- a heat injury for example, exertional/classical heat stroke, heat exhaustion, for cardiac cooling post cardiac arrest, or cardiopulmonary bypass
- a second scenario is for conditioning a person prior/subsequent to undergoing an extended period under high ambient temperature conditions (for example, while fire fighting, during explosives disposal, while participating in endurance exercises, during athletic events, and the like).
- the apparatus 100 can be moved to a desired location, and connected to a power supply (mains supply or portable generator).
- a power supply mains supply or portable generator.
- the tub 180 is filled with chilled water/water with ice while activating the requisite circulatory loops, before placement of a patient within the tub 180.
- the patient is placed within the tub 180 in a manner whereby the patient’s head is positioned at the support structure 185.
- liquid contents in the tub 180 are also circulated within the tub 180, correspondingly aiding in the lowering of the patient’s temperature. This is more effective than relying on conduction from the patient’s body to stagnant cold water in the tub 180.
- the patient is then hooked up to both the patient monitoring device 173 and the temperature monitor 178.
- the medical professional(s) overseeing the patient in the tub 180 then monitors parameters of the patient using both the patient monitoring device 173 and the temperature monitor 178, also monitors the temperature of the liquid contents in the tub 180 using the touchscreen control panel 183.
- the temperature of the liquid contents in the tub 180 is monitored using a cooling curve.
- the contents can be chilled water that starts at 15°C, which is chilled rapidly down to 3°C to 5°C using an initial load of ice to further lower the chilled water temperature, whereby the apparatus 100 then maintains the temperature of the liquid contents at 3°C to 5°C even when a substantial heat load is introduced into the tub 180, eg.
- the touchscreen control panel 183 can be used to set a temperature for the liquid contents in the tub 180, and the cooling module 110 of the apparatus 100 ensures that a desired temperature of the liquid contents in the tub 180 is maintained by inputting chilled glycol from the reservoir 160 into its circulatory loop in the glycol supply sub-module 140, via the second heat exchanger 170. Maintaining the liquid contents in the tub 180 at a fixed temperature consequently leads to an optimized cooling rate, such the cooling gradient does not deviate substantially.
- toggling the switch 188 to activate the plurality of nozzles 195 is for directing (circulating) water flow across the patient in order to prevent stationary or relatively slow-moving bodies of warm water from building up over the casualty, forming a thermal barrier and impairing heat exchange between the patient and the liquid contents in the tub 180.
- a direction of flow from the plurality of nozzles 195 should be counter-current to the patient’s arterial blood flow, that is, from the periphery, inwards towards the heart.
- the apparatus 100 can be moved to a desired location, and connected to a power supply (mains supply or portable generator).
- a power supply mains supply or portable generator.
- the tub 180 is filled with chilled water, before placement of forearms, legs or head of a patient within the tub 180.
- the patient’s forearms are placed within the tub 180 in a manner whereby the forearms are fully submerged by the liquid contents in the tub 180.
- the medical professional(s) controlling the apparatus 100 then monitors the temperature of the liquid contents in the tub 180 using the touchscreen control panel 183.
- the temperature of the liquid contents in the tub 180 is monitored using a cooling curve.
- the contents can be chilled water at 15°C and maintained at that temperature even with a substantial heat load in the tub 180, eg. forearms that have just exited a high temperature environment are placed within the tub 180.
- the touchscreen control panel 183 can be used to set a temperature range for the liquid contents in the tub 180, and the cooling module 110 of the apparatus 100 ensures that a desired temperature range of the liquid contents in the tub 180 is maintained by inputting chilled glycol from the reservoir 160 into its circulatory loop in the glycol supply sub-module 140 via the second heat exchanger 170. Maintaining the liquid contents in the tub 180 at a fixed temperature range consequently leads to an optimized cooling rate, such the cooling gradient does not deviate substantially.
- the apparatus 100 whilst the apparatus 100 is used to manage patients with heat injuries or to condition patients prior/subsequent to undergoing an extended period under high ambient temperature conditions, it is the portable aspect of the apparatus 100 that is advantageous.
- the ease of powering the apparatus 100 using a single grounded connector is also advantageous.
- the apparatus 100 is also designed to enable enhanced liquid circulation to both the front and the back of a patient, as well as optimize heat exchange from the patient by disrupting thermal barriers forming from stasis of water.
- the use of the apparatus 100 to enable the carrying out a method for managing a temperature of at least one body part of a patient is important.
- a first method 400 for managing a temperature of at least one body part of a patient will be described with reference to FIG 4.
- the method 400 is carried out when at least one body part of a patient is placed within the tub 180.
- the method 400 is appropriate for the second scenario that was mentioned earlier.
- the method 400 will be described with reference to components of the apparatus 100, but it should be noted that the method 400 can also be carried out without use of the apparatus 100.
- the sequence of steps for the method 400 can also be varied without changing a purpose of the method 400.
- temperature readings are obtained of liquid contents in the tub 180 of the apparatus 100 at fixed intervals.
- the interval can be within a range of two to ten minutes.
- the liquid can be, for example, water, or any appropriate non-toxic liquid.
- a desired temperature for the liquid in the tub 180 is defined.
- the temperature can be within a range of 10°C to 15°C, 3°C to 5°C, and so forth.
- the desired temperature can be input via the touchscreen control panel 183.
- a cooling curve of the liquid in the tub 180 is determined. This can be done using the temperature readings obtained at step 410. Subsequently, at step 440, a cooling gradient can be determined from the cooling curve obtained at step 430.
- Step 450 it is then determined if the cooling gradient is greater than a pre-defined value, for example, 0.3. If the cooling gradient is above the pre-defined value, there is a clear indication that the liquid in the tub 180 is heating up. Step 460 then follows, whereby cooled glycol is circulated within its circulatory loop in the glycol sub-module 140 in order to lower the cooling gradient in the tub 180.
- a pre-defined value for example, 0.3.
- Step 470 then follows, whereby the liquid in the tub 180 is left “as-is” as the liquid is at the desired temperature or temperature range.
- FIG 6 there is shown an example of a cooling station 600 that can be configured to carry out the method 400.
- the method 400 is carried out in the water receptacles 610 located on either side of a seating structure 620.
- the method 400 described when using apparatus 100 can also be used for the cooling station 600.
- the cooling station 600 as shown is located within a vehicle, the vehicle having a power supply and liquid refrigeration capabilities. Water introduced into the receptacles 610 can be drained after use and refilled with chilled water thereafter.
- a second method 500 for managing a temperature of at least one body part of a patient will be described with reference to FIG 5.
- the method 500 is carried out when at least one body part of a patient is placed within the tub 180.
- the method 500 is appropriate for the first scenario that was mentioned earlier.
- the method 500 will be described with reference to components of the apparatus 100, but it should be noted that the method 500 can also be carried out without use of the apparatus 100.
- the sequence of steps for the method 500 can also be varied without changing a purpose of the method 500.
- cooling of the liquid in the tub 180 is “boosted” to enable rapid cooling of the liquid.
- ice can be placed in the tub 180 to enable the rapid cooling of the liquid.
- a cooling curve of the liquid in the tub 180 is determined. This can be done using the temperature readings obtained at step 510. Subsequently, at step 550, a cooling gradient can be determined from the cooling curve obtained at step 540.
- Step 560 it is then determined if the cooling gradient is greater than a pre-defined value, for example, 0.3. If the cooling gradient is above the pre-defined value, there is a clear indication that the liquid in the tub 180 is heating up. Step 570 then follows, whereby cooled glycol is circulated within its circulatory loop in the glycol sub-module 140 in order to lower the cooling gradient in the tub 180.
- a pre-defined value for example, 0.3.
- Step 580 then follows, whereby the liquid in the tub 180 is left “as-is” as the liquid is at the desired temperature or temperature range.
