背景技术
本公开内容涉及用于操纵缝线的外科手术器械。特别地,本公开内容涉及用于使缝线穿过组织的器械。
在许多外科手术中,缝线用于使伤口闭合并且可以用于修复对韧带和软组织的损伤。作为修复的一部分,缝线可以被引导通过组织以便将组织缝合或保持在一起,或者用于捕获组织并将组织锚定至外科植入物(诸如,缝线锚定器)的目的。用于传递缝线的已知器械通常包括刺穿部分或针(其可以是弧形的),以及用于将缝线保持在针的一部分内以便能够实现在修复手术期间操纵缝线并使其穿过组织。
在缝线穿引针是中空的示例中,在组织捕获中可能产生潜在的问题,其中,在使用期间,针的开口的最远端可能钩破并撕裂组织。此外,由于针的开口的存在,使用者可能感受到刺穿的阻力。另外,并且也许更成问题的是,在针的远端处具有开口可能使器械变弱并且导致针尖的弯曲和可能的器械失效。
替代的器械包括可滑动地容纳在针的管腔内的缝线穿引器和缝线取回器。例如,缝线穿引器可以利用预记忆成型的软线(以镍钛合金带的形式)用于捕获缝线。然而,当被用于捕获缝线时,这样的器械的操作可能需要高的精确度。可以通过器械手柄来控制该软线。
另外的器械可以使用从远端针延伸并且能够用于捕获已经通过缝线穿引器前进的缝线的线圈,该线圈从该器械的针中的第二开口离开该器械。穿引器可以具有被修改的远端以便在缝线已被捕获在取回器的圈中之后使缝线容易地前进和脱离。可以相对于该器械纵向地并转动地操纵穿引器和取回器,使得刺穿通过待缝合的组织的针能够穿过缝线取回器。使器械缩回将引起缝线的一端被反向拉拽通过组织。当用这个装置捕获并取回缝线时,缝线被固定在该装置的远端处。
与这些器械的至少一些相关联的一个问题是,当缝线容纳在器械内时,这些器械不允许缝线在装置内滑动。缝线滑动对于一些其它类型的缝线穿引装置是个优点。对于不允许缝线滑动的那些器械而言,保持被捕获的缝线的方法是通过向下夹住缝线,这导致损伤缝线的增加的危险。此外,由于这些更复杂的器械所需的大的外形,所以在缝线的通过期间组织被钩破和撕裂也是问题。
如果克服了这些障碍,那么更方便的关节镜方法可以具有显著的优势。特别地,提供这样的低外形的装置是期望的:该装置允许被捕获缝线在器械内滑动而不钩破组织并且导致降低的组织损伤。因此,对于更好的关节镜方法存在需求。
发明内容
本公开内容通过提供一种外科手术器械而设法克服上述问题中的至少一些,该手术器械具有捕获、传递、取回缝线以及使缝线在关节内穿过的能力,同时还允许保持在器械内的缝线滑动。该器械还提供用于刺穿的单点,并且所有其它组织和/或软骨接合表面是平滑的。而且,远端刺穿尖端和相应的组织刺穿面可以是非空心的,因此提供可重复且不间断的刺穿。
根据本公开内容,外科手术器械被提供,在一个示例实施例中,该外科手术器械可以包括:具有管状构件的轴组件,该管状构件带有近端和远端;以及尖端(例如,在管状构件远端的针尖)。有利地,轴组件可以进一步限定管腔,例如在近端和远端之间延伸的管腔。在一些实施例中,该轴可以包括远端部分,该远端部分在其侧壁(例如,靠近尖端的侧壁)中具有开口。在示例实施例中,该开口可以通过侧通道与管腔连通。该外科手术器械可以进一步包括手柄部件(例如,连接至轴的近端)和缝线圈套器,该圈套器可滑动地容纳在管腔和侧通道的内部,并且可在延伸位置和缩回位置之间移动以用于捕获缝线。

在一些实施例中,由针尖的渐细部分限定的针尖轴线可以基本上顺着轴在轴向上定向,例如顺着轴的近端直的部分或者轴的远端弧形部分定向。在其它实施例中,由针尖的外边缘限定的外针轴线可以基本上顺着轴在轴向上定向,例如顺着由轴的近端直的区域限定的轴线定向,或者顺着由轴的远端弧形区域(例如,在该区域的远端处)限定的轴线定向。在一些实施例中,由针的渐细部分限定的针尖轴线可以基本上相对于轴的外直径(例如,轴的弧形远端区域的远端的外侧直径)对准。在其它实施例中,由针的渐细部分限定的针尖轴线可以基本上相对于轴的内直径(例如,轴的弧形远端区域的远端的内侧直径)对准。优选地,侧通道直径小于管腔直径。适合地,管腔直径沿其长度是恒定的。替代地,管腔具有阶梯形直径,并且包括具有不同直径的两个或更多个区段。优选地,该阶梯形直径向远端减小。
以上特征促使当处于延伸位置中时圈套器与轴成角度地突出。这允许外科医生掠过宽的面积从而在外科手术位置内捕获缝线,从而降低用于操作该器械的技术要求。此外,所形成的圈套器突出轨迹有助于降低缝线被夹住或陷住的危险,并且在每次使用圈套器时提供更可靠的延伸。圈套器可以以钩的几何结构而形成。这个钩结构可以允许在向装置的针尖和自身轴线最小地移动的情况下将缝线捕获并拉回至针中。常规的装置要求更精确地操纵针尖,使得缝线需要被容纳在缝线穿引器的口部内,从而将缝线闭合且保持在装置中。采用钩的构造的圈套器允许在沿着钩的缩回路径的任意位置处捕获缝线,并且提供用于缝线的大得多的捕获区域。因此,当刺穿通过脆弱的组织时,将不要求使用者过度地平移装置从而捕获缝线,因为该钩具有用于缝线捕获的较大的掠过面积。
在示例实施例中,管腔可连接至轴的侧壁中的凹槽(slot),该凹槽通至针尖的凹陷部分中。管腔可以有利地在该凹陷的远端部分终止,并且不延伸通过针尖的远端。在示例性实施例中,凹槽和/或凹陷的侧壁可以使得圈套器能够适当地安置并延伸,诸如以便当圈套器在延伸位置和缩回位置之间移动时防止该圈套器转动(例如,通过偏压圈套器以便保持在顺着其预弯曲部的平面中)。
在一些实施例中,圈套器的至少一部分的以及管腔和侧通道中的至少一个或者凹槽/凹陷可以包括互补的非圆形横截面,以便当圈套器在延伸位置和缩回位置之间移动时防止该圈套器转动。替代地,管腔和侧通道或凹槽/凹陷可以包括与圈套器互补的圆形和/或非圆形横截面的任意组合。因此,由于在装置内部的约束可以维持圈套器的对准,并且在使用期间圈套器不会不经意地转动。
使器械内的圈套器的相对角度位置固定可以在缝线的操纵期间提供更稳定的器械,并且允许使用相同的器械重复该技术而没有任何明显的性能损失。由于这个结构的结果,保持在器械内的缝线是更加牢固的,并且由于圈套器不能转动,所以器械使缝线掉落的危险被极大地降低。
在未示出的替代实施例中,圈套器可以在顺时针或逆时针方向上围绕器械轴线转动。优选地,该转动被限制在大约60度,尽管可能期望转动角度大于或小于60度。圈套器的转动可以由拇指致动器控制,该拇指致动器可以加入这样的结构:该结构允许通过使用者的拇指操作而使圈套器在顺时针或逆时针方向上转动。转动手柄中的拇指致动器然后将会以直观的方式使圈套器转动。替代地,可以将拇指致动器划分成左侧部分和右侧部分,该左侧部分和右侧部分能够取决于哪一侧的拇指滑动件被致动而使圈套器逆时针或顺时针转动。一起移动致动器的两侧可以使圈套器延伸而不转动。
优选地,圈套器包括远端的钩。适合地,器械尖端包括邻近于开口并且在开口远端的凹陷。优选地,当圈套器处于缩回位置时,该凹陷容纳圈套器钩。这个结构减小器械的外形并且有助于最小化在缝线穿过期间把器械推过组织时引起的任何损伤。
优选地,尖端基本上是实心的。这个结构沿刺穿边缘不提供开放腔体,并且因此有助于防止组织钩破、不需要的组织去除以及在外科手术位置中的任何相关联的组织损伤。此外,实心的尖端为器械的远端区域提供强度和刚性,从而降低器械失效的可能性并且导致更好的组织刺穿能力。
优选地,当圈套器处于完全缩回位置时,该器械包括横向延伸通过尖端的通路,该通路形成在圈套器钩和尖端的凹陷之间。当圈套器处于完全缩回位置时,该通路允许在器械中捕获的缝线滑动。当执行某些外科手术步骤时,被捕获的缝线的滑动能力消除了对额外器械的需求。因此,该器械在单个器械内提供捕获、传递和取回缝线的能力。这允许该器械被保持得小且简单(通常只需要圈套器的完全延伸位置和完全缩回位置),并且通过同一装置获得全部功能。
适合地,圈套器由不锈钢、镍钛合金、塑料、聚合物或前述的任意组合构成。优选地,圈套器由不锈钢构成。更优选地,圈套器由302类型的不锈钢构成。从强度的角度来看不锈钢的使用是理想的,并且有助于最小化圈套器失效的危险。因此,当在多次延伸/缩回循环期间,圈套器可以可靠地且持续地展开,并且降低圈套器在器械内变弯的危险。可选地,可以以适于特定应用的许多方式对钢或镍钛合金进行回火或热处理。可选地,圈套器由具有形状记忆特性的材料构成。在其中圈套器在展开和缩回期间变形的器械中,通过管腔和侧通道的变形路径被优化,使得圈套器的完整性和强度被保持,使得该器械能够在具体手术期间多次使用。优选地,圈套器可以作为弹簧使用,因为当处于打开位置时该圈套器能够完全地延伸,但随后当缩回至闭合位置中时与内管腔的形状一致。适合地,这个弹簧特性可以使得圈套器能够闭合至针尖中或促进圈套器闭合至针尖中。

参照图1和图2,示出有缝线穿引器10,该缝线穿引器具有从手柄12向远端延伸的细长轴11。轴11包括弧形的远端区域13和尖的针尖14。在图示的实施例中,该弧形与轴的近端区域成大约30度(例如,24度)的角度并且向上定向。在替代的器械中可以采用其它角度。在未图示的替代实施例中,该远端区域是弯曲的而不是弧形的,或者可以包括弯曲区域和弧形区域。在未图示的另外的替代实施例中,该轴和远端区域是直的。
手柄12是内嵌式手柄。在一些实施例中,手柄12可以包括用于容纳使用者的手指的开口17。在替代的实施例中,手柄不包括这样的开口,并且使用者的手指仅仅贴合在手柄周围。手柄12还可以包括形式为拇指激活的滑动件的致动器16,该滑动件可以远离手柄12向远端移动以激活器械。在未图示的一些实施例中,可以通过适合的机构(例如弹簧)偏压该致动器,以便当去除所施加的力时(例如,当使用者去除来自手指或拇指的压力时)默认至特定位置。在未图示的替代实施例中,手柄可以是枪柄式手柄。
现在参照图3A和图3B,缝线穿引器包括缝线圈套器15,该缝线圈套器可滑动地容纳在管状轴11的管腔内并且可从该管腔延伸。根据这个结构并且如在图3B中所示,当圈套器15借助于致动器16向远端前进时,如将在下面更详细地描述,圈套器15的一部分从管状构件中突出并且能够用于捕获缝线(未图示)。然后,圈套器能够缩回至在图3A中所示的位置(也参见图4)。在致动器被偏压的实施例中,取决于偏压件的相对定向,可以通过去除使用者的手指或拇指的力而实现圈套器的缩回。
轴11由基本上实心的针尖14以及从实心尖端向近端延伸至该轴的近端的一个或多个同轴的管状构件形成(参见图6)。这个结构使用了封闭的远端尖端,该尖端防止缝线陷入针的端部并且提供针的刺穿部分的强度。实心的尖端还呈现出不间断的刺穿边缘,并且有助于防止钩破和撕裂以及组织从手术位置的不期望的去除(图5A和图5B)。在图示的实施例中,针尖14被焊接至管状构件的远端以便形成轴11。替代地,针尖14可以与轴的管状部整体地形成,或者通过诸如压接、型锻或压溃(crushing)近端管的端部的方式附接以便形成实心的远端尖端。
如在图6和图7中所描绘的,轴11包括在靠近针尖14的轴的侧壁中的开口(未图示),该开口通过侧通道23与轴的管腔连通。在图示的实施例中,侧通道23以与管腔的轴线成一角度而弯曲(在示例实施例中,该侧通道可以相对于针尖的轴线处于大致30度的角度,或者相对于管腔的轴线处于大致150度的角度)。在未图示的替代实施例中,该通道是弧形的或者可以包括一个或多个弯曲部和/或弧形区段。因此,管状构件和侧通道提供通过器械的通路,该通路从器械轴的近端延伸至该开口。
如在图3A和图3B中所示,针尖14被定向至器械轴的弧形远端区域13的外直径。在未图示的替代实施例中,尖端可以顺着轴在轴向上定向,或者朝向内直径定向。形成作为基本上实心部件的尖端包括凹陷或口部20,该凹陷或口部环绕开口并且提供通向器械的弧形远端区域中的侧通道的入口。
如上所述,缝线穿引器10包括容纳在轴的管腔内的缝线圈套器15(参见图7A和图7B),该缝线圈套器从轴的管腔中可滑动地延伸,如在图3B中所描绘。圈套器15的远端包括在使用中用于捕获缝线的钩21,以及靠近钩21的弯折区域22。如在图3A中所示,该弯折区域具有与通道23的形状基本上互补的形状,并且该形状允许当圈套器处于完全缩回位置中时,钩21在凹陷20内紧密地配合。当圈套器被展开并且被移动到延伸位置时,该弯折区域可以有利地导致被捕获在圈套器内的缝线被推离器械的轴。图3A还示出的是,当圈套器15处在完全缩回位置中时,横向通路24创建在钩21和针尖14的凹陷20之间。在使用中,当圈套器被相应地定位时,横向通路24允许被捕获在器械中的缝线滑动。为了给外科医生提供对于缝线可以滑动的速率的控制度,可以期望的是,当缝线被捕获在通路24内时缝线经受少量的阻碍或阻力。被捕获的缝线的滑动能力是有利的,因为该能力去除了在执行某些手术步骤时对额外的仪器的需求。因此,该器械为外科医生提供捕获、传递和取回缝线的能力,而全部不需要更换仪器,该能力在关节镜手术中是特别有利的。因此,该器械能够被保持得小且相对简单,因为仅有用于圈套器的两个位置是必需的:完全延伸的位置和完全缩回的位置。
如将理解的,各种针的凹陷的几何结构可以被设计成与钩配合,并且这些几何结构可以被用于控制当缝线滑动时存在的阻力的程度。此外,钩的表面、凹槽的表面或者凹槽和钩两者的表面可以被纹理化以便提供对缝线的阻碍。替代地,相应表面可以被高度抛光或者可以用降低摩擦力的材料涂覆相应表面以增强缝线滑动。另外,相应表面的一个或多个可以包括涂层(诸如,PTFE或者硅基润滑剂)以便促进圈套器在器械的轴内的移动。
在未图示的另外的替代实施例中,器械包括额外的缩回位置,在该位置中,缝线被保持紧固并且不能滑动:这样的实施例提供多位置的致动器机构,该机构允许打开位置、缝线滑动位置和闭合位置。
现在参照图7B,通道23具有比管腔的直径θ2 小的直径θ1,并且圈套器15的尺寸被设计成对于通道23具有狭窄的公差(例如,小于20%的公差,更理想地小于15%,并且最理想地小于10%)。在通道23和圈套器15之间的狭窄的公差使得圈套器以期望的轨迹突出远离轴11。因此,这极大地降低了所经历的任何“楔”效应(wedge effect)的严重程度(由于该楔效应缝线可能会陷于圈套器和器械的轴之间)。该楔效应通常发生在当缝线已经被捕获在器械内并且圈套器处于缩回位置时。该楔效应可以发生在组织刺穿之前或之后,并且当圈套器延伸且圈套器与针尖之间的距离太小以至于不能容纳缝线时,并且因此缝线可能会在器械中楔住或塞住(cleated)。楔效应也可以发生在穿过组织期间,当缝线已经被捕获且圈套器处于缩回位置时。在这个情形中,在器械经过组织时,在与刺穿方向相对的方向上张力被施加在缝线上。上面关于图7B描述的结构有助于降低这些效应,从而允许当圈套器延伸时缝线被容易地释放。此外,这个结构通过增大其中能够套住缝线的有效面积而使器械更易于使用。
在图示的实施例中,管腔和侧通道具有圆形的横截面。然而,在替代实施例中,期望的是当圈套器在延伸位置和缩回位置之间移动时可靠地防止圈套器相对于器械的轴的转动。因此,管腔或侧通道(或两者)的至少一部分,以及圈套器的至少一部分形成有互补的非圆形横截面,以便在圈套器的展开期间防止圈套器的转动。
如上所述,圈套器15的直径略小于侧通道23的直径θ1,以便提供在通道内的紧密配合。然而,因为管腔的直径θ2较大,所以可能存在圈套器在高负载下在管腔内变弯的倾向。为了防止这种情况,管腔能够形成有较小的直径。替代地,圈套器能够形成有阶梯形直径,该直径朝向器械的近端较大并且朝向器械的远端以一个或多个阶梯使直径减小。这些不同的结构每一个提供在管腔内的各器械部件之间的更紧密的配合,以便降低圈套器在负载下变弯的可能性。
在图8至图11中描述了上述问题的另外的替代解决方案。如图10中最清楚地示出,圈套器套管25在圈套器15之上滑动并且在远端和近端压接位置30和31处压接至圈套器15。这个特定的组装形式免于对焊接的需求,并且允许使用压接操作的洁净室组装。简略地,可以通过首先在近端***圈套器经过轴、经过远端开口而组装器械。随后,使圈套器套管在圈套器的近端之上并且从器械的近端开始向远端滑动。然后,在图9至图11中所示的近端和远端位置被压接至一起之前,圈套器和圈套器套管适当地对准。对于远端压接位置,孔30被设置在器械的轴中,使得适合的压接器械能够接合该孔以便提供必需的挤压压接力。对于近端压接位置,可以在拇指致动器区域中设置合适的孔,或者能够在将手柄附接至该装置之前完成压接。如将理解的,这只是可以组装并压接器械部件的一种方式,并且其它可能性将被理解。
此外,该器械包括人体工程学手柄,该手柄被形成为适合外科医生的手的抓握的形状。如上所述,手柄中的拇指致动器位于易于使用的位置。可以通过向前推按钮而使圈套器的钩延伸,并且通过拉回该按钮而使圈套器缩回。这些动作是直观且简单的以便使用者执行。
参照图12,其描绘了缝线穿引器器械的远端的另外的示例实施例。该示例实施例图示了宽公差的侧通道23和具有预弯曲部的缝线圈套器15的使用。特别地,例如与在图7B中所示的实施例相反,图12中的侧通道23的直径θ1显著地大于缝线圈套器15的直径。例如,在一些实施例中,侧通道的直径θ1可以是缝线圈套器的直径的1.25、1.5、1.75、2、或2.5倍。在一些实施例中,侧通道的直径θ1可以是大致0.02至0.04英寸,缝线圈套器的直径可以是大致0.01至0.02英寸。在示例实施例中,侧通道23的直径θ1可以被构造成与缝线穿引器器械的轴11的管腔的直径大致相同。一般来说,侧通道23在直径θ1上的宽公差可以有利地允许用于缝线圈套器15的行进和变形的较不曲折的路径,例如,由此减小摩擦力和磨损。特别地,可以涂敷润滑涂层(诸如,硅基液体)或应用诸如ME-92的表面硬化技术来进一步减小磨损。关于缝线圈套器15的预弯曲部的使用有利地增加了圈套器15相对于轴11和缝线穿引器器械的针尖14的冲出(kick-out)间距。
在示例实施例中,侧通道23的直径θ1可以被构造成小于由缝线圈套器的弯折区域22确定的缝线圈套器15的钩端21的偏移距离D(例如,是偏移距离D的例如0.9、0.8、0.7、0.6、或0.5倍)。以此方式,圈套器15的弯折区域22(通常可具有与侧通道23相同的长度)可以使得钩端21枢转(例如,在顺着弯折区域22和直径θ1的平面中),以便当圈套器15处于缩回位置时允许钩端21牢固地装配在针尖14的凹陷20内(例如,参见图3A的实施例)。在一些实施例中,缝线穿引器器械(例如,侧通道23和/或圈套器15)可以被构造成使得在圈套器15的缩回期间,在弯折区域22进入侧通道23之前,钩端21的偏移区域21a(例如,偏移的细长区域)基本上平行于侧通道23的轴线Ac。而且,在一些实施例中,缝线穿引器器械(例如,侧通道23和/或圈套器15)可以被构造成使得当圈套器15处于缩回位置中并且弯折区域22缩回至侧通道23中时,钩端21的偏移区域21a枢转至基本上平行于轴11的轴线As(诸如,由轴的近端直区域限定的轴线、或者由轴的远端弧形区域(例如,在该区域远端处)限定的轴线)。

仍然参照图12,在示例实施例中,侧通道23可以具有以宽公差为特征的侧通道23的第一直径θ1,以及以窄公差为特征的垂直于该第一直径的第二直径。以此方式,侧通道23可以防止/阻止缝线圈套器15在侧通道中转动,例如,通过偏压圈套器15的预弯曲部以便使其保持在平面中(例如,顺着针尖的外边缘14a并且垂直于针尖14的渐细平面14b的平面)。
现在参照图13,其描绘了缝线穿引器器械的远端的另外的示例实施例。该示例实施例再次描绘了宽公差的侧通道23和具有预弯曲部的缝线圈套器15的使用。图13的实施例有利地图示了使缝线穿引器器械的轴11的弧形远端区域与圈套器15的预弯曲部以及侧通道23对准。特别地,轴11被构造成使得弧形远端区域在与圈套器15中的预弯曲部的方向以及侧通道23的分叉方向相同的方向上成弧形。轴11的弧形远端区域与圈套器的预弯曲部以及侧通道23的这个对准有利地进一步减小磨损,例如,相对于在图7A、图7B和图12中所示的实施例。

如在图20D中所描绘的,类似的塞住问题可能由于离开点太靠远端而导致(例如,接近凹陷20的远端唇部20a)和/或由于圈套器15没有相对于凹陷20的槽提供充分的角度或间距而导致。因此,例如,可以包括偏压元件23d(例如,楔形物),以用于改善间距并且防止离开点太靠远端。图20D中的塞住问题可能发生在缝线被容纳在装置中并且该装置在钩处于闭合位置中的情况下经过组织之后。当被刺穿的组织使残余近端张力保持在缝线上并且圈套器延伸至打开位置时,圈套器的预弯曲部能够压过(ride over)缝线自身(因为由组织施加的近端张力使缝线不能向远端移动)。此时,缝线将可能在圈套器的预弯曲部和针尖的凹槽之间被塞住。
图21关于图19A-图19C的实施例图示了凹槽23a的宽度D以及凹陷20的侧壁的宽度(具体地在凹陷20的近端唇部20b处)可以被如何构造以便相对于圈套器15展现出窄的公差,由此防止/阻止缝线圈套器的转动(例如,通过偏压圈套器15的预弯曲部使其保持在顺着凹陷20和凹槽23a的壁的平面中)。在示例实施例中,凹槽和/或凹陷的侧壁可以基本上彼此平行。

Claims (18)

1. 一种外科手术器械,包括:
轴,所述轴包括具有近端和远端的管状构件,在所述近端和远端之间延伸的管腔,以及在所述管状构件的远端的尖端;以及
连接至所述轴的所述近端的手柄;
其中,所述轴包括远端部分,所述远端部分在其靠近所述尖端的侧壁中具有开口,所述开口通过侧通道与所述管腔连通;以及
其中,所述器械进一步包括缝线圈套器,所述圈套器可滑动地容纳在所述管腔和侧通道内,并且可在延伸位置和缩回位置之间移动以用于捕获缝线。
2.根据权利要求1所述的器械,其中,所述侧通道相对于管腔轴线成角度。
3.根据权利要求1或2所述的器械,其中,所述侧通道包括至少一个弯曲部。
4.根据权利要求1或2所述的器械,其中,所述侧通道是弧形的,或者所述侧通道是弧形的并且包括至少一个弯曲部。
5.根据权利要求1至4的任一项所述的器械,其中,所述轴的所述远端部分是弧形的。
6.根据权利要求1至4的任一项所述的器械,其中,所述轴的所述远端部分是弯曲的,或者包括至少一个弯曲部分和弧形部分。
7.根据权利要求5或6所述的器械,其中,所述开口位于所述远端部分的外直径上。
8.根据权利要求5或6所述的器械,其中,所述开口位于所述远端部分的内直径上。
9.根据前述权利要求的任一项所述的器械,其中,所述侧通道和管腔每一个具有一直径,并且所述侧通道直径小于所述管腔直径。
10.根据权利要求9所述的器械,其中,所述管腔直径沿所述管腔的长度是恒定的。
11.根据权利要求9所述的器械,其中,所述管腔具有阶梯形直径并且包括具有不同直径的两个或更多个区段;优选地,所述阶梯形直径向远端减小。
12.根据权利要求1至11的任一项所述的器械,其中,所述管腔和侧通道中的至少一个以及所述圈套器的至少一部分包括互补的非圆形横截面,以便当所述圈套器在延伸位置和缩回位置之间移动时,防止所述圈套器转动。
