SPECIFICATION forming part of Letters Patent No. 761,119, dated May 31, 1904. Application filed January 14, 1904. Serial No. 188,943. (No model.)
To all whom it may concern:
Be it known that I, GHARLEs W. WESTON, J r. a citizen of the United States of America, residing in the borough of Manhattan, city, county, and State of New York, have invented certain new and useful Improvements in Card- Indexes, of which thefollowing is a specification, reference being had therein to the accompanying drawing.
My invention relates to card-index systems; and its objects are to provide a card for such systems having combined therewith a removable slip or subsidiary card the information on which is at all times visible.
In the oflices of corporations a record is ordinarily kept of the stockholders of the company on cards alphabetically arranged, which cards contain the name of the stockholder,
with his post-oflice address. Companies also keep on file written instructions from the several stockholders as to whom and to what address to send any dividends that may be payable on the stock.
The object of my invention is to provide means whereby'the written instructions from the several stockholders of the company may be readily combined with the cards containing the names and addresses of the stockholders, though of course I do not limit myself to the above-described application of my invention.
To these ends my invention consists of a card provided with spaces on the front thereof and on the back thereof with a pocket of transparent materi lthrough which the information contained on a slipor card inserted in the pocket may be read.
A form of my invention is illustrated in the accompanying drawing, in which the subsidiary slip or card B is shown. partly withdrawn from the pocket 0 on the back of the card A.

Any information or data on the front of the card B (the same being provided with spaces for entry of data) will be visible. It will be readily understood that the information or data contained on the fronts of the cards A and B may be read Without separating the same, but that they may be easily separated when occasion should arise.
In the particular use of my invention above described the slip or subsidiary card B contains data furnished by the stockholder and usually the stockholders signature, and it will be understood that one of the main features of my invention is to maintain such data in a position for ready reference in a card system.
What I claim, and desire to secure by Letters Patent, is
The combination of a card having spaces on the front thereof for entering data, with a piece of transparent material secured to the back thereof to form a pocket, and a smaller card having spaces thereon for entering data, said smaller card being adapted to be inserted in the pocket of said larger card.
