技术领域
本发明涉及医学领域中的人工髋关节及其快速组装和拆卸方法，尤其涉及一种简易、便捷组合式个性化设计的髋关节假体。
背景技术
在医学创伤骨科及外科临床治疗中，治疗各种髋关节疾病晚期如股骨头坏死、骨关节炎、类风湿性关节炎、强直性脊柱炎、先天性髋关节发育不良、髋关节外伤及外伤后关节炎，以及各种原因造成的髋关节破坏等，普遍采用髋关节置换手术的方法。
髋关节置换手术的主要原理是：通过人工制造的髋关节植入体内，以重建人体髋关节的解剖结构，并使其获得正常的力学性能完成一定的生理功能，由此达到缓解疼痛、改善功能的目的。人工髋关节是由股骨柄、髋臼杯、和股骨头等部件构成，其结构设计和材料性能直接关系到髋关节置换手术的治疗效果。传统的人工金属髋关节一般采用钛合金、钴铬钼合金等材料制造，其结构一般分为整体式及组合式。目前，髋关节假体经过不断发展，关节材料已从髋臼及假体柄的形状、球头的直径大小、股骨柄的锥度等宏观设计逐步进展到世纪末的金属表面处理技术、材料力学、金属工艺学等微观方面。在材料种类方面，钴基合金和钛合金以其良好的耐磨性、耐腐蚀性和优良的力学特性已成为人工关节最普遍采用的材料之一，高分子材料或陶瓷材料在该领域也有所应用。
髋关节假体良好的匹配性直接影响到髋关节置换的手术效果，但由于人的个性化差异，现有的髋关节假体并不能完全符合各类型人群的需求，临床上没有一种假体能够放之四海而皆准，每种假体都应该针对髋关节各自不同的临床要求而设计。除此之外，股骨杯以关节方式连接于髋臼球，在常规髋关节假体中，髋臼杯以关节方式连接于股骨球，在翻修手术中，外科医生需要从翻修手术的患者的股骨植入物分离股骨杯或股骨球，杯或球从植入物的分离有时需要释放通过莫氏锥度实现的与植入物的结合，通常需要采用特殊的手术工具，手术难度大，容易造成二次伤害。因此，急需一种既符合个性化需求又能够快速组装和拆卸的组合式髋关节假体，从而在不使股骨植入物松动的情况下从股骨植入物移除股骨杯或股骨球。
发明内容
本发明提供一种组合式髋关节假体，包括股骨柄、髋臼杯，其中，股骨柄包括股骨柄上端、股骨柄中端和股骨柄下端，股骨柄上端的端部具有与髋臼杯相连的连接部，所述连接部为向内凹陷的下沉孔，下沉孔的一侧侧壁上设置有可容纳旋拧螺钉等锁定结构恰好穿过的通孔，通孔与下沉孔彼此垂直设置，所述通孔内至少局部设置有内螺纹；股骨柄中端为表面粗糙区域，粗糙区域的上部设置有云朵状个性化信息标记处，用来标记髋关节假体的型号、适合人群、生产日期、注意事项等信息，粗糙区域的下部为横条纹状凹凸不平，有助于植入后周围细胞的攀附和长入；股骨柄下端为光滑区域，光滑区域的表面设置2-4处长条形边界均匀过渡的沟槽。
所述沟槽延股骨柄轴向延伸，在植入时光滑区域可减小植入阻力，并利于植入后进一步的细胞长入以实现良好的稳定性，降低了股骨柄远端的刚度和强度。
髋臼杯包括球形杯体，杯体内部设置有髋臼杯固定柱和可使旋拧螺钉等锁定结构穿过的开孔，所述固定柱朝向开孔一侧设置有棘齿，另外还设置有与开孔配合使用的丁字形软垫，以对所述开孔进行封堵，所述封堵为密封式封堵，即软垫与开孔周围过盈密封配合，封堵后软垫外端面与髋臼杯外表面平整圆滑过渡，所述开孔与股骨柄上的所述通孔可实现互相对齐以便于将旋拧螺钉等锁定结构顺利放入。
所述旋拧螺钉的一端设置有含内六方槽的螺帽，另一端设置有与髋臼头固定柱上的棘齿相配合的斜齿，所述斜齿与所述固定柱上的棘齿配合锁定，从而对髋臼杯与股骨柄进行有效固定。另外，还提供与螺帽上的内六方孔相配合使用的竖直的六棱柱结构，通过垂直旋拧六棱柱结构对旋拧螺钉进行拧紧或拧松。
所述旋拧螺钉可与压簧配合使用，以使在取出时更加方便的弹出。
所述股骨柄、髋臼杯由钛合金、钴铬钼合金等材料制成，也可为具有微孔结构的钽制件，与所述开孔配合使用的所述软垫为橡胶软垫或弹性聚合物软垫。

当对髋关节假体进行植入时，对股骨柄与髋臼杯进行快速组装，通过定位显影标记使髋臼杯上的开孔与股骨柄上的通孔相对齐，调整合适位置后，使用六棱柱结构将旋拧螺钉通过髋臼杯上的开孔正向拧入股骨柄上的通孔，直到所述旋拧螺钉底部的斜齿与所述固定柱上的棘齿抵接锁定，随后，将橡胶软垫或弹性聚合物软垫盖在髋臼杯的开孔上，对其进行封堵，封堵后软垫外端面与髋臼杯外表面平整圆滑过渡。
当需要对髋关节假体进行翻修时，使用镊子等手术器械将橡胶软垫或弹性聚合物软垫取出，将六棱柱结构垂直***髋臼杯上的开孔，对旋拧螺钉进行反向拧出股骨柄上的通孔，使所述旋拧螺钉底部的斜齿与所述固定柱上的棘齿相互分离，此时可顺利将股骨柄和髋臼杯进行拆分，避免采用特殊的手术工具将其强行拆分。

本发明股骨柄中端和下端设置有不同的利于细胞长入的结构，形成内长入结构，与人体骨组织有机结合为一个整体，达到真正的生物固定，增强了股骨柄植入后良好的稳定性，还可以减轻由于股骨柄柄体远端刚度而造成的术后大腿疼痛；本发明通过旋拧螺钉等锁定结构将股骨柄与髋臼杯进行固定连接，该连接方式操作简单，易于组装和拆分；其云朵状个性化信息标记满足了不同患者个性化定制需求，方便医护人员准确按照预定方案进行植入。大大提高了该髋关节假体的植入舒适度和使用寿命，尤其适用于畸形严重及后期可能需要翻修的髋关节置换，达到了设计目的。

如图1-3所示，一种组合式髋关节假体1，包括股骨柄2、髋臼杯3，其中，股骨柄包括股骨柄上端4、股骨柄中端5和股骨柄下端6，股骨柄上端4的上端部具有与髋臼杯3相连的连接部7，所述连接部7为向内凹陷的下沉孔，下沉孔的一侧侧壁上设置有可容纳旋拧螺钉9等锁定结构恰好穿过的通孔8，通孔8与下沉孔彼此垂直设置，所述通孔8内至少局部设置有内螺纹；股骨柄中端5为表面粗糙区域，粗糙区域的上部设置有云朵状个性化信息标记处10，用来标记髋关节假体的型号、适合人群、生产日期、注意事项等信息，粗糙区域的下部为横条纹状凹凸不平，有助于植入后周围细胞的攀附和长入；股骨柄下端6为光滑区域，光滑区域的表面设置2-4处长条形边界均匀过渡的沟槽11，所述沟槽11延股骨柄轴向延伸，在植入时光滑区域可减小植入阻力，并利于植入后进一步的细胞长入以实现良好的稳定性，降低了股骨柄远端的刚度和强度。
所述髋臼杯2包括球形杯体12，杯体内部设置有髋臼杯固定柱13和可使旋拧螺钉9等锁定结构穿过的开孔14，所述固定柱13朝向开孔14一侧设置有棘齿，另外还设置有与开孔14配合使用的丁字形软垫，以对所述开孔进行封堵，所述封堵为密封式封堵，即软垫与开孔14周围过盈密封配合，封堵后软垫外端面与髋臼杯外表面平整圆滑过渡，所述开孔14与股骨柄2上的所述通孔8可实现互相对齐以便于将旋拧螺钉等锁定结构顺利放入。
所述旋拧螺钉9的一端设置有含内六方槽的螺帽，另一端设置有与髋臼头固定柱13上的棘齿相配合的斜齿，所述斜齿与所述固定柱13上的棘齿配合锁定，从而对髋臼杯3与股骨柄2进行有效固定。另外，还提供与螺帽上的内六方孔相配合使用的竖直的六棱柱结构，通过垂直旋拧六棱柱结构对旋拧螺钉9进行拧紧或拧松。
所述旋拧螺钉9可与压簧配合使用，以使在取出时更加方便的弹出。
所述股骨柄1、髋臼杯2由钛合金、钴铬钼合金等材料制成，也可为具有微孔结构的钽制件，与所述开孔配合使用的所述软垫为橡胶软垫或弹性聚合物软垫。
所述锁定结构9也可以为弹性压配卡合结构或者鲁班锁锁合结构。
所述云朵状个性化信息标记处10为在股骨柄2本体上雕刻、模压或镶嵌形成的云朵状醒目图案，其可以与股骨柄本体具有相同颜色，也可以涂覆不同颜色以示区别，其所标示信息可以有效避免假体在运输、装配或使用过程中出现失误，医护人员在植入前需以此信息为准进行个性化植入和记录存档。
髋臼杯3上的所述开孔14与股骨柄2上的所述通孔8处均设置有定位显影标记。
所述固定柱13上的棘齿长度为5-15mm。
当对髋关节假体1进行植入时，对股骨柄2与髋臼杯3进行快速组装，通过定位显影标记使髋臼杯3上的开孔14与股骨柄2上的通孔8相对齐，调整合适位置后，使用六棱柱结构将旋拧螺钉9通过髋臼杯3上的开孔14正向拧入股骨柄2上的通孔8，直到所述旋拧螺钉9底部的斜齿与所述固定柱13上的棘齿抵接锁定，随后，将橡胶软垫或弹性聚合物软垫盖在髋臼杯3的开孔14上，对其进行封堵，封堵后软垫外端面与髋臼杯3外表面平整圆滑过渡。
当需要对髋关节假体1进行翻修时，使用镊子等手术器械将橡胶软垫或弹性聚合物软垫取出，将六棱柱结构垂直***髋臼杯3上的开孔14，对旋拧螺钉9进行反向拧出股骨柄2上的通孔8，使所述旋拧螺钉9底部的斜齿与所述固定柱13上的棘齿相互分离，此时可顺利将股骨柄2和髋臼杯3进行拆分，避免采用特殊的手术工具将其强行拆分。
本发明具有结构简单、植入稳定、力学性能类似人骨、满足人体骨骼个性化匹配需求，兼具方便快速组装和拆卸等特点，其能够在不使股骨植入物松动的情况下快速的从股骨植入物移除股骨杯或股骨球。
以上所述仅是本发明的优选实施方式，应当指出，对于本技术领域的普通技术人员来说，在不脱离本发明原理的前提下，还可以做出若干改进和润饰，这些改进和润饰也应视为本发明的保护范围。

Claims (7)

1.一种用于植入的组合式髋关节假体，包括股骨柄、髋臼杯，其中，股骨柄包括股骨柄上端、股骨柄中端和股骨柄下端，股骨柄上端的端部具有与髋臼杯相连的连接部，其特征在于，所述连接部为向内凹陷的下沉孔，下沉孔的一侧侧壁上设置有可容纳旋拧螺钉等锁定结构恰好穿过的通孔，通孔与下沉孔彼此垂直设置，所述通孔内至少局部设置有内螺纹；股骨柄中端为表面粗糙区域，粗糙区域的上部设置有云朵状个性化信息标记处，用来标记髋关节假体的型号、适合人群、生产日期、注意事项等信息，粗糙区域的下部为横条纹状凹凸不平；股骨柄下端为光滑区域，光滑区域的表面设置2-4处长条形边界均匀过渡的沟槽。
2.根据权利要求1所述的用于植入的组合式髋关节假体，所述封堵为密封式封堵，即软垫与开孔周围过盈密封配合，封堵后软垫外端面与髋臼杯外表面平整圆滑过渡。
3.根据权利要求1和2所述的用于植入的组合式髋关节假体，所述旋拧螺钉的一端设置有含内六方槽的螺帽，另一端设置有与髋臼头固定柱上的棘齿相配合的斜齿，所述斜齿与所述固定柱上的棘齿配合锁定，还包括与螺帽上的内六方孔相配合使用的竖直的六棱柱结构，通过垂直旋拧六棱柱结构对旋拧螺钉进行拧紧或拧松。
4.根据权利要求1所述的用于植入的组合式髋关节假体，所述旋拧螺钉可与压簧配合使用，以使在取出时更加方便的弹出。
5.根据权利要求1所述的用于植入的组合式髋关节假体，所述股骨柄、髋臼杯由钛合金、钴铬钼合金等材料制成，也可为具有微孔结构的钽制件，与所述开孔配合使用的所述软垫为橡胶软垫或弹性聚合物软垫。
6.根据权利要求1所述的用于植入的组合式髋关节假体，所述锁定结构也可以为弹性压配卡合结构或者鲁班锁锁合结构。
7.根据权利要求1所述的用于植入的组合式髋关节假体，所述云朵状个性化信息标记处为在股骨柄本体上雕刻、模压或镶嵌形成的云朵状醒目图案，其可以与股骨柄本体具有相同颜色，也可以涂覆不同颜色以示区别。
