Description

IMPLANT DE PROTHESE DENTAIRE DENTAL PROSTHESIS IMPLANT
La présente invention a trait au domaine des prothèses dentaires et notamment aux moyens permettant d'assurer a l'implant d'une prothèse dentaire, une forme s ' adaptant le plus parfaitement possible et dans les meilleures conditions de bio-compatibiliteThe present invention relates to the field of dental prostheses and in particular to the means making it possible to provide the implant of a dental prosthesis with a shape that adapts as perfectly as possible and under the best conditions of bio-compatibility.
In general, the invention applies more particularly to implants of the type consisting of a rod formed from a racmal front body for anchoring in the bone tissue formed by a thread defined by a thread. helical spongy type to be received in the bone and to be retained there by osteomtegration,
- et d'un corps postérieur aérien servant de support a l fixation d'une couronne prothetique partie esthétique de la dent réalisée classiquement en ceramo-metal- and an aerial posterior body serving as a support for fixing a prosthetic crown, aesthetic part of the tooth conventionally made of ceramic-metal
Bien qu'ils constituent un progrès indéniable dans le domaine des prothèses dentaires ces implants présentent pourtant plusieurs inconvénients, a savoir entre autres Ue la partie filetée du corps antérieur racmal d'ancrage, s ' arrêtant au ras de l'os laisse des risques d'invagination et de crateπsation lies a la migration bactérienne En outre, ils ne présentent aucune stabilisation latérale lors des mises en charge et leur ancrage dans la couronne prothetique dentaire est mal aise, car il ne comporte pas de composant prothetiqueAlthough they constitute an undeniable progress in the field of dental prostheses, these implants nevertheless have several disadvantages, namely, inter alia U e the threaded part of the anterior racmal anchoring body, stopping close to the bone leaves risks invagination and crateπsation related to bacterial migration In addition, they have no lateral stabilization during loading and their anchoring in the dental prosthetic crown is uncomfortable, because it does not contain any prosthetic component
Based on this state of affairs and the strengths of their respective trades and combined micromecamcian and dental practitioner, the applicants conducted research aimed at enabling the production of a prosthetic implant, the shape of which meets optimal conditions for bio- compatibility and retention of prosthetic crowns.
This research thus resulted in the original concept of a dental prosthesis implant making it possible to overcome the drawbacks of the aforementioned implants. According to the invention, the implant of dental prosthesis with endosseous anchoring of the type of that consisting of a rod formed: of an anterior root body anchoring in the bone tissue formed by a thread defined by a spongy helical thread to be received in the bone and retained there by osseointegration,
And of an aerial posterior body used to retain a dental prosthesis, is remarkable in that the above-mentioned anterior rooting anchoring body has, with respect to its longitudinal direction, a first lower part called apical consisting of the above threading and a second upper so-called cervical part which, connected to the aforesaid posterior aerial body, extends the first part in a cylindrical shape with a diameter at least equal to the largest diameter of the thread of the lower apical part. The presence of the second cylindrical upper cervical part above the thread of the first apical part, allows, like a tampon, to ensure a compressive state of the bone at the level of the crest to limit the risks of invagination by the no helical net and premature craters linked to bacterial migration.
According to a preferred embodiment of the implant of the invention, the diameter of the second cylindrical cervical upper part of the anterior root body is less than that of the base of the posterior aerial body so that the underside of said base creates a shoulder forming a bearing with respect to the aforesaid second upper cervical part.
This shoulder-bearing in contact with the cortex is intended to provide immediate lateral stabilization of the implant while strengthening the buffering effect of the second cylindrical cervical upper part of the anterior body of the implant. According to another preferred embodiment of the implant of the invention, the posterior aerial body connected to the aforesaid second cylindrical cervical upper part by its base, acts as a support for attachment and articulation to a prosthetic ring on which the dental prosthesis is placed. The purpose of this arrangement is to have a ring which, by virtue of its angular expansion on its pivot support, allows the prosthetic crown associated with the ring to correct the correct orientation in the event of a defect in the inclination of the axis of the stem of the implant screwed into the bone tissue.
The fundamental concepts of the invention having just been exposed above in their most elementary form, other details and other characteristics will emerge more clearly on reading the description which follows, giving by way of example not limiting and with reference to the accompanying drawings, two embodiments of a dental prosthesis implant, in accordance with the invention. Ces deux implants 100 et 100' se composent chacun d'une tige formée : d'un corps antérieur racinal 110 et 110', dit d'ancrage dans le tissu osseux et formé d'un filetage défini par un filet hélicoïdal de type spongieux 111 etAs illustrated in the drawings of FIGS. 1a and 2a, the two dental prosthesis implants of the invention referenced 100 and 100 ′ are formed from an implantable titanium alloy or any other biocompatible plantable material. These two implants 100 and 100 'each consist of a rod formed: of a root anterior body 110 and 110', said to anchor in the bone tissue and formed of a thread defined by a helical thread of spongy type 111 and
111' pour être reçu dans l'os et y être retenu par ostéointégrâtion,111 'to be received in the bone and to be retained there by osteointegration,
- d'un corps postérieur aérien 120 et 120' servant de support d'attachement et d'articulation à une bague prothetique 200 et 200' sur laquelle est placée une couronne de prothèse dentaire non représentée, et d'un corps accessoire 130 et 130' formant mandrin porte-outil qui destiné à être inséré dans un contre angle pour implantologie , assure le maintien et la rotation de l'implant lors de l'opération d'ancrage dans le tissu osseux, référencé S sur les dessins des figures lb et 2b.- an aerial posterior body 120 and 120 'serving as an attachment and articulation support to a prosthetic ring 200 and 200' on which is placed a crown of dental prosthesis not shown, and an accessory body 130 and 130 forming a tool-holder mandrel which is intended to be inserted in a contra angle for implantology, ensures the maintenance and rotation of the implant during the anchoring operation in the bone tissue, referenced S in the drawings of FIGS. 1b and 2b.
The front root anchoring body 110 and 100 'has, with respect to its longitudinal direction, a first lower part called apical 111 and 111' constituted by the above threading and a second upper part called cervical 112 and 112 'which extends the first part 111 and 111 'in a cylindrical shape with a height of the order of 2 millimeters and a diameter at least equal to the largest diameter of the thread of the part apical threaded lower 112 and 112 '. This second cervical part 112 and 112 ′ ensures, as can be seen in the drawings of FIGS. 1b and 2b, a compressive state of the bone S at the level of the crest, like a tampon which enters force into the bone tissue S by closing the opening drilled and self-tapped by the first apical lower part 111 and 111 'preformed for this purpose, self-drilling and self-tapping. To meet good endosseous anchoring conditions for the implant, the entire anterior root anchoring body will have a diameter between 1.8 millimeters and 2.4 millimeters with a preference of 2 millimeters over a height included between 7 millimeters and 12 millimeters.
Un autre objet de l'invention est de dimensionner le diamètre (de l'ordre de 2 millimètres) de la deuxième partie supérieure cervicale cylindrique 112 et 112 ' du corps antérieur racinal 110 et 110', inférieur au diamètreAnother object of the invention is to size the diameter (of the order of 2 millimeters) of the second cylindrical upper cervical part 112 and 112 'of the anterior racial body 110 and 110', less than the diameter
(de l'ordre de 3 millimètres) de l'embase du corps postérieur aérien 120 et 120', de manière à ce que la face inférieure de ladite embase crée un épaulement formant palier 121 et 121' par rapport à la susdite deuxième partie supérieure cervicale 112 et 112' et assure, en reposant périphériquement sur le tissu osseux, une stabilisation immédiate de l'implant ancré, comme montré sur les dessins des figures lb et 2b.(of the order of 3 millimeters) of the base of the aerial rear body 120 and 120 ′, so that the underside of said base creates a shoulder forming a bearing 121 and 121 ′ with respect to the above second upper part cervical 112 and 112 'and ensures, by resting peripherally on the bone tissue, immediate stabilization of the anchored implant, as shown in the drawings of Figures 1b and 2b.
The second aerial posterior body 120 and 120 ′ which will be developed a little further in its functionality as well as in its two variables of embodiment, is therefore connected at one end to the anterior racial body 110 and 110 ′, and, at its opposite end and in the longitudinal axis of the implant 100 and 100 ′, to the third accessory body 130 and 130 ′, via a constricted area known as embrittlement 140 and 140 ′, capable of breaking under the effect of a minimum torsional threshold exerted between the accessory body 130 and 130 'and the posterior body 120 and 120' and subject to the endosseous penetration resistance of the implant and more particularly of the front anchoring body 110 and 110 ′. This minimum torsion threshold will be preset according to the bone resistance coefficient. According to a first embodiment of the implant 100 as illustrated in the drawings of FIGS. 1 a and 1 b and intended to receive a fixed prosthetic crown, the posterior aerial body 120 adopts the shape of a truncated cone which, connected by its circular base 121 to the aforesaid second cylindrical upper cervical part 112, receives a prosthetic ring 200 (cf. the figure) preformed to be fitted with play on the above body frustoconical aerial 112. This prosthetic ring 200 with a height of the order of 6 to 8 millimeters and preferably 7 millimeters, can adopt internally, either the shape of an inverted truncated cone, or the shape of a cylinder with circular base.
When the ring 200 is cylindrical internally, it will preferably adopt a diameter of the order of 2.70 millimeters, to allow its adjustment to a height of 2 millimeters above the shoulder 121 of the posterior aerial body thus avoiding compressing the gingival mucosa. This internal cylinder of the prosthetic ring 200, by the cylinder / cone relationship, creates a lateral clearance in its upper part, which thus remedies the differences in parallelism of the implants when they are placed in the bone tissue S.
According to the other variant, when the ring 200 internally adopts the shape of an inverted truncated cone, with a preferential diameter of 2.70 millimeters at the bottom always to allow its adjustment to a height of 2 millimeters above 1 shoulder 121 and 2.90 millimeters at the top to allow travel, the side play will be facilitated by the two inverted cones. This prosthetic ring 200 externally adopts a frustoconical profile externally comprising several circular and horizontal grooves 210 in its upper part in order to obtain retention inside the prosthetic crown, and a smooth part 220 in contact with the gum in its upper part. According to a second embodiment of the implant 100 ′ as illustrated in the drawings of FIGS. 2a and 2b and intended to receive a removable prosthetic crown, the posterior aerial body 120 ′ has, with respect to its longitudinal direction, a first cylindrical lower portion 122 'connected to the cylinder of smaller diameter of the aforesaid second cervical upper part 112', and a second spherical portion 123 'which removably receives a prosthetic ring 200' fitting with play on the sphere 123 ' of said aerial body 120. This prosthetic ring 200 ′, of a height of the order of 4 millimeters, is of known type and will therefore not be detailed below in its description, because it has been judiciously chosen by the applicants in this specific case of an attachment for a removable prosthetic crown for the freedom of angular expansion which it allows and its press-button appearance on the spherical portion 123 ′. The presence of the first cylindrical lower portion 122 ′ is to position the prosthetic ring 200 ′ very exactly at the desired level for aesthetic reasons, as a function of the thickness of the gingival tissue which overcomes the bone cortex.
It is understood that the dental prosthesis implants 100 and 100 ′, which have just been described and shown above, have been for the purpose of disclosure rather than limitation. Of course, various arrangements, modifications and improvements could be made to the above examples, without departing from the scope of the invention taken in its broadest aspects and spirit. Thus, for example, the frusto-conical posterior aerial body 120 which preferably has a diameter of 3 millimeters at the base, 2 millimeters at the top over an axial height of 8 millimeters, will be provided with two vertical grooves 120a intended to cooperate with a screwdriver adapted for unscrewing during the removal of the implant 100.
Likewise, the posterior aerial body 120 ′ which preferably has a diameter of 3 millimeters over a height of 1.5 millimeters for its first cylindrical lower portion 122 ′ and a diameter of 2.4 millimeters over a height of 4.4 millimeters for its second spherical portion 123 ′, will be preformed with cut faces 120a ′ at the level of the connection of the two zones 122 ′ and 123 ′, for the purpose of cooperation with a hollow tool suitable for unscrewing during removal of the implant 100 '.
