例中外枕套本体表面的金黄色葡萄球菌数在使用前、使用后2d和使用后7d分别为2.3×104、2.4×104、2.7×104cfu/g,分别助长了1×103、3×103cfu/g,增长数量较小,克雷伯氏肺炎杆菌在使用前、使用后2d和使用后7d分别为1.8×104、2.0×104、2.3×104cfu/g,分别助长了2×103、3×103cfu/g,增长数量也较小;对比例中外枕套本体表面的金黄色葡萄球菌数在使用前、使用后2d和使用后7d分别为5.2×104、6.3×105、3.2×107cfu/g,分别助长了5.78×105、3.14×107cfu/g,金黄色葡萄球菌数成指数级大幅度增长,克雷伯氏肺炎杆菌在使用前、使用后2d和使用后7d分别为4.0×104、5.1×105、5.9×107cfu/g,分别助长了4.7×105、5.85×107cfu/g,克雷伯氏肺炎杆菌成指数级大幅度增长;说明实施例中外枕套本体对金黄色葡萄球菌数和克雷伯氏肺炎杆菌均有抑制作用。
二、在相同的护理条件下7d内,分别记录实施例组和对比例中患者的交叉感染、伤口愈合和压力性损伤的病例数,并把交叉感染情况中的并发症人数、并发症发生率、伤口复发人数和伤口复发率,伤口愈合情况中的伤口愈合人数、伤口愈合率和伤口愈合时间,压力性损伤情况中压力性损伤人数和压力性损伤率记录在下表2中。
表2:对比例和实施例中交叉感染、伤口愈合和压力性损伤指标的比较:
从表2分析可以得出:实施例中共计有4例患者因交叉感染出现了并发症,并发症发生率为4.0%,而对比例中共计有18例患者因交叉感染出现了并发症,并发症发生率高达18.0%,对比例中患者出现并发症的概率远高于实施例中患者出现并发症的概率,实施例中共计有2例患者因交叉感染出现了伤口复发,伤口复发率为2.0%,而对比例中共计有15例患者因交叉感染出现了伤口复发,伤口复发率高达15.0%,从而可以得出:对比例中患者出现伤口复发的概率远高于实施例中患者出现伤口复发的概率;
在护理7d内,实施例中共计有93例患者伤口愈合了,伤口愈合率为93.0%,且伤口平均愈合时间为3.1d,而对比例中共计有72例患者伤口愈合了,伤口愈合率仅为72.0%,且伤口平均愈合时间高达5.7d,从而可以得出:对比例中患者的伤口愈合率显著低于实施例中患者的伤口愈合率,而比例中患者的伤口平均愈合时间却显著高于实施例中患者的伤口平均愈合时间。
实施例中共计有2例患者出现了压力性损伤,压力性损伤率为2.0%,而对比例中共计有11例患者出现了压力性损伤,压力性损伤率高达11.0%,从而可以得出:比例中患者的出现压力性损伤的概率显著高于实施例中患者出现压力性损伤的概率。
以上所述的,仅为本发明较佳的具体实施方式,但本发明的保护范围并不局限于此,任何熟悉本技术领域的技术人员在本发明揭露的技术范围内,根据本发明的技术方案及其发明构思加以等同替换或改变,都应涵盖在本发明的保护范围之内。
Claims (6)
1.一种防压抗感染伤口多功能保护枕,包括外枕套(1)以及位于外枕套(1)内侧的枕芯(2),其特征在于,所述外枕套(1)包括外枕套本体(11),所述外枕套本体(11)的顶端的中部设置有头部放置槽(15),且外枕套本体(11)的顶端靠近头部放置槽(15)的两侧均设置有侧面放置槽(14),所述外枕套本体(11)的顶端位于头部放置槽(15)的前侧设置有颈部定位槽(10),且外枕套本体(11)的顶端的后侧开设有贯穿头部放置槽(15)内侧的引流管开口槽(17),所述外枕套本体(11)的顶端靠近引流管开口槽(17)的一侧设置有固定带(19),且外枕套本体(11)的顶端靠近引流管开口槽(17)的另一侧设置有魔术母贴(16),所述外枕套本体(11)的一侧设置有条形更换口(13),所述固定带(19)的一侧设置有魔术子贴(18),所述引流管开口槽(17)的内侧设置有凸起块(110),所述条形更换口(13)的内侧设置有拉链(12)。
2.根据权利要求1所述的一种防压抗感染伤口多功能保护枕,其特征在于,所述外枕套本体(11)包括外面料层(111),所述外面料层(111)的下侧设置有防水透气层(112),所述防水透气层(112)的内侧设置有里面料层(113)。
3.根据权利要求2所述的一种防压抗感染伤口多功能保护枕,其特征在于,所述外面料层(111)的材质为改性抗菌棉纤维,所述防水透气层(112)的材质为聚四氟乙烯微孔薄膜,所述里面料层(113)的材质为无纺布。
4.根据权利要求1所述的一种防压抗感染伤口多功能保护枕,其特征在于,所述枕芯(2)包括枕芯内套(21),所述枕芯内套(21)的内侧设置有乳胶层(23),所述乳胶层(23)的内侧设置有艾草芯(24),且乳胶层(23)的外表面的上侧均匀开设有透气孔(22)。
5.根据权利要求4所述的一种防压抗感染伤口多功能保护枕,其特征在于,所述枕芯内套(21)的材质为棉纱布,且枕芯内套(21)嵌入在外枕套本体(11)的内侧。
6.根据权利要求1~5任一所述的一种防压抗感染伤口多功能保护枕,其特征在于,外枕套本体(11)制作工艺包括以下步骤:
S1、将80%的纯棉纤维与20%的聚丙烯腈纤维进行混纺,制成合成棉纤维布料,再依次对合成棉纤维布料进行皂洗和水洗,使其保持洁净;
S2、对合成棉纤维布料进行化学预处理,使合成棉纤维布料与NaOH溶液进行碱水解反应,使得纤维大分子链上大量存在的氰基(-CN)转化成羧酸钠(-COONa)反应基团,得到羧酸钠型纤维(Na-PANF);
S3、将合成棉纤维布料浸入到季鏻盐溶液中,使得羧酸钠型纤维(Na-PANF)与季鏻盐进行离子交换,将含不同烷基链长度的三苯基溴化磷季鏻阳离子与Na-PANF上的Na+进行离子交换,使得季鏻阳离子与纤维结构进行有机结合,得到季鏻阳离子纤维布料;
S4、用AgNO3溶液对季鏻阳离子纤维布料进行洗涤,使得部分Na+与硝酸银进行离子交换,再用蒸馏水清洗、干燥后,制得改性抗菌棉纤维布料,即外面料层(111)。
S5、将聚四氟乙烯微孔薄膜经过双向拉伸覆盖在改性抗菌棉纤维布料的下表面,形成防水透气层(112),再在防水透气层(112)的下表面覆盖一层无纺布,得到里面料层(113),从而形成外枕套本体(11)。
