技术领域

背景技术

具体实施方式
图1和2示出根据第一实施例的骨锚固装置1。骨锚固装置1包括单轴骨螺钉2和管状杆20。骨螺钉2具有带有骨螺纹以用于锚固在骨中的锚杆(shank)3,在一端(未示出)的尖端和在相对端的接收部分5。接收部分5包括用于接收杆20的大体U形的凹槽5a。借助于凹槽,形成两个自由支腿5b,5b’,所述自由支腿具有用于接收锁定元件的内螺纹5c,以将杆20固定在凹槽中。在所示的实施例中,锁定元件是可以拧入支腿之间的内螺钉50。螺纹可以是平螺纹以防止当拧紧内螺纹时支腿张开。然而,可以使用其它任何螺纹形状,例如公制螺纹、锯齿螺纹或负角螺纹。在已组装状态中,杆20置于凹槽5a的底部上。
在内螺钉50和杆之间设有填充件60。当从顶上观察时,填充件60的外轮廓是大体矩形,其带有两个相对的长直边和两个相对的向内弯曲的短边。填充件在其面对杆的侧面上具有杆接触表面61,其形状适应杆表面的形状。在所示的实施例中,杆20为圆柱形。因此,杆接触表面61由填充件中的截圆柱形凹槽形成。杆接触表面61的尺寸选择成当杆被夹在填充件和U形凹槽4a的底部之间时提供理想的载荷分布。与杆接触表面61相对的表面62是基本平坦的,在中心具有圆柱形突起63。圆柱形突起63可***到设在内螺钉的下侧的内螺钉50的相应圆柱形孔51中。借助于此,内螺钉可相对于填充件旋转。突起63在其自由端可以具有向外突出边缘64,所述向外突出边缘与内螺钉中的相应圆形凹槽配合使得填充件可旋转地支撑在内螺钉中。内螺钉在相对侧面上进一步具有用于接合工具的凹槽52。

骨锚固装置的所有部分由生物相容材料制成,例如由诸如钛这样的金属或金属合金或生物相容塑料材料制成。
在使用中,将至少两个骨锚固元件锚固到打算用杆稳定的两个椎骨或两个骨部分中。之后,将杆***到接收部分中。然后***带有填充件的内螺钉并且拧紧内螺钉。
从图3a和3b可以看出,当填充件用其杆接触表面61压靠在杆的表面上时,管状杆的变形被最小化,甚至从未发生。杆的表面上的均匀载荷分布防止了杆的变形。
如图4和6中所示,根据第二实施例的骨锚固装置1’是多轴型的并且包括采用骨螺钉形式的骨锚固元件2’,所述骨锚固元件具有带骨螺纹和尖端(未示出)的锚杆3和头部4。在所示的例子中,头部4具有球截形形状。用于接合拧入工具的凹槽(未示出)设在头部4的自由端。
骨锚固装置进一步包括接收部分5’,该接收部分具有第一端6和相对的第二端7,中心轴线C穿过分别由第一端和第二端限定的平面,并且同轴孔8从第一端延伸到离第二端一段距离处。在第二端7设有开口9,该开口的直径小于孔8的直径。球形部分10设在开口9附近,形成用于头部4的座。形成所述座的部分可以具有其它形状,例如圆锥形。
接收部分5’具有基本为U形的凹槽11,该凹槽11在第一端6开始并且延伸到离第二端7一段距离处以用于接收杆20’。借助于U形凹槽11形成两个自由支腿12、13。另外,接收部分5’进一步包括在支腿12、13上的内螺纹14。
骨锚固装置1进一步包括压力元件15。在所示的实施例中,压力元件15具有基本为圆柱形的构造,该圆柱形构造的外径仅仅稍小于孔8的内径以允许压力元件15被引入到孔8中并沿轴向方向运动。压力元件15在其面向第二端7的下侧面上包括球形凹槽16,该球形凹槽的半径基本上等于骨螺钉的头部4的半径。在相对侧面上设有基本为U形的凹槽17,该凹槽的深度大于杆20的直径。杆20连接至少两个骨螺钉。借助于U形凹槽17提供了两个自由支腿18、19,当杆就座在压力元件15中时,所述支腿延伸到杆20的表面之上。压力元件15进一步包括同轴孔21以用于允许用拧入工具操作头部4。
所述骨锚固装置进一步包括在图7-11中更具体描绘的填充件22。在所示的实施例中,填充件尺寸被制成可在压力元件的U形凹槽17中滑动。它具有面对压力元件15的支腿18、19的内侧的两个相对的平坦侧壁。此外,它具有两个相对的圆形侧面24、24’。填充件22在其下侧面上包括用于容纳杆20的凹槽25。在所示的实施例中,杆20是圆柱形杆,并且凹槽25适应杆20的形状和尺寸。凹槽的深度小于或等于杆20的半径,使得填充件22能够从上方将压力施加在杆20的表面上。填充件22在凹槽25的相对侧面上包括基本平坦的表面26。此外,它包括同轴孔27。围绕同轴孔设有从平坦表面26突出的接触表面28。该接触表面例如是环形的。
在如图4中所示的该实施例中,填充件22的尺寸可以使得在已组装状态中当杆20置于压力元件15的U形凹槽17中并且填充件22被放置在杆的顶部上时,表面26稍稍位于压力元件15的支腿18、19的端面之下。
所述骨锚固装置包括用于固定杆和头部的锁定装置29。在所示的实施例中,锁定装置29由采用内螺钉形式的第一锁定元件30组成,所述内螺钉与接收部分5’的内螺纹14配合。例如,如图6中所示,配合螺纹被成形为平螺纹。然而,任何其它螺纹形状是可以的。第一锁定元件30包括用于接合拧入工具的接合结构32和用于接收成形为固定螺钉的第二锁定元件31的同轴螺纹孔33。第二锁定元件31也包括用于接合拧入工具的接合结构34。在面对杆的侧面上设有突起35,该突起成形为装配在填充件22的孔27中并且允许在填充件22和第二锁定元件31之间旋转运动。
锁定装置29、压力元件15和填充件22的尺寸使得在如图6中所示的已组装状态中第一锁定元件30接触压力元件的自由支腿18、19的上表面,但是不接触填充件22。第二锁定元件31接触第二锁定元件22的接触表面28的表面,但是不接触压力元件。借助于此,可以独立地锁定接收部分中头部4的旋转位置和锚固装置中杆20的位置。
制成骨锚固元件的材料优选地是身体相容材料,例如钛或钛合金。
杆20’被成形为管,该管在其壁的至少一部分中具有螺旋形凹槽40。螺旋形凹槽40提供抵抗轴向力和弯曲力并且在特定应用中还抵抗扭力的弹性。螺旋形凹槽40的特征例如螺距、凹槽的宽度、螺旋的精确形状和其它参数可以根据杆20’的预期挠性性质而变化。杆20’进一步可以具有芯体41,该芯体的直径小于管的内径。芯体41可以被设计成可在管内滑动。制成芯体41的材料和其直径或具体形状选择成使得能够实现杆20’的预期弹性性质。例如,芯体41可以被提供用于增强挠性管状杆20’的硬度,例如用以避免扭转。
管状杆20’可以由与骨锚固元件相同的材料或由另一种材料制成。例如杆可以由本身具有增强弹性的材料制成。这样的材料可以是塑料材料或具有形状记忆和/或超弹性性质的形状记忆合金。
在使用中,骨螺钉2、接收部分5’和压力元件15可以被预组装,使得头部4可枢转地保持在接收部分5’中并且压力元件15宽松地保持和防旋转地固定在接收部分5’中。将预组装的骨锚固元件拧入到运动脊柱段的相邻椎骨中。之后,将杆20’***到接收部分中使得它置于压力元件中和接收部分的U形凹槽11的底部中。第二填充件22可以借助于延伸穿过填充件的孔27的第二锁定元件的突起35宽松地连接到第二锁定元件31。然后,将预组装的锁定装置29与填充件22一起***到接收部分中。最后调整接收部分相对于骨螺钉的角位置并且通过向下拧紧第一锁定元件30将骨螺钉的头部4锁定在该位置中。由于第一锁定元件30用其下表面邻接压力元件的支腿18、19的上表面而不接触填充件22,因此头部可以由压力元件15固定。之后,调整杆20的位置。最后向下拧紧第二锁定元件31直到它压靠在填充件的接触表面28上。分别作用在压力元件和杆之间的摩擦力将杆保持就位。
由填充件提供的杆的表面上的均匀载荷分布防止了变形。所以,在管状杆内可运动的芯体41不会被杆的变形卡住。
在进一步改进中,锁定装置仅仅包括单一锁定元件。在该情况下,压力元件15不具有在杆的表面上延伸的支腿18、19。填充件压靠在杆上并且杆压靠在压力元件上使得同时固定头部和杆。
锁定装置的其它改进是可以的,例如锁定元件包括外螺母或帽。填充件22然后可旋转地与锁定装置的元件连接。而且,如上所述的两部分锁定装置可以被改进,例如第一锁定元件30可以在其同轴孔中具有无螺纹部分,孔的直径大于填充件的直径使得第一锁定元件30不接触填充件的表面26。在该情况下填充件可以延伸到支腿18、19之上。
接收部分可以被设计成允许从底部引入螺钉元件。
杆可以完全或部分是管状的。芯体可以被省略。杆可以是刚性的或者完全或部分是挠性的。
进一步改进是可以的。代替骨螺钉,可以提供钩或钉状锚固元件。所述各种实施例的元件可以被组合。例如,两部分锁定装置也可以与单轴螺钉一起使用。

Claims (14)

1.骨锚固装置,其包括:
锚固元件(2,2’),其包括将被锚固到骨或椎骨中的锚杆(3)和连接到所述锚杆的接收部分(5,5’);
杆(20,20’),其用于连接至少两个锚固元件;
其中接收部分(5,5’)包括用于接收所述杆的凹槽(5a,11),并且所述杆至少在其一部分中具有管状形状;
锁定元件(50,29),其与所述接收部分配合以将所述杆固定到所述凹槽中;
填充件(60,22),其被布置在锁定元件(50,29)和杆(20,20’)之间以用于将压力施加到所述杆上,从而将所述杆锁定在所述凹槽中,所述填充件包括接触所述杆的一部分的杆接触表面(61,25),其中所述杆接触表面的形状适应所述杆的所述一部分的形状。
