Abstract
본 발명은 신규하게 동정된 vMSC의 분비체 및 이의 용도에 관한 것으로, 본 발명의 특정 배양 조건으로 배양하여 수득한 vMSCs는 종래의 MSCs와 달리 CD141 세포 표면 항원을 발현하는 신규한 MSCs이고, vMSC가 분비하는 분비체의 구성이 종래의 MSC들과 상이하며, 다른 줄기세포들의 혈관 발아를 현저히 증가시키므로, 이를 줄기세포의 신생혈관 촉진 용도 및 혈관 질환 또는 혈관 기능 장애의 예방 또는 치료 용도로 활용할 수 있다.
Description
본 발명은 신규하게 동정된 vMSC의 분비체 및 이의 용도에 관한 것이다.
새로운 혈관은 혈관신생(angiogenesis), 동맥신생(arteriogenesis) 및 맥관형성(vasculogenesis) 과정에 의해 생성되는데, 혈관신생 과정은 이미 존재하는 성숙한 혈관내피세포(endothelial cell)들로부터 자라나는 혈관내피세포의 증식 및 이동과 관련이 있으며, 동맥신생은 이미 존재하는 소동맥 연결(arteriolar connection)을 측부혈관(collateral vessels)으로 리모델링하는 과정이고, 맥관형성은 혈관내피전구세포(endothelial progenitor cells)가 성숙한 혈관 내피세포로 분화되는 과정을 통해 진행된다. 따라서 골수로부터 나와서 순환하는 혈관내피전구세포는 혈관 손상 부위로 이동하고, 새롭게 형성되는 혈관으로의 직접적인 삽입 또는 다양한 신생혈관 형성 및 영양 인자들의 분비를 통해 새로운 혈관 형성에 관여한다. 따라서 혈관내피전구세포는 재생의학 분야 및 재혈관신생을 통한 치료목적에 잠재적인 표적으로 주목받고 있다. 이에 혈관내피전구세포의 기능을 증가시키기 위한 연구가 진행되고 있는데, 일 예로 생체 외(ex vivo) 유전자를 변형시킨 재조합 EPCs의 제조에 대한 연구가 진행된 바 있고, 혈관내피 성장인자(vascular endothelial growth factor, VEGF) 또는 저산소증 유도인자-1α(hypoxiainducible factor-1α)를 이용하여 EPCs의 혈관신생능(proangiogenic capacity)을 증가시키는 기술이 연구된 바 있다. 현재 허혈성 혈관 질환에서의 세포치료로 이용되는 것은 크게 2 가지 세포로 나누어진다. 혈관 재생 역할을 하는 혈관내피전구세포 (Endothelial progenitor cell; EPC)와 혈관 생성에 기여하는 성장인자(growth factor)를 다량으로 분비하여 간접적으로 혈관 형성의 도움을 주는 측분비 효과(paracrine effect) 방식의 중간엽줄기세포(Mesenchymal stem cell)가 있다. 그러나, 기존 EPC는 in vitro 세포 확장이 매우 어렵고 또한 세포 확장 중 기능을 유지하기가 어려우며, 동종 및 이종의 경우 면역 거부 등의 문제가 높은 세포군이고, 복잡한 구조의 혈관으로의 분화 및 기능이 어려운 한계가 있다. 또한, 허혈성 질환을 타겟으로한 기존 중간엽줄기세포 기반으로 개발된 치료제는 생착되지 않고 소실되기 때문에 혈관생성에 의한 치료방식이 아닌 측분비 효과에 의존하므로 임상에서 뚜렷한 치료 효과를 나타내지 못하고 있는 실정이다.
줄기세포 치료제를 포함한 세포치료제(Cell therapy product)란 세포와 조직의 기능을 복원시키기 위하여 살아있는 자가(autologous), 동종(allogeneic) 또는 이종(xenogeneic) 세포를 체외에서 증식 또는 선별하거나 여타한 방법으로 세포의 생물학적 특성을 변화시키는 등의 일련의 행위를 통하여 치료, 진단 및 예방의 목적으로 사용하는 의약품으로 정의된다. 세포치료제를 이용한 세포치료 이식술에서는 신체 내 정상적 혹은 건강한 상태와 유사한 조직으로서 형질 발현된 신체를 구성하는 각종의 조직을 대신 할 수 있는 다수의 세포들로 구성되는 각 세포 재료를 분리, 준비하고, 체외 환경에서 배양과정을 통해, 세포의 생물학적 특성을 변화시키는 일련의 행위를 가하여, 이식물을 수령할 개체에게 주입하거나, 이들 세포로 인체조직을 만듦으로써 치료용으로 쓰이게 된다. 줄기세포 치료제는 세포치료제 중 특히 줄기세포를 사용하는 경우를 의미하며, 현재 대표적인 응용 분야는 신경질환, 심장질환, 폐질환, 간질환, 암과 같이 손실된 세포의 회복과 재생이 필수적이나 잘 재생되지 않는 분야에서 활발하게 연구가 진행되고 있다. 줄기세포는 다분화 잠재능을 가지고 있어서 손상된 조직에서 필요한 세포로의 분화를 유도할 수 있다는 점에서 세포치료에서 잠재력이 크지만, 현재 개발수준에서는 체내 이식 후 생존율이 높지 않아서 실제로 임상 적용에서 광범위하고 안정적으로 성공한 예를 찾기 어렵다.

본 발명에서 사용된 용어 "생분해성 고분자"는 생체 내에서 일정 기간 후 자발적으로 서서히 분해되는 것으로, 생체적합성, 혈액친화성, 항(抗)석회화 특성, 세포의 영양성분 및 세포간 기질 형성능 중에서 하나 이상의 특성을 갖춘 고분자를 말한다. 이러한 생분해성 고분자는 본 발명에서 특별히 그 종류를 한정하지는 않으나, 대표적으로 피브린, 콜라겐, 젤라틴, 키토산, 알지네이트, 히알루론산, 덱스트란, 폴리락트산, 폴리글리콜산(poly(glycolic acid), PGA), 폴리(락트산-co-글리콜산)(poly(lactic-co-glycolic acid), PLGA), 폴리-ε-(카프로락톤), 폴리안하이드리드, 폴리오르토에스테르, 폴리비닐알코올, 폴리에틸렌글리콜, 폴리우레탄, 폴리아크릴산, 폴리-N-이소프로필아크릴아마이드, 폴리(에틸렌옥사이드)-폴리(프로필렌옥사이드)-폴리(에틸렌옥사이드) 공중합체, 이들의 공중합체, 이들의 혼합물 등이 사용될 수 있다. 상기 복합 지지체는 기존의 공지된 방법, 예를 들어, 염 침출법(solvent-casting and particle-leaching technique), 가스발포법(gas forming technique), 고분자 섬유를 부직포로 만들어 고분자 메쉬(mesh)로 제조하는 방법(fiber extrusion and fabric forming process), 상분리법(thermally induced phase separation technique), 유화 동결 건조법(emulsion freeze drying method), 고압 기체 팽창법(high pressure gas expansion) 등에 따라 생분해성 고분자를 성형하여 제조할 수 있다.

본 발명의 약학적 조성물은 약학적으로 유효한 양으로 투여한다. 본 발명에서 사용되는 용어, "약학적으로 유효한 양"은 의학적 치료에 적용 가능한 합리적인 수혜/위험 비율로 질환을 치료하기에 충분하며 부작용을 일으키지 않을 정도의 양을 의미하며, 유효 용량 수준은 개체의 건강상태, 혈관 질환 또는 혈관 기능 장애의 종류, 중증도, 약물의 활성, 약물에 대한 민감도, 투여 방법, 투여 시간, 투여 경로 및 배출 비율, 치료기간, 배합 또는 동시 사용되는 약물을 포함한 요소 및 기타 의학 분야에 잘 알려진 요소에 따라 결정될 수 있다. 본 발명의 조성물은 개별 치료제로 투여하거나 다른 치료제와 병용하여 투여될 수 있고, 종래의 치료제와 순차적으로 또는 동시에 투여될 수 있으며, 단일 또는 다중 투여될 수 있다. 상기한 요소들을 모두 고려하여, 부작용없이 최소한의 양으로 최대 효과를 얻을 수 있는 양을 투여하는 것이 중요하며, 이는 당업자에 의해 용이하게 결정될 수 있다.
본 발명의 약학적 조성물은 생물학적 제제에 통상적으로 사용되는 담체, 희석제, 부형제 또는 둘 이상의 이들의 조합을 포함할 수 있다. 본 발명에서 사용되는 용어, "약학적으로 허용가능한"이란 상기 조성물에 노출되는 세포나 인간에게 독성이 없는 특성을 나타내는 것을 의미한다. 상기 담체는 조성물을 생체 내 전달에 적합한 것이면 특별히 제한되지 않으며, 예를 들면, Merck Index, 13th ed., Merck & Co. 본 발명의 조성물에 추가로 동일 또는 유사한 기능을 나타내는 유효성분을 1종 이상 함유할 수 있다.
본 발명의 약학적 조성물은 약제학적으로 허용 가능한 첨가제를 더 포함할 수 있으며, 이때 약제학적으로 허용 가능한 첨가제로는 전분, 젤라틴화 전분, 미결정셀룰로오스, 유당, 포비돈, 콜로이달실리콘디옥사이드, 인산수소칼슘, 락토스, 만니톨, 엿, 아라비아고무, 전호화전분, 옥수수전분, 분말셀룰로오스, 히드록시프로필셀룰로오스, 오파드라이, 전분글리콜산나트륨, 카르나우바 납, 합성규산알루미늄, 스테아린산, 스테아린산마그네슘, 스테아린산알루미늄, 스테아린산칼슘, 백당, 덱스트로스, 소르비톨 및 탈크 등이 사용될 수 있다. 본 발명에 따른 약제학적으로 허용 가능한 첨가제는 상기 조성물에 대해 0.1 중량부 내지 90 중량부 포함되는 것이 바람직하나, 이에 한정되는 것은 아니다.
본 발명의 조성물은 목적하는 방법에 따라 비 경구 투여(예를 들어 정맥 내, 피하, 복강 내 또는 국소에 적용)하거나 경구 투여할 수 있으며, 투여량은 환자의 체중, 연령, 성별, 건강상태, 식이, 투여시간, 투여방법, 배설률 및 질환의 중증도 등에 따라 그 범위가 다양하다. 본 발명에 따른 조성물의 일일 투여량은 0.0001 ~ 10 ㎎/㎖이며, 바람직하게는 0.0001 ~ 5 ㎎/㎖이며, 하루 일 회 내지 수회에 나누어 투여하는 것이 더욱 바람직하다.
본 발명의 조성물의 경구 투여를 위한 액상 제제로는 현탁제, 내용액제, 유제, 시럽제 등이 해당되는데, 통상적으로 사용되는 단순 희석제인 물, 액체 파라핀 이외에 다양한 부형제, 예컨대 습윤제, 감미제, 방향제, 보존제 등이 함께 포함될 수 있다. 비경구 투여를 위한 제제에는 멸균된 수용액, 비수성 용제, 현탁제, 유제, 동결건조 제제, 좌제 등이 포함된다.
일 측면에서, 본 발명은 CD141 세포 표면 항원을 발현하는 줄기세포를 인간 혈소판 용해물(human platelet lysate)을 포함하고 혈청을 포함하지 않는 배지에서 배양하는 단계를 포함하는, vMSCs 유래 분비체의 제조방법에 관한 것이다.
일 구현예에서, 상기 배양하는 단계 이후에 배양액을 수집하는 단계 및 수집한 배양액을 원심분리하여 배양 상등액을 수집하는 단계를 추가로 포함할 수 있다.

하기의 실시예를 통하여 본 발명을 보다 상세하게 설명한다. 그러나 하기 실시예는 본 발명의 내용을 구체화하기 위한 것일 뿐 이에 의해 본 발명이 한정되는 것은 아니다.
실시예 1. 골수 유래 신규한 다분화능 줄기세포의 분리
1-1. vMSCs 최적 배양
냉동 보존된 인간 골수 단핵세포(bone marrow mononuclear cells, cryopreserved, BM-MNCs) (STEMCELL Technologies, Vancouver, Canada, LONZA, Basel, Switzerland)를 37 ℃에서 해동하거나, 또는 환자의 동의 하에 얻은 골수 흡인액을 Ficoll-paque (Cytiva, MA, USA)로 밀도 구배 원심 분리하여 BM-MNC를 얻는다. 이렇게 얻은 BM-MNC를 TC-treated(Tissue culture treated) 플라스크에 2% 인간 PL(human platelet lysate) (PL bioscience, GmbH, Germany) 및 2 unit/mL 헤파린(heparin) (Sigma-Aldrich, St. Louis)으로 보충된 FBS(Fetal bovine serum)-결핍된 EGM-2 bulletkit medium (Lonza; Basel, Switzerland) (EGM-2-FBS+2%hPL)을 이용하여 1.0 내지 2.0 × 105 cells/cm2의 밀도로 분주하고 7-12일 동안 초대 배양하였다. 초대 배양 기간 동안 배지 교체는 1-4일에 한번 수행하였다. 본 발명의 신규한 다분화능 줄기세포인 vMSCs(Vasculogenic Multipotent Stem Cells, vMSCs)의 콜로니는 배양 3 내지 5일부터 관찰되기 시작하며, 초대 배양시에 관찰되는 콜로니를 이루는 vMSCs 세포는 방추(spindle) 형태로 길이와 크기가 매우 다양하고 7일에서 10일 사이에 콜로니가 현저하게 확장되는 것으로 관찰되었다. vMSCs의 최초 계대배양 시기는 대다수의 콜로니가 80-90% 이상의 밀도를 보일 때 진행하며, 일반 부착세포를 계대배양하는 방법과 동일하게 트립신 또는 트립신 대체재를 이용하여 단일 세포로 분리 후에 새 플라스크로 1.0 내지 5.0 × 103 cells/cm2로 분주하고 70-90%의 밀도일 때 계대배양을 진행하며 5-7번 계대배양이 가능하다. 이와 같이, 본 발명의 vMSCs는 EGM-2 kit의 기본 구성성분 그대로 사용하여 FBS가 포함된 EGM-2로 배양하는 EPC(endothelial progenitor cell)와 달리, EGM-2에서 FBS를 배제하고 인간 PL(human platelet lysate)을 첨가하여 배양하는 것에 특징이 있다.
1-2. vMSCs의 세포외기질 부착 특성 확인
인간 섬유아세포(Human fibroblast) 유래 ECM 복합체인 HumaTein (100 μg/mL, ROKIT healthcare, Seoul, Republic of Korea), 인간 1형 콜라겐(human type I collagen) (0.2 μg/cm2)(Corning, NY, USA), 피브로넥틴(Fibronectin) (1 μg/cm2) (Sigma-Aldrich), 돼지 콜라겐 1-P형(Cellmatrix Type I-P) (Nitta Gelatin, Osaka, Japan), 래트 꼬리유래 1형 콜라겐(Collagen type I from rat tail) (50-825 μg/mL) (ThremoFisher, MA, USA) 및 콜라겐 1형 펩타이드(Corning) 등 다양한 세포외기질(Extracellular matrix, ECM)로 코팅된 배양 플라스크에 vMSCs를 배양하여 확인한 결과, vMSC의 콜로니를 구성하는 세포는 모든 코팅 재료에서 방추 형태로 동일하게 나타났으며, 다양한 세포외기질에서 배양이 가능한 것으로 나타났다. 상기와 같은 ECM 코팅된 플라스크에서 배양하면 일반 TC-treated 플라스크에서 보다 더 많은 콜로니를 얻을 수 있었으나, 유의미한 차이는 없는 것으로 나타났다 (도 1). 즉, ECM 코팅 유무 및 종류에 따라 초대 배양 수득율은 다를 수 있으나, P1 이후 증식률에는 코팅 종류 및 유무에 따른 차이가 거의 없었다.
본 발명의 신규한 vMSCs는 인간 PL 대신에 기존 EGM-2 bullet kit의 FBS를 이용하여 배양하면 CD141+vMSC의 특성을 유지할 수 없으며, 인간 PL의 추가로 인해 CD141의 발현이 더욱 증가하므로, 이의 배양액은 EGM-2 bullet kit (Lonza)에 인간 PL을 추가하고, FBS를 배제하는 것을 필수로 하고, IGF 및 VEGF를 제거해도 성장에 문제가 없으므로, IGF 및 VEGF를 제외할 수 있다.
실시예 2. vMSCs의 마커 특성 분석
2-1. EPC 및 혈관내피세포 마커 발현 확인
본 발명의 vMSC에서의 종래의 EPC 및 혈관내피세포의 마커 발현 여부를 유세포 분석으로 확인하고, 성숙한혈관내피세포가 발현하는 eNOS 및 VE-cadherin의 발현을 면역형광염색 분석으로 확인하였으며, 이를 BM-MSC(bone marrow mononuclear cells) 및 HUVEC(Human umbilical cord endothelial cell)와 각각 비교하였다. 구체적으로, 유세포 분석을 위해, vMSC 및 BM-MSC를 2mM EDTA 및 0.5% BSA(Bovine serum albumin) (Sigma-Aldrich)를 포함하는 PEB 버퍼에 각각 재현탁한 후, human FcR blocking reagent (Miltynyi biotec)으로 4 ℃에서 10 분간 블라킹하였다. 그 후 Alexa flour 488 결합된 vWF 항체 (Abcam, 1:500), fluorescein으로 표지된 UEA-1 (Vectorlabs, 3:100), APC(allophycocyanin)-결합된 CD29, CD31, CD34, CD44, CD45, CD73, CD90, CD105 및 CD309 항체 (Miltenyl Biotec, 1:50)로 각각 염색하였다. 이 때, APC (Miltenyl Biotec. 1:50) 및 Alexa flour 488 (Abcam, 1:500)가 결합된 Isotype IgG를 대조 그룹으로 사용하였다. 항체 염색 후 세포를 PEB 버퍼로 세척하고 1,000 ×g 및 4 ℃에서 5분간 원심분리하였다. 최종적으로 세포를 PEB 버퍼로 재현탁하고 NovoCyte 3000 유세포분석기 (Agilent Technologies, Santa Clara, CA, USA) 및 NovoExpress 소프트웨어를 이용하여 분석하였다. 또한, 면역형광염색 분석을 위해, vMSC 및 HUVEC을 각각 커버 슬립 위에 배양하고 3.7% 포름알데하이드 (Sigma-Aldrich)로 고정한 후 0.2% Triton X-100으로 세포막을 침투시켰다. 블라킹을 위해 20% NGS(normal goat serum)으로 1시간 동안 상온에서 반응시키고, 1차 항체로서 20% NGS에 희석한 e-NOS (Cell Signaling Technology, MA, USA, 1:400) 및 VE-cadherin (Cell Signaling Technology, 1:400)와 각각 4℃에서 하룻밤 동안 반응시켰다. 다음날 20% NGS에 희석한 alexa fluor 488 결합한 항-래빗 2차 항체 (Invitrogen, MA, USA, 1:1000)와 1시간 동안 상온에서 반응시켰다. 샘플 관찰을 위해 DAPI가 포함된 마운팅 용액 (Vector Laboratories, CA, USA)로 마운팅한 후, 형광 현미경 (Leica)으로 이미지를 수득하였다.
유세포 분석 결과, 본 발명의 vMSC는 MSC 마커로 알려져 있는 CD105, CD90, CD29, CD73 및 CD44를 모두 발현하고 있으며, 기존에 보고된 EPC가 공통적으로 발현하는 CD31, CD309 및 CD34를 발현하지 않는 것으로 나타났다 (도 2A). 또한, 면역형광염색 분석 결과, 본 발명의 vMSC는 양성대조 그룹인 HUVEC과 달리 eNOS 및 VE-cadherin을 발현하지 않는 것으로 나타났다 (도 2C). 이를 통해, vMSC는 EPC보다 MSC와 더 많은 마커를 공유하는 것을 알 수 있다.
2-2. vMSCs 특이적 신규 마커 발굴
BM-MSC와 구별되는 본 발명의 vMSC 특이적 마커를 발굴하기 위하여 마커 스크리닝을 진행하였다. 이를 위해 378개의 표면항원마커를 유세포 분석기로 분석할 수 있는 MACS®Marker Screen, human, version 02 (Miltenyi Biotec)를 사용하였고, 제조사의 지침에 따라 실험을 수행하였다. 구체적으로, 동일한 공여자에서 유래한 Passage 3의 vMSC (5.7x107) 및 BM-MSC (5.7x107)를 PEB 버퍼로 재현탁한 후 human FcR blocking reagent (Miltenyi Biotec)로 4℃에서 10분간 블라킹하였다. 그 후 6개의 Isotype control과 378개의 항체가 들어있는 96웰 플레이트 4개에 1.2x105/well로 각각 분주한 후 4℃에서 20-30분간 반응시켰다. PEB 버퍼로 세척 후 300 xg에서 5분간 원심분리한 후, PEB 버퍼를 이용하여 200 μl/well 농도로 재현탁하여 NovoSampler (Agilent Technologies)를 장착한 NovoCyte 3000 유세포분석기 (Agilent Technologies)와 NovoExpress 소프트웨어를 이용하여 분석하였다.
마커 스크리닝 결과, vMSC와 BM-MSC 간에 차이가 나는 약 21개의 마커를 선정하였고 (도 3A), 이의 검증을 위해, APC 결합된 CD141, CD282, PEAR1 (Miltenyi biotec. 1:50)을 사용하여 10명 이상의 도너에서 유래한 vMSC 및 BM-MSC에서 CD141, CD282 및 PEAR1(Platelet Endothelial Aggregation Receptor 1)에 대해 유세포분석을 수행하였다. 그 결과, CD141은 지속적으로 vMSC에서 70 내지 99% 발현하는 것으로 나타났으나, BM-MSC에서는 발현되지 않는 것으로 나타났다 (도 3B). 또한, CD282는 BM-MSC에서 발현되지 않고, vMSC에서 적게는 30%에서 많게는 90%까지 발현이 되어 공여자 간 편차가 큰 것으로 나타났다 (도 3C). 아울러, PEAR1은 vMSC에서 70~99% 발현되나, BM-MSC의 경우 적게는 20%에서 많게는 90%까지 발현되는 것으로 나타나, 공여자 간 편차가 매우 큰 것으로 나타났다 (도 3D). 이를 통해, 본 발명의 vMSC와 BM-MSC를 구분하는 명확한 마커는 CD141인 것으로 나타났다.
상기 결과들에서와 같이, 본 발명에서 동정한 다분화능 및 신생혈관 촉진능을 가지는 줄기세포 CD141+vMSC가 EPC와 달리 CD31, CD309 및 CD34를 발현하지 않으며, 혈관내피세포와 달리 eNOS 및 VE-cadherin을 발현하지 않고, 중간엽 줄기세포와 달리 CD141을 발현하는 특징을 가지므로, 이를 CD141+ Vasculogenic Multipotent Stem Cells로 명명하여 한국세포주연구재단(KCLRF)에 기탁하고 KCLRFBP00524호 기탁번호를 부여받았다.
실시예 3. vMSC의 분비체 분석
3-1. Human Angiogenesis Array
vMSC(CD141+vMSC) 또는 BM-MSC가 분비하는 분비체(secretome)를 분석하기 위해, 상기 실시예의 배양 방법으로 골수 유래의 vMSC(CD141+vMSC) 및 골수 유래의 MSC(BM-MSC)를 각각 배양하고 이의 분비체를 이용하여 Human Angiogenesis Array를 수행하였다. 구체적으로, 계대 3의 vMSCs 및 BM-MSC를 T-75 플라스크에 각각 1.0x105개 분주하여 각각 EGM2-2%FBS+2%hPL 배지 및 StemMACS™ MSCS 확장(expansion) 배지로 배양한 뒤, 세포 배양 상등액을 배양 4일차에 수집하였다. 이후 Human Angiogenesis Array (R&D Systems, ARY007, Minneapolis, USA)를 제조사의 지침에 따라 수행하였다. 대조군으로는 각각의 배양배지 (BM-MSC의 배양배지: StemMACS™ MSCS expansion media, 및 vMSC의 배양배지: EGM2-2%FBS+2%hPL)를 이용하였으며, 대조군에 비해 vMSC 배양 상등액 또는 BM-MSC 배양 상등액에서 현저히 증가하는 인자들을 확인하였다. Image J 프로그램의 protein array analyzer를 이용하여 상기 분석 결과의 블롯을 정량적으로 분석하였다.
그 결과, vMSC(CD141+vMSC)의 배양 상등액에서 Endoglin, HGF, IGFBP-2, IL-8, Pentraxin 3, TIMP-1 및 uPA가 대조군 배양배지에 비해 현저히 높은 수준으로 나타났고, 특히 HGF는 6.5배, IL-8는 80.3배, Pentraxin 3는 27.1배 및 uPA는 15배 높은 수준으로 나타났다 (도 4). 또한, BM-MSC의 배양 상등액에서 Angiogenin, Endoglin, HGF, IGFBP-2, IGFBP-3, IL-8, Pentraxin 3, TIMP-4, uPA 및 VEGF가 대조군 배양배지에 비해 현저히 높은 수준으로 나타났고, 특히 Endoglin은 4.6배, IBFBP-2와 IL-8은 3.9배, uPA는 39.7배 및 VEGF는 6.5배 높은 수준으로 나타났다 (도 5).
3-2. ELISA 분석
상기 실시예 3-1에서 확인된 vMSC(CD141+vMSC) 또는 BM-MSC가 분비하는 분비체에서, 혈관신생에 관여하는 인자들 중 HGF 및 VEGF의 수준을 확인하기 위해, 상기 실시예 3-1에서와 같이 vMSC(1.3x105)와 BM-MSC(2x105)를 배양하고 배양 4 또는 5일차에 세포 배양 상등액을 각각 수집하여 세포를 계수하여 세포수를 반영하였다. 또한, vMSC와 BM-MSC를 동일하게 성장인자가 결여된 배양 조건에서 배양했을 때의 HGF 및 VEGF의 분비 수준을 확인하기 위해 계대 3의 vMSC 및 BM-MSC를 각각 6웰 플레이트에 9x104/well로 분주하고 성장인자가 부족한 배지로서 0.2%hPL (PL BioScience, Aachen, Germany)가 함유된 MEMα (Gibco, NY, USA)에서 배양하였다. 1일차에 새 배지로 교환한 후, 세포 배양 상등액을 각각 4일차에 수집하였다. 이 후, Quantikine ELISA (R&D systems, Minneapolis, USA)를 이용하여 제조사의 지침에 따라 수행하였으며, 흡광도를 마이크로 플레이트 리더 (Molecular Device, SpectraMax ABS)를 이용하여 450nm에서 측정하여, HGF 및 VEGF의 수준을 ELISA로 분석하였다.
HGF의 ELISA 분석 결과, vMSC의 배양 상등액에서 5,400pg/mL 내지 53,600pg/mL (평균 25,200pg/mL)의 HGF가 측정되었으며 (도 6A), HGF 농도를 세포수로 나눠 세포당 분비한 HGF의 양을 확인한 결과 vMSC는 104 세포당 340pg 내지 5,600pg (평균 1,700pg)의 HGF를 분비하는 것으로 나타났다 (도 6B). 또한, 성장인자가 부족한 배지를 이용하여 vMSC 및 BM-MSC를 동일한 조건으로 배양한 경우, vMSC의 배양 상등액에서 HGF가 10,600pg/mL로 측정되었고, BM-MSC의 배양 상등액에서는 vMSC의 배양 상등액에서보다 약 9배 낮은 1,100pg/mL로 측정되었다 (도 6C).
아울러, VEGF의 ELISA 분석 결과, BM-MSC의 배양 상등액에서 200pg/mL 내지 1,600pg/mL (평균 820pg/mL)의 VEGF가 측정되었으며 (도 7A), 세포당 분비한 VEGF의 양을 확인한 결과 104 세포당 VEGF를 55pg 내지 160pg (평균 100pg)로 분비하는 것으로 나타났다 (도 7B). 또한, 성장인자가 부족한 배지를 이용하여 vMSC 및 BM-MSC를 동일한 조건으로 배양한 경우, vMSC의 배양 상등액에서 VEGF가 330pg/mL로 측정되었으며, BM-MSC의 배양 상등액에서는 vMSC보다 약 2배 높은 620pg/mL로 측정되었다 (도 7C).
이를 통해, vMSC(CD141 양성 세포)는 HGF를 고농도로 분비하는 세포이고, BM-MSC는 VEGF를 고농도로 분비하는 세포인 것을 확인하였다. 또한, vMSC는 HGF 뿐만 아니라 혈관신생에 관여하는 IL-8, Pentraxin 3 및 uPA 또한 매우 높은 수준으로 분비하고, BM-MSC는 VEGF 뿐만 아니라 uPA를 매우 높은 수준으로 분비하는 세포임을 확인하였다.
실시예 4.
vMSC 분비체의 혈관내피세포의 세포 발아 촉진 효과
vMSC의 분비체에 의한 혈관내피세포의 신생혈관 형성 촉진 효과를 확인하기 위해, 행잉 드롭(Hanging drop) 방법으로 HUVEC 세포를 스페로이드로 배양하고, vMSC와 비접촉 공동배양한 뒤, 스페로이드 발아 분석(Sprouting assay)을 수행하였다. 구체적으로, 10% FBS 및 0.2% 메틸셀룰로오스가 포함된 M199 배지 (Sigma-Aldrich, St. Louis, MO, USA)에 HUVECs을 1.33 x 104 cells/mL로 현탁하고, 현탁된 세포를 페트리 접시 뚜껑에 30㎕씩 분주하였다. 페트리 접시 바닥에는 습도를 유지시키기 위하여 멸균 증류수를 넣고, 페트리 접시 뚜껑을 180도 뒤집어 멸균수가 든 페트리 접시 바닥에 올려 37℃ 및 5% CO2 조건에서 24시간 동안 배양함으로써 HUVEC 세포가 스페로이드(spheroid)를 형성하였다 (도 8의 Hanging drop). 이 후, HUVEC 스페로이드를 20% FBS 및 0.95% 메틸셀룰로오스가 포함된 M199 배지에 현탁하고, 30mg/mL의 type Ⅰ-A collagen Gelatin, 10x Medium (Sigma-Aldrich) 및 10x reconstitution buffer (0.05 N NaOH, 0.261 M NaHCO3 및 0.2M HEPES)를 8:1:1 (v/v)의 비율로 혼합한 Type Ⅰ 콜라겐 용액과 상기 현탁한 스페로이드를 1:1의 비율 (v/v)로 혼합하였다. 이를 24웰 플레이트에 0.7mL씩 분주한 후, 37℃ 및 5% CO2의 조건에서 30분간 중합하였다. 중합되어 HUVEC 스페로이드를 포함하는 콜라겐 젤 위에 M199 배지 100㎕에 희석한 vMSC를 1x105로 분주하여 24시간 동안 배양하였다. 이 때, vMSC의 분비체들 중 HGF를 중화하기 위해, 500 ng/mL의 HGF 중화 항체 (MAB294, R&D systems; Anti-HGF Ab)를 M199 배지에 혼합하여 중합되어 HUVEC 스페로이드를 포함하는 콜라겐 젤 위에 100㎕씩 처리하고 24시간 동안 배양하였으며, 이의 대조군으로는 마우스 IgG (IgG) (500ng/mL)를 사용하였다 (도 8의 sprouting assay). 스페로이드의 발아(sprouting) 이미지는 현미경 (Nikon)으로 촬영하여 얻었으며, Image J 소프트웨어 (NIH)를 사용하여 발아 수, 누적 발아 길이(하나의 스페로이드에서 형성된 모든 sprout 길이의 총합) 및 발아 평균 길이(모든 스페로이드에서 형성된 모든 발아 길이의 평균값)을 정량 분석하였다.
그 결과, CD141+ vMSC와 비 접촉 공동 배양 시 HUVEC 스페로이드에서 발아 촉진 효과가 있는 것으로 나타났으며, HGF 중화 항체 처리에 의해 HUVEC 발아 효과가 억제되는 것으로 나타났다 (도 9). 이를 통해, vMSC의 분비체 중의 HGF가 혈관내피세포 HUVEC의 세포 발아(cell sprouting) 효과를 나타내는, 즉, 신생혈관능을 촉진하는 물질임을 확인하였으며, HUVEC 발아 촉진에 HGF가 충분(sufficient)하지 않지만, vMSC에 의한 HUVEC 발아 촉진에 HGF가 필요(required) 하다고 판단할 수 있다.
따라서, 골수 유래 vMSC는 혈관 신생과 관련한 HGF를 과량으로 분비하고, 그 분비체는 HUVEC의 신생혈관 발아를 촉진하므로, vMSC를 무혈청 배양 배지에서 배양하여 분리한 분비체(secretome)를 혈관재생촉진용으로 사용할 수 있다.
실시예 5. vMSC 분비체의 자가분비 효과
vMSC의 분비체에 의한 vMSC의 신생혈관 형성 촉진 효과, 즉 자가분비 효과(autocrine effect) 를 확인하기 위해, 행잉 드롭 방법으로 vMSC 세포를 스페로이드로 배양하고, vMSC와 비접촉 공동배양한 뒤, 스페로이드 발아 분석을 수행하였다. 구체적으로, 10% FBS 및 0.2% 메틸셀룰로오스가 포함된 M199 배지 (Sigma-Aldrich, St. Louis, MO, USA)에 vMSC를 2 x 104 cells/mL로 현탁하고, 현탁된 세포를 페트리 접시 뚜껑에 30㎕씩 분주하였다. 페트리 접시 바닥에는 습도를 유지시키기 위하여 멸균 증류수를 넣고, 페트리 접시 뚜껑을 180도 뒤집어 멸균수가 든 페트리 접시 바닥에 올려 37℃및 5% CO2 조건에서 24시간 동안 배양함으로써 vMSC 세포가 스페로이드를 형성하였다 (도 10의 Hanging drop). 이 후, vMSC 스페로이드를 0.95% 메틸셀룰로오스가 포함된 M199 배지에 현탁하고, 30mg/mL의 type Ⅰcollagen Gelatin, 10x Medium (Sigma-Aldrich) 및 10x reconstitution buffer (0.05 N NaOH, 0.261 M NaHCO3 및 0.2M HEPES)를 8:1:1의 비율로 혼합한 Type Ⅰ콜라겐 용액과 상기 현탁한 스페로이드를 1:1의 비율로 혼합하였다. 이를 24웰 플레이트에 0.7mL씩 분주한 후, 37℃및 5% CO2의 조건에서 30분간 중합하였다. vMSC 스페로이드를 포함하여 젤화된 젤에 혈관신생 촉진 인자들로서 HGF 또는 VEGF를 농도별로 M199 배지 100㎕에 희석하여 처리하고 48시간 동안 배양하였다. 또는, 혈관신생 촉진 인자들 대신에 vMSC를 분주하여 vMSC 스페로이드와 비접촉 공동 배양하면서, vMSC의 분비체 중 하나인 HGF를 중화하기 위해, 500 ng/mL의 HGF 중화 항체 (MAB294, R&D systems; Anti-HGF Ab)를 M199 배지에 혼합하여 처리하였다 (도 10의 sprouting assay). 스페로이드의 발아(sprouting) 이미지는 현미경 (Nikon)으로 촬영하여 얻었으며, Image J 소프트웨어 (NIH)를 사용하여 발아 수, 누적 발아 길이(하나의 스페로이드에서 형성된 모든 sprout 길이의 총합) 및 발아 평균 길이(모든 스페로이드에서 형성된 모든 발아 길이의 평균값)을 정량 분석하였다.
그 결과, vMSC 스페로이드는 bFGF에 반응하여 발아가 촉진되었으나, VEGF에는 반응하지 않았으며 (도 11), HGF 20 ng/mL 및 50 ng/mL 농도에서 발아가 촉진되는 것으로 나타났다 (도 12). 또한, vMSC 스페로이드가 vMSC와 비접촉 공동배양되어 HGF가 풍부한 것으로 확인한 vMSC 분비체와 접촉했을 때 vMSC 스페로이드의 발아가 촉진되었으며, HGF 중화 항체에 의해 증가된 발아가 억제되어 (도 13), vMSC의 분비체에 의해 촉진된 vMSC 스페로이드의 발아 능력이 분비체 중 HGF에 의한 것임을 유추할 수 있으며, HGF가 vMSC의 자가분비 효과를 내는 성장인자임을 알 수 있다.
따라서, 골수 유래 CD141+vMSC는 혈관신생과 관련된 HGF를 과량으로 분비하고, 그 분비체가 CD141+vMSC의 발아를 촉진시키므로 CD141+vMSC를 무혈청 배양 배지에서 배양하여 분리한 분비체를 다양한 혈관재생용 줄기세포치료제(CD141+vMSC 또는 EPC)의 첨가제(adjuvant)로 사용할 수 있다.
실시예 6. BM-MSC 분비체의 측분비 효과
BM-MSC의 분비체에 의한 vMSC의 신생혈관 형성 촉진 효과, 즉 측분비 효과(paracrine effect) 를 확인하기 위해, 상기 실시예 5에서와 같이 행잉 드롭 방법으로 vMSC 세포를 스페로이드로 배양하고 (도 14의 Hanging drop), BM-MSC와 비접촉 공동배양한 뒤, 스페로이드 발아 분석을 수행하였다 (도 14의 sprouting assay). 구체적으로, vMSC 스페로이드를 포함하여 젤화된 젤에 BM-MSC를 3x104개 포함하는 M199 배지 100㎕를 분주하여 48시간 동안 vMSC 스페로이드와 비접촉 공동 배양하거나, bFGF (25 ng/mL)를 처리하였다. 이 때, BM-MSC의 분비체 중 하나인 bFGF를 억제하기 위해 bFGF 신호전달계 억제제인 PD173074 (Tocris, Bristol, UK) (10nM)를 3시간 전에 처리하였다. 스페로이드의 발아 이미지는 현미경 (Nikon)으로 촬영하여 얻었으며, Image J 소프트웨어 (NIH)를 사용하여 발아 수, 누적 발아 길이(하나의 스페로이드에서 형성된 모든 sprout 길이의 총합) 및 발아 평균 길이(모든 스페로이드에서 형성된 모든 발아 길이의 평균값)을 정량 분석하였다. 아울러, vMSC 및 BM-MSC에서 bFGF의 mRNA 발현 정도를 비교하기 위해 각 세포에서 총 RNA를 TRIzol reagent (Invitrogen)을 사용하여 분리하고, SuperScript ⅢReverse Transcriptase (Invitrogen)을 사용하여 cDNA를 합성한 뒤, SYBR Green reagent (Invitrogen)을 사용하여 RT-PCR 분석을 수행하였다. 이 때, 인간 RPS9(ribosomal protein S9 gene)을 내재적 대조군으로 사용하였다.
그 결과, vMSC(vMSCs) 및 BM-MSC(MSCs)의 bFGF mRNA 수준을 비교한 결과, BM-MSC에서 bFGF의 발현양이 vMSC에 비해 현저히 높게 나타났다 (도 15). 또한, bFGF에 의해 vMSC 스페로이드에서 발아가 촉진되었으며, 이는 PD173074를 3시간 전처리 하였을 때 억제되는 것으로 나타났다 (도 16). 아울러, vMSC 스페로이드와 BM-MSC(MSCs)를 비접촉 공동배양한 경우 BM-MSC의 분비체에 의해 vMSC 스페로이드의 발아가 촉진되었으며, 분비체 중 bFGF를 이의 억제제를 전처리하여 억제한 결과, vMSC 스페로이드의 발아가 억제되는 것을 확인하였다 (도 17).
이를 통해, 비접촉 공동배양에서 BM-MSC(골수 유래 중간엽 줄기세포)의 분비체가 CD141+vMSC 스페로이드의 발아를 촉진시키고 bFGF 신호전달계 억제에 의해 발아 촉진 효과가 억제되었으므로, vMSC의 혈관신생 효과에 BM-MSC가 분비하는 bFGF가 작동하는 것을 알 수 있다. 따라서, vMSC 스페로이드는 VEGF가 아닌 bFGF에 의해 발아가 촉진되며, bFGF를 분비하는 BM-MSC의 분비체가 vMSC에게 측분비 혈관신생 효과(paracrine angiogenic effect)를 제공하는 것을 확인하였다.
상기 실시예들을 통해, BM-MSC의 분비체 및 vMSC의 분비체는 각각 단독으로 또는 이들의 조합으로 혈관재생촉진용 의약품 및 다양한 혈관재생용 줄기세포치료제 (CD141+vMSC 또는 EPC)의 첨가제로 사용할 수 있다.

- 제 1항에 있어서, 골수 유래 중간엽 줄기세포(bone marrow-derived mesenchymal stem cells, BM-MSCs)에서 분비된 분비체 대비 HGF이 증가된, 분비체.
- 제 1항에 있어서, 골수 유래 중간엽 줄기세포에서 분비된 분비체 대비 VEGF이 감소된, 분비체.
- CD141 세포 표면 항원을 발현하는 줄기세포의 배양액을 유효성분으로 포함하는, 혈관 재생 또는 혈관 형성용 약학적 조성물.
- 제 7항에 있어서, CD141 세포 표면 항원을 발현하는 줄기세포는 수탁번호 KCLRF-BP-00524로 기탁된 혈관형성 다능성 줄기세포인, 혈관 재생 또는 혈관 형성용 약학적 조성물.
- 제 7항에 있어서, 배양액은 CD141 세포 표면 항원을 발현하는 줄기세포를 무혈청 배지에서 배양하여 얻은 배양액인, 혈관 재생 또는 혈관 형성용 약학적 조성물.
- 제 7항에 있어서, 골수 유래 중간엽 줄기세포의 배양액을 추가로 포함하는, 혈관 재생 또는 혈관 형성용 약학적 조성물.
- 제 1항의 분비체를 포함하는, 혈관 재생 또는 혈관 형성용 약학적 조성물.
- 제 11항에 있어서, Endoglin, HGF, IGFBP-2, IL-8, Pentraxin 3, TIMP-1 또는 uPA를 포함하는, 혈관 재생 또는 혈관 형성용 약학적 조성물.
- 제 11항에 있어서, 세포 유래 단백질, 사이토카인, 성장인자, 엑소좀 또는 핵산을 포함하는, 혈관 재생 또는 혈관 형성용 약학적 조성물.
- 제 11항에 있어서, 골수 유래 중간엽 줄기세포에서 분비된 분비체를 추가로 포함하는, 혈관 재생 또는 혈관 형성용 약학적 조성물.
- 제 1항의 분비체 또는 CD141 세포 표면 항원을 발현하는 줄기세포의 배양액을 처리한 줄기세포.
- 제 15항에 있어서, 줄기세포는 심장 전구 세포(cardiac progenitor cell, CPC), EPCs(Endothelial Progenitor Cells), ECFCs(Endothelial Colony Forming Cells), VPCs(Vasculogenic Progenitor Cells), 중간엽 줄기세포(Mesenchymal Stem Cells), 배아줄기세포(Embryonic Stem Cells) 또는 근모세포(Myoblasts)인, 줄기세포.
- 제 16항에 있어서, 중간엽 줄기세포는 제대혈 유래 중간엽 줄기세포(umbilical cord blood-derived mesenchymal stem cells, UCB-MSC), 제대 유래 중간엽 줄기세포(umbilical cord-derived mesenchymal stem cells, UC-MSC), 지방 유래 중간엽 줄기세포(adipose-derived mesenchymal stem cells, AD-MSC) 또는 골수 유래 중간엽 줄기세포(bone marrow-derived mesenchymal stem cells, BM-MSC)인, 줄기세포.
- 제 15항에 있어서, 줄기세포는 CD141 세포 표면 항원을 발현하는 줄기세포인, 줄기세포.
- 제 18항에 있어서, CD141 세포 표면 항원을 발현하는 줄기세포는 골수, 지방, 혈액, 치수 또는 편도 유래인, 줄기세포.
- 제 15항에 있어서, 분비체 또는 배양액 처리에 의해 혈관 형성능이 증진된, 줄기세포.
- 제 15항에 있어서, 골수 유래 중간엽 줄기세포의 배양액 또는 골수 유래 중간엽 줄기세포에서 분비된 분비체가 추가로 처리된, 줄기세포.
- 제 15항의 줄기세포를 유효성분으로 포함하는, 혈관 재생 또는 혈관 형성용 세포 치료제 조성물.
- 제 1항의 분비체 또는 제 15항의 줄기세포를 유효성분으로 포함하는, 혈관 질환 또는 혈관 기능 장애의 예방 또는 치료용 약학적 조성물.
- 제 23항에 있어서, 혈관 질환이 외상성 혈관 손상, 말초혈관 질환, 심혈관 질환, 뇌혈관 질환 또는 허혈성 질환인, 혈관 질환 또는 혈관 기능 장애의 예방 또는 치료용 약학적 조성물.
- 제 1항의 분비체 또는 제 15항의 줄기세포를 유효성분으로 포함하는, 뇌신경계 질환의 예방 또는 치료용 약학적 조성물.
- 제 25항에 있어서, 뇌신경계 질환은 퇴행성 뇌질환, 정신질환 또는 발달장애인, 뇌신경계 질환의 예방 또는 치료용 약학적 조성물.
- 제 26항에 있어서, 퇴행성 뇌질환은 인지기능 장애, 알츠하이머 치매(Alzheimer`s disease), 루이체 치매(Dementia with Lewy bodies), 전측두엽 치매(frontotemporal dementia), 파킨슨병(Parkinson`s disease), 크로이츠펠트-야곱병(Creutzfeldt-Jakob disase; CJD), 헌팅톤병(Huntington`s disase), 다발성 경화증(multiple sclerosis) 또는 길리안-바레 증후군(Gillain-Barre Syndrome; GBS)인, 뇌신경계 질환의 예방 또는 치료용 약학적 조성물.
- 제 26항에 있어서, 정신 질환은 양극성 장애, 자폐증, 우울증, 과잉 행동, 주의력 결핍, 자폐증, 외상 후 스트레스 장애(PTSD), 불안 장애, 수면 장애, 공황 장애, 지능 장애, 기억력 저하, 약물 중독, 조현병, 강박증, 과대망상, 성격장애, 알코올중독 또는 조울증인, 뇌신경계 질환의 예방 또는 치료용 약학적 조성물.
- 제 26항에 있어서, 발달장애는 뇌전증(epilepsy), 뇌성마비(cerebral palsy), 발달성 언어장애(developmental language disorder), 감각장애(disturbanee of sense), 학습장애(learning disorder), 주의력 결핍 과잉행동 장애(attention deficit hyperactivity disorder; ADHD), 자폐 범주성 장애(Autism Spectrum Disorder; ASD), 지적 장애(Intellectual disability), 인지 장애(Cognitive impairment) 또는 충동조절 장애 (impulse control disorders)인, 뇌신경계 질환의 예방 또는 치료용 약학적 조성물.
- CD141 세포 표면 항원을 발현하는 줄기세포를 인간 혈소판 용해물(human platelet lysate)을 포함하고 혈청을 포함하지 않는 배지에서 배양하는 단계를 포함하는, vMSCs 유래 분비체의 제조방법.
- 제 30항에 있어서, 배양한 배양액을 여과하는 단계를 추가로 포함하는, vMSCs 유래 분비체의 제조방법.
- 혈관 재생 또는 혈관 형성에 사용하기 위한, CD141 세포 표면 항원을 발현하는 줄기세포에서 분비된 분비체의 용도.
- CD141 세포 표면 항원을 발현하는 줄기세포에서 분비된 분비체 또는 CD141 세포 표면 항원을 발현하는 줄기세포의 배양액을 처리한 줄기세포의 혈관 재생 또는 혈관 형성 용도.
