技术领域
本发明涉及医学图像处理、超声波温度成像,射频烧灼技术领域,特别是涉及用于射频烧灼手术中,结合回波时移技术和2-D热传导方程监控组织射频消融。
背景技术
原发性肝癌(primary liver cancer)是世界范围内的第五大常见癌症,死亡率居于所有癌症中的第三位[1]。肝细胞癌患者(hepatocellular carcinoma,HCC)更是占了原发性肝癌患者中的85%至90%。手术切除和肝脏移植是目前公认的两种主要的肝癌治疗方式。然而,只有少数身体条件允许的患者才能接受这两种方式的治疗[2]。因此,发展其他更适用的肝癌治疗方式迫在眉睫。
射频消融(radiofrequency ablation,RFA)是一种微创的侵入式肿瘤治疗方法,是目前大多数机构优先选择的肿瘤烧灼模式之一[3-4]。当早期肝细胞癌患者不适用于手术切除和肝脏移植治疗时,RFA是公认的最好替代方法之一[5-6]。在射频消融手术治疗过程中,一根射频电极(radiofrequency electrode,RF electrode)会被***到患者的癌变部位,射频电极会释放出强电流脉冲,组织中的带电离子受电流影响而发生震荡并产生热量引起组织温度升高。当组织中的温度到达50℃左右时,就会引起蛋白质变性,导致组织凝固型坏死,从而达到***的目的[7-8]。射频消融手术的关键之一就是如何利用图像引导射频电极正确***患者的癌变部位。超声波成像技术以其灵活性,价格低廉,实时反馈等众多优点成为目前最受欢迎的图像引导手段之一[9-11]。
为了更好地控制烧灼剂量和给患者造成最小的伤害,医生必须在射频消融过程中实时的监控烧灼区域的温度分布情况。组织内部的温度变化将会导致组织的散射介质特性改变,从而改变了超声波传输过程中的一些声学特性,例如:超声波声速,衰减系数,逆散射系数。利用这些声学特性的改变,超声波就能估计出组织内部的温度变化。基于回波时移的超声波温度成像技术就是利用温度变化导致的超声波声速和组织发生热膨胀效应来估计烧灼组织温度变化的一种超声波温度成像技术。回波时移技术因其在20到43℃的温度区间内的高精确性受到了广泛的关注。在过去的十几年内,已经有很多研究成功应用基于回波时移的超声波温度成像技术监测射频消融手术。
但是回波时移技术还存在很多的缺陷。之前已经有研究表明,当组织温度超过50℃之后,超声波声速将不会随着温度的升高而发生明显变化。对于脂肪型组织,超声波声速反而会随着组织温度的升高而降低,产生这种现象的原因在于组织的凝固型坏死给组织的声学特性带来不可逆转的变化,从而导致回波时移在组织坏死之后不能有效的估计组织内部的温度变化。在组织已经坏死的烧灼降温阶段,回波时移技术存在较大的缺陷。

发明内容
为更好地监测整个射频消融过程的组织烧灼区域温度分布,本发明提供一种超声波温度成像方法。本发明在组织升温阶段使用回波时移技术估计组织的温度分布,在停止烧灼的降温阶段,使用物理学中的热传导方程在已有的超声波温度图像的基础上,估计组织的降温过程中的组织温度变化。技术方案如下:

在组织升温阶段:
A.在烧灼之前,获取超声波射频信号r(x,z,t0),此时的超声波声速取 c0=1540m/s;
B.开始烧灼;
D.为了降低温度估计中出现的纹波效应,使用8阶低通巴特沃斯滤波器(Butterworth) 对获得的回波信号时间偏移图进行轴向和侧向滤波;
E.根据下列公式:

在组织降温阶段:
A.首先获得烧灼停止时刻的原始射频信号,记为r(x,z,tk);
B.使用基于回波时移的超声波温度成像技术获得时刻tk的组织温度分布图;
C.根据以下公式:
其中代表位置(xi,yj)在时刻tk的组织温度,l=α2(tk+1-tk),为热扩散系数,k为热传导系数,c为介质比热容,ρ为介质密度,取α2=0.586W/m·k;对于位置为(xi,yj)的像素点在时刻tk+1的温度可以通过该点以及该点周围的点在时刻tk的温度uk估计出:设计一个滑动窗使用该滑动窗扫描整个图像矩阵,估计时刻tk+1的温度图像中每一个像素点的温度值,另外,由于图像边缘的像素点使用滑动窗的时候会超出图像矩阵的索引范围,使用室内温度值T0对图像矩阵进行扩充,使图像的大小变为(m+2)×(n+2);
D.经过步骤C即可得到时刻tk+1的组织温度分布;然后对该图像重复步骤B即可得到下一时刻的组织温度分布图像。

具体实施方式
下面分几个方面对本发明进行说明。
超声波温度成像
温度变化对于超声波回波的影响早在19世纪80年代就被提出且被广泛讨论,主要的影响包括回波信号的强度以及回波信号的时间偏移。在本研究中,我们主要讨论该技术的原理和假设。1996年,Roberto Maass-Moreno认为温度变化造成的超声波回音时间偏移主要原因来自于两个物理变化:一个是温度变化造成超声波声速的改变;另一个则是温度变化造成的组织的热膨胀现象。根据分析,温度变化δT和时间延迟δt之间的关系可以被描述为:
其中,z为沿着超声波传播方向的轴向深度,α(z)为是热膨胀系数,β(z)代表了超声波声速变化与组织温度变化之间的线性关系,c0(z)为依赖于组织深度z的超声波声速。为简化分析做出如下假设:(i)随着组织温度升高,组织特性不发生改变。(ii)热膨胀系数α在组织内部都是相同的。以上两个假设在本研究中的温度范围内都满足,因此热膨胀造成的非线性影响可以被忽略,公式(1)被化简为:

其中,T(z)为温度,c0(z)与T0为初始声速与初始温度,为时间偏移对轴向的微分,k为两种效应相互作用的结果。从公式(2)和(3)可以看出,通过获取超声波回波信号时间偏移并对轴向微分,再乘以系数k,就可以得到组织的温度变化,而信号的时间偏移可以通过互相关分析法获得。
使用回波时移技术估计组织温度的主要步骤如下:
1.首先,在烧灼之前,获取超声波射频信号r(x,z,t0)。此时的超声波声速取 c0=1540m/s。
2.开始烧灼。
4.为了降低温度估计中出现的纹波效应,我们使用一个8阶的低通巴特沃斯滤波器 (Butterworth)对获得的回波信号时间偏移图进行轴向和侧向滤波。
回波时移法仍然存在着很多固有缺陷。在烧灼结束的降温阶段,由于组织的凝固型坏死会导致组织的声学特性也跟着改变。在这种情况下继续使用互相关算法分析温度变化前后的 RF信号的时间偏移将会变得十分困难。更严重的是,互相关算法对组织移动和组织中的气泡非常敏感,烧灼产生的大量气泡会使计算得到的时间偏移值存在较大误差。因此降温阶段继续使用回波时移法估计超声波温度图像将不再是一个好的选择。
2-D热传导方程估计温度分布
目前,已经有大量的研究对二维热传导方程进行讨论和求解,我们在这里仅仅讨论二维热传导方程在估计超声波温度图像上的应用。烧灼停止时,使用回波时移技术估计出此时的组织的温度分布图,假设该温度为烧灼过程中温度最大值。以图像的左顶点为原点建立直角坐标系,沿着超声探头的方向为纵轴z,与超声探头垂直的方向为横轴x。u(x,z,t)表示点 (x,z)在t时刻的温度。根据传热学中的Fourier实验定律,从时刻t1到t2流进此曲面的全部热量与在相同的时间间隔中所应吸收的热量相等。由于RFA已经结束,所以组织内并无热源,因此我们可以得到热传导的齐次方程:
有限差分法求解偏微分方程的基本思想是:把求解区域内的连续分布函数通过离散化转换成离散网格内的分立函数值,用有限个差分方程去近似模拟连续的微分方程,从而求出偏微分方程的近似解。在RFA降温阶段,假设室温恒定,通过回波时移技术获得加热结束时刻的温度分布图,温度分布图中的每一个像素点代表该位置的组织温度值,因此离散方程的个数是有限的。随着降温过程的进行,组织温度将会无限趋近于室温,初始条件和终止条件已知,因此差分方程必定有解。热传导方程的有限差分格式为:
其中m=α2(tk+1-tk)。因为人体大部分由水构成,因此取α2=0.586W/m·k。
本发明使用二维热传导方程估计RFA降温过程可分为如下几步:
A.首先获得烧灼停止时刻的原始射频信号,记为r(x,z,tk)。
B.使用基于回波时移的超声波温度成像技术获得时刻tk的组织温度分布图,大小为 m×n。
C.根据公式(7),考虑坐标为(xi,yj)的一点,该点在时刻tk+1的温度可以通过该点以及该点周围的点在时刻tk的温度uk估计出。因此我们设计了一个滑动窗使用该滑动窗扫描整个图像矩阵,估计时刻tk+1的温度图像中每一个像素点的温度值。另外,由于图像边缘的像素点使用滑动窗的时候会超出图像矩阵的索引范围,因此我们使用室内温度值T0对图像矩阵进行扩充,使图像的大小变为(m+2)×(n+2)。
D.经过步骤C即可得到时刻tk+1的组织温度分布。然后对该图像重复步骤B。
射频消融治疗前将猪肝脏样品切成适当大小,放入一个充满生理盐水的塑料盒,然后消融电极通过一个小孔***到离体肝脏内部。粘土材料被用来避免盐水溶液泄漏。超声波探头被放置在肝脏和浸泡在盐水溶液中,换能器和样品之间的距离取决于换能器的焦距,这是可调的。因此,样品可以位于在超声的扫描的焦点区域。然后超声波***打开,就可以发现电极的位置。在射频消融的过程中,超生波***的换能器不停的捕获由猪肝组织散射的逆散射信号,然后再经由换能器捕获之后,由超声波自带的软件***自动保存为二进制数据。然后我们就可以通过对这些二进制数据的处理得到超声波图像。
RFA***在默认的自动模式操作,开始在50W/min,之后自动增加10W/分钟,因为高阻抗和射频***的输出对不同的电极长度是相同的。在加热阶段(12分钟),原始的RF 数据分别从组织中获得,包括256个扫描超声逆散射信号线。采样频率和脉冲长度被设置为 30兆赫和0.7毫米。加热阶段后,射频消融***自动停止工作。然后每个电极长度(0.5,1,和1.5厘米)和5个猪肝样品(n=15)再分别做一次实验。
将所得到的超声波数据读取到Matlab程序中,然后按照上文所述的算法进行处理,然后得到最后的超声波图像,再与试验过程中的实际烧灼情况进行对比,然后验证此算法的可行性。

其中,T(z)为温度,c0(z)为依赖于组织深度z的超声波声速,为时间偏移对轴向的微分,α(z)为热膨胀系数,β(z)代表超声波声速变化与组织温度变化之间的线性关系;将D步骤的计算结果对时间偏移图进行轴向微分,再乘上系数k和即可得到组织温度分布图,大小为m×n;
在组织降温阶段:
A.首先获得烧灼停止时刻的原始射频信号,记为r(x,z,th);
B.使用基于回波时移的超声波温度成像技术获得时刻th的组织温度分布图;
C.根据以下公式:
其中代表位置(xi,yj)在时刻th的组织温度,l=α2(th+1-th),为热扩散系数,q为热传导系数,c为介质比热容,ρ为介质密度,取q=0.586,单位:W/m·K,对于位置为(xi,yj)的像素点在时刻th+1的温度可以通过该点以及该点周围的点在时刻th的温度uh估计出:设计一个滑动窗使用该滑动窗扫描整个图像矩阵,估计时刻th+1的温度图像中每一个像素点的温度值,另外,由于图像边缘的像素点使用滑动窗的时候会超出图像矩阵的索引范围,使用室内温度值T0对图像矩阵进行扩充,使图像的大小变为(m+2)×(n+2);
D.经过组织降温阶段步骤C即可得到时刻th+1的组织温度分布;然后对该图像重复组织降温阶段步骤B即可得到下一时刻的组织温度分布图像。
