Abstract

本发明涉及用于在哺乳动物组织内持续释放蛋白质的生物可降解的植入物。所述植入物被配制为提供体内呈生物活性形式的蛋白质例如抗体的持续释放至少1个月(30天或更久)。所述植入物用于治疗影响眼前区或后区的眼部病状(包括眼病)以及影响整个身体的区域和组织的医学病状。特定实例包括配制用于眼内和关节内施用的植入物。还描述了制备缓释的含蛋白质的植入物的方法。
背景
人们对开发可提供蛋白质长期释放的可注射、生物相容性组合物有极大兴趣。特别有利的是,开发以受控速率释放治疗水平的生物活性蛋白1、2或3个月或更久的生物相容性且生物可降解的组合物。
能够长期递送治疗水平的功能性蛋白的生物可降解的组合物,例如眼内植入物,可对治疗眼病极其有用,其中使用蛋白质(例如抗体)通常需要频繁的眼内注射或高剂量全身给药。
除与频繁注射相关的不适和时间外,眼内直接注射可涉及对患者的某些风险,包括视网膜脱离、晶状体损伤和感染。由于晶状体和其它眼内组织处蛋白质药物的高脉冲浓度,因此眼内直接注射还可能导致局部毒性。另外,全身施用的蛋白质向视网膜的渗透受血视网膜屏障(BRB)严重限制。
通常化合物通过扩散到小带后间隙,经由眼房水清除,或通过反式视黄醛消除,从眼玻璃体消除。大多数高分子量化合物利用前一种途径,而亲脂性化合物和那些具有反式视黄醛运输机制的化合物将利用后一种途径。不幸的是,通过视网膜消除的化合物的半衰期极短。因此,对于这些化合物而言,难以通过眼内直接注射维持治疗浓度。将需要频繁注射。即使对于经由眼房水清除的大分子,例如蛋白质而言,玻璃体半衰期相对于治疗持续时间也较短。因此,化合物例如必须频繁地如每月一次通过玻璃体内注射给药。
因此通过开发可呈单剂量提供安全、有效且长期释放的生物活性蛋白的生物可降解的植入物,同时避免与脉冲给药相关的瞬时高浓度,以避免对频繁注射蛋白质的需要,这对于患者而言具有很大价值。
概述
因此,本发明的一些实施方案提供了生物可降解的药物递送***(DDS),其可在体内释放治疗有效量的生物活性蛋白1个月或更久,2个月或更久,或3个月或更久(即,90天或更久)。
生物可降解的药物递送***可包含生物可降解的聚合物基质和与生物可降解的聚合物基质缔合的蛋白质,由或基本上由生物可降解的聚合物基质和与生物可降解的聚合物基质缔合的蛋白质组成。本发明范围内药物递送***的实例包括挤出长丝(即,通过挤出法形成的植入物)和压制片。
例如,生物可降解的药物递送***可呈通过单或双挤出法制备的挤出长丝的形式。挤出长丝可被配制和定尺寸以置于哺乳动物眼内,并且更特别地置于眼的眼部区域内。这种长丝可被称为“眼内植入物”、“生物可降解的眼内植入物”或更特别地称为“挤出的生物可降解的眼内植入物”。挤出长丝也可被配置用于植入关节内区域以治疗其疾病或医学病状。这种长丝可被称为“关节内植入物”。挤出长丝可为固体、半固体或具粘弹性。
因此,在一个实施方案中,本发明提供了包含生物可降解的聚合物基质和与生物可降解的聚合物基质缔合的蛋白质或由生物可降解的聚合物基质和与生物可降解的聚合物基质缔合的蛋白质组成的生物可降解的眼内植入物,其中在将植入物置于哺乳动物眼内后,植入物提供呈生物活性形式的蛋白质的持续释放至少30、60或90天。在一个特定实施方案中,本发明提供了包含生物可降解的聚合物基质和与生物可降解的聚合物基质缔合的蛋白质的生物可降解的眼内植入物,其中在将植入物置于哺乳动物眼内后,植入物提供呈生物活性形式的蛋白质的持续释放3个月以上。蛋白质可经生物可降解的聚合物基质封装和/或分散于生物可降解的聚合物基质中。蛋白质可均匀或不均匀地分布于聚合物基质中。植入物可包含单一蛋白质或第一蛋白质和第二蛋白质,例如针对第一蛋白质靶标和第二蛋白质靶标的第一抗体和第二抗体,由此将抗体设计为与体内第一蛋白质靶标和第二蛋白质靶标结合并阻滞其活性。一个实例是包含抗体、DARPin(经设计的锚蛋白重复蛋白),或特异性结合(换言之对其有特异性)血管内皮生长因子(VEGF)或血小板源生长因子(PDGF)的抗转运蛋白(anticalin)的植入物。例如,根据本发明所述的植入物可包含抗VEGF或抗PDGF抗体或抗VEGF抗体与抗PDGF抗体。
可包括在内并借此通过眼内或关节内植入物递送的蛋白质的非限制性实例包括单克隆和多克隆抗体、双特异性抗体、双特异性抗体、抗体片段、抗转运蛋白、DARPin和酶。其它实例包括糖蛋白和血清白蛋白。单克隆或多克隆抗体可以其例如由细胞产生时的天然形式使用,并且可能含有或不含翻译后修饰,或可以其从细胞培养物或其它生物样品中分离后产生的化学或酶修饰形式使用。抗体可为嵌合型。在特定实施方案中,所述抗体可为IgA、IgD、IgE、IgG或IgM。有用的抗体片段包括由木瓜蛋白酶(例如,Fab片段)或胃蛋白酶裂解抗体生成的片段。更通常地,有用的抗体片段包括Fab'、F(ab)2、Fabc和Fv片段。抗体片段可通过修饰全抗体产生或使用重组DNA方法重新合成而产生,并且进一步包括现通过常规技术制成的“人源化”抗体。“抗体片段”包含全长抗体的一部分,通常为其抗原结合或可变结构域。
本发明的一些实施方案提供了包含抗VEGF抗体或蛋白质(即,与血管内皮生长因子蛋白或VEGF-A特异性结合的抗体或蛋白质)的生物可降解的植入物。有用的抗VEGF抗体包括但不限于雷珠单抗和贝伐单抗有用的抗VEGF蛋白包括阿柏西普(也称为VEGF TRAP)、VEGF结合DARPin和VEGF结合抗转运蛋白。VEGF Trap(Regeneron Pharmaceuticals,New York)是与含有人抗体Fc区(C端)连接的两个不同VEGF受体的胞外结构域部分的融合蛋白。在一些实施方案中,所述植入物可包含选自由抗VEGF抗体、抗VEGF受体抗体、抗PDGF(血小板源生长因子)抗体、抗整联蛋白抗体、其治疗有效片段及其组合组成的组的抗体。
在另一实施方案中,本发明提供了包含抗TNF(肿瘤坏死因子)抗体或蛋白质的挤出的生物可降解的植入物。有用的抗TNF抗体包括但不限于阿达木单抗英夫利昔单抗赛妥珠单抗和戈利木单抗有用的抗TNF蛋白质包括融合蛋白依那西普包含抗TNF蛋白质的植入物可能对治疗葡萄膜炎和***特别有用。
根据本发明包含在眼内或关节内植入物内的其它蛋白质包括生长因子例如神经生长因子、酸性成纤维细胞生长因子和碱性成纤维细胞生长因子;神经营养因子例如睫状神经营养因子、脑源性神经营养因子和胶质细胞源性神经营养因子;细胞因子例如干扰素-γ和白细胞间素-10;抗增生性化合物例如利妥昔单抗;和纤维蛋白溶解蛋白例如组织纤溶酶原激活剂。这种蛋白质的治疗用途通常在本领域中已知。
因此,根据本发明所述的生物可降解的眼内或关节内植入物可包含选自由抗VEGF抗体(与VEGF特异性结合的抗体)、抗PDGF抗体、抗VEGF受体抗体、抗整联蛋白抗体、其治疗有效片段及其组合组成的组的抗体。
血管内皮生长因子A(VEGF-A,也称为VEGF)是对血管内皮细胞有特异性,可刺激内皮细胞体外生长和体内血管生成的分泌型有丝***原。VEGF-A可以不同的同种型存在。VEGF-A的所有同种型,除VEGF-A121外,均结合肝素。在人类中,VEGF-A最丰富的同种型为165个氨基酸的多肽,VEGF-A165。参见,例如,Houck等,Mol.Endocrin.5:1806(1991)和Leung等,Science 246:1306(1989)。
因此,本发明的一些实施方案提供了包含生物可降解的聚合物基质和与VEGF-A165结合的抗体、DARPin或抗转运蛋白的生物可降解的植入物。相同抗体、DARPin或抗转运蛋白可识别并结合VEGF-A的所有同种型,因为所述抗体、DARPin或抗转运蛋白可识别存在于VEGF-A所有同种型中的表位。例如,所述抗体、DARPin或抗转运蛋白可识别并特异性结合VEGF-A所有同种型(包括VEGF121)中存在的VEGF-A受体结合结构域区域内的表位。因此,本发明植入物内的单一抗VEGF抗体(或DARPin或抗转运蛋白)可识别并特异性结合VEGF-A的所有同种型。已经描述了与VEGF-A或VEGF受体结合的抗体,包括单克隆抗体和人源化抗VEGF抗体。参见,例如,美国专利5,955,311和美国专利6,884,879。贝伐单抗是特异性结合并抑制人VEGF-A的单克隆抗体的一个实例。还已经描述了针对PDGF的抑制性抗体。参见,例如,WO 2003/025019。
包含VEGF(血管内皮生长因子)或血小板源生长因子(PDGF)抑制剂或二者(例如特异性结合体内VEGF和/或PDGF的抗体或抗体组合)的缓释生物可降解的植入物,可能对治疗有需要的患者体内的黄斑变性(包括湿性年龄相关性黄斑变性)、视网膜病变、糖尿病性视网膜病变、增生性糖尿病性视网膜病变、镰状红细胞性视网膜病变、早产儿视网膜病变、缺血性视网膜病变、眼新血管形成(眼内新血管异常生长)、脉络膜新血管形成、由于视网膜静脉内阻塞导致的新血管形成、角膜新血管形成、糖尿病性视网膜缺血和黄斑水肿特别有用。
在一个实施方案中,本发明提供了包含生物可降解的聚合物基质及第一抗体和第二抗体,或分别特异性结合VEGF和PDGF的双特异性抗体的生物可降解的眼内植入物,其中在将植入物置于哺乳动物眼内之后,植入物提供呈生物活性形式的第一抗体和第二抗体或双特异性抗体的连续释放至少30、60或90天。在一种形式中,抗体或双特异性抗体对血管内皮生长因子-A(VEGF-A)和血小板源生长因子-B(PDGF-B)有特异性。这些植入物可用于治疗眼部肿瘤、眼新血管形成、脉络膜新血管形成和黄斑变性。用“PDGF-B”指PDGF的B链多肽。
该植入物被设计为维持所述抗体的生物活性,以致当所述抗体从***释放时,所述抗体将特异性结合其指定蛋白质靶标。抗体与蛋白质靶标的结合可在蛋白质与其配体或受体之间提供干扰,因此蛋白质/受体相互作用介导的功能可受所述抗体抑制或降低。在本领域中已知几种测定抗体是否与蛋白质(多肽)靶标特异性结合或“具有免疫反应性”的方法。免疫化学发光定量测定(ICMA)、酶联免疫吸附测定(ELISA)和放射免疫测定(RIA)为一些实例。
在一些实施方案中,本发明提供了包含生物可降解的聚合物基质和与VEGF或血小板源生长因子(PDFG)或VEGF和PDFG相互作用(例如,结合并且减小或抑制其活性)的一种或多种单克隆抗体、双特异性抗体、DARPin、抗转运蛋白、抗体片段、源自抗体可变区的重组多肽或其混合物的挤出的生物可降解的植入物。例如,本发明提供了包含生物可降解的聚合物基质和特异性结合VEGF和PDFG的双特异性抗体的挤出的生物可降解的植入物,其中在将所述植入物置于哺乳动物眼内后,所述植入物提供呈生物活性形式的抗体的连续释放至少90天。在其它实施方案中,根据本发明所述的植入物可包含生物可降解的聚合物基质和对VEGF受体有特异性的抗体。
可使用本领域中普通技术人员已知的常规方法获得用于该药物递送制剂中的单克隆抗体。简言之,向动物例如小鼠注射所需靶蛋白或其部分(抗原),例如VEGF或VEGFR。靶蛋白优选与载体蛋白偶联。向动物加强注射一次或多次靶蛋白,并且在融合前3天通过静脉内(IV)加强剂量导致超免疫。从所述小鼠分离脾细胞并且通过标准方法使其与骨髓瘤细胞融合。可在标准次黄嘌呤/氨基蝶呤/胸腺嘧啶(HAT)培养基中,根据标准方法选择杂交瘤细胞。使用标准免疫技术鉴定、培养和亚克隆分泌识别靶蛋白的抗体的杂交瘤细胞,并且例如通过亲和色谱法纯化抗体。在本递送***的某些实施方案中,抗VEGF或抗VEGFR单克隆抗体从ImClone Systems,Inc.(NY,NY)得到。例如,本制剂可包含可从ImClone Systems获得的名称为IMC-18Fl的抗体,或名称为IMC-1121Fab的抗体。可用于本药物制剂中的另一种抗VEGF抗体片段为雷珠单抗,结合VEGF-A的Fab片段。用于本药物递送***的另一种抗VEGF抗体为贝伐单抗,结合VEGF-A的单克隆抗体。
在一些实施方案中,药物递送***(例如,眼内或关节内植入物)中的蛋白质长度可为至少20个、至少30个、至少50个、至少100个、至少200个或至少300个氨基酸。在一些实施方案中,所述蛋白质包含3个或更多个氨基酸。在一些情况下,所述蛋白质长度可为30-50个氨基酸或长度可为100-500个氨基酸。在本发明的一些形式中,所述蛋白质的分子量小于5千道尔顿(kDa)。在其它形式中,所述蛋白质的分子量大于5kDa、大于10kDa、大于20kDa、大于50kDa或大于100kDa。在本发明的某些形式中,所述蛋白质的分子量为10-30kDa或20-50kDa。例如,所述蛋白质的分子量为14-16kDa,或14-21kDa。所述蛋白质可为直链、支链或环状,并且可经化学合成(例如,使用固相合成)或者天然或重组产生。所述蛋白质可为融合蛋白。所述蛋白质可能相对于其自然存在的形式截短。所述蛋白质可包含不超过一条氨基酸链或两条或更多条氨基酸链。所述两条或更多条链可相互共价或非共价缔合。例如,所述氨基酸链可通过二硫键缔合,或所述两条或更多条链可仅通过非共价力相互缔合。所述蛋白质可能含有或可能不含经合成或翻译后修饰的氨基酸。用于本植入物的蛋白质可经重组(通过哺乳动物或原核细胞培养)、合成(例如,如通过固相合成)产生,或从天然来源(例如,哺乳动物或细菌细胞培养物、血浆、血清、植物、真菌等)分离。所述蛋白质中的一种或多种氨基酸可能非天然存在。
根据本发明的生物可降解的眼内或关节内植入物可包含约1.0%至约50%蛋白(按植入物的重量计)(即,%w/w),约5%至约30%蛋白(按植入物的重量计),约5%至约40%蛋白(按植入物的重量计),约10%至约25%蛋白(按植入物的重量计),或约5%、10%、20%或30%蛋白(按植入物的重量计)。
除蛋白质和生物可降解的聚合物基质外,所述植入物可包含一种或多种赋形剂、盐、缓冲剂、防腐剂、聚电解质或其任何组合。
有用的赋形剂包括碳水化合物,例如海藻糖(例如,海藻糖、α,α-海藻糖-二水合物)、菊粉和蔗糖;表面活性剂,例如聚山梨醇酯20、聚山梨醇酯80和普朗尼克F127(pluronic F127);聚乙二醇,例如聚乙二醇3350(PEG 3350);氨基酸,例如甘氨酸、天冬氨酸、谷氨酸、精氨酸、赖氨酸和组氨酸;螯合剂,例如乙二胺四乙酸二钠盐脱水物;多元醇,例如丙三醇、山梨糖醇和甘露糖醇;胆固醇;白蛋白;环糊精;葡聚糖;聚乙烯醇;甘油;和氯化锌,及其混合物。赋形剂以按重量计0.01%(w/w)-30%(w/w),按重量计0.01-20%或按重量计0.01%-约15%的量存在于植入物中。
适合的水溶性缓冲剂可能包括但不限于碱和碱土碳酸盐、磷酸盐、重碳酸盐、柠檬酸盐、硼酸盐、醋酸盐、琥珀酸盐等,例如磷酸钠(例如,磷酸一钠(NaH2PO4)和磷酸二钠(NaHPO4)、柠檬酸钠、硼酸钠、醋酸钠、碳酸氢钠、碳酸钠等。这些试剂有利地以足以将***的pH维持在2和9之间,更具体地,在4和8之间的量存在。缓冲剂可以按重量计0.01%-10%(w/w),诸如例如约0.01%-5%w/w的量存在于植入物中。

适合的聚电解质包括聚精氨酸、聚组氨酸、聚赖氨酸、精蛋白、组蛋白、硫酸多粘菌素B、聚丙烯胺、聚(乙烯亚胺)、DEAE-葡聚糖、角叉菜胶、硫酸软骨素、藻酸硫酸盐、硫酸葡聚糖、肝素、聚苯乙烯磺酸盐、聚乙烯硫酸盐和聚磷酸盐。
适合的盐包括NaCl、KCl、MgCl2等。
生物可降解的聚合物基质可包含聚(D,L-丙交酯)、聚(D,L-丙交酯-共-乙交酯)(PLGA)或其组合。在一些实施方案中,所述基质包含第一和第二PLGA共聚物的混合物。第一PLGA共聚物可具有酯端基而第二PLGA共聚物可具有酸端基。更具体地,第一PLGA共聚物可为RG752S而第二PLGA共聚物可为RG502H。在一个特定实施方案中,药物递送***中(例如,在挤出植入物中)RG752S重量与RG502H重量的比例为约90:10。在另一实施方案中,第一PLGA共聚物为RG753S而第二PLGA共聚物为RG502H。在一个有用形式中,挤出植入物中RG753S重量与RG502H重量的比例为约90:10。
在一些实施方案中,植入物可进一步包含可再吸收聚合物。可再吸收聚合物为在体内溶解,但是不降解的一种聚合物。因此,可再吸收聚合物通常为水溶性。可再吸收聚合物的一个实例为聚乙二醇3350(PEG 3350)。
用于本发明的聚合物或聚合材料的一些优选特征可包括生物相容性,与治疗组分的相容性,易于使用聚合物制备本发明的药物递送***,在生理环境中的半衰期为至少约6小时优选大于约1天,玻璃体粘度未显著增加和水不溶性。
为形成基质而包括在内的生物可降解的聚合物理想地经受酶促或水解不稳定性。稳定性程度根据单体的选择、采用均聚物还是共聚物,采用聚合物混合物和聚合物是否包括末端酸性基团而广泛变化。
聚合物基质可包含生物可降解的聚合物或第一和第二聚合物的组合,或由生物可降解的聚合物或第一和第二聚合物的组合组成。除一种或多种聚合物外并且除与基质缔合的所述一种或多种蛋白质外,所述聚合物基质还可任选包含一种或多种赋形剂、盐、缓冲剂、防腐剂或聚电解质,其实例上文已做描述。
聚丙交酯或PLA包括聚(D-丙交酯)、聚(L-丙交酯)和聚(D,L-丙交酯),并且还可通过CAS编号26680-10-4鉴定,并且可用下式表示:

用于本发明目的生物可降解的植入物内蛋白质(例如抗体)的量可为用于减轻眼部或关节内病状的一种或多种症状有效的量。例如,这种量可介于约20μg和约500μg蛋白质之间或更多。而且,植入物(例如挤出植入物)将优选释放治疗有效量的蛋白质一段持续时间,可为30-90天或更久。蛋白质的治疗有效量可为约0.5μg蛋白质/天至约4μg蛋白质/天的释放速率。在一些情况下,治疗有效量可为约0.5μg蛋白质/天至约2μg蛋白质/天的释放速率。例如,植入物在其释放约2μg蛋白质/天时可提供治疗有效量的抗体。此外,植入物可提供3个月以上的疗效,这不但是由于连续释放蛋白质1、2或3个月或更久,而且是由于已将植入物置于其中的眼或组织内蛋白质的残留效应。
至少约1、2或3个月连续释放的蛋白质的量可为用于治疗眼部病状(包括眼前或眼后病状)或关节内疾病或病状的治疗上有效量。

因此,根据本发明所述的药物递送***的一个实例为包含抗体和生物可降解的聚合物基质的挤出长丝(例如,眼内或关节内植入物),其中所述聚合物基质包含第一和第二生物可降解的聚合物的混合物,并且其中从将长丝置于眼部或关节内区域时起,所述长丝释放治疗有效量的呈生物活性形式的抗体至少约1个月。在其特定实施方案中,从将长丝置于眼部或关节内区域时起,所述长丝释放治疗有效量的生物活性抗体至少约1、2个或至少约3个月。
另一实例为包含抗体和生物可降解的聚合物基质的挤出长丝,其中所述聚合物基质包含不超过一种生物可降解的聚合物,并且其中从将长丝置于眼部或关节内区域时起,所述长丝释放治疗有效量的呈生物活性形式的抗体至少约1个月。
如先前所解释,不管挤出长丝是否包含一种或一种以上的生物可降解的聚合物,然而所述长丝仍可进一步包含一种或多种可提高稳定性和/或调节抗体从长丝释放的速率的赋形剂、盐、缓冲剂、防腐剂或聚电解质。与不具所述一种或多种赋形剂、盐、缓冲剂、防腐剂或聚电解质的相同长丝相比,释放调节可表现为更呈线性的释放速率形式和/或更长释放期。有用的赋形剂、盐、缓冲剂、防腐剂或聚电解质已经在上文先前得以陈述,并且包括聚山梨醇酯20、海藻糖和磷酸钠。
在包含第一和第二生物可降解的聚合物的混合物的长丝中,第一和第二聚合物可独立地选自由酯封端聚(D,L-丙交酯-共-乙交酯)共聚物和酸封端聚(D,L-丙交酯-共-乙交酯)共聚物组成的组。在一个特定实施方案中,所述植入物中酯封端共聚物重量与酸封端共聚物重量的比例为90:10。
在包含单一生物可降解的聚合物而不含其它生物可降解的聚合物的长丝中,所述单一生物可降解的聚合物可选自由酯封端聚(D,L-丙交酯-共-乙交酯)共聚物(具有酯端基的PLGA共聚物)和酸封端聚(D,L-丙交酯-共-乙交酯)共聚物(具有酸端基的PLGA共聚物)组成的组。
在任何前述实施方案中,酯封端和/或酸封端聚(D,L-丙交酯-共-乙交酯)共聚物可独立地选自由以下组成的组:
i)具有酯端基,特性粘度为0.16-0.24dl/g(在25℃下0.1%w/v于氯仿中)并且D,L-丙交酯:乙交酯比例为约75:25的聚(D,L-丙交酯-共-乙交酯)(RG752S);

在一个实施方案中,本发明提供了包含抗体和生物可降解的聚合物基质的挤出的生物可降解的长丝(即,挤出植入物),其中所述聚合物基质包含RG753S和RG502H。在该植入物的一种形式中,所述抗体为抗VEGF抗体。
在另一实施方案中,本发明提供了包含抗体(诸如例如抗VEGF抗体)和生物可降解的聚合物基质的挤出的生物可降解的植入物,其中所述聚合物基质包含RG752S和RG502H。在该实施方案的一个特定形式中,所述植入物包含重量重量比(RG752S:RG502H)为约90:10的RG752S和RG502H,并且所述抗体为抗VEGF抗体。实例包括下面表1中描述的3号和5号制剂。
在任何前述实施方案中,所述长丝可被配置为置于眼部或关节内区域。即所述长丝可被配置为用作眼内或关节内植入物。
定径、配置并且适合置于眼部区域的挤出长丝(即,眼内植入物)可为杆状或非圆柱形并且长度为约0.5mm至约10mm。例如,所述长丝长度可为约1mm、2mm、3mm、4mm、5mm、6mm或约7mm。直径可为约250μm至约1mm,或小于约500μm。在一个实例中,被配置为置于玻璃体内的长丝长度为约1至约7mm并且直径为约0.5至约2mm。在另一实例中,被配置为置于前房内的长丝长度为约0.5至约2mm并且直径为约50μm至约500μm。这些植入物也可能适合施用至关节内区域以向有需要的患者,诸如例如关节患有炎症或自身免疫性疾病的患者,递送治疗有效量的蛋白质。选择用于这些植入物中的蛋白质将是对减轻炎症或治疗自身免疫性疾病有效的蛋白质。
眼内或关节内植入物的总重量可为约100μg至约5000μg,约1000至约2000μg,或5000μg以上。例如,挤出植入物可能重约500μg、约1000μg、约2000μg或约5000μg。
可采用各种技术生产生物可降解的植入物。有用的技术包括挤出法和压缩法。制备含蛋白质的植入物的优选方法包括蛋白粉与所述聚合物和任选一种或多种赋形剂、盐、缓冲剂、防腐剂或聚电解质按适当比例粉末掺混或研磨。通常通过冻干或喷雾干燥制备蛋白粉,其中赋形剂可共同冻干或共同喷雾干燥。随后用热熔挤出机例如定制活塞式挤出机或双螺杆挤出机在适当挤出温度、挤出速率和挤出机喷嘴/螺杆尺寸下将粉末混合物挤成长丝。可由从喷嘴挤出并切割成所需尺寸的长丝得到杆状植入物。喷嘴的孔口直径范围可能为200至440μm。
挤出温度可为约25℃至约150℃,或约60℃至约90℃,约60℃至约100℃,或约50℃至约80℃。例如,可通过升温至约60℃至约100℃以便蛋白质/聚合物混合约5分钟至1小时,1分钟至约30分钟,或5-20分钟,来生成植入物。例如,时间周期可为约10至约30分钟,诸如例如约20分钟。然后可在约60℃至约90℃,或约60℃至约100℃的温度下挤出植入物。
可通过配制具有不同比例的聚合物、蛋白质和其它组分,例如本文所述赋形剂、盐或缓冲液中的任一种的几种植入物,根据经验测定蛋白质从植入物释放的速率。USP批准的溶解或释放试验方法可用于测量释放速率(USP 23;NF 18(1995)第1790-1798页)。例如,使用无限沉入法,将药物递送装置的称重样品加入至测量体积的于水(或其它适合的释放介质例如磷酸盐缓冲盐水(PBS))中含0.9%NaCl的溶液中,其中所述溶液体积将使得释放后蛋白质浓度小于20%,并且优选小于5%的饱和量。将混合物维持在37℃下并缓缓搅拌以确保蛋白质释放。可通过本领域中已知的各种方法,例如分光光度法、高效(有时称为高压)液相色谱(HPLC)、质谱等关注作为时间的函数释放到释放介质中的蛋白质的量。在本发明的情况下,可使用尺寸排阻高效液相色谱(SEC-HPLC)和酶联免疫吸附测定(ELISA)监测完整和生物活性抗体或其它蛋白质从挤出植入物的释放。
可将本文所述的药物递送***,和因此生物可降解的植入物,置于(或植入)患者眼的眼部区域内以治疗有需要的患者体内的各种眼部病状,包括黄斑变性(包括湿性年龄相关性黄斑变性);眼新血管形成(眼内新异常血管形成),包括但不限于脉络膜新血管形成和虹膜新血管形成;急性黄斑视神经视网膜病变;黄斑水肿(包括囊样黄斑水肿和糖尿病性黄斑水肿);***;视网膜病变,包括糖尿病性视网膜病变和增生性糖尿病性视网膜病变;增殖性玻璃体视网膜病变;视网膜动脉阻塞性疾病;视网膜中央静脉阻塞;视网膜分支静脉阻塞;葡萄膜炎视网膜疾病;视网膜脱离;视网膜前膜病症;前部缺血性视神经病变;非视网膜病变糖尿病性视网膜功能障碍;色素性视网膜炎;和青光眼。
本文公开的植入物也可置于关节内以治疗有需要的患者体内的关节内病状,例如骨关节炎、类风湿性关节炎、牛皮癣性关节炎、髌股疼痛综合征、关节痛和关节发炎。因此,在一个实施方案中,本发明提供了一种治疗有需要的患者体内关节上的关节内病状(例如以上命名的任何病状)的方法,所述方法包括将生物可降解的植入物置于关节内,其中所述植入物包含对治疗所述病状有效的蛋白质和生物可降解的聚合物基质,其中所述蛋白质与所述基质缔合并且其中所述植入物在置于关节之后,减轻所述病状的至少一种症状3个月或更久。

“个体”、“受试者”或“患者”指活人或非人类哺乳动物,例如灵长类动物、猴子、马、狗、兔、大鼠、小鼠、豚鼠或猪。个体或受试者可被进一步分类为需要治疗(患有)眼部或医学病状的患者、个体或受试者。

前眼部病状是影响前(眼前面)眼部区域或部位,例如眼周肌肉、眼睑或眼球组织或位于晶状体囊或睫状肌后壁之前的流体的疾病或病状。因此,前眼部病状主要影响或涉及结膜、角膜、前房、虹膜、后房、晶状体或晶状体囊和使前眼部区域或部位血管化或受神经支配的血管和神经。
前眼部病状的实例包括无晶状体;假晶状体;散光;脸痉挛;白内障;结膜疾病;结膜炎;角膜病;角膜溃疡;干眼综合征;眼睑疾病;泪器疾病;泪道阻塞;近视;老花眼;瞳孔病症;屈光异常和斜视。也可将青光眼视为前眼部病状,因为青光眼治疗的临床目标可为降低眼前房内房水的高压,即降低眼内压。

在本发明的某些形式中,药物递送***呈生物可降解的眼内或关节内植入物的形式。可通过挤出法生产植入物。通常,植入物为固体或半固体。“植入物”是远远大于微球体的药物递送装置。生物可降解的植入物通常将包含与生物可降解的聚合物基质缔合的蛋白质。所述基质可包含两种或更多种生物可降解的聚合物和/或两种或更多种结构不同的蛋白质。所述植入物优选配置用于植入眼的眼部区域或身体的关节内间隙,例如指、肘或膝关节,并且配制为提供治疗有效量的呈生物活性形式的蛋白质释放至少约1、2、3或6个月。在一些实施方案中,所述植入物可连续释放生物活性蛋白2个月或更久,或3个月或更久。

该方法中使用的聚合药物递送***可为整体式,即蛋白质均匀分布于整个聚合物基质中。

本发明的一个实施方案为治疗有需要的患者体内的眼部病状的方法,所述方法包括将生物可降解的眼内植入物置于患者眼内,其中所述植入物包含生物可降解的聚合物基质和对治疗眼部病状有效的蛋白质。所述植入物可为挤出长丝并且所述蛋白质可与生物可降解的聚合物基质缔合。可将所述植入物置于眼的前房、玻璃体、结膜下间隙或筋膜囊下隙内,从而长期,例如1、2、3或4个月或更久,治疗眼部病状。
本发明的另一实施方案为治疗有需要的患者体内的关节内病状的方法,所述方法包括将生物可降解的关节内植入物置于患者的关节内,其中所述植入物包含生物可降解的聚合物基质和对治疗关节内病状有效的蛋白质。所述植入物可为挤出长丝并且所述蛋白质可与生物可降解的聚合物基质缔合。
可通过各种方法,包括用镊子、注射器(装备有套管或针头)、套针或其它适合装置,将本发明的植入物***眼的眼部区域或身体的关节内区域。适合的设备(装置)包括美国专利公布第2004/0054374号和美国专利6,899,717中公开的那些设备(装置)。
包括适当大小的针,例如22号规格针、27号规格针或30号规格针的注射装置可有效地用于将植入物注入人或动物的眼内。由于蛋白质从植入物缓释,常常不必重复注射。这在有时必需频繁注射以维持治疗有效水平的蛋白质的年龄相关性黄斑变性的治疗上可能具有特殊价值。
本发明包括但不限于下列实施方案(1-24):
1.一种挤出的生物可降解的眼内植入物,其包含生物可降解的聚合物基质和与所述生物可降解的聚合物基质缔合的蛋白质,其中所述生物可降解的聚合物基质包含聚(D,L-丙交酯-共-乙交酯),并且其中在将所述植入物置于哺乳动物眼内之后,所述植入物提供呈生物活性形式的所述蛋白质的连续释放至少30天。

3.根据2所述的植入物,其中第一聚(D,L-丙交酯-共-乙交酯)重量与第二聚(D,L-丙交酯-共-乙交酯)重量的比例为约90:10。
4.根据1-3中任一项所述的植入物,其中所述蛋白质对眼部病状的治疗在治疗上有效。
5.根据4所述的植入物,其中所述蛋白质对减轻选自由眼新血管形成、脉络膜新血管形成、黄斑变性和黄斑水肿组成的组的眼部病状的至少一种症状在治疗上有效。
6.根据5所述的植入物,其中所述蛋白质为抗体、抗体片段、双特异性抗体、抗转运蛋白或DARPin。
7.根据6所述的植入物,其中所述蛋白质为结合血管内皮生长因子(VEGF)或血小板源生长因子(PDGF)的抗体、抗体片段、DARPin或抗转运蛋白。
8.根据7所述的植入物,其中所述蛋白质为抗VEGF抗体。
9.根据6所述的植入物,其中所述蛋白质为结合血管内皮生长因子(VEGF)和血小板源生长因子(PDGF)的双特异性抗体。
10.根据7所述的植入物,其中所述蛋白质为结合血管内皮生长因子(VEGF)或血小板源生长因子(PDGF)的抗体,并且其中在将所述植入物置于哺乳动物眼内后,所述植入物有效抑制或降低哺乳动物眼内的VEGF或PDGF体内活性至少4周。
11.根据10所述的植入物,其中所述蛋白质为结合VEGF的抗体(抗VEGF抗体),并且其中在将所述植入物置于哺乳动物眼内后,所述植入物提供呈生物活性形式的抗VEGF抗体的连续释放约90天。
12.根据1-10中任一项所述的植入物,其中所述生物可降解的聚合物基质包含:
a)具有酯端基,特性粘度为0.16-0.24dl/g(25℃下0.1%于氯仿中)并且D,L-丙交酯:乙交酯比例为约75:25的聚(D,L-丙交酯-共-乙交酯)(RG752S);
b)具有酯端基,特性粘度为0.32-0.44dl/g(25℃下0.1%于氯仿中)并且D,L-丙交酯:乙交酯比例为约75:25的聚(D,L-丙交酯-共-乙交酯)(RG753S);
c)具有酸端基,特性粘度为0.16-0.24dl/g(25℃下0.1%于氯仿中)并且D,L-丙交酯:乙交酯比例为约50:50的聚(D,L-丙交酯-共-乙交酯)(RG502H);和
d)具有酸端基,特性粘度为0.32-0.44dl/g(25℃下0.1%于氯仿中)并且D,L-丙交酯:乙交酯比例为约50:50的聚(D,L-丙交酯-共-乙交酯)(RG503H);或
e)其组合。
13.根据12所述的植入物,其中所述植入物进一步包含海藻糖、磷酸钠、聚山梨醇酯20或其组合。
14.根据13所述的植入物,所述植入物包含约82.5重量%的具有酸端基、特性粘度为0.32-0.44dl/g(25℃下0.1%于氯仿中)并且D,L-丙交酯:乙交酯比例为约50:50的聚(D,L-丙交酯-共-乙交酯)(RG503H);约10重量%的抗体、DARPin或抗转运蛋白;约6重量%的海藻糖;约0.1重量%的聚山梨醇酯20;和约1.4重量%的磷酸钠。
15.根据13所述的植入物,所述植入物包含约8.8重量%的具有酸端基、特性粘度为0.16-0.24dl/g(25℃下0.1%于氯仿中)并且D,L-丙交酯:乙交酯比例为约50:50的聚(D,L-丙交酯-共-乙交酯)(RG502H);约79.7重量%的具有酯端基、特性粘度为0.16-0.24dl/g(25℃下0.1%于氯仿中)并且D,L-丙交酯:乙交酯比例为约75:25的聚(D,L-丙交酯-共-乙交酯)(RG752S);约10重量%的抗体、DARPin或抗转运蛋白;约0.1重量%的聚山梨醇酯20;和约1.4重量%的磷酸钠。
16.根据1-15中任一项所述的植入物,其中在60℃和90℃之间的温度下挤出所述植入物。
17.一种用于将眼内植入物注入哺乳动物眼内的装置,所述装置包括i)具有纵轴的细长外壳;和ii)从所述外壳纵向延伸的套管,所述套管具有近端、远侧尖端和经由那里延伸的内腔,所述套管进一步包括如以上1-16中任一项所定义的植入物,其中所述植入物位于所述套管的内腔中。

24.一种制备生物可降解的眼内植入物的方法,在将所述植入物置于哺乳动物眼内后,所述植入物提供呈生物活性形式的蛋白质的连续释放至少30、60或90天,所述方法包括:
a)提供包含一种或多种蛋白质和任选一种或多种赋形剂、盐、缓冲剂、防腐剂、聚电解质或其组合的干粉;
b)将所述干粉与一种或多种聚(D,L-丙交酯-共-乙交酯)共聚物掺混以形成混合物;
c)在60℃和90℃之间的温度下挤出所述混合物以形成长丝;
d)切割所述长丝以形成适合置于眼的眼部区域的长度为0.5-10mm的植入物。
实施例1
制剂
通过挤压将单克隆抗体掺入PLGA或PLA植入物中。(一种市售的单克隆抗体)的组成为25mg/ml贝伐单抗、60mg/mlα,α海藻糖-二水合物、51mM磷酸钠(pH 6.2)和0.04%聚山梨醇酯20。用Zeba脱盐离心柱进行缓冲液更换以用不同类型和量的碳水化合物、盐和表面活性剂重新配制组合物。然后冻干重新配制的组合物以形成粉末。若需要,可添加额外的赋形剂例如溶解性增强组分、释放调节剂和防腐剂并且与所述蛋白质组合物一起共同冻干。按适当比例用Turbula振荡器将冻干粉与聚合物和赋形剂掺混。组分可将组分单独碾磨或与Retsch M200一起碾磨。然后将粉末掺混物挤压成具有不同直径尺寸的长丝,随后切成一定长度范围的植入物。如下面的实施例所述,制成了几种PLGA/PLA-单克隆抗体植入物。体外评估单克隆抗体从植入物的释放。
将植入物置于含释放介质(含0.01%叠氮化钠的PBS)的小瓶内并且在37℃下振荡。在适当时间点,从释放介质取样进行分析并且用新鲜介质全部更换所述介质以维持沉降条件。通过SEC-HPLC测定样品中的完整抗体并且将药物从植入物释放的累积百分比记为时间的函数。通过ELISA测定抗体的活性。
实施例2
含单克隆抗体和生物可降解的聚合物的植入物的生产和试验
如以上实施例1中所述,由其原始组成用Zeba脱盐离心柱(Thermo scientific)重新配制全长单克隆抗体贝伐单抗。制剂中赋形剂和缓冲剂的相对量在磷酸钠(0mM至51mM,包含磷酸一钠和磷酸二钠,冻干之前pH为约6-7)、α,α海藻糖-二水合物(0mg/ml至60mg/ml)和聚山梨醇酯20(0%至0.04%),的浓度范围内变化,并且与贝伐单抗抗体一起共同冻干。若需要,可添加额外的赋形剂例如PEG 3350(0mg/ml至10mg/ml)并共同冻干。使用适合蛋白质的冻干循环,在FTS Lyostar上获得冻干粉。
通过热重量分析(TGA)测得干粉的水分含量范围为3-6%并且通过尺寸排阻色谱-HPLC(SEC-HPLC)确定抗体的纯度范围为70-100%。
将TSK凝胶G3000SWxl(7.8mm×30cm)柱用于分离。SEC-HPLC以0.5ml/min的流速等度运行,运行时间为30分钟。检测器设为280nm波长。
由Boehringer Ingelheim Corp获得不同的聚合物组合物,包括产品RG502H、RG503H、RG504H、RG502、RG503、RG504、RG752S、RG753S、RG755S、R203S和R203H。使用TurbulaT2F型振荡器(Glenn Mills)在96rpm下按适当比例掺混聚合物组分、额外的赋形剂(若有)和冻干粉10分钟(两次)或使用Retsch MM200型在20cpm下研磨10分钟(两次)。用设为50psi的改进型气动驱动粉末压实机(Janesville Tool)将粉末混合物压入不锈钢筒内。使用定制活塞式挤出机挤出粉末混合物。根据制剂的组成和所需直径尺寸选择工艺参数。喷嘴直径范围从400到480μm。挤出温度范围从60到90℃。使***平衡20分钟,然后以0.0025in/min的速率挤出。丢弃前2-4英寸的挤出物。随后,将3-5英寸片段切入离心管。标记样品并储存在有干燥剂的密封铝箔袋中。
由每种制剂切6个5-6mm长的样品(1-1.5mg)。将样品称重并置于4mL玻璃小瓶中。向每个小瓶加2ml释放介质(含0.2mg/ml叠氮化钠的杜贝尔科磷酸盐缓冲盐水)。将每个小瓶封膜并置于设为37℃和50rpm的振荡水浴中。在每个时间点,从释放介质取样进行分析,并且用2mL新鲜介质完全更换所述介质。使用Waters 2690分离模块和Waters 2487双波长吸光度检测器分析样品。TSK凝胶G3000SWxl(7.8mm×30cm)柱可用于分离。通过计算随时间推移,在给定体积的介质中正在释放的抗体的量测定释放速率。使用重组人VEGF-A作为捕获抗体和抗人Fc/HRP作为检测抗体,通过酶联免疫吸附测定(ELISA)分析所选样品的活性。表1列出了根据这种工艺生产的含抗体的眼内植入物(挤出长丝)的实例。在表头处用聚合物编号标识每种制剂中包括的PLGA和PLA聚合物。图1-6和8中示出了抗体从这些植入物中释放的速率和持续时间并且在下面进行了描述。

图1显示了贝伐单抗从PLGA-贝伐单抗植入物的体外释放,所述植入物包含82.5重量%RG503H、10重量%贝伐单抗、6重量%海藻糖、1.4重量%磷酸钠和0.1重量%聚山梨醇酯20(1号制剂)。该图显示第一天内约10%的单克隆抗体从聚合植入物释放。前28天观察到相对恒定的释放速率,到第28天累积释放约45%的药物。到第35天,释放最小量的完整抗体。重要的是,通过SEC-HPLC和ELISA观察了比较释放曲线,说明在掺入生物可降解的聚合物和从生物可降解的聚合物中释放之后,单克隆抗体保持其生物活性。
图2说明了通过选择聚合物,增加药物从PLGA植入物释放的持续时间的能力。通过按90:10的比例掺混RG753S和RG502H(2号制剂),PLGA-贝伐单抗植入物显示释放的持续时间从1个月增加到2个月。
图3说明了通过改变制剂组成,将持续时间增加到至少3个月(约90天)的能力。在该实施例中,移除海藻糖赋形剂并且所述聚合物为比例为90:10的RG752S和RG502H的掺混物(3号制剂)。
图4展示了通过改变聚合物组成,特别通过聚合物掺混(例如,RG752S和RG502H)调节贝伐单抗从PLGA-贝伐单抗植入物释放的能力。掺入RG502H(50:50丙交酯:乙交酯,具酸端基)可增加水吸收并因此促进如在图4中由RG752S和RG502H的掺混物构成的植入物中的释放。图4比较了贝伐单抗抗体从具有4、5和6号制剂的植入物的体外释放。
图5显示了通过改变赋形剂载量,特别是改变海藻糖含量,继而改进贝伐单抗从PLGA-贝伐单抗植入物释放的能力。掺入水溶性碳水化合物例如海藻糖可增加水吸收并产生孔道,因此促进释放。图5比较了贝伐单抗抗体从具有4、7和8号制剂的植入物的体外释放。
图6说明了药物载量对PLGA-贝伐单抗植入物释放曲线的影响。当增加药物载量时,初始突释增加而持续时间减少。图6比较了贝伐单抗抗体从具有1和9号制剂的植入物的体外释放。
图8示出了从具有10号制剂的植入物获得的突释-平坦释放曲线,第1天约65%贝伐单抗释放并且在第7天累积释放约70%。7天后观察到最少释放或未释放。
实施例3
含抗体的植入物的体内试验
图7示出了来自体内试验的结果,其中经外科手术将挤出植入物(1号制剂)置于兔一只眼的玻璃体内。在这种药效学模型中,以2周间隔用VEGF激发兔以诱导血视网膜屏障断裂并且通过荧光素血管造影术捕获图像以评估从植入物释放的药物能够如何有效地减少该断裂。荧光素图像显示,与PLGA植入物(安慰剂)相比,PLGA-贝伐单抗植入物(DDS,药物递送***)在第2和4周有效地减少断裂,表明持续释放至少1个月。
这个结果显著,因为其证明了大分子例如全长单克隆抗体,可在冻干期间保持其三级结构,可掺入在高温下处理的聚合药物递送***(DDS)中,并且可呈生物活性形式从聚合药物递送***释放。
这些图和实施例证明了配制不同贝伐单抗制剂以获得不同持续释放时间的能力。此外,这些实施例说明大分子可在整个生产过程(在冻干和高温下挤压)和从聚合物释放期间保持其结构和活性。这些结果显著,因为其证明了从聚合植入物成功递送活性单克隆抗体几个月的能力。
Claims (14)
1.一种挤出的生物可降解的眼内植入物,其包含生物可降解的聚合物基质和与所述生物可降解的聚合物基质缔合的蛋白质,其中所述生物可降解的聚合物基质包含第一聚(D,L-丙交酯-共-乙交酯)和第二聚(D,L-丙交酯-共-乙交酯),所述第一聚(D,L-丙交酯-共-乙交酯)具有酯端基并且D,L-丙交酯:乙交酯比例为约75:25,并且所述第二聚(D,L-丙交酯-共-乙交酯)具有酸端基并且D,L-丙交酯:乙交酯的比例为约50:50,其中在将所述植入物置于哺乳动物眼内后,所述植入物提供呈生物活性形式的所述蛋白质的连续释放约90天。
2.根据权利要求2所述的植入物,其中第一聚(D,L-丙交酯-共-乙交酯)重量与第二聚(D,L-丙交酯-共-乙交酯)重量的比例为约90:10。
3.根据权利要求2所述的植入物,其中所述蛋白质对减轻选自由眼新血管形成、脉络膜新血管形成、黄斑变性和黄斑水肿组成的组的眼部病状的至少一种症状在治疗上有效。
4.根据权利要求3所述的植入物,其中所述蛋白质为结合血管内皮生长因子(VEGF)或血小板源生长因子(PDGF)的抗体、抗体片段、DARPin或抗转运蛋白。
5.根据权利要求4所述的植入物,其中所述蛋白质为结合血管内皮生长因子(VEGF)或血小板源生长因子(PDGF)的抗体,并且其中在将所述植入物置于哺乳动物眼内后,所述植入物有效抑制或降低哺乳动物眼内的VEGF或PDGF体内活性至少4周。
6.一种挤出的生物可降解的眼内植入物,其包含约8.8重量%的RG502H,所述RG502H为具有酸端基、特性粘度为0.16-0.24dl/g(25℃下0.1%于氯仿中)并且D,L-丙交酯:乙交酯比例为约50:50的聚(D,L-丙交酯-共-乙交酯);约79.7重量%的RG752S,所述RG752S为具有酯端基、特性粘度为0.16-0.24dl/g(25℃下0.1%于氯仿中)并且D,L-丙交酯:乙交酯比例为约75:25的聚(D,L-丙交酯-共-乙交酯);约10重量%的结合血管内皮生长因子(VEGF)的抗体;约0.1重量%的聚山梨醇酯20;和约1.4重量%的磷酸钠,其中在将所述植入物置于哺乳动物眼内后,所述植入物有效抑制或降低哺乳动物眼内的VEGF体内活性至少4周。

13.根据权利要求12所述的方法,其中在将所述植入物置于所述眼内后,所述植入物有效治疗所述眼部病状约90天。
14.一种制备生物可降解的眼内植入物的方法,在将所述植入物置于哺乳动物眼内后,所述植入物提供呈生物活性形式的蛋白质的连续释放至少90天,所述方法包括:
a)提供包含一种或多种蛋白质和任选一种或多种赋形剂、盐、缓冲剂、防腐剂、聚电解质或其组合的干粉;
b)将所述干粉与一种或多种聚(D,L-丙交酯-共-乙交酯)共聚物掺混以形成混合物;
c)在60℃和90℃之间的温度下挤出所述混合物以形成长丝;
d)切割所述长丝以形成适合置于眼的眼部区域的长度为0.5-10mm的植入物。
