Description
本发明涉及包含酰胺局部***的凝胶形式的药物组合物。
现有技术
手术后疼痛是对由手术引起的组织创伤的复杂反应,其引起中枢神经***超敏反应。手术后疼痛可能会在任何手术后出现,并增加手术后并发症的风险,也干扰患者的康复和恢复正常活动。当患者不能独立移动、进食或睡眠而不会感到疼痛时,愈合过程大幅度减慢,因为在远离手术部位的区域也会感到疼痛。疼痛还在患者中诱发一种心理上的虚脱状态,这进一步加重了总体临床现象。如今,谨慎的手术后疼痛的处置是必不可少的,且得到世界卫生组织大力推动,因为将其视为患者的一项基本权利。
疼痛感不仅取决于手术类型(腹部手术和胸部手术比牵涉四肢的手术更疼痛,并且牵涉腹膜或胸膜时腹部或胸部切口引起甚至更剧烈的疼痛),还取决于主观情况,例如在先的疼痛经历、焦虑和遗传易感性(genetic predisposition);由于这个原因,医护人员试图通过使镇痛措施适应每个患者所报告的强度来实施镇痛措施,所述强度可以使用疼痛评价量表进行正确测定。在各种可利用的量表中,使用最广泛的是一维量表,它易于管理,并且其中唯一考虑的参数是患者感知到的疼痛程度的表达。最著名的是:NRS量表(数字等级量表(Numerical Rating Scale)–Downie等人Ann Rheum Dis,1978,378-381,其中操作员要求患者以量表上的0到10(0=无疼痛;10=最大疼痛)口头表达他们感觉到的疼痛的量;和VAS量表(视觉类比量表(Visual Analogical Scale)-Scott,Huskisson,Pain,1976,2,175-184),其中感觉到的疼痛由患者在类似于尺子的长度为10cm的线上给出。后一种量表被广泛使用,因为它易于被患者理解,包括儿科年龄的患者。其它更复杂的量表用于无法自我表达的患者或患有妨碍正确言语和/或运动表达的伴随障碍患者。
手术后疼痛处置依赖于药物治疗。为了减轻手术期间和手术后即刻发生的疼痛,通常静脉内施用药物;所讨论的药物一般是阿片类药物,由于其严重的副作用,其使用仅限于相当短的时间。
当患者能够摄入药物并且总体临床情况也允许时,然后口服施用药物,例如对乙酰氨基酚、传统的NSAID或COX-2,其副作用是众所周知的。治疗手术后疼痛的另一种方法涉及使用局部***的神经阻滞。根据手术类型而定,当***沿着神经路径局部施用时,神经阻滞镇痛(analgesia)可以是周围性的,或者当***靠近神经的脊神经后根(posterior spinal root)施用时,神经阻滞镇痛可以是中枢性的。周围性镇痛减轻由麻醉的特定神经例如手臂或腿支配的身体的精确区域中的疼痛,且通常通过靠近待阻滞的神经的皮内注射获得;中枢性镇痛以更宽泛的方式起作用,并且通过导管或泵注射到硬膜外腔进行施用,如果需要,可以在手术后将所述导管或泵原位留置几天。但是,这种治疗始终是在手术后开始的。
在这些情况中最常用的局部***是利多卡因、布比卡因、依替卡因、罗哌卡因等,通常称作“酰胺局部***”。这些亲脂性分子的碱形式具有药理活性,能穿透神经细胞的膜,但水溶性差,因此在市场上以质子化和成盐形式(通常与盐酸成盐)可获得,这些形式活性较低。质子化在叔氮原子上发生,该氮原子获得部分正电荷,由此能与阴离子(如前所述,通常为氯离子)键合。由于活性成分迅速吸收到血液中,所以使用局部***的局部或中枢镇痛的持续时间非常短;被治疗的区域的血管化程度越大,吸收越快。这可以通过将局部***与血管收缩药组合使用来纠正,尤其是对于周围神经阻滞。然而,在那种情况下,需要以相对短的间隔重复施用。此外,鉴于它们的脂溶性,酰胺局部***倾向于在组织中存在的脂肪中蓄积,因此重复施用或更高剂量不仅容易产生依赖性形式,而且还容易引起明显的副作用,这可能是神经性的(嗜睡、沉醉的感觉、耳鸣、视觉障碍、达到惊厥点的激动、昏迷和心脏呼吸抑制)或心脏的(心率障碍、室性期外收缩(ventricular exrasystole)、室性和室上性心动过速、和传导障碍)。因此,一般而言,为了实现有效的、持久的疼痛抑制,同时最小化与局部***相关的风险,在临床实践中,它往往与口服NSAID组合使用。
为了改善镇痛作用的持续时间,已经研究了麻醉剂与从中释放麻醉剂的载体的组合。
特别地,由于它们与活性成分的活性形式的亲和性,与疏水性载体的组合是已知的;例如,已经基于掺入布比卡因的生物可降解的脂质微珠设计了一种可注射的制剂,其逐渐释放布比卡因该制剂只能通过注射施用,常在注射部位引起发红、瘙痒和红斑,但是,更重要的是,其具有短暂的作用,不超过24小时。通过使用聚乳酸、聚乙醇酸和相应的共聚物、聚原酸酯等的基质进行了其它尝试,它们能释放与NSAID组合的局部***。在这种情况下,鉴于两种活性成分的伴随施用,很容易发生显著的毒性问题和不良反应(US2017035777)。此外,存在将局部***以某种方式保留在与其化学相似的载体中的可能性,存在不希望的蓄积和随之而来的药代动力学特性改变的风险。为了解决这方面的问题,还已经进行了使用亲水性基质的其它尝试,其中以其盐酸盐形式将局部***掺入亲水性基质中,所述盐酸盐形式活性较低,但可在市场上得到,并且与基质的性质相似。例如,US2010266693将布比卡因HCl与透明质酸和纤维蛋白原的基质组合。Lu等人(Arch MedSci,2013,9,614-621)描述了将罗哌卡因HCl与亲水性聚合物一起使用。尽管亲水性基质表现出更好的生物相容性,没有困难地掺入罗哌卡因盐并且不引起蓄积作用,但是就镇痛功效和持续时间而言,得到的结果并不令人满意:在强烈的不期望的初次突释后,由于已经描述的原因,镇痛作用在相当短的时间内就会消失。
发明内容
现已发现,可以通过将酰胺局部***以亲脂性的碱形式掺入包含透明质酸衍生物的亲水性基质中获得酰胺局部***的缓慢释放(slow release);在本上下文中,“碱形式”意指非质子化形式。
因此,本发明的主题是凝胶形式的药物组合物,所述药物组合物包含由透明质酸衍生物组成的亲水性基质,所述透明质酸衍生物含有碱形式的酰胺局部***,并且任选地含有已知的药学上可接受的赋形剂。本发明的组合物特别可用于治疗源自任意种类的手术的手术后疼痛,尤其是矫形手术后的手术后疼痛,并且在应用后发挥其镇痛活性长达5天。
它们可以在手术时通过各种途径施用,例如通过注射和/或渗透到手术伤口周围的组织中、通过直接滴入开放性伤口中、或者通过涉及扩散的简单局部应用来施用。镇痛效果持续令人惊讶的长时间(长达5天),尤其是当将所述组合物施用在伤口周围的组织中、特别是施用在矫形手术伤口中时。
本发明还涉及制备所述药物组合物的方法,所述方法包括向非质子化碱形式的局部***的水性混悬液中加入透明质酸衍生物,所述非质子化碱形式的局部***是通过用适合的碱中和局部***的盐、特别是盐酸盐的水溶液获得的。
可以通过“单罐”法进行的所述方法将局部***以其活性的、亲脂性的碱形式捕获在由透明质酸衍生物组成的亲水性基质的网中。
本发明的另一个主题是一种控制释放(controlled-release)***(药物递送***),所述***包含由透明质酸衍生物组成的基质,所述由透明质酸衍生物组成的基质能以控制的方式释放碱形式的局部***,并且还含有本领域技术人员已知的药学上可接受的赋形剂。
发明详述
酰胺局部***包括利多卡因、布比卡因、依替卡因和罗哌卡因。特别优选的是罗哌卡因,这归因于其药理毒理学特性。
透明质酸(HA)是由D-葡糖醛酸和N-乙酰基-D-葡糖胺的交替残基组成的直链杂多糖,其具有50,000至13×106Da的分子量(MW),这取决于获得它的来源和使用的制备方法。本文的“平均分子量”意指用“固有粘度(intrinsic viscosity)”法计算的重均分子量(Terbojevich等人Carbohydr Res,1986,363-377)。
透明质酸普遍存在于人体中,其在人体中进行无数种活动,从对许多组织例如皮肤、肌腱、肌肉和软骨的细胞的机械支撑,到对众多不同细胞生理学和生物学过程(增殖、迁移、细胞分化和血管生成)的调节、组织水化和关节润滑。近来,已证明HA还通过调节炎性细胞因子、特别是IL-1的释放而起抗炎作用,并且还能通过模拟镇痛作用而与特定的阿片样物质受体结合。
由于HA的特殊化学结构,其能借助适合的化学反应以各种方式转化为衍生物,这些衍生物保持起始聚合物的生物学特性,但是在物理化学方面与起始聚合物有区别,并且因此适合用于多种应用。
根据本发明,所述透明质酸衍生物选自如下衍生物:
·通过内部酯化获得的、酯化度不超过20%、优选在0.05%至10%之间、甚至更优选在4%至5%之间的、并且由重均MW在160kDa至230kDa之间、优选为200kDa的HA制备的自交联衍生物,下文称作(EP341745);
·通过使用交联剂例如BDDE(1,4-丁二醇二缩水甘油醚(1,4-butanedioldiglycidyl ether))获得的、衍生化度在2.5%摩尔至25%摩尔之间、优选在5%摩尔至15%摩尔之间(相对于透明质酸的重复单元)的、并且由重均MW在500kDa至730kDa之间的HA制备的交联衍生物,下文称作“HBC”(EP2470230);
·酰胺化度在0.1%至50%之间的、并且由重均MW在500kDa至730kDa之间的HA制备的酰胺衍生物,即在HA羧基与脂族、芳基脂族、脂环族、芳族、环状和杂环系列的胺的氨基之间的酰胺,如EP1095064中所述。
优选的是脂族系列的酰胺衍生物,特别是十六烷基、十八烷基或十二烷基酰胺,更优选由重均MW在500kDa至730kDa之间的HA制备的并且具有在0.1%摩尔至10%摩尔之间、优选在1%摩尔至3%摩尔之间的平均衍生化(酰胺化)度的十六烷基酰胺,所述平均衍生化(酰胺化)度是在酰胺水解和释放的十六烷基胺与荧光团物质缀合后用HPLC测定的。在本发明的范围内,具有上述特征并且具有在1%摩尔至3%摩尔之间的平均衍生化(酰胺化)度的十六烷基酰胺称作其制备描述于EP1853279中。
上述衍生物、尤其是十六烷基酰胺特征在于甚至在低浓度下也能在水溶液中凝胶化、从而形成稳定的凝胶的能力。所得的凝胶在将碱形式的局部***捕获在其网中并逐渐释放所述局部***方面是非常有效的;释放的***被快速地转变为质子化形式(作为由手术过程和生理组织修复机制引起的炎症的结果,经过手术的部位富含酸性流体),并且随着其活性逐渐消失,逐渐被从凝胶的网中释放出的另外的碱形式的***代替。此外,凝胶的流变学特性有利于最终产品的挤出,使得将其与小口径的针头和应用合理的力一起应用成为可能。
亲水性HA衍生物、尤其是酰胺衍生物与碱亲脂形式的酰胺局部***、特别是罗哌卡因的组合,在不加入其它活性剂的情况下,产生令人惊讶地有效的控制释放***;碱形式的局部***恒定地且连续地从亲水性基质中释放出来,产生有效的镇痛作用,这种镇痛作用比用成盐形式可获得的镇痛作用持久得多。具体而言,通过如下所示的体外试验,通过与含有盐酸盐形式的等效组合物进行比较评估了罗哌卡因碱的释放。所述比较表明,本发明的组合物具有长达5天的镇痛作用。
本发明的组合物是通过以下方法制备的:将酰胺***的盐、特别是罗哌卡因盐酸盐转化成相应的碱,其因此以细沉淀物形式稳定地混悬在HA衍生物的基质中,并且恒定地且逐渐地被释放。
以扼要形式,在下列步骤中制备本发明的组合物:
·使局部***以其成盐形式以一份或多份溶解在水性载体例如盐水溶液或PBS、优选PBS中;这降低了溶液的pH,其变成酸性;所达到的pH值取决于所用的***;
·通过用碱处理步骤a)中获得的溶液直至达到在6.5至8之间的pH值沉淀出碱形式的局部***。所述碱可以是含有碱金属或碱土金属、优选碱金属的碱,甚至更优选是NaOH;
·向所得的混悬液中加入选择的粉状透明质酸衍生物,导致凝胶形成。
特别地,具体针对罗哌卡因而言,在将罗哌卡因盐酸盐溶解在水性载体中后,作为结果pH值降低至低于6,用碱处理将所述盐转化为亲脂性的水不溶性的碱形式,其因此以非常细的形式沉淀出来。罗哌卡因的溶解阶段和随后的沉淀可以在单个步骤中完成(罗哌卡因HCl的浓度至多约15-18mg/ml),或者当需要更高浓度时可分为多个步骤。在第二种情况下,在每个步骤中加入的碱、典型地为NaOH的量相对于罗哌卡因HCl而言接近化学计量(从而诱导其几乎全部沉淀),同时pH值维持在6.5至8之间。无论进行多少个步骤,所获得的沉淀物都混悬在其母液中,向其中加入粉末形式的所选择的透明质酸衍生物、优选上文所定义的酰胺形成稳定的、可灭菌的凝胶,所述凝胶在其网中掺入了所述沉淀物并逐渐释放它。制备使用单罐反应进行,其非常简单且短暂,不浪费试剂,因此从工业角度来说是经济的,并且对于本发明的所有HA衍生物和所有局部***而言是可完美再现的。
显然施用的剂量根据手术的程度和患者的总体状况来定制,显然要考虑到防止剂量过量的必要限制。通常,透明质酸衍生物、特别是十六烷基酰胺以在5mg/ml至15mg/m之间、优选在7mg/ml至12mg/ml之间、甚至更优选为8mg/ml的浓度制备。本发明的其它HA衍生物、即自交联的衍生物/>和用BDDE获得的交联的衍生物(HBC)优选以在10mg/ml至40mg/ml之间、优选在20mg/ml至30mg/ml之间的浓度使用。浓度在5mg/ml至40mg/ml之间的HA衍生物的浓度产生稳定的凝胶,其以恒定的延长的方式释放活性成分。考虑到延长释放***,局部***可以以不同浓度包括在载体中,所述浓度可以根据需要进行调节。特别是对于罗哌卡因(优选使用的***)而言,以及对于所述的其它局部***而言,相对于最终组合物而言,浓度在10mg/ml至35mg/ml之间,优选为10mg/ml、15mg/ml、25mg/ml或35/mg/ml。基本上,关于基于/>8mg/ml和罗哌卡因碱35mg/ml的所述的药物组合物,每ml最终组合物将含有8mg />和35mg罗哌卡因碱。
针对HA衍生物和所用的碱形式的局部***所表达的那些以及它们的浓度的类似考虑也可以扩展至上述控制释放***,所述控制释放***包含基质,所述基质因此优选由透明质酸的十六烷基酰胺和碱形式的罗哌卡因组成,所述的透明质酸的十六烷基酰胺/>以在5mg/ml至15mg/ml之间、优选在7mg/ml至12mg/ml之间、甚至更优选为8mg/ml的浓度制备,所述的碱形式的罗哌卡因相对于最终组合物而言浓度在10mg/ml至35mg/ml之间、优选为10mg/ml、15mg/ml、25mg/ml或35/mg/ml。作为/>的替代品,控制释放***的基质可以由浓度在10mg/ml至40mg/ml之间、优选在20mg/ml至30mg/ml之间的上文所述的透明质酸衍生物/>或HBC以及上文所给出的浓度的碱形式的局部***、优选罗哌卡因组成。
上文所述的本发明的组合物和控制释放***可用于治疗手术后疼痛,并且具有以下特征:
·它们均是生物相容性的和生物可降解的;
·它们由亲水性基质组成,所述亲水性基质防止活性成分的异常蓄积;
·它们是可通过传统方法(高压灭菌器)灭菌的;
·它们在室温下、甚至在40°下稳定至少6个月;
·它们容易通过施加合理的压力用至多27G的针头挤出;
·它们可以通过注射和/或渗透到手术伤口周围的组织中、通过直接滴入开放性伤口中、以及通过简单的局部施用、例如通过在假体植入时在假体边缘上铺展来施用;也可以根据所进行的手术类型组合所述施用途径;
·它们可以通过单罐反应以非常简单的方式制备,并且因此从工业角度来说是经济的;
·镇痛作用维持长达5天,特别是当在伤口周围的组织中、尤其是在矫形手术伤口中施用时;
·因此,本发明的组合物可以在手术时以单剂量施用,并且可以大幅度减少或甚至完全消除伴随使用NSAID或另外的口服止痛药。
本发明的组合物适合在大量手术应用中作为镇痛药使用,所述手术包括:
·矫形手术:例如完全或部分膝或髋关节置换、肩、踝、手或拇囊肿手术或概括而言的足手术、脊椎手术等;
·腹部手术:例如疝切除术(herniectomy)、阑尾切除术、结肠切除术、胃切除术、结直肠手术等;
·泌尿生殖道手术(***切除术、肾切除术、子宫切除术、卵巢切除术、剖宫产术)等;
·血管手术(痔切除术)。
下列实施例更详细地对本发明进行了举例说明。
实施例1:(8mg/ml)和罗哌卡因碱(15mg/ml)凝胶的制备
将79.6ml PBS 3mM,pH 7.3±0.1倾入250ml配备搅拌器的反应器中;然后加入1.70g罗哌卡因HCl,其溶解时将pH调整至约5.7,将该混合物维持搅拌至少20分钟。此时,在搅拌下缓慢地加入0.4M NaOH,直至达到pH 7.2-7.4;pH的变化导致碱形式的罗哌卡因沉淀出来,导致形成非常细的混悬液。
在用PBS pH 7.3±0.1将该混悬液调整至100ml体积后,在搅拌下在反应器中加入0.8g粉状将搅拌维持约1小时,然后将该混合物保持静置至少3小时。将得到的胶凝化的混悬液混合至少10分钟,通过具有105μm的孔隙率的不锈钢过滤器过滤,再次混合至少另外10分钟。然后将胶凝化的混悬液用于填充注射器,用高压灭菌器灭菌。
实施例2:(8mg/ml)和罗哌卡因碱(25mg/ml)凝胶的制备

实施例3:(8mg/ml)和罗哌卡因碱(35mg/ml)凝胶的制备
将64ml PBS 3mM,pH 7.3±0.1倾入250ml配备搅拌器的反应器中;然后加入1.40g罗哌卡因HCl,其溶解时将pH调整至约5.7,将该混合物维持搅拌至少20分钟。
在搅拌下缓慢地加入9.6ml 0.4M NaOH,得到首次沉淀的罗哌卡因碱。
加入另外1.40g罗哌卡因HCl,其pH再降至酸性值(约5.8),维持搅拌20分钟。此时,在搅拌下缓慢地加入9.6ml 0.4M NaOH。
加入另外1.6g罗哌卡因HCl,将得到的酸性溶液(pH约5.8)维持在搅拌下20分钟。

此时,在搅拌下缓慢地加入0.4M NaOH,直至达到pH 7.2-7.4,得到罗哌卡因碱的非常细的混悬液。

实施例7:(20mg/ml)和罗哌卡因碱(15mg/ml)凝胶的制备
将80.0ml 1.5mM PBS溶液,pH 6.9±0.1倾入250ml配备搅拌器的反应器中;加入1.70g罗哌卡因HCl,将该混合物维持在搅拌下至少20分钟。该混合物的pH值为5.7。
此时,在搅拌下缓慢地加入0.4M NaOH,直至达到pH 6.8-7.0;pH的变化导致碱形式的罗哌卡因沉淀出来,导致形成非常细的混悬液。
用PBS 0.5mM,pH 6.9±0.1调整至100ml体积后,在搅拌下在反应器中加入2.0g粉末,其酯化度总计为4.3%。
持续搅拌约1小时,然后将该混合物保持静置至少3小时。
将得到的胶凝化的混悬液混合至少10分钟,通过具有105μm的孔隙率的不锈钢过滤器过滤,再次混合至少另外10分钟。然后将胶凝化的混悬液用于填充注射器,用高压灭菌器灭菌。
实施例8:HBC(20mg/ml)和罗哌卡因碱(25mg/ml)凝胶的制备
将75ml PBS 3mM,pH 7.3±0.1倾入250ml配备搅拌器的反应器中;加入1.4g罗哌卡因HCl,将该混合物维持在搅拌下20分钟;该混合物的pH为6.0。
在搅拌下缓慢地加入9.6ml 0.4M NaOH,得到首次沉淀的罗哌卡因碱。
加入另外1.43g罗哌卡因HCl,将该混合物维持在搅拌下20分钟;该混合物的pH为6.0。
此时,在搅拌下缓慢地加入0.4M NaOH,直至达到pH 7.2-7.4,得到罗哌卡因碱的非常细的混悬液。用PBS 3mM,pH 7.3±0.1将体积调整至100ml,然后加入2.0g具有约5.5%摩尔的衍生化度的HBC粉末。将该混合物加热至60℃,然后在搅拌下维持5小时,以允许HBC粉末胶凝话;然后将其冷却至20℃。
将得到的胶凝化的混悬液混合至少10分钟,通过具有105μm的孔隙率的不锈钢过滤器过滤,再次混合至少另外10分钟,最终用于填充注射器,用高压灭菌器灭菌。
罗哌卡因碱与罗哌卡因HCl从载体中的释放测试对比
该测试使用经典的释放测试仪器进行,所述仪器由供体室和接收室组成。在这种特定情况下,供体室是盛有1ml待分析样品并用截止值为100kDa的透析膜封闭的容器(Spectra/Por Float-A-Lyzer);接收室为含有15ml萃取溶剂(PBS pH 7.0±0.1)和磁性锚(magnetic anchor)的50ml试管。在将待分析的制剂装载到供体室中并将供体室***接收室后,将组件在温和搅拌下放入恒温至37°的室内。分析的样品是:
·制剂A:在8mg/ml中的罗哌卡因碱10mg/ml,如实施例4中所述制备;
·制剂B:在8mg/ml中的罗哌卡因HCl 11.34mg/ml(相当于罗哌卡因碱10mg/ml),通过省去用NaOH碱化的阶段根据实施例4简单地制备。
该测试在漏槽条件(sink condition)下进行;换句话说,在预设的时间,将萃取溶剂完全用等体积的“新鲜”溶剂替代,以防止萃取溶剂中待测物质种类的浓度的逐渐增加影响其进一步释放,从而改变测试结果。在这种特定情况下,在2、4、6、8、24、48、72和144小时后替代萃取溶剂,并基于替换的每个体积测定罗哌卡因浓度。累积结果在图1中给出。
正如可以观察到的,两种制剂之间的行为特性存在巨大差异。
本发明的制剂A以逐渐、恒定、连续、延长的方式释放罗哌卡因碱,直到其作用被耗尽,即,在约140小时(5天)后基本上释放了所有初始的罗哌卡因。相反,制剂B非常迅速地释放罗哌卡因HCl;8小时后,它释放了超过90%的***,24小时后,它释放了不少于98%的***,因此其作用实际上已经耗尽。
考虑到两种制剂的基质是相同的(相同浓度的),并且***的浓度是相等的,该测试明确证明,由透明质酸的酰胺衍生物与碱形式和亲脂性的罗哌卡因组成的亲水性基质的创新组合在以逐渐、恒定、持久的方式释放局部***、从而确保长期的止痛覆盖范围方面令人意外地有效。
通过下面所述的用动物进行的临床前功效测试确证了由此获得的实验结果。
在载体中的罗哌卡因碱的镇痛作用与罗哌卡因HCl的镇痛作用对比评价
本研究用猪进行,按照实验动物护理和使用道德委员会(Laboratory AnimalCare and Use Ethics Commission)所规定的那样对猪进行治疗。应用的模型是Brennan手术后疼痛评价模型(Brennan postoperative pain assessmen model)(Brennan TJ等人Pain,1996,64,493-501)。它在动物的皮肤上提供了一个1cm的纵向切口,其通过皮肤、筋膜和下面的肌肉切开;缝合后,按照规定的时间间隔,按照von Frey方法测量手术后疼痛,该方法如下进行:给动物皮肤施加已知直径的细丝,并对其施加力,直到感觉到的疼痛触发动物的反应。感觉到的疼痛越低,则施加的力越高,以克/mm2(g)表示;因此,在基线(切口之前)时,力值会很高;切口后,将需要更少的力来诱导动物的疼痛反应。因此,Fon Frey细丝触发动物反应所用的力越强,测试物种的镇痛作用越大。
基于本领域技术人员的知识如下确定每种待测种类的施用剂量:
·可以在不发生副作用的情况下施用的罗哌卡因HCl的最大剂量是约2.5mg/kg(Srichartz GR,Berde CB:Miller RD编辑Miller’s Anesthesia.第6版.Philadelphia:Elsevier Churchill Livingstone,2005:573-603)。因此,具体地,当在本研究中使用的每只猪的重量为约10kg时,使用2.5ml (罗哌卡因HCl 10mg/ml),等同于25mg罗哌卡因/动物;
·在文献报道的对比试验中(Davidson EM等人Anesth Analg,2016,122,1663-72),相对于水溶液中的盐酸盐形式,将罗哌卡因在亲脂性载体中以8:1的比例制备。如此高的剂量对于测试控制释放是必须的,因为已知的是质子化形式的局部***会非常迅速地代谢,并停止起作用。因此遵循该指示,向每只动物施用一定剂量的待测试的药物组合物,其含有8倍量的罗哌卡因HCl,即,相当于200mg罗哌卡因HCl。由于所测试的药物组合物含有不同浓度的罗哌卡因碱,因此计算出恰好相当于规定剂量的罗哌卡因HCl(200mg)的每种组合物的体积,因此所施用的体积明显不同。
使用30只猪,分成5组,每组6只,如下:
第1组:用载体、即使用11.76ml (8mg/ml)治疗动物,并且构成阴性对照;
第2组:用11.76ml如实施例1中所述制备的含有相当于200mg罗哌卡因HCl的量的罗哌卡因碱15mg/ml的8mg/ml治疗动物;
第3组:用7.06ml如实施例2中所述制备的含有相当于200mg罗哌卡因HCl的量的罗哌卡因碱25mg/ml的8mg/ml治疗动物;
第4组:用5.05ml如实施例3中所述制备的含有相当于200mg罗哌卡因HCl的量的罗哌卡因碱35mg/ml的8mg/ml治疗动物;
第5组:用2.5ml (罗哌卡因HCl 10mg/ml)水溶液治疗动物,相当于25mg罗哌卡因HCl,并且构成阳性对照。
在缝合伤口前通过以三种不同水平将一半体积注射入伤口部位并且将一半注射入伤口周围的组织给予治疗。
在基线、即在切口之前以及在手术后的3、8、12、24、36、48和72小时应用von Frey测试。测试结果在图2中给出。数值具有统计学显著性。
绝对明显的是:
·如所预期的,载体不具有作用;
·3小时后,的效果较好,在仅8小时后,/>的效果非常轻微,在随后的几个小时中/>的效果不复存在;
·在所用的罗哌卡因的所有浓度下,含有的测试药物组合物均具有强烈的止痛活性,在治疗后3小时,一些浓度下的止痛活性(第2组)远高于/>的止痛活性,或者在任何情况下与之类似,并且在后面的时间则均高得多。增加的时间降低了较低浓度的罗哌卡因的组合物的止痛作用(第2组和第3组),而增加了较高浓度的该种类的止痛作用(第4组)。这进一步证明了本发明中所述的控制释放***的功效。
尽管出于道德原因在72小时(3天)后停止了施用von Frey测试,但将图2中的数据与图1中的释放曲线数据一起进行分析,表明所测试的种类的止痛功效此后保持了很长时间,因为,例如,在罗哌卡因的最大浓度下(第4组)该种类在治疗后72小时具有非常强的镇痛作用:需要比基线所需的力高约80%的力来诱导动物的疼痛刺激。这方面很重要,因为这意味着可以基于所需的止痛作用的持续时间、根据患者要进行的手术操作以及其它临床考虑来调节罗哌卡因的量。
除了清楚地证明了本发明的药物组合物的长期功效的这些结果之外,还应当注意,尽管总体施用的活性成分的剂量远高于盐酸盐形式的标准剂量,但治疗的动物中没有任何一只出现因罗哌卡因剂量过量引起的不良反应。
这意味着本文所测试的药物组合物:
·局部***以控制释放***起作用,从而延长了其随时间推移的镇痛作用;
·不引起剂量过量和/或蓄积,因此防止了众所周知的毒性副作用;
·消除了重复施用的需要;
·允许对止痛作用的持续时间进行调节,
因此代表了在治疗手术后疼痛方面对本领域现有技术的确定的改进。
Claims (7)
1.用于治疗手术后疼痛的凝胶形式的药物组合物,其由亲水性基质组成,所述亲水性基质由以下物质组成:浓度等于8mg/ml的、由重均分子量在500kDa至730kDa之间的透明质酸制备的并且具有在1%摩尔至3%摩尔之间的平均酰胺化度的透明质酸的十六烷基酰胺,和浓度在10mg/ml至35mg/ml之间的碱形式的罗哌卡因,并且任选地含有药学上可接受的赋形剂。
2.根据权利要求1所述的组合物,其能以延长的方式释放亲脂性的碱形式的罗哌卡因长达5天的时间。
3.制备权利要求1或2的组合物的方法,所述方法包括:
·使罗哌卡因以其成盐形式以一份或多份溶解在水性载体中;
·通过用碱处理前一步骤中获得的溶液直至达到在6.5至8之间的pH值沉淀出碱形式的罗哌卡因;
·向所得的混悬液中加入由重均分子量在500kDa至730kDa之间的透明质酸制备的并且具有在1%摩尔至3%摩尔之间的平均酰胺化度的透明质酸的十六烷基酰胺。
4.根据权利要求3所述的方法,其中所述的水性载体是PBS。
5.根据权利要求3或4所述的方法,其中在第二个步骤中用于处理获得的溶液的所述碱是NaOH。
6.控制释放***,其由基质组成,所述基质由以下物质组成:浓度为8mg/ml的、由重均分子量在500kDa至730kDa之间的透明质酸制备的并且具有在1%摩尔至3%摩尔之间的平均酰胺化度的透明质酸的十六烷基酰胺,和浓度在10mg/ml至35mg/ml之间的碱形式的罗哌卡因。
7.权利要求6所述的控制释放***在制备用于治疗手术后疼痛的药剂中的用途。
