Be it knownthat I, BARNARD WILLIS, a

citizen of the United States of America, a-

resident of the city of St. Louis, State of Missouri, have invented certain new and useful Improvements in Statemcnt-Sheets, of which the following is a full, clear, and exact description, reference being had to the accompanying drawing, forming a part of this specification.

My invention relates t0 a foldable statement sheet, and has for its object the production of a simple and inexpensive statement sheet having its elements so shaped and arranged that the address on the statement v,will serve as the mailing address, without yusing a separate envelop as a container for the statement and adapted to be used as an inclosing envelop. for loose matter accompanying the statement.

Figure l is a perspectiveview of a statement sheet embodying the features of the invention.

Fig. Il is a perspective view illustrating the sheet as it appears after the first folding operation, a portion of the sheet being broken away to show the cancelled bank checks which are to be inclosed in the folded article.

The blank shown in Fig. I is preferably, not essentially, scored along the lines l, 2, 3, 4 and 5 to forni an address section A at the top of the sheet, a window section B at the' bottom of the sheet, intermediate statement sections C and D between the address section and window section, and ungummed iiaps E which extend from the intermediate section C. A window 6 is formed in the window section B', 'and the lower margin 7 of this seci checks.

depositor will appear near the top of the sheet, on the address section A, as shown in Fig. I.

The matter to be inclosed by the statement A sheet is placed upon the intermediate section C, and after the sheet has been folded On the fold score 2, the different sections will occupy the relative positions shown in Fig. II. The address section A is then folded back wardly, onto the back of the intermediate section D as shown in Fig. Hl. lt is important to note at thistiine that the address section A is narrower than the' other sections, and whenthis narrow section is folded baclrwardly (Fig. III) it does not completely' cover the intermediate section D. In Fig. III, it is seen that the fa'ce of the naro x address section A is partly exposed so that the address thereon will serve as the mailing address, and that the portion of the section D not covered by reason of the narrowness of section A is exposed to receive the gummed margin of the window section. Before sealing the article,`the unguinmed fiaps E are folded over the intermediate sections G and D, and also over the narrow address section A. The window section B is then folded over all of the other elements and its gummed margin 7 is stuck onto the ungunnned liaps E and onto the back of the intermediate sec- .tion D. The folded statement sheet will lthen constitute a sealed enveloplFig. lV) in which the address will be seen through the window G of the window section B.

Since the address section A (Fig. Hl) is narrower than the other sections, the gummed lower margin of window section B will overlie the section D at a point beyond said address section. The window section B will, therefore, be cemented to the back of the 'statement sheet, instead of to the front of the address section.

To open theenvelop, a knife may be inserted underbne lof the non-adhering aps E at the point indicated by X in Fig. IV, and by cutting along three margins 0f the e11- velop as indicated by. arrows, the iiaps E and the window section will be severed from the intermediate section C. |The window section A and liaps E will remain fastened to the back of the statement sheet by the adhesive A, D and C, none of the severed elements being attached to the face or' the statement sheet.
