なお、従来、臨床で広く用いられている5-FU及びその誘導体は、体内でリン酸化されてフルオロデオキシウリジンモノホスフェート(FdUMP)になり、チミジル酸合成を抑制し、DNA合成を阻害する。また、5-FU及びその誘導体は、細胞内でフルオロウリジントリホスフェート(FUTP)に代謝され、RNAへ取り込まれてRNA機能障害を引き起こす。体内で生じたFdUMPは、FdUTPとしてDNAに取り込まれることが知られているが、その量は極めて少なく、DNA機能障害を引き起こさない(非特許文献3)。つまり、5-FU及びその誘導体は、DNA合成阻害やRNA機能障害を介して抗腫瘍効果を発揮する薬剤であり、それ自体はDNA機能障害を介して抗腫瘍効果を発揮する薬剤ではなく、上記のFTD・TPI配合剤や、dUTPase阻害薬とフッ化ピリミジン系代謝拮抗薬を含有する薬剤とは異なるものである。
適応的免疫反応の活性化は、抗原ペプチド-MHC complexとT細胞受容体(TCR)の結合により始まる。この結合はさらに共刺激分子であるB7 familyとそのレセプターであるCD28 family間の結合によるcostimulation或いはcoinhibitionによって規定される。すなわち、T細胞が抗原特異的に活性化するためには、2つの特徴的なシグナル伝達イベントを必要とし、B7 familyからの共刺激を受けずに抗原刺激のみを受けたT細胞は不応答状態(anergy)となり、免疫寛容が誘導される。
癌細胞はこのしくみを利用し、抗原特異的なT細胞の活性化を抑制することで、免疫監視機構から逃避し、増殖し続ける。そのため、Costimulationの強化やCoinhibitionのブロックにより癌患者の生体内における抗腫瘍免疫応答を誘導させ、腫瘍の免疫逃避を制御することは癌治療に有効であると考えられ、Costimulatory分子(刺激性の共刺激分子)或いはCoinhibitory分子(抑制性の共刺激分子)を標的とした癌免疫療法が種々提案されている(非特許文献4)。例えば、PD-1とそのリガンド(PD-L1及びPD-L2)の結合を阻害することによりT細胞を活性化する免疫調節剤としてニボルマブ(ヒトPD-1に対するヒト型IgG4モノクローナル抗体)が悪性黒色腫等の治療に用いられている(特許文献4、非特許文献5)。
しかしながら、上記のとおり、5-FUなどのフッ化ピリミジン系代謝拮抗薬は、DNA機能障害を介して抗腫瘍効果を発揮する薬剤ではない。よって、これまでに、DNA機能障害剤と抗PD-1抗体等のような免疫調節剤との併用療法は行なわれていない。

具体的なDNA機能障害剤としては、FTDを含有する薬剤、及びdUTPase阻害薬とフッ化ピリミジン系代謝拮抗薬を含有する薬剤が挙げられる。
本発明におけるFTDを含有する薬剤としては、FTD及びTPIを含有する配合剤が挙げられ、FTD及びTPIをモル比1:0.5で含有する配合剤が好ましい。FTD及びTPIはそれぞれ公知の化合物であり、例えば、国際公開第96/30346号パンフレットに記載の方法に従って合成することができる。また、FTD及びTPIをモル比1:0.5で含有する配合剤も公知である(非特許文献1及び2)。また、FTD・TPI配合剤は、日本及び米国において進行・再発の結腸直腸癌の治療剤として承認されており、その用法用量はFTDとして70mg/m2/dayを、1日2回、5日間連続経口投与したのち2日間休薬する。これを2回繰り返したのち14日間休薬する。これを1コースとして投与を繰り返す、と定義されている。
このようなものとしては、Costimulatory分子(刺激性の共刺激分子)の機能を促進する物質、或いはCoinhibitory分子(抑制性の共刺激分子)の機能を抑制する物質が挙げられる。現在、B7 familyとCD28 familyは数多く同定され、本発明では特に限定されることなくこれらをターゲットにした物質を用いることができる。例えば、PD-1経路アンタゴニスト、ICOS経路アゴニスト、CTLA-4経路アンタゴニスト、CD28経路アゴニスト、BTLA経路アンタゴニスト等が挙げられる。
本発明において、好ましくはPD-1経路アンタゴニスト、ICOS経路アゴニスト、CTLA-4経路アンタゴニスト、CD28経路アゴニスト又はこれらの組み合わせであり、より好ましくはPD-1経路アンタゴニスト又はCTLA-4経路アンタゴニストであり、副作用抑制の観点から、さらにより好ましくはPD-1経路アンタゴニストである。
また、CTLA-4経路アンタゴニストは、T細胞上に発現するCTLA-4や、そのリガンドであるB7-1(CD80)やB7-2(CD86)による免疫抑制シグナルを阻害するものであり、抗CTLA-4抗体、CTLA-4細胞外ドメイン、CTLA-4-Ig、抗B7-1/CD80抗体、抗B7-2/CD86抗体が好ましく、抗CTLA-4抗体、CTLA-4-Igがより好ましい。なかでも、好ましくは抗CTLA-4抗体である。
好ましくは、ヒト化抗体又は完全ヒト型抗体であり、モノクローナル抗体である。
本発明における抗PD-L1抗体としては、具体的には、アテゾリズマブ、デュルバルマブ又はアベルマブが挙げられ、アテゾリズマブが好ましい。
本発明における抗CTLA-4抗体としては、具体的には、イピリムマブ又はトレメリムマブが挙げられ、イピリムマブが好ましい。
本発明におけるCTLA-4-Igとしては、具体的には、アバタセプトが挙げられる。
これらの抗体は、通常公知の抗体作製方法により製造でき、たとえば、前記特許文献2の方法によって製造することができる。
また、抗PD-1抗体は、ニボルマブ又はペムブロリズマブとして、抗PD-L1抗体は、アテゾリズマブ、デュルバルマブ又はアベルマブとして、抗CTLA-4抗体は、イピリムマブ又はトレメリムマブとして、CTLA-4-Igは、アバタセプトとして既に販売され、また販売予定であり、これを用いることもできる。
また、テガフール・ギメラシル・オテラシルカリウム配合剤の投与日における1日あたりの投与量は、テガフール量として10~200mg/m2/dayが好ましく、20~80mg/m2/dayがより好ましく、40~72mg/m2/dayが特に好ましい。
また、DNA機能障害剤が化合物1又はその薬学的に許容される塩、及びカペシタビンを含有する薬剤である場合、化合物1の投与日における1日あたりの投与量は、12~3000mg/m2/dayが好ましく、240~1200mg/m2/dayがより好ましく、480~720mg/m2/dayが特に好ましい。また、化合物1の投与日における1日あたりの投与量を患者個体あたりで規定する場合、20~5000mg/body/dayが好ましく、400~2000mg/body/dayがより好ましく、800~1200mg/body/dayが特に好ましい。
また、カペシタビンの投与日における1日あたりの投与量は、200~3000mg/m2/dayが好ましく、480~1400mg/m2/dayがより好ましく、600~900mg/m2/dayが特に好ましい。

例えば、身長175cm、体重70kgの癌患者の体表面積を上記1の計算式を用いて計算した場合、体表面積は([175(cm)×70(kg)]/3600)1/2=1.84(m2)と算出される。当該患者において投与量を60mg/m2/dayとする場合は、1.84×60=111mgとなり、1日合計投与量は約110mgと設定される。
具体的には、ニボルマブを単独で投与する場合の推奨用量は、日本において承認を受けた投与量である1回2mg/kg(体重)であることから、本発明におけるニボルマブの投与日における1日あたりの投与量は、1回1~2mg/kg(体重)が好ましく、より好ましくは1回2mg/kg(体重)である。
アテゾリズマブを単独で投与する場合の推奨用量は、米国において承認を受けた投与量である1回1200mgであることから、本発明におけるアテゾリズマブの投与日における1日あたりの投与量は、1回600~1200mgが好ましく、より好ましくは1回1200mgである。
なお、本発明において「推奨用量」とは、臨床試験などにより決定された、重篤な副作用を発症せずに安全に使用できる範囲で、最大の治療効果をもたらす投与量であり、具体的には、日本独立行政法人医薬品医療機器総合機構(PMDA;Pharmaceuticals and Medical Devices Agency)、米国食品医薬品局(FDA;Food and Drug Administration)、欧州医薬品庁(EMA;European Medicines Agency)等の公的機関や団体により承認・推奨・勧告され、添付文書・インタビューフォーム・治療ガイドライン等に記載された投与量があげられ、PMDA、FDA又はEMAのいずれかの公的機関により承認された投与量が好ましい。
FTD・TPI配合剤の場合は、5日間連日投与と2日間休薬を2回繰り返した後、2週間休薬する投与スケジュール、又は5日間連日投与と9日間休薬を2回繰り返す投与スケジュールが好ましい。
化合物1又はその薬学的に許容される塩、テガフール・ギメラシル・オテラシルカリウム配合剤及びカペシタビンの場合は、1~4週間投与と1~2週間休薬を繰り返す投与スケジュールが好ましく、2~3週間投与と1週間休薬を繰り返す投与スケジュールがより好ましく、2週間投与と1週間休薬を繰り返す投与スケジュールが特に好ましい。
ニボルマブ又はアテゾリズマブの場合は、3週間間隔で投与する投与スケジュールが好ましい。
FTD・TPI配合剤の場合は1日2回、化合物1又はその薬学的に許容される塩、テガフール・ギメラシル・オテラシルカリウム配合剤及びカペシタビンの場合は1日2回、ニボルマブ又はアテゾリズマブの場合は1日1回が好ましい。
本発明のDNA機能障害剤及び免疫調節剤の投与順序は、癌種や病期等に応じて適宜選択しうるが、どちらを先に投与しても、同時に投与しても構わない。
本発明はまた、癌患者に対するDNA機能障害剤の抗腫瘍効果を増強するための免疫調節剤を含む抗腫瘍効果増強剤に関する。当該抗腫瘍効果増強剤は、上記抗腫瘍剤の製剤形態を有する。
本発明はまた、DNA機能障害剤を投与された癌患者を治療するための免疫調節剤を含む抗腫瘍剤に関する。当該抗腫瘍剤は、上記の製剤形態を有する。
「治療」には、腫瘍を外科的に摘出した後に再発防止のために行われる術後補助化学療法、腫瘍を外科的に摘出するために事前行われる術前補助化学療法が包含される。
本発明はまた、癌患者に対する免疫調節剤と併用することを特徴とする、DNA機能障害剤を含む抗腫瘍剤に関する。当該抗腫瘍剤は、上記の製剤形態を有する。

ヒト大腸癌株(KM20C)の培養細胞(1×107cells/マウス)を5~6週齢のBALB/cA Jcl-nuマウスの腹腔内に移植した。各群の平均体重が均等になるようにマウスを割り付け、群分け(n=10)を実施した日をDay0とした。
FTD・TPI配合剤(FTDとTPIとのモル比1:0.5の混合物、以下同じ)は、FTDとして、75、100、150、300及び450mg/kg/dayとなるよう調製した。薬剤の投与はDay3から開始し、FTD・TPI配合剤は5日間連日経口投与・2日間休薬を6週間行った。
抗腫瘍効果の指標として、各群のマウスの生存数を確認し、各群の生存期間、延命率を比較した。延命率(ILS;Increased Life Span)は以下のように計算した。
一方、ヒトにおけるFTD・TPI配合剤の単独で投与する場合のRDは、FTDとして70mg/m2/dayであることが知られている。よって、FTD・TPI配合剤のFTDとしての投与量は、マウスにおける150mg/kg/dayと、ヒトにおける70mg/m2/dayが相当する。
マウス大腸がん株(CMT-93)を生後5~6週齢のC57BL/6マウスの右側胸部に移植した。腫瘍移植後に腫瘍の長径(mm)及び短径(mm)を測定し、腫瘍体積(TV;tumor volume)を算出後、各群の平均TVが均等になるように各群にマウスを割り付け、群分け(n=6)を実施した日をDay0とした。
Anti-mouse PD-1抗体(clone RMP1-14、BioXCell社、以下同じ)は、マウスにおいて抗腫瘍効果が報告されている用量である0.1mg/body/dayとなるように調製した(Clin Cancer Res. 2013 Oct 15;19(20):5626-35.)。Anti-mousePD-1抗体はDay1、Day5及びDay9に腹腔内投与した。
抗腫瘍効果の指標として、各群でDay0、4、8、11、15、18、22、25、28におけるTVを算出し、下記式によりDay0に対する相対腫瘍体積(RTV;relative tumor volume)を求め、無処置群(control)のRTVと比較した。
結果を表2に示した。

FTD・TPI配合剤は、FTDの投与量として150mg/kg/dayとなるように調製した。Anti-mouse PD-1抗体は0.1mg/body/dayとなるように調製した。FTD・TPI配合剤はDay1-14に連日経口投与し、Anti-mousePD-1抗体はDay1、Day5及びDay9に腹腔内投与した。
抗腫瘍効果の指標として、各群でDay0、4、7、11、15、18、22、26、28におけるTVを算出し、上記式によりDay0に対する相対腫瘍体積(RTV)を求め、無処置群(control)のRTVと比較した。
上記RTVを各測定日についてプロットし、無処置群、FTD・TPI配合剤投与群、Anti-mousePD-1抗体単独投与群及びFTD・TPI配合剤とAnti-mousePD-1抗体の併用投与群の経日的推移を比較した結果を図3に示す。
また、Day28におけるRTV値に基づく腫瘍増殖抑制割合(TGI)を算出した。
結果を表4に示した。
また、両薬剤の併用効果が相加的と仮定した場合に予測される併用時のTGIは93.4%であるのに対し、実際には98.2%とより優れた抗腫瘍効果が確認されたことから、FTD・TPI配合剤とAnti-mousePD-1抗体の併用効果は相乗的であることが示された。
また、FTD・TPI配合剤とAnti-mousePD-1抗体併用投与群において、-20%超える重篤な体重減少は見られず、許容できる副作用であった。
実施例1に準じて、マウス大腸がん株(CMT-93)を移植したマウスに対して、FTD・TPI配合剤を75、100、150mg/kg/day(FTDとして)、Anti-mouse PD-1抗体を0.1mg/body/day投与し、抗腫瘍効果と体重減少を測定した。結果を図5~13、表6及び表7に示す。
また、両薬剤の併用効果が相加的と仮定した場合に予測される併用投与時のTGIは、FTDとして75、100、150mg/kg/dayの群でそれぞれ87.8%、90.1%、92.6%であるのに対し、実際には91.8%、95.7%、98.4%とより優れた抗腫瘍効果が確認されたことから、FTD・TPI配合剤とAnti-mousePD-1抗体の併用効果は相乗的であることが示された。
また、全ての併用投与群において、-20%超える重篤な体重減少は見られず、許容できる副作用であった。
また、FTD量を半量にしても、PD-1抗体の抗腫瘍効果を有意に増強できたことは、予想外の結果であった。
また、FTDが150mg/kg/dayの併用群においては、5匹中4匹で腫瘍の消失が確認できた。これは併用時の抗腫瘍効果が非常に高いことを裏付けるものである。
実施例1に準じて、マウス大腸がん株(CMT-93)を移植したマウスに対して、FTD・TPI配合剤を150mg/kg/day(FTDとして)、Anti-mouse PD-L1抗体(clone 10F.9G2、BioXCell社、以下同じ)を0.1、0.2mg/body/day投与し、抗腫瘍効果と体重減少を測定した。結果を図14~15、表8及び表9に示す。
また、両薬剤の併用効果が相加的と仮定した場合に予測される併用投与時のTGIは、Anti-mousePD-L1抗体が0.1、0.2mg/body/dayの群でそれぞれ74.7%、79.8%であるのに対し、実際には93.0%、93.5%とより優れた抗腫瘍効果が確認されたことから、FTD・TPI配合剤とAnti-mousePD-L1抗体の併用効果は相乗的であることが示された。
また、全ての併用投与群において、-20%超える重篤な体重減少は見られず、許容できる副作用であった。
マウス大腸がん株(CMT-93)を生後5~6週齢のC57BL/6NJclマウスの右側胸部に移植した。腫瘍移植後に腫瘍の長径(mm)及び短径(mm)を測定し、腫瘍体積(TV)を算出後、各群の平均TVが均等になるように各群にマウスを割り付け、群分け(n=8)を実施した日をDay0とした。
0.5%ヒドロキシプロピルメチルセルロース水溶液にテガフール・ギメラシル・オテラシルカリウム配合剤(「S-1」、テガフール:ギメラシル:オテラシル=1:0.4:1(モル比)、以下同じ)および化合物1((R)-N-(1-(3-(シクロペンチルオキシ)フェニル)エチル)-3-((2,4-ジオキソ-3,4-ジヒドロピリミジン-1(2H)-イル)メトキシ)プロパン-1-スルホンアミド)を懸濁した。S-1投与量は6.9mg/kg/day(テガフール量)に設定し(Anticancer Res.32:2807-2812(2012))、化合物1は、マウスにおいて投与可能な最高用量と推測される2400mg/kg/dayとした。Anti-mouse PD-1抗体は0.1mg/body/dayとなるように調製した。
群分けの翌日より1日1回28日間、被検動物に対しS-1及びS-1と化合物1の混合投与液を連日経口投与し、Anti-mousePD-1抗体はDay1、8、15,及び22に腹腔内投与した。また、control群は0.5%ヒドロキシプロピルメチルセルロース水溶液を1日1回28日間連日経口投与した。
抗腫瘍効果の指標として、各群でTVを算出し、下記式によりDay0に対する相対腫瘍体積(RTV;relative tumor volume)を求め、薬剤投与群と無処置群(control)のRTVの平均値から、以下記式よりTreated/control(T/C、%)を算出して抗腫瘍効果を評価した。
S-1+Anti-mousePD-1抗体投与群とAnti-mousePD-1抗体投与群で同じT/C(%)であったことから、明らかな増強効果は確認できなかった。
一方、S-1+化合物1+Anti-mousePD-1抗体投与群とS-1+化合物1投与群及びAnti-mousePD-1抗体投与群の比較では、3剤併用投与群はいずれの群に対しても統計上有意な併用効果が認められた。
以上から、フッ化ピリミジン系代謝拮抗薬であるS-1は単体では抗PD-1抗体の抗腫瘍効果を大きくは増強しないものの、S-1にdUTPase阻害薬である化合物1を併用することにより抗PD-1抗体の抗腫瘍効果を大きく増強することが明らかになった。つまり、本試験結果はDNA機能障害剤が抗PD-1抗体の抗腫瘍効果を増強できることを示すものである。
Claims (81)
- DNA機能障害剤と免疫調節剤を併用投与することを特徴とする抗腫瘍剤。
- DNA機能障害剤が、トリフルリジンを含有する薬剤、又はデオキシウリジントリホスファターゼ阻害薬及びフッ化ピリミジン系代謝拮抗薬を含有する薬剤である請求項1記載の抗腫瘍剤。
- DNA機能障害剤が、トリフルリジン及びチピラシル塩酸塩をモル比1:0.5で含有する配合剤である請求項1又は2記載の抗腫瘍剤。
- DNA機能障害剤が、(R)-N-(1-(3-(シクロペンチルオキシ)フェニル)エチル)-3-((2,4-ジオキソ-3,4-ジヒドロピリミジン-1(2H)-イル)メトキシ)プロパン-1-スルホンアミド又はその薬学的に許容される塩、及びフッ化ピリミジン系代謝拮抗薬を含有する薬剤である請求項1又は2記載の抗腫瘍剤。
- フッ化ピリミジン系代謝拮抗薬が、テガフール、ギメラシル及びオテラシルカリウムをモル比1:0.4:1で含有する配合剤、又はカペシタビンである請求項4記載の抗腫瘍剤。
- 免疫調節剤が、PD-1経路アンタゴニスト、ICOS経路アゴニスト、CTLA-4経路アンタゴニスト、CD28経路アゴニスト又はこれらの組み合わせである請求項1~5のいずれか1項記載の抗腫瘍剤。
- PD-1経路アンタゴニストが、抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体又はこれらの組み合わせである請求項6記載の抗腫瘍剤。
- 抗PD-1抗体がニボルマブ又はペムブロリズマブであり、抗PD-L1抗体がアテゾリズマブ、デュルバルマブ又はアベルマブである請求項7記載の抗腫瘍剤。
- CTLA-4経路アンタゴニストが、抗CTLA-4抗体である請求項6記載の抗腫瘍剤。
- 抗CTLA-4抗体がイピリムマブ又はトレメリムマブである請求項9記載の抗腫瘍剤。
- トリフルリジンの投与日における1日投与量が、単独で投与する場合における推奨用量の50~115%である請求項1~3及び6~10のいずれか1項記載の抗腫瘍剤。
- トリフルリジンの投与日における1日投与量が35~80mg/m2/dayである請求項1~3及び6~10のいずれか1項記載の抗腫瘍剤。
- 対象となる癌が、消化器癌、肺癌又は乳癌である請求項1~12のいずれかに記載の抗腫瘍剤。
- 対象となる癌が、大腸癌である請求項1~13のいずれか1項記載の抗腫瘍剤。
- 免疫調節剤の抗腫瘍効果を増強するための、DNA機能障害剤からなる抗腫瘍効果増強剤。
- DNA機能障害剤の抗腫瘍効果を増強するための、免疫調節剤からなる抗腫瘍効果増強剤。
- 免疫調節剤を投与された癌患者を治療するための、DNA機能障害剤からなる抗腫瘍剤。
- DNA機能障害剤を投与された癌患者を治療するための、免疫調節剤からなる抗腫瘍剤。
- 免疫調節剤と併用することを特徴とする、DNA機能障害剤からなる抗腫瘍剤。
- DNA機能障害剤と併用することを特徴とする、免疫調節剤からなる抗腫瘍剤。
- DNA機能障害剤を含む抗腫瘍剤と使用説明書を含むキット製剤であって、当該使用説明書には、癌患者に対して、DNA機能障害剤と免疫調節剤が併用投与されることが記載されていることを特徴とするキット製剤。
- 抗腫瘍剤を製造するための、DNA機能障害剤と免疫調節剤の使用。
- DNA機能障害剤が、トリフルリジンを含有する薬剤、又はデオキシウリジントリホスファターゼ阻害薬及びフッ化ピリミジン系代謝拮抗薬を含有する薬剤である請求項22記載の使用。
