REVENDICATIONS
1. Instrument de dilatation coulissant sur un fil guide, caractérisé en ce qu'il est réalisé dans une matière flexible et en ce qu'il est conformé de sorte que son extrémité distale a une forme d'ogive et est prolongée par un tronçon cylindrique initial, lequel est suivi d'un tronçon conique dont la pente est comprise entre I et 10 degrés, ce tronçon conique étant prolongé par un tronçon cylindrique terminal dont le diamètre est égal au diamètre maximum du tronçon conique, un canal central étant aménagé dans le corps de l'instrument, selon l'axe de celui-ci et sur toute sa longueur.
2. Instrument de dilatation selon la revendication 1, caractérisé en ce que la matière flexible est un plastique tel que le chlorure de polyvinyle.
3. Instrument de dilatation selon l'une des revendications 1 et 2, caractérisé en ce que la dureté de l'instrument est comprise entre 50et 100 sur l'échelle SHORE A.
4. Instrument de dilatation selon l'une des revendications 1 et 3, caractérisé en ce que la matière utilisée est transparente.

Instrument de dilatation selon l'une des revendications 1 à 4, caractérisé en ce que la longueur du tronçon cylindrique initial est comprise entre 60 et 100 mm dans le cas d'une dilatation ¯sopha- gienne, de manière que ce tronçon initial ait une longueur au moins égale à celle du rétrécissement à dilater.
6. Instrument de dilatation selon l'une des revendications I à 5, caractérisé en ce que le diamètre du tronçon cylindrique initial est compris entre 2 et 15 mm.

14. Instrument de dilatation selon les revendications 4 et 12, caractérisé en ce que le canal central de l'instrument est visible sur toute la longueur de l'instrument, permettant ainsi un contrôle optique de la propreté dudit canal.
15. Ensemble de dilatation formé de plusieurs instruments conformes à l'une des revendications I à 14, caractérisé en ce que la différence de diamètre du tronçon cylindrique terminal de deux instruments voisins par leur calibre est comprise entre 0.5 et 3 millimètres.
16. Ensemble de dilatation selon la revendication 15, caractérisé en ce que le diamètre du tronçon cylindrique terminal d'un instrument est supérieur au diamètre du tronçon cylindrique initial de l'instrument voisin de calibre immédiatement supérieur.
17. Ensemble de dilatation selon la revendication 15, caractérisé en ce que la différence de diamètre des tronçons cylindriques terminaux de deux instruments voisins par leur calibre décroît au fur et à mesure que les instruments sont de calibre plus important.
La présente invention se rapporte à un instrument de dilatation coulissant sur un fil guide ainsi qu'à un ensemble de dilatation composé de plusieurs de ces instruments.
Les rétrécissements des organes tubulaires, dont notamment les sténoses de l'oesophage, sont particulièrement désagréables, voire dangereux, pour le malade. Dans le cas de sténose de l'oesophage, la qualité de l'alimentaflon- du malade est compromise.
Pour combattre ces maux, la médecine offre plusieurs voies; la dilatation des sténoses en est une.
Les premières dilatations pratiquées le furent à l'aveugle par l'enfoncement d'une sonde dans l'organe à dilater. Les organes tubulaires sont souvent caractérisés par les méandres qu'ils forment.
Cette caractéristique peut être accentuée en cas de sténose. Les dilatations à l'aveugle ont abouti à de nombreuses perforations ou fausses routes.
Afin de guider les sondes, on introduit dans l'organe à dilater des fils guides sur lesquels les sondes coulissent, n'ayant plus ainsi à se frayer un chemin par elles-mêmes. Des fils de soie ont été et sont toujours utilisés comme guides. On leur préfère cependant, dans la mesure du possible, des fils métalliques.
Le matériel de dilatation utilisé le plus couramment se compose notamment d'un jeu de dix-neuf olives métalliques, d'une longueur de 42 mm, coulissant comme des perles sur un fil guide métallique grâce à un canal central. Considérant uniquement les olives utilisées, on peut noter plusieurs inconvénients et difficultés d'utilisation.
De par leur forme, les olives présentent un cône d'attaque relativement agressif. Outre le fait que la dilatation est réalisée de façon peu progressive, le diamètre maximum de chaque olive n'est atteint et maintenu que sur une très courte distance (la section de l'olive croissant jusqu'à un point maximum pour décroître immédiatement après). De cette particularité résultent deux inconvénients majeurs.
La force exercée par le médecin pour effectuer la dilatation est libérée abruptement par le passage de la zone la plus resserrée de la sténose. Une accélération soudaine de la pénétration peut donner lieu à une déchirure des tissus. Un deuxième inconvénient majeur est que, Si l'olive a passé avec succès le rétrécissement, on peut en conclure que chaque zone de la sténose tolère le diamètre de cette olive; en revanche, on ne peut être certain que toutes les zones le tolèrent simultanément. Or un nombre important de dilatations sont faites dans le but de pouvoir poser une prothèse qui se présente sous la forme d'un tube renforcé, lequel sollicitera simultanément une dilatation de toutes les parties de la sténose.
Un autre inconvénient du système à olives est le nombre important des olives nécessaires pour effectuer une dilatation maximum et, par conséquent, la multiplication d'une manipulation délicate. Cet inconvénient est lié pour une part à l'agressivité du cône d'attaque des olives, qui interdit une dilatation de plusieurs millimètres par la pénétration d'une seule olive.
Enfin, le système à olives comporte l'inconvénient que chaque olive doit être vissée sur un long poussoir métallique coulissant également sur le fil pilote. Ce poussoir métallique doit avoir une certaine flexibilité pour suivre dans une certaine mesure les méandres du conduit à dilater. L'utilisation d'un poussoir métallique flexible a pour effet que lorsque l'olive engagée rencontre un obstacle résistant, le seuil de flexibilité du poussoir est soudainement atteint et il en résulte que la force imprimée sur l'extrémité du poussoir prend subitement une composante latérale qui absorbe le surcroît de force qui aurait permis à l'olive de vaincre l'obstacle.
Pour pallier dans une certaine mesure les inconvénients du système à olives, et principalement pour réduire le nombre des manipulations, il a été proposé d'utiliser des sondes présentant plusieurs cônes de dilatation successifs. Cet enseignement est contenu dans le brevet GB-2,023,009 A.
Si le dispositif présenté dans ce document parvient à réduire le nombre des manipulations, à réduire dans une certaine mesure le ca ractére abrupt du passage des olives, il ne diminue en rien l'inconvénient que représente l'agressivité du cône d'attaque. De plus, ce dispositif occasionne à son tour des inconvénients non négligeables. En
effet, présentant plusieurs cônes successifs, ce dispositif suppose une pénétration importante dans l'organe pour que les derniers cônes puissent produire leur effet. Dans le cas de sténose de l'oesophage située à proximité de l'estomac, il en résulte un enroulement indésirable du dispositif à l'intérieur de l'estomac.
Un autre inconvénient important est lié à la présence de plusieurs cônes: en effet, le médecin effectuant la dilatation n'a de repère sur la manière dont s'effectue la pénétration que la sensation de résistance qu'il perçoit. La présence de plusieurs cônes fait que, lorsque le médecin ressent une résistance plus marquée, il ne peut savoir Si elle correspond à la difficulté rencontrée par un cône déjà partiellement engagé et freiné par la zone finale de la sténose ou Si, au con- traire, c'est le diamètre supérieur d'un nouveau cône qui n'est pas toléré.
En outre, dans une version qui a été offerte sur le marché, ce dispositif a une très grande souplesse. Si, pour la partie qui porte les différents cônes, cette souplesse paraît nécessaire (enroulement dans l'estomac), elle est discutable pour la partie sur laquelle le médecin
imprime la poussée de pénétration car elle donne lieu à un effet flasque qui rend l'utilisation difficile.
La présente invention a pour but de procurer un jeu d'instru
ments de dilatation permettant une dilatation plus progressive et
plus douce des sténoses tout en réduisant le nombre des manipula
tions et en donnant au médecin une sécurité et un confort d'utilisa
tion plus grands.
La solution selon l'invention est exposée dans les revendications
1 et 15.
On va décrire ci-après une variante d'exécution de l'instrument
de dilatation selon l'invention en se fondant sur le dessin ou:
la fig. I montre une coupe longitudinale de l'instrument,
la fig. 2 montre une coupe transversale de l'instrument selon
l'axe I/Il;
la fig. 3 montre un détail de l'instrument en coupe et en perspec
tive.
Comme on peut le voir à la fig. 1, l'instrument I possède un
canal central circulaire 6 qui est pratiqué selon l'axe A de l'instru
ment, sur toute la longueur de l'instrument. Ce canal permet, lors de
l'utilisation, de faire coulisser l'instrument sur un fil guide. Le dia
mètre du canal est de I mm.
L'extrémité distale 2 de l'instrument est façonnée en forme
d'ogive.
Le tronçon cylindrique initial 3 a une longueur choisie en fonc
tion de la longueur maximale habituelle des sténoses de l'organe à
dilater. On veillera à donner au tronçon cylindrique initial une lon
gueur au moins égale à celle de la sténose. Le fait que la sténose
entière soit engagée sur l'instrument avant que la dilatation propre
ment dite ne commence diminue les risques de perforation.
Le tronçon conique 4 effectue par son passage la dilatation pro
prement dite. Dans la variante décrite, l'angle a) du cône est compris
entre 4 et 5 degrés.
Le tronçon cylindrique terminal 5 a un diamètre égal au diamè
tre maximum du tronçon conique 4 qui le précède, sa longueur est
fonction de la longueur des tronçons précédents et est déterminée de
telle sorte que la longueur totale de l'instrument soit voisine de
700mu.
Sur la partie 7 du tronçon cylindrique terminal 5, partie qui est proche du tronçon conique 4, une poudre de métal lourd est noyée
dans la matière de l'instrument. On utilise de préférence de la poudre de tungstène qui est disposée en surface ou immédiatement
sous celle-ci en une couche mince.
La largeur du marqueur sera choisie de manière que le canal central reste visible, comme cela est montré à la fig. 3. De ce fait, le marqueur a la forme d'une bague, comme le montre la fig. 2.
Le marqueur 8 est disposé dans la partie 7 du tronçon cylindri
que terminal 5, pour indiquer l'avancement de la dilatation au diamètre maximum de l'instrument d'un calibre donné.
L'instrument de dilatation est réalisé dans une masse de PYC extrudé. A l'état brut, l'instrument a un diamètre constant qui correspond à celui du tronçon cylindrique terminal 5; il possède sur toute sa longueur le canal central 6 déjà mentionné.
La matière utilisée doit être à la fois souple et relativement incompressible pour qu'une poussée axiale soit possible même Si l'instrument suit une courbe. Pour obtenir l'effet désiré, une dureté de 90 à 93 sur l'échelle SHORE A donne les meilleurs résultats.
Dans la variante exposée, le façonnage de l'extrémité distale 2, du tronçon cylindrique initial 3 et du tronçon conique 4 est effectué dans un four à succion; les parois du moule étant glacées afin d'obtenir une surface lisse et de préserver la transparence de l'instrument.
Il va de soi que l'instrument est également prévu pour être réalisé entièrement par moulage de la matière.
Le marqueur opaque en radioscopie 8 est réalisé lorsque le fa çonnage est terminé. Il est alors nécessaire d'enlever une pellicule de matière dans la zone 7 sur une largeur et une épaisseur choisie et d'appliquer dans la dépression ainsi réalisée un mélange de plastique et de poudre, qui est préchauffé pour former un liquide pâteux. On peut ensuite recouvrir la bague réalisée avec une nouvelle pellicule de plastique.
Les instruments tels que décrits ci-dessus sont réalisés dans des calibres différents, soit des tronçons cylindriques terminaux de diamètres différents.
Un ensemble de dilatation est formé de plusieurs instruments de calibres progressivement plus importants.
On va décrire, à titre d'exemple, un ensemble constitué de sept instruments dont les calibres sont respectivement de 5, 7, 9, 11,12,8, l4et 15mu.
Pour les instruments dont les tronçons cylindriques terminaux sont de 5 et 7 mm, le diamètre du tronçon cylindrique initial est dans les deux cas de 3 mm, l'angle du cône est de 4 degrés.
Pour les instruments dont les tronçons cylindriques terminaux sont de 9 et il mm, le diamètre du tronçon cylindrique initial est de 4 mm dans les deux cas, l'angle du cône est de 5 degrés.
Pour les instruments dont les tronçons cylindriques terminaux sont de 12,8, 14 et 15 mm, le diamètre du tronçon cylindrique initial est dans les trois cas de 6 mm, l'angle du cône étant de 5 degrés.
On peut constater que le diamètre du tronçon cylindrique initial est toujours nettement inférieur au diamètre du tronçon cylindrique terminal de l'instrument de calibre immédiatement inférieur. Grâce à cette caractéristique, lorsque l'instrument de calibre supérieur est iii- troduit, le diamètre auquel la sténose vient d'être portée par l'instrument précédent n'est atteint que de façon progressive, de sorte que la dilatation supplémentaire apportée par chacun des instruments est précédée par une redilatation douce et progressive.
L'instrument selon l'invention présente de multiples avantages dont les principaux sont énumérés ci-après:
le faible angle du cône entraîne une diminution appréciable du risque de déchirement des tissus;
¯grâce à la longueur du tronçon cylindrique initial, la dilatation produite par le tronçon conique ne débute que lorsque la sténose entière est engagée sur le tronçon cylindrique initial; il en résulte une diminution du risque de perforation;
¯la longueur du tronçon cylindrique terminal offre la possibilité d'effectuer un calibrage prolongé de sténoses étendues;
¯pour le médecin qui effectue la dilatation, un avantage appréciable de l'instrument est que la résistance qu'il perçoit exprime, grâce à la longueur du cône, la résistance de l'ensemble de la zone sténosée, et non pas uniquement la résistance de la sténose au point précis du diamètre maximum;
au fur et à mesure que le diamètre maximum du cône avance dans la sténose, la force à imprimer pour parcourir le reste diminue, de sorte que l'achèvement du passage se fait en douceur;
-- également fondé sur la longueur du cône, un autre avantage, lié au précédent, doit être mentionné: étant donné que l'ensemble de la sténose prend appui sur le cône, la force appliquée par le médecin est répartie, de sorte qu'aucune zone n'en supporte seule la charge entière; cela évite dans une large mesure le risque d'un déchirement des tissus dans le sens de l'avance de l'instrument;
¯la flexibilité de l'instrument, telle que décrite plus haut, permet de conjuguer simultanément deux qualités essentielles.
La flexibilité entre enjeu progressivement, sans qu'un seuil apparent doive être franchi; elle est cependant contenue dans des limites qui permettent au médecin un guidage axial permanent de la force imprimée, ce qui lui assure une parfaite maîtrise de la répercussion de son geste.
Les avantages de l'ensemble composé de plusieurs instruments selon l'invention comptent évidemment les avantages de l'instrument lui-même, mais en outre:
le fait que les dimensions de chacun des instruments par rapport à ceux qui le précèdent ou le suivent dans l'échelle croissante des diamètres permettent à chaque étape de la dilatation de parvenir de façon progressive au diamètre obtenu lors de l'étape précédente, avant que de porter la dilatation au diamètre supérieur de l'instrument utilisé;
¯le fait que le nombre d'instruments à introduire, et par conséquent le nombre de manipulations, reste limité pour obtenir une dilatation au diamètre maximum (le nombre d'instruments est de sept dans la variante de l'ensemble décrite ci-dessus).

The present invention relates to an expansion instrument sliding on a guide wire and to an expansion assembly composed of several of these instruments.
The narrowing of the tubular organs, including in particular the stenosis of the esophagus, is particularly unpleasant, even dangerous, for the patient. In the case of esophageal stenosis, the quality of the patient's food is compromised.
To combat these ailments, medicine offers several ways; dilation of the strictures is one.
The first dilations performed were done blindly by driving a probe into the organ to be dilated. Tubular organs are often characterized by the meanders they form.
This characteristic can be accentuated in case of stenosis. Blind dilations have resulted in numerous perforations or false routes.
In order to guide the probes, guide wires on which the probes slide are introduced into the member to be dilated, thus no longer having to make their own way. Silk threads have been and are still used as guides. However, as far as possible, metal wires are preferred.
The most commonly used expansion material consists in particular of a set of nineteen metallic olives, 42 mm long, sliding like pearls on a metallic guide wire thanks to a central channel. Considering only the olives used, we can note several disadvantages and difficulties of use.
By their shape, the olives have a relatively aggressive attack cone. In addition to the fact that the expansion is not very gradual, the maximum diameter of each olive is reached and maintained only over a very short distance (the section of the olive growing to a maximum point to decrease immediately after) . This peculiarity results in two major drawbacks.
The force exerted by the doctor to perform the dilation is released abruptly by the passage of the tightest area of the stenosis. Sudden acceleration of penetration can cause tissue tearing. A second major drawback is that, if the olive has successfully shrunk, it can be concluded that each area of the stenosis tolerates the diameter of this olive; however, it cannot be certain that all areas will tolerate it simultaneously. However, a large number of dilations are made in order to be able to place a prosthesis which is in the form of a reinforced tube, which will simultaneously request dilation of all parts of the stenosis.
Another disadvantage of the olive system is the large number of olives necessary to effect maximum expansion and, consequently, the multiplication of delicate handling. This drawback is linked in part to the aggressiveness of the cone of the olives, which prevents expansion of several millimeters by the penetration of a single olive.
Finally, the olive system has the disadvantage that each olive must be screwed onto a long metal plunger also sliding on the pilot wire. This metal pusher must have a certain flexibility to follow to a certain extent the meanders of the duct to be expanded. The use of a flexible metal pusher has the effect that when the engaged olive encounters a resistant obstacle, the threshold of flexibility of the pusher is suddenly reached and it follows that the force impressed on the end of the pusher suddenly takes on a component lateral which absorbs the additional force which would have allowed the olive to overcome the obstacle.
To overcome to some extent the drawbacks of the olive system, and mainly to reduce the number of manipulations, it has been proposed to use probes having several successive dilation cones. This teaching is contained in patent GB-2,023,009 A.
If the device presented in this document manages to reduce the number of manipulations, to reduce to a certain extent the abrupt character of the passage of the olives, it does not in any way diminish the drawback represented by the aggressiveness of the attack cone. In addition, this device in turn causes significant drawbacks. In
Indeed, having several successive cones, this device assumes significant penetration into the organ so that the last cones can produce their effect. In the case of esophageal stenosis located near the stomach, this results in an undesirable coiling of the device inside the stomach.
Another important drawback is linked to the presence of several cones: in fact, the doctor carrying out the dilation has no point of reference on the manner in which the penetration takes place except the sensation of resistance which he perceives. The presence of several cones means that, when the doctor feels more marked resistance, he cannot know whether it corresponds to the difficulty encountered by a cone already partially engaged and slowed down by the final area of the stenosis or If, on the contrary , it is the upper diameter of a new cone that is not tolerated.
In addition, in a version which has been offered on the market, this device has very great flexibility. If, for the part which carries the different cones, this flexibility seems necessary (winding in the stomach), it is questionable for the part on which the doctor
prints the penetration thrust because it gives rise to a flaccid effect which makes use difficult.

The width of the marker will be chosen so that the central channel remains visible, as shown in fig. 3. Therefore, the marker has the shape of a ring, as shown in fig. 2.
Marker 8 is placed in part 7 of the cylindrical section
terminal 5, to indicate the progress of the expansion to the maximum diameter of the instrument of a given caliber.
The dilation instrument is made from a mass of extruded PYC. In the raw state, the instrument has a constant diameter which corresponds to that of the terminal cylindrical section 5; it has over its entire length the central channel 6 already mentioned.
The material used must be both flexible and relatively incompressible for an axial thrust to be possible even if the instrument follows a curve. To obtain the desired effect, a hardness of 90 to 93 on the SHORE A scale gives the best results.
In the variant described, the shaping of the distal end 2, of the initial cylindrical section 3 and of the conical section 4 is carried out in a suction furnace; the mold walls being glazed in order to obtain a smooth surface and to preserve the transparency of the instrument.
It goes without saying that the instrument is also intended to be produced entirely by molding the material.
The opaque marker in radioscopy 8 is produced when the shaping is finished. It is then necessary to remove a film of material in zone 7 over a selected width and thickness and to apply in the vacuum thus produced a mixture of plastic and powder, which is preheated to form a pasty liquid. We can then cover the ring made with a new plastic film.
The instruments as described above are produced in different calibers, that is to say terminal cylindrical sections of different diameters.
A dilation set is made up of several instruments of progressively larger calibers.
We will describe, by way of example, a set consisting of seven instruments whose calibers are respectively 5, 7, 9, 11,12,8, 14 and 15mu.
For instruments with terminal cylindrical sections of 5 and 7 mm, the diameter of the initial cylindrical section is in both cases 3 mm, the angle of the cone is 4 degrees.
For instruments with terminal cylindrical sections of 9 and 11 mm, the diameter of the initial cylindrical section is 4 mm in both cases, the angle of the cone is 5 degrees.
For instruments whose terminal cylindrical sections are 12.8, 14 and 15 mm, the diameter of the initial cylindrical section is in all three cases 6 mm, the angle of the cone being 5 degrees.
It can be seen that the diameter of the initial cylindrical section is always much smaller than the diameter of the terminal cylindrical section of the instrument of caliber immediately below. Thanks to this characteristic, when the higher caliber instrument is produced, the diameter to which the stenosis has just been carried by the previous instrument is only achieved gradually, so that the additional dilation brought by each of the instruments is preceded by a gentle and progressive redilation.
The instrument according to the invention has multiple advantages, the main ones of which are listed below:
the small angle of the cone leads to a significant reduction in the risk of tearing of the tissues;
¯thanks to the length of the initial cylindrical section, the expansion produced by the conical section does not begin until the entire stenosis is engaged on the initial cylindrical section; this results in a reduction in the risk of perforation;
¯the length of the terminal cylindrical section offers the possibility of performing extended calibration of extensive stenoses;
¯for the doctor performing dilation, an appreciable advantage of the instrument is that the resistance which he perceives expresses, thanks to the length of the cone, the resistance of the entire stenosed zone, and not only the resistance stenosis at the precise point of the maximum diameter;
as the maximum diameter of the cone advances in the stenosis, the force to print to travel the rest decreases, so that the completion of the passage is done smoothly;
- also based on the length of the cone, another advantage, linked to the previous one, should be mentioned: since the entire stenosis rests on the cone, the force applied by the doctor is distributed, so that no zone alone supports the entire load; this largely avoids the risk of tearing the tissue in the direction of advance of the instrument;
¯the flexibility of the instrument, as described above, makes it possible to combine two essential qualities simultaneously.
Flexibility comes into play gradually, without an apparent threshold having to be crossed; it is however contained within limits which allow the doctor a permanent axial guidance of the printed force, which ensures him a perfect control of the repercussion of his gesture.
The advantages of the assembly composed of several instruments according to the invention obviously count the advantages of the instrument itself, but in addition:
the fact that the dimensions of each of the instruments relative to those which precede or follow it in the increasing scale of diameters allow each stage of the expansion to progressively reach the diameter obtained during the preceding stage, before to bring the dilation to the upper diameter of the instrument used;
¯the fact that the number of instruments to be introduced, and consequently the number of manipulations, remains limited to obtain expansion to the maximum diameter (the number of instruments is seven in the variant of the assembly described above) .
