Dispositif de détermination de paramètres biomécaniques du plancher pelvien
La présente invention a pour objet un dispositif de détermination de paramètres biomécaniques du plancher pelvien.
Dans l'élaboration du diagnostic de l'incontinence urinaire d'effort chez la femme, la détermination des paramètres biomécaniques du plancher pelvien est une des investigations les plus importantes.
La présente invention a dès lors pour objet un dispositif de détermination de tels paramètres, permettant de réaliser, par une prise d'appui sur la symphyse pubienne, une action de levier sur le plancher pelvien pour l'amener à l'écart de cette dernière. Ce dispositif comporte également des moyens pour relever des grandeurs significatives de l'écartement réalisé et de la force mise en oeuvre pour aboutir à cet écartement, grandeurs qui permettent la détermination des paramètres biomécaniques recherchés du plancher pelvien.
Suivant une caractéristique de l'invention, le dispositif de détermination de paramètres biomécaniques du plancher pelvien se présente essentiellement sous forme d'une pince constituée d'un support, formant une première branche de la pince, sur lequel est articulé un levier formant la seconde branche de la pince, les branches de la pince étant solidaires d'un dispositif sensible aux déplacements, susceptible d'émettre un signal représentatif de l'écartement de l'extrémité du levier par rapport au support, tandis qu'un des bras du levier comporte un dispositif sensible à l'effort exercé sur ce bras, susceptible d'émettre un signal représentatif de cet effort.
Suivant une autre caractéristique de l'invention, la première branche de la pince, constituant support, est essentiellement rectiligne, et le levier constituant la deuxième branche de la pince est un levier inter-appui, coudé, dont les deux bras sont disposés d'un même côté de la branche constituant support.
Suivant une caractéristique supplémentaire de l'invention, le dispositif sensible à l'effort exercé sur un des bras de levier est une jauge de contrainte.

Suivant une autre caractéristique de l'invention, le dispositif sensible aux déplacements angulaires est un dispositif à fluide constitué d'une chambre à volume variable centrée sur l'articulation de la pince, dont une paroicirculaire et une paroi radiale radiale, fixes, sont solidaires de la branche constituant support, et dont une paroi radiale, mobile, est solidaire de la branche constituant levier.
Suivant encore une autre caractéristique de l'invention, le dispositif est constitué sous forme d'une pince, dont la première branche est un élément rectiligne à bords arrondis, et dont la seconde branche est un élément coudé, également à bords arrondis, articulé à la première branche à l'endroit du coude, cette seconde branche étant 'constituée, respectivement de part et d'autre du coude, d'un bras d'appui destiné à venir au contact du tissu pariétal et orienté radialement par rapport à l'articulation, et d'un bras d'actionnement, un potentiomètre étant solidaire, à l'endroit de l'articulation, respectivement par son élément résistant d'une des branches de la pince et par son curseur de l'autre branche de la pince, tandis que le bras d'appui de l'élément coudé est sectionné.

L'invention sera mieux comprise en se reportant à la description, en même temps qu'au dessin annexé qui représente, uniquement à titre d'exemple, divers modes de réalisation de l'invention, et dans lequel: - la fig 1 est un schéma de principe d'un dispositif suivant l'invention, - la fig 2 est une vue schématique d'un mode de réalisation du dispositif de mesure de l'invention, mettant en oeuvre des capteurs à fluide, pneumatiques ou hydrauliques, - la fig 3 est une vue en élévation, partiellement en coupe, d'un mode de réalisation préféré d'un dispositif de l'invention.
En se reportant plus particulièrement à la représentation schématique de la fig. 1, le dispositif suivant l'invention se présente sous forme d'une pince 1 à deux branches, dont une branche 2 est destinée à venir au contact de la symphyse pubienne pour y prendre appui et constituer ainsi support, et dont le seconde branche 3 est destinée à appuyer par son extrémité 4 sur le plancher pelvien.
La branche 3 est articulée en 5 sur la branche 1 en vue de constituer levier inter-appui, et comporte un premier dispositif 6 de mesure de position angulaire, solidaire de l'articulation , émettant un signal représentatif de la position angulaire du bras d'appui 7 du levier par rapport au support 2, et donc de l'écartement de l'extrémité 4 de ce bras par rapport au support 2, et un dispositif 8 de mesure de force, monté dans la branche 3, émettant un signal représentatif de l'effort exercé sur le plancher pelvien. Dans la représentation de principe de la fig 1, le dispositif de mesure de force 8 est monté dans le bras d'actionnement 9 de la branche 3 constituant levier, ce bras étant réalisé en deux parties articulées l'une à l'autre, entre lesquelles est inséré le dispositif 8 de mesure de force, représenté par un dynamomètre.
En se reportant au mode de réalisation de la fig. 2, qui représente de manière schématique un dispositif suivant l'invention mettant en oeuvre des capteurs à fluide, la pince 21 se compose d'une branche 22 rectiligne, constituant support, et d'une branche 23, constituant levier

inter-appui, articulée à la branche support 22 par une articulation 25.
La branche constituant levier 23 est coudée et formée d'un bras d'appui 27, destiné à exercer, par son extrémité 24, un effort d'écartement sur le plancher pelvien, et d'un bras d'actionnement 29 en deux éléments, dont un élément 291 est rigidement solidaire du bras d'appui 23 et dont l'autre élément 292, destiné à recevoir l'effort d'actionnement, est articulé librement sur l'articulation 25.
Le bras d'appui 27 de la branche 23 est solidaire, à l'articulation 25, d'un dispositif 26 sensible à la position angulaire de ce bras par rapport à la branche support 22. Ce dispositif 26 est constitué d'une chambre 260, limitée par une enveloppe circulaire 261, solidaire du support 22, par une paroi fixe 262 également solidaire du support 22 , et par une paroi mobile 263, solidaire du bras d'appui 27. Le volume de la chambre 260 est ainsi fonction de la position angulaire du bras d'appui 27 par rapport au support 22. Etant donné que la longueur du bras 27 est fixe, le volume de la chambre 260 est également fonction de l'écartement de l'extrémité 24 de ce bras par rapport au support 22.
Il suffit donc que la chambre 260 soit remplie d'un fluide, par exemple hydraulique, et soit reliée à un dispositif de mesure de volume (non représenté) pour obtenir un signal indicatif de l'écartement de l'extrémité 24 du bras d'appui 27 par rapport au support 22.
Le bras 29 comporte également un dispositif 28 de mesure de force.
Dans la représentation schématique illustrée, ce dispositif 28 est constitué d'une chambre 281, solidaire de l'élément 291 du bras d'actionnement, et d'un piston 282 solidaire de l'élément 292 du bras d'actionnement, le piston 282 coopérant avec la chambre 281. Lors d'un effort exercé sur l'élément 292 du bras d'actionnement dans le sens de la flèche 293, l'élément 292 qui pivote librement autour de l'articulation 25 transmet cet effort à l'élément 291 du bras d'actionnement, solidaire du bras d'appui 27. La transmission de l'effort s'effectue par l'action du piston 282 sur le fluide contenu dans la chambre 281.
Il suffit donc qu'un dispositif de mesure de pression (non représenté) soit relié à la chambre 281 pour obtenir une grandeur représentative de la force exercée sur l'élément 292 du bras d'actionnement 29, et donc de l'effort transmis au bras d'appui 27.

Enfin, en se reportant au mode de réalisation illustré en fig.3, qui constitue un mode de réalisation préféré de l'invention, la pince 31 est constituée d'une branche 32 constituant support et d'une branche 33 constituant levier inter-appui. Comme dans le mode de réalisation illustré en fig.2, la branche 32 constituant support est rectiligne, tandis que la branche 33 est coudée et est articulée, au droit du coude, à la branche 32 par l'articulation 35. Cette branche 35 est constituée d'un bras d'appui 37 et d'un bras d'actionnement 39, disposés de part et d'autre du coude.
La pince 31 comporte, à l'endroit de l'articulation 35, un potentiomètre 36 constituant dispositif sensible aux déplacements angulaires. Ce potentiomètre est formé d'une piste conductrice 361 solidaire de la branche 33 et d'un curseur 362 solidaire de la branche 32. Tout pivotement de la branche 33 par rapport à la branche 32 se traduit ainsi par un déplacement du curseur 362 sur la piste 361, et donc par une modification de la résistance du potentiomètre- L'on peut ainsi étalonner le potentiomètre pour que la valeur mesurée soit convertie en distance ou écartement de l'extrémité 34 du bras d'appui 37 par rapport à la branche support 32. Pour la facilité de cette conversion, le bras 37 s'étend radialement par rapport à l'articulation 35.
Cette pince 31 comporte également un dispositif de mesure de force 38 dans le bras d'appui 37. Ce dispositif de mesure de force est constitué d'une jauge de contrainte 381 solidaire du bras 37. Etant donné les relativement faibles contraintes à mesurer, suivant le mode de réalisation illustré en fig. 3 le bras 37 est sectionné en 40, et les deux parties du bras sont réunies par une pièce métallique 382 de faible épaisseur, à laquelle est fixée la jauge de contrainte 381. La pièce métallique 382 est fixée aux deux parties du bras 37 par des vis 383, 383', l'ensemble du dispositif de mesure.de force, y compris la fente 40, étant noyé dans une matière élastomère.
Les signaux provenant de la jauge de contrainte 381 et du potentiomètre 36 sont acheminés par des fils au travers de la branche 39. Ils peuvent ensuite être acheminés à des dispositifs de mesure, ou même à une unité de traitement de l'information, pour déterminer, en fonction des mesures effectuées, les paramètres biomécaniques du

plancher pelvien.
La pince 31 illustrée en fig 3 comporte en outre un dispositif élastique 41 permettant de transformer la pince en dispositif de rééducation. Ce dispositif élastique 41 est constitué d'une tige filetée 410 articulée sur un pivot 411 solidaire de la branche 33 et passant au travert d'un guide 412 solidaire de la branche support 32. Un ressort 413 est comprimé entre un bouchon 414 vissé sur la tige filetée 410 et le guide 412. Le bouchon 414 peut bien entendu être dévissé jusqu'à une position où il n'exerce aucune action sur le ressort, pour que la pince puisse être utilisée exclusivement comme instrument de mesure. Enfin, une butée 415 permet de limiter l'action du ressort 413 à une partie de la course de la pince 31, la compression du ressort 413 par le bouchon 414 déterminant elle l'importance de l'effort à exercer sur cette partie de la course de la pince.
Il convient de noter que la tige filetée, dont la direction est déterminée par le pivot 411 et le guide 412, passe à proximité immédiate de l'articulation 35, et ainsi que les variations de distance entre le pivot 411 et le guide 412, et par voie de conséquence la course de compression du ressort, sont très faibles, ce qui garantit un effort substantiellement constant sur toute la course de compression du ressort 413 résultant du mouvement de la pince.
Pour des raisons anatomiques évidentes, les bords latéraux ainsi que l'extrémité de la branche support 32 et du bras d'appui 37 sont arrondis.
Lors de l'utilisation de la pince 31 en instrument de détermination de paramètres biomécaniques du plancher pelvien, le bouchon 414 est dévissé jusqu'à ce que plus aucune compression ne soit exercée sur le ressort 413. La pince est alors introduite dans le vagin et positionnée pour que la branche support 32, venant au contact du tissu pariétal, prenne appui sur la symphyse pubienne. Dans cette position, une action est exercée sur le bras d'actionnement 39 en direction du support 32.
Cette action entraîne un pivotement de la branche 33 autour de l'articulation 35 et un écartement concomitant du bras d'appui 37 à l'écart de la branche support 32. Cet écartement entraîne une poussée de l'extrémité 34 du bras d'appui 37, venant au contact du tissu pariétal, sur le plancher pelvien, tendant à écarter ce dernier de la symphyse

pubienne. L'importance de l'écartement, et l'effort à réaliser pour y aboutir, permettent de déterminer des paramètres biomécaniques du plancher pelvien intéressants pour l'élaboration du diagnostic de l'incontinence urinaire d'effort chez la femme.
L'invention n'est bien entendu pas limitée aux dispositifs de mesure de déplacement et d'effort décrits ci-dessus. C'est ainsi, à titre d'exemple, que le dispositif de mesure d'effort pourait être un détecteur piézo-électrique solidaire de l'extrémité d'appui du levier, et que le dispositif de mesure de déplacement pourrait être un élément curviligne solidaire d'une branche de la pince et traversant la seconde branche.

Referring to the embodiment of FIG. 2, which schematically represents a device according to the invention using fluid sensors, the clamp 21 consists of a straight branch 22, constituting a support, and a branch 23, constituting a lever

inter-support, articulated to the support branch 22 by a hinge 25.
The lever branch 23 is bent and formed by a support arm 27, intended to exert, by its end 24, a separation force on the pelvic floor, and an actuation arm 29 in two elements, one element 291 of which is rigidly secured to the support arm 23 and the other element 292 of which, intended to receive the actuating force, is freely articulated on the articulation 25.
The support arm 27 of the branch 23 is integral, at the articulation 25, with a device 26 sensitive to the angular position of this arm relative to the support branch 22. This device 26 consists of a chamber 260 , limited by a circular envelope 261, secured to the support 22, by a fixed wall 262 also secured to the support 22, and by a movable wall 263, secured to the support arm 27. The volume of the chamber 260 is thus a function of the angular position of the support arm 27 relative to the support 22. Since the length of the arm 27 is fixed, the volume of the chamber 260 is also a function of the spacing of the end 24 of this arm relative to the support 22.
It is therefore sufficient that the chamber 260 is filled with a fluid, for example hydraulic, and is connected to a volume measuring device (not shown) to obtain a signal indicative of the spacing of the end 24 of the arm of support 27 relative to support 22.
The arm 29 also includes a device 28 for measuring force.
In the schematic representation illustrated, this device 28 consists of a chamber 281, integral with the element 291 of the actuating arm, and a piston 282 integral with the element 292 of the actuating arm, the piston 282 cooperating with the chamber 281. During a force exerted on the element 292 of the actuating arm in the direction of the arrow 293, the element 292 which pivots freely around the articulation 25 transmits this force to the element 291 of the actuating arm, integral with the support arm 27. The force is transmitted by the action of the piston 282 on the fluid contained in the chamber 281.
It is therefore sufficient for a pressure measuring device (not shown) to be connected to the chamber 281 to obtain a quantity representative of the force exerted on the element 292 of the actuating arm 29, and therefore of the force transmitted to the support arm 27.

Finally, with reference to the embodiment illustrated in FIG. 3, which constitutes a preferred embodiment of the invention, the clamp 31 consists of a branch 32 constituting support and of a branch 33 constituting inter-support lever . As in the embodiment illustrated in FIG. 2, the branch 32 constituting the support is rectilinear, while the branch 33 is bent and is articulated, in line with the elbow, to the branch 32 by the articulation 35. This branch 35 is consisting of a support arm 37 and an actuating arm 39, arranged on either side of the elbow.
The clamp 31 comprises, at the location of the articulation 35, a potentiometer 36 constituting a device sensitive to angular displacements. This potentiometer is formed by a conductive track 361 secured to the branch 33 and a cursor 362 secured to the branch 32. Any pivoting of the branch 33 relative to the branch 32 thus results in a displacement of the cursor 362 on the track 361, and therefore by modifying the resistance of the potentiometer. It is thus possible to calibrate the potentiometer so that the measured value is converted into distance or spacing of the end 34 of the support arm 37 relative to the support branch. 32. For the ease of this conversion, the arm 37 extends radially with respect to the articulation 35.
This clamp 31 also includes a force measurement device 38 in the support arm 37. This force measurement device consists of a strain gauge 381 secured to the arm 37. Given the relatively low stresses to be measured, according to the embodiment illustrated in fig. 3 the arm 37 is sectioned at 40, and the two parts of the arm are joined by a metal piece 382 of small thickness, to which the strain gauge 381 is fixed. The metal piece 382 is fixed to the two parts of the arm 37 by screws 383, 383 ', the entire force measuring device, including the slot 40, being embedded in an elastomeric material.
The signals from the strain gauge 381 and the potentiometer 36 are routed by wires through the branch 39. They can then be routed to measuring devices, or even to an information processing unit, to determine , depending on the measurements made, the biomechanical parameters of the

The clamp 31 illustrated in FIG. 3 further comprises an elastic device 41 making it possible to transform the clamp into a rehabilitation device. This elastic device 41 consists of a threaded rod 410 articulated on a pivot 411 secured to the branch 33 and passing through a guide 412 secured to the support branch 32. A spring 413 is compressed between a plug 414 screwed onto the threaded rod 410 and guide 412. The plug 414 can of course be unscrewed to a position where it exerts no action on the spring, so that the clamp can be used exclusively as a measuring instrument. Finally, a stop 415 makes it possible to limit the action of the spring 413 to part of the stroke of the clamp 31, the compression of the spring 413 by the plug 414 determining the importance of the force to be exerted on this part of the gripper stroke.
It should be noted that the threaded rod, the direction of which is determined by the pivot 411 and the guide 412, passes in the immediate vicinity of the articulation 35, and so do the variations in distance between the pivot 411 and the guide 412, and consequently the compression stroke of the spring are very small, which guarantees a substantially constant force over the entire compression stroke of the spring 413 resulting from the movement of the clamp.
For obvious anatomical reasons, the lateral edges as well as the end of the support branch 32 and of the support arm 37 are rounded.
When using the clamp 31 as an instrument for determining biomechanical parameters of the pelvic floor, the plug 414 is unscrewed until no more compression is exerted on the spring 413. The clamp is then introduced into the vagina and positioned so that the support branch 32, coming into contact with the parietal tissue, is supported on the pubic symphysis. In this position, an action is exerted on the actuating arm 39 in the direction of the support 32.
This action causes the branch 33 to pivot around the articulation 35 and a concomitant separation of the support arm 37 away from the support branch 32.
