背景技术
CD30为120千道尔顿(KD)的跨膜糖蛋白(可参见:Froese et al.,1987,J.Immunol.139:2081-87),是肿瘤坏死因子受体(TNFR)超家族的成员。CD30是霍奇金淋巴瘤(HL)和间变性大细胞淋巴瘤(ALCL)内的恶性细胞成熟的标志。CD30最初是使用单克隆抗体Ki-1在培养的H-R(Hodgkin's-Reed Steinberg)细胞中被鉴定的。

发明内容

本发明尤其提供了卵巢癌(例如卵巢浆液性癌)、皮肤癌(例如黑色素瘤和皮肤鳞状细胞癌)、乳癌(例如三阴性乳腺癌)、甲状腺癌(例如甲状腺未分化癌)、胰腺癌(例如未分化胰癌)、肺癌(例如小细胞和鳞状上皮细胞肺癌)、***癌(例如***鳞状细胞癌)、胸腺癌、子宫内膜癌和原发不明癌的诊断、预后、预防、治疗以及监测治疗的方法。本发明尤其提供了泌尿生殖鳞状细胞癌、妇科癌肉瘤、尿道鳞状细胞癌、子宫癌肉瘤、支持细胞瘤、***瘤和胰腺癌的诊断、预后、预防、治疗以及监测治疗的方法。在一些方面,CD30的抗体被用于诊断、预后、预防、治疗以及监测治疗的方法。这些方法部分基于实施例中所呈现的结果的前提下,即CD30在某些癌症中表达。在来自癌组织的***固定石蜡包埋(FFPE)的样品中,利用结合CD30的抗体检测这种表达。
II.针对CD30的抗体
针对CD30的抗体可用于癌症(例如,卵巢癌、皮肤癌、乳腺癌、甲状腺癌、胰脏癌、肺癌、***癌、胸腺癌、子宫内膜癌、原发不明癌)中CD30的检测以及癌症的治疗。取决于抗体的性质,某些抗体优选用于检测,而其他抗体优选用于治疗。
A.针对CD30的抗体的一般性描述
抗CD30的抗体包括单克隆、嵌合(具有人恒定区和小鼠可变区)、人源化、镶饰(veneered)或人抗体;单链抗体等等。免疫球蛋白分子可以是任何类型或类别(例如,IgG、IgE、IgM、IgD、IgA和IgY)的或亚类(例如,IgGl、IgG2、IgG3、IgG4、IgA1和IgA2)的。
抗CD30的抗体可以是结合抗原的抗体片段,例如,Fab、F(ab')、F(ab')2、Fd链,单链Fv(scFv),单链抗体,二硫键连接的Fv(sdFv),包含VL或VH结构域的片段,包括纳米抗体或来自骆驼、美洲驼(llamas)等的片段,或Fab表达文库产生的片段,或上文所述的任何抗体的CD30结合片段。结合抗原的抗体片段(包括单链抗体)可包含单独的可变区或与下述全部或部分组合的可变区:铰链区、CH1,CH2,CH3和CL结构域。此外,结合抗原的片段还可包含可变区与铰链区、CH1,CH2,CH3和CL结构域的任何组合。
抗体可以是单特异性、双特异性、三特异性或更多特异性的。多特异性抗体可以是对CD30不同的表位特异的,或者可以对CD30以及异源蛋白两者均特异(可参考,例如,WO93/17715;WO 92/08802;WO 91/00360;W092/05793;Tutt et al.,1991,J.Immunol.147:60-69;美国专利号4,474,893;4,714,681;4,925,648;5,573,920和5,601,819;Kostelny etal.,1992,J.Immunol.148:1547-1553)。可用于实施本文所述的方法的多特异性抗体(包括双特异性和三特异性抗体)是与CD30以及第二细胞表面受体或受体复合物(例如,免疫球蛋白基因超家族成员、TNF受体超家族成员、整联蛋白、细胞因子受体、趋化因子受体、主要组织相容性蛋白、凝集素(C型、S型或I型)或补体控制蛋白)二者均免疫特异性结合的抗体。
还可按照它们与CD30的结合亲和力(10-7M,5x 10-8M,10-8M,5x 10-9M,10-9M,5x 10-10M,10-10M,5x 10-11M,10-11M,5x 10-12M,10-12M,5x 10-13M,10-13M,5x 10-14M,10-14M,5x 10- 15M或10-15M)来描述抗CD30抗体。
抗CD30抗体可以是嵌合抗体。嵌合抗体是这样一种分子,该抗体中的不同部分源于不同的动物物种,例如,具有源于鼠单克隆抗体的可变区和人免疫球蛋白恒定区的抗体。用于产生嵌合抗体的方法是本领域己知的(参考,例如,Morrison,Science,1985,229:1202;Oi et al.,1986,BioTechniques 4:214;Gillies et al.,1989,J.Immunol.Methods125:191-202;美国专利号5,807,715;4,816,567和4,816,397)。

可通过己知方法测定抗体与CD30的特异性结合,例如,竞争性和非竞争性免疫测定***,使用诸如蛋白质印迹、放射免疫测定、ELISA(酶联免疫吸附测定)、“夹层”免疫测定、免疫沉淀测定、沉淀素反应、凝胶扩散沉淀素反应、免疫扩散测定、凝集测定、补体固定测定、免疫放射分析测定、荧光免疫测定、蛋白A免疫测定(可参考,例如,Ausubel et al.,eds.,Short Protocols in Molecular Biology(John Wiley&Sons,Inc.,New York,4thed.1999);Harlow&Lane,Using Antibodies:A Laboratory Manual(Cold Spring HarborLaboratory Press,Cold Spring Harbor,N.Y.,1999)。
此外,可通过竞争性结合测定法来测定抗体与CD30的结合亲和性以及抗体CD30相互作用的解离速率(off-rate)。竞争性结合测定的一个例子是放射免疫测定,包括:在存在增加量的未经标记的CD30的情况下,将经标记的CD30(例如,3H或125I)与目的抗体一起孵育,并且检测与经标记的CD30结合的抗体。然后,可通过Scatchard作图分析的数据来确定抗体对CD30的亲和性和结合解离速率。还可使用放射免疫测定来测定与第二抗体的竞争。在这种情况下,在存在增加量的未经标记的第二抗体的情况下,将CD30与缀合到经标记的化合物(例如,3H或125I)上的目标抗体一起孵育。或者,可通过表面等离振子共振来测定抗体与CD30的结合亲和性以及抗体CD30相互作用的结合和解离速率。

用于产生识别特定表位的抗体片段的技术也是本领域公知的。例如,可通过使用木瓜蛋白酶(以产生Fab片段)或胃蛋白酶(以产生F(ab')2片段),对免疫球蛋白分子进行蛋白水解切割,来产生Fab和F(ab')2片段。F(ab')2片段含有可变区、轻链恒定区和重链的CH1结构域。还可使用例如WO 92/22324;Mullinax et al.,1992,BioTechniques 12(6):864-869;和Sawai et al.,1995,AJRI 34:26-34;和Better et al.,1988,Science 240:1041-1043(这些文献的公开内容全部通过引用并入本文)中公开的方法,使用重组产生Fab,Fab'和F(ab')2片段的技术。

例如,为重组表达抗CD30抗体,表达载体可编码与启动子有效连接的其重链或轻链或重链或轻链可变区。表达载体可包括,例如,编码抗体分子的恒定区的核苷酸序列(可参考,例如WO 86/05807;W0 89/01036和美国专利号5,122,464),并且抗体的可变结构域可被克隆进此类载体,用于表达整条重链或轻链。通过己知技术将表达载体转入宿主细胞,然后培养经转染的细胞以产生抗CD30抗体。典型地,为表达双链抗体,编码重链和轻链的载体可在宿主细胞中共表达,以表达整个免疫球蛋白分子。
可利用多种原核和真核宿主表达载体***来表达抗CD30抗体或其衍生物。典型地,真核细胞(特别是对于整个重组的抗CD30抗体分子而言)被用于表达重组蛋白。例如,与载体(例如,来自人巨细胞病毒的主要中间早期基因启动子元件或中国仓鼠卵巢EF-1α启动子)结合的哺乳动物细胞(例如,中国仓鼠卵巢细胞(CHO)(例如DG44或CHO-S)),是生产抗CD30抗体及其衍生物的有效表达***(参考,例如,Foecking et al.,1986,Gene 45:101;Cockett et al.,1990,Bio/Technology 8:2;Allison,U.S.Patent No.5,888,809)。

B.用于检测CD30的抗体
本发明中描述的用于检测癌症中CD30的抗体是那些特异性结合CD30的抗体。单克隆抗体与其目标抗原的特异性结合意味着至少为106,107,108,109或1010M-1的亲和力,与至少一种不相关的目标相比,被检测到的数量级更高,且与非特异性结合有区别。特异性结合是特定功能基团或特定空间配合(例如,锁和钥匙模型)之间的键形成的结果,而非特异性结合通过是范德华力的作用。对用于检测方法中的针对CD30的抗体的选择取决于是否通过需要检测变性的CD30或天然CD30(细胞上表达的)的技术来检测CD30。优选用于识别可以从针对CD30的制剂中获益的患者的抗体是那些特异性结合到CD30胞外结构域的抗体。在一些方面,抗体(例如,Ber-H2)将结合到被***固定且被石蜡包埋(FFPE)的癌症样本的CD30上。
C.针对CD30的制剂
针对CD30的制剂包括具有任何针对CD30的细胞毒性剂的试剂。针对CD30的制剂包括抗CD30抗体和抗CD30抗体药物缀合物,以及其他的抗CD30结合试剂和其缀合物。
用于治疗性应用的抗体特异性结合到表达于癌细胞的原始(native)CD30的胞外结构域。尽管本发明的实际应用不依赖于对机制的理解,但我们相信该抗体具有细胞毒性或抑制细胞生长的效果,要么是与CD30结合且内化到细胞内部引起的,要么是与CD30结合且在细胞外面聚集引起的。不管是哪一种情况,细胞毒性效果可以通过将抗体缀合到细胞毒性试剂上而增强。通过抗体结合到CD30而在细胞外展现出的细胞毒性可以通过抗体恒定(效应)功能额外地或选择性地增强。抗体恒定结构域介导多种Ig效应功能,例如,抗体参与抗体依赖性细胞毒性(ADCC)、补体依赖性细胞毒性(CDC)和/或抗体依赖性细胞吞噬(ADCP)。任选地,可通过US2012/0014943中描述的多种方法,增大CD30结合试剂的效应功能。
依据本发明的方法,适用的抗CD30抗体包括任何特异性结合CD30抗原的抗体。本发明的抗CD30抗体优选为单克隆抗体,可能包括,例如,嵌合抗体(例如,具有人恒定区和鼠可变区),人源化抗体或人抗体。免疫球蛋白分子是IgG型,可以是免疫球蛋白分子的任何亚型(例如,IgG1,IgG2,IgG3,IgG4)及其变体。免疫球蛋白分子优选为IgG1。依据本发明的方法,多特异性抗体也同样适用。本发明的抗体可以通过本领域任何适用的方法产生。抗CD30抗体的具体例子包括但不限于人源化或嵌合AC10抗体。鼠AC10已经保藏,ATCC(美国菌种保藏中心)编号为PTA-6679。在一个具体实施例中,抗CD30抗体是cAC10抗体。在本文中,cAC10抗体是一种具有鼠AC10的重链和轻链可变区、人γ恒定区和人κ恒定区的抗体。
针对CD30的抗体及其衍生物可与细胞毒性或细胞抑制性部分缀合,形成抗体药物缀合物(ADC)。特别适合与抗体或抗体衍生物缀合的部分是化学治疗剂、前体药物转化酶、放射性同位素或化合物或者毒素。例如,抗CD30抗体或其衍生物可与细胞毒性剂,例如,化学治疗剂或毒素(例如,细胞抑制性或杀细胞性试剂,例如,相思豆毒蛋白、蓖麻毒蛋白A、假单胞菌外毒素或白喉毒素)缀合。细胞毒性剂的有用类别的例子包括auristatins(一种人工合成的细胞毒素,微管聚合物抑制剂)、喜树碱类、多卡米星类、依托泊普类、美登木素生物碱类和长春花生物碱类。用于将治疗剂与蛋白质(特别是与抗体)缀合的技术是公知的。(可参考,例如,Alley et al.,Current Opinion in Chemical Biology 2010 14:1-9;Senter,Cancer J.,2008,14(3):154-169)
合适的细胞毒性剂包括,例如,auristatins(例如,auristatin E、AFP、MMAF、MMAE)、DNA小沟结合剂(例如,烯二炔类和5-噻唑甲酰胺(lexitropsin))、多卡米星类、紫杉烷类(例如,紫杉醇和多西他赛)、长春花生物碱类、阿霉素、吗琳代-阿霉素和氰基吗琳代-阿霉素。
合适的抗体药物缀合物包括基于auristatin的抗体药物缀合物,意味着药物组分是auristatin药物。Auristatin结合到微管蛋白,已经证明其与微管动力学和细胞核、细胞***相关,且其具有抗癌活性。Auristatin可以是auristatin E或其衍生物。Auristatin是,例如,auristatin E和酮酸之间形成的酯。例如,auristatin E与对乙酰苯甲酸或苯甲酰戊酸反应,分别产生AEB和AEVB。其它典型的auristatins包括MMAF和MMA。示例的auristatin的合成和结构被描述于美国专利公开号7,659,241;7,498,298;2009-

基于auristatin的抗体药物缀合物实施例包括如下所示的vcMMAE、vcMMAF和mcMMAF抗体药物缀合物,其中,Ab是抗CD30抗体,“val-cit”代表缬氨酸-瓜氨酸二肽或其药学上可接受的盐:
载药量用p表示,即每个抗体的药物连接分子的数目。根据上下文,p可能代表每个抗体的药物连接分子的平均数目,也指平均药物载药量。P的范围为1~20,优选为1~8。在优选实施例中,当p代表平均载药量时,p的范围为约2~5。在一些实施例中,p约为2,约为3,约为4或约为5。在制备中,每个抗体中药物的平均数目可以用常规手段,例如,质谱、HIC、ELISA和HPLC表征。
在一个特定的优选实施例中,基于auristatin的抗CD30抗体药物缀合物是本妥昔单抗(Brentuximab vedotin),一种抗体药物缀合物,具有以下结构:
本妥昔单抗是一种针对CD30的抗体药物缀合物,由三种成分组成:(i)嵌合的IgG1抗体cAC10,对人CD30是特异性的;(ii)微管破坏剂MMAE;以及(iii)将MMAE共价结合到cAC10的蛋白酶可裂解连接分子。在本妥昔单抗结构中,药物与抗体的比例或载药量用p表示,其范围为1~8之间的整数。在药学制备中,平均载药量是3~5。
Ⅲ.检测CD30
用于诊断性应用的待检测组织样品可通过手术程序获得,例如,活组织检查。典型地,通过免疫测定来检测CD30,其中,将含有己知或怀疑来自癌症(例如,卵巢癌(例如卵巢浆液性癌)、皮肤癌(例如黑色素瘤和皮肤鳞状细胞癌)、乳癌(例如三阴性乳腺癌)、甲状腺癌(例如甲状腺未分化癌)、胰腺癌(例如未分化胰癌)、肺癌(例如小细胞和鳞状上皮细胞肺癌)、***癌(例如***鳞状细胞癌))的细胞的样品与抗CD30抗体接触。接触之后,测定样品中抗体与细胞的结合事件的存在与否。结合与在该样品中癌性细胞上表达的抗原的存在与否相关。在一些方面,将样品与能产生可检测信号的抗CD30抗体的经标记特异性结合配偶体接触。在其他方面,抗CD30抗体本身可被标记。标记的类型的例子包括酶标记、聚合物标记、放射性同位素标记、非放射性标记、荧光标记、毒素标记和化学发光标记。对来自标记的信号的检测指示与样品中CD30特异性结合的抗体的存在。
来自患者的组织样品可被固定,冷冻,固定,离心和/或包埋,例如,石蜡包埋。优选地,进行分析的样品被固定或冷冻,以允许进行组织学切片。优选地,在醛固定剂(例如,甲醛、多聚甲醛、戊二醛)或重金属固定剂(例如,氯化汞)中固定经切除的组织样品。更优选地,在与抗体一起孵育之前,在***中固定经切除的组织样品,并且将其包埋于石蜡中。经***固定石蜡包埋(FFPE)的样品的优点是在组织切片中保存了细胞和构造形态细节(可参考,例如,Fox et al.,1985,J.Histochem.Cytochem.33:845-853)。任选地,可用柠檬酸盐、EDTA,酶促消化或热来处理FFPE样品,以增加表位的可及性(可参见,例如Shi etal.,1991,J Histochem Cytochem.39:741-7488)。
在一些实施例中,免疫组化技术被用于检测CD30。免疫组化是指基于特异性抗体和抗原相互作用原理检测组织切片细胞中的抗原的方法。结合的抗体可以通过多种方式被检测,包括,例如,荧光检测方法、酶检测方法和基于聚合物的检测***。
可选择地,可从己知或疑似的癌症细胞中分离蛋白部分,并通过ELISA、蛋白质印迹、免疫沉淀等等进行分析。在另一变型中,可通过流式细胞分析对表达CD30的细胞加以分析,优选与其他癌细胞标记组合。
除了那些利用抗体的方法,也可以用其他方法检测CD30。例如,可从己知或疑似的癌细胞中提取mRNA。然后可通过与结合编码CD30的DNA的核酸探针的杂交,来分析mRNA或源于其的核酸,例如cDNA。
在另一变型中,可通过向患者施用经标记的抗CD30抗体并经由体内成像对抗体加以检测,来对癌症(卵巢癌、胰腺癌、皮肤癌、乳腺癌、甲状腺癌、胰腺癌、小细胞肺癌、***癌、胸腺癌或子宫内膜癌或未知原发性癌症)进行体内检测。
对组织样品中CD30的检测可定性或定量进行或者两者均进行。定性检测表示检测CD30表达存在与否。定量表达表示测定CD30的表达水平。可(但不是必须)参照一个或多个标准品来测定所讨论的癌症组织样品中CD30的存在和/或水平。可根据历史纪录或同时测定标准品。标准品可以是,例如,来自不同受试者的己知不是癌的样品、来自患者或其它受试者的己知不表达CD30的组织或者相应细胞系。标准品还可以是在分析下与不与CD30结合的对照抗体接触的患者样品。因为CD30在非癌性的卵巢、胰腺、皮肤、乳腺、子宫内膜、甲状腺、胰腺、小细胞肺、***或胸腺组织中不会以显著程度表达,当用特异性CD30检测方法进行检测时,此类非癌组织可作为零(背景)表达标准品。

在一些方面,来自抗CD30抗体结合的可检测信号的存在足以识别可以进行针对CD30的制剂治疗的患者。在一些方面,至少10%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少15%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少15%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少20%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少25%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少30%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少35%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少40%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少45%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少50%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少75%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少80%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在一些方面,至少85%的表达水平被用于识别可以进行针对CD30的制剂(例如,具有抗CD30抗体或抗CD30抗体药物缀合物的制剂)治疗的患者,其中表达水平是显示出可检测到CD30表达的样品(例如,组织切片)中恶性和/或非典型细胞的百分比。在任意这些实施例中,表达强度也可以被确定,用于获取样品中总表达的量度或样品中表达CD30的细胞的量度。
在一些方面,在细胞膜、高尔基体和/或细胞质中检测CD30的表达。
Ⅳ.诊断、预后、设计和监测治疗
在来自癌症(例如,卵巢癌、胰腺癌、皮肤癌、乳腺癌、甲状腺癌、胰腺癌、小细胞肺癌、***癌、胸腺癌或子宫内膜癌或未知原发性癌症)患者的肿瘤组织样品中检测到CD30的表达可以作为样品是癌性的指示。通过CD30的存在和/或水平提供的对癌症的指示可以与通过医师、X射线、CT扫描(计算机断层扫描)、PET扫描(正电子成像术)、PET/CT扫描、超声波、MRI(核磁共振成像)、内窥镜检查、ERCP(内镜下逆行胰胆管造影术)、组织学检查、细胞遗传学和组织培养对患者的内在或外在检查等诊断手段结合,以实现总体诊断。
可能对医师来说,最相关的是CD30的存在和水平提供了用于为患者设计治疗方案(特别是向患者施用针对CD30的制剂)的有用信息。因为在正常组织中基本上不存在可检测的CD30表达,该受体在癌症中的存在提供了治疗性治疗的靶标。治疗之后,对CD30进行的持续分析提供了监测治疗是否有效的手段,CD30阳性信号水平(即CD30阳性癌细胞存在的代表)的降低表明治疗有效。
V.适于接受治疗的患者
适于通过所述方法来治疗的患者通常在其癌组织或其它组织中具有可检测到的CD30水平,并伴随上文所述的其它癌症病征或症状。在一些实施例中,适于通过本发明方法来治疗的患者仅仅需要在他们的癌组织中具有可检测到的CD30水平。在其他实施例中,适于通过本发明方法来治疗的患者具有至少10%,至少15%,至少20%,至少25%,至少30%,至少35%,至少40%,至少45%,至少50%,至少60%,至少70%,至少75%,至少80%或至少85%的CD30表达水平,其中表达水平是显示出可检测的CD30的表达的样品中恶性或非典型细胞的百分比。在优选实施例中,当使用CD30特异性的抗CD30抗体作为检测抗体时,样品中至少10%,15%,20%,25%,30%,35%,40%,45%,50%,60%,70%,75%,80%或85%的细胞表达CD30。该样品典型地是来自患者的肿瘤样品。
有时,受本发明的方法治疗的患者此前接受过其它类型的治疗(例如,手术、化疗和/或放疗),但并没有导致康复或者甚至减缓癌的生长。在一些此类患者中,癌症对一种或多种此类疗法的治疗是顽固的。
有时,受本发明的方法治疗的患者此前接受过其它类型的治疗(例如,手术、化疗和/或放疗),但是复发了。
有时,受本发明的方法治疗的患者是初次治疗(例如,其癌症没有经历手术、化疗或放疗)。有时,受本发明的方法治疗的患者是新近诊断的。
一些处于癌症风险的患者也可在疾病病征和症状出现之前被预防性治疗。此类个体包括亲属曾经历这些疾病的个体以及通过遗传学或生物化学标记分析确定为具有风险的个体。
Ⅵ.治疗方法
本发明提供了通过针对CD30的制剂(例如,本文公开的抗体和ADC以及其它抗CD30结合试剂)(合称为试剂))来治疗或预防表达CD30的癌症(例如,卵巢癌、皮肤癌、乳腺癌、甲状腺癌、胰腺癌、肺癌、***癌、胸腺癌、子宫内膜癌以、未知原发性癌、胸腺癌、泌尿生殖器的鳞状细胞癌、妇科癌肉瘤、支持细胞瘤、***瘤以及胰腺癌)的方法。可向患者施用组合物。在一些方面,卵巢癌是卵巢浆液性癌;皮肤癌是黑色素瘤和皮肤鳞状细胞癌;乳癌是三阴性乳腺癌;肺癌是小细胞和鳞状上皮细胞肺癌);甲状腺癌是甲状腺未分化癌;胰腺癌是恶性或未分化胰腺癌;***癌是***鳞状细胞癌)。
多种递送***可用于施用所述试剂,包括皮内、肌内、腹膜内、静脉内、皮下、鼻内、硬膜外和口服途径。试剂可例如,通过输注或推注,通过经由上皮或皮肤粘膜内衬(例如,口腔黏膜、直肠和肠黏膜等等)的吸收来施用,并且可与其它生物活性试剂(例如,化学治疗剂)一起施用。施用可以是全身性的或局部性的。
试剂可通过注射、通过导管、通过栓剂或者通过植入物(植入物可以是有孔的、无孔的或凝胶状材料,包括膜,例如sialastic膜,或纤维)施用。
可选择地,试剂可以在控释***中递送。例如,可使用泵(可参见,Langer,1990,Science 249:1527-1533;Sefton,1989,CRC Crit.Ref.Biomed.Eng.14:201;Buchwald etal.,1980,Surgery 88:507;Saudek et al.,1989,N.Engl.J.Med.321:574)。或者,可使用聚合材料(可参考Medical Applications of Controlled Release(Langer&Wise eds.,CRC Press,Boca Raton,Florida,1974);Controlled Drug Bioavailability,DrugProduct Design and Performance(Smolen&Ball eds.,Wiley,New York,1984);Ranger&Peppas,1983,Macromol.Sci.Rev.Macromol.Chem.23:61.See also Levy et al.,1985,Science 228:190;During et al.,1989,Ann.Neurol.25:351;Howard et al.,1989,J.Neurosurg.71:105.)。其它控释***在,例如,上文提及的Langer中进行了讨论。
试剂可作为包含治疗或预防有效量的试剂和一种或多种药物相容成分的药物组合物施用。例如,药物组合物典型地包括一种或多种药物载体(例如,无菌液体,例如,水和油,包括石油、动物、植物或合成来源的那些,例如,花生油、大豆油、矿物油、芝麻油等等)。当药物组合物静脉内施用时,水是更典型的载体。盐水溶液和右旋糖和甘油水溶性也可用作为液体载体,特别是用于可注射溶液。合适的药物赋形剂包括,例如,淀粉、葡萄糖、乳糖、蔗糖、明胶、麦芽、稻、面粉、白垩、硅胶、硬脂酸钠、单硬脂酸甘油酯、滑石、氯化钠、脱脂奶粉、甘油、丙二醇、水、乙醇等等。如果需要的话,组合物还可含有少量润湿剂或乳化剂、pH缓冲剂(例如,氨基酸)和/或增溶剂或稳定剂(例如,非离子表面活性剂,例如吐温或糖,例如蔗糖、海藻糖等等)。这些组合物可采用溶液剂、混悬剂、乳剂、片剂、丸剂、胶囊剂、粉剂、持续释放制剂等等的形式。组合物可与常规粘合剂和载体(例如,甘油三酯)一起配制为栓剂。口服制剂可包括标准载体,例如药物级的甘露醇、乳糖、淀粉、硬脂酸镁、糖精钠、纤维素、碳酸镁等等。合适的药物载体的例子被描述于E.W.Martin的“Remington’s PharmaceuticalSciences”中。此类组合物将含有治疗有效量的核酸或蛋白质(典型地,纯化形式的)以及合适量的载体,以提供对患者适当施用的形式。制剂与施用方式相对应。
典型地,用于静脉内施用的组合物是在无菌等渗水性缓冲液中的溶液。如果需要的话,药物还可包括增溶剂和局部麻醉剂,例如利多卡因,以减轻注射部位的疼痛。通常,成分单独提供或者混合到一起以单位剂量形式提供,例如,作为标出活性剂的量的密封容器(例如,安瓿或小药囊)中的冻干粉或浓缩物。当药物通过输注施用时,可将其分散于含有无菌药物级的水或盐水的输注瓶中。当药物通过注射施用时,可提供用于注射的无菌水或盐水的安瓿,使得各成分在施用之前被混合。
可通过标准临床技术来测定对于治疗或预防癌症有效的试剂的量。此外,任选地,可利用体外测定法来帮助鉴定最优剂量范围。用于制剂中的精确剂量也取决于施用途径以及癌症的分期,这应当根据执业医师的判断和每位患者的状况来决定。有效剂量可从源于体外或动物模型测试***的剂量反应曲线外推得到。可在动物模型中配制剂量,以获得包括IC50(即,实现对症状的半最大抑制的测试化合物浓度)(如在细胞培养物中测定)的循环血浆浓度范围。
例如,可通过用于测定LD50(使50%的群体致死的剂量)和ED50(在50%的群体中治疗有效的剂量)的标准药物程序,在细胞培养物或实验动物中测定试剂的毒性和治疗效果。毒性和治疗效果的剂量比是治疗指数,其可表示为LD50/ED50比。显示出大的治疗指数的试剂是优选的。当试剂展示出毒性副作用时,可使用将试剂靶向至受影响的组织位点的递送***,使得对不表达CD30的细胞的潜在危害最小,由此降低副作用。
通常,施用给患有表达CD30的癌症的患者的抗体或ADC为0.01mg/kg至25mg/kg受试者体重或0.1mg/kg至25mg/kg受试者体重。更典型地,施用给受试者的剂量通常为0.lmg/kg至l0mg/kg受试者体重,进一步更典型为0.lmg/kg至5mg/kg,0.lmg/kg至3mg/kg受试者体重。通常,人抗体在人体内具有比来自其它物种的抗体更长的半寿期,这是由于对外来蛋白的免疫应答导致的。因此,包含人源化、嵌合或人抗体的ADCs的较低剂量和较低施用频率通常是可能的。
用于治疗霍奇金淋巴瘤或间变性大细胞淋巴瘤的本妥昔单抗的推荐给药剂量是每三周1.8mg/kg,直到最多16个周期。尽管目前霍奇金淋巴瘤的推荐剂量是1.8mg/kg,对于其他癌症,可以预期其他更高或更低的剂量,例如2.4mg/kg。通过超过30分钟的静脉输注进行给药。在一个具体实施方式中,本妥昔单抗将以每三周1.8mg/kg或2.4mg/kg的给药剂量被提供给本文所述的患者。但是,其他的给药方案或路径也被本发明预期和包含。在一个实施例中,可选择的剂量方案是每4周中有3周的周剂量是约0.8mg/kg~1.2mg/kg。给药频率和剂量取决于许多因素,包括患者的情况和疾病的严重程度。
针对CD30的制剂还可以与一种或多种用于其它治疗剂组合施用(包括按顺序),以治疗或预防癌症,特别地,针对CD30的制剂可能与其他用于即将治疗特定疾病的医护标准制剂(例如,一线护理标准(front-line standard of care)或二线或三线治疗,或甚至抢救治疗)一起给药。在一些方面,组合疗法可能包括第二种细胞抑制剂或细胞毒性剂(例如,未经缀合的细胞抑制剂或细胞毒性剂,例如,那些常规用于癌症治疗的那些制剂)。组合疗法还可包括,例如,施用靶向表达CD30的癌细胞表面上CD30之外的受体或受体复合物的试剂。典型地。此类抗体或配体与肿瘤中表达CD30的癌细胞或其他细胞上的细胞表面受体结合,通过向表达CD30的癌细胞递送细胞毒性信号或通过减少抗凋亡机制来增强抗CD30抗体的细胞毒性效果。
可与所述试剂一起施用的其它药物包括生长因子抑制剂或者抗血管发生因子。
本发明的方法可与其它治疗手段(例如手术、放疗、靶向疗法、免疫疗法、使用生长因子抑制剂或抗血管发生因子)组合。
手术是优选的治疗,为了获得组织样品以用于经由其组织学进行差异诊断,手术通常是必需的。提高的存活率归因于更精确的疾病分期和更高的肿瘤主动手术切除率。手术类型取决于诊断时癌症扩散的程度(癌症分期)以及癌症的假定类型和级别。
针对CD30的制剂可同时施用给正在经历手术、化疗或放疗治疗的患者。在一些其他实施例中,患者可在施用针对CD30的制剂之前或之后至少1小时至多达数月(例如,在施用ADC之前或之后至少1小时、5小时、12小时、1天、1周、1月或3个月)时经历手术、化疗或放疗。
在以下具体实施例中对本发明进行进一步描述,这些具体实施例并不构成对本发明保护范围的限制。
实施例
利用Ber-H2检测CD30在肿瘤和非肿瘤癌症组织中的表达
肿瘤组织来自于具有卵巢癌(例如卵巢浆液性癌)、皮肤癌(例如黑色素瘤和皮肤鳞状细胞癌)、乳癌(例如三阴性乳腺癌)、甲状腺癌(例如甲状腺未分化癌)、胰腺癌(例如未分化胰癌)、肺癌(例如小细胞和鳞状上皮细胞肺癌)、***癌(例如***鳞状细胞癌)、子宫内膜癌和原发不明癌、胸腺癌、泌尿生殖鳞状细胞癌、妇科癌肉瘤、***瘤和支持细胞瘤的患者。该组织样品依据常规的方法被固定且被包埋在石蜡中。由石蜡块制备组织切片(4~6微米)。
间接IHC技术分析被用于利用抗CD30抗体(商业化购买的克隆Ber-H2)检测CD30。在该方法中,未经缀合的初始抗体(Ber-H2,2ug/ml)被用作初始抗体。通过过氧化酶阻断步骤中和内源性过氧化物酶活性。基于聚合物的EnVisionTM FLEX+***(Dako,格洛斯楚普,丹麦)被用于CD30检测。聚合物技术使用标记有右旋糖酐分子的酶,在右旋糖酐分子上带有平均70个酶分子和10个第二抗体分子。加入底物色素原(例如,3,3-二氨基联苯胺(DAB)或红色检测***),褐色或红色(分别地)沉淀物附在标记的聚合物上。然后,切片被苏木精复染色,以完成染色过程。由病理学家对切片进行评估。在膜、细胞质、高尔基体或这三种亚细胞位点的组合中检测到CD30信号。在每个染色步骤中,使用阴性对照和合适的阳性对照。基于涉及的肿瘤的百分比,评估CD30的免疫组化(IHC)表达。
所有肿瘤组织的结果如表1所示
CD30+患者的治疗
具有CD30+卵巢癌、乳腺癌、胰腺癌和黑色素瘤的患者采用本妥昔单抗治疗,以每3周1.8mg/kg的剂量给药4次。治疗了12名患有卵巢癌的患者,5名达到了病情稳定状态。治疗了2名患有乳腺癌的患者,1名达到了病情稳定状态。治疗了1名患有黑色素瘤的患者,达到了病情稳定状态。治疗了1名患有胰腺癌的患者,在第12个周期中实现了部分应答。

Claims (1)
1.本妥昔单抗在制备用于治疗CD30阳性癌症的制剂中的应用,所述CD30阳性癌症为黑色素瘤或胰腺癌,其中,所述CD30阳性黑色素瘤中CD30阳性肿瘤细胞的百分比为10~50%,所述CD30阳性胰腺癌中CD30阳性肿瘤细胞的百分比为45~60%,所述CD30阳性肿瘤细胞的百分比是肿瘤样品中恶性细胞和/或异型细胞中的CD30阳性的恶性细胞和/或异型细胞所占的百分比比例,其中所述肿瘤样品来自黑色素瘤患者的皮肤肿瘤部位或胰腺癌患者的胰腺肿瘤部位。
