ナンドロロンデカン酸エステル、メテノロン酢酸エステルの眼瞼皮膚投与がウサギにおいて、涙液中のTL量を増加させるか否かを検討した。
表1に示す組成となるように、1%(w/v)ナンドロロンデカン酸エステル含有クリーム、1%(w/v)メテノロン酢酸エステル含有クリームおよび基剤を調製した。
試験にはウサギ(日本白色種、雌性、北山ラベスより購入)を用いた。予め、全身麻酔下でウサギの下方涙腺および瞬膜を切除しておき、それに加えてアンドロゲンの産生抑制を目的として薬物塗布の4週間前からケトコナゾールを連日経口投与した。塗布開始前にバリカンを用いて眼瞼周囲部の除毛を行ったのち、エリザベスカラーを装着し、薬物投与群には片眼の上眼瞼皮膚に1%(w/v)ナンドロロンデカン酸エステル含有クリーム、もしくは1%(w/v)メテノロン酢酸エステル含有クリームを、1日1回連日塗布(各40μL)し、反対眼は無処置とした(n=4)。基剤群には基剤を同様に塗布した(n=4)。
塗布前および塗布開始から2週間後に、シルメル試験紙(昭和薬品化工株式会社製)を下眼瞼結膜嚢に挿入して涙液を回収し、回収した各涙液検体をスラブゲル電気泳動に供した。泳動後のゲルをクマシ―ブリリアントブルー染色して、分子量約17キロダルトンのTLバンドを含む全ての蛋白質バンドを検出し、ゲルの画像解析によって涙液総蛋白質中のTL百分率(%TL)を算出した。各個体で塗布前の%TLと塗布後の%TLの差分を求め、%TL増加量(Δ%TL)とした。
表2に示すように、薬物投与群では基剤群と比べてTLの大幅な増加が認められることから、ナンドロロンデカン酸エステル、メテノロン酢酸エステルの眼瞼皮膚投与が涙液中TLを増加させることが示された。
以上の結果から、ナンドロロンまたはそのエステル、メテノロンまたはそのエステルがTL量を増加させることを見出し、ドライアイに対して治療効果を有することが示された。特に、ナンドロロンデカン酸エステルの眼瞼皮膚投与は、メテノロン酢酸エステルと比較して、TL量を増加させることから、ドライアイに対して強い治療効果を有することが示唆された。
メテノロン酢酸エステルの眼瞼皮膚投与後の涙腺中メテノロン濃度の経時変化を検討した。
メテノロン酢酸エステル0.4gをヒマシ油で20mLにメスアップして溶解させ、2%(w/v)メテノロン酢酸エステル含有軟膏を調製した。
試験にはウサギ(日本白色種、雄性、北山ラベスより購入)を用いた。薬物投与前日に、全身麻酔下でバリカンにより右上眼瞼を毛刈りし、投与前にエリザベスカラーを装着した。2%(w/v)メテノロン酢酸エステル含有軟膏(50μL)を右上眼瞼皮膚に単回塗布した。
投与後2、4、8および24時間にウサギを安楽殺処置し、右上方涙腺組織を摘出した(各時点3匹)。涙腺組織をメタノールでホモジナイズし、遠心上清中のメテノロン(メテノロン酢酸エステルの活性本体)濃度をLC-MS/MSにより測定した。ホモジナイズ前に測定した組織湿重量と測定検体のメテノロン濃度から、涙腺組織中メテノロン濃度を算出した。
図1に示すように、メテノロン酢酸エステルの眼瞼皮膚投与後の涙腺中メテノロン濃度は投与後4時間から24時間までほとんど変化することなく、長時間維持されることが確認された。
以上のように、メテノロン酢酸エステルを眼瞼皮膚投与した場合、涙腺組織中のメテノロン濃度は少なくとも24時間持続することから、メテノロンまたはそのエステルは、1日1回の頻度で眼瞼皮膚に投与することで、継続的にドライアイに対して治療効果を示すことが示された。
ナンドロロンデカン酸エステルの眼瞼皮膚投与および点眼投与後の涙腺中ナンドロロン濃度の経時変化を検討した。
ナンドロロンデカン酸エステル0.4gをヒマシ油で20mLにメスアップして溶解させ、2%(w/v)ナンドロロンデカン酸エステル含有軟膏を調製した。また、同一組成の製剤をナンドロロンデカン酸エステル含有点眼液として利用した。
試験にはウサギ(日本白色種、雄性、北山ラベスより購入)を用いた。薬物投与前日に、全身麻酔下でバリカンにより右上眼瞼を毛刈りし、投与前にエリザベスカラーを装着した。そのうち1群(n=4)には、2%(w/v)ナンドロロンデカン酸エステル含有軟膏(50μL)を右上眼瞼皮膚に単回塗布した。
投与後2、4、8および24時間にウサギを安楽殺処置し、右上方涙腺組織を摘出した(各時点3匹)。涙腺組織をメタノールでホモジナイズし、遠心上清中のナンドロロン(ナンドロロンデカン酸エステルの活性本体)濃度をLC-MS/MSにより測定した。ホモジナイズ前に測定した組織湿重量と測定検体のナンドロロン濃度から、涙腺組織中ナンドロロン濃度を算出した。
図2に示すように、ナンドロロンデカン酸エステルの眼瞼皮膚投与後の涙腺中ナンドロロン濃度は投与後4時間から少なくとも24時間まで高濃度で維持されることが確認された。一方で、ナンドロロンデカン酸エステルの点眼投与後の涙腺中ナンドロロン濃度は投与後少なくとも24時間まで維持されることが確認されたが、眼瞼皮膚投与した場合のそれよりもはるかに低いことが明らかとなった。
以上から、ナンドロロンデカン酸エステルを眼瞼皮膚投与した場合、これを点眼投与した場合と比較して、より強いドライアイ治療効果が得られることが示された。さらに、ナンドロロンデカン酸エステルを眼瞼皮膚投与した場合の涙腺組織中のナンドロロン濃度は少なくとも24時間高濃度で維持することから、ナンドロロンまたはそのエステルは、1日1回の頻度で眼瞼皮膚に投与することで、継続的にドライアイに対して治療効果を有することが示された。
ナンドロロンデカン酸エステル、メテノロン酢酸エステルを涙腺組織細胞に作用させ、Lipophilin CL2(L-CL2)の遺伝子に対する両剤の発現量亢進作用を比較、検討した。なお、Lipophilin類は涙腺において合成され、ヒトやウサギの涙液中に多量に認められる蛋白質である。
各薬剤をDMSOに溶解した後、DMSO終濃度が0.1%となるようDMEM/F-12(D/F培地)で希釈した。
ウサギ(日本白色種、雌性、北山ラベスより購入)より採取した涙腺を細切し、EDTAを含むHBSS中での振盪処理およびコラゲナーゼ、ヒアルロニダーゼおよびDNase Iを含むDMEM中での振盪処理を繰り返して涙腺組織を消化した。未消化組織を取り除いた後、2%~4%Ficollの密度勾配遠心を行って単離涙腺細胞を調製した。細胞をD/F培地に懸濁し、I型コラーゲンをコートした96穴プレートに播種した。37℃、5%CO2+95%気相下で2日間細胞を培養した後、各種濃度の薬剤を含むD/F培地に培地を交換してさらに2日間培養した。
薬剤処置した涙腺細胞からCellAmp Direct RNA Prep KitとPrimeScript RT MasterMix(いずれもタカラバイオ)を用いて合成したcDNA、QuantiFast SYBR green PCR kit(Qiagen)、Rabbit L-CL2遺伝子(Accession番号:NM_001082095)およびRabbit GAPDH遺伝子(Accession番号:NM_001082253)の各々に合わせて設計したプライマーを用いて定量的PCRを行った。薬剤処置した各細胞のL-CL2遺伝子発現レベルは各々のGAPDH遺伝子発現で補正を行い、基剤(0.1%DMSOを含むD/F培地)を処置した細胞の発現レベルに対する相対比で表現した。
図3に示すように、ナンドロロンデカン酸エステル(ND)、メテノロン酢酸エステル(MA)は涙腺細胞のL-CL2発現亢進作用を有し、その強度はほぼ同程度であることが示された。
ナンドロロンデカン酸エステル、メテノロン酢酸エステルを涙腺組織細胞に作用させたところ、いずれもL-CL2遺伝子発現量亢進作用を示し、涙腺賦活化効果は同程度である。それにも関わらず、前記薬理試験(1)で記載したように、ナンドロロンデカン酸エステル製剤の眼瞼皮膚投与は同濃度のメテノロン酢酸エステル製剤の眼瞼皮膚投与よりもTLを増強する効果が強かった。さらに、前記薬物動態試験(1)及び(2)で記載したように、眼瞼皮膚投与時の活性本体の涙腺移行量は、メテノロン酢酸エステルよりもナンドロロンデカン酸エステルの方が少量であるから、メテノロン酢酸エステルと比較して、ナンドロロンデカン酸エステルは涙液蛋白質の特にTLに対して強力な増加作用を有する。
製剤例を挙げて本発明の薬剤をさらに具体的に説明するが、本発明はこれらの製剤例にのみ限定されるものではない。

Claims (63)
- ナンドロロン、そのエステル若しくはそれらの塩、またはメテノロン、そのエステル若しくはそれらの塩を有効成分として含有するドライアイ治療剤。
- 眼瞼皮膚に投与されることを特徴とする、請求項1記載のドライアイ治療剤。
- ナンドロロンデカン酸エステルを有効成分として含有する、請求項1または2記載のドライアイ治療剤。
- メテノロン酢酸エステルを有効成分として含有する、請求項1または2記載のドライアイ治療剤。
- 0.1~10%(w/w)の濃度のナンドロロンデカン酸エステルまたはメテノロン酢酸エステルを有効成分として含有する、請求項3または4記載のドライアイ治療剤。
- 1~5%(w/w)の濃度のナンドロロンデカン酸エステルまたはメテノロン酢酸エステルを有効成分として含有する請求項3または4記載のドライアイ治療剤。
- 眼瞼皮膚に1日1回投与されることを特徴とする、請求項1~6のいずれか一項記載のドライアイ治療剤。
- 投与剤型が軟膏またはクリーム剤である、請求項1~7のいずれか一項記載のドライアイ治療剤。
- 投与剤型が貼付剤である、請求項1~7のいずれか一項記載のドライアイ治療剤。
- ナンドロロン、そのエステル若しくはそれらの塩、またはメテノロン、そのエステル若しくはそれらの塩を有効成分として含有する、涙腺からの分泌物の分泌を促進するための組成物。
- 眼瞼皮膚に投与されることを特徴とする、請求項10記載の組成物。
- ナンドロロンデカン酸エステルを有効成分として含有する、請求項10または11記載の組成物。
- メテノロン酢酸エステルを有効成分として含有する、請求項10または11記載の組成物。
- 0.1~10%(w/w)の濃度のナンドロロンデカン酸エステルまたはメテノロン酢酸エステルを有効成分として含有する、請求項12または13記載の組成物。
- 1~5%(w/w)の濃度のナンドロロンデカン酸エステルまたはメテノロン酢酸エステルを有効成分として含有する請求項12または13記載の組成物。
- 眼瞼皮膚に1日1回投与されることを特徴とする、請求項10~15のいずれか一項記載の組成物。
- 投与剤型が軟膏またはクリーム剤である、請求項10~16のいずれか一項記載の組成物。
- 投与剤型が貼付剤である、請求項10~16のいずれか一項記載の組成物。
- ナンドロロン、そのエステル若しくはそれらの塩、またはメテノロン、そのエステル若しくはそれらの塩及び薬学的に許容される添加剤を含有する、ドライアイの治療用の医薬組成物。
- 眼瞼皮膚に投与されることを特徴とする、請求項19記載の医薬組成物。
- ナンドロロンデカン酸エステルを有効成分として含有する、請求項19または20記載の医薬組成物。
- メテノロン酢酸エステルを有効成分として含有する、請求項19または20記載の医薬組成物。
- 0.1~10%(w/w)の濃度のナンドロロンデカン酸エステルまたはメテノロン酢酸エステルを有効成分として含有する、請求項21または22記載の医薬組成物。
- 1~5%(w/w)の濃度のナンドロロンデカン酸エステルまたはメテノロン酢酸エステルを有効成分として含有する、請求項21または22記載の医薬組成物。
- 眼瞼皮膚に1日1回投与されることを特徴とする、請求項19~24のいずれか一項記載の医薬組成物。
- 投与剤型が軟膏またはクリーム剤である、請求項19~25のいずれか一項記載の医薬組成物。
- 投与剤型が貼付剤である、請求項19~25のいずれか一項記載の医薬組成物。
- ドライアイの治療用の医薬の製造のためのナンドロロン、そのエステル若しくはそれらの塩、またはメテノロン、そのエステル若しくはそれらの塩の使用。
- 医薬が眼瞼皮膚に投与されることを特徴とする、請求項28記載の使用。
- 医薬がナンドロロンデカン酸エステルを有効成分として含有する、請求項28または29記載の使用。
- 医薬がメテノロン酢酸エステルを有効成分として含有する、請求項28または29記載の使用。

- 医薬が眼瞼皮膚に1日1回投与されることを特徴とする、請求項28~33のいずれか一項記載の使用。
- 医薬の投与剤型が軟膏またはクリーム剤である、請求項28~34のいずれか一項記載の使用。

- 0.1~10%(w/w)の濃度のナンドロロンデカン酸エステルまたはメテノロン酢酸エステルを投与することを特徴とする、請求項39または40記載のナンドロロン、そのエステル若しくはそれらの塩、またはメテノロン、そのエステル若しくはそれらの塩。
- 1~5%(w/w)の濃度のナンドロロンデカン酸エステルまたはメテノロン酢酸エステルを投与することを特徴とする、請求項39または40記載のナンドロロン、そのエステル若しくはそれらの塩、またはメテノロン、そのエステル若しくはそれらの塩。
- 眼瞼皮膚に1日1回投与されることを特徴とする、請求項37~42のいずれか一項記載のナンドロロン、そのエステル若しくはそれらの塩、またはメテノロン、そのエステル若しくはそれらの塩。
