生体内に存在する活性硫黄化合物の多彩な生理活性作用に基づいて、ウイルス由来のプロテアーゼを阻害することにより、ウイルス感染症に対する予防・治療効果を奏する薬剤を提供する。
Description
本発明は、活性硫黄化合物を主要薬効成分とする薬剤に関する。
システインヒドロポリスルフィド(CysSSH)、グルタチオンポリスルフィド(GSSH)などの反応性硫黄分子種/活性硫黄分子種(reactive sulfur species, RSS)の生理学的重要性が、報告されている(非特許文献1)。
システインヒドロポリスルフィド(CysSSH)は、さまざまな生物で大量に発生するが、その生合成と生理学的機能についてはほとんど知られていない。大腸菌および哺乳動物細胞のシステイン含有タンパク質では広範な過硫化物の形成が明らかであり、これは、生体内の硫黄代謝物に関連する化学反応を含む翻訳後プロセスから生じると考えられている。
原核生物および哺乳類のシステイニル-tRNAシンテターゼ(CARS)によって触媒される反応である、基質L-システインからの効果的なCysSSH合成が存在する。マウスおよびヒト細胞のミトコンドリアCARSをコードする遺伝子の標的破壊は、CARSが内因性のCysSSH産生に重要な役割を果たしていることを示し、これらの酵素がin vivoで主要なシステイン過硫黄合成酵素として機能することを示唆している。CARSは、同時翻訳システイン過硫化も触媒し、ミトコンドリアの生合成と生体エネルギーの調節に関与している。
したがって、CARS依存性過硫化代謝物の産生メカニズムの解明は、生理学的および病態生理学的条件における異常な酸化還元シグナル伝達を理解し、酸化ストレスおよびミトコンドリア機能障害に基づいた治療標的を示唆する可能性がある。
RSS代謝プロファイリングを使用して、活性硫黄メタボロミクス分析を確立することにより、原核生物と真核生物の両方で内生的および遍在的に生成されるかなりの量のRSSの生体内動態が明らかになるであろう。ただし、ポリスルフィドの化学的性質は、その反応性または複雑なレドックス活性特性のため、完全には理解され、そして、解明されていない。
一方、新型コロナウイルス感染症(COVID-19)の原因となる新型コロナウイルス(SARS-CoV-2)の病態解明と予防・治療法の確立が急務となっている。COVID-19の治療薬としてSARS-CoV-2プロテアーゼ阻害剤が報告されている(非特許文献2,3)。なお、WO/2017/005768公報には、眼科手術時に眼内組織が十分に保謹され、更に安全性が高い眼内灌流液を提供することを課題とし、システインパ一スルフィド、グル夕チオンパ一スルフィドに代表される活性イオウ分子の少なくともー種を含有した眼内灌流液により課題を解決することが、開示されている。

本発明は、生体内に存在する活性硫黄化合物の多彩な生理活性作用に基づいて、ウイルス由来のプロテアーゼを阻害することにより、ウイルス感染症に対する予防・治療効果を奏する薬剤を提供することを課題とし、かつ目的とする。既述の特許文献3には、このことは開示も示唆もされていない。
前記目的を達成するために、第1の発明は、活性硫黄化合物を主要薬効成分として含有する抗ウイルス薬であることを特徴とする。前記活性硫黄化合物は、例えば、R1S(S)nR2(R1及びR2は、独立してチオール基を有するアミノ酸及びポリペプチドから選択され、チオール基以外の部分を表す。nは2以上の整数であり、7以下であることが好ましい)である。前記活性硫黄化合物は、例えば、前記R1及びR2が夫々L-システイン(Cys)、ホモシステイン(Hcys)またはグルタチオン(GSH)である。前記活性硫黄化合物は、例えば、グルタチオンポリスルフィドであり、特に、GSSSGである。
前記ウイルスはコロナウイルス(CoV)であり、特に、新型コロナウイルス(SARS-CoV-2)である。前記抗ウイルス薬は、例えば、SARS-CoV-2肺炎の予防・治療薬である。
第2の発明はグルタチオンポリ(トリ)スルフィド(GSSSG等)を主要薬効成分とする、新型コロナウイルス感染症(COVID-19)の予防・治療薬であり、第3の発明はグルタチオンポリ(トリ)スルフィドを主要薬効成分とする免疫制御(調整)剤であり、第4の発明は、グルタチオンポリ(トリ)スルフィドを主要薬効成分とする抗炎症剤である。
第5の発明は活性硫黄化合物を主成分とするウイルス性肺炎等の肺炎予防・治療薬である。当該活性硫黄化合物の態様は既述のとおりである。
本発明によれば、生体内に存在する活性硫黄化合物の多彩な生理活性作用に基づいて、例えば、ウイルス由来のプロテアーゼを阻害する等して、ウイルス感染症に対する予防、及び/又は、治療効果を奏する薬剤を提供することができる。
SARS-CoV-2は、自身のウイルスゲノムに基づいて、ふたつの異なるシステイン(チオール)プロテアーゼを発現している。これは、パパイン様プロテアーゼ(papain-like protease, PLpro)と3C様プロテアーゼ(3C-like protease, 3CLpro、あるいは、メインプロテアーゼ, main protease, Mproとも呼ばれる)であり、これらのプロテアーゼは、ウイルスの宿主細胞内での複製に必須である。これらプロテアーゼの結晶構造は明らかになっている。
発明者は、活性イオウ分子の供与体であり、活性硫黄化合物の一つであり、特に、グルタチオンポリスルフィド(glutathione trisulfide,GSSSG等,G:グルタチオン)が、これらふたつのプロテアーゼ、PLproと3CLproの活性中心のチオール基に特異的に共有結合することで、その酵素活性を強力に阻害する発明を確立した。既に、同様の阻害様式を示すエブセレンが、結果として、新型コロナウイルス(SARS-CoV-2)に対して顕著な増殖抑制効果を示すことが報告されている(既述の非特許文献2、3)。酸化型のポリスルフィドの中央の硫黄(GSSSG)の求核性は、還元型のポリスルフィド(GSSSH)よりも強力であるために、酸化型のポリスルフィドの方が有用である。
本願発明者は、グルタチオンポリスルフィドとしてのGSSSGがSARS-CoV-2やその変異株のプロテアーゼ活性を阻害することで直接的な抗ウイルス効果をもたらすことを利用して新規抗SARS-CoV-2薬を発明し(図1)、さらに、GSSSGの強力な炎症・免疫制御活性(炎症・免疫抑制)を利用して(図2)、COVID-19重症(劇症)肺炎の病態とされるサイトカインストーム、および、その合併症あるいは後遺症(合併症)として重要視されている間質性肺炎・肺線維症などの難治性疾病の発生予防を行うことで、単純な抗ウイルス療法にとどまらない統合的な抗COVID-19治療剤の発明に至った(図2)。グルタチオンポリスルフィドは、SARS-CoV-2等のウィルス感染の結果の炎症に対する抑制効果を発揮する。
図2において、グルタチオントリスルフィドは、二つのシステインプロテアーゼを阻害することによって、抗ウイルス作用を達成する。グルタチオントリスルフィドは、SARS-CoV-2感染により誘発される免疫応答によるインターロイキン-6産生を抑制し、免疫制御、抗炎症、抗サイトカイン作用を達成する。さらに、グルタチオントリスルフィドは、一酸化窒素(NO)と反応・結合して、NOによる血管拡張や血小板凝集抑制活性を増強することで、COVID-19の合併症として知られる血栓形成や血管炎・血管損傷などに対する強力な血管保護作用を発揮する。
抗ウイルス剤の開発に向けて、これまでウイルスプロテアーゼ阻害剤探索が活発に行われてきた。しかしながら、多くの化合物は、いずれかのプロテアーゼのみに特化し、事実、各プロテアーゼのみに限定した阻害活性を有するに留まり、グルタチオンポリスルフィド(GSSSG)のように、SARS-CoV-2の二つのシステインプロテアーゼを同時に阻害して強力な薬効を発揮するものは、前例がない。
GSSSGは、ウイルス性肺炎に対して、その強力な抗炎症作用を介して治療効果を発揮する。実際、GSSSGをマウスに経鼻投与するとことで、A型インフルエンザウイルス肺炎の予防と治療効果が発揮されることが分かってきた(図3)。図3は、GSSSGによる抗インフルエンザ効果の実施例の結果を示すものである。インフルエンザウイルスPR8株(A/PR/8/34 (H1N1)、Akaike T. et al, Journal of Infectious Diseases, 170, 1023-1026, 1994)の2000 pfu/ml(PBS)を複数のB6マウス(各グループ10匹、8週令)夫々に投与して感染させた。この投与は、PBS 40μLのウイルス液を麻酔下で気管内投与によって行った。さらに、GSSSG(50μL,1mM)をウイルス接種6時間前に、麻酔下で経鼻投与し、経時的に、マウスの生存率、肺障害(感染14日後)、および、各種炎症細胞数(感染3日後)を測定・解析した。生存率、肺障害の有無、および、各種炎症細胞数を、GSSSGを投与した群とコントロール群(PBS投与群/ GSSSG非投与)とを対比した。肺障害については、HE染色により、肺炎像を評価した。炎症細胞数の測定については、PBSを用いて、気管支肺胞洗浄(bronchoalveolar lavage, BAL)を行い、BAL中の細胞数やサイトカインを解析した。
また、気管支喘息、慢性閉塞性肺疾患(chronic obstructive pulmonary disease, COPD)や肺線維症モデルに対してもGSSSGの治療効果が確認された(図4)。過去に経験されているSARSの後遺症として特発性間質肺炎・肺線維症が報告されており、COVID-19においても同様の難治性肺疾患の発生が懸念されているが、GSSSGは、この様なSARS-CoV-2感染初期から後期までの全経過と予後・合併症に及ぶすべての病態について幅広く予防と治療効果をもたらす。
図4は、GSSSGによるエラスターゼ誘導性COPDの治療効果を示す実施例であり、Cars2欠損B6マウス*1のブタ膵臓エラスターゼ(PPE)誘発肺気腫モデルにおいて、GSSSG(50μL,1mM)を6時間前および2日後と4日後の3回気道投与し、PPE投与5日後の気管支肺胞洗浄液(bronchial alveolar lavage fluid, BALF)中の炎症性細胞数を測定し、CTにより肺気腫病変を解析した。炎症性細胞数の測定は図3の実施例と同様にして行った。GSSSG投与群(GSSSG(+))では、GSSSG非投与群(GSSSG(-)/コントロール群)に比較して、炎症性細胞数が少なく、かつ、CTから得られた画像から分かるように、GSSSG非投与群に比べて肺気腫が緩解されていることが確認された。

生体内には多様な活性硫黄分子が存在するが、主要なものとしてグルタチオンに過剰に硫黄分子が付加したGSSSGがあり、これは比較的安定で、大量に高純度で合成することができることから、発明者は、これまでその薬効を検討してきた。一方、この様な活性硫黄が、タンパク質のシステインのチオール基にも形成されていることが分かっており、例えば、パパインなどのチオールプロテアーゼの活性中心のシステインのチオールは、このイオウ付加反応(過イオウ化:persulfidation)により活性が抑制されることが明らかとなった。
また、発明者は、GSSSGがマウスのインフルエンザ肺炎モデルにおいて重症肺炎の改善効果を示すだけでなく、マウスの気管支喘息、COPD、肺線維症モデルにおいて、炎症性サイトカインの産生抑制を介して、気道や肺損傷、肺繊維化を軽減することを見出している。一般に、COVID-19は、この様な基礎疾患を有する患者において重症化することが知られている。既述のCARS2欠損マウスにおいては、IL-6などのサイトカインが上昇することから、GSSSGなどの活性硫黄分子(RSS)の抗サイトカイン活性が確認された。
薬剤投与にあたってGSSSGを、注射剤、輸液の他、吸入薬(ドライパウダー)や点眼剤として、ウイルスの標的臓器に局所投与することができることは大きな利点がある。類似したアプローチとして、一酸化窒素(NO)吸入療法が試験的に行われているが、NOは、高濃度では毒性もあり、投与量の設定と調整が難しく、NO自身が炎症や酸化ストレスのメディエータでもあるので、COVID-19感染病態を逆に増悪させることが懸念されている。特に、肺の基礎疾患をもつ患者に対してはかえって重篤化させる可能性がある。
グルタチオンポリスルフィドは、その強力な抗炎症と免疫制御機能という視点から極めて高い優位性を持つ。即ち、サイトカインストームや感染後の肺繊維化などの多様な合併症の発生を抑制するという薬効も期待される。また、作用点が、当該ウイルスプロテーゼの活性中心のチオール基をピンポイントに狙うものなので、他の薬剤との多剤併用療法に優れた効果が見込める。さらに、GSSSGは、ウイルスのみならず、宿主側のプロテアーゼも幅広くターゲットできるので、SARS-CoV-2以外のコロナウイルスやインフルエンザを含めた様々なウイルスに対しても予防・治療薬として応用できる。
活性硫黄化合物の好適なものとしてのポリスルフィドには、還元型と酸化型があり、R-(S)n-SH(n=1,2,・・・・)が還元型であり、R-(S)n-R(n=2,3,・・・・)が酸化型である。
図5に、GSSSGによるSARS-CoV-2プロテアーゼの阻害試験の結果を示す。GSSSGは、特願2017-543669の段落[0025]、[0031]の記載に基づいて生成することができる。SARS-CoV-2由来の組換えパパイン様プロテーゼ(PLpro)およびメインプロテアーゼ(3CLpro)を、各0.3 mMのGSSSGやエブセレン(2-フェニル-1,2-ベンゾイソセレナゾール-3(2H)-オン)、NO(一酸化窒素)と37℃で15分間インキュベート後、プロテアーゼ活性をMCA蛍光基質および蛍光プレートリーダー(励起波長380 nm、蛍光波長460 nm)を用いて測定した。
図5によれば、GSSSGは、コントロール(GSSSG(-))に対して、エブセレンと同程度の強い阻害活性が既述の二つのプロテアーゼに対して確認された。エブセレンが二つのプロテアーゼに対して強い阻害活性を有することは既述の非特許文献3、4において報告されている。PLproと3CLproは、SARS-CoV-2のゲノムは公開されているため、それに基づき合成された。
SARS-CoV-2のPapain-like proteaseおよびMain proteaseのクローニングについて、SARS-CoV-2ゲノム(Accession number: NC_045512)からpapain-like protease (4946-5923)、main protease(10055-10972)領域について人工遺伝子合成を行なった。塩基配列は大腸菌のコドン使用頻度に近似させた。これらの人工遺伝子断片についてpapain-like proteaseはpET53 vectorにクローニング後、NiCo21(DE3)に形質転換し、main proteaseはpE SUMO vectorにクローニングした後、BL21(DE3)に形質転換した。IPTG誘導によりpapain-like proteaseはHisタグ融合タンパク質、main proteaseはHisタグSUMOタグ融合タンパク質として大腸菌内で発現させた。これらの組換えタンパク質はNi-NTA agaroseで精製し、main protease組換えタンパク質はさらにSUMO protease処理をすることでHisタグSUMOタグ除去を行なった。

GSSSGの製剤中の濃度は、通常0.0003μg/mL以上であり、0.003μg/mL以上であってよく、0.01μg/mL以上であってよく、0.03μg/mL以上であってよく、0.1μg/mL以上であってよく、0.3μg/mL以上であってよく、1μg/mL以上であってよく、3μg/mL以上であってよく、10μg/mL以上であってよく、30μg/mL以上であってよい。一方上限は、通常1000μg/mL以下であり、300μg/mL以下であってよく、100μg/mL以下であってよく、30μg/mL以下であってよい。
次に、SARS-CoV-2のJPN/TY/WK-521株(国立感染症研究所より入手)をII型膜貫通型セリンプロテアーゼ(TMPRSS2)過剰発現アフリカミドリザル腎上皮細胞(VeroE6/TMPRSS2細胞、JCRB細胞バンクより入手)にMOI 0.01で感染させて増殖させたモデルを作成し、2019新型コロナウイルス検出試薬キット(株式会社島津製作所)およびCFX Connect Real-Time System (Bio-Rad Laboratories, Inc.)を用いた定量PCR法によってGSSSGの抗SARS-CoV-2作用を確認した。モデルは、GSSSG投与群(10μM、100μM、300μM)とGSSSG非投与群とから構成した。
図6に、GSSSGによる抗SARS-CoV-2作用の試験結果を示す。図6によれば、GSSSG投与量を増加させるに従い、RNAウイルス量が減少する結果となった。
グルタチオントリスルフィドは、例えば、WO2018/117186公報に基づいて合成できる。GS(S)nG(n=1-7)は、さらに、7606pp-7611pp /PNAS/May 27, 2014/ vol. 111/no. 21(www.pnas.org/cgi/doi/10.1073/pnas.1321232111)に基づいて合成されることが可能である。GSH(20mM)を、20mM Tris-HCl緩衝液(pH7.4)中のI2(20mM)の存在下で、室温で15分間、20mM NaHSと反応させた。反応混合物を、GS(S)nGの精製のためにRP-HPLCに供した。HPLCは、UV検出器を210nmに設定したWaters e2695シリーズを使用して実施した。サンプルはTSKgel ODS-80Tsカラム(4.6×150 mm;東ソー)、摂氏40°、移動相A(H2O + 0.1%トリフルオロ酢酸)およびB(メタノール)を、15分で2%から70%Bまでの直線勾配で、1 ml / minの流速で実行された。GS(S)nG(n=3-7)の夫々について、GSSSGと同様な効果が確認できる。
