- the present invention proposes a device for the proximal locking of an intrameduUary nail for osteosynthesis of fractures in the trochanteric region of the femur, said nail being provided with a transverse through-hole, characterized in that it comprises a transcervical longitudinal element self-threading at one end for fixing to the head of the femur and provided at the other end with an opening for receiving an insertion instrument which performs a translational movement thereof inside a bell which can be stably positioned inside said transverse hole of said nail, during said translation a rotation of said transcervical longitudinal element inside said bell being prevented, said translation being able to occur between two opposite stop positions, at least one of which is determined by the bearing contact of a pin which extends integrally from the internal surface of said bell into a slot formed on the surface of said transcervical longitudinal element.

- Said element consists of a bell 15, the cross-section of which is narrower at 17 at the opposite end to the threaded end 12 of the transcervical element 11 which it contains, this narrowed section 17 thus forming an stop stop for the transcervical element 11 which is free to move inside the bell.
- an opening 13 with a narrowed cross-section for receiving an insertion instrument, of which in Figs. 1 and 2 for the sake of brevity only the end 14 having a shape matching the opening 13 is shown.
- the external surface of the bell is tapered at 16, while in an intermediate position 18 it is threaded or corrugated or knurled with a series of recesses and/or projections.
- a pin 8 extends integrally from the internal surface of said bell so as to engage with said slot.
- the element 11 is thus able to translate without rotation inside the bell 15 between two opposite stop positions defined in the example of Figs. 1-3 by the bearing contact of the pin 8 against the opposite edges of the slot 9.
- All the components described are made of biocompatible materials, for example stainless steel.
- said device 10 comprising the bell 15 and the transcervical longitudinal element 11 is inserted inside a transverse through-hole 19 of an intrameduUary nail 20 for osteosynthesis of fractures in the trochanteric region of the femur, using the surgical technique normally adopted for this kind of femoral nail.
- the nail in question has a cavity 21 along its longitudinal axis, which is threaded at 25 at the top with respect to the said transverse through-hole 19, for receiving a means 22 for clamping the device 10, for example a threaded pin terminating at the bottom in a frustoconical/cylindrical section 23 which creates with its edge a notch able to interfere with the said threading 18 formed on the external surface of the bell 15 so as to lock it on the intrameduUary nail 20.

- the element 11 is displaced inside the bell 10 until the pin 8 makes bearing contact against the edge of the slot 9 in the position again shown in Fig. 1.
- the end of the element 11 is located at the maximum distance from the stop represented by the section 17 of the bell.
- the distance indicated by 24 represents the maximum working stroke which the transcervical element 11 is able to perform under compression, for example as a result of the static or dynamic load exerted on the femur of the patient in whom the nail has been implanted, and therefore on the fissures of the fracture.
- Fig. 3 shows the other limit position of displacement of the element 11 inside the bell as far as the stop position represented by the bearing contact of the end of 11 against the section 17 of the bell, corresponding in the example shown to the bearing contact of the other end of the slot 9 against the pin 8.
- the situation shown in Fig. 3 therefore represents the situation of maximum compression permitted on the fracture fissures by the device according to the invention.
- a means for example a spring 26, for opposing the pressure exerted by the static or dynamic load on the fracture fissures, thus acting as a damping device, has been arranged between the bottom end of the element 11 and the narrowed section 17 which acts as the base of the bell 15.
- a rigid means for preventing or limiting the compression stroke such as for example a screw or a stopper or a plug. This may be appropriate in certain clinical cases where it is not desired to use the whole compression (working stroke 24 shown in Fig. 2) made available by the device according to the invention.
- the characteristic feature of preventing rotation between the transcervical element and bell may be provided solely by the same pin of the bell which engages with the slot of the transcervical element and not by the particular anti-rotational geometry of their sections.
- the cross-section of the displaceable transcervical element/bell pair will therefore be circular.
- the same pin/slot pair could moreover provide itself both the opposite stop positions described above without using narrowed sections of the bell acting as a stop surface for the base of the transcervical element.
