Description

自动踝肱压指数***
技术领域
本发明涉及依据踝肱压指数(Ankle Brachial Pressure Index,ABPI)来识别和评估下肢动脉疾病。
背景技术
人们将ABPI表示成踝血压与最高肱(上臂)血压的比率。一般来说,通过测量收缩血压值来确定此指数。当动脉中的某个部分受到闭塞性疾病影响时,在流动方向上在闭塞下游的血压会低于闭塞上游的血压,于是会得到异常的踝肱血压指数,并可诊断出动脉疾病。
在一种已知的ABPI测量技术中,需要让患者仰卧并休息5到10分钟。然后,通过在上臂和踝处施加箍带,依次测量四肢中每一肢内的收缩压。通常使用手持式多普勒(Doppler)探头来听肱动脉中的血液流动以观察臂中的血压,或者听足背动脉和胫后动脉中的血压流动以观察踝血压。但是这项技术在以下方面存在困难:使用多普勒探头来识别足部中的动脉,以及在箍带膨胀和放气期间保持血管与多普勒探头的接触。此外,血压在不断变化,测试开始时测量到的压力可能与测试结束时测量到的压力毫无关联。
使用动脉光电体积描记(arteiral photo-plethysmography,PPG)可克服多普勒探头的不足。但是,这种技术依赖于手指或脚趾以及充足的动脉流动的存在,而后者可能会因温度或动脉疾病而受到影响。近年来,人们使用脉搏血氧计在测量收缩压时检测动脉搏动。但是,使用脉搏血氧计可能会在气压计箍带放气时使压力读数中出现误差,原因在于这种仪器内固有的取平均数的过程。
发明内容
本发明寻求作出改进。因此,本发明提供一种ABPI测量装置,该装置包括:多个可膨胀的箍带,至少一个箍带要卷绕在上肢上,且至少一个箍带要卷绕在下肢上,每个箍带都有第一腔和第二腔;用以使第一腔同时膨胀到预定压力、使第一腔保持在所述压力并使第二腔同时膨胀到所要的压力、记录下所述压力并使第一腔和第二腔放气的构件。
在优选实施例中,当不可能将箍带卷绕在踝上时(例如在糖尿病足的情况下),将至少一个箍带卷绕在手指上,并将至少一个箍带卷绕在脚趾上。优选将第二腔膨胀到一个高于所要压力的压力,且然后将其缓慢放气到所要的压力。
优选将每个第一腔同时膨胀到一个压力,直到检测到与肢中的动脉流动有关的信号为止。优选将每个第二腔同时膨胀到所要的压力,直到不再检测到来自每个第一腔的信号为止。然后,将每个第二腔膨胀到一个高于所要压力的压力,且最后将第二腔放气,直到再次检测到来自每个第一腔的信号为止,并将每个第二腔中的这个所要压力记录下来。
优选所述装置包括四个箍带,每个箍带要卷绕在每个肢或者脚趾或手指上。
根据本发明的另一方面,提供一种测量ABPI的方法,该方法包括以下步骤:将可膨胀的箍带施加到患者每个上臂或手指或者每个踝或脚趾上,箍带连接到流体源和控制设备,每个箍带具有第一腔和第二腔;将每个箍带的每个第一腔同时膨胀到第一压力,直到检测到与肢中的动脉流动有关的信号为止,然后将每个箍带的每个第二腔同时膨胀到不再检测到所述信号的压力,并将第二腔进一步膨胀到更高的压力,将第二腔缓慢地放气,直到在第一腔处再次检测到所述信号为止,控制设备记录下第二腔的压力值,并依据所记录的压力来计算ABPI值。
根据本发明的实施例,测量ABPI的方法包括将可膨胀的箍带施加到每个上臂或手指上,并获得压力值,并且对踝或脚趾重复所述方法,以获得每个踝的压力值,且然后提供对ABPI的显示。所述装置与常规ABPI测量方法相比的优点在于,其可同时测量成对肢的收缩压,例如同时测量臂或腿或者手指或脚趾,或者同时测量全部四肢或者手指或脚趾。对于对全部肢或者手指或脚趾同时膨胀感到不舒服的有些患者,可能有必要将成对的肢或者手指或脚趾膨胀。同时使全部的肢或者手指或脚趾膨胀不需要在测试前有休息时期。

具体实施方式
参看图,ABPI测量***1包含用于患者每个左踝和右踝的两个踝血压测量装置10(图2b)和用于患者每个左臂和右臂的两个臂血压测量装置10(图2a)。如图2c所示,也可设想不是两个踝血压测量装置10,而是可有两个脚趾血压测量装置,且不是两个臂血压测量装置10,而是可有两个手指血压测量装置。每个装置10包括具有两个腔11、12的箍带,所述两个腔11、12以任一腔均不会干扰另一个腔的方式连接在一起。箍带10适合于恰当地卷绕在踝和脚(图2b)上或上臂和前臂(图2a)上。
腔11、12通过弹性材料13连接起来,从而使得一个腔的动作不会干扰另一个腔的动作,同时在腔之间保持固定的距离,并可简单方便地施加到患者身上。腔11、12连接到常规的膨胀用流体源和具有用以控制膨胀的压力传感器的控制设备14。流体源和控制设备是常规的且对于所属领域的技术人员是已知的,且本文中将不再进一步描述。
在使用中,将四个血压测量装置10卷绕在患者的相应左右上肢和下肢上,并将每个腔11同时膨胀到适合于所述肢的压力(例如对于踝和臂是60mmHg,而对于脚趾和手指是不到30mmHg),直到在腔11处检测到与肢中的动脉流动有关的肺动脉体积描记(PAPG)信号为止。一旦在全部四个腔11处检测到PAPG信号,接着便使腔12膨胀,直到每一肢中的PAPG信号消失为止,腔12持续膨胀到比所述压力高出10mmHg到20mmHg。然后将腔12放气,并针对每个肢记下PAPG信号返回时所处的压力,使用这个压力值根据以下等式来计算ABPI,其中最高的肱压是左肱压和右肱压中的较大值:

***显示测量出的收缩压和计算出的左ABPI和右ABPI。如果测量的是脚趾血压,那么便显示左TBPI和右TBPI。也可将ABPI/TBPI值作为波形15打印出来,或者将其传送到远程地点。
对ABPI/TBPI值进行解释,从而给出对患者动脉***的状况的指示。典型的结果如下所示:
1)ABPI>=1.00    正常
2)ABPI>1.30      疑似动脉钙化。推荐脚趾压。
3)ABPI0.8-1.0     轻度PAD。
4)ABPI0.5-0.8     间歇性跛行。勿挤压。
5)ABPI<0.5       请咨询脉管专家。勿挤压。
6)TBPI<0.64      异常,有PAD
7)TBPI0.64-0.7    临界
8)TBPI>0.7       正常
通过在测试前或测试后将腔12膨胀到60mmHg压力,使***可如图3所示显示与肢中的动脉流动有关的波形15。来自每个腔12的波形可用于进一步的波形分析以便对动脉疾病进行分级。
以上描述了一种同时对四肢进行测量的血压测量***。但是,也可通过先测量臂或手指压力然后测量踝或脚趾压力来交错地测量成对的肢的压力。这对于病人舒适度和临床安全性方面可能是所需的。

Claims (6)

1.一种ABPI测量装置,其包括:多个可膨胀的箍带,至少一个箍带要卷绕在上肢上,且至少一个箍带要卷绕在下肢上,每个所述箍带都具有第一腔和第二腔,所述第一腔和所述第二腔分开一固定距离并通过弹性材料的纵向端连接起来,从而使得一个腔的动作不会干扰另一个腔的动作;用以使所述第一腔同时膨胀到预定压力、使所述第一腔保持在该压力并使所述第二腔同时膨胀到所要的压力、记录下所述压力并使所述第一腔和第二腔放气的构件。
2.根据权利要求1所述的ABPI测量装置,其中至少一个箍带卷绕在手指上,且至少一个箍带卷绕在脚趾上,每个所述箍带都具有第一腔和第二腔;构件使所述第一腔同时膨胀到预定压力,使所述第一腔保持在该压力并使所述第二腔同时膨胀到所要的压力,记录下所述压力并使所述第一腔和第二腔放气。
3.根据权利要求1或2所述的ABPI装置,其中所述第二腔同时膨胀到大于所述所要压力的压力,且然后同时缓慢地放气到所述所要的压力。
4.根据权利要求1所述的ABPI装置,其中每个第一腔同时膨胀到一压力,直到检测到与所述肢或者手指或脚趾中的动脉流动有关的信号为止。
5.根据权利要求4所述的ABPI装置,其中每个第二腔同时膨胀到所要的压力,直到不再检测到来自每个第一腔的所述信号为止,每个第二腔接着同时膨胀到高于所述所要压力的压力,且最后同时放气,直到再次检测到来自每个第一腔的所述信号为止,且每个第二腔中的这个所要压力被记录下来,以用于计算所述ABPI值或TBPI值。
6.根据权利要求1所述的装置,其中提供四个箍带,每个肢或者手指或脚趾上要卷绕一个箍带。
