Dispositif distributeur d'aérosol
La présente invention concerne un dispositif distributeur qui convient tout particulièrement bien à la distribution et à l'administration de quantités dosées de fluides. L'utilisation principale d'un dispositif de ce genre est la distribution de quantités dosées d'un liquide contenant un médicament sous forme d'aérosol en vue d'un traitement thérapeutique par inhalation.
De manière plus particulière, l'invention concerne un dispositif distributeur de ce type grâce auquel la dose mesurée du médicament est administrée en réponse à l'inhalation ou inspiration du patient.
En médecine, on connaît parfaitement des inhalateurs à doses mesurées qui conviennent au traitement, ou au soulagement des effets de troubles respiratoires, par exemple l'asthme et ces inhalateurs comprennent, dans l'ensemble, un récipient distributeur d'aérosols sous pression, monté de manière amovible dans un support, ainsi qu'un dispositif pour actionner une valve dans le recipient de manière à provoquer la libération d'une quantité dosée ou mesurée du liquide contenant le médicament à libérer en direction d'une chambre comportant une embouchure à l'usage du patient.
Le moyen destiné à l'actionnement de la valve peut être un dispositif à gâchette que l'on commande à la main, ou bien le patient peut simplement exercer une pression sur

l'extrémité fermée du récipient à l'aide d'un pouce ou d'un doigt, mais, en tous cas, le patient est sensé coordonner l'actionnement de la valve et l'inhalation afin de tirer un bénéfice maximal du médicament.
Malheureusement, de nombreux patients dont l'état exige ce type de traitement sont incapables de coordonner leur respiration et l'actionnement manuel de la valve.
La présente invention à pour objet un inhalateur à doses mesurées grâce auxquel la libération de l'aérosol médicamenteux est commandée par l'inhalation du patient.
Selon l'une de ses caractéristiques, la présente invention à pour objet un dispositif distributeur susceptible d'être commandé ou actionné par l'inhalation, à utiliser avec un récipient distributeur d'aérosols sous pression, caractérisé en ce qu'il comprend : un réceptacle pour le récipient précité, un moyen délimitant une chambre d'emmagasinage ou de stockage agencée de manière à recevoir une dose mesurée à partir du récipient et comportant une sortie, une valve comportant une position fermée dans laquelle, en cours d'utilisation, elle ferme la sortie sous pression de la dose dans la chambre et une position ouverte dans laquelle la sortie est ouverte pour permettre à la dose de quitter la chambre et de pénétrer dans l'ajutage de sortie,
un ajutage de sortie par lequel un utilisateur peut procéder à des inhalations et un dispositif de libération sensible à l'inhalation d'un. utilisateur de manière à déplacer la valve vers sa position ouverte.

Selon un agencement préféré, la réponse du piston à l'inhalation provoque le fonctionnement d'une valve de façon à libérer le médicament emmagasiné à partir du moyen récepteur et de stockage dans l'embouchure.

Comme on peut le voir dans les figures 1 et 2, un dispositif d'inhalation 2 comprend un boîtier 4 dans lequel on a prévu un compartiment 6 pour un distributeur d'aérosols médicamenteux 8. Le distributeur 8 comprend une boite ou récipient 10 et un tube de sortie 12, les détails de ce dernier apparaissant clairement dans la figure 2.
La boite 10 contient un médicament en suspension ou dissous dans un propulseur pour aérosol liquide, le médicament convenant à un traitement thérapeutique par inhalation. L'intérieur de la boite 10 communique avec le tube de sortie 12 par l'intermédiaire d'une valve de sortie (qui est de forme classique et qui n'est pas représentée), la valve comportant une chambre de dosage. possède une ouverture de transfert (non visible dans les dessins) qui, lorsque, et uniquement lorsque, le tube se déplace vers l'intérieur par rapport à la boite 10, assure l'existence d'une communication entre l'intérieur de la chambre de dosage et l'intérieur du tube.
Le boîtier 4 comporte un trou foré 14, coaxial au compartiment 6 et le tube de sortie 12 s'adapte dans le trou foré, la face d'extrémité externe du tube étant en contact avec un épaulement 16 au fond du trou foré 14.
Le boîtier 4 comporte un ajutage de sortie qui se présente sous la forme d'une embouchure 18 et d'une partie intérieure creuse 20. Coaxialement au compartiment 6 et en dessous du niveau de l'embouchure 18, on peut voir une courte partie, cylindrique et s'étendant verticalement 22, qui comporte un trou foré 24.
Une saillie 26 s'étend dans la partie creuse 20 et le trou foré 14 s'étend dans la partie supérieure de cette saillie. Un tube débiteur 28 délimitant une chambre débitrice 29 est logé dans un trou foré 30, coaxial au trou foré 14 dans la saillie 26 et s'étend vers le haut sur une courte distance au-dessus de l'épaulement 16 au fond du trou foré 14.
Un orifice de sortie 32 raccorde la chambre 29 à la partie intérieure 20 et à l'embouchure 18 du boîtier 4.
Dans le trou foré 24 du boîtier, vient se logé un piston 34 possédant une jupe 36 et une tête ou partie supérieure 42. Une multiplicité de trous 38 tranversent la jupe tout autour de sa périphérie, Un disque d'étanchéité à faible friction 39 est fixé par un adhésif au fond de la saillie 26 et un très mince fil rigide 40 passe vers le haut à travers le disque

d'étanchéité, en contact à friction avec ce dernier et passe librement à travers le tube débiteur creux 28. Le fil s'étend également vers le bas et traverse librement un trou 41 dans la partie supérieure 42 du piston 34. A la partie supérieure extrême du fil est fixée une tête de valve 44 qui, lorsque l'inhalateur n'est pas en usage, repose en contact étanche sur la surface supérieur du tube débiteur 28 sous l'effet du poids du piston 34.
Deux disques 46 et 48 sont fixés sur le fil 40, l'un de ces disques (46) se situant à peu près au point médiant de la longueur du fil, cependant que l'autre disque (48) se situe au voisinage de l'extrémité inférieure du fil et en dessous de la partie 42 du piston 34. La boite ou récipient 10 peut glisser vers le haut et vers le bas dans le compartiment 6 et peut, par conséquent, être déplacé à la main par pression digitale directe sur l'extrémité de la boîte, ou bien on peut prévoir un système à levier ou un agencement à vis. Dans l'exemple représenté sur la figure 1, un levier 50 est articulé autour d'un pivot 52 assujetti dans une fourche 54 solidaire du boîtier et on peut amener une saillie 56 à faire pression vers le bas sur la tête de la boite 10 en basculant le levier manuellement.
En cours d'utilisation, le patient presse la boite vers le bas dans le compartiment 6 comme on vient juste de le décrire, de manière à faire fonctionner la valve de sortie qui se trouve dans la boîte. Ceci provoque le passage d'une dose mesurée de médicament et de propulseur via la chambre de dosage dans la chambre creuse 13 délimitée dans le tube de sortie 12 au-dessus du tube débiteur 28 qui, à ce moment est hermétiquement fermé par la tête de valve 44. L'étanchéité est assurée par le fait que le propulseur dans la chambre 13 se trouve à une

pression sensiblement supérieure à la pression atmosphérique et force par conséquent la tête de la valve 44 à s'appuyer sur l'extrémité du tube 28. Il faut comprendre que la totalité de la dose ne pénètre pas dans la chambre 13 à ce moment.
Ceci est dû au fait que la chambre 13 est en communication avec la chambre doseuse par l'intermédiaire de l'orifice ou lumière de transfert susmentionné, si bien que la dose est maintenue partiellement dans la chambre 13 et partiellement dans la chambre doseuse, en fonction de leurs calibres respectifs.
Tout en continuant à maintenir la boite dans sa position descendue, le patient embouche l'embouchure 18 du boîtier et inhale. Le maintient de la boite dans sa position descendue à pour effet d'empêcher la dose retenue dans la chambre 13 de s'échapper par l'ouverture de transfert. L'inhalation par le patient provoque une diminution de la pression dans la partie intérieure creuse 20 du boîtier, ce qui amène le piston 34 à se déplacer vers le haut jusque dans la position représentée en trait d'axe sur la figure 1, où sa face supérieure vient en contact avec le disque 46, déplaçant ainsi le fil 40 vers le haut pour dégager la tête de valve 44 de son siège sur la partie supérieure du tube débiteur 28.
Etant donné que la section transversale du piston 34 est très grande en comparaison de la surface de la tête de valve 44, une différence de pression relativement petite en travers du piston suffit à surmonter une différence d'opposition bien plus importante en travers de la tête de la valve. La dose mesurée de médicament et de propulseur dans la chambre doseuse de la valve de sortie du distributeur et dans la chambre 13 passe dans le tube débiteur 28 et traverse l'orifice de sortie 32 où elle se mélange à de l'air

pénétrant dans la partie creuse 20 du boîtier par les trous 38 prévus dans les parois du piston en position haute 34. Par conséquent, le patient inhale une dose mésurée de médicament.
Si on le souhaite, des moyens (non représentés) peuvent être prévus pour maintenir la boite dans sa position descendue au cours de l'inhalation, sans exiger que le patient continue d'excercer une pression vers le bas sur le levier 50, par exemple, ces moyens peuvent se présenter sous la forme d'une vis à spire hélicoïdale grossière de 90 .
Dans la forme de réalisation représentée plus haut, le poids du piston suffit à ramener le fil et la tête de la valve dans leur position abaissée et cependant suffisament faible que pour être levée sous l'effet de l'inhalation du patient. La friction du disque d'étanchéité 39 sur le fil 40 est suffisament limitée pour garantir une très faible résistance au mouvement axial du fil, mais convient à provoquer une étanchéité effective de l'extrémité inférieure du tube débiteur 28. Il faut noter qu'il n'est pas nécessaire de prévoir un degré élevé d'étanchéité entre le disque d'étanchéité 39 et le fil 40, étant donné que la dose mesurée ne se situe seulement que dans la région immédiatement au-dessus du disque d'étanchéité transitoirement au cours de son voyage vers l'orifice de sortie 32.
En alternative, on peut aussi utiliser un diaphragme ou une membrane souple, que l'on assujettit, par exemple à l'aide d'un adhésif, autour de sa marge périphérique, à la face inférieure de la saillie 26 et au fil 40. Il est nécessaire de parvenir à une flexion suffisante de la membrane ou du diaphragme pour permettre la faible élévation nécessaire à la levée de la tête de la valve 44 de

son siège sur le tube débiteur.
Si cela se révèle nécessaire, on peut interposer un léger ressort à compression 150 comme le montre la figure 2a, entre le disque d'étanchéité 39 et le disque 46 afin de pousser la tête de la valve vers son siège sur le tube débiteur. Ceci garantit que la tête de la valve repose toujours sur le tube débiteur avant l'actionnement du distributeur par le patient, éliminant ainsi la possibilité que la prémière partie de la dose pénétrant dans la chambre 13 puisse s'échapper au-delà de la tête de la valve, avant que cette tête de la valve ait eu le temps d'assurer l'étanchéité sous l'effet de la pression dans la chambre 13. Cet artifice assure également que la dose ne peut quitter la chambre 13 du simple fait de l'inversion du dispositif avant l'actionnement du distributeur.
Si le ressort n'était pas là, ceci pourrait se produire du fait de la chute du piston en direction du disque 46 et touchant celui-ci.
Si on le souhaite, le piston 34 peut être retenu dans le trou foré 24 en prévoyant une grille, un tamis métallique ou analogues par dessus l'extrémité ouverte du trou foré. Ceci est représenté sur la figure 2a, dans laquelle on voit qu'une coiffe terminale 152 comporte des trous à air 154.
En alternative, on peut pourvoir la face intérieure de la partie cylindrique 22 d'un rebord annulaire dirigé vers l'intérieur, qui s'étend en dessous des parois 36 du piston. En tout cas, il n'est pas nécessaire d'attacher le piston 34 au fil 40 et on peut omettre la partie inférieure du fil et le disque 48. Cependant, on doit alors disposer du ressort à compression sus-mentionné pour ramener la tête de la valve 46 dans sa position de fermeture étanche.
Selon un autre agencement, on équipe la

partie cylindrique 22 de trous tout autour de sa périphérie et on inverse le piston, qui a par conséquent sa partie 42 au fond, des trous 38 étant également répartis autour de la chemise tournée vers le haut. Les trous dans la partie cylindrique sont effectivement isolés de manière étanche par la partie continue de la jupe 36 lorsque le piston se trouve dans sa position la plus basse. Selon cet agencement, l'inhalation par le patient amène le piston à s'élever et lorsque les trous dans la jupe s'alignent sur les trous prévus dans la partie cylindrique 22, l'air sécoule à travers les trous alignés en direction de l'embouchure.
Afin d'assurer un alignement des trous dans la jupe sur ceux présent dans la partie cylindrique, on peut prévoir un agencement à cannelures entre le piston 34 et le trou foré 24. En alternative, les trous 38 peuvent être interconnectés par une rainure ou gorge annulaire formée dans la surface externe de la chemise.
Selon un autre agencement encore, représenté sur la figure 3, la chambre intermédiaire est chargée d'une dose mesurée de médicament, qu'on libère dans l'embouchure par l'intermédiaire d'une soupape à champignon à commande magnétique, que l'inhalation du patient active.
Comme la figure 3 le représente, la boite 10 est logée dans un boîtier 104 et son tube de sortie 12 s'étend dans un trou foré 114 qui est raccordé par un trou de petit diamètre 116 à une chambre intermédiaire 120. La chambre 120 est raccordée à une embouchure 118 par un élément de sortie 119 comportant un orifice de sortie 121 entouré d'un siège de valve 122. Une valve à champignon 124 est poussée en direction du siège de valve 122 par un léger ressort 126. La valve à champignon peut glisser

dans un trou foré 128 formé dans le boîtier 104 et un trou foré 130 supplémentaire s'étend à partir d'une cavité 132 dans une partie cylindrique 134 du boîtier vers le trou foré 128. Les trous forés 128 et 130 ne sont séparés que par une très mince lame seulement.
Dans le trou foré 130 s'engage une partie cylindrique 138 d'un piston qui comporte une tête de piston 140 normalement en contact avec un siège 142 et y retenu par un léger ressort 144. La cavité 132 est raccordée à l'embouchure par un passage 146.
On réalise la soupape à champignon 124 et la partie cylindrique 138 de façon qu'elles puissent s'attirer mutuellement par voie magnétique. A cette fin, la partie cylindrique 138 comporte un élément 139 qui est un aimant ou qui est en une matière magnétisable et la soupape à champignon comprend un aimant ou, pour autant que l'élément 139 soit un aimant, on la réalise en une matière magnétisable.
Pour le fonctionnement, on abaisse la boite de la manière antérieurement décrite, pour charger la chambre intermédiaire 120 d'une dose mesurée de médicament et de propulseur. Le patient inhale ensuite par l'embouchure 118 et ceci entraîne l'apparition d'une chute de pression dans la cavité 132. La différence de pression de l'un et l'autre côté de la tête de piston 140 suffit à surmonter l'action du ressort 144 et à amener la partie cylindrique 138 et son élément 139 à se déplacer d'avantage dans le trou foré 130 jusqu'à ce qu'il entre en contact avec la lame 136. La proximité de l'élément 139 agit pour retirer la soupape à champignon 124 de sa position d'étanchéité en contact avec le siège de valve 122 et le médicament se mélange à de l'air s'écoulant au delà de la tête de piston 140 et est alors inhalé par le patient.

Aerosol dispensing device
The present invention relates to a dispensing device which is particularly suitable for dispensing and administering metered quantities of fluids. The main use of such a device is the dispensing of metered amounts of a liquid containing a drug in aerosol form for therapeutic treatment by inhalation.
More particularly, the invention relates to a dispensing device of this type by which the measured dose of the medicament is administered in response to the patient's inhalation or inspiration.
In medicine, metered dose inhalers are well known which are suitable for the treatment or relief of the effects of respiratory disorders, for example asthma, and these inhalers generally comprise a pressure aerosol dispenser container, mounted removably in a support, as well as a device for actuating a valve in the container so as to cause the release of a metered or measured amount of the liquid containing the drug to be released towards a chamber comprising a mouthpiece at the use of the patient.

Unfortunately, many patients whose condition requires this type of treatment are unable to coordinate their breathing and manual valve actuation.
The present invention relates to a metered dose inhaler thanks to which the release of the medicinal aerosol is controlled by the patient's inhalation.

As can be seen in FIGS. 1 and 2, an inhalation device 2 comprises a housing 4 in which a compartment 6 is provided for a dispenser for medicated aerosols 8. The dispenser 8 comprises a box or container 10 and a outlet tube 12, the details of which appear clearly in FIG. 2.
Box 10 contains a drug suspended or dissolved in a liquid aerosol propellant, the drug suitable for therapeutic treatment by inhalation. The interior of the box 10 communicates with the outlet tube 12 via an outlet valve (which is of conventional shape and which is not shown), the valve comprising a metering chamber. The tube 12

has a transfer opening (not visible in the drawings) which, when, and only when, the tube moves inward relative to the box 10, ensures the existence of communication between the interior of the chamber and the inside of the tube.
The housing 4 has a drilled hole 14, coaxial with the compartment 6 and the outlet tube 12 fits into the drilled hole, the outer end face of the tube being in contact with a shoulder 16 at the bottom of the drilled hole 14.
The housing 4 has an outlet nozzle which is in the form of a mouthpiece 18 and a hollow interior part 20. Coaxially with the compartment 6 and below the level of the mouthpiece 18, a short part can be seen, cylindrical and vertically extending 22, which has a drilled hole 24.

seal, in friction contact with the latter and passes freely through the hollow delivery tube 28. The wire also extends downward and freely passes through a hole 41 in the upper part 42 of the piston 34. At the upper part end of the wire is fixed a valve head 44 which, when the inhaler is not in use, rests in sealed contact on the upper surface of the delivery tube 28 under the effect of the weight of the piston 34.
Two discs 46 and 48 are fixed on the wire 40, one of these discs (46) being located approximately at the midpoint of the length of the wire, while the other disc (48) is located in the vicinity of the 'lower end of the wire and below the part 42 of the piston 34. The box or container 10 can slide up and down in the compartment 6 and can, therefore, be moved by hand by direct digital pressure on the end of the box, or else a lever system or a screw arrangement can be provided. In the example shown in FIG. 1, a lever 50 is articulated around a pivot 52 secured in a fork 54 secured to the housing and it is possible to cause a projection 56 to press down on the head of the box 10 in tilting the lever manually.
In use, the patient presses the box down into compartment 6 as we have just described, so as to operate the outlet valve which is in the box. This causes a measured dose of medicament and propellant to pass through the metering chamber into the hollow chamber 13 delimited in the outlet tube 12 above the delivery tube 28 which, at this time, is hermetically sealed by the valve 44. According to this arrangement, the inhalation by the patient causes the piston to rise and when the holes in the skirt align with the holes provided in the cylindrical part 22, the air flows through the holes aligned in the direction of the 'mouth.
In order to ensure alignment of the holes in the skirt with those present in the cylindrical part, it is possible to provide a grooved arrangement between the piston 34 and the drilled hole 24. Into the drilled hole 130 engages a cylindrical part 138 of a piston which comprises a piston head 140 normally in contact with a seat 142 and retained there by a light spring 144. The cavity 132 is connected to the mouth by a passage 146.
The mushroom valve 124 and the cylindrical part 138 are produced so that they can attract each other by magnetic means. To this end, the cylindrical part 138 comprises an element 139 which is a magnet or which is made of a magnetizable material and the mushroom valve comprises a magnet or, insofar as the element 139 is a magnet, it is made of a material magnetizable.
For operation, the box is lowered in the manner previously described, to load the intermediate chamber 120 with a measured dose of medicament and propellant.
