技术领域
本发明属于外用脐贴药物领域,具体涉及一种治疗急性肠炎的脐贴及制备方法。
背景技术
急性肠炎是夏秋季的常见病、多发病。多由细菌及病毒等微生物感染所致,其表现主要为腹痛、腹泻、恶心、呕吐、发热等,严重者可致脱水、电解质紊乱、休克等。病人多表现为恶心、呕吐在先,继以腹泻,每天3 ~5 次,甚至数10 次不等,大便呈水样,深黄色或带绿色,恶臭,可伴有腹部绞痛、发热、全身酸痛等症状。在中医学中没有急性肠炎的病名,根据急性胃肠炎的主要临床表现,该类疾病属于中医的呕吐、腹泻、泄泻、霍乱、绞肠痧、脱证等病症范畴。其病因有感受时邪、饮食所伤、情志失调及脏腑虚弱等,但主要关键在于脾胃功能障碍和胃肠功能失调。
不良的生活、饮食习惯、饮酒过量等,极易引起急性胃肠炎的发生,且会反复发作、常伴精神心理障碍、导致生活质量下降、需长期服药或反复就诊,造成大量医疗资源消耗、患者经济负担沉重,除部分症状较轻者可通过药物治愈外,严重者需施行手术治疗,影响人体内食物消化及营养的吸收,同时会在体内引起其他损伤和疾病,严重危害人类健康。
目前,对于该类疾病的治疗多以抗菌素类药物为主,然而抗菌素多具有一定的毒副作用,长时间应用会导致肠道菌群紊乱,并且易产生耐药性。中医常规以“扶正祛邪”为治则,但长期以来并没有筛选出有效的典型药物作为主体来进行配伍,现有药方疗效皆难达到满意效果。
因此,通过科学组方,筛选一种有效治疗肠炎的中药,快速恢复消化***功能,是目前所要研究解决的重要课题。
发明内容
本发明针对现有技术存在的上述不足,提供一种治疗急性肠炎的脐贴,内病外治、用药安全,使用方便、见效快,同时解决了因口服药物制剂对胃肠道产生不良刺激而加重肝肾负担的弊端,避免了因中药原料药中存在的有毒成分服用后引起的一些不良反应。
本发明还提供了上述治疗急性肠炎的脐贴的制备方法。
本发明一种治疗急性肠炎的脐贴,由下述重量份的原料药制成:刘寄奴20~25、马蹄金16~20、白菊花20~30、白英16~25、仙人掌18~26、乌梅12~16、木香 6~8、乌药5~8、苍术12~16、冬瓜皮12~16、赤石脂16~25、丁香12~16和甘草8~10。
本发明一种治疗急性肠炎的脐贴的制备方法,步骤包括:
1)称取规定量的刘寄奴、马蹄金、白菊花、白英、仙人掌、乌梅、木香、乌药、苍术、冬瓜皮、丁香和甘草,清洗晾干,磨粉至80~90目,混合均匀,加入药材总重量3~5倍的浓度为80%~90%的乙醇水溶液密封浸泡10~15天,期间搅拌两次,过滤,滤渣加2~3倍的95%乙醇浸泡3~5h,回流提取1~2h,降温过滤,合并滤液,蒸干,加入规定量的赤石脂,混合磨粉至30~40目,得到中药提取物,备用;
2)称取0.5~0.8份的医用二甲基亚砜,0.3~0.6份的月桂氮卓酮,1~3份的薄荷醇和1~3份的冰片,混合均匀,加入到15~20份的纯化水中,缓缓搅拌至充分溶解均匀,得到促透剂混合液,备用;
3)将40~50份步骤1)得到的中药提取物和步骤2)得到的促透剂混合液,加入到40~60份的甘油中,搅拌20~30min,超声10min,备用;
4)称取30~50份的蜂蜡,溶化后,加入步骤3)的甘油中,搅拌8~17min,超声至混合均匀,得药膏,备用;
5)将步骤4)所得药膏加热,均匀涂抹到棉布上,于25~30℃、湿度55~60%条件下,静置2~3天,即得本发明治疗急性肠炎的脐贴。
本发明治疗急性肠炎的脐贴制备方法中,所用辅料中蜂蜡有收涩、生肌、止痛、解毒的功效,甘油是一种非常好的保湿用品,用于配制医药的药膏及皮肤保护油膏用的原料,制备方法中采用的基质(辅料)及促透剂,化学性稳定,制备工艺简便、有利于脐部皮肤穿透和吸收,特别对脐皮肤处无过敏反应,更无毒副作用。
本发明治疗急性肠炎的脐贴用法简单:外用,将脐部周围用温水洗净,擦干,将药片对准脐部,粘贴牢固即可,一日一次,一次一帖。
本发明治疗急性肠炎的脐贴以祛病消炎止痛为主,辅以固本养正之药,实现标本兼治,药性温和,攻邪补养相结合,辅助元气,增强免疫力,纯中药配方,无毒副作用。方中,以刘寄奴、马蹄金、白菊花、白英、仙人掌、乌梅为主药,具有镇痛消炎、抗菌、消肿化瘀之功效,木香、乌药、苍术、冬瓜皮、赤石脂、丁香和甘草为辅药,起清热利湿、温中散寒、和中益气、清热解毒、改善肠胃机能的作用,诸药相合、配伍精当,共奏消炎镇痛、消肿解毒、调理肠胃之功效,并到助消化的目的,使用本发明脐贴治疗急性肠炎,以及由此引起的消化不良症状具有安全、稳定、高效的临床疗效,能缓解症状、并减少疾病复发。
刘寄奴:苦、温,归心、肝、脾经,具有疗伤止血,破血通经,消食化积,醒脾开胃的功效,可用于急性黄疸型肝炎,牙痛,慢性气管炎,口腔炎,咽喉炎,扁桃体炎,肾炎,疟疾;外用治眼结膜炎,中耳炎,疮疡,湿疹,外伤出血。
马蹄金:味苦、辛,性微寒,主人肺经和胃经,有保肝降酶、镇痛消炎、抗菌、解热利胆、增强免疫及抗癌的作用,用于肝炎,胆囊炎,痢疾,肾炎水肿,***,泌尿系结石,扁桃体炎,跌打损伤。
白菊花:性味辛、甘、苦,微寒,归肺、肝经,有疏散风热、平肝明目、清热解毒的功效,主治风热感冒,发热头痛,目赤昏花,肝肾不足,目暗昏花,眩晕惊风,治肝阳上亢,头痛眩晕,善疏风清热,清肝泻火,兼能益阴明目,故可用治肝经风热,或肝火上攻所致目赤肿痛,共奏疏风清肝明目之效,清热解毒,尤善解疔毒,故可用治疔疮肿毒。
白英:甘、苦,寒,清热利湿、解毒、消肿、抗癌等功能;用于感冒发热,肾炎水肿,胆囊炎,痈疖肿毒,风湿性关节炎等症,临床上用于治疗各种癌症。
仙人掌:苦,寒,入心、肺、胃三经,行气活血,清热解毒,消肿止痛;主治心胃气痛,痞块,痢疾,痔血。《广西中草药》:止泻,治肠炎腹泻。《陆川本草》:消炎解毒,排脓生肌。《闽东本草》:能去瘀,解肠毒,健胃,止痛,滋补,舒筋活络,疗伤止血。治肠风痔漏下血,肺痈,胃痛,跌打损伤。
乌梅:归肝、脾、肺、大肠经,具有改善肝脏机能的作用,乌梅营养丰富,含蛋白质、脂肪、碳水化合物、柠檬酸、苹果酸、琥珀酸、齐墩果酸样物质及钾,谷甾醇等,乌梅具有收缩胆囊,促进蛋汁分泌和抗过敏的作用。
木香:味辛、苦,性温,归脾、大肠、三焦经。行气止痛,温中和胃,中寒气滞,胸腹胀痛等;《本草》言治气之总药,和胃气、通心气、降肺气、疏肝气、快脾气、暖肾气、消积气、温寒气、顺逆气、达表气、通里气,管统一身上下内外诸气,独推其功。
乌药:温中散寒,理气止痛,心胃气痛,吐泻腹痛;乌药配伍木香行气止痛散寒,长于温散下焦寒湿;木香行气止痛,功擅行脾胃气滞。两药相须为用,其散寒、行气、止痛之功效更著。
苍术:味辛、苦,性温,归脾、胃、肝经,燥湿健脾,祛风散寒,明目,可作用胃肠道平滑肌而实现对胃肠运动产生双向调节作用,其丙酮提取物能明显促进胃肠运动。
冬瓜皮:性微寒而味甘、淡,归脾、胃、大肠、小肠经,具有清热、利水、消肿之功效,能清热利水消肿,常用于水肿胀满,尤其适用于湿热所致之小便不利等症,冬瓜皮所含营养丰富,含多种挥发性成分、三萜类化合物、胆固醇衍生物,又含维生素B1、维生素C、烟酸、胡萝卜素等矿物质和微量元素。冬瓜皮还可治疗糖尿病,坚持饮服冬瓜皮汤,可使糖尿病患者的“三多”症状得到明显的改善。
赤石脂:固崩止遗、生肌敛疮;涩肠,止血,收湿,生肌;治久泻,久痢,便血,脱肛,遗精,崩漏,带下,溃疡不敛;《药性论》:“补五脏虚乏。”:“补心血,生肌肉,厚肠胃,除水湿,收脱肛。”:《本草汇言》“渗停水,去湿气,敛疮口,固滑脱,止泻痢肠澼,禁崩中淋带。”
丁香:温中、暖肾、降逆;主治呕吐,反胃,心腹冷痛;《医林纂要》:“补肝、润命门,暖胃、去中寒,泻肺、散风湿。”《药材学》:“治慢性消化不良,胃肠充气及子宫疝痛.”
甘草:和中益气,调和诸药补脾益气,清热解毒,祛痰止咳,缓急止痛,调和诸药。用于脾胃虚弱,倦怠乏力,心悸气短,咳嗽痰多,脘腹、四肢挛急疼痛,痈肿疮毒,缓解药物毒性、烈性。
本发明一种治疗急性肠炎的脐贴及制备方法,相对于现有技术,其有益效果为:
1)根据每位中药的不同特性和不同配比调制而成,配方独特,选料安全,用于急性肠炎疗效显著;
2)脐部皮肤给药,制备过程中添加医用二甲基亚砜、月桂氮卓酮、薄荷醇、冰片作为促透剂,更有利于药物透皮吸收而发挥药效,方便快捷,用药安全,使用方便、见效快,作用持久;
3)内病外治,不经过口服,不刺激胃和血管,同时解决了因口服药物制剂对胃肠道产生不良刺激而加重肝肾负担的弊端,同时避免了因中药原料药中存在的有毒成分服用后引起的一些不良反应。
具体实施方式
为了更好地理解本发明的实质,下面通过临床试验数据和具体实施例来进一步说明治疗急性肠炎的脐贴及制备方法,及使用本发明药物的有益效果:
实施例1
本实施例一种治疗急性肠炎的脐贴,由下述重量份的原料药制成:刘寄奴22、马蹄金17、白菊花22、白英18、仙人掌20、乌梅14、木香6、乌药6、苍术12、冬瓜皮15、赤石脂20、丁香13和甘草9。
本实施例一种治疗急性肠炎的脐贴的制备方法,步骤包括:
1)按本实施例处方称取刘寄奴、马蹄金、白菊花、白英、仙人掌、乌梅、木香、乌药、苍术、冬瓜皮、丁香和甘草,清洗晾干,磨粉至80目,混合均匀,加入药材总重量4倍的浓度为86%的乙醇水溶液密封浸泡15天,期间搅拌两次,过滤,滤渣加3倍的95%乙醇浸泡3h,回流提取2h,降温过滤,合并滤液,蒸干,加入处方量的赤石脂,混合磨粉至30目,得到中药提取物,备用;
2)称取0.5g的医用二甲基亚砜,0.3g的月桂氮卓酮,2g的薄荷醇和2g的冰片,混合均匀,加入到15g的纯化水中,缓缓搅拌至充分溶解均匀,得到促透剂混合液,备用;
3)将45g步骤1)得到的中药提取物和步骤2)得到的促透剂混合液,加入到45g的甘油中,搅拌30min,超声10min,备用;
4)称取35g的蜂蜡,溶化后,加入步骤3)的甘油中,搅拌15min,超声至混合均匀,得药膏,备用;
5)将步骤4)所得药膏加热,均匀涂抹到棉布上,于30℃、湿度60%条件下,静置2天,即得本实施例治疗急性肠炎的脐贴。
按本实施例步骤及配比制得的治疗急性肠炎的脐贴为一贴药物。
实施例2
本实施例一种治疗急性肠炎的脐贴,由下述重量份的原料药制成:刘寄奴25、马蹄金20、白菊花28、白英22、仙人掌25、乌梅16、木香8、乌药8、苍术16、冬瓜皮16、赤石脂22、丁香15和甘草9。
制备方法同实施例1。
动物实验研究
1、番泻叶致小鼠腹泻模型,小鼠模型共60只,雌雄各半,随机均分4 组:脐贴组、肠炎宁片组、盐酸小檗碱组、生理盐水组。
2、对小鼠排便的影响:
脐贴组,施用实施例1 制得的脐贴,8g生药/kg体重,可按体重比删减药量,进行小鼠止泻的观察实验,一日一次,一次一帖。
肠炎宁片组,8g /kg,每日3次。
盐酸小檗碱组,给予盐酸小檗碱片溶液,0.70g/kg体重,每日3次。
生理盐水组,给予等体积生理盐水。
连续给药5 次后禁食12h,末次给药30min 后,灌胃给予35%生大黄液1ml/20g 体重( 生大黄液用15%活性碳混悬液配制),观察8 小时内排黑便的潜伏期及粪便粒数。脐贴组与肠炎宁片组及生理盐水组比较,能明显延长小鼠排黑便的潜伏期,减少黑便数量,具有明显的止泻作用。结果见表1。

2、对小鼠小肠蠕动的影响
小鼠模型共60只,雌雄各半,随机均分4 组:脐贴组、肠炎宁片组、盐酸小檗碱组、生理盐水组。小鼠给药6 天,每天1 次,剂量同上,第5 次给药后禁食24h,末次药后1h,各组灌胃给予含15 %碳素墨汁的生理盐水(0.3ml/10g)。给药后15min脱颈处死,打开腹腔剪取上端至幽门,下端至回盲部的肠管,置于托盘上。轻轻将小肠拉成直线,测量肠管长度。从幽门至墨汁前沿的距离作为墨汁在肠内推进距离。用公式计算墨汁推进率。
墨汁推进率(%) =墨汁在肠内推进距离(cm)/ 小肠总长度(cm)×100%
表2 对小鼠小肠蠕动的影响
结果表明本发明脐贴能有效降低小肠蠕动频率,效果优于肠炎宁片。
临床试验资料(一)
1、患者资料
参照《中医病证诊断疗效标准》中的诊断依据,随机收集就诊于山东省立医院消化内科门诊的急性肠炎患者50 例,其中男性22 例,女性28例,患者年龄为16~59岁,病程为1~5天;用药前50 例患者症状具体表现为腹胀、腹痛、腹泻、恶心、呕吐。
2、治疗方法与疗效评价
将肚脐部位皮肤清洁,帖敷本发明贴剂,每天更换1 次。
参照《中药新药临床研究指导原则》进行疗效评价:①痊愈:症状消失或基本消失,且疗效指数≥90%;②显效:症状明显减轻,大部分体征消失,疗效指数≥70%;③有效:症状减轻,部分体征消失,疗效指数≥35%;④无效:症状及体征无减轻或加重者,疗效指数﹤35%。
疗效指数( 尼莫地平法) =[( 治疗前积分﹣治疗后积分)/ 治疗前积分]×100%
3、治疗结果
治疗后34 例治愈,治愈率为68%;13例显效,显效率为26%;3例有效,有效率为6%;总有效率为100%;无过敏及不良反应。
另外,所有患者在治疗过程中未发生刺激、过敏和其他任何不良反应。
4、典型病例
病例一:刘某,男,19岁。用药前腹痛、腹泻;使用本发明实施例1脐贴,用药1 天后,症状减轻,连续用药2 天后,症状消失。
病例二:王某,女,41岁。用药前腹胀、腹痛、恶心、呕吐;使用本发明实施例1脐贴,用药1天后,症状减轻,连续用药3天后,症状消失。
病例三:周某,女,52岁。用药前腹痛,恶心、呕吐酸腐,腹泻;使用本发明实施例2脐贴,用药1天后,腹泻,腹痛,恶心症状减轻;连续用药2天后,腹痛,恶心、腹泻症状消失,呕吐症状减轻,连续用药3天后,症状消失,再继续用药2天巩固疗效。
临床试验资料(二)
1、患者资料
参照《中医病证诊断疗效标准》中的诊断依据,随机收集2012年5月~2014年1月,就诊于山东省立医院消化内科门诊的急性肠炎患者60例,随机分为4组,每组15例。治疗组1和2为使用本发明药物组共30例,其中男11 例,女19 例,平均年龄42岁;对照组1为肠炎宁片(康恩贝)组15 例,其中男8 例,女7例,平均年龄42岁;对照组2为诺氟沙星组15例,其中男9例,女6例,平均年龄43岁。
2、治疗方法与疗效评价
治疗组1:本发明实施例1药物,将肚脐部位皮肤清洁,帖敷本发明贴剂,每天更换1次。
治疗组2:本发明实施例:2药物,将肚脐部位皮肤清洁,帖敷本发明贴剂,每天更换1 次。
对照组1:肠炎宁片(康恩贝)组,一日3 次,饭前30min 服用。
对照组2:口服诺氟沙星,每次10mg,每日3 次,饭前30min 服用。
疗效评价:痊愈、显著、有效、无效。
3、治疗结果(见表3)

治疗结果分析:由临床患者统计数据表明,本发明药物具有利胆助消化的功效,用药安全,使用方便、见效快。同时解决了因口服药物制剂对胃肠道产生不良刺激而加重肝肾负担的弊端,避免了因中药原料药中存在的有毒成分服用后引起的一些不良反应。
Claims (3)
1.一种治疗急性肠炎的脐贴,其特征在于,由下述重量份的原料药制成:刘寄奴20~25、马蹄金16~20、白菊花20~30、白英16~25、仙人掌18~26、乌梅12~16、木香 6~8、乌药5~8、苍术12~16、冬瓜皮12~16、赤石脂16~25、丁香12~16和甘草8~10。
2.一种权利要求1所述治疗急性肠炎的脐贴的制备方法,其特征在于,步骤包括:
1)称取规定量的刘寄奴、马蹄金、白菊花、白英、仙人掌、乌梅、木香、乌药、苍术、冬瓜皮、丁香和甘草,清洗晾干,磨粉至80~90目,混合均匀,加入药材总重量3~5倍的浓度为80%~90%的乙醇水溶液密封浸泡10~15天,期间搅拌两次,过滤,滤渣加2~3倍的95%乙醇浸泡3~5h,回流提取1~2h,降温过滤,合并滤液,蒸干,加入规定量的赤石脂,混合磨粉至30~40目,得到中药提取物,备用;
2)称取0.5~0.8份的医用二甲基亚砜,0.3~0.6份的月桂氮卓酮,1~3份的薄荷醇和1~3份的冰片,混合均匀,加入到15~20份的纯化水中,缓缓搅拌至充分溶解均匀,得到促透剂混合液,备用;
3)将40~50份步骤1)得到的中药提取物和步骤2)得到的促透剂混合液,加入到40~60份的甘油中,搅拌20~30min,超声10min,备用;
4)称取30~50份的蜂蜡,溶化后,加入步骤3)的甘油中,搅拌8~17min,超声至混合均匀,得药膏,备用;
5)将步骤4)所得药膏加热,均匀涂抹到棉布上,于25~30℃、湿度55~60%条件下,静置2~3天,即得治疗急性肠炎的脐贴。
3.根据权利要求1所述的脐贴,其特征在于:外用,将脐部周围用温水洗净,擦干,将药片对准脐部,粘贴牢固即可。
