Abstract
췌장 신경내분비 종양 감별진단용 바이오마커 조성물 및 이를 이용한 췌장 신경내분비 종양 감별진단에 필요한 정보를 제공하는 방법에 관한 것으로, 췌장 신경내분비 종양이 의심되는 개체의 생물학적 샘플에서 CD163, ACTR3, S100A9 등의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정하고 췌관선암 환자의 발현 수준과 비교하는 편리하고 간단한 방법을 통하여 췌장 신경내분비 종양을 감별진단함으로써, 적합한 치료 방법을 조기에 적용할 수 있으므로, 췌장 신경내분비 종양의 진단 및 치료에 있어서 유용하게 활용될 것이다.
Description
췌장 신경내분비 종양 감별진단용 바이오마커 조성물 및 이를 이용한 췌장 신경내분비 종양 감별진단에 필요한 정보를 제공하는 방법에 관한 것이다.
신경내분비 종양(NeuroEndocrine Tumor; NET)은 신경내분비 세포에서 기인하여 위장관, 췌장 및 직장에서 주로 발생하는 것으로, 발생률이 낮은 종양이지만, 종양의 크기와 양상에 따라 악성 종양의 특성을 나타내기 때문에 조기 진단과 적극적인 치료가 필요한 종양 중의 하나이다.
췌장 신경내분비 종양(Pancreas NeuroEndocrine Tumor; PNET)은 췌장에서 발생하는 종양의 약 1~2%에 해당하며, 임상 양상에 따라 기능성과 비기능성종양으로 분류할 수 있으나, 병리학적 소견으로는 구분이 어려우며 활성 신경내분비 호르몬과 연관된 증상의 발현 여부로 구분할 수 있다. 하지만, 약 80%에서 증상이 나타나지 않기 때문에 진단이 늦어지는 경우가 많다.
PNET은 약 10만명중 1명의 유병율(0.001%)을 나타내는 것으로 알려져 있으나, 이와 관련된 국내외 연구가 부족하고, 검진으로도 잘 발견되지 않는 췌장에 발생할 경우 예후가 좋지 않아 사망에 이르는 무서운 질병 중 하나이며, 최근에 발생 빈도가 꾸준히 증가하고 있다.
한편, 췌관선암(pancreatic ductal adenocarcinoma; PDAC)은 췌관을 구성하는 세포에서 발생하는 종양으로, 췌장 신경내분비 종양과 동일하게 악성 종양에 속하고 초기 증상이 명확하지 않다는 공통점이 있다. 또한, 췌장 신경내분비 종양과 췌관선암은 유사하게 종괴를 형성하는 종양의 형태를 가지고 있기 때문에 기존의 혈액학적 검사와 영상학적 검사(CT, MRI 등)를 통한 감별진단이 어려운 상황이다. 하지만, 췌장 신경내분비 종양과 췌관선암은 치료 방법에 있어서 상이하기 때문에, 조기에 이를 감별하여 진단할 수 있는 간단한 방법이 필요한 실정이다.
이에, 본 발명자들은 간단하고 편리한 방법으로 췌장 신경내분비 종양을 정확하게 감별진단할 수 있는 바이오마커를 개발하기 위하여, 췌장 신경내분비 종양 환자와 췌관선암 환자의 혈액 샘플에서 단백질체 분석을 수행한 결과, 췌관선암 환자와 비교하여 유의하게 발현이 증가한 CD163, ACTR3, S100A9 등을 확인 및 선별함으로써 본 발명을 완성하였다.
본 발명의 목적은, 간단하고 편리한 방법으로 췌장 신경내분비 종양(Pancreas NeuroEndocrine Tumor; PNET)을 정확하게 감별진단할 수 있는 췌장 신경내분비 종양 감별진단용 바이오마커 조성물을 제공하는데 있다.
다른 목적은, 상기 조성물을 포함하는 췌장 신경내분비 종양 감별진단용 키트를 제공하는데 있다.
또 다른 목적은, 상기 바이오마커 조성물을 이용하여 췌장 신경내분비 종양 감별진단에 필요한 정보를 제공하는 방법을 제공하는데 있다.
또 다른 목적은, 췌장 신경내분비 종양을 감별진단하기 위한, CD163, ACTR3 및 S100A9로 구성된 군으로부터 선택되는 하나 이상의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정할 수 있는 제제를 포함하는 바이오마커의 용도를 제공하는 데 있다.
또 다른 목적은, 개체로부터 수득한 생물학적 샘플에서 CD163, ACTR3 및 S100A9로 구성된 군으로부터 선택되는 하나 이상의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정하는 단계를 포함하는, 췌장 신경내분비 종양을 감별진단하는 방법을 제공하는데 있다.
상기 목적을 달성하기 위하여, 일 양상은 CD163, ACTR3 및 S100A9로 구성된 군으로부터 선택되는 하나 이상의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정할 수 있는 제제를 포함하는, 췌장 신경내분비 종양(Pancreas NeuroEndocrine Tumor; PNET) 감별진단용 바이오마커 조성물을 제공한다.
상기 바이오마커 조성물은 HSPA4, DSC1, PRDX1, PGM2, APOC4, LECT2 및 OGN로 구성된 군으로부터 선택되는 하나 이상의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정할 수 있는 제제를 추가로 포함할 수 있다.
상기 바이오마커 조성물은 췌장 신경내분비 종양을 췌관선암(pancreatic ductal adenocarcinoma)과 구별하여 진단할 수 있다.
용어 "CD163(Cluster of Differentiation 163)"은 Scavenger receptor cysteine-rich type 1 protein M130, M130, MM130 또는 SCARI1라고도 불리우며, 헤모글로빈-합토글로빈 복합체에 대한 고 친화성 스캐빈저 수용체(scavenger receptor)를 의미한다. CD163은 단핵구/대식세포 계통의 세포 마커이며, 그람 양성균 및 그람 음성균에 대한 선천적 면역 센서 역할을 하는 것으로 알려져 있다.
용어 "ACTR3(Actin-related protein 3)"은 ARP3라고도 불리우며, ARP2/3 복합체의 주요 구성요소를 의미한다. ACTR3의 특정 기능은 아직 밝혀지지 않았으나, ARP2/3 복합체는 세포 표면에 위치하며 lamellipodial actin의 조립 및 돌출을 통해 세포 모양 및 운동성에 필수적인 역할을 하는 것으로 알려져 있다.
용어 "S100A9(S100 calcium-binding protein A9)"는 calgranulin B, migration inhibitory factor-related protein 14 (MRP14), 60B8AG, CAGB, CFAG, CGLB, L1AG, LIAG, MAC387, MIF, NIF 또는 P14라고도 불리우며, S100 단백질 패밀리에 속하는 칼슘- 및 아연-결합 단백질을 의미한다. S100A9는 골수 세포 계통에 의해 높게 발현되며 염증성 병태 동안 세포외 환경에서 발견되며, S100A9는 S100 패밀리의 다른 구성원인 S100A8과 헤테로다이머를 형성하거나, 특정 기능을 수행하는 모노머를 형성할 수도 있다. 인간 S100A9는 약 13 kDa의 분자량을 가지며 114 아미노산 잔기로 구성된다. S100A8/A9 복합체는 S100A9 및 헤파란 설페이트 프로테오글리칸의 상호작용을 통해, 또는 S100A8/A9 복합체 및 염증 활성화 후 내피 세포에 의해 독점적으로 발현되는 카르복실화된 N-글리칸의 상호 작용을 통해, 내피에 결합 할 수 있다. S100A9 단백질은 비공유적으로 결합된 호모다이머로 배열된다. 또한, 칼슘의 존재 하에, S100A8 및 S100A9는 칼슘 농도의 세포 조절에 관여하는 것으로 추정되는, S100A8/A9 또는 칼프로텍틴(calprotectin)으로 불리는, 비공유결합 헤테로다이머를 형성한다. S100A9 단백질은 자가면역질환 환자의 혈청 및 염증 부위에서 분비되고 발견된다. S100A9는 베타2 인테그린 Mac-1을 활성화시켜 호중구와 단핵구의 염증 부위로의 이동을 자극하여 이들 세포가 내피 세포에 부착되어 이동하도록 하고, NF-κB와 인플라마솜(inflammasome)을 활성화시킴으로써 인간 단핵구 및 주된 호중구에 의한 TNFα, IL-1β, IL-6와 같은 사이토카인의 분비를 유도하는 것으로 알려져 있다. 또한, S100A9는 식세포 작용, 탈과립화에 대한 강력한 유도제이고, 중성구와 단핵구에 의한 반응성 산소종 생산의 마일드한 유도제이며, NF-κB와 인플라마솜을 활성화시킴으로써 단핵구에 의한 MIP-1α, RANTES, MCP-1, IL-6 및 TNFα와 같은 사이토카인 분비를 유도하는 것으로 알려져 있다.
용어 "췌장 신경내분비 종양(Pancreas NeuroEndocrine Tumor; PNET)"은 신경계와 내분비계 조직이 뭉쳐 발병하는 신경내분비 종양의 일종으로, 췌장의 내분비세포에서 발생하는 신경내분비 종양을 의미한다.
용어 "췌관선암(pancreatic ductal adenocarcinoma; PDAC)"은 췌장에 발생한 악성 종양 중 가장 흔한 유형으로, 췌관을 구성하는 세포에서 발생하는 췌장 종양을 의미한다.
췌장 신경내분비 종양은 췌관선암과 유사하게 종괴를 형성하는 종양의 형태를 가지고 있기 때문에 기존의 혈액학적 검사와 영상학적 검사(CT, MRI 등)를 통한 감별진단이 어려운 상황이다. 한편, 췌관선암 환자의 5년 생존율은 10-15%정도이고, 치료 방법으로는 수술적 절제를 동반한 항암 화학요법(chemotherapy)이 우선적으로 고려된다. 하지만, 췌장 신경내분비 종양의 경우에는 종양의 호르몬 기능성의 유무, 종양의 크기, 세포 분열 활성화 정도에 따라 수술적 절제보다는 소마토스타틴(somatostatin) 유도체, 분자 표적 치료제(Molecular targeted therapy) 등의 화학 항암제 투여 등을 우선시할 수 있다. 또한, 췌장 신경내분비 종양 환자의 5년 생존률은 30% 이상으로서 췌관선암과 비교하여 상대적으로 매우 좋은 것으로 보고되고 있다. 따라서, 췌장 신경내분비 종양의 정확한 감별진단을 통하여 불필요한 수술을 최소화할 수 있으며, 종양 치료에 가장 최적화된 치료 방법을 결정할 수 있다.
용어 "진단"은 병리 상태의 존재 또는 특징을 확인하는 것을 의미한다.
용어 "감별진단"은 다른 질환과 구별하여 병리 상태의 존재 또는 특징을 확인하는 것을 의미한다.
용어 "바이오마커"는 진단 마커로도 쓰일 수 있으며, 생물학적 샘플에서 췌장 신경내분비 종양 발병 여부를 진단할 수 있는 물질을 의미한다. 다른 종류의 췌장 종양 환자로부터 수득한 생물학적 샘플에 비하여 췌장 신경내분비 종양 환자에서 채취한 생물학적 샘플에서 증가 또는 감소를 보이는 폴리펩타이드 또는 핵산(예를 들어, mRNA 등), 지질, 당지질, 당단백질 또는 당(예를 들어, 단당류, 이당류, 올리고당류 등) 등과 같은 유기 생체 분자 등을 포함할 수 있다.
일 구현예에서, PNET 환자와 PADC 환자의 혈액 샘플에서 단백질체 분석을 수행한 결과, PNET 환자의 혈액 샘플에서 CD163, ACTR3, S100A9, HSPA4(Heat shock 70 kDa protein 4), DSC1(Desmocollin-1, CDHF1, DG2/DG3), PRDX1(Peroxiredoxin-1, MSP23, NKEF-A, NKEFA, PAG, PAGA, PAGB, PRX1, PRXI, TDPX2), PGM2(Phosphoglucomutase-2, MSTP006), APOC4(Apolipoprotein C-IV, apolipoprotein C4, APO-CIV, APOC-IV), LECT2(Leukocyte cell-derived chemotaxin-2, chm-II, chm2) 및 OGN(Osteoglycin, mimecan, OG, OIF, SLRR3A) 단백질의 발현 수준이 유의하게 증가한 것을 확인할 수 있다. 특히, 상기 10종의 단백질 중에서도 PADC 환자 대비 PNET 환자의 혈액 샘플에서 CD163, ACTR3 및 S100A9 단백질의 발현 수준이 현저하게 증가한 것을 확인할 수 있다.
상기 10종의 단백질의 발현 수준 측정은 췌장 신경내분비 종양을 감별진단하기 위하여 개체의 생물학적 샘플에서 상기 10종의 단백질 중에서 하나 이상의 존재 여부와 그 단백질의 발현 정도를 확인하는 과정으로, 상기 단백질에 대하여 특이적으로 결합하는 분자를 이용하여 단백질의 양을 확인하는 것이다.
상기 단백질의 발현 수준을 측정할 수 있는 제제는 상기 단백질에 특이적으로 결합하는 모노클로날 항체, 폴리클로날 항체, 키메릭(chimeric) 항체, 리간드, PNA(Peptide nucleic acid), 압타머(aptamer) 또는 나노파티클일 수 있으나, 이에 제한되는 것은 아니다.
이를 위한 분석 방법으로는 웨스턴 블랏팅(Western blotting), ELISA(enzyme linked immunosorbent assay), 방사선면역분석(Radioimmunoassay; RIA), 방사선면역확산법(radioimmunodiffusion), 오우크테로니(Ouchterlony) 면역 확산법, 로케트(rocket) 면역전기영동, 면역침전 분석법(immunoprecipitation assay), 면역조직화학적 분석(immunohistochemical analysis), 보체 고정 분석법(Complement Fixation Assay), FACS(Fluorescence activated cell sorter) 또는 단백질 칩(protein chip) 등이 있으나, 이에 제한되는 것은 아니다.
상기 단백질을 코딩하는 유전자의 발현 수준의 측정은 췌장 신경내분비 종양을 감별진단하기 위하여 개체의 생물학적 샘플에서 상기 10종의 단백질을 코딩하는 유전자 중에서 어느 하나의 존재 여부와 그 유전자의 발현 정도를 확인하는 과정으로, 상기 유전자에 특이적으로 결합하는 분자를 이용하여 유전자의 양을 확인하는 것이다.
상기 단백질을 코딩하는 유전자의 발현 수준을 측정할 수 있는 제제는 상기 유전자에 특이적으로 결합하는 프라이머 쌍, 프로브, 안티센스 뉴클레오티드일 수 있으나, 이에 제한되는 것은 아니다.
이를 위한 분석 방법으로는 역전사 중합효소 연쇄반응(reverse transcriptase-polymerase chain reaction; RT-PCR), 실시간 중합효소 연쇄반응(real time-polymerase chain reaction), RNase 보호 분석법(RNase 보호 분석법(RNase protection assay; RPA), 노던 블랏팅(Northern blotting) 및 DNA 칩 등이 있으나, 이에 제한되는 것은 아니다.
다른 양상은, 상기 바이오마커 조성물을 포함하는 췌장 신경내분비 종양 진단용 키트를 제공한다.
상기 키트는 개체로부터 분리된 생물학적 샘플을 이용하여 췌장 신경내분비 종양을 감별진단할 수 있다. 구체적으로는, 상기 키트는 개체로부터 채취한 혈액 샘플을 이용하여 췌장 신경내분비 종양을 감별진단할 수 있다. 개체로부터 용이하게 채취할 수 있는 혈액 샘플을 이용함에 따라 보다 간단하고 편리한 방법으로 췌장 신경내분비 종양의 감별진단이 가능할 수 있다.
상기 키트에는 상기 단백질 또는 유전자의 발현 수준을 측정하기 위한 제제뿐만 아니라, 면역학적 분석에서 일반적으로 사용되는 도구, 시약 등이 포함될 수 있다.
상기 도구 또는 시약의 일 예로, 적합한 담체, 검출 가능한 신호를 생성할 수 있는 표지 물질, 발색단, 용해제, 세정제, 완충제, 안정화제 등이 포함될 수 있으나, 이에 제한되는 것은 아니다. 표지 물질이 효소인 경우에는 효소 활성을 측정할 수 있는 기질 및 반응 정지제를 포함할 수 있다. 담체는 가용성 담체, 불용성 담체가 있고, 가용성 담체의 일 예로 해당 기술분야에서 공지된 생리학적으로 허용되는 완충액, 예를 들어 PBS가 있고, 불용성 담체의 일 예로 폴리스틸렌, 폴리에틸렌, 폴리프로필렌, 폴리에스테르, 폴리아크릴로니트릴, 불소 수지, 가교 덱스트란, 폴리사카라이드, 라텍스에 금속을 도금한 자성 미립자와 같은 고분자, 기타 종이, 유리, 금속, 아가로오스 및 이들의 조합일 수 있다.

도 1는 일 양상에 의한 단백질체 분석을 통하여 40개의 개별 혈장 샘플에 대한 전체 단백질의 정량 값을 박스 플롯으로 나타낸 결과이다.
도 2는 일 양상에 의한 단백질체 분석을 통하여 췌장 신경내분비 종양(PNET) 환자와 췌장선암(PDAC)의 혈액 샘플에서 차별적으로 발현되는 단백질(DEP)를 확인한 결과이다. X축은 로고2로 치환된 효과 크기를 나타내며, 양수는 PNET 환자에서 단백질의 발현이 높은 것을, 음수는 PDAC 환자에서 단백질의 발현이 높은 것을 나타낸다. Y축은 월콕슨 순위-합 검정의 P값을 Benjamini-Horchberg로 보정한 P값을 로그10으로 치환한 후에 마이너스를 붙인 값이다.
이하 본 발명을 실시예에 의해 보다 상세하게 설명한다. 그러나 이들 실시예는 본 발명을 예시적으로 설명하기 위한 것으로 본 발명의 범위가 이들 실시예에 의해 제한되는 것은 아니다.
실시예 1. 실험 대상 선별
실험 대상으로 수술 후 조직 검사를 통해 췌장 신경내분비 종양 또는 췌관선암으로 진단받은 환자를 모집하고 각 환자로부터 40 ㎕의 혈장 샘플을 채집하였다. 췌관선암 환자는 총 23명이고, 평균 나이는 54.7세였으며, 남성 12명과 여성 11명이었다. 췌장 신경내분비 종양 환자는 총 17명이고, 평균 나이는 52.9세였으며, 남성 6명과 여성 11명이었다.
실시예 2. 단백질체 분석을 위한 혈장 샘플 준비
고성능 액체 크로마토그래피(High-performance liquid chromatography; HPLC)에 연결한 고농도 혈장 단백질 제거 컬럼인 MARS14 컬럼(100 x 4.6 mm; Agilent Technology, Palo Alto, CA, USA)을 이용하여 저농도 단백질을 분리한 후 이를 Suspension-trap(S-trap) kit를 사용하여, 각 환자로부터 채취한 40 ㎕의 혈장 샘플로부터 펩티드를 제조하는 전처리 과정을 수행하였다.
구체적으로, 40 ㎕의 혈장 샘플을 완충액(buffer) A(Agilent; 5185-5987)로 4배 희석하고, Shimadzu HPLC LC20AT 시스템의 MARS14 컬럼에 주입하여 혈장에 포함된 상위 14개의 풍부한 단백질(알부민, IgA, IgG, IgM, α1-항트립신, α1-산 당단백질, 아포지질단백질 A1, 아포지질단백질 A2, complement C3, 트랜스페린, α2-마크로글로불린, 트란스타이레틴, 합토글로빈 및 피브리노겐)을 제거하였다.
결합되지 않은 저농도 혈장 단백질의 분획을 동결건조한 후에 5% 소듐 도데실 설페이트(sodium dodecyl sulfate; SDS) 및 50 mM 트리에틸암모늄 중탄산염(triethylammonium bicarbonate; TEAB) 완충액으로 재흡수하여 10 mM이 되도록 1,4-디티오트레이톨(1,4-Dithiothreitol)을 첨가하고 95 ℃에서 10분 동안 반응시켜 이황화결합을 환원시켰다. 그리고 나서, 알킬화를 위해 20 mM이 되도록 아이오도아세트아미드(iodoacetamide)를 첨가하고 암조건의 실온에서 30분 동안 반응시킨 후, 10 ㎍의 trypsin/LysC mixture를 첨가하여 37 ℃에서 16시간 동안 분해하여 펩티드를 제조하였다. 제조된 펩티드를 용출하여 콜드 트랩(CentriVap Cold Traps, LABCONCO)으로 동결 건조하고 사용할 때까지 영하 80 ℃에서 보관하였다.
실시예 3. 질량 분석법을 이용한 혈장 샘플의 정량적 단백질체 분석
총 40명의 혈장 샘플로부터 단백질을 정량적으로 분석하기 위해서, 액체크로마토그래피-질량분석기(Liquid chromatography-Mass spectrometry)을 이용하여 단백질체 분석 기법인 DDA(Date-Dependent Acquisition)을 이용하였다.
액체 크로마토 그래피-탠덤 질량 분석법(Liquid chromatography-tandem mass spectrometry; LC-MS/MS) 분석 전에, 건조된 펩티드 샘플을 150 ㎕의 완충액 A(0.1% formic acid in HPLC water)로 녹인 후에 5 ㎕를 LC-MS/MS에 주입하였다. 샘플은 다음과 같은 LC-MS/MS 설정으로 Q Exactive™ Plus Hybrid Quadrupole-Orbitrap™ Mass Spectrometer(Thermo Fisher Scientific)를 사용하여 분석하였다: LC 분리를 위해, UltiMate™ 3000 BioRS Systems(Thermo Fisher Scientific)에서 Acclaim PepMap™ 100 C18 컬럼(100 μm x 2 cm, 기공 크기 100 _)과 분석 컬럼으로서 Acclaim PepMap™ RSLC 컬럼(75 μm x 50 cm, 입자 크기 3 μm, 기공 크기 100 _)의 온도를 50 ℃로 유지하였다. 샘플을 완충액 A를 사용하여 3 ㎕/min의 유속으로 트랩 컬럼에 로딩하였다. 10분 후, 펩티드 혼합물은 완충액 A(0.1% formic acid, 5% DMSO in HPLC water)와 완충액 B(0.1% formic acid, 5% DMSO, 80% acetonitrile in HPLC water)을 사용하여 200분 동안 250 nL/min의 유속으로 농도 구배를 통한 분리를 수행하였다. 완충액 B를 150분 동안 0-40% 변경하고, 2분 동안 40-95% 변경하고, 23분 동안 95-95% 변경하고, 10분 동안 95-5%로 유지하고, 15분 동안 5%로 유지하였다.
MS 매개 변수는 다음과 같이 설정하였다: 질량 스펙트럼은 전체 스캔과 20개의 데이터 종속 MS/MS 스캔 사이에서 자동 전환되는 데이터 종속 모드에서 수집하고, 350 ~ 1800 질량 대 전하 비율(m/z)에서 선택된 전체 스캔 MS 스펙트럼의 목표 값은 3,000,000이며, 최대 주입 시간은 100 ms이고, MS1의 분해능은 400 m/z에서 70,000이었다. 2 Da 전구체 이온 분리 창 및 27% 정규화된 충돌 에너지를 사용하였고, MS/MS의 이온 표적 값은 최대 주입 시간이 50 ms이고 MS2의 분해능이 400 m/z에서 17,500 내지 1,000,000으로 설정하였다. 반복된 펩티드의 동적 제외는 20초 동안 적용하였다.
펩티드로 제조된 혈장 단백질 샘플 40개를 LC-MS로 분석함에 있어서 분석 간 Batch별 bias를 최소화하기 위해 무작위로 혼합하여 순차적으로 LC-MS/MS 분석을 수행하였다.
실시예 4. 데이터 분석
40개의 샘플에 대해 LC-MS/MS 분석을 수행하여 40개의 스펙트럼(spectrum) 결과를 획득하였다. 획득한 RAW 데이터는 UNIPROT의 SwissProt 인간 단백질 서열 데이터베이스(2019년 6월에 다운로드)에 대해 Proteome Discoverer(버전 2.2, Thermo Fisher Scientific, USA)에서 SequestHT를 사용하여 검색하였다. 검색 매개 변수는 고정된 변형으로 시스테인-카바미도-메틸화를 포함하는 기본 값으로 설정하고, 두 개까지 잘못된 펩티드 절단을 허용하며, 가변 변형으로 N-말단 아세틸화 및 메티오닌 산화를 설정하였다. 펩티드는 전구체 이온의 초기 질량 편차가 최대 10 ppm이고, 허용된 단편 질량 편차가 20 ppm으로 설정된 검색을 기반으로 식별하였다. 단백질을 펩타이드에 할당할 때 고유한 펩타이드와 공유 펩타이드를 모두 사용하였다. 라벨 없는 정량은 각 단백질의 고유한 펩티드에 대한 피크 강도를 사용하여 수행하였다.
40개 샘플로부터 총 1,068개의 단백질들을 정성 및 정량 분석하였다. 도 1에 나타난 바와 같이, 23명의 췌관선암 및 17명의 췌장 내분비신경 종양 환자에서의 중간값 기반의 정규화된 단백질들의 정량 정보를 로그 스케일(Log scale)로 분석한 결과, 재현성을 나타내는 것을 확인하였다. 또한, 두 그룹의 40개의 혈장 샘플에서 80% 이상 검출되면서 MARS-14 high abundant protein depletion에 관련되지 않은 총 549개 단백질들의 정량 정보를 이용하여 통계 분석을 수행하였다.
실시예 5. 통계적 분석
췌관선암 환자군과 췌장 내분비신경 종양 환자군 간의 차별적으로 발현된 단백질(DEP)을 확인하기 위하여, 비모수통계 방법인 윌콕슨 순위-합 검정(Wilcoxon-Mann-Whitney rank-sum test) 방법을 이용하여 P값을 계산한 후, 여러 단백질을 차이나는 가설들에 대해서 최소한 하나의 제1종 오류가 발생할 가능성(familywise error rate; FWER)을 보정하기 위해서 Benjamini-Horchberg 방법을 적용하여 보정된 P값을 계산하여 0.05 이하인 단백질을 선별하였다. 이와 동시에 두 군의 평균의 차이(효과 크기)가 1.5배 차이가 나는 단백질을 동시에 고려하여 췌장 신경내분비 종양 환자에서 높은 양으로 존재하는 10개의 단백질을 다음과 같이 선별하였다(표 1 및 도 2).

Claims (13)
- CD163, ACTR3 및 S100A9로 구성된 군으로부터 선택되는 하나 이상의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정할 수 있는 제제를 포함하는, 췌장 신경내분비 종양(Pancreas NeuroEndocrine Tumor) 감별진단용 바이오마커 조성물.
- 제1항에 있어서,HSPA4, DSC1, PRDX1, PGM2, APOC4, LECT2 및 OGN로 구성된 군으로부터 선택되는 하나 이상의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정할 수 있는 제제를 추가로 포함하는 것인, 췌장 신경내분비 종양 감별진단용 바이오마커 조성물.
- 제1항에 있어서,췌장 신경내분비 종양을 췌관선암(pancreatic ductal adenocarcinoma)과 구별하여 진단할 수 있는 것인, 췌장 신경내분비 종양 감별진단용 바이오마커 조성물.
- 제2항에 있어서,췌장 신경내분비 종양을 췌관선암(pancreatic ductal adenocarcinoma)과 구별하여 진단할 수 있는 것인, 췌장 신경내분비 종양 감별진단용 바이오마커 조성물.
- 제1항에 있어서,상기 단백질의 발현 수준을 측정할 수 있는 제제는 상기 단백질에 특이적으로 결합하는 모노클로날 항체, 폴리클로날 항체, 키메릭(chimeric) 항체, 리간드, PNA(Peptide nucleic acid), 압타머(aptamer) 또는 나노파티클인, 췌장 신경내분비 종양 감별진단용 바이오마커 조성물.
- 제1항에 있어서,상기 단백질을 코딩하는 유전자의 발현 수준을 측정할 수 있는 제제는 상기 유전자에 특이적으로 결합하는 프라이머 쌍, 프로브 또는 안티센스 뉴클레오타이드인, 췌장 신경내분비 종양 감별진단용 바이오마커 조성물.
- 제1항 내지 제6항 중 어느 한 항에 기재된 바이오마커 조성물을 포함하는, 췌장 신경내분비 종양 감별진단용 키트.
- 개체로부터 수득한 생물학적 샘플에서 CD163, ACTR3 및 S100A9로 구성된 군으로부터 선택되는 하나 이상의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정하는 단계를 포함하는, 췌장 신경내분비 종양 감별진단에 필요한 정보를 제공하는 방법.
- 제8항에 있어서,HSPA4, DSC1, PRDX1, PGM2, APOC4, LECT2 및 OGN로 구성된 군으로부터 선택되는 하나 이상의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정하는 단계를 추가로 포함하는 것인, 췌장 신경내분비 종양 감별진단에 필요한 정보를 제공하는 방법.
- 제8항 또는 제9항에 있어서,상기 단백질 또는 이를 코딩하는 유전자의 발현 수준이 췌장선암 환자로부터 수득한 생물학적 샘플에서 측정한 발현 수준보다 증가한 경우에 췌장 신경내분비 종양으로 진단하는 단계를 추가로 포함하는, 췌장 신경내분비 종양 감별진단에 필요한 정보를 제공하는 방법.
- 제8항에 있어서,상기 생물학적 샘플은 혈액, 혈장, 혈청, 소변, 타액, 객담, 뇌척수액, 세포 배양액, 조직 추출물 또는 종양 조직인, 췌장 신경내분비 종양 감별진단에 필요한 정보를 제공하는 방법.
- 췌장 신경내분비 종양(Pancreas NeuroEndocrine Tumor)을 감별진단하기 위한, CD163, ACTR3 및 S100A9로 구성된 군으로부터 선택되는 하나 이상의 단백질 또는 이를 코딩하는 유전자의 발현 수준을 측정할 수 있는 제제를 포함하는 바이오마커의 용도.
