Abstract
Description
The invention relates to the field of medicine, namely to endocrinology, surgery and neurology, and can be applied in practical health care for the treatment of such a variety of late complications of diabetes as peripheral diabetic polyneuropathy.
Diabetes mellitus is a chronic metabolic disease characterized by a shift in glucose metabolism due to absolute or partial insulin deficiency.
One of the common disorders in diabetes is the development of a functional shift of the peripheral nervous system with the formation of diabetic peripheral polyneuropathy (develops in more than 6090 patients with diabetes). Symptoms of this disease include pain in the limbs, muscle weakness up to the development of paralysis, as well as various dysfunctions of the autonomic nervous system, for example, diarrhea or impotence. In the case of the specified diabetes-induced disorders, the speed of conduction of excitation along sensitive and motor nerves decreases, which is a measure of determining the degree of development of a shift in the peripheral nervous system.
There are known methods of treating diabetic polyneuropathy through the use of insulin therapy, as well as the use of oral hypoglycemic drugs (1,2), which make it possible to achieve normoglycemia.
But a decrease in the speed of nerve impulse conduction in patients with good indicators of glycemic control is not the only factor in the development of diabetic polyneuropathy. That is why the achievement of normoglycemia does not always have the proper effect under the conditions of treatment of diabetic polyneuropathy.
The method of treatment of diabetic polyneuropathy through the use of known asymmetric esters of 1,4-dihydropyridine-3,5-dicarboxylic acids, known in clinical practice as calcium receptor antagonists, is the closest to the proposed one. When using these dihydropyridine derivatives in the form of tablets, dragees, granules, mixtures, emulsions, suspensions or solutions in the range of doses from 0.1 to 1Omg/kg of body weight, their special therapeutic effect on the peripheral nervous system is noted.
But the negative side of using this remedy for the treatment of diabetic polyneuropathy is the fact that these pharmacological agents are effective under the conditions of compensation and subcompensation of carbohydrate metabolism, and to achieve the result, it is necessary to take them for a long time, which is associated with side and toxic effects on organs and fabrics. Do not forget about the high price of these drugs. In addition, the authors cannot explain the mechanism of implementation of the therapeutic effect of dihydropyridine derivatives in the treatment of diabetic polyneuropathy, calling it "unexpected" (ZI), due to which it is possible to assume that possible negative effects due to its long-term use have not been investigated.
The invention is based on the task of improving the method of treating diabetic neuropathy by using drugs that block the activity of the key enzyme of nitric oxide synthesis - MO-synthase and contribute to the intensification of metabolism, which will increase the effectiveness of the indicated complication of diabetes and prevent its progression and the development of diabetic foot syndrome as little as possible .
The task is solved by the fact that, according to the invention, rats with streptozotocin-induced diabetes are given a single intraperitoneal injection of 100 mg/kg of pentoxifylline, the mechanism of action of which is to block the activity of MO-synthase, and additionally for 4 weeks alpha-lipoic acid is administered intraperitoneally in a dose 5Omg/kg.
The essence of the invention is to block the activity of endogenous nitric oxide under the conditions of diabetes, as well as to strengthen metabolic processes in the body.
It is known that almost 10 years ago nitric oxide (NO) was declared the "molecule of the year" by the British magazine Zsiepse. Recently, it was established that the simplest substance -NO - is continuously synthesized by an enzymatic pathway in the human and animal body, performing the function of one of universal regulators of metabolism. In addition to the regulation of the body's physiological functions - regulation of vascular tone as an adrenoblocker, inhibition of platelet aggregation and their adhesion to vessel walls, regulation of synaptic transmission of nerve impulses, activation and/or suppression of the activity of many proteins and enzymes, etc. - is known the role of MO in mediating acute inflammatory reactions. The production of MO by activated macrophages has been shown. It has been proven that the cytostatic and cytotoxic effects of macrophages under conditions of inflammation are carried out through MO - by activating MO synthase, which converts arginine into MO. Synthesized nitric oxide under conditions of acute inflammation contributes to the development vasodilatation (most often local ), local damage to the parenchyma of the organ subject to inflammation, and its subsequent ischemia. The specified effects of MO contribute to the development of acute inflammatory damage to the parenchyma of the pancreas - such a chain process precedes the development of diabetes.
It is also known that alpha-lipoic acid (AL) has antioxidant, membrane-stabilizing, hypolipidemic, and antidiabetic effects.
The method is carried out as follows.
Experimental studies were conducted under the conditions of a chronic experiment on 61 male Wistar rats fed a standard diet. Rats were provided with free access to food and water, they were housed in standard conditions from 12:00 p.m. by the change of light and darkness.
Experimental diabetes mellitus (ECD) was modeled by intraperitoneal administration of streptozotocin (ST3; bomg/kg; "Zegma, Germany; the drug was prepared in a solution of sodium citrate buffer, pH: 4.5), which induces the selective death of B cells of the pancreas. The formation of ECD was checked for 2 - a day after the introduction of STZ by determining the glucose content in the blood of rats, obtained from the tail vein, using a strip-strip (Ope Toshsi, Germany). Only those rats whose blood glucose concentration exceeded 15 mmol/l were selected for subsequent experiments. The rats would be studied for weeks without treatment - this period is sufficient for the formation of experimental diabetic peripheral polyneuropathy (EDPN) - after which the treatment would begin. Rats with EDPN were intraperitoneally injected with pentoxifylline at a dose of 100 mg/kg, as well as LC at a dose of 50 mg/kg once a day 6 times a week (1-6 days, except for the first week, when LC was administered for 2-6 days) during 4 weeks.
Виділяли таки дослідні групи: 1) контрольна група (п-10), щурам якої вводили розчин натрієвого цитратного буферу (рН-4.5); 2) щури з ЕДПН (п-10), котрих не лікували; 3) щури з ЕДПН, котрим вводили пентоксифілін дозою 100 мг/кг (п-12); 4) щури з ЕДПН, котрим вводили ЛК дозою 50 мг/кг (п-12); 5) щури з ЕДПН, котрим вводили пентоксифілін дозою 100мг/кг та ЛК дозою 5О0мг/кг (п--17).Research groups were also distinguished: 1) a control group (n-10), whose rats were injected with a solution of sodium citrate buffer (pH-4.5); 2) rats with EDPN (n-10), which were not treated; 3) rats with EDPN injected with pentoxifylline at a dose of 100 mg/kg (p-12); 4) rats with EDPN, which were injected with LC at a dose of 50 mg/kg (p-12); 5) rats with EDPN, which were injected with pentoxifylline at a dose of 100 mg/kg and LC at a dose of 500 mg/kg (p--17).
In rats with EDPN, body weight and blood glucose content were determined, and the speed of conduction of excitation along the tail nerve was measured. All determinations were made on the 3rd and 6th days of each week of treatment (that is, during 7-10 weeks of experiments). According to the normalization of the studied indicators, the effectiveness of the use of medical measures was expressed.
Thanks to the simultaneous introduction of pentoxifylline and LC, the speed of conduction of excitation along the tail nerve of rats was normalized.
An example of a specific application of the method.
Experimental diabetes mellitus was reproduced in male Wistar rats by administration of streptozotocin (bOmg/kg, IV). On the second day after the introduction of streptozotocin, the concentration of glucose in the blood increased three times compared to the previous indicator, as well as the data of control observations. Over the course of six weeks, the rats in the experimental group continued to experience a decrease in the speed of conduction of excitation along the tail nerve.
After the simultaneous administration of pentoxifylline (10mg/kg) and LC (50mg/kg), the studied indicator became statistically similar to that measured in rats in control observations.
In comparison with the prototype, the proposed method of treatment of experimental diabetic polyneuropathy is more easily reproduced and the goal of pharmacological compounds in diabetes that contribute to the intensification of metabolism in the body of patients.
