Abstract
Die vorliegende Erfindung betrifft eine pharmazeutische Zusammensetzung umfassend Tyrothricin, Benzalkoniumchlorid, und/oder Benzocain zur Verwendung in der anti-viralen Therapie bzw. in der Behandlung einer Virus-Infektion.
Description
Zusammensetzung mit antiviralem Effekt
Gebiet der Erfindung
Die vorliegende Erfindung betrifft eine pharmazeutische Zusammensetzung umfassend Ty- rothricin, Benzalkoniumchlorid, und/oder Benzocain zur Verwendung in der anti-viralen Therapie bzw. in der Behandlung einer Virus-Infektion.
Hintergrund der Erfindung
Viren und Bakterien sind die Erreger der sehr häufig auftretenden akuten Pharyngitis. Zur Behandlung derselben sind bereits seit Jahrzehnten Medikamente auf dem Markt verfügbar, beispielsweise Dorithricin® Halstabletten Classic. Diese werden üblicherweise mit den darin enthaltenen antiseptischen und antibakteriellen Wirkstoffen zur klinisch effektiven Linderung der typischen Pharyngitis-Symptome Schluckschmerzen und Rachenschleimhautrötung eingesetzt.
Tyrothricin ist ein Gemisch verschiedener antibakteriell wirksamer linearer und cyclischer Polypeptide aus den Gruppen der Gramicidine und Tyrocidine. Sie werden endotoxinartig durch den anaeroben sporenbildenden Bacillus aneurinolyticus (Syn. Bacillus brevis) gebildet. Der Wirkungsbereich umfasst vorwiegend grampositive Bakterien, aber auch einige gramnegative Bakterien und verschiedene Pilzarten, wie beispielsweise Candida albicans. Tyrothricin enthält 50 bis 70 % Tyrocidine und 25 bis 50 % Gramicidine, die insgesamt mindestens 85 % des Wirkstoffs ausmachen. Daneben kommen in kleinen Mengen weitere strukturverwandte Polypeptide vor.
Benzalkoniumchlorid ist ein Gemisch von Alkylbenzyldimethylammoniumchloriden, deren Alkylteil aus C8- bis C18-Ketten besteht. Es besitzt allgemein desinfizierende und konservie-
rende Wirkung und wirkt gegen Bakterien, Pilze, Hefen und Algen (in geringem Maße auch antiviral).
Benzocain (4-Aminobenzoesäureethylester) ist ein Lokalanästhetikum und findet hauptsächlich zur lokalen Schmerztherapie der Haut und Schleimhaut, wie beispielsweise im Mund- und Rachenbereich, ebenso in Arzneimitteln gegen Erkältungskrankheiten, hustenstillende Zubereitungen, schmerzstillende Mittel wie Lösungen oder Lutschtabletten bei Hals-, Magen- und Zahnungsbeschwerden, Adstringenzien, Anwendung.
Zusammenfassung der Erfindung
Die vorliegende Erfindung betrifft eine pharmazeutische Zusammensetzung umfassend Ty- rothricin, Benzalkoniumchlorid, und/oder Benzocain zur Verwendung in der anti-viralen Therapie bzw. in der Behandlung einer Virus-Infektion. In einer bevorzugten Ausführungsform umfasst die Zusammensetzung alle drei genannten Wirkstoffe in Kombination.

Ausführliche Beschreibung der Erfind
In einer ln-vitro- Studie wurde die dosisabhängige antivirale Wirkung der oben genannten Wirkstoffe gegen die humanpathogenen Viren RSV, HRV14 und HlNl untersucht. Die Daten dieser Studie demonstrieren, dass eine aus den Wirkstoffen hergestellte und auf eine hohe physiologische Wirkstoffkonzentration eingestellte Lösung die virale Aktivität um 68,4% (HRV14), 52,7% (HlNl) bzw. 74,3% (RSV) reduziert. Erst bei sehr hoher Wirkstoffverdünnung war der antivirale Effekt nicht mehr nachweisbar. Eine pharmazeutische Zusammensetzung umfassend Tyrothricin, Benzalkoniumchlorid und Benzocain zeichnet sich somit durch ausgeprägte, dosisabhängige und synergistische antivirale Eigenschaften gegen die Viren RSV, HRV14 und HlNl aus.
Diese Beobachtung ist neu und unerwartet, weil die aktiven Wirkstoffe
Tyrothricin als Antibiotikum als aktiv gegen Bakterien wirkend eingestuft wird, Benzalkoniumchlorid und Benzocain als Analgetika gegen Schmerzen wirkend klassifiziert sind (1-4).
Die vorliegende Erfindung betrifft also eine pharmazeutische Zusammensetzung umfassend
Tyrothricin,
Benzalkoniumchlorid, und/oder
Benzocain
zur Verwendung in der anti-viralen Therapie bzw. in der Behandlung einer Virus-Infektion.
Die Zusammensetzung der vorliegenden Erfindung kann die oben genannten Wirkstoffe umfassen, aus diesen bestehen oder im Wesentlichen aus diesen bestehen. Die Formulierung "besteht im Wesentlichen aus" bedeutet, dass die pharmazeutische Zusammensetzung diese Bestandteile als wirksame Bestandteile enthält. Sie kann jedoch weiterhin ein oder mehrere Lösungsmittel, Hilfsstoffe etc. enthalten, die zwar zur Herstellung einer pharmazeutischen Zusammensetzung notwendig sind, jedoch nicht oder nicht wesentlich zur pharmakologischen Wirksamkeit beitragen.
Der Begriff„pharmazeutische Zusammensetzung" wie hierin verwendet, umfasst insbesondere perorale Darreichungsformen, beispielsweise feste, halbflüssige oder flüssige Zusammensetzungen zur oralen Verabreichung. In einer bevorzugten Ausführungsform liegt die Zusammensetzung in Form einer Tablette, insbesondere einer Lutschtablette bzw. Halstablette vor.
Alternativ sind jedoch auch andere Darreichungsformen möglich, beispielsweise feste perorale Darreichungsformen wie Pulver oder Kapseln. Hierbei können begrenzte Zusätze von Mag- nesiumstearat oder Calciumbehenat als Gleitmittel und von Stärke als Zerfallsbeschleuniger verwendet werden. Im Falle der Granulation können wässrige Lactose- Lösung, Ethanol und geeignete Konzentrationen von Stärkekleistern eingesetzt werden.
Übliche Hilfsstoffe zur Herstellung einer Halstablette umfassen beispielsweise Sorbitol (E 420), Talkum, Saccharose- Fettsäureester, Saccharin-Natrium-Dihydrat, Minzöl, Povidon 25, und Carmellose-Natrium.
Neben peroralen Arzneiformen kann die pharmazeutische Zusammensetzung der vorliegenden Erfindung Parenteralia, d.h. flüssige Verdünnungen zur Injektion sowie flüssige Einreibungen sowie Salben, Suppositorien, Augentropfen, Nasentropfen umfassen.
In einer bevorzugten Ausführungsform wird die Zusammensetzung zur Behandlung einer durch Rhinoviren, Influenzaviren, und/oder Pneumoviren hervorgerufenen Infektion verwendet.
In einer Ausführungsform findet die pharmazeutische Zusammensetzung Verwendung in einem Verfahren zur Behandlung einer viralen Infektion, wobei die virale Infektion durch HRV14 (humanes Rhinovirus Typ 14), RSV (humanes Respiratory-Syncytial-Virus), und/oder H1N1 (Influenza-A-Virus H1N1) hervorgerufen wird.
Bevorzugt enthält eine Einzeldosis der pharmazeutischen Zusammensetzung ungefähr 0,5 mg Tyrothricin, ungefähr 1,0 mg Benzalkoniumchlorid und/oder ungefähr 1,5 mg Benzocain sowie einen oder mehrere pharmazeutisch verträgliche Hilfsstoffe. Der Begriff "ungefähr" be-
deutet in diesem Zusammenhang eine Variationsbreite von ± 50%, bevorzugt ± 20%, am meisten bevorzugt ± 10% bezogen auf die oben angegebene Wirkstoff masse.
Die pharmazeutische Zusammensetzung der vorliegenden Erfindung wird in einer täglichen Dosis von ungefähr 2-4 mg Tyrothricin, 4-8 mg Benzalkoniumchlorid und 6-12 mg Benzocain verabreicht.
Die vorliegende Erfindung wird nunmehr unter Bezugnahme auf die nachfolgenden Ausführungsbeispiele erläutert, in denen die Effekte der erfindungsgemäßen Zusammensetzung dargelegt werden.
Ausführungsbeispiele
1 Einleitung
Viren und Bakterien sind die Erreger der sehr häufig auftretenden akuten Pharyngitis. Bei dieser auch als Rachenkatarrh bekannten Erkrankung handelt es sich um eine Entzündung der Rachenschleimhaut und der darunterliegenden pharyngealen Strukturen, die mit den typischen Symptomen Schluckschmerzen, Kratzen, Brennen, Trockenheitsgefühl im Hals und Rötung der Rachenschleimhaut einhergeht (1).
Vorrangig erfordert die akute Pharyngitis eine symptomatische Behandlung zur Linderung der Schmerzen. Die antibiotische Therapie ist seltener indiziert und sollte nur bei den 10-15% der Patienten angewendet werden, die von einer Streptokokken-Pharyngitis betroffen sind (1-3), bzw. in den Fällen, in denen Spätfolgen wie hämorrhagisches Fieber verhindert werden sollen. Halsschmerztabletten wie Dorithricin® haben sich bei der symptomatischen Behandlung wiederholt klinisch bewährt (4). Die darin enthaltene Kombination aus den antibakteriellen und analgetischen Wirkstoffen Tyrothricin, Benzalkoniumchlorid und Benzocain lindert effektiv die typischen Pharyngitis-Beschwerden (4).
Interessanterweise wirken einzelne Bestandteile dieser Kombination nicht nur antibakteriell, sondern wie wir hier beschreiben auch antiviral. Fraglich, ob von der in Dorithricin® enthalte-
nen Wirkstoffkombination als Ganze eine antivirale Wirkung ausgeht. Vor diesem Hintergrund wurde die im Folgenden vorgestellte Untersuchung durchgeführt mit dem Ziel, die dosisabhängige antivirale Wirkung von Dorithricin® Halstabletten Classic gegen humanpathogene Viren in vitro zu untersuchen.
2 Material und Methoden
Getestet wurden in Lösung gebrachte Dorithricin®-Tabletten (MEDICE, Iserlohn, Deutschland) in verschiedenen Verdünnungsstufen auf Viren, die beim Menschen Infektionen der Atemwege hervorrufen. Eine Tablette enthält Tyrothricin 0,5 mg, Benzalkoniumchlorid 1,0 mg, und Benzocain 1,5 mg. Es handelte sich hierbei um folgende Viren: humanes Rhinovirus Typ 14 (HRV14, unbehülltes RNA-Virus), das Respiratory Syncytial Virus (RSV, Paramyxovirus, behüll- tes RNA-Virus) und das Influenza-A-Virus H1N1 (FluA, H1N1, Orthomyxovirus, behülltes RNA- Virus).
Zur Herstellung einer Prüflösung wurde eine Dorithricin®-Tablette zunächst pulverisiert, dann mit 1 ml Dimethylsulfoxid (DMSO) angelöst, mit destilliertem Wasser auf 1:100 verdünnt und sterilfiltriert (0,45 μηΊ-Filter). Von dem auf pH 7,4 eingestellten Filtrat wurden mittels Komplettnährmedium in log2- bzw. loglO-Stufen Verdünnungsreihen hergestellt. Das gemäß der beschriebenen Prozedur aufbereitete Lösungsmittel (DMSO) wurde als Lösungsmittelkontrolle verwendet.
Um Artefakte aufgrund von vitalitäts- bzw. stoffwechselreduzierenden Effekten der Prüflösungen auf die Zellen für die antivirale Testung auszuschließen, wurden in einem ersten Schritt die Prüflösungen einem Vitalitätstest in der Zellkultur unterzogen. In Parallelansätzen wurden die Prüflösungen in sämtlichen Verdünnungsstufen sowie die Lösungsmittelkontrolle zu wachsenden virussensitiven HeLa-, MDCK- und HEp-2-Zellen gegeben und für insgesamt
3 Tage (HeLa, MDCK) bzw. 6 Tage (HEp-2) bei 37°C und 5% C02 kultiviert. Als weitere Kontrolle wurden nur mit dem Zellkulturmedium kultivierte Zellen in der Zellkultur mitgeführt. Die Beurteilung möglicher vitalitätsreduzierender Effekte erfolgte an den Tagen 1, 2, 3 und 6 nach Substanzgabe mittels des MTT-Tests. Dazu wurden die behandelten Zellen für 45 Minuten (HEp-2, HeLa) bzw. für 60 Minuten mit der MTT-Lösung inkubiert. Die Reduktion des da-
rin enthaltenen gelben Farbstoffs 3-(4,5-Dimethylthiazol-2-y)-2,5-diphenyltetrazoliumbromid (MTT) in blau-violettes Formazan kann nur von intakten, vitalen Zellen durchgeführt werden und ist ein Nachweis für Zellvitalität. Nach Solubilisierung der Formazankristalle in 1% DMSO wurde die optische Dichte der Zellkulturüberstände photometrisch bei einer Wellenlänge von 570 nm bestimmt. Eine höhere optische Dichte deutet auf eine höhere Anzahl von vitalen Zellen hin. Des Weiteren wurde eine eventuelle Veränderung der Zellmorphologie am Tag 3 (HeLa, MDCK) und Tag 6 (HEp-2) nach Substanzzugabe mikroskopisch beurteilt. Die OD- Werte der Prüfmusteransätze wurden auf die als 100% Vitalität definierten OD-Werte der unbehandelten Kontrollen normiert und entsprechend als Prozentwerte angegeben. Somit konnte die mittlere inhibitorische Konzentration (IC50) über mehrere Parallelproben in Dosiswirkungskurven der Prüfmuster bestimmt werden, bis hin zu der Konzentration, die keinerlei testsubstanzbedingte Beeinträchtigung der Zellvitalität aufwies und sich somit für den Einsatz in den antiviralen Prüfungen eignete.
Im Anschluss an die Vitalitätsprüfungen erfolgte die Testung der antiviralen Aktivität gegen HRV14, FluA und RSV. Dazu wurden die virussensitiven Zellen mit den entsprechenden Viren in einer mittleren Infektionsdosis (MOI) zwischen 0,0004 und 0,0008 infiziert (HeLa mit HRV14; MDCK mit FluA; HEp-2 mit RSV). Die virusinfizierten Zellen wurden 1 h post infectio- nem mit den Prüflösungen in physiologischen Substanzkonzentrationen im semipermeablen Aga roseüberstand behandelt. An Tag 2 (FluA), Tag 3 (HRV14) und Tag 5 (RSV) nach Testansatz wurden im Plaquereduktionsassay (plaque forming units/ml; PFU/ml) die im Zellrasen gebildeten Virusplaques mittels eines AID EliSpot Reader Systems (Autoimmun Diagnostika GmbH, Strassberg, Deutschland) ausgezählt und als prozentuale Hemmung gegenüber der als vollständige Virusinfektion definierten Plaquezahl im keinen Wirkstoff enthaltenden Kontrollansatz ausgewertet.
Zur Verifizierung des beschriebenen Testverfahrens wurde neben den Virus- und Negativkontrollen der antivirale Wirkstoff Ribavirin Virazole® gegen RNA-Viren als laborinterne Kontrollen mitgeführt. Die Effektivität der Referenzsubstanzen konnte im Testsystem bestätigt werden. Alle Kontrollen waren sämtlichen Kultivierungsbedingungen der antiviralen Assays unterworfen.
3 Ergebnisse
Für den antiviralen Test wurden die Prüflösungen mit Wirkstoffkonzentrationen verwendet, die der Verdünnung von 1:400 und höher einer Dorithricin®-Tablette entspricht (Tabelle 1). Bei diesen Wirkstoffkonzentrationen konnten im Vitalitätstest keine Vitalitätsreduzierenden Effekte auf die verwendeten HeLa-, MDCK- und HEp2-Zellen nachgewiesen werden.
Im Ergebnis wies die 1:100 und die 1:400 verdünnte Dorithricin®-Wirkstoffkombination eine beachtliche antivirale Wirkung auf und reduzierte deutlich die Plaques der RNA-Viren HRV14, HlNl und RSV um 97% bzw. um 68%, 53% bzw. 74%. Dieser Effekt auf die PIaquebildung nahm mit zunehmender Verdünnung ab. Erst bei hohen Verdünnungen war eine Wirkung nicht mehr nachweisbar: bei HRV14 ab 1:3200, bei RSV ab 1:6400 und bei HlNl ab 1:64000. Die relative Hemmwirkung in Abhängigkeit von dem Dorithricin®-Verdünnungsgrad ist in Abbildung 1 dargestellt.
Die synergistische Wirkung der 3 aktiven Substanzen wird belegt durch die selbige Versuchsreihe mit jeweils der Einzelsubstanz, die jeweils nur sehr schwache antivirale Aktivität bei Verdünnung 1:100 zeigen (Abbildung 2, 3, 4).
Das zur Kontrolle eingesetzte Ribavirin (RIBA) erbrachte eine Viruspiaque-Reduktion von 53% bei HRV14 (RIBA 10 μg/m\), von 96% bei FluA (RIBA 3 μg/m\) und von 97% bei RSV (RIBA 1 μg/ml).
Der zugrundeliegende Wirkmechanismus wurde durch weitere Versuche aufzuklären versucht. Dabei wurde geprüft, ob diesem ein synergistischer Effekt zugrunde liegt und welche der drei Komponenten gegen welche Viren die Hauptwirkung besitzt.

4 Fazit
Die pharmazeutische Zusammensetzung der vorliegenden Erfindung zeichnet sich neben den bereits bekannten antibakteriellen und analgetischen Wirkungen zusätzlich durch ausgeprägte, dosisabhängige und synergistische antivirale Eigenschaften gegen die Viren RSV, HRV14 und H1N1 aus.

Claims
1. Pharmazeutische Zusammensetzung umfassend
Tyrothricin,
Benzalkoniumchlorid, und/oder
Benzocain
zur Verwendung in der anti-viralen Therapie oder in der Behandlung einer Virus-Infektion.
2. Pharmazeutische Zusammensetzung zur Verwendung in einem Verfahren zur Behandlung einer viralen Infektion nach Anspruch 1, wobei die virale Infektion durch Rhinoviren, Influenzaviren, und/oder Pneumoviren hervorgerufen wird.
3. Pharmazeutische Zusammensetzung zur Verwendung in einem Verfahren zur Behandlung einer viralen Infektion nach Anspruch 1 oder 2, wobei die virale Infektion durch HRV14 (humanes Rhinovirus Typ 14), RSV (humanes Respiratory-Syncytial-Virus), und/oder HlNl (Influenza-A-Virus HlNl) hervorgerufen wird.
4. Pharmazeutische Zusammensetzung zur Verwendung in einem Verfahren zur Behandlung einer viralen Infektion nach einem oder mehreren der vorhergehenden Ansprüche, umfassend
ungefähr 0,5 mg Tyrothricin,
ungefähr 1,0 mg Benzalkoniumchlorid,
ungefähr 1,5 mg Benzocain
als Einzeldosis und einen oder mehrere pharmazeutisch verträgliche Hilfsstoffe.
5. Pharmazeutische Zusammensetzung zur Verwendung in einem Verfahren zur Behandlung einer viralen Infektion nach einem oder mehreren der vorhergehenden Ansprüche, bei der es sich um eine Halstablette handelt.
6. Pharmazeutische Zusammensetzung zur Verwendung in einem Verfahren zur Behandlung einer viralen Infektion nach einem oder mehreren der vorhergehenden Ansprüche, wo-
bei die Zusammensetzung in einer täglichen Dosis von ungefähr 2-4 mg Tyrothricin, 4-8 mg Benzalkoniumchlorid und 6-12 mg Benzocain verabreicht wird.
