葡萄糖的代谢依靠胰岛素,胰岛素是胰腺中胰岛的胰腺β细胞分泌的激素。
在一些患有糖尿病的病人内,胰腺β细胞被破坏或不足,并且不再分泌胰岛素或分泌不足量(称为“1型”糖尿病)。
在上下文所谓的“1型”糖尿病中,治疗通常包括注射一定剂量的胰岛素至病人体内,有时每天几次,并且监测病人的日常饮食。
有限数量的患有另一种类型的称为“2型”的糖尿病的人们(用于通过胰岛素获取和使用葡萄糖的病人的细胞变得对胰岛素不敏感),也使用胰岛素注射。
除了胰岛素注射以外,每一天还需要几次监测病人的血糖水平以确定病人不处于血糖过低的状态,这种状态对病人的健康是不利的。
另外,胰岛素需求取决于人的身体、脑力状况和活动,并且由激素调节。
为了确定血糖过多,通常执行下述步骤。
通过针刺和按压病人的手指以形成血滴。连接至用于确定血糖过多的装置的标签(languette)被用于该血滴。装置通常在LCD屏幕上指示病人的血糖水平(血糖过多)。因此这一操作是繁琐的。
基于这一理由,已经寻找较少限制的方案,以使得能够监测血糖过多并确保胰岛素注射率更好地适合病人的需要。
近来的发展已经能够持续监测血糖过高。
一种方案包括将电化学传感器引入病人的身体,其中,电化学传感器包括连接至酶、葡萄糖氧化酶的电极。存在氧气时,葡萄糖氧化酶与血液中存在的葡萄糖反应,以产生氧化水和葡萄糖酸内酯。氧化水随后通过电极被氧化且转变为水,其随后产生与血糖浓度成比例的电流。
这一方案的主要缺点是,由于使用的技术,其不能收集关于胰岛素需求的信息,并且不考虑葡萄糖体内平衡,也就是,身体保持血糖平衡的能力,使得即使外部约束趋向使身体离开这一平衡也能使其运行,其中该技术仅探测血糖过多但不检测其它激素调节或葡萄糖代谢。此外,信息不能被实时收集。
存在无创方案,例如:
-表面增强的拉曼光谱学;
-荧光光谱学;
-反向离子电渗疗法;
-光声光谱学;
-热或阻抗光谱学;或
-电磁场测量。
然而,这些无创方案不适于用于持续医学监测、例如医院环境的外部。
上述的所有常规方案使用称为血糖水平的单一参数来确定胰岛素和/或葡萄糖的需求,然而其它化合物,例如其它激素、脂类和特定氨基酸类生理上调节胰岛素和/或葡萄糖需求。因此,血糖水平并不总是仅反应胰岛素和/或葡萄糖的真实需求。鉴于这一原因,需要考虑到生理情况的复杂算法,以克服单一参数的使用。
此外,在对于病人的健康危险的血糖过低状态的检测中,这些方案产生较低可靠性结果。
最后,它们使得胰岛素能够被波动分配。胰岛素的波动分配将能够避免胰岛素阻抗,也就是,虽然存在胰岛素,病人的身体不能成功地使葡萄糖代谢的情况。
同样,在研究胰腺β细胞的领域中,关于新治疗原则或毒剂的研究对于筛查途径是相对复杂和困难的。这些研究也需要使用基因治疗,其因而改变了胰腺β细胞的自然态。
发明内容
因此,本发明将要克服上述缺点中的至少一个。
本发明也涉及胰腺β细胞或胰岛状态的长期观察,以解释物质对它们的电活动的影响。
本发明提出了依照权利要求1所述的传感器。
使用胰腺β细胞或胰岛的一个优点是,传感器可以比上述葡萄糖传感器提供更接近于病人生理状态的信息。
这样的传感器还能够使得细胞在它们从干细胞向胰腺β细胞分化(可选地,将其结构化为胰岛)的过程中被监测。
这一传感器的其它可选的和非限制性特征被权利要求2至10覆盖。
依照权利要求11或12,本发明涉及一种装置。
依照权利要求13,本发明也涉及一种用于生产这样的传感器和这样的装置的过程。
依照权利要求14,本发明还涉及一种这样的传感器的使用。
使用胰腺β细胞或胰岛的优点是,传感器可以比上述葡萄糖传感器提供更接近于病人生理状态的信息。

具体实施方式
传感器
参考图1和图5至图7,在下文描述依照本发明的传感器2。
传感器2可以用于适于通过分配胰岛素来治疗1型或2型糖尿病的装置。
传感器2主要包括一组微电极21;和胰腺β细胞23或包括胰腺β细胞23的胰岛230在所述一组微电极21上培养。
所述一组微电极21适于实时且持续地测量由胰腺β细胞23或胰岛230在它们的生理活化期间产生的电信号V。
如图1所示,微电极21在传感器2上的分布为使得微电极21依照规则且惯用的几何形状、一致地覆盖传感器2。微电极21间相互隔开的距离在50μm和200μm之间。一组微电极21的微电极密度在每平方毫米20个微电极和200个微电极之间。
在图1所示的示例实施方式中,传感器2包括在1mm2上分布的一组60个微电极。微电极21具有直径10μm的圆盘形。
传感器2也包括在一组60个微电极21上培养的胰腺β细胞23。
传感器2区分于用于常规的1型或2型糖尿病治疗装置中的传感器在于:其不仅测量血糖过多。事实上,如前所述,常规的传感器使用葡萄糖酸内酯以间接地测量葡萄糖,其在血液中的水平被认为反映病人的胰岛素需求,其并不必然正确,如前面所见。
使用胰腺β细胞使得更接近于健康个体的胰腺的自然生物功能成为可能,其被寻求在患有1型或2型糖尿病的个体(病人)中再生。事实上,胰腺β细胞通过分泌胰岛素调节血糖过多。它们的生理活化产生电信号,例如电势。因此,胰腺β细胞的生理活化包括在其膜的两侧上的电势的动态变化。该动态变化具有被称为波形并且反应胰腺β细胞的生理行为以及因而病人的胰岛素需求的基于时间的振幅轮廓。例如,具有本领域技术人员所知的称为动作电势的特定轮廓,并且其相应于电势超过势能阈值的快速和放大的变化。这些动作电势可以被识别。
在胰腺β细胞的情形下,动作电势的频率随着胰岛素的分泌而增加。如图3所示,在毫秒范围内,实时且连续地、也就是动态地测量电势。
胰岛的使用使得更接近于自然生物功能成为可能,因为代表胰岛细胞三分之二的胰腺β细胞,处于它们的自然环境中。
胰腺β细胞或胰岛的使用使得能够获得关于病人对胰岛素的需求的信息,使得能够以类似于在健康个体中血糖过多调节的方法调节病人的血糖过多。
传感器2还可以包括至少一组处理单元25。为了形成测量的电信号V,并且获得至少一个指示胰岛素的需求的参数P,每一个微电极21随后被联接至单一的处理单元25。
参数P也可以指示用于生理研究的胰腺β细胞23或胰岛230的电活动状态。可以在筛查有毒分子并监测细胞以研究它们从干细胞到胰腺β细胞的分化、也就是它们结构化成胰岛的背景下执行这一生理研究。也可以出于治疗目的执行这一生理研究。
每一个微电极21具有一个处理单元25的优点是,其能够持续且实时地处理测量的电信号V。
优选地,处理单元25定制为模拟微电子电路,例如采用亚微米CMOS技术生产的。对于获得高电路集成密度(几mm2)和低能量消耗,使得能够持续使用传感器2较长时间且限制传感器2周围的温度增加,这一微电子集成方案是最优的。信号的模拟处理实际上使得无需进行数字计算(在标准或专用处理器上)而执行成为可能,并且因此通过在单一线路上编码每一个信号而最小化到电路的内部连接。此外,标准CMOS技术处理单元25的生产是不昂贵的。
处理单元25包括子单元251和子单元253。
信号的形成由子单元251执行,子单元251接收来自于微电极21的电信号V且依照放大和/或过滤型(de type filtrage)数学函数对它们进行处理。在测量电势的情况下,子单元25可以特别地测量动作电势。
通过获得子单元253执行参数P的获得;参数P的值基于子单元253的输入信号计算,其中输入信号包括形成的电信号V以及设定点值Vc。设定点值Vc指示胰腺β细胞23或胰岛230的正常活化。
传感器2还包括调整器26,用于将指示胰岛素需求的参数P转变为控制信号C,以控制胰岛素分配器3。具有用于所有处理单元25的调整器26。获得的控制信号C取决于由处理单元25获得的参数P和设定点值Vc之间的差值,以及这一差值的导数。特别地,控制信号C基于由于对测量的电信号V进行时间和统计处理而形成的电信号V的波形、和/或形成的电信号V的频率特性。调整器26还考虑安全设定点,例如被分配的胰岛素量的相对和绝对限值和/或被分配的胰岛素量的变化限值。
例如,调整器26可以依照形成的电信号V的标准偏差或振动频率实现阈值检测变量;小波滤波;等等。
所述一组微电极21和/或一组处理单元25和/或调整器26可以生产于或位于半导体材料,例如硅。
所述一组微电极21和/或一组处理单元25和/或调整器26可以生产于或位于相同的半导体衬底或单独的衬底,随后因此电力地相互连接(由于金属化迹线M或柔性连接F)。
例如,如在图5至7所示,一组微电极21、一组处理单元25和调整器26生产于单独的硅衬底21s、25s、26s且通过柔性连接F连接(参见图5至7)。供选择地,单独的硅衬底21s、25s、26s可以位于相同衬底28,随后以本领域技术人员已知的方式彼此电力连接(参见图6)。
在图7的示例中,所述一组微电极21和一组处理单元25直接生产于相同的硅衬底21-25s,并且通过金属化迹线M电力地连接。
在图8的示例中,所述一组微电极21、一组处理单元25和调整器26直接生产于相同的硅衬底21-25-26s,并且通过金属化迹线M电力地连接。
直接在半导体衬底上生产所述一组处理单元25的优点是,其避免了由胰腺β细胞23或胰岛230产生且由所述一组微电极21测量的电信号V的退化,以及关于电信号V的体外处理的信息的损失。
在半导体衬底21s、23s、25s、21-25s、21-25-26s上生产所述一组微电极21、一组处理单元25和调整器26是本领域技术人员已知的且不在下文更详细描述。将引用微电极阵列(MEA)技术的示例。
传感器2可以适于以至少部分地被植入于病人身体9内,如图5至8所示。如果传感器2部分地可植入病人身体9内,至少所述一组微电极21和胰腺β细胞23或胰岛230处于病人身体9内。优选地,所述一组处理单元25也位于病人身体9内(图7)。还可能生产包括一组微电极21、胰腺β细胞23或胰岛230、所述一组处理单元25和调整器26的传感器2,并且全部可植入病人身体9内(图8)。
优选地,传感器2被植入间质组织内,接近病人身体9的动脉或腹膜。
身体的抗体攻击所有被识别为外来的或与病人身体9的免疫***不一致的分子,如在移植的情形中。
为了防止这一反抗,传感器2还可以包括半渗透膜27,适于过滤液体,例如血液,其必须接近胰腺β细胞23且阻止包含于液体中的重量大于65kDa(千道尔顿;1Da=1.67·10-24g)的分子。因此,不需要选择与病人9的免疫***一致的胰腺β细胞或胰岛:在膜27的存在下,反抗的风险被降低且不需要指定免疫抑制剂。
传感器2的供电电源可以是内部的或外部的。在后者情形下,电源必须是可移动的且能够在病人9走来走去时陪伴他或她。外部电源可以由外部感应产生。内部电源可以由电池产生。
胰腺β细胞23或胰岛230可以来源于猪、鼠科(例如老鼠)或人类。胰腺β细胞也可以来源于无性繁殖(也就是,它们已经被从一个或多个胰腺β细胞无性繁殖)或从干细胞(也就是,从还没有分化且能够被转变为任何类型细胞的细胞)获得。人类的胰腺β细胞也可以是尸体的,也就是,来自于死后其生理功能被影响之前的捐赠人。
在微电极21上胰腺β细胞23的数量在健康个体的胰腺内的胰腺β细胞数量的0.01%和1%之间。优选地,胰腺β细胞23的数量为健康个体的胰腺内的胰腺β细胞数量的0.1%。
胰岛230的数量被确定为使得包含于这些胰岛230内的胰腺β细胞的数量在健康个体的胰腺内的胰腺β细胞数量的0.01%和1%之间。优选地,胰岛230的数量被确定为使得胰腺β细胞的数量为健康个体的胰腺内的胰腺β细胞数量的0.1%。
传感器2可以用于除了胰岛素治疗的其它目的。实际上,传感器2可以用于多种试验,例如筛选(在其中数百甚至数千种成分沉积于至少同样多传感器上的试验)或实验室分析(例如,体外测量葡萄糖需求)。
获得的结果
可以在图1的传感器2上进行实验室试验。
图2示出了在葡萄糖和氯甲苯噻嗪存在的情况下,传感器2的胰腺β细胞23的活动。
已知的是,在活体内,葡萄糖的存在导致电信号的产生,例如以电势的形式,其中电势的轮廓可以具有在健康个体的胰腺β细胞中的动作电势的形式。胰腺β细胞随后释放胰岛素。还已知的是,在活体内,氯甲苯噻嗪抑制葡萄糖对健康个体的胰腺β细胞的影响,也就是在葡萄糖和氯甲苯噻嗪共同存在的情况下,胰腺β细胞不产生动作电势且因而不分泌胰岛素。
图2的第一条线表示葡萄糖存在于20mM的时间,也就是从0分到50分。第二条线表示氯甲苯噻嗪存在于200μM的时间,也就是从大约13分至16分和从大约27分至35分。在第三条线中,每一条表示电信号V中识别的动作电势的发生。第四条线示出了作为时间(以分钟计)的函数的这些动作电势的瞬时频率(以赫兹的IFR)。
可以看到,在存在葡萄糖而没有氯甲苯噻嗪时,传感器2的胰腺β细胞23具有持续的生理活动,同时,当存在氯甲苯噻嗪时,传感器2的胰腺β细胞23具有基本的生理活动(零胰腺β细胞23活化)。
还可以看到,氯甲苯噻嗪的影响不是决定性的。实际上,当不再有氯甲苯噻嗪时,传感器2的胰腺β细胞23的电活化由于仅存在葡萄糖而再次变得持续。
因此,传感器2的胰腺β细胞23相对于氯甲苯噻嗪具有生理行为。
图3示出了在电信号V中识别的动作电势发生的变化,其中,电信号V在胰腺β细胞23上测得,动作电势发生的变化作为葡萄糖浓度的函数。第一条线示出了葡萄糖浓度。在第二条线中,每一条表示动作电势的发生。第四条线示出了以类似形式动态地且持续地测量的电信号V。在虚垂直线处可看到动作电势波形。
在图3中可以看出,当葡萄糖浓度为20mM时,在电信号V中动作电势的发生高于当葡萄糖浓度为2.8mM时。
因此,胰腺β细胞23的电活动随葡萄糖浓度而改变,并且动作电势的发生随之增加,这与健康个体的胰腺的胰腺β细胞发生的情况相对应。
图4示出了肠促胰岛素激素GLP-1对传感器2的胰腺β细胞23内的动作电势的产生的影响。已知的是,对于健康个体,肠促胰岛素激素GLP-1增加胰腺β细胞内的动作电势的产生和因此的胰岛素的分泌。
图4的第一条线示出了传感器2的胰腺β细胞23位于与由垂直箭头所示的肠促胰岛素激素GLP-1接触的时间。第二条线示出了实时动态地且持续地测量的电信号V,具有可以以向下峰值的形式看出的动作电势的发生。
可以看出的是,在注入肠促胰岛素激素GLP-1大约二十秒之后,传感器2的胰腺β细胞23的电活化显著地增加。这样清晰地复制了在健康个体的胰腺的胰腺β细胞的自然环境中所观察到的。
生产传感器的方法
参考图9,描述用于生产上述传感器2的方法。
这一方法包括以下步骤:
-E1,提供衬底21s、21-25s、21-25-26s,具有一组微电极21,选择地一组处理单元25和/或调整器26;
-E2,通过等离子清理衬底21s、21-25s、21-25-26s;以及
-E3,在所述一组微电极21上培养胰腺β细胞23或胰岛230。
使用等离子清理衬底(“等离子清理器”)的E2使得衬底21s、21-25s、21-25-26s的表面成为疏水的且准备其用于胰腺β细胞23在衬底21s、21-25s、21-25-26s上的粘附。
以常规的方式获得胰腺β细胞23或胰岛230。
胰岛素分配装置
参考图5至8,在下面描述用于分配胰岛素的可行装置1的一个示例。
装置1包括胰岛素分配器3,用于分配注入病人身体9内的胰岛素的量。
装置1还包括上述的传感器2。
胰岛素分配器3可以植入病人身体9内。这不是必须的,由于胰岛素分配器3还可以位于病人身体9外部。
如果胰岛素分配器3和传感器2全部可植入病人身体9内,病人9将不被连接至他的或她的身体的外部物体的存在打扰。此外,这是审美和心理上的优点。装置1的电源可以与传感器2的相同。
通过这一装置2以闭环执行该血糖过多调节。实际上,胰腺β细胞23产生电信号V作为病人9的胰岛素需求的函数,所述一组微电极21测量这些电信号V并且所述一组处理单元25形成它们且从此获得参数P。这一参数P由调整器26接收,调整器26基于这一参数P,产生并发送控制信号C至胰岛素分配器3。胰岛素分配器3分配相应于控制信号C的胰岛素量。分配的胰岛素对血糖过多起作用并且因而改变病人9后来的胰岛素需求,并且血糖过多的降低导致了胰腺β细胞的活化的降低与动作电势的频率的降低,并因此回到基本线且停止控制信号C,直到有了血糖过多的另一个增加,例如在进餐期间。
Claims (14)
1.一种传感器(2),包括:
-一组微电极(21);以及
-胰腺β细胞(23)或胰岛(230),在所述一组微电极(21)上培养;
其特征在于:所述一组微电极(21)适于实时且持续地动态测量由所述胰腺β细胞(23)或胰岛(230)在它们的生理活化期间产生的电信号(V)。
2.如权利要求1所述的传感器(2),其中,所述一组微电极(21)适于测量电势形式的电信号。
3.如权利要求1或2所述的传感器(2),还包括至少一组处理单元(25),其中每一个处理单元(25)被联接至一个微电极(21),且适于:
-形成测量的电信号(V);以及
-实时且持续地获得指示病人(9)的胰岛素需求的至少一个参数(P)。
4.如权利要求2和3所述的传感器(2),其中,处理单元(25)适于检测动作电势。
5.如权利要求3或4所述的传感器(2),还包括调整器(26),用于将指示胰岛素需求的所述参数(P)转变为控制胰岛素分配器(3)的控制信号(C)。
6.如权利要求1至5之一所述的传感器(2),还包括半导体衬底(21s、21-25s、21-25-26s),包括至少所述一组微电极(21)。
7.如权利要求1至6之一所述的传感器(2),还包括半渗透膜(27),适于:
-过滤需要进入所述胰腺β细胞(23)或所述胰岛(230)的液体,并且
-阻止重量大于65kDa的所述液体的分子。
8.如权利要求7所述的传感器(2),适于被植入病人身体(9)内。
9.如权利要求1至8之一所述的传感器(2),其中,所述胰腺β细胞(23)或所述胰岛(230)来源于猪、鼠科或人类。
10.如权利要求1或2所述的传感器(2),还包括至少一组处理单元(25),其中每一个处理单元(25)被与一个微电极(21)联接,且适于:
-形成测量的电信号(V);以及
-获得指示所述胰腺β细胞(23)或所述胰岛(230)的电活动状态的至少一个参数(P),用于:
-用于筛选有毒分子的生理研究;或
-用于治疗结束的生理研究;或
-用于监测从干细胞到胰腺β细胞的分化的生理研究。
11.一种装置(1),包括用于将一定量胰岛素分配至病人身体(9)内的胰岛素分配器(3),
该装置(1)的特征在于,其还包括依照权利要求1至9之一所述的传感器(2)。
12.如权利要求11所述的装置(1),适于被植入病人身体(9)内。
13.用于生产如权利要求1至10之一所述的传感器(2)的过程,其特征在于,其包括以下步骤:
-E1,提供具有一组微电极(21)的衬底(21s、23-25s、23-25-26s);
-E2,采用等离子清理所述衬底;
-E3,在所述一组微电极(21)上培养所述胰腺β细胞或胰岛(230)。
14.如权利要求1或10之一所述的传感器的使用,用于:
-用于筛选有毒分子的生理研究;或
